Abstract:The morphology of Fetal Electrocardiogram (FECG) plays an important role in the early diagnosis of fetal health condition. However, it is intractable to extract the clean morphology of FECG signals, which are usually contaminated by Maternal ECG (MECG) and various noises. To extract the clean morphology of FECG signals from noninvasive abdominal ECG records, a high-performance and high-efficient two-stage Slow-Fast Long Short Term Memory (SFLSTM) based architecture is proposed. The MECG elimination and the FECG enhancement are realized by the elaborately designed slow LSTM and fast LSTM to filter out the MECG and the residual noise components, respectively. Qualitative and quantitative experiments are conducted on the records from two public databases. The experimental results show that our proposed MECG elimination and FECG enhancement schemes improve the Signal-to-Noise Ratio (SNR) by 3.09 dB and 1.81 dB, respectively. The proposed fast LSTM reduces the amount of computation by approximately 50%, without any degradation in performance. Our proposed method may leverage the noninvasive FECG monitoring for the early detection of fetal heart diseases.
Extracting Fetal Heart Rate from Abdominal ECGs Based on Fast Multivariate Empirical Mode Decomposition

Abstract:Abdominal electrocardiogram is an important means to obtain fetal health condition during high-risk pregnancy. In this paper, a novel method for extracting fetal heart rate from multi-channel mother abdomen electrocardiograms is proposed using fast multivariate empirical mode decomposition technique (FMEMD). Firstly, FMEMD decomposes the multichannel ECG signals into a set of modes. Two significant channels are selected according to the standard deviation of the fifth layer. Then the continuous wavelet transform technique (CWT) is applied to these two channels to denoise. The baseline is removed by zero-crossing rate. Following, the interference of the mother QRS complexes and non-overlapped fetal R-peaks can be eliminated and detected by CWT coefficient. The overlapped fetal R-peaks are obtained by combining the dynamic pattern matching program and creative algorithm. The proposed method achieves an accuracy of 99.9% on the existing data set, and the calculating time is only 1/6.39 of the MEMD-based method.

Abstract:In the last two decades, stillbirth has caused around 2 million fetal deaths worldwide. Although current ultrasound tools are reliably used for the assessment of fetal growth during pregnancy, it still raises safety issues on the fetus, requires skilled providers, and has economic concerns in less developed countries. Here, we propose deep coherence, a novel artificial intelligence (AI) approach that relies on 1 min non-invasive electrocardiography (ECG) to explain the association between maternal and fetal heartbeats during pregnancy. We validated the performance of this approach using a trained deep learning tool on a total of 941 one minute maternal-fetal R-peaks segments collected from 172 pregnant women (20–40 weeks). The high accuracy achieved by the tool (90%) in identifying coupling scenarios demonstrated the potential of using AI as a monitoring tool for frequent evaluation of fetal development. The interpretability of deep learning was significant in explaining synchronization mechanisms between the maternal and fetal heartbeats. This study could potentially pave the way toward the integration of automated deep learning tools in clinical practice to provide timely and continuous fetal monitoring while reducing triage, side-effects, and costs associated with current clinical devices.

Point-of-Care Real-Time Signal Quality for Fetal Doppler Ultrasound Using a Deep Learning Approach

Abstract:In this study, we present a deep learning framework designed to integrate with our previously developed system that facilitates large-scale 1D fetal Doppler data collection, aiming to enhance data quality. This system, tailored for traditional Indigenous midwives in low-resource communities, leverages a cost-effective Android phone to improve the quality of recorded signals. We have shown that the Doppler data can be used to identify fetal growth restriction, hypertension, and other concerning issues during pregnancy. However, the quality of the signal is dependent on many factors, including radio frequency interference, position of the fetus, maternal body habitus, and usage of the Doppler by the birth attendants. In order to provide instant feedback to allow correction of the data at source, a signal quality metric is required that can run in real-time on the mobile phone. In this study, 191 DUS signals with durations mainly in the range between 5 to 10 minutes were evaluated for quality and classified into five categories: Good, Poor, (Radiofrequency) Interference, Talking, and Silent, at a resolution of 3.75 seconds. A deep neural network was trained on each 3.75-second segment from these recordings and validated using five-fold cross-validation. An average micro F1 = 97.4\% and macro F1 = 94.2\% were achieved, with F1 = 99.2\% for `Good' quality data. These results indicate that the algorithm, which will now be implemented in the midwives' app, should allow a significant increase in the quality of data at the time of capture.

