Abstract:The single oral dose pharmacokinetics of tiaprofenic acid (Surgam) has been investigated in fasting and non-fasting healthy volunteers (200 and 300 mg) and in fasting patients with severe renal insufficiency (200 mg). A dose independent pharmacokinetics of tiaprofenic acid was shown in fasting healthy volunteers and the following parameters were calculated after administration of 200 mg: tl = 0.53 +/- 0.15 h, tm = 1.28 +/- 0.19 h, cm = 27.1 micrograms/ml, ka = 2.79 +/- 0.93 h-1, lambda 2 = 1.06 +/- 0.14 h-1, t1/2 = 3.0 +/- 0.2 h, AUCl-infinity = 80 +/- 7 mg X h/l, Clt = 43.8 +/- 3.7 ml/min and V beta = 11.1 +/- 0.8 l. A small, but significant positive deviation from linearity was observed with increasing dose for cm and AUCl-infinity in non-fasting healthy volunteers, probably due to a slightly higher bioavailability of the 300 mg formulation in the non-fasting state as compared with the 200 mg formulation. Intake of food decreased cm significantly at both dosage levels from 27.1 to 19.1 micrograms/ml and from 47.9 to 39.1 micrograms/ml for 200 and 300 mg, respectively. The absorption kinetics of tiaprofenic acid was not significantly different in fasting healthy volunteers and in fasting patients with severe renal insufficiency. However, a significant increase in t1/2 and AUCl-infinity to 5.8 +/- 0.9 h and 173 +/- 34 mg X h/l, respectively, and a significant decrease in total body clearance to 25.5 +/- 5.3 ml/min were observed in this category of patients. No correlation was found between creatinine clearance and tiaprofenic acid clearance.

Radiation dose and image quality of a Medtronic O‐arm commonly used during spine surgery, and a Philips hybrid operating room equipped with XperCT C‐arm 3D cone‐beam CT (hCBCT) are compared. The mobile O‐arm (mCBCT) offers three different radiation dose settings (low, standard, and high), for four different patient sizes (small, medium, large, and extra large). The patient's radiation dose rate is constant during the entire 3D scan. In contrast, C‐CBCT spine imaging uses three different field of views (27, 37, and 48 cm) using automatic exposure control (AEC) that modulates the patient's radiation dose rate during the 3D scan based on changing patient thickness. hCBCT uses additional x‐ray beam filtration. Small, medium, and large trunk phantoms designed to mimic spine and soft tissue were imaged to assess radiation dose and image quality of the two systems. The estimated measured “patient” dose for the small, medium, and large phantoms imaged by the mCBCT considering all the dose settings ranged from 9.4–27.6 mGy, 8.9–33.3 mGy, and 13.8–40.6 mGy, respectively. The “patient” dose values for the same phantoms imaged with hCBCT were 2.8–4.6 mGy, 5.7–10.0 mGy, and 11.0–15.2 mGy. The CNR for the small, medium, and large phantoms was 2.9 to 3.7, 2.0 to 3.0, and 2.5 to 2.6 times higher with the hCBCT system, respectively. Hounsfield unit accuracy, noise, and uniformity of hCBCT exceeded the performance of the mCBCT; spatial resolution was comparable. Added x‐ray beam filtration and AEC capability achieved clinical image quality for intraoperative spine surgery at reduced radiation dose to the patient in comparison to a reference O‐arm system without these capabilities.</p>

Image quality and radiation dose in spinal surgery: a comparison of three imaging systems in phantom

