Abstract:For wearable electrocardiogram (ECG) acquisition, it was easy to infer motion artifices and other noises. In this paper, a novel end-to-end ECG denoising method was proposed, which was implemented by fusing the Efficient Channel Attention (ECA-Net) and the cycle consistent generative adversarial network (CycleGAN) method. The proposed denoising model was optimized by using the ECA-Net method to highlight the key features and introducing a new loss function to further extract the global and local ECG features. The original ECG signal came from the MIT-BIH Arrhythmia Database. Additionally, the noise signals used in this method consist of a combination of Gaussian white noise and noises sourced from the MIT-BIH Noise Stress Test Database, including EM (Electrode Motion Artifact), BW (Baseline Wander) and MA (Muscle Artifact), as well as mixed noises composed of EM+BW, EM+MA, BW+MA and EM+BW+MA. Moreover, corrupted ECG signals were generated by adding different levels of single and mixed noises to clean ECG signals. The experimental results show that the proposed method has better denoising performance and generalization ability with higher signal-to-noise ratio improvement (SNRimp), as well as lower root-mean-square error (RMSE) and percentage-root-mean-square difference (PRD).

Abstract:The morphology of Fetal Electrocardiogram (FECG) plays an important role in the early diagnosis of fetal health condition. However, it is intractable to extract the clean morphology of FECG signals, which are usually contaminated by Maternal ECG (MECG) and various noises. To extract the clean morphology of FECG signals from noninvasive abdominal ECG records, a high-performance and high-efficient two-stage Slow-Fast Long Short Term Memory (SFLSTM) based architecture is proposed. The MECG elimination and the FECG enhancement are realized by the elaborately designed slow LSTM and fast LSTM to filter out the MECG and the residual noise components, respectively. Qualitative and quantitative experiments are conducted on the records from two public databases. The experimental results show that our proposed MECG elimination and FECG enhancement schemes improve the Signal-to-Noise Ratio (SNR) by 3.09 dB and 1.81 dB, respectively. The proposed fast LSTM reduces the amount of computation by approximately 50%, without any degradation in performance. Our proposed method may leverage the noninvasive FECG monitoring for the early detection of fetal heart diseases.

Abstract:The non-invasive fetal electrocardiogram can provide significant clinical diagnostic information for the fetal health condition. However, since the signal is collected from the abdomen of the pregnant woman, its signal-to-noise ratio is low and it contains a lot of noise, which causes much trouble for the extraction of the fetal electrocardiogram. The non-invasive fetal electrocardiogram is non-stationary, and the noise may overlap with the useful signal in the frequency domain. To solve this challenging denoising work, we propose a method based on stationary wavelet transform and spatially selective noise filtration. The method is evaluated in the synthetic dataset and clinic dataset, and the experiment results show that this method performs better than other two methods. Moreover, the filtered signal is clearer, and the contour is smoother, which is helpful for fetal electrocardiogram morphological analysis and visualization.

Abstract:Objective: To monitor fetal health and growth, fetal heart rate is a critical indicator. The non-invasive fetal electrocardiogram is a widely employed measurement for fetal heart rate estimation, which is extracted from the electrodes placed on the surface of the maternal abdomen. The qualities of the fetal ECG recordings, however, are frequently affected by the noises from various interference sources. In general, the fetal heart rate estimates are unreliable when low-quality fetal ECG signals are used for fetal heart rate estimation, which makes accurate fetal heart rate estimation a challenging task. So, the signal quality assessment for the fetal ECG records is an essential step before fetal heart rate estimation. In other words, some low-quality fetal ECG signal segments are supposed to be detected and removed by utilizing signal quality assessment, so as to improve the accuracy of fetal heart rate estimation. A few supervised learning-based fetal ECG signal quality assessment approaches have been introduced and shown to accurately classify high- and low-quality fetal ECG signal segments, but large fetal ECG datasets with quality annotation are required in these methods. Yet, the labeled fetal ECG datasets are limited. Proposed methods: An unsupervised learning-based multi-level fetal ECG signal quality assessment approach is proposed in this paper for identifying three levels of fetal ECG signal quality. We extracted some features associated with signal quality, including entropy-based features, statistical features, and ECG signal quality indices. Additionally, an autoencoder-based feature is calculated, which is related to the reconstruction error of the spectrograms generated from fetal ECG signal segments. The high-, medium-, and low-quality fetal ECG signal segments are classified by inputting these features into a self-organizing map. Main results: The experimental results showed that our proposal achieved a weighted average F1-score of 90% in three-level fetal ECG signal quality classification. Moreover, with the acceptable removal of detected low-quality signal segments, the errors of fetal heart rate estimation were reduced to a certain extent.