Abstract:Monitoring the electrical pulse of fetal heart through a non-invasive fetal electrocardiogram (fECG) can easily detect abnormalities in the developing heart to significantly reduce the infant mortality rate and post-natal complications. Due to the overlapping of maternal and fetal R-peaks, the low amplitude of the fECG, systematic and ambient noises, typical signal extraction methods, such as adaptive filters, independent component analysis, empirical mode decomposition, etc., are unable to produce satisfactory fECG. While some techniques can produce accurate QRS waves, they often ignore other important aspects of the ECG. Our approach, which is based on 1D CycleGAN, can reconstruct the fECG signal from the mECG signal while maintaining the morphology due to extensive preprocessing and appropriate framework. The performance of our solution was evaluated by combining two available datasets from Physionet, "Abdominal and Direct Fetal ECG Database" and "Fetal electrocardiograms, direct and abdominal with reference heartbeat annotations", where it achieved an average PCC and Spectral-Correlation score of 88.4% and 89.4%, respectively. It detects the fQRS of the signal with accuracy, precision, recall and F1 score of 92.6%, 97.6%, 94.8% and 96.4%, respectively. It can also accurately produce the estimation of fetal heart rate and R-R interval with an error of 0.25% and 0.27%, respectively. The main contribution of our work is that, unlike similar studies, it can retain the morphology of the ECG signal with high fidelity. The accuracy of our solution for fetal heart rate and R-R interval length is comparable to existing state-of-the-art techniques. This makes it a highly effective tool for early diagnosis of fetal heart diseases and regular health checkups of the fetus.

Abstract:This paper proposes a five-stage based methodology to extract the fetal electrocardiogram (FECG) from the single channel abdominal ECG using differential evolution (DE) algorithm, extended Kalman smoother (EKS) and adaptive neuro fuzzy inference system (ANFIS) framework. The heart rate of the fetus can easily be detected after estimation of the fetal ECG signal. The abdominal ECG signal contains fetal ECG signal, maternal ECG component, and noise. To estimate the fetal ECG signal from the abdominal ECG signal, removal of the noise and the maternal ECG component presented in it is necessary. The pre-processing stage is used to remove the noise from the abdominal ECG signal. The EKS framework is used to estimate the maternal ECG signal from the abdominal ECG signal. The optimized parameters of the maternal ECG components are required to develop the state and measurement equation of the EKS framework. These optimized maternal ECG parameters are selected by the differential evolution algorithm. The relationship between the maternal ECG signal and the available maternal ECG component in the abdominal ECG signal is nonlinear. To estimate the actual maternal ECG component present in the abdominal ECG signal and also to recognize this nonlinear relationship the ANFIS is used. Inputs to the ANFIS framework are the output of EKS and the pre-processed abdominal ECG signal. The fetal ECG signal is computed by subtracting the output of ANFIS from the pre-processed abdominal ECG signal. Non-invasive fetal ECG database and set A of 2013 physionet/computing in cardiology challenge database (PCDB) are used for validation of the proposed methodology. The proposed methodology shows a sensitivity of 94.21%, accuracy of 90.66%, and positive predictive value of 96.05% from the non-invasive fetal ECG database. The proposed methodology also shows a sensitivity of 91.47%, accuracy of 84.89%, and positive predictive value of 92.18% from the set A of PCDB.

