Percutaneous Curved Vertebroplasty in the Treatment of Thoracolumbar Osteoporotic Vertebral Compression Fractures: a Prospective Study

Abstract:Objective To investigate the clinical effect of percutaneous curved vertebroplasty in the treatment of thoracolum?bar osteoporotic vertebral compression fractures (OVCFs). Methods All of 85 patients with single thoracolumbar vertebral OVCFs who met the admission criteria from January 2017 to July 2018 were divided into three groups according to the random dig?its table method. They were treated with percutaneous curved vertebroplasty, routine unipedicular PVP and routine bipedicular PVP respectively. There were 25 patients in the percutaneous curved vertebroplasty group, 6 males and 19 females; aged 56-80 years, with an average age of 70.6±9.7 years. Fracture vertebral body distribution:T10 2 cases,T11 4 cases,T12 3 cases, L1 9 cases, L2 3 cases, L3 1 case, L4 1 case and L5 2 cases. There were 32 patients in the routine unipedicular PVP group, 6 males and 26 fe? males; aged 58-75 years, with an average age of 69.5±9.3 years. Fracture vertebral body distribution: T10 2 cases, T11 4 cases, T12 5 cases, L1 11 cases, L2 6 cases, L3 1 case, L4 1 case and L5 2 cases. There were 28 patients in the routine bipedicular PVP group, 5 males and 23 females; aged 59-81 years, with an average age of 69.8±8.8 years. Fracture vertebral body distribution: T10 2 cases, T11 4 cases, T12 4 cases, L1 10 cases, L2 4 cases, L3 1 case, L4 1 case and L5 2 cases. The operation time, injected cement volume, in?traoperative blood loss were recorded and analyzed. Preoperative, postoperative 1 week and 3 months visual analogue scale scores and oswestry disability index were adopted to value the clinical improvements. Preoperative, postoperative 1 week and 3 months relative vertebral height and kyphosis correction, and the cement leakage rate were measured and analyzed. Results There was no significant difference in the data of gender, age, VAS scores, ODI and distribution of fracture vertebrae among the three groups (P>0.05), and the baseline data was comparable. The average VAS score in the percutaneous curved vertebroplasty group was 2.3±0.5 at 1 week after surgery, that of the routine unipedicular PVP group was 2.4±0.4 and that of the routine bipe?dicular PVP group was 2.4±0.4; the average ODI in the percutaneous curved vertebroplasty group was 19.8%±3.9%, that of the routine unipedicular PVP group was 20.0%±4.1% and that of the routine bipedicular PVP group was 19.9%±3.8%; they were lower than the preoperative data, which were statistically significant (P<0.001). The average relative vertebral height in the percutaneous curved vertebroplasty group was 48.99%±9.23% at 3 months after surgery, that of the routine unipedicular PVP group was 47.11%±10.12% and that of the routine bipedicular PVP group was 46.71%±11.16%; the average kyphosis cor?rection in the percutaneous curved vertebroplasty group was 6.21%±1.94%, that of the routine unipedicular PVP group was 5.22%±2.07% and that of the routine bipedicular PVP group was 5.97%±2.09%; there was 1 cement leakage case in the per?cutaneous curved vertebroplasty group; those of the routine unipedicular PVP group were 4 cases and those of the routine bipe?dicular PVP group were 6 cases; there was no significant difference among the three groups (P>0.05). Operation time 39.10± 2.00 min vs 38.70±1.70 min, injected cement volume 3.60±0.11 ml vs 3.50±0.13 ml and blood loss 5.10±0.30 ml vs 5.00±0.40 ml of the percutaneous curved vertebroplasty group and the routine unipedicular PVP group were less than those of the routine bipedicular PVP group, which were statistically significant (P<0.05). Conclution Percutaneous curved vertebroplasty could achieve satisfactory clinical outcomes for OVCFs, with advantages of less operation time, less blood loss, limited X?ray expo?sure, less injected cement volume, and more balanced augmentation for stabilization of the affected vertebrae and total verte?bral column.