Abstract:This paper proposes a denoising method based on an adversarial deep learning approach for the post-processing of multi-channel fetal electrocardiogram (ECG) signals. As it's well known, noise leads to misinterpretations of fetal ECG signals and thus limits the use of fetal electrocardiography for healthcare applications. Therefore, denoising algorithms are essential for the exploitation of non-invasive fetal ECG. The proposed method is based on the combination of three end-to-end trained sub-networks to convert noisy fetal ECG signals into clean signals. The first two sub-networks are linked by skip connections and form a deep convolutional network that downsamples the noisy signals into a latent representation and subsequently upsamples this latent representation to recover clean signals. The third sub-network aims to boost the decoder sub-network to generate realistic clean signals. Experiments carried out on synthetic and real data showed that the proposed method improved by the signal-to-noise ( SNR ) of fetal ECG signals with input SNR ranging from to 0 dB by an average of 20 dB, and improve fetal signal quality by significantly increasing the number of true detected QRS complexes and halving QRS complex detection errors.

Abstract:The fetal ECG derived from abdominal leads provides an alternative to standard means of fetal monitoring. Furthermore, it permits long-term and ambulant recordings, which expands the range diagnostic possibilities for evaluating the fetal health state. However, due to the temporal and spectral overlap of maternal and fetal signals, the usage of abdominal leads imposes the need for elaborated signal processing routines.In this work a modular combination of processing techniques is presented. Its core consists of two maternal ECG estimation techniques, namely the extended Kalman smoother (EKS) and template adaption (TA) in combination with an innovative detection algorithm. Our detection method employs principles of evolutionary computing to detect fetal peaks by considering the periodicity and morphological characteristics of the fetal signal. In a postprocessing phase, single channel detections are combined by means of kernel density estimation and heart rate correction.The described methodology was presented during the Computing in Cardiology Challenge 2013. The entry was the winner of the closed-source events with average scores for events 4/5 with 15.1/3.32 (TA) and 69.5/4.58 (EKS) on training set-A and 20.4/4.57 (TA) and 219/7.69 (EKS) on test set-B, respectively. Using our own clinical data (24 subjects each 20 min recordings) and statistical measures beyond the Challenge's scoring system, we further validated the proposed method. For our clinical data we obtained an average detection rate of 82.8% (TA) and 83.4% (EKS). The achieved results show that the proposed methods are able produce reliable fetal heart rate estimates from a restricted number of abdominal leads.

Abstract:The abdominal electrocardiogram (ECG) provides a non-invasive method for monitoring the fetal cardiac activity in pregnant women. However, the temporal and frequency overlap between the fetal ECG (FECG), the maternal ECG (MECG) and noise results in a challenging source separation problem. This work seeks to compare temporal extraction methods for extracting the fetal signal and estimating fetal heart rate. A novel method for MECG cancelation using an echo state neural network (ESN) based filtering approach was compared with the least mean square (LMS), the recursive least square (RLS) adaptive filter and template subtraction (TS) techniques. Analysis was performed using real signals from two databases composing a total of 4 h 22 min of data from nine pregnant women with 37,452 reference fetal beats. The effects of preprocessing the signals was empirically evaluated. The results demonstrate that the ESN based algorithm performs best on the test data with an F1 measure of 90.2% as compared to the LMS (87.9%), RLS (88.2%) and the TS (89.3%) techniques. Results suggest that a higher baseline wander high pass cut-off frequency than traditionally used for FECG analysis significantly increases performance for all evaluated methods. Open source code for the benchmark methods are made available to allow comparison and reproducibility on the public domain data.