Abstract:Purpose: Using optimal settings for x-ray scans is crucial for obtaining three-dimensional images of high quality while keeping the patient dose low. Our work compares dose and image quality (IQ) of three intraoperative imaging systems [O-arm cone-beam computed tomography (CBCT), ClarifEye C-arm CBCT, and Airo computed tomography] used for spinal surgery. Approach: Patients of 70, 90, and 110 kg were simulated with an anthropomorphic phantom by adding tissue-equivalent material. Titanium inserts were placed in the phantom spine for reproducing metal artifacts in the images. Organ dose was measured with thermo-luminescent dosimeters for effective dose (E) calculation. Subjective IQ was assessed by ranking the images acquired with the manufacturer-defined imaging protocols. Objective IQ was assessed with a customized Catphan phantom. Results: The ClarifEye protocols resulted in the lowest E ranging from 1.4 to 5.1 mSv according to phantom size and protocol. The highest E was measured for the high-definition protocol of O-arm (E 2.2 to 9 mSv) providing the best subjective IQ for imaging of the spine without titanium inserts. For the images with metal, the best IQ was obtained with ClarifEye. Airo (E 5.5 to 8.4 mSv) was ranked with the lowest IQ for images without metal while the rank improved for images with metal. Airo images had better uniformity, noise, and contrast sensitivity compared with CBCTs but worse high-contrast resolution. The values of these parameters were comparable between the CBCT systems. Conclusions: Both CBCT systems provided better IQ compared with Airo for navigation of lumbar spinal surgery for the original phantom. Metal artifacts particularly affect O-arm images decreasing the subjective IQ. The high spatial resolution of CBCT systems resulted in a relevant parameter for the visibility of anatomical features important for spine navigation. Low dose protocols were enough to obtain a clinically acceptable contrast-to-noise ratio in the bones.

Abstract:Objective: Newer cone-beam computed tomography (CBCT) imaging systems offer reconstruction algorithms including metal artifact reduction (MAR) and extended field-of-view (eFoV) techniques to improve image quality. In this study a new CBCT imager, the new Varian HyperSight CBCT, is compared to fan-beam CT and two CBCT imagers installed in a ring-gantry and C-arm linear accelerator, respectively. Approach: The image quality was assessed for HyperSight CBCT which uses new hardware, including a large-size flat panel detector, and improved image reconstruction algorithms. The decrease of metal artifacts was quantified (structural similarity index measure (SSIM) and root-mean-squared error (RMSE)) when applying MAR reconstruction and iterative reconstruction for a dental and spine region using a head-and-neck phantom. The geometry and CT number accuracy of the eFoV reconstruction was evaluated outside the standard field-of-view (sFoV) on a large 3D-printed chest phantom. Phantom size dependency of CT numbers was evaluated on three cylindrical phantoms of increasing diameter. Signal-to-noise (SNR) and contrast-to-noise (CNR) were quantified on an abdominal phantom. Main results: In phantoms with streak artifacts, MAR showed comparable results for HyperSight CBCT and CT, with MAR increasing the SSIM (0.97 to 0.99) and decreasing the RMSE (62 HU to 55 HU) compared to iterative reconstruction without MAR. In addition, HyperSight CBCT showed better geometrical accuracy in the eFoV than CT (Jaccard Conformity Index increase of 0.02-0.03). However, the CT number accuracy outside the sFoV was lower than for CT. The maximum CT number variation between different phantom sizes was lower for the HyperSight CBCT imager (~100HU) compared to the two other CBCT imagers (~200HU), but not fully comparable to CT (~50HU). Significance: This study demonstrated the imaging performance of the new HyperSight CBCT imager and the potential of applying this CBCT system in more advanced scenarios by comparing the quality against fan-beam CT.

Abstract:An image-quality CT phantom was scanned with three different 3D X-ray imaging guidance devices in the operating theatre: O-Arm, Loop-X, and Airo TruCT. Default acquisition and reconstruction parameters for lumbar spine procedures were used on each device. The tube current was set to a dose level of around 27 mGy. A task-based image quality assessment was performed by calculating the noise power spectrum (NPS) and task transfer function (TTF). A detectability index (d') was calculated for three simulated bone lesions. The noise magnitude of the O-Arm was higher than the Airo TruCT, and the Loop-X had higher noise than the Airo TruCT. The highest average NPS frequency was for the O-Arm images, and the lowest was for the Loop-X. The TTFs at 50% values were similar for the Airo TruCT and Loop-X devices. Compared to Airo TruCT, the TTF at 50% value increased with the O-Arm by 53.12% and 41.20% for the Teflon and Delrin inserts, respectively. Compared to Airo TruCT, the d' value was lower with Loop-X by −26.73%, −27.02%, and −23.95% for lytic lesions, sclerotic lesions, and high-density bone, respectively. Each 3D-imaging spine surgery guidance device has its own strengths and weaknesses in terms of image quality. Cone-beam CT systems apparently offer the best compromise between noise and spatial resolution for spine surgery.

