Abstract:Objective To investigate the clinical outcomes in the treatment of Osteoporotic Vertebral Compression Fractures with Percutaneous Vertebroplasty(PVP).Methods The clinical results of a group of 21 patients,who had suffered from the Osteoporotic Vertebral Compression Fractures and treated with Percutaneous Vertebroplasty,were compared Fractures and treated with Percutaneous Vertebroplasty,were compared with another group of 19 patients treated by non-operative procedure.Results The duration of hospitalization,the time of pain remission and the out of bed activity were significantly reduced in the PVP.group as compared with those undergone non-operative procedure(P<0.01).On following up from six months to tweleve months,the anterior height of injured vertebral body of all patients were generally recovered obviously(P<0.05),and the Cobb angles were not rectified obviously(P>0.05).Conclusion The PVP is an effective procedure of simple microinjure,with less complication,wide indication and good outcome,and it is essential to master its indication meanwhile.
Percutaneous Curved Vertebroplasty in the Treatment of Thoracolumbar Osteoporotic Vertebral Compression Fractures: a Prospective Study

Abstract:Objective To investigate the clinical effect of percutaneous curved vertebroplasty in the treatment of thoracolum?bar osteoporotic vertebral compression fractures (OVCFs). Methods All of 85 patients with single thoracolumbar vertebral OVCFs who met the admission criteria from January 2017 to July 2018 were divided into three groups according to the random dig?its table method. They were treated with percutaneous curved vertebroplasty, routine unipedicular PVP and routine bipedicular PVP respectively. There were 25 patients in the percutaneous curved vertebroplasty group, 6 males and 19 females; aged 56-80 years, with an average age of 70.6±9.7 years. Fracture vertebral body distribution:T10 2 cases,T11 4 cases,T12 3 cases, L1 9 cases, L2 3 cases, L3 1 case, L4 1 case and L5 2 cases. There were 32 patients in the routine unipedicular PVP group, 6 males and 26 fe? males; aged 58-75 years, with an average age of 69.5±9.3 years. Fracture vertebral body distribution: T10 2 cases, T11 4 cases, T12 5 cases, L1 11 cases, L2 6 cases, L3 1 case, L4 1 case and L5 2 cases. There were 28 patients in the routine bipedicular PVP group, 5 males and 23 females; aged 59-81 years, with an average age of 69.8±8.8 years. Fracture vertebral body distribution: T10 2 cases, T11 4 cases, T12 4 cases, L1 10 cases, L2 4 cases, L3 1 case, L4 1 case and L5 2 cases. The operation time, injected cement volume, in?traoperative blood loss were recorded and analyzed. Preoperative, postoperative 1 week and 3 months visual analogue scale scores and oswestry disability index were adopted to value the clinical improvements. Preoperative, postoperative 1 week and 3 months relative vertebral height and kyphosis correction, and the cement leakage rate were measured and analyzed. Results There was no significant difference in the data of gender, age, VAS scores, ODI and distribution of fracture vertebrae among the three groups (P>0.05), and the baseline data was comparable. The average VAS score in the percutaneous curved vertebroplasty group was 2.3±0.5 at 1 week after surgery, that of the routine unipedicular PVP group was 2.4±0.4 and that of the routine bipe?dicular PVP group was 2.4±0.4; the average ODI in the percutaneous curved vertebroplasty group was 19.8%±3.9%, that of the routine unipedicular PVP group was 20.0%±4.1% and that of the routine bipedicular PVP group was 19.9%±3.8%; they were lower than the preoperative data, which were statistically significant (P<0.001). The average relative vertebral height in the percutaneous curved vertebroplasty group was 48.99%±9.23% at 3 months after surgery, that of the routine unipedicular PVP group was 47.11%±10.12% and that of the routine bipedicular PVP group was 46.71%±11.16%; the average kyphosis cor?rection in the percutaneous curved vertebroplasty group was 6.21%±1.94%, that of the routine unipedicular PVP group was 5.22%±2.07% and that of the routine bipedicular PVP group was 5.97%±2.09%; there was 1 cement leakage case in the per?cutaneous curved vertebroplasty group; those of the routine unipedicular PVP group were 4 cases and those of the routine bipe?dicular PVP group were 6 cases; there was no significant difference among the three groups (P>0.05). Operation time 39.10± 2.00 min vs 38.70±1.70 min, injected cement volume 3.60±0.11 ml vs 3.50±0.13 ml and blood loss 5.10±0.30 ml vs 5.00±0.40 ml of the percutaneous curved vertebroplasty group and the routine unipedicular PVP group were less than those of the routine bipedicular PVP group, which were statistically significant (P<0.05). Conclution Percutaneous curved vertebroplasty could achieve satisfactory clinical outcomes for OVCFs, with advantages of less operation time, less blood loss, limited X?ray expo?sure, less injected cement volume, and more balanced augmentation for stabilization of the affected vertebrae and total verte?bral column.
Clinical Effect of PKP in Treatment of Patients with Multi-segmental Osteoporotic Vertebral Compressive Frac-tures

