Abstract:The morphology of Fetal Electrocardiogram (FECG) plays an important role in the early diagnosis of fetal health condition. However, it is intractable to extract the clean morphology of FECG signals, which are usually contaminated by Maternal ECG (MECG) and various noises. To extract the clean morphology of FECG signals from noninvasive abdominal ECG records, a high-performance and high-efficient two-stage Slow-Fast Long Short Term Memory (SFLSTM) based architecture is proposed. The MECG elimination and the FECG enhancement are realized by the elaborately designed slow LSTM and fast LSTM to filter out the MECG and the residual noise components, respectively. Qualitative and quantitative experiments are conducted on the records from two public databases. The experimental results show that our proposed MECG elimination and FECG enhancement schemes improve the Signal-to-Noise Ratio (SNR) by 3.09 dB and 1.81 dB, respectively. The proposed fast LSTM reduces the amount of computation by approximately 50%, without any degradation in performance. Our proposed method may leverage the noninvasive FECG monitoring for the early detection of fetal heart diseases.
Extracting Fetal Heart Rate from Abdominal ECGs Based on Fast Multivariate Empirical Mode Decomposition

Abstract:Abdominal electrocardiogram is an important means to obtain fetal health condition during high-risk pregnancy. In this paper, a novel method for extracting fetal heart rate from multi-channel mother abdomen electrocardiograms is proposed using fast multivariate empirical mode decomposition technique (FMEMD). Firstly, FMEMD decomposes the multichannel ECG signals into a set of modes. Two significant channels are selected according to the standard deviation of the fifth layer. Then the continuous wavelet transform technique (CWT) is applied to these two channels to denoise. The baseline is removed by zero-crossing rate. Following, the interference of the mother QRS complexes and non-overlapped fetal R-peaks can be eliminated and detected by CWT coefficient. The overlapped fetal R-peaks are obtained by combining the dynamic pattern matching program and creative algorithm. The proposed method achieves an accuracy of 99.9% on the existing data set, and the calculating time is only 1/6.39 of the MEMD-based method.
A Novel Deep Learning Technique for Morphology Preserved Fetal ECG Extraction from Mother ECG using 1D-CycleGAN

Abstract:Monitoring the electrical pulse of fetal heart through a non-invasive fetal electrocardiogram (fECG) can easily detect abnormalities in the developing heart to significantly reduce the infant mortality rate and post-natal complications. Due to the overlapping of maternal and fetal R-peaks, the low amplitude of the fECG, systematic and ambient noises, typical signal extraction methods, such as adaptive filters, independent component analysis, empirical mode decomposition, etc., are unable to produce satisfactory fECG. While some techniques can produce accurate QRS waves, they often ignore other important aspects of the ECG. Our approach, which is based on 1D CycleGAN, can reconstruct the fECG signal from the mECG signal while maintaining the morphology due to extensive preprocessing and appropriate framework. The performance of our solution was evaluated by combining two available datasets from Physionet, "Abdominal and Direct Fetal ECG Database" and "Fetal electrocardiograms, direct and abdominal with reference heartbeat annotations", where it achieved an average PCC and Spectral-Correlation score of 88.4% and 89.4%, respectively. It detects the fQRS of the signal with accuracy, precision, recall and F1 score of 92.6%, 97.6%, 94.8% and 96.4%, respectively. It can also accurately produce the estimation of fetal heart rate and R-R interval with an error of 0.25% and 0.27%, respectively. The main contribution of our work is that, unlike similar studies, it can retain the morphology of the ECG signal with high fidelity. The accuracy of our solution for fetal heart rate and R-R interval length is comparable to existing state-of-the-art techniques. This makes it a highly effective tool for early diagnosis of fetal heart diseases and regular health checkups of the fetus.