Abstract:Despite wearable electronic devices having enormous potential applications, the recording quality has not sufficiently met the requirements for fetal disease detection. In this article, we aim to propose a novel framework for fetal electrocardiography (ECG) extraction, where the hybrid approach mainly considers the combination of a novel adaptive maternal QRS removal (AQRSR) algorithm and a tensor-based joint blind source separation (JBSS) approach. To adapt to the dynamic change of the ECG signal and suppress the powerful maternal heartbeat, AQRSR is explored with an adaptive template, which is generated by a model consisting of a generator, discriminator, and transformer. By replacing the oldest segmentation with the newest cycle, the approach is capable of quickly adapting to the estimated output to match the new input. In addition, due to the significant crossover between the target signal and the noise, even if the maternal ECG removal processing is implemented, the residual signal inevitably contains fetal ECG, part of maternal ECG, and noise. As a consequence of this, a novel JBSS that incorporates tensor decomposition is formulated to separate fetal heartbeat from long-term recordings contaminated with maternal movements noise and heartbeat interference. By dividing the collected data into multiple segments, this method can extract fetal ECG signals by exploiting the signals that are not only statistically independent within each segment but can be dependent on different segments. Experimental results show that even if the recorded signals are contaminated by the noise, the proposed framework has comparable better performance on fetal ECG extraction.

Abstract:Objective. Perinatal asphyxia poses a significant risk to neonatal health, necessitating accurate fetal heart rate monitoring for effective detection and management. The current gold standard, cardiotocography, has inherent limitations, highlighting the need for alternative approaches. The emerging technology of non-invasive fetal electrocardiography shows promise as a new sensing technology for fetal cardiac activity, offering potential advancements in the detection and management of perinatal asphyxia. Although algorithms for fetal QRS detection have been developed in the past, only a few of them demonstrate accurate performance in the presence of noise and artifacts. Approach. In this work, we propose Power-MF, a new algorithm for fetal QRS detection combining power spectral density and matched filter techniques. We benchmark Power-MF against three open-source algorithms on two recently published datasets (Abdominal and Direct Fetal ECG Database: ADFECG, subsets B1 Pregnancy and B2 Labour; Non-invasive Multimodal Foetal ECG-Doppler Dataset for Antenatal Cardiology Research: NInFEA). Main results. Our results show that Power-MF outperforms state-of-the-art algorithms on ADFECG (B1 Pregnancy: 99.5 % ± 0.5 % F1-score, B2 Labour: 98.0 % ± 3.0 % F1-score) and on NInFEA in three of six electrode configurations by being more robust against noise. Significance. Through this work, we contribute to improving the accuracy and reliability of fetal cardiac monitoring, an essential step toward early detection of perinatal asphyxia with the long-term goal of reducing costs and making prenatal care more accessible.

Abstract:Despite broad application during labor and delivery, there remains considerable debate about the value of electronic fetal monitoring (EFM). EFM includes the surveillance of fetal heart rate (FHR) patterns in conjunction with the mother's uterine contractions, providing a wealth of data about fetal behavior and the threat of diminished oxygenation and cerebral perfusion. Adverse outcomes universally associate a fetal injury with the failure to timely respond to FHR pattern information. Historically, the EFM data, stored digitally, are available only as rasterized pdf images for contemporary or historical discussion and examination. In reality, however, they are rarely reviewed systematically or purposefully. Using a unique archive of EFM collected over 50 years of practice in conjunction with adverse outcomes, we present a deep learning framework for training and detection of incipient or past fetal injury. We report 94% accuracy in identifying early, preventable fetal injury intrapartum. This framework is suited for automating an early warning and decision support system for maintaining fetal well-being during the stresses of labor. Ultimately, such a system could enable obstetrical care providers to timely respond during labor and prevent both urgent intervention and adverse outcomes. When adverse outcomes cannot be avoided, they can provide guidance to the early neuroprotective treatment of the newborn.

Abstract:Background: Fetal heart rate (FHR) extracted from abdominal electrocardiogram (ECG) is a powerful non-invasive method in appropriately assessing the fetus well-being during pregnancy. Despite significant advances in the field of electrocardiography, the analysis of fetal ECG (FECG) signal is considered a challenging issue which is mainly due to low signal to noise ratio (SNR) of FECG. Objective: In this study, we present an approach for accurately locating the fetal QRS complexes in non-invasive FECG. Materials and methods: In this experimental study, the proposed method included 4 steps. In step 1, comb notching filter was employed to pre-process the abdominal ECG (AECG). Furthermore, low frequency noises were omitted using wavelet decomposition. In next step, principal component analysis (PCA) and signal quality assessment (SQA) were used to obtain an optimal AECG reference channel for maternal R-peaks detection. In step 3, maternal ECG (MECG) was removed from mixture signal and FECG was extracted. In final step, the extracted FECG was first decomposed by discrete wavelet transforms at level 10. Then, by employing details of levels 2, 3, 4, the new FECG signal was reconstructed in which various noises and artifacts were removed and FECG components whose frequency were close to the fetal QRS complexes remained which increased the performance of the method. Results: For evaluation, 15 recordings of PhysioNet Noninvasive FECG database were used and the average F1 measure of 98.77% was obtained. Conclusion: The results indicate that use of both an efficient analysis of major component of AECG along with a signal quality assessment technique has a promising performance in FECG analysis.