Abstract:Objective To investigate clinical effect and complication of percutaneous kyphoplasty (PKP) in treatment of multi-segmental osteoporotic vertebral compressive fractures ( OVCF) . Methods A total of 73 patients with osteoporotic vertebral compressive fractures OVCF during February 2008 and July 2013 were recruited in this study, which included 60 patients with two-segmental OVCF and 13 patients with three-segmental OVCF, and there were 159 vertebral bodies altogether. All new fractured vertebral bodies were treated with PKP. Conditions of symptom improve-ment were evaluated by Visual analogue score ( VAS) and Oswestry disability index ( ODI) sore before and after surgery. Radiographic outcomes were evaluated using indexes such as vertebral compression degree, restoration degree of vertebral height and kyphosis losing degree in postoperative 1 year. Conditions of distribution and penetrance of bone cement were studied by postoperative computed tomography scan. Results Symptomatic relief rate of patients were 95. 89% (70/73), and postoperative computed tomography scan showed that the total penetrance of bone cement was 31. 51%. Values of VAS and ODI score in postoperative 1 week and 1 year were lower than those before operation(P<0. 01), and values of VAS and ODI score in postoperative 1 year were lower than those in postoperative 1 week (P<0. 01). Conclusion PKP can effectively treat multi-segmental OVCF with satisfactory analgesic effect, but incident rates of bone cement leak-aging and second fracture are high, and therefore anti-osteoporotic medication should be supplemented.
Efficacy analysis of percutaneous pedicle screw fixation combined with percutaneous vertebroplasty in the treatment of osteoporotic vertebral compression fractures with kyphosis

Abstract:Abstract Background To investigate the clinical effect of percutaneous pedicle screw fixation (PPSF) combined with percutaneous vertebroplasty (PVP) in the treatment of osteoporotic compression vertebral fracture (OVCF) of the thoracolumbar vertebra with kyphosis. Methods One hundred sixty-six patients before June 2017 were retrospectively analyzed, and patients were divided into PPSF + PVP group A and PVP group B. Operative time, bone mineral density, postoperative bed time, high compression ratio, bone cement leakage rate, and bone cement dose were recorded. Comparison of vertebral anterior edge height, Cobb angle, visual analogue score (VAS), and low back pain dysfunction index (ODI) between the two groups in preoperative, postoperative 3 days, postoperative 6 months, postoperative 12 months, and postoperative 24 months, postoperative complications were observed in the two groups. Results The operation time of group A was longer than that of group B (59.0 ± 8.6 min, 26.6 ± 5.2 min), longer postoperative bed rest time (3.3 ± 0.7 days, 1.2 ± 0.5 days), the differences were statistically significant ( P < 0.01), there was no difference in the amount of bone cement between the two groups (5.4 ± 0.6 ml, 5.3 ± 0.8 ml) ( P > 0.05). The height of the anterior edge and Cobb angle of the two groups recovered significantly in postoperative 3 days. The height of anterior edge (2.7 ± 0.3 cm, 2.6 ± 0.2 cm, 2.5 ± 0.7 cm; 2.3 ± 0.6 cm, 1.7 ± 0.5 cm, 1.6 ± 0.3 cm) and Cobb angle (4.9 ± 2.2, 5.5 ± 2.3, 5.7 ± 2.3; 12.4 ± 3.2, 17.2 ± 2.5, 13.2 ± 2.3) was statistically significant in postoperative 6 months, postoperative 12 months, and postoperative 24 months ( P < 0.01). VAS and ODI scores of postoperative 6 months and 12 months were significantly different between the two groups ( P < 0.05). Postoperative complications in group B were much higher than those in group A. Conclusion The efficacy of PVP alone was not satisfactory, and the rate of complications was high for OVCF patients with severe anterior edge compression with kyphosis. PPSF combined with PVP is recommended, the vertebral height loss was not obvious, the satisfaction was good, and the complication rate was lower during 2 years follow-up.