Extraction of fetal ECG signal by an improved method using extended Kalman smoother framework from single channel abdominal ECG signal

Abstract:This paper proposes a five-stage based methodology to extract the fetal electrocardiogram (FECG) from the single channel abdominal ECG using differential evolution (DE) algorithm, extended Kalman smoother (EKS) and adaptive neuro fuzzy inference system (ANFIS) framework. The heart rate of the fetus can easily be detected after estimation of the fetal ECG signal. The abdominal ECG signal contains fetal ECG signal, maternal ECG component, and noise. To estimate the fetal ECG signal from the abdominal ECG signal, removal of the noise and the maternal ECG component presented in it is necessary. The pre-processing stage is used to remove the noise from the abdominal ECG signal. The EKS framework is used to estimate the maternal ECG signal from the abdominal ECG signal. The optimized parameters of the maternal ECG components are required to develop the state and measurement equation of the EKS framework. These optimized maternal ECG parameters are selected by the differential evolution algorithm. The relationship between the maternal ECG signal and the available maternal ECG component in the abdominal ECG signal is nonlinear. To estimate the actual maternal ECG component present in the abdominal ECG signal and also to recognize this nonlinear relationship the ANFIS is used. Inputs to the ANFIS framework are the output of EKS and the pre-processed abdominal ECG signal. The fetal ECG signal is computed by subtracting the output of ANFIS from the pre-processed abdominal ECG signal. Non-invasive fetal ECG database and set A of 2013 physionet/computing in cardiology challenge database (PCDB) are used for validation of the proposed methodology. The proposed methodology shows a sensitivity of 94.21%, accuracy of 90.66%, and positive predictive value of 96.05% from the non-invasive fetal ECG database. The proposed methodology also shows a sensitivity of 91.47%, accuracy of 84.89%, and positive predictive value of 92.18% from the set A of PCDB.

Abstract:Early detection of potential hazards in the fetal physiological state during pregnancy and childbirth is very important. Noninvasive fetal electrocardiogram (FECG) can be extracted from the maternal abdominal signal. However, due to the interference of maternal electrocardiogram and other noises, the task of extraction is challenging. This paper introduces a novel single-lead noninvasive fetal electrocardiogram extraction method based on the technique of clustering and PCA. The method is divided into four steps: (1) pre-preprocessing; (2) fetal QRS complexes and maternal QRS complexes detection based on k-means clustering algorithm with the feature of max-min pairs; (3) FQRS correction step is to improve the performance of step two; (4) template subtraction based on PCA is introduced to extract FECG waveform. To verify the performance of the proposed algorithm, two clinical open-access databases are used to check the performance of FQRS detection. As a result, the method proposed shows the average PPV of 95.35%, Se of 96.23%, and F1-measure of 95.78%. Abstract:The fetal electrocardiogram (FECG) records changes in the graph of fetal cardiac action potential during conduction, reflecting the developmental status of the fetus in utero and its physiological cardiac activity. Morphological alterations in the FECG can indicate intrauterine hypoxia, fetal distress, and neonatal asphyxia early on, enhancing maternal and fetal safety through prompt clinical intervention, thereby reducing neonatal morbidity and mortality. To reconstruct FECG signals with clear morphological information, this paper proposes a novel deep learning model, CBLS-CycleGAN. The model's generator combines spatial features extracted by the CNN with temporal features extracted by the BiLSTM network, thus ensuring that the reconstructed signals possess combined features with spatial and temporal dependencies. The model's discriminator utilizes PatchGAN, employing small segments of the signal as discriminative inputs to concentrate the training process on capturing signal details. Evaluating the model using two real FECG signal databases, namely "Abdominal and Direct Fetal ECG Database" and "Fetal Electrocardiograms, Direct and Abdominal with Reference Heartbeat Annotations", resulted in a mean MSE and MAE of 0.019 and 0.006, respectively. It detects the FQRS compound wave with a sensitivity, positive predictive value, and F1 of 99.51%, 99.57%, and 99.54%, respectively. This paper's model effectively preserves the morphological information of FECG signals, capturing not only the FQRS compound wave but also the fetal P-wave, T-wave, P-R interval, and ST segment information, providing clinicians with crucial diagnostic insights and a scientific foundation for developing rational treatment protocols.