Abstract:Object To explore the feasibility and efficiency of treatment of minimally invasive percutaneous pedicle screws osteosynthesis(MIPPSO) and vertebroplasty by filling the injured vertebrae with injectable artificial bones on thoracolumbar fractures.Methods From Feb 2008 to Sep 2011,10 patients were treated with MIPPSO and vertebroplasty by filling the injured vertebrae with injectable artificial bones on thoracolumbar fractures.Results The average operative time was 80 minutes ﹙range,70~90min﹚,with a mean blood loss of 72ml﹙range,50~100ml﹚ and mean postoperative hospital stay of 8 days ﹙range,7~10 days﹚.23 cases were followed up 3~5 months ﹙average 3.5 months﹚,During the following-up,no case showed internalfixation device loosening or breakage;No case had anterior height loss of body of spine or collapse.CT results showed all injected artificial bones were absorbed within 3~4 months postoperatively.The anterior height and Cobb angle of the injured vertebrae body were significantly less than before treatment[﹙10.78�.53﹚%、﹙53.41�.35﹚%,P<0.0l;8.6�.9��22.72�.6� P<0.0l].Conclusion Fixation with vertebral pedicle screw system and vertebroplasty using injectable artificial bones is a safe and effective minimally invasivesurgical method.

Abstract:Objective. To investigate the clinical application of bone filling mesh container vertebroplasty in osteoporotic vertebral compression fractures (OVCFs). Methods. Patients with OVCF from October 2018 to April 2020 were selected. Patients in the control and study groups underwent percutaneous kyphoplasty (PKP) and bone filling mesh container vertebroplasty, respectively. The Oswestry Disability Index (ODI), Japanese Orthopaedic Association (JOA), visual analog scale (VAS) scores before and after surgery, and the incidence of complications were compared between the two groups. Results. The operation time and fluoroscopy time of the study group were significantly lower than those of the control group ( P < 0.05 ). There was no significant difference in the injection volume of bone cement between the study group and the control group ( P > 0.05 ). There was no significant difference in Cobb angle between the two groups. Three months after the operation, the height of the anterior edge increased and the Cobb angle decreased in the two groups ( P < 0.05 ), but there was no significant difference in the height of the anterior edge and the Cobb angle between the two groups ( P > 0.05 ). The JOA scores increased, while the ODI and VAS scores decreased in both groups after surgery ( P < 0.05 ). There was no significant difference in the total effective rate between the study group (96.15%) and the control group (92.31%) ( P > 0.05 ). The incidence of complications in the study group (3.85%) was significantly lower than that in the control group (15.38%) ( P < 0.05 ). Conclusions. For the treatment of OVCFs, bone filling mesh container vertebroplasty is comparable to PKP in terms of functional recovery, but it can safely reduce operative time, fluoroscopy time, and complication rates.

Abstract:OBJECTIVE:To determine the efficacy and feasibility of the combination of postural reduction and percutaneous kyphoplasty for traumatic thoracolumbar spine fractures.METHODS:Thirty-seven patients with single level traumatic thoracolumbar spine fractures were included in this study. There are 28 males and 9 females, with an average age of 48 years (range 24 to 79 years). Patients were treated with postural reduction and then percutaneous vertebroplasty with Calcium Phosphate Cement (CPC) were performed on the fractured vertebra. The results were quantitatively evaluated, according to the concept of estimated vertebral height loss and kyphotic angle of the vertebral fractures by preoperative and postoperative plain standing lateral radiographs. Visual analog scale (VAS) and the fracture vertebra shape changes were recorded.RESULTS:Patients were followed up for 9 to 24 months (average 13 months), pain was significantly relieved compared with the preoperative, VAS was reduced averagely from 7.6 +/- 2.5 to 1.8 +/- 1.5, the anterior and middle vertebral height was restored and kyphotic angle was corrected. During the period of follow up, outcomes were satisfactory, without notable correction loss.CONCLUSION:If the indications are correctly handled, the combination of postural reduction and percutaneous kyphoplasty for the treatment of traumatic thoracolumbar spine fractures can provide significant pain relieve and restore the vertebral height and kyphotic angle.

Abstract:Objective To determine the efficacy and feasibility of the combination of postural reduction and percutaneous vertebroplasty for traumatic thoracolumbar spine fractures.Methods Forty-eight consecutive patients with single level traumatic thoracolumbar spine fractures were included in this study.There are 36 males and 12 females,with average age of 50 years(range 24-76 years).Patients were treated with postural reduction by extending the operating table,then percutaneous bilateral transpedicular reduction of the endplate was performed,and calcium phosphate cement was injected.The results were quantitatively evaluated,according to the concept of estimated vertebral height loss and kyphotic angle of the vertebral fractures by radiographs.Visual analog scale(VAS)and the affected vertebra shape changes were recorded.Results Patients were followed up for 15-37 months(average 18 months);all patients reported significant alleviation of the pain after treatment.VAS scores was reduced averagely from 6.9±1.6 to 1.7±0.8.the anterior and middle vertebral height was restored and kyphotic abnormality was corrected.During the period of follow up,postoperative outcomes were satisfactory without notable corrected vertebral height loss.Conclusion If the indications are respected,percutaneous vertebroplasty following postural reduction can provide significant pain relief and restore the vertebral height and kyphotic angle in patients with traumatic thoracolumbar spine fractures.