Vertebroplasty versus kyphoplasty in osteoporotic vertebral compression fracture: a meta-analysis of prospective comparative studies

Abstract:PurposeThe goal of this article is to evaluate the efficacy and the safety of the percutaneous vertebroplasty (PVP) versus percutaneous kyphoplasty (PKP) in dealing with the osteoporotic vertebral compression fracture (OVCF).MethodsIn July 2014, a comprehensive systematic computer-based online search was performed by using the databases of PubMed, EMBASE, Cochrane Library, Web of Science, Wan Fang, and the China Biological Medicine. Only prospective comparative trials (PCT) and randomized controlled trials (RCT) that compared PVP with PKP were included. Trials were screened based on the inclusion and exclusion criteria previously formed. The Cochrane collaboration guidelines were also used to assess the quality of these included studies. The primary data of these studies [volume of the cement, postoperative vertebral height, visual analog scale (VAS) score and Oswestry Disability Index (ODI) score after the surgery, and so on] were carefully abstracted and processed by Revman 5.2.0 software The publication bias of the main results (cement leakage and adjacent-level fracture) were examined by Stata 12.0 (Begg and Egger test). Furthermore, the stability of the main results were also detected by sensitivity and cumulative analyses.ResultsSix RCT and 14 PCT studies involving 1,429 patients met our criteria and were included finally. Comparing these two methods, the PKP group took more operation time [SMD = 0.66, 95 % CI (0.28, 1.03), p = 0.0006] with higher anterior vertebral body height [SMD = 1.40, 95 % CI (0.49, 2.32), p = 0.003], greatly reduced Cobb angle in the long run [SMD = −0.61, 95 % CI (−1.04, −0.19), p = 0.005] and had lower risk of cement leakage. However, in VAS scores and ODI scores after the surgery whether for the short-term efficacy (no more than 1 week after the surgery) or long-term efficacy (more than six months), Cobb angle in the short run and new fracture in the adjacent level, no statistically differences were found between the two groups.ConclusionsBased on current evidence, PVP takes less time in the operation, while it has greater risk of cement leakage, was inferior in reducing Cobb angle in the long term and results in lower anterior vertebral body height after the surgery. For pain relief, which is the main desire of the patients, both procedures provide significant improvement in VAS and ODI pain scores. PVP is still an effective procedure.

Abstract:Objective To analyze the clinical effect of percutaneous pedicle screw fixation and percutaneous kyphoplasty (PKP) for osteoporotic vertebral compressive fractures(OVCFs). Methods From October 2011 to May 2013,we selected 49 mild to moderate osteoporotic patients with single fractured vertebra,20 treated by percutaneous pedicle screw fixation and 29 by PKP, all the patients had unambiguous history of trauma. We recorded the operation time,the amount of blood loss,visual analog scale(VAS),the relative height of anterior and middle column of the vertebra and the local kyphotic angle before and after the operation. Results The difference of operation time,the amount of blood loss between the two group were significant (P<0.05). The VAS scores,the relative height of anterior and middle column of the vertebra,the kyphotic angle improved significantly after both operation(P<0.05). Conclusion Percutaneous pedicle screw fix-ation is an effective treatment for patients with mild to moderate osteoporotic vertebral fractures in terms of pain relief,the height of anterior and middle column vertebra and the kyphotic angle improving as perfectly as the PKP.
Efficacy of PVP with High Viscosity Bone Cement and PKP with Low Viscosity Bone Cement for Osteoporotic Vertebral Compression Fractures