Recovery of the fetal electrocardiogram for morphological analysis from two trans-abdominal channels via optimal shrinkage

Abstract:We propose a novel algorithm to recover fetal electrocardiogram (ECG) for both the fetal heart rate analysis and morphological analysis of its waveform from two or three trans-abdominal maternal ECG channels. We design an algorithm based on the optimal-shrinkage and the nonlocal Euclidean median under the wave-shape manifold model. For the fetal heart rate analysis, the algorithm is evaluated on publicly available database, 2013 PhyioNet/Computing in Cardiology Challenge, set A. For the morphological analysis, we propose to simulate semi-real databases by mixing the MIT-BIH Normal Sinus Rhythm Database and MITDB Arrhythmia Database. For the fetal R peak detection, the proposed algorithm outperforms all algorithms under comparison. For the morphological analysis, the algorithm provides an encouraging result in recovery of the fetal ECG waveform, including PR, QT and ST intervals, even when the fetus has arrhythmia. To the best of our knowledge, this is the first work focusing on recovering the fetal ECG for morphological analysis from two or three channels with an algorithm potentially applicable for continuous fetal electrocardiographic monitoring, which creates the potential for long term monitoring purpose.

Abstract:Fetal electrocardiogram (FECG) contains crucial information about the fetus during pregnancy, making the extraction of FECG signal essential for monitoring fetal health. However, extracting FECG signal from abdominal electrocardiogram (AECG) poses several challenges: (1) FECG signal is often contaminated by noise, and (2) FECG signal is frequently overshadowed by high-amplitude maternal electrocardiogram (MECG). To address these issues and enhance the accuracy of signal extraction, this paper proposes an improved Cycle Generative Adversarial Networks (CycleGAN) with integrated contrastive learning for FECG signal extraction. The model introduces a dual-attention mechanism in the generator of the generative adversarial network, incorporating a multi-head self-attention (MSA) module and a channel-wise self-attention (CSA) module to enhance the quality of generated signals. Additionally, a contrastive triplet loss is integrated into the CycleGAN loss function, optimizing training to increase the similarity between the extracted FECG signal and the scalp fetal electrocardiogram. The proposed method is evaluated using the ADFECG dataset and the PCDB dataset both from the Physionet. In terms of signal extraction quality, Mean Squared Error is reduced to 0.036, Mean Absolute Error (MAE) to 0.009, and Pearson Correlation Coefficient reaches 0.924. When validating the model performance, Structural Similarity Index achieves 95.54%, Peak Signal-to-Noise Ratio (PSNR) reaches 38.87 dB, and R-squared ( R 2 ) attains 95.12%. Furthermore, the positive predictive value (PPV), sensitivity (SEN) and F1-score for QRS wave cluster detection on the ADFECG dataset also reached 99.56%, 99.43% and 99.50%, respectively. On the PCDB dataset, the positive predictive value (PPV), sensitivity (SEN) and F1-score for QRS wave cluster detection also reached 98.24%, 98.60% and 98.42%, respectively. All of them are higher than other methods. Therefore, the proposed model has important applications in effective monitoring of fetal health during pregnancy.

A New Approach to Extract Fetal Electrocardiogram Using Affine Combination of Adaptive Filters

Abstract:The detection of abnormal fetal heartbeats during pregnancy is important for monitoring the health conditions of the fetus. While adult ECG has made several advances in modern medicine, noninvasive fetal electrocardiography (FECG) remains a great challenge. In this paper, we introduce a new method based on affine combinations of adaptive filters to extract FECG signals. The affine combination of multiple filters is able to precisely fit the reference signal, and thus obtain more accurate FECGs. We proposed a method to combine the Least Mean Square (LMS) and Recursive Least Squares (RLS) filters. Our approach found that the Combined Recursive Least Squares (CRLS) filter achieves the best performance among all proposed combinations. In addition, we found that CRLS is more advantageous in extracting FECG from abdominal electrocardiograms (AECG) with a small signal-to-noise ratio (SNR). Compared with the state-of-the-art MSF-ANC method, CRLS shows improved performance. The sensitivity, accuracy, and F1 scores improved by 3.58%, 2.39%, and 1.36%, respectively.