Abstract:Objective: To compare the effect of percutaneous kyphoplasty (PKP) with different phases bone cement for treatment of osteoporotic vertebral compression fracture (OVCF). Methods: The clinical data of 219 OVCF patients who treated with PKP and met the selection criteria between June 2016 and May 2018 were retrospectively analyzed. According to the different time of intraoperative injection of bone cement, they were divided into observation group [116 cases, intraoperative injection of polymethyl methacrylate (PMMA) bone cement in low-viscosity wet-sand phase)] and control group (103 cases, intraoperative injection of PMMA bone cement in low-viscosity wire-drawing phase). There was no significance in general date of gender, age, disease duration, body mass index, bone mineral density T value, fracture vertebral body, preoperative fracture severity of the responsible vertebral body, anterior height ratio of the responsible vertebral body, preoperative pain visual analogue scale (VAS) score, and Oswestry disability index (ODI) between the two groups ( P>0.05). The VAS score and ODI score were used to evaluate the improvement of patients' symptoms at immediate, 2 days, 3 months after operation and at last follow-up. At 1 day, 3 months after operation, and at last follow-up, X-ray film and CT of spine were reexamined to observe the distribution of bone cement in the vertebral body, bone cement leakage, and other complications. During the follow-up, the refracture rate of the responsible vertebral body and the fracture rate of the adjacent vertebral body were recorded. Results: The injection amount of bone cement in the observation group and control group were (4.53±0.45) mL and (4.49±0.57) mL, respectively, showing no significant difference between the two groups ( t=1.018, P=0.310). Patients in both groups were followed up 6-18 months (mean, 13.3 months). There were 95 cases (81.9%) and 72 cases (69.9%) of the bone cement distribution range more than 49% of the cross-sectional area of the vertebral body in the observation group and the control group, respectively, showing significant difference in the incidence between the two groups ( χ 2=4.334, P=0.037). The VAS score and ODI score of the postoperative time points were significantly improved compared with those before operation ( P<0.05), and there were significant differences among the postoperative time points ( P<0.05). The VAS score and ODI score of the observation group were significantly better than those of the control group ( P<0.05) at immediate, 2 days, and 3 months after operation, and there was no significant difference between the two groups at last follow-up ( P>0.05). At 1 day after operation, the cement leakage occurred in 18 cases of the observation group (8 cases of venous leakage, 6 cases of paravertebral leakage, 4 cases of intradiscal leakage) and in 22 cases of the control group (9 cases of venous leakage, 8 cases of paravertebral leakage, 5 cases of intradiscal leakage). There was no significant difference between the two groups ( P>0.05). During the follow-up, 5 cases (4.3%) in the observation group, 12 cases (11.7%) in the control group had responsible vertebral refracture, and 6 cases (5.2%) in the observation group and 14 cases (13.6%) in the control group had adjacent vertebral fracture, the differences were significant ( χ 2=4.105, P=0.043; χ 2=4.661, P=0.031). Conclusion: Bone cement injection with wet-sand phase in PKP is beneficial for the bone cement evenly distributed, strengthening the responsible vertebral, relieving the short-term pain after operation, decreasing the rate of responsible vertebral refracture and adjacent vertebral fracture without increasing the incidence of relevant complications and can enhance the effectiveness.
Puncture assisted by a "TINAVI" orthopaedic robot versus freehand puncture in vertebroplaty for osteoporotic vertebral compression fracture of the upper thoracic vertebra