Abstract:Objective By comparing clinical efficacy of percutaneous vertebroplasty with high viscosity bone cement and percutaneous kyphoplasty with conventional low viscosity bone cement for osteoporotic vertebral compression fractures,to explore the clinical value of treatment of osteoporotic vertebral compression fractures with high viscosity bone cement.Methods In this prospective study,100 patients with one-level OVCF and older than 70 years were diagnosed and treated in our hospital from June 2014 to December 2015,which were randomly divided into 2 groups (50 patients in each),applied unilateral percutaneous vertebroplasty with high viscosity bone cement and bilateral percutaneous kyphoplasty with conventional low viscosity bone cement,respectively.Collected data of basic clinical informations (including age,gender,body mass index,etc.),preoperative and postoperative visual analog scales (VAS),Oswestry disability questionnaire (ODI),operative time,intraoperative X-ray times,the amount of bone cement injection,leakage of bone cement,anterior vertebral height and kyphosis (Cobb angle) of preoperative and postoperative,and statistical analysis.Results There were significant improvement of both VAS and ODI scores of the postoperative 1 day,3 months and 12 months compared with preoperative,but there was no significant difference between two groups;The operative time,intraoperative fluoroscopy times,bone cement injection amount and intraoperative bone cement leakage of high viscosity bone cement PVP group were significantly less than those of low viscosity bone cement PKP group (P＜0.05);the anterior vertebral height and kyphosis improved,and there was a significant difference between preoperative and postoperative of low viscosity bone cement PKP group (P＜0.05).Conclusion The efficacies of unilateral PVP with high viscosity bone cement and bilateral PKP with conventional low viscosity bone cement for osteoporotic vertebral compression fractures are the same,which can relieve pain and improve function of life significantly.However,the high viscosity bone cement PVP is simpler surgical procedure,with shorter operative time,less intraoperative radiation,less bone cement leakage rate.The domestic bone cement injection device can significantly reduce the cost of surgery,it worth to be widely used for clinical application.

Abstract:Objective To compare and analyze the therapeutic effect of percutaneous vertebroplasty (PVP) and percutaneous kyphoplasty (PKP) combined with triple therapy on elderly patients with mild to moderate osteoporotic vertebral compression fractures (OVCF). Methods A total of 114 cases of elderly patients with mild to moderate osteoporotic vertebral compression fractures were identified as research subjects in our hospital from January 2017 to January 2018, and a total of 136 vertebrae were included. The patients who underwent PVP operation were included as the control group with 67 injured vertebrae, and the patients who underwent PKP operation were included as the experimental group with 69 injured vertebrae. Results The operation time and bone cement injection volume of the experimental group were significantly higher than the control group. The visual analog scale (VAS) scores of the two groups at 3 months and 6 months after operation were lower than those before operation, with lower VAS scores observed in the experimental group at 3 months and 6 months after operation. The anterior height of the vertebral body in the experimental group was higher than that of the control group. The experimental group outperformed the control group in the incidence of postoperative complications. The postoperative Oswestry dysfunction index (ODI) scores of the two groups were lower before the operation, in which the experimental group had lower scores than the control group ( P < 0.05). Conclusion PVP and PKP combined with postoperative triple therapy can achieve a promising analgesic effect. PKP has a higher volume of bone cement injection volume, and a lower incidence of complications, which gives rise to a better vertebral body recovery height than that of PVP, with rapid postoperative body function recovery and good quality of life.