Abstract:The fetal ECG derived from abdominal leads provides an alternative to standard means of fetal monitoring. Furthermore, it permits long-term and ambulant recordings, which expands the range diagnostic possibilities for evaluating the fetal health state. However, due to the temporal and spectral overlap of maternal and fetal signals, the usage of abdominal leads imposes the need for elaborated signal processing routines.In this work a modular combination of processing techniques is presented. Its core consists of two maternal ECG estimation techniques, namely the extended Kalman smoother (EKS) and template adaption (TA) in combination with an innovative detection algorithm. Our detection method employs principles of evolutionary computing to detect fetal peaks by considering the periodicity and morphological characteristics of the fetal signal. In a postprocessing phase, single channel detections are combined by means of kernel density estimation and heart rate correction.The described methodology was presented during the Computing in Cardiology Challenge 2013. The entry was the winner of the closed-source events with average scores for events 4/5 with 15.1/3.32 (TA) and 69.5/4.58 (EKS) on training set-A and 20.4/4.57 (TA) and 219/7.69 (EKS) on test set-B, respectively. Using our own clinical data (24 subjects each 20 min recordings) and statistical measures beyond the Challenge's scoring system, we further validated the proposed method. For our clinical data we obtained an average detection rate of 82.8% (TA) and 83.4% (EKS). The achieved results show that the proposed methods are able produce reliable fetal heart rate estimates from a restricted number of abdominal leads.

Abstract:Standard clinical practice to assess fetal well-being during labour utilises monitoring of the fetal heart rate (FHR) using cardiotocography. However, visual evaluation of FHR signals can result in subjective interpretations leading to inter and intra-observer disagreement. Therefore, recent studies have proposed deep-learning-based methods to interpret FHR signals and detect fetal compromise. These methods have typically focused on evaluating fixed-length FHR segments at the conclusion of labour, leaving little time for clinicians to intervene. In this study, we propose a novel FHR evaluation method using an input length invariant deep learning model (FHR-LINet) to progressively evaluate FHR as labour progresses and achieve rapid detection of fetal compromise. Using our FHR-LINet model, we obtained approximately 25% reduction in the time taken to detect fetal compromise compared to the state-of-the-art multimodal convolutional neural network while achieving 27.5%, 45.0%, 56.5% and 65.0% mean true positive rate at 5%, 10%, 15% and 20% false positive rate respectively. A diagnostic system based on our approach could potentially enable earlier intervention for fetal compromise and improve clinical outcomes.

The Development and Implementation of Innovative Blind Source Separation Techniques for Real-Time Extraction and Analysis of Fetal and Maternal Electrocardiogram Signals

Abstract:This article presents an innovative approach to analyzing and extracting electrocardiogram (ECG) signals from the abdomen and thorax of pregnant women, with the primary goal of isolating fetal ECG (fECG) and maternal ECG (mECG) signals. To resolve the difficulties related to the low amplitude of the fECG, various noise sources during signal acquisition, and the overlapping of R waves, we developed a new method for extracting ECG signals using blind source separation techniques. This method is based on independent component analysis algorithms to detect and accurately extract fECG and mECG signals from abdomen and thorax data. To validate our approach, we carried out experiments using a real and reliable database for the evaluation of fECG extraction algorithms. Moreover, to demonstrate real-time applicability, we implemented our method in an embedded card linked to electronic modules that measure blood oxygen saturation (SpO2) and body temperature, as well as the transmission of data to a web server. This enables us to present all information related to the fetus and its mother in a mobile application to assist doctors in diagnosing the fetus's condition. Our results demonstrate the effectiveness of our approach in isolating fECG and mECG signals under difficult conditions and also calculating different heart rates (fBPM and mBPM), which offers promising prospects for improving fetal monitoring and maternal healthcare during pregnancy.