Abstract:Objective:To compare the clinical efficacy between puncture assisted by a "TINAVI" orthopaedic robot versus freehand puncture in vertebroplaty for osteoporotic vertebral compression fracture(OVCF) of the upper thoracic vertebra.Methods:A retrospective study was conducted of the 19 patients (20 vertebral bodies) with OVCF of the upper thoracic vertebra who had been treated at Department of Spine Surgery, Honghui Hospital from January 2018 to March 2019 by robotic vertebroplasty (robot group) and of another 21 counterpart patients (21 vertebral bodies) who had been treated by conventional vertebroplasty from January 2016 to December 2017 (freehand group). Puncture was conducted by a "TINAVI" orthopaedic robot in the robotic vertebroplasty but freehand in the conventional vertebroplasty. The robot group had 5 males and 14 females, aged from 62 to 88 years; the freehand group had 6 males and 15 females, aged from 64 to 83 years. The 2 groups were compared in terms of operation time, bone cement volume, postoperative complications (cement leakage, infection and embolism), visual analogue scale (VAS), Oswestry disability index (ODI), anterior height (AH) and kyphosis angulation (KA) of the injured vertebra at day 1 and last follow-up after surgery.Results:The 2 groups were comparable because there were no significant differences between them in the preoperative general data ( P>0.05). Vertebroplasty via unilateral puncture approach was completed uneventfully in the 19 patients (20 vertebral bodies) in the robot group and in the 21 patients (21 vertebral bodies) in the freehand group. The 40 patients were followed up for 6 to 12 months (mean, 8.3 month). The operation time [(37.9±8.2) min], bone cement volume [(2.3±0.9) mL] and rate of cement leakage (10.0%, 2/20) in the robot group were all significantly less or lower than those in the freehand group [(46.2±9.4) min, (4.2±1.3) mL and 42.9% (9/21)] ( P<0.05). No infection or embolism was observed in either group. There were no significant differences between the 2 groups in VAS, ODI, AH or KA of the injured vertebra at day 1 or last follow-up after surgery ( P>0.05). Conclusion:In vertebroplaty for OVCF of the upper thoracic vertebra, compared with conventional freehand puncture, puncture assisted by a "TINAVI" orthopaedic robot can lead to satisfactory clinical efficacy because it reduces operation time, volume of bone cement injection, and thus incidence of bone cement leakage.
Treatment of Thoracolumbar Fracture by Combined Stabilization with Pedicle-screw and Vertebroplasty through Approach of Para-vertebral Muscle Sparing

Abstract:Objective To investigate the surgical technique and clinical outcome of treatment of acute traumatic thoracolumbar fractures through the approach of para-vertebral muscle sparing by transpedicular vertebroplasty with artificial cement and posterior instrumentation.Methods Thirty-one patients,22 males and 9 females,averaging 46 years of age with single level traumatic thoracolumbar fracture with light or without neurologic deficit were prospectively studied.These patients had 22 compression fractures and 9 burst fractures,located 3 at T11,9 at T12,14 at L1 and 5 at L2.They were treated by short-segment pedicle-screw reduction and fixation and augmentation of the injured vertebra with quick hardening artificial cement through the approach of para-vertebral muscle sparing.Vertebral central and anterior height,kyphotic angle of the vertebral fractures by radiographs and visual analog scale were calculated pre-operatively and post-operatively.Results All 31 patients were operated successfully and were followed for 12-36 months.None neurologic deterioration was observed.The operating time was averaged 95 min and blood loss was 150 mL.All patients reported significant pain alleviation shortly after operation.VAS scores was reduced from 8.97±0.66 to 3.23±0.98 and anterior vertebral body height compressional ratio improved from 44.58±6.71 before surgery to 5.43±1.63 after surgery,whereas the central vertebral body height was improved from 36.42±5.56 to 4.01±0.90; and sustained kypotic abnormality was corrected.Cement was ossified within 3-8 months after operation.There was no instrumentation failure or measurable loss of sagittal curve and vertebral body height correction in any group of patients.Conclusion Posterior instrumentation combing with transpedicular vertebroplasty with artificial cement through the approach of para-vertebral muscle sparing seems to b a feasible option in the management of some thoracolumbar fracture and is less invasive restoring vertebral anterior-middle column strength and decreasing the failure of instrumentation.
Short-segment Pedicle Screw Fixation Combined with Polymethyl Methacrylate Vertebroplasty in Treating Thoracolumbar Osteoporotic Compression Fractures