Abstract:Background: The image-guided operating room (OR) is an emerging standard for dealing with complex cases in many surgical disciplines including neurosurgery, thoracic surgery, maxillofacial trauma, and orthopedic surgery. Its use in head and neck oncological surgery is not well established. The primary aim of this study was to assess the image quality of cone-beam CT (CBCT) under real clinical conditions. The secondary aim was to assess the effect on surgical performance and decision making. Methods: Intraoperative 3D imaging was performed using a CBCT capable C-Arm mounted on a multi-axis robot (Siemens Zeego) in the image-guided OR. All patients had immediate preoperative imaging taken with further intraoperative imaging performed as required. Ten initial patients, comprising 28 intraoperative scans, were used for questionnaire-based image reviews conducted with experienced head and neck clinicians. Scans were assessed for aspects of both image quality and clinical utility, on separate 5-point Likert scales (1-5). Results: The median rating for bony detail was 4 out of 5. Vascular detail was increased (P < 10-8 ) from 1 to 3 with the use of IV contrast (region of interest CT# was 284 HU [SD, 47 HU]). Images were rated as 4 for freedom from artifact. Soft tissue definition was 2, with no significant improvement (P = .2) with the addition of IV iodinated contrast. Surgeons rated the greatest clinical utility (4) for the CBCT when assessing postreconstruction imaging of a complex case. Conclusions: The image quality of CBCT in the image-guided OR is good for bony detail and complex oncological reconstructions in the head and neck setting but probably has limited benefit for intraoperative soft tissue delineation. Future studies must also focus on clinical outcomes to help demonstrate the value of the image-guided OR.

Abstract:BACKGROUND Intraoperative 3D imaging with cone-beam CT (CBCT) improves assessment of implant position and reduces complications in spine surgery. It is also used for image-guided surgical techniques, resulting in improved quality of care. However, in some cases, metal artifacts can reduce image quality and make it difficult to assess pedicle screw position and reduction. PURPOSE The objective of this study was to investigate whether a change in CBCT acquisition trajectory in relation to pedicle screw position during dorsal instrumentation can reduce metal artifacts and consequently improve image quality and clinical assessability. STUDY DESIGN Experimental cadaver study. METHODS A human cadaver was instrumented with pedicle screws in the thoracic and lumbar spine region (Th11 to L5). Then, the acquisition trajectory of the CBCT (Cios Spin, Siemens, Germany) to the pedicle screws was systematically changed in 5° steps in angulation (−30° to +30°) and swivel (−25° to +25°). Subsequently, radiological evaluation was performed by 3 blinded, qualified raters on image quality using 9 questions (including anatomical structures, implant position, appearance of artifacts) with a score (1–5 points). For statistical evaluation, the image quality of the different acquisition trajectories was compared to the standard acquisition trajectory and checked for significant differences. RESULTS The angulated acquisition trajectory significantly increased the score for subjective image quality (p<.001) as well as the clinical assessability of pedicle screw position (p<.001) with particularly strong effects on subjective image quality in the vertebral pedicle region (d=1.61). Swivel of the acquisition trajectory significantly improved all queried domains of subjective image quality (p<.001) as well as clinical assessability of pedicle screw position (p<.001). CONCLUSIONS In this cadaver study, the angulation as well as the swivel of the acquisition trajectory led to a significantly improved image quality in intraoperative 3D imaging (CBCT) with a constant isocenter. The data show that maximizing the angulation/swivel angle towards 30°/25° provides the best tested subjective image quality and enhances clinical assessability. Therefore, a correct adjustment of the acquisition trajectory can help to make intraoperative revision decisions more reliably. CLINICAL SIGNIFICANCE The knowledge of enhanced image quality by changing the acquisition trajectory in intraoperative 3D imaging can be used for the assessment of critical screw positions in spine surgery. The implementation of this knowledge requires only a minor change of the current intraoperative imaging workflow without additional technical equipment and could further reduce the need for revision surgery.