Abstract:Objective: To monitor fetal health and growth, fetal heart rate is a critical indicator. The non-invasive fetal electrocardiogram is a widely employed measurement for fetal heart rate estimation, which is extracted from the electrodes placed on the surface of the maternal abdomen. The qualities of the fetal ECG recordings, however, are frequently affected by the noises from various interference sources. In general, the fetal heart rate estimates are unreliable when low-quality fetal ECG signals are used for fetal heart rate estimation, which makes accurate fetal heart rate estimation a challenging task. So, the signal quality assessment for the fetal ECG records is an essential step before fetal heart rate estimation. In other words, some low-quality fetal ECG signal segments are supposed to be detected and removed by utilizing signal quality assessment, so as to improve the accuracy of fetal heart rate estimation. A few supervised learning-based fetal ECG signal quality assessment approaches have been introduced and shown to accurately classify high- and low-quality fetal ECG signal segments, but large fetal ECG datasets with quality annotation are required in these methods. Yet, the labeled fetal ECG datasets are limited. Proposed methods: An unsupervised learning-based multi-level fetal ECG signal quality assessment approach is proposed in this paper for identifying three levels of fetal ECG signal quality. We extracted some features associated with signal quality, including entropy-based features, statistical features, and ECG signal quality indices. Additionally, an autoencoder-based feature is calculated, which is related to the reconstruction error of the spectrograms generated from fetal ECG signal segments. The high-, medium-, and low-quality fetal ECG signal segments are classified by inputting these features into a self-organizing map. Main results: The experimental results showed that our proposal achieved a weighted average F1-score of 90% in three-level fetal ECG signal quality classification. Moreover, with the acceptable removal of detected low-quality signal segments, the errors of fetal heart rate estimation were reduced to a certain extent.

This paper introduces a novel three-stage hybrid adaptive approach for fECG extraction and FHR determination from aECG. The proposed framework is an amalgamation of independent component analysis (ICA), adaptive exponential functional link network (AEFLN), and novel wavelet thresholding (NWT). The proposed work aims to determine the best quality fetal R-peaks for calculating fetal heart rate (FHR). ICA is a blind source separation technique that uses all the information from the multichannel inputs to determine the maternal Electrocardiogram (mECG) and enhanced aECG. In the next stage, a non-linear adaptive filter, AEFLN, is used for the extraction of fECG. This approach requires no additional algorithm for polarity reversal and time alignment before AEFLN. At last, NWT determines the fetal R-peak envelopes from the residual fECG. The R-peak envelopes are used to analyze FHR, and its accuracy was accessed using the FHR trace. So far, FastICA-AEFLN-NWT has not been used as a hybrid method for fECG extraction and fetal R-peak detection. The algorithm's effectiveness was tested on both real databases from Daisy and Physionet and synthetic databases from Physionet. The proposed hybrid method attains 95.29 % of sensitivity (SE), 96.68 % of Positive Predictive Value (PPV), 95.92 % of F1 score, and FHR measurement (FHRmeas) is 29.72 bpm 2 . The proposed methodology outperforms the state-of-art methods in fECG extraction.

Abstract:Non-invasive foetal electrocardiography (NI-FECG) has become an important prenatal monitoring method in the hospital. However, due to its susceptibility to non-stationary noise sources and lack of robust extraction methods, the capture of high-quality NI-FECG remains a challenge. Recording waveforms of sufficient quality for clinical use typically requires human visual inspection of each recording. A Signal Quality Index (SQI) can help to automate this task but, contrary to adult ECG, work on SQIs for NI-FECG is sparse. In this paper, a multi-channel signal quality classifier for NI-FECG waveforms is presented. The model can be used during the capture of NI-FECG to assist technicians to record high-quality waveforms, which is currently a labour-intensive task. A Convolutional Neural Network (CNN) is trained to distinguish between NI-FECG segments of high and low quality. NI-FECG recordings with one maternal channel and three abdominal channels were collected from 100 subjects during a routine hospital screening (102.6 min of data). The model achieves an average 10-fold cross-validated AUC of 0.95 ± 0.02. The results show that the model can reliably assess the FECG signal quality on our dataset. The proposed model can improve the automated capture and analysis of NI-FECG as well as reduce technician labour time.