Abstract:Objective To evaluate the clinical efficacy and safety of short-segment pedicle screw fixation combined with vertebroplasty for the treatment of thoracolumbar osteoporotic compression fractures.Methods A retrospective case control study was conducted on 63 patients with fresh thoracic or lumbar osteoporotie compression fractures who were surgically treated from January 2010 to December 2013.There were 26 males and 37 females,with age of 63-87 years [(76.3 ± 5.7) years].According to the surgical method,the patients were assigned to simple vertebroplasty group (Group A),short-segment pedicle screw fixation group (Group B),and short-segment pedicle screw fixation group combined with vertebroplasty group (Group C),with 21 cases in each group.Length of hospital stay,operation time,and blood loss were recorded.The visual analog scale (VAS),anterior vertebral body height,angle of the kyphotic deformity,and complications before operation,immediately after operation,3 months after operation,and at the last follow up were compared among three groups.Complications of each group were recorded.Results The hospital stay,operation time,and blood loss in Group C were significantly higher than those in Group A (P ＜ 0.05),but there were no significant differences between Group B and Group C (P ＞ 0.05),except for a longer operation time in Group C.The pre-operative VAS showed no significant difference among three groups (P ＞ 0.05).However,the mean VAS in Groups A,B and C at the last follow up were 1.0(0.0,2.0)points,1.0(0.0,2.0)points,0.0(0.0,1.0)points,respectively.The VAS in Group C was significantly lower than that in Group B or A (P ＜ 0.05).The mean anterior vertebral body height and angle of the kyphotic deformity in Group C had no significant difference from that in Group A or B before operation and immediately after operation (P ＞ 0.05).At the last follow up,the mean anterior vertebral body height and angle of the kyphotic deformity in Groups A,B and C were (68 ±14.7)％,(72.3 ±9.0)％,(81.5 ±5.6)％ and (10.6 ±3.9)°,(10.7 ±5.0)°,(7.4 ± 5.0) °,respectively.The loss of anterior vertebral body height and angle of the kyphotic deformity correction in Group C were significantly less than that in Group A or B (P ＜ 0.05).Superficial infection was found in Groups B (n =2) and C (n =1),and the infection was cured after antibiotic therapy and dress change.Bone cement leakage was found in Groups A (n =8) and C (n =5),with no nerve compression.Internal fixation failure was seen in Group B (n =3),and the implant was removed directly.Conclusion Short-segment pedicle screw fixation combined with vertebroplasty can effectively reduce the loss of anterior vertebral body height and angle of the kyphotic deformity and hence enhance the clinical efficacy.

[Confidence high viscosity bone cement system and postural reduction in treating acute severe osteoporotic vertebral compression fractures]

Abstract:Objective: To evaluate the effectiveness of Confidence high viscosity bone cement system and postural reduction in treating acute severe osteoporotic vertebral compression fracture (OVCF). Methods: Between June 2004 and June 2009, 34 patients with acute severe OVCF were treated with Confidence high viscosity bone cement system and postural reduction. There were 14 males and 20 females with an average age of 72.6 years (range, 62-88 years). All patients had single thoracolumbar fracture, including 4 cases of T11, 10 of T12, 15 of L1, 4 of L2, and 1 of L3. The bone density measurement showed that T value was less than -2.5. The time from injury to admission was 2-72 hours. All cases were treated with postural reduction preoperatively. The time of reduction in over-extending position was 7-14 days. All patients were injected unilaterally. The injected volume of high viscosity bone cement was 2-6 mL (mean, 3.2 mL). Results: Cement leakage was found in 3 cases (8.8%) during operation, including leakage into intervertebral space in 2 cases and into adjacent paravertebral soft tissue in 1 case. No clinical symptom was observed and no treatment was performed. No pulmonary embolism, infection, nerve injury, or other complications occurred in all patients. All patients were followed up 12-38 months (mean, 18.5 months). Postoperatively, complete pain relief was achieved in 31 cases and partial pain relief in 3 cases; no re-fracture or loosening at the interface occurred. At 3 days after operation and last follow-up, the anterior and middle vertebral column height, Cobb angle, and visual analogue scale (VAS) score were improved significantly when compared with those before operation (P < 0.05); and there was no significant difference between 3 days and last follow-up (P > 0.05). Conclusion: Confidence high viscosity bone cement system and postural reduction can be employed safely in treating acute severe OVCF, which has many merits of high viscosity, long time for injection, and easy-to-control directionally.