Abstract:Monitoring fetal electrocardiogram (FECG) is crucial for diagnosing potential congenital heart defects and fetal distress during pregnancy. Due to strong interferences and background noise, it is challenging to extract FECG from maternal abdominal electrocardiogram (AECG). In this article, we propose a novel method for extracting FECG using multiple-channel AECGs. Our approach incorporates canonical correlation analysis (CCA) into the local principal component analysis (LPCA). First, the FECG is preliminarily extracted from multichannel AECGs via CCA approach, which greatly suppresses the maternal interference and retains the waveform details of FECG. However, the extracted signal may still suffer from significant noise. Second, a fast adaptive LPCA algorithm is proposed for denoising purpose, which avoids the computationally expensive eigenvalue decomposition of high dimensional covariance matrix. The convergence of the adaptive LPCA is analyzed based on the deterministic discrete-time (DDT) theory. The proposed algorithm is evaluated on both synthetic signals and real recordings. The simulation results demonstrate the robustness of our method against Gaussian noise, the signal to noise ratio (SNR) of extracted FECG achieves 19.65 dB. The experimental results on real-world dataset further illustrate the outstanding performance of our method in terms of computational load and fetal QRS complex detection accuracy with $F_{1}$ score of 96.55% on ADFECGDB dataset.

Noninvasive Extraction of Maternal and Fetal ECG using Periodic Progressive FastICA Peel-off

Abstract:The abdominal electrocardiogram (AECG) gives a safe and non-invasive way to monitor fetal well-being during pregnancy. However, due to the overlap with maternal ECG (MECG) as well as significant external noise, it is challenging to extract weak fetal ECG (FECG) using surface electrodes. In this study, we introduce a novel periodic progressive FastICA peel-off (PPFP) method for noninvasive extraction of weak surface FECG signals, leveraging the two-step FastICA method and a peel-off strategy from the progressive FastICA peel-off (PFP) approach. Specifically, for ECG signals, the periodic constrained FastICA that integrates ECG signal characteristics enables precise extraction of MECG and FECG spike trains. Additionally, a peel-off strategy incorporating SVD waveform reconstruction ensures comprehensive identification of subtle source signals. The performance of the proposed method was examined on public datasets with reference, synthetic data and clinical data, with an F1-scores for FECG extraction on public dataset of 99.59%, on synthetic data with the highest noise level of 99.50%, which are all superior to other comparative methods. Furthermore, clearly periodic and physiologically consistent FECG signals were extracted from clinically collected data. The results indicates that our proposed method has potential and effectiveness to separate MECG and weak FECG from multichannel AECG with high precision in high noise condition, which is of vital importance for ensuring the safety of both the fetus and the mother, as well as the advancement of artificial intelligent clinical monitoring.

Abstract:Non-invasive fetal electrocardiogram (FECG) is used to monitor the electrical pulse of the fetal heart. Decomposing the FECG signal from maternal ECG (MECG) is a blind source separation problem, which is hard due to the low amplitude of FECG, the overlap of R waves, and the potential exposure to noise from different sources. Traditional decomposition techniques, such as adaptive filters, require tuning, alignment, or pre-configuration, such as modeling the noise or desired signal. to map MECG to FECG efficiently. The high correlation between maternal and fetal ECG parts decreases the performance of convolution layers. Therefore, the masking region of interest using the attention mechanism is performed for improving signal generators' precision. The sine activation function is also used since it could retain more details when converting two signal domains. Three available datasets from the Physionet, including A&D FECG, NI-FECG, and NI-FECG challenge, and one synthetic dataset using FECGSYN toolbox, are used to evaluate the performance. The proposed method could map abdominal MECG to scalp FECG with an average 98% R-Square [CI 95%: 97%, 99%] as the goodness of fit on A&D FECG dataset. Moreover, it achieved 99.7 % F1-score [CI 95%: 97.8-99.9], 99.6% F1-score [CI 95%: 98.2%, 99.9%] and 99.3% F1-score [CI 95%: 95.3%, 99.9%] for fetal QRS detection on, A&D FECG, NI-FECG and NI-FECG challenge datasets, respectively. These results are comparable to the state-of-the-art; thus, the proposed algorithm has the potential of being used for high-performance signal-to-signal conversion.