Deep Learning-Based Inference of Uterine Contractions from Maternal ECG

Abstract:Background: Preterm birth and stillbirth have multifaceted causes, many of which remain unknown. Factors such as maternal age, sexually transmitted infections, and genetic abnormalities are notably implicated. Globally, there has been minimal improvement in preterm birth rates, leading to substantial economic losses and underscoring the need for innovative approaches. Objective: This study aims to analyze and detect uterine contractions by applying deep learning techniques to physiological data derived from maternal electrocardiograms (ECGs). Leveraging ECGs, which are easily obtainable from wearable devices, allows for effective and convenient monitoring of uterine contractions outside clinical settings. Since uterine contractions are a critical indicator for assessing the risk of preterm birth, our algorithm has the potential to facilitate early identification of high-risk individuals, thereby contributing to timely interventions and improved maternal-fetal outcomes. Methods: Participants meeting all inclusion criteria and none of the exclusion criteria were recruited from patients admitted to or attending outpatient services at Saintpaulia Misao Ladies Hospital in Gifu, Japan, between December 6, 2023, and July 31, 2024. Our deep learning model was developed using maternal ECG data, uterine contraction waveforms obtained from cardiotocograms (CTGs) . The collected data were divided into training and evaluation datasets. The AI-generated uterine contraction waveforms from the developed model were compared with ground truth labels obtained from the fetal monitoring devices. This study was reviewed and approved by the Ethics Committee of the Graduate School of Medicine at Gifu University and was conducted as a collaborative research effort with nonat Inc. (the lead research institution) and Saintpaulia Misao Ladies Hospital (joint research institution). Results: Finally, 73 participants took part in this study, and 57 datasets were used for algorithm development and evaluation. Multiple measurements were taken on the same subjects on different days only with their consent. Our analysis demonstrated a strong correlation between the uterine contraction waveforms estimated by the developed model and the ground truth waveforms from CTGs, achieving an average correlation across all evaluation data (Pearson correlation coefficient = 0.53). Conclusion: This is the world's first trial to detect uterine contractions from maternal ECGs using deep learning algorithms. In this study, we successfully developed a deep learning algorithm capable of accurately inferring uterine contraction waveforms from maternal ECGs. Given that ECGs can be easily obtained using wearable devices, this approach may provide healthcare professionals with objective and precise information on uterine contractions-a crucial indicator of preterm labor-even in resource-limited settings outside hospital environments.

Abstract:Background: The utilization of non-invasive techniques for fetal cardiac health surveillance is pivotal in evaluating fetal well-being throughout the gestational period. This process requires clean and interpretable fetal Electrocardiogram (fECG) signals. Method: The proposed work is the novel framework for the elicitation of fECG signals from abdominal ECG (aECG) recordings of the pregnant mother. The comprehensive approach encompasses pre-processing of the raw ECG signal, Blind Source Separation techniques (BSS), Decomposition techniques like Empirical Mode Decomposition (EMD), and its variants like Ensemble Empirical Mode Decomposition (EEMD), and Complete Ensemble Empirical Mode Decomposition with Additive Noise (CEEMDAN). The Robust Set Membership Affine Projection (RSMAP) Algorithm is deployed for the enhancement of the obtained fECG signal. Result: The results show significant improvements in the elicited fECG signal with a maximum Signal Noise Ratio (SNR) of 31.72 dB and correlation coefficient = 0.899, Maximum Heart Rate(MHR) obtained in the range of 108–142 bpm for all the records of abdominal ECG signals. The statistical test gave a p -value of 0.21 accepting the null hypothesis. The Abdominal and Direct Fetal Electrocardiogram Database (ABDFECGDB) from PhysioNet has been used for this analysis. Conclusion: The proposed framework demonstrates a robust and effective method for the elicitation and enhancement of fECG signals from the abdominal recordings.