Methods: From January 2019 to March 2020, a total of 83 patients were enrolled and reviewed. The patients were divided into two groups according to the operation method. The clinical and radiographic parameters were recorded and compared between these two groups. Those who received percutaneous vertebroplasty with haemorrhage aspiration were recorded as group A (n=42). In group A, the haemorrhage in the vertebral fracture was aspirated compared with conventional percutaneous vertebroplasty. Patients who underwent conventional percutaneous vertebroplasty were classified as group B (n=41). Results: Visual analogue scale (VAS) values and Oswestry Disability Index (ODI) scores showed no significant difference between the two groups preoperatively, postoperatively or at the final follow-up (FU) (P> 0.05). The intraoperative VAS score (bone cement injection) in group A was significantly lower than that in group B (3.83± 0.79 vs 5.44± 1.32, P < 0.01). The local kyphotic angle (LKA) (final follow-up), LKA loss, fractured vertebral anterior height loss (FVAHL) and anterior vertebral height loss ratio (AVHLR) were significantly lower in group A than in group B. The anterior vertebral height ratio (AVHR) at the final FU in group A was higher than that in group B (P=0.013). The distribution of bone cement was significantly different (P=0.034). By analysing the distribution pattern of bone cement, it was found that the values of LKA loss, FVAHL and AVHLR were superior in the type A bone cement distribution to those in types B and C. Conclusion: Compared with traditional surgical methods, bone haemorrhage aspiration could improve the diffusion degree of bone cement and reduce the height loss and deformity of injured vertebrae. This method provides a feasible new scheme for improving the dispersion of bone cement. Keywords: osteoporotic vertebral compression fractures, percutaneous vertebroplasty, bone cement distribution, fracture haemorrhage aspiration Osteoporotic vertebral compression fractures (OVCFs) are the most common fractures in senior citizens, and the incidence of OVCFs increases with age. 1–4 OVCFs cause back pain, neurological symptoms, functional limitations, malformations and a reduction in quality of life. Percutaneous vertebroplasty (PV) is a commonly used surgical treatment for patients with OVCFs. This treatment can effectively strengthen the compressed vertebral body, relieve the pain caused by thoracolumbar fractures, improve the quality of life of patients, and avoid the complications caused by long-term bedrest. 5–7 Recent studies have found that the distribution of bone cement may affect vertebral body height restoration and local kyphosis, and a better dispersion distribution may indicate better clinical recovery. 1,3,8,9 However, the existing articles do not reveal the factors affecting bone cement distribution patterns of the vertebral body, and we believe that bleeding inside the fracture occupies part of the space in the vertebral body. Therefore, during PV surgical puncture, the fracture haemorrhage was aspirated and then filled with bone cement to improve the distribution pattern of bone cement. Additionally, this method was compared with traditional surgical methods. The study was approved by the Institutional Review Board of The First Affiliated Hospital of Chongqing Medical University and was conducted according to the principles of the Declaration of Helsinki. All the patients provided their written informed consent to participate in our study prior to the storage of their data in the hospital database. Patients who received PV treatment from January 2019 to March 2020 were reviewed. Abstract:By establishing a finite element biomechanical model of surgical vertebral refracture after PVP in patients with OVCF, we found that when the contact spacing between the upper boundary of the bone cement and the vertebral body's upper endplates is 0 mm, the pressure generated by the load can be effectively eliminated and transmitted, thus reducing the stresses exerted on cancellous bone, and further reducing the risk of vertebral refracture in the postoperative period. Objective Patients with osteoporotic vertebral compression fractures (OVCF) treated with vertebroplasty (PVP) are experiencing an increasing number of problems such as pain recurrence, mainly due to recompression fractures of the operated vertebral body within a certain period of time after the operation, which is closely related to the distribution of intraoperative bone cement. The aim of this study is to investigate the effect of different spacing distributions between the upper boundary of the cement and the upper endplate of the operated vertebra on the biomechanics of the operated vertebra after percutaneous vertebroplasty for OVCF using finite element analysis (FEA). Methods One patient with L1 vertebral body OVCF was selected, and computed tomography (CT) of the thoracolumbar segment was performed. The CT data were extracted to establish an FEA model of the T12–L2 vertebral bodies. Bone cement was injected into the L1 vertebral body. Based on the spacing between the upper boundary of the bone cement and the vertebral body's upper endplates, the model vertebrae were divided into 0, 2, 4, and 6 mm spacing groups, and the human body's upright, flexion–extension, lateral flexion, and rotational positions were simulated. The biomechanical effects of different spacing distributions on the postoperative L1 vertebral body and the injected bone cement were evaluated. Results In this paper, we found that the Von Mises stress of the L1 vertebrae was the smallest when the spacing between the upper boundary of the bone cement and the vertebral body's upper endplates was 0 mm. The larger the spacing in a certain range between the upper boundary of the bone cement and the vertebral body's upper endplates, the greater the Von Mises stress of the L1 vertebrae. However, in the stress comparison of the injected bone cement, the Von Mises stress of the bone cement was greatest when the spacing between the upper boundary of the bone cement and the upper endplate of the vertebral body was 0 mm; the larger the spacing, the smaller the Von Mises stress. Conclusion When the contact spacing between the upper boundary of the bone cement and the upper endplate of the vertebral body is 0 mm, it can effectively eliminate and transfer the pressure caused by the load, thus reducing the stress on the cancellous bone and further reducing the risk of vertebral refracture after surgery.