Do low doses of radiation induce a response modulating induction or repair of DNA single-strand breaks?

Abstract:Many recent reports have indicated interesting structure in the low-dose region of survival curves for mammalian cells (e.g., variations in the effects of oxygen, high-order modulation of the dose-response function, induction of repair, activation of protein-modifying enzymes, influences of cytokines, etc.). Since we have recently identified and corrected several problems causing variability in the results of the alkaline elution methodology, we felt that it was important to investigate the formation and repair of single-strand breaks (SSBs) in this low-dose region. To date we have asked three questions relevant to the structure features noted above: (1) Is the dose response linear at very low radiation doses? (2) Is repair of SSBs complete? (3) Do low "priming" doses of radiation induce subsequent variations in the sensitivity, or rate or extent of repair? To date, we have found no basis in SSB induction and repair for the interesting substructure noted in the survival responses above. At doses from 0.25 Gy to 4 Gy in air, no significant deviations from a linear dose response were observed, and a "priming" dose of 2 Gy has a minimal effect on subsequent sensitivity of SSB formation. Repair of SSBs appears complete at clinical doses of 2 Gy, but substantial unrepaired SSB damage remains 1 h after doses (11 Gy) corresponding to a surviving fraction of 0.01. No significant change in repair of SSBs at 4 h after a 2-Gy priming dose has yet been determined.

Abstract:Floppy eyelid syndrome is a clinical entity associated with a chronic papillary conjunctivitis which is resistant to topical therapeutic agents. The disorder is usually found in older, obese males with upper eyelids which are loose, rubbery, and easily everted. This paper is a review of the clinical characteristics and treatment of the syndrome and includes a case of a 16-year-old male with floppy eyelid syndrome, one of the youngest reported.
Computed Tomography Intraoperative Navigation in Spinal Surgery: Assessment of Patient Radiation Exposure in Current Practices

Abstract:BACKGROUND: Patient radiation exposure associated with the use of computed tomography (CT) navigation during spinal surgeries was widely compared with other intraoperative imaging techniques. The aim of this study is to explore the use of navigation with regard to current spinal surgery practices and the technical limitations of such imaging systems.METHODS: Dosimetric data from 101 patients who underwent intraoperative, CT-navigated spine surgery were retrospectively collected. The study population was divided into 3 groups according to the primary surgical indication. The number of CT image acquisitions per patient, the field length, and the time of exposure per acquisition during a single surgery were compared as well as the radiation dose emitted to patients.RESULTS: Dose-length products (DLP) per acquisition were 678.52, 656.8, and 649.36 mGy·cm with no significant difference for spinal deformity (SD), degenerative disease (DD), and vertebral fracture (VF) procedures, respectively. Analyzing the number of CT image acquisitions per patient revealed that repeated intraoperative scans were often performed for patients who were suffering from an SD due to technical limitations of the navigation. As a consequence, the cumulative dose was higher in the SD group (DLP total = 1175 mGy·cm) than in the DD (DLP total = 762.74 mGy·cm) and VF (DLP total = 649.36 mGy·cm) groups.CONCLUSIONS: CT navigation is an efficient intraoperative imaging technique that reduces the rate of surgical complications, but its technical limitations lead to an increased risk of patient radiation exposure, especially for complex surgeries where multiple scanning acquisitions are needed.CLINICAL RELEVANCE: To avoid patient's overexposure, spine surgeons should minimize the number of intraoperative acquisitions while considering the complexity of the surgery and the limitations of the guidance system. Abstract:Cone-beam computed tomography (CBCT) has been increasingly used during surgical procedures for providing accurate three-dimensional anatomical information for intra-operative navigation and verification. High-quality CBCT images are in general obtained through reconstruction from projection data acquired at hundreds of view angles, which is associated with a non-negligible amount of radiation exposure to the patient. In this work, we have applied a novel image-reconstruction algorithm, the adaptive-steepest-descent-POCS (ASD-POCS) algorithm, to reconstruct CBCT images from protection data at a significantly reduced number of view angles. Preliminary results from experimental studies involving both simulated data and real data show that images of comparable quality to those presently available in clinical image-guidance systems can be obtained by use of the ASD-POCS algorithm from a fraction of the projection data that are currently used. The result implies potential value of the proposed reconstruction technique for low-dose intra-operative CBCT imaging applications.