Abstract:Objective. Perinatal asphyxia poses a significant risk to neonatal health, necessitating accurate fetal heart rate monitoring for effective detection and management. The current gold standard, cardiotocography, has inherent limitations, highlighting the need for alternative approaches. The emerging technology of non-invasive fetal electrocardiography shows promise as a new sensing technology for fetal cardiac activity, offering potential advancements in the detection and management of perinatal asphyxia. Although algorithms for fetal QRS detection have been developed in the past, only a few of them demonstrate accurate performance in the presence of noise and artifacts. Approach. In this work, we propose Power-MF, a new algorithm for fetal QRS detection combining power spectral density and matched filter techniques. We benchmark Power-MF against three open-source algorithms on two recently published datasets (Abdominal and Direct Fetal ECG Database: ADFECG, subsets B1 Pregnancy and B2 Labour; Non-invasive Multimodal Foetal ECG-Doppler Dataset for Antenatal Cardiology Research: NInFEA). Main results. Our results show that Power-MF outperforms state-of-the-art algorithms on ADFECG (B1 Pregnancy: 99.5 % ± 0.5 % F1-score, B2 Labour: 98.0 % ± 3.0 % F1-score) and on NInFEA in three of six electrode configurations by being more robust against noise. Significance. Through this work, we contribute to improving the accuracy and reliability of fetal cardiac monitoring, an essential step toward early detection of perinatal asphyxia with the long-term goal of reducing costs and making prenatal care more accessible.

Abstract:This paper proposes a five-stage based methodology to extract the fetal electrocardiogram (FECG) from the single channel abdominal ECG using differential evolution (DE) algorithm, extended Kalman smoother (EKS) and adaptive neuro fuzzy inference system (ANFIS) framework. The heart rate of the fetus can easily be detected after estimation of the fetal ECG signal. The abdominal ECG signal contains fetal ECG signal, maternal ECG component, and noise. To estimate the fetal ECG signal from the abdominal ECG signal, removal of the noise and the maternal ECG component presented in it is necessary. The pre-processing stage is used to remove the noise from the abdominal ECG signal. The EKS framework is used to estimate the maternal ECG signal from the abdominal ECG signal. The optimized parameters of the maternal ECG components are required to develop the state and measurement equation of the EKS framework. These optimized maternal ECG parameters are selected by the differential evolution algorithm. The relationship between the maternal ECG signal and the available maternal ECG component in the abdominal ECG signal is nonlinear. To estimate the actual maternal ECG component present in the abdominal ECG signal and also to recognize this nonlinear relationship the ANFIS is used. Inputs to the ANFIS framework are the output of EKS and the pre-processed abdominal ECG signal. The fetal ECG signal is computed by subtracting the output of ANFIS from the pre-processed abdominal ECG signal. Non-invasive fetal ECG database and set A of 2013 physionet/computing in cardiology challenge database (PCDB) are used for validation of the proposed methodology. The proposed methodology shows a sensitivity of 94.21%, accuracy of 90.66%, and positive predictive value of 96.05% from the non-invasive fetal ECG database. The proposed methodology also shows a sensitivity of 91.47%, accuracy of 84.89%, and positive predictive value of 92.18% from the set A of PCDB.

Abstract:Most of the calculation methods for the electrocardiograph (ECG) inverse problem are based on priori assumptions of the instantaneous characteristics of the cardiac electrophysiology. In this paper, we have proposed a novel algorithm based on low rank and sparse decomposition (LSD) + total variation (TV) to solve the illposedness of dynamic ECG-inverse problem. The TV constraint filters out the disturbance of the noise and maintains the local smoothness of the potential. The LSD separates the sparse details from the potential background to prevent the potential details from being lost under the effect of smoothing constraint, thereby improving the accuracy of cardiac potential recovery.