Abstract: This study provides a baseline examination of teenage sexual relationships in 1991 in a sample drawn from 9 state and independent schools in southwest England; a follow up was conducted in 1992. 1025 students in year 11 and aged 15-16 responded in 1991; the follow up in 1992 showed 315 returning the questionnaire. Information was requested on knowledge of sexually transmitted diseases (STDs), age at first intercourse, sexual intercourse without use of contraception, the frequency of "one night stands" or relationships measured in days, and knowledge of friends who had STDs. Relative risk ratios were calculated for those engaging in sexual activity before and after the age of 16 years. The results showed that in the follow up more girls and persons with more academic interests, as represented by the school subjects taken for the General Certificate of Secondary Education, responded. There were no differences by social class. The average age of the sample was 16.9 years. 54% were sexually active. 84 had sexual intercourse when they were under 16 years and 84 has sexual intercourse between the ages of 16 and the administration of the questionnaire. Those engaging in intercourse prior to the age of 16 were twice as likely to not have used contraception (relative risk [RR] = 1.86, confidence interval [CI] = 1.33 - 2.60 at 95%). They also were more likely to have had sex within a short relationship (RR = 2.24, CI = 1.38 - 3.62). They were 3 times more likely to know a close friend with an STD (RR = 2.97, CI = 1.55 - 5.70). Only 5 of the nonsexually active teenagers knew someone with an STD. The extent of sexual activity was lower than reported from other surveys. The results confirm prior understanding that younger teenagers engage in more sexual risk taking and are less likely to use contraception. The suggestion is that this risk-taking behavior is related to the cognitive development of younger teenagers of the age of first intercourse.

Abstract:Abstract Purpose Percutaneous vertebroplasty (PVP) is a routine operation for the treatment of osteoporotic lumbar compression fractures (OLCFs). As is well known, unilateral puncture is a common method. However, with the conventional transpedicular approach (CTPA), the cement may be asymmetrically dispersed, so some surgeons use the transverse process root-pedicle approach (TPRPA). The objective of this study was to compare the clinical results and bone cement distribution of PVP for OLCF with unilateral TPRPA and CTPA to identify the advantages and disadvantages of the two surgical options. Patients and methods From January 2016 to June 2019, seventy-two elderly patients who underwent unilateral PVP for single-level OLCF were retrospectively reviewed. Operation time, injection amount and type of bone cement distribution, and bone cement leakage and surgical complications were recorded. The visual analog scale (VAS) scores and Oswestry disability index (ODI) scores were used to evaluate the clinical results. All patients were followed up for more than 12 months, and the assessment was based primarily on clinical and radiological outcomes. Results There were significant differences in the surgical time and the volume and the type of bone cement distribution and the lost of operative vertebra height between the two groups. However, there was no significant difference in bone cement leakage. Moreover, there were no significant differences in VAS and ODI between the two groups at 2 days and 12 months after the operation. Conclusions Unilateral TPRPA and CTPA are practical and feasible methods in PVP for the treatment of OLCF, and they have similar clinical effects. However, TPRPA has the advantages of a better distribution of bone cement and a shorter operation time and a better maintenance effect of injured vertebra height, without increasing the rate of bone cement leakage.