Abstract:In this study we evaluated the task-based image quality of a low contrast clinical task for the abdomen protocol (e.g., pancreatic tumour) of three different CT vendors, exploiting three model-based iterative reconstruction (MBIR) levels. We used three CT systems equipped with a full, partial, advanced MBIR algorithms. Acquisitions were performed on a phantom at three dose levels. Acquisitions were reconstructed with a standard kernel, using filtered back projection algorithm (FBP) and three levels of the MBIR. The noise power spectrum (NPS), the normalized one (nNPS) and the task-based transfer function (TTF) were computed following the method proposed by the American Association of Physicists in Medicine task group report-233 (AAPM TG-233). Detectability index (d') of a small lesion (small feature; 100 HU and 5-mm diameter) was calculated using non-prewhitening with eye-filter model observer (NPWE).The nNPS, NPS and TTF changed differently depending on CT system. Higher values of d' were obtained with advanced-MBIR, followed by full-MBIR and <a class="link-external link-http" href="http://partial-MBIR.Task" rel="external noopener nofollow">this http URL</a>-based image quality was assessed for three CT scanners of different vendors, considering a clinical question. Detectability can be a tool for protocol optimisation and dose reduction since the same dose levels on different scanners correspond to different d' values.

Abstract:The aim of our study was to evaluate whether software-based artifact reduction can achieve an improved image quality, using intraoperative 3D imaging in spinal surgery. A total of 49 intraoperative 3D image datasets of patients, who underwent surgery with pedicle screw placement, were retrospectively evaluated. The visibility of anatomical structures and the diameter of the pedicle screws were examined, with and without the application of the artifact reduction software. All software prototypes can improve the visibility of anatomical structures (<i>P</i> < 0.01), except MAR (metal artifact reduction) combined with IRIS (iterative reconstruction in image space) (<i>P</i> = 0.04). The algorithms MAR and MAR-2 can reduce the blooming artifacts significantly (<i>P</i> < 0.01), but SL (Shepp & Logan) cannot (<i>P</i> = 0.08–0.988). In summary, software-based artifact reduction for intraoperative 3D datasets can improve the current image quality. Additional information regarding the implant placement and the fracture reduction is therefore generated for the surgeon.

Abstract:Objectives: The aim of this study was to identify cone-beam computed tomography (CBCT) protocols that offer an optimal balance between effective dose (ED) and 3D model for orthognathic virtual surgery planning, using CT as a reference, and to assess whether such protocols can be defined based on technical image quality metrics. Methods: Eleven CBCT (VISO G7, Planmeca Oy, Helsinki, Finland) scan protocols were selected out of 32 candidate protocols, based on ED and technical image quality measurements. Next, an anthropomorphic RANDO SK150 phantom was scanned using these 11 CBCT protocols and 2 CT scanners for bone quantity assessments. The resulting DICOM (Digital Imaging and Communications in Medicine) files were converted into Standard Tessellation Language (STL) models that were used for bone volume and area measurements in the predefined orbital region to assess the validity of each CBCT protocol for virtual surgical planning. Results: The highest CBCT bone volume and area of the STL models were obtained using normal dose protocol (F2) and ultra-low dose protocol (J13), which resulted in 48% and 96% of the mean STL bone volume and 48% and 95% of the bone area measured on CT scanners, respectively. Conclusions: The normal dose CBCT protocol "F2" offered optimal bone area and volume balance for STL. The optimal CBCT protocol can be defined using contrast-to-noise ratio and modulation transfer function values that were similar to those of the reference CT scanners'. CBCT scanners with selected protocols can offer a viable alternative to CT scanners for acquiring STL models for virtual surgical planning at a lower effective dose.