Abstract:Purpose There is still debate over whether vertebroplasty (VP) or kyphoplasty (KP) is superior for the treatment of osteoporosis vertebral compression fractures (VCFs). We performed a systematic review and meta-analysis of randomised and non-randomised controlled trials comparing VP with KP to reach a relatively conclusive answer. Methods We searched computerised databases comparing efficacy and safety of VP and KP in osteoporotic fractures. These trials reported pain relief (Visual Analogue Scale), disability (Oswestry disability score) and complications (i.e., cement leakage, incident fractures) as the primary outcome. Results Eight studies involving 848 patients were identified. The outcome showed that VP is more effective in the short-term (no more than seven days) pain relief. Kyphoplasty had a superior capability for intermediate-term (around three months) functional improvement. As for long-term pain relief and functional improvement, there is no significant difference between these two interventions. Consistently, both interventions have similar risk for subsequent fracture and cement leakage. Conclusion Thus considering the higher cost of the KP procedure, we recommend VP over KP for the treatment of osteoporotic VCFs.
Molecular identification of cyanobacteria associated with stromatolites from distinct geographical locations.

Abstract:Modern stromatolites represent a significant resource for studying microbial ecology and evolution. A preliminary investigation was undertaken employing specific genetic probes to characterize the cyanobacteria responsible for stromatolite construction in a range of environments, including microbial mats found in Australia not previously examined with molecular methods. Isolates of cyanobacteria were collected from stromatolites in thermal springs, hypersaline lakes, and oceanic fringes on two continents. A polymerase chain reaction specific for DNA of cyanobacterial 16S rRNA was developed, the resulting products of the DNA amplification reaction were sequenced, and the data were used to infer relatedness between the isolates studied and other members of the cyanobacterial radiation. Complete sequence was generated for the region from position 27 to 408 for 13 strains of cyanobacteria associated with stromatolites. All stromatolite-derived sequences were most closely related to cyanobacteria, as indicated by local sequence alignment. It was possible to correlate genetic identity with morphological nomenclatures and to expand the phylogeny of benthic cyanobacteria. These inferences were also expanded to temporal variation in the dominant resident cyanobacterial species based on sampling of surface and core sinter laminations. Under the methods employed, only one cyanobacterial strain was detected in each sample, suggesting the possible dominance of a specific clonal population of cyanobacteria at any one time in the biota of the samples tested. The data indicate that internal core samples of a stromatolite at least 10 years old can be successfully analyzed by DNA-based methods to identify preserved cyanobacteria.
Comparative study of percutaneous vertebroplasty and kyphoplasty in the treatment of osteoporotic vertebral compression fractures

Abstract:ObjectiveTo compare the therapeutic effect of percutaneous vertebroplasty (PVP) and percutaneous kyphoplasty (PKP) in the treatment of osteoporotic vertebral compression fractures (VCFs).MethodsA total of 244 patients with VCFs were treated by PVP or PKP and 192 had follow-up for at least 1 year. Clinical outcomes were determined by pain Visual Analog Scale (VAS) and Short Form 36 Health Survey (SF-36). Preoperative and postoperative radiographic assessment included measurement of posterior and anterior vertebral body height (AH and PH), as well as the kyphotic angle by the Cobb method.ResultsA total of 192 cases had follow-up for at least 1 year and 52 cases lost. The average amount of polymethylmethacrylate (PMMA) cement introduced per vertebra was 3.4 ± 1.5 ml in PVP and 4.5 ± 0.8 ml in PKP (P < 0.05). All patients subjectively reported immediate relief of their typical fracture pain, and the mean VAS decreased significantly from presurgery to postsurgery during the 1-year of follow-up. The RP, BP and GH dimensionality values of SF-36 in PKP were higher than PVP (P < 0.05). The improvement on AH was 11.13 ± 5.68% in PVP and 21.46 ± 9.87% in PKP (P < 0.01); on PH was 2.25 ± 1.36% in PVP and 7.57 ± 2.49% in PKP (P < 0.01). The average improvement in the kyphotic angle after the procedure was 5.21 ± 2.33° in PVP and 11.69 ± 5.18° in PKP (P < 0.01).ConclusionsPVP and PKP have the ability of reducing pain in osteoporotic VCF patients. The correction of kyphotic deformity and restoration of the anterior vertebral body heights associated with osteoporotic VCFs was better in PKP.