Abstract:With the ageing of the global population, the incidence of osteoporotic vertebral compression fractures (OVCFs) is increasing. To assess the safety and efficacy of O-arm- and guide-device-assisted personalized percutaneous kyphoplasty (PKP) for treating thoracolumbar OVCFs, a total of 38 consecutive thoracolumbar OVCF patients who underwent bilateral PKP assisted with an O-arm and a guide device (O-GD group, n = 16) or traditional fluoroscopy (TF group, n = 22) from January 2020 to December 2021 were retrospectively reviewed, and their epidemiologic, clinical and radiological outcomes were analysed. The operation time was significantly decreased (p < 0.001) in the O-GD group (38.3 ± 12.2 min) compared with the TF group (57.2 ± 9.7 min). The number of intraoperative fluoroscopy exposures was significantly decreased (p < 0.001) in the O-GD group (31.9 ± 4.5) compared with the TF group (46.7 ± 7.2). Intraoperative blood loss was significantly decreased (p = 0.031) in the O-GD group (6.9 ± 2.5 mL) compared with the TF group (9.1 ± 3.3 mL). No significant difference (p = 0.854) in the volume of injected cement was observed between the O-GD group (6.8 ± 1.3 mL) and the TF group (6.7 ± 1.7 mL). Both the clinical and radiological outcomes, including the visual analogue scale score for pain, Oswestry Disability Index and anterior height and local kyphotic angle of the fractured vertebrae, were significantly improved at the postoperative and final follow-up but did not differ between the two groups. The incidence of cement leakage and refracture of the vertebral body was similar in the two groups (p = 0.272; p = 0.871). Our preliminary study demonstrated that O-GD-assisted PKP is a safe and effective procedure that presents a significantly shorter operation time, fewer intraoperative fluoroscopy exposures and less intraoperative blood loss than the TF technique.

Abstract:Objective: To investigate the biomechanical effects of percutaneous vertebroplasty combined with cement pedicle plasty (PVCPP) on the unstable osteoporotic vertebral fractures (OVFs) through finite element (FE) analysis. The study compares the biomechanical stability of finite element models between percutaneous vertebroplasty (PVP) and percutaneous vertebroplasty combined with cement pedicle plasty. Methods: Two patients with unstable OVFs underwent computed tomography (CT) examination at the thoracolumbar vertebral body levels, respectively. The CT images were reconstructed into three-dimensional finite element models to simulate stress conditions across six dimensions and to evaluate the vertebral von Mises stress before and after bone cement reinforcement. Results: The study found that stress distribution differed between groups mainly at the pedicle base. In the surgical vertebral bodies, the maximum stress in the PVP group decreased during flexion and left bending, while it increased in other states. In the PVCPP group, all maximum stresses decreased. In the inferior vertebral bodies, the maximum stress in the PVP group generally increased, while it decreased in the PVCPP group. In the superior vertebral bodies, postoperatively, the maximum stress in the PVP group generally increased, while it almost remained unchanged in the PVCPP group. PVP group had higher cement stress and displacement. Conclusion: PVCPP is an effective treatment method for patients with unstable OVFs. It can quickly relieve pain and enhance the stability of the three columns, thereby reducing the risk of some complications.

Abstract:Background Osteoporosis vertebral compression fractures (OVCF) are common with the aging process. This study aimed to compare the effects of unilateral transverse process extrapedicular (UEPKP) and bilateral transpedicular percutaneous kyphoplasty (BTPKP) for patients with thoracolumbar OVCF. Methods Data from 136 patients with OVCF treated with single-level PKP in our hospital between May 2019 and April 2021 were studied. Patients were grouped based on surgical procedure: there were 62 patients in the UEPKP group and 74 in the BTPKP group. All clinical and radiological data were collected from medical records. Clinical outcomes, including visual analog scale (VAS), Oswestry Disability Index (ODI), and Japanese Orthopaedic Association (JOA) scores of the lumbar spine, were evaluated preoperatively, postoperatively, and at the follow-up visit. The radiological evaluations (anterior vertebral height rate and local kyphosis angle) and complications were also collected. Results All patients had successfully improved after surgery. In the UEPKP group, patients showed a significantly shorter operating time and lower fluoroscopy frequency than patients in the BTPKP group ( p < 0.05). However, a significantly better distribution score and cement volume were found in the BTPKP group ( p < 0.05). The UEPKP group achieved a significantly better VAS score (0.6 ± 0.5 vs. 0.9 ± 0.8) and ODI (24.7 ± 3.1 vs. 27.5 ± 1.8) at the final follow-up visit than the BTPKP group ( p < 0.05). The UEPKP group showed significantly worse radiological outcomes (anterior height rate and local kyphosis angle) at the 6- and 12-month follow-ups ( p < 0.05). As for complications, the UEPKP group showed significantly fewer facet joint violations and intraspinal leakages ( p < 0.05). Conclusion UEPKP could be a safe and effective alternative procedure for patients with thoracolumbar osteoporotic vertebral compression fracture, which possesses an apparent advantage in reducing intraspinal leakage and facet joint violation over BTPKP.