Abstract:Despite wearable electronic devices having enormous potential applications, the recording quality has not sufficiently met the requirements for fetal disease detection. In this article, we aim to propose a novel framework for fetal electrocardiography (ECG) extraction, where the hybrid approach mainly considers the combination of a novel adaptive maternal QRS removal (AQRSR) algorithm and a tensor-based joint blind source separation (JBSS) approach. To adapt to the dynamic change of the ECG signal and suppress the powerful maternal heartbeat, AQRSR is explored with an adaptive template, which is generated by a model consisting of a generator, discriminator, and transformer. By replacing the oldest segmentation with the newest cycle, the approach is capable of quickly adapting to the estimated output to match the new input. In addition, due to the significant crossover between the target signal and the noise, even if the maternal ECG removal processing is implemented, the residual signal inevitably contains fetal ECG, part of maternal ECG, and noise. As a consequence of this, a novel JBSS that incorporates tensor decomposition is formulated to separate fetal heartbeat from long-term recordings contaminated with maternal movements noise and heartbeat interference. By dividing the collected data into multiple segments, this method can extract fetal ECG signals by exploiting the signals that are not only statistically independent within each segment but can be dependent on different segments. Experimental results show that even if the recorded signals are contaminated by the noise, the proposed framework has comparable better performance on fetal ECG extraction.

Abstract:The abdominal electrocardiogram (ECG) provides a non-invasive method for monitoring the fetal cardiac activity in pregnant women. However, the temporal and frequency overlap between the fetal ECG (FECG), the maternal ECG (MECG) and noise results in a challenging source separation problem. This work seeks to compare temporal extraction methods for extracting the fetal signal and estimating fetal heart rate. A novel method for MECG cancelation using an echo state neural network (ESN) based filtering approach was compared with the least mean square (LMS), the recursive least square (RLS) adaptive filter and template subtraction (TS) techniques. Analysis was performed using real signals from two databases composing a total of 4 h 22 min of data from nine pregnant women with 37,452 reference fetal beats. The effects of preprocessing the signals was empirically evaluated. The results demonstrate that the ESN based algorithm performs best on the test data with an F1 measure of 90.2% as compared to the LMS (87.9%), RLS (88.2%) and the TS (89.3%) techniques. Results suggest that a higher baseline wander high pass cut-off frequency than traditionally used for FECG analysis significantly increases performance for all evaluated methods. Open source code for the benchmark methods are made available to allow comparison and reproducibility on the public domain data.

To extract the fetal electrocardiogram (ECG) from a single-lead maternal abdominal ECG, we need to solve both challenges. We propose a novel method to extract the fetal ECG from a single-lead maternal abdominal ECG, without any additional measurement. The algorithm is composed of three components. First, the maternal and fetal heart rates are estimated by the de-shape short time Fourier transform (STFT), which is a recently proposed nonlinear time-frequency analysis technique. The beat tracking technique is the second component which is applied to accurately obtain the maternal and fetal R peaks. The third component consists of establishing the maternal and fetal ECG waveforms by the nonlocal median. The algorithm is tested on two real databases with the annotation provided by experts (adfecgdb database and CinC2013 database) and a simulated database (fecgsym), and provides the state-of-the-art results. We conclude that with the proposed algorithm, the fetal ECG waveform and the fetal heart rate could be accurately obtained from the single-lead maternal abdominal ECG.