Abstract:Abstract Background: Percutaneous kyphoplasty (PKP) or percutaneous vertebral plasty (PVP) has been widely applied in the treatment of osteoporotic vertebral compression fractures (osteoporotic vertebral compression will fracture, OVCF) because of its minimally invasive and effective. To compare the clinical efficacy and safety of percutaneous vertebroplasty versus percutaneous kyphoplasty for osteoporotic vertebral compression fractures with posterior wall broken. Methods: 82 patients with osteoporotic vertebral compression fracture with posterior wall broken were divided into PVP group(group A) and PKP group(group B).The operation time, perspective times, bone cement volume injected. cement leakage, hospitalization expenses, preoperative visual analog score(VAS) and Oswestry disability index(ODI), restoration height of vertebral, the vertebrae height loss and new fracture of adjacent vertebra were evaluated during the follow-up. Results: The PVP group incurred significantly shorter operation time(40.37 ±8.26 min) and less perspective times (22.23 ±3.79 times)than the PKP group(46.74 ±9.58 min and 27.96 ±5.71 times respectively)( P<0.05). The PVP group incurred significantly less expenses than the PKP group(P<0.05). The VAS scores and ODI at 1 day and 6 months post-operation were significantly lower than pre-operation in both groups(P<0.05). Conclusion: Both PVP and PKP Can obtain satisfactory clinical efficacy for the treatment of osteoporotic vertebral compression fractures with posterior wall broken,but the former may have advantages of less expenses,shorter operation time. Abstract:：Objective To observe theclinical effect of percutaneous vertebroplasty f PVP)and percutaneous kyphoplasty(PKP)inthe treatment of ostcoporotic vertebral compressive fracture. Methods Forty-two patientswith osteoporotic vertebral compressive fractures were treated with PVP or PKP from August2007 to July 2009.VAS and SF-36 scoring systems were employed to evaluate the Dain andquality of life.X-ray was used to evaluate the vertehral height restoration rate and thekyphosis correction rate.The bone cement leakage was determined based on the Chest X-ray.Results There was staitistical difference on PMMA leakage between PVP and PKP group.VASand SF-36 scores at 2 wePks and 6months after operation were much better than thosecounted before operation in both PVP and PKP groups(P 0.05).At tWO weeks after operation,the heightrestoration rate of the fractured vertebral body(anterior and central column)in the PKPgroup waa better than that in the PVP group(P 0.05).Conclusions In thetreatment of osteoporotic vertebral compressive fracture,PVP and PKP have the similareffect on the pain relief,can refresh the height of the fractured vertebral body andcorrect the kyphotic angel of the fracture level to some extent.PVP has more PMMA leakagethan PKP.