Abstract:Despite wearable electronic devices having enormous potential applications, the recording quality has not sufficiently met the requirements for fetal disease detection. In this article, we aim to propose a novel framework for fetal electrocardiography (ECG) extraction, where the hybrid approach mainly considers the combination of a novel adaptive maternal QRS removal (AQRSR) algorithm and a tensor-based joint blind source separation (JBSS) approach. To adapt to the dynamic change of the ECG signal and suppress the powerful maternal heartbeat, AQRSR is explored with an adaptive template, which is generated by a model consisting of a generator, discriminator, and transformer. By replacing the oldest segmentation with the newest cycle, the approach is capable of quickly adapting to the estimated output to match the new input. In addition, due to the significant crossover between the target signal and the noise, even if the maternal ECG removal processing is implemented, the residual signal inevitably contains fetal ECG, part of maternal ECG, and noise. As a consequence of this, a novel JBSS that incorporates tensor decomposition is formulated to separate fetal heartbeat from long-term recordings contaminated with maternal movements noise and heartbeat interference. By dividing the collected data into multiple segments, this method can extract fetal ECG signals by exploiting the signals that are not only statistically independent within each segment but can be dependent on different segments. Experimental results show that even if the recorded signals are contaminated by the noise, the proposed framework has comparable better performance on fetal ECG extraction.

Abstract:Non-invasive fetal electrocardiogram (FECG) is used to monitor the electrical pulse of the fetal heart. Decomposing the FECG signal from maternal ECG (MECG) is a blind source separation problem, which is hard due to the low amplitude of FECG, the overlap of R waves, and the potential exposure to noise from different sources. Traditional decomposition techniques, such as adaptive filters, require tuning, alignment, or pre-configuration, such as modeling the noise or desired signal. to map MECG to FECG efficiently. The high correlation between maternal and fetal ECG parts decreases the performance of convolution layers. Therefore, the masking region of interest using the attention mechanism is performed for improving signal generators' precision. The sine activation function is also used since it could retain more details when converting two signal domains. Three available datasets from the Physionet, including A&D FECG, NI-FECG, and NI-FECG challenge, and one synthetic dataset using FECGSYN toolbox, are used to evaluate the performance. The proposed method could map abdominal MECG to scalp FECG with an average 98% R-Square [CI 95%: 97%, 99%] as the goodness of fit on A&D FECG dataset. Moreover, it achieved 99.7 % F1-score [CI 95%: 97.8-99.9], 99.6% F1-score [CI 95%: 98.2%, 99.9%] and 99.3% F1-score [CI 95%: 95.3%, 99.9%] for fetal QRS detection on, A&D FECG, NI-FECG and NI-FECG challenge datasets, respectively. These results are comparable to the state-of-the-art; thus, the proposed algorithm has the potential of being used for high-performance signal-to-signal conversion.

Abstract:Background: The utilization of non-invasive techniques for fetal cardiac health surveillance is pivotal in evaluating fetal well-being throughout the gestational period. This process requires clean and interpretable fetal Electrocardiogram (fECG) signals. Method: The proposed work is the novel framework for the elicitation of fECG signals from abdominal ECG (aECG) recordings of the pregnant mother. The comprehensive approach encompasses pre-processing of the raw ECG signal, Blind Source Separation techniques (BSS), Decomposition techniques like Empirical Mode Decomposition (EMD), and its variants like Ensemble Empirical Mode Decomposition (EEMD), and Complete Ensemble Empirical Mode Decomposition with Additive Noise (CEEMDAN). The Robust Set Membership Affine Projection (RSMAP) Algorithm is deployed for the enhancement of the obtained fECG signal. Result: The results show significant improvements in the elicited fECG signal with a maximum Signal Noise Ratio (SNR) of 31.72 dB and correlation coefficient = 0.899, Maximum Heart Rate(MHR) obtained in the range of 108–142 bpm for all the records of abdominal ECG signals. The statistical test gave a p -value of 0.21 accepting the null hypothesis. The Abdominal and Direct Fetal Electrocardiogram Database (ABDFECGDB) from PhysioNet has been used for this analysis. Conclusion: The proposed framework demonstrates a robust and effective method for the elicitation and enhancement of fECG signals from the abdominal recordings.