Abstract:The potential for statistical image reconstruction methods such as penalized-likelihood (PL) to improve C-arm cone-beam CT (CBCT) soft-tissue visualization for intraoperative imaging over conventional filtered backprojection (FBP) is assessed in this work by making a fair comparison in relation to soft-tissue performance. A prototype mobile C-arm was used to scan anthropomorphic head and abdomen phantoms as well as a cadaveric torso at doses substantially lower than typical values in diagnostic CT, and the effects of dose reduction via tube current reduction and sparse sampling were also compared. Matched spatial resolution between PL and FBP was determined by the edge spread function of low-contrast (∼ 40-80 HU) spheres in the phantoms, which were representative of soft-tissue imaging tasks. PL using the non-quadratic Huber penalty was found to substantially reduce noise relative to FBP, especially at lower spatial resolution where PL provides a contrast-to-noise ratio increase up to 1.4-2.2 × over FBP at 50% dose reduction across all objects. Comparison of sampling strategies indicates that soft-tissue imaging benefits from fully sampled acquisitions at dose above ∼ 1.7 mGy and benefits from 50% sparsity at dose below ∼ 1.0 mGy. Therefore, an appropriate sampling strategy along with the improved low-contrast visualization offered by statistical reconstruction demonstrates the potential for extending intraoperative C-arm CBCT to applications in soft-tissue interventions in neurosurgery as well as thoracic and abdominal surgeries by overcoming conventional tradeoffs in noise, spatial resolution, and dose.

Abstract:Abstract Objectives Photon-counting detector CT (PCD-CT) promises a leap in spatial resolution due to smaller detector pixel sizes than implemented in energy-integrating detector CTs (EID-CT). Our objective was to compare the visualization of smallest bone details between PCD-CT and EID-CT using a mouse as a specimen. Materials and methods Two euthanized mice were scanned at a 20-slice EID-CT and a dual-source PCD-CT in single-pixel mode at various CTDI Vol values. Image noise and signal-to-noise ratio (SNR) were evaluated using repeated ROI measurements. Edge sharpness of bones was compared by the maximal slope within CT value plots along sampling lines intersecting predefined bones of the spine. Two readers evaluated bone detail visualization at four regions of the spine on a three-point Likert scale at various CTDI Vol ’s. Two radiologists selected the series with better detail visualization among each of 20 SNR-matched pairs of EID-CT and PCD-CT series. Results In CTDI Vol -matched scans, PCD-CT series showed significantly lower image noise (Noise CTDI = 5 mGy : 16.27 ± 1.39 vs. 23.46 ± 0.96 HU, p < 0.01), higher SNR (SNR CTDI = 5 mGy : 20.57 ± 1.89 vs. 14.00 ± 0.66, p < 0.01), and higher edge sharpness (Edge Slope lumbar spine : 981 ± 160 vs. 608 ± 146 HU/mm, p < 0.01) than EID-CT series. Two radiologists considered the delineation of bone details as feasible at consistently lower CTDI Vol values at PCD-CT than at EID-CT. In comparison of SNR-matched reconstructions, PCD-CT series were still considered superior in almost all cases. Conclusions In this head-to-head comparison, PCD-CT showed superior objective and subjective image quality characteristics over EID-CT for the delineation of tiniest bone details. Even in SNR-matched pairs (acquired at different CTDI Vol ’s), PCD-CT was strongly preferred by radiologists. Key Points • In dose-matched scans, photon-counting detector CT series showed significantly less image noise, higher signal-to-noise ratio, and higher edge sharpness than energy-integrating detector CT series . • Human observers considered the delineation of tiny bone details as feasible at much lower dose levels in photon-counting detector CT than in energy-integrating detector CT . • In direct comparison of series matched for signal-to-noise ratio, photon-counting detector CT series were considered superior in almost all cases .