Abstract:Non-invasive foetal electrocardiography (NI-FECG) has become an important prenatal monitoring method in the hospital. However, due to its susceptibility to non-stationary noise sources and lack of robust extraction methods, the capture of high-quality NI-FECG remains a challenge. Recording waveforms of sufficient quality for clinical use typically requires human visual inspection of each recording. A Signal Quality Index (SQI) can help to automate this task but, contrary to adult ECG, work on SQIs for NI-FECG is sparse. In this paper, a multi-channel signal quality classifier for NI-FECG waveforms is presented. The model can be used during the capture of NI-FECG to assist technicians to record high-quality waveforms, which is currently a labour-intensive task. A Convolutional Neural Network (CNN) is trained to distinguish between NI-FECG segments of high and low quality. NI-FECG recordings with one maternal channel and three abdominal channels were collected from 100 subjects during a routine hospital screening (102.6 min of data). The model achieves an average 10-fold cross-validated AUC of 0.95 ± 0.02. The results show that the model can reliably assess the FECG signal quality on our dataset. The proposed model can improve the automated capture and analysis of NI-FECG as well as reduce technician labour time.

Abstract:Fetal electrocardiograms (FECGs) provide important clinical information for early diagnosis and intervention. However, FECG signals are extremely weak and are greatly influenced by noises. FECG signal extraction and detection are still challenging. In this work, we combined the fast independent component analysis (FastICA) algorithm with singular value decomposition (SVD) to extract FECG signals. The improved wavelet mode maximum method was applied to detect QRS waves and ST segments of FECG signals. We used the abdominal and direct fetal ECG database (ADFECGDB) and the Cardiology Challenge Database (PhysioNet2013) to verify the proposed algorithm. The signal-to-noise ratio of the best channel signal reached 45.028 dB and the issue of missing waveforms was addressed. The sensitivity, positive predictive value and F1 score of fetal QRS wave detection were 96.90%, 98.23%, and 95.24%, respectively. The proposed algorithm may be used as a new method for FECG signal extraction and detection.

Abstract:Estimation of the fetal heart rate (FHR) has gained interest in the last century, low heart rate variability has been studied to identify intrauterine growth restricted fetuses (prepartum), and abnormal FHR patterns have been associated with fetal distress during delivery (intrapartum). Several monitoring techniques have been proposed for FHR estimation, including auscultation and Doppler ultrasound. This thesis focuses on the extraction of the non-invasive fetal electrocardiogram (NI-FECG) recorded from a limited set of abdominal sensors. The main challenge with NI-FECG extraction techniques is the low signal-to-noise ratio of the FECG signal on the abdominal mixture signal which consists of a dominant maternal ECG component, FECG and noise. However the NI-FECG offers many advantages over the alternative fetal monitoring techniques, the most important one being the opportunity to enable morphological analysis of the FECG which is vital for determining whether an observed FHR event is normal or pathological. In order to advance the field of NI-FECG signal processing, the development of standardised public databases and benchmarking of a number of published and novel algorithms was necessary.

Abstract:Background: Fetal heart rate (FHR) extracted from abdominal electrocardiogram (ECG) is a powerful non-invasive method in appropriately assessing the fetus well-being during pregnancy. Despite significant advances in the field of electrocardiography, the analysis of fetal ECG (FECG) signal is considered a challenging issue which is mainly due to low signal to noise ratio (SNR) of FECG. Objective: In this study, we present an approach for accurately locating the fetal QRS complexes in non-invasive FECG. Materials and methods: In this experimental study, the proposed method included 4 steps. In step 1, comb notching filter was employed to pre-process the abdominal ECG (AECG). Furthermore, low frequency noises were omitted using wavelet decomposition. In next step, principal component analysis (PCA) and signal quality assessment (SQA) were used to obtain an optimal AECG reference channel for maternal R-peaks detection. In step 3, maternal ECG (MECG) was removed from mixture signal and FECG was extracted. In final step, the extracted FECG was first decomposed by discrete wavelet transforms at level 10. Then, by employing details of levels 2, 3, 4, the new FECG signal was reconstructed in which various noises and artifacts were removed and FECG components whose frequency were close to the fetal QRS complexes remained which increased the performance of the method. Results: For evaluation, 15 recordings of PhysioNet Noninvasive FECG database were used and the average F1 measure of 98.77% was obtained. Conclusion: The results indicate that use of both an efficient analysis of major component of AECG along with a signal quality assessment technique has a promising performance in FECG analysis.