HKF: Hierarchical Kalman Filtering with Online Learned Evolution Priors for Adaptive ECG Denoising

Abstract:Electrocardiography (ECG) signals play a pivotal role in many healthcare applications, especially in at-home monitoring of vital signs. Wearable technologies, which these applications often depend upon, frequently produce low-quality ECG signals. While several methods exist for ECG denoising to enhance signal quality and aid clinical interpretation, they often underperform with ECG data from wearable technology due to limited noise tolerance or inadequate flexibility in capturing ECG dynamics. This paper introduces HKF, a hierarchical and adaptive Kalman filter, which uses a proprietary state space model to effectively capture both intra- and inter-heartbeat dynamics for ECG signal denoising. HKF learns a patient-specific structured prior for the ECG signal's intra-heartbeat dynamics in an online manner, resulting in a filter that adapts to the specific ECG signal characteristics of each patient. In an empirical study, HKF demonstrated superior denoising performance (reduced mean-squared error) while preserving the unique properties of the waveform. In a comparative analysis, HKF outperformed previously proposed methods for ECG denoising, such as the model-based Kalman filter and data-driven autoencoders. This makes it a suitable candidate for applications in extramural healthcare settings.

The Challenge of Fetal Cardiac MRI Reconstruction Using Deep Learning

Abstract:Dynamic free-breathing fetal cardiac MRI is one of the most challenging modalities, which requires high temporal and spatial resolution to depict rapid changes in a small fetal heart. The ability of deep learning methods to recover undersampled data could help to optimise the kt-SENSE acquisition strategy and improve non-gated kt-SENSE reconstruction quality. In this work, we explore supervised deep learning networks for reconstruction of kt-SENSE style acquired data using an extensive in vivo dataset. Having access to fully-sampled low-resolution multi-coil fetal cardiac MRI, we study the performance of the networks to recover fully-sampled data from undersampled data. We consider model architectures together with training strategies taking into account their application in the real clinical setup used to collect the dataset to enable networks to recover prospectively undersampled data. We explore a set of modifications to form a baseline performance evaluation for dynamic fetal cardiac MRI on real data. We systematically evaluate the models on coil-combined data to reveal the effect of the suggested changes to the architecture in the context of fetal heart properties. We show that the best-performers recover a detailed depiction of the maternal anatomy on a large scale, but the dynamic properties of the fetal heart are under-represented. Training directly on multi-coil data improves the performance of the models, allows their prospective application to undersampled data and makes them outperform CTFNet introduced for adult cardiac cine MRI. However, these models deliver similar qualitative performances recovering the maternal body very well but underestimating the dynamic properties of fetal heart. This dynamic feature of fast change of fetal heart that is highly localised suggests both more targeted training and evaluation methods might be needed for fetal heart application.

Abstract:Monitoring of electrocardiogram (ECG) provides vital information as well as any cardiovascular anomalies. Recent advances in the technology of wearable electronics have enabled compact devices to acquire personal physiological signals in the home setting; however, signals are usually contaminated with high level noise. Thus, an efficient ECG filtering scheme is a dire need. In this paper, a novel method using Ensemble Kalman Filter (EnKF) is developed for denoising ECG signals. We also intensively explore various filtering algorithms, including Savitzky-Golay (SG) filter, Ensemble Empirical mode decomposition (EEMD), Normalized Least-Mean-Square (NLMS), Recursive least squares (RLS) filter, Total variation denoising (TVD), Wavelet and extended Kalman filter (EKF) for comparison. Data from the MIT-BIH Noise Stress Test database were used. The proposed methodology shows the average signal to noise ratio (SNR) of 10.96, the Percentage Root Difference of 150.45, and the correlation coefficient of 0.959 from the modified MIT-BIH database with added motion artifacts.

Abstract:Estimation of the fetal heart rate (FHR) has gained interest in the last century, low heart rate variability has been studied to identify intrauterine growth restricted fetuses (prepartum), and abnormal FHR patterns have been associated with fetal distress during delivery (intrapartum). Several monitoring techniques have been proposed for FHR estimation, including auscultation and Doppler ultrasound. This thesis focuses on the extraction of the non-invasive fetal electrocardiogram (NI-FECG) recorded from a limited set of abdominal sensors. The main challenge with NI-FECG extraction techniques is the low signal-to-noise ratio of the FECG signal on the abdominal mixture signal which consists of a dominant maternal ECG component, FECG and noise. However the NI-FECG offers many advantages over the alternative fetal monitoring techniques, the most important one being the opportunity to enable morphological analysis of the FECG which is vital for determining whether an observed FHR event is normal or pathological. In order to advance the field of NI-FECG signal processing, the development of standardised public databases and benchmarking of a number of published and novel algorithms was necessary.