Abstract:Purpose/Objective: To compare image quality of clinically available onboard CT techniques for image guided radiation therapy(IGRT): MV cone beam CT(CBCT), MV fan beam CT, and kilovoltage CBCT.Materials and Methods:Images were acquired using (1) The 6 MV treatment beam (TBL) of a Siemens ONCOR linac (2) The 4.2 MeV imaging beam line (IBL) of the ONCOR (3) The 3.5MV beam of a Tomotherapy unit (Tomo) (3) The 120kV beam of an Elekta XVI and (4) a Varian Trilogy OBI (both 125kV and 100 kV beams ). A cylindrical water phantom of 16cm diameter and big water phantom of 50×38×18cm with eight tissue equivalent inserts were imaged. Contrast‐to‐noise ratio (CNR) and spatial resolution were obtained from phantom CTimages. The dose at isocenter were measured for both phantoms and all imaging techniques. Results: With the same isocenter dose,CNRs for the cylindrical water phantom were generally highest for images acquired with the OBI. For dense bone, XVIimages had higher CNR than IBL, followed by Tomo, then TBL. For lung and soft tissues, IBL, Tomo, and Elekta are comparable. For CNRs measured with the big water phantom, XVI and OBI have a higher CNR than IBL and Tomo, with the largest difference being for both dense bone and lung. IBL generally has a better CNR than TBL. The spatial resolution is higher for OBI, then followed by XVI, Tomo,IBL and TBL. Conclusions: At the same dose to isocenter, KV CBCTimages have higher CNR and spatial resolution than those of MV beams. Work underway to be presented at the meeting will extend this comparison to other measures of greater clinical importance than isocenter dose, including surface dose and average dose to the central slice. Abstract:Background: High-quality 3D-anatomy of the day is needed for treatment plan adaptation in radiotherapy. For online x-ray-based CBCT workflows, one approach is to create a synthetic CT or to utilize a fan-beam CT with corresponding registrations. The former potentially introduces uncertainties in the dose calculation if deformable image registration is used. The latter can introduce burden and complexity to the process, the facility, and the patient. Purpose: Using the CBCT of the day, acquired on the treatment device, for direct dose calculation and plan adaptation can overcome these limitations. This study aims to assess the accuracy of the calculated dose on the CBCT scans acquired on a Halcyon linear accelerator equipped with HyperSight. Methods: HyperSight's new CBCT reconstruction algorithm includes improvements in scatter correction, HU calibration of the imager, and beam shape adaptation. Furthermore, HyperSight introduced a new x-ray detector. To show the effect of the implemented improvements, gamma comparisons of 2%/2 mm, 2%/1 mm, and 1%/1 mm were made between the dose distribution in phantoms calculated on the CBCT reconstructions and the simulation CT scans, considering this the standard of care. The resulting gamma passing rates were compared to those obtained with the Halcyon 3.0 reconstruction and hardware without HyperSight's technologies. Various anatomical phantoms for dosimetric evaluations on brain, head and neck, lung, breast, and prostate cases have been used in this study. Results: The overall results demonstrated that HyperSight outperformed the Halcyon 3.0 version. Based on the gamma analysis, the calculated dose using HyperSight was closer to the CT scan-based doses than the calculated dose using iCBCT Halcyon 3.0 for most cases. Over all plans and gamma criteria, Halcyon 3.0 achieved an average passing rate of 92.9%, whereas HyperSight achieved 98.1%. Conclusion: Using HyperSight CBCT images for direct dose calculation, for example, in (online) plan adaptation, seems feasible for the investigated cases.

Abstract:To examine the effect of extreme old age on muscle plasticity, 6- (adult) and 36-mo-old (old) male Fischer 344 x Brown Norway hybrid rats underwent bilateral surgical ablation of the gastrocnemius muscle to functionally overload (OV) the fast-twitch plantaris muscle for 8 wk. Plantaris muscle wet weight, muscle cross-sectional area (CSA), and average fiber CSA decreased by 44, 42, and 40%, respectively, in old compared with adult rats, and peak isometric tetanic tension decreased by 83%. Compared with muscles in age-matched controls, plantaris muscle mass increased by 53% and type I, IIA, and IIX/IIB CSA increased by 91, 76, and 103%, respectively, in adult-OV rats, but neither wet mass nor fiber CSA increased in old-OV rats. OV decreased type I, IIA, and IIX/IIB mean fiber CSA by 31, 35, and 30%, respectively, in old-OV rats. Collectively, our data indicate that in extreme old age the plantaris muscle undergoes significant loss of mass, fiber CSA, and contractile function, as well as its capacity to undergo hypertrophy in response to a chronic increase in mechanical load.