Abstract:Objective: To compare the effect of percutaneous kyphoplasty (PKP) with different phases bone cement for treatment of osteoporotic vertebral compression fracture (OVCF). Methods: The clinical data of 219 OVCF patients who treated with PKP and met the selection criteria between June 2016 and May 2018 were retrospectively analyzed. According to the different time of intraoperative injection of bone cement, they were divided into observation group [116 cases, intraoperative injection of polymethyl methacrylate (PMMA) bone cement in low-viscosity wet-sand phase)] and control group (103 cases, intraoperative injection of PMMA bone cement in low-viscosity wire-drawing phase). There was no significance in general date of gender, age, disease duration, body mass index, bone mineral density T value, fracture vertebral body, preoperative fracture severity of the responsible vertebral body, anterior height ratio of the responsible vertebral body, preoperative pain visual analogue scale (VAS) score, and Oswestry disability index (ODI) between the two groups ( P>0.05). The VAS score and ODI score were used to evaluate the improvement of patients' symptoms at immediate, 2 days, 3 months after operation and at last follow-up. At 1 day, 3 months after operation, and at last follow-up, X-ray film and CT of spine were reexamined to observe the distribution of bone cement in the vertebral body, bone cement leakage, and other complications. During the follow-up, the refracture rate of the responsible vertebral body and the fracture rate of the adjacent vertebral body were recorded. Results: The injection amount of bone cement in the observation group and control group were (4.53±0.45) mL and (4.49±0.57) mL, respectively, showing no significant difference between the two groups ( t=1.018, P=0.310). Patients in both groups were followed up 6-18 months (mean, 13.3 months). There were 95 cases (81.9%) and 72 cases (69.9%) of the bone cement distribution range more than 49% of the cross-sectional area of the vertebral body in the observation group and the control group, respectively, showing significant difference in the incidence between the two groups ( χ 2=4.334, P=0.037). The VAS score and ODI score of the postoperative time points were significantly improved compared with those before operation ( P<0.05), and there were significant differences among the postoperative time points ( P<0.05). The VAS score and ODI score of the observation group were significantly better than those of the control group ( P<0.05) at immediate, 2 days, and 3 months after operation, and there was no significant difference between the two groups at last follow-up ( P>0.05). At 1 day after operation, the cement leakage occurred in 18 cases of the observation group (8 cases of venous leakage, 6 cases of paravertebral leakage, 4 cases of intradiscal leakage) and in 22 cases of the control group (9 cases of venous leakage, 8 cases of paravertebral leakage, 5 cases of intradiscal leakage). There was no significant difference between the two groups ( P>0.05). During the follow-up, 5 cases (4.3%) in the observation group, 12 cases (11.7%) in the control group had responsible vertebral refracture, and 6 cases (5.2%) in the observation group and 14 cases (13.6%) in the control group had adjacent vertebral fracture, the differences were significant ( χ 2=4.105, P=0.043; χ 2=4.661, P=0.031). Conclusion: Bone cement injection with wet-sand phase in PKP is beneficial for the bone cement evenly distributed, strengthening the responsible vertebral, relieving the short-term pain after operation, decreasing the rate of responsible vertebral refracture and adjacent vertebral fracture without increasing the incidence of relevant complications and can enhance the effectiveness.
Effect of percutaneous vertebroplasty versus percutaneous kyphoplasty on post‐operative wound pain in patients with osteoporotic vertebral compression fractures

Abstract:This research is intended to evaluate the efficacy of percutaneous vertebroplasty (PVP) versus percutaneous kyphoplasty (PKP) in osteoporotic vertebral compression fracture (OVCF), which is associated with post‐operative pain. Eligible studies were screened by searching multiple databases and sources such as PubMed, Cochrane and EMBASE for search terms updated to October 2023, and relevant literature sources were searched. Randomized, controlled, prospective or retrospective, and cohort studies were eligible. For the analysis of the primary results, an analysis of the data was carried out, such as mean difference (MD) or odds ratio (OR), and 95% confidence interval (CI). In the present research, 1933 research was screened in 4 databases, and 30 articles were chosen to be examined under strict exclusion criteria. No statistical significance was found in the use of bone cement in the PVP group and PKP (MD, −0.60; 95% CI, −1.40, 0.21, p = 0.15); PKP was associated with a reduced risk of cement leak compared with PVP group (OR, 2.18; 95% CI, 1.38, 3.46, p = 0.0009); no statistical significance was found in the wound VAS score in PVP operation compared with that of PKP (MD, 0.16; 95% CI, −0.07, 0.40, p = 0.17); no statistical significance was found between the time of PVP operation and the time of PKP operation (MD, −2.65; 95% CI, −8.91, 3.60, p = 0.41). Compared with PVP technology, the PKP treatment of osteoporotic vertebral compression fractures reduces post‐operative cement leakage, but there is no significant difference in the number of operative cement and wound VAS after operation. Nor did there appear to be a statistically significant difference in time between the two operations.

Abstract:Introduction The aim of this study was to evaluate the use of percutaneous curved vertebroplasty procedure (PCVP) and bilateral-pedicle-approach percutaneous vertebroplasty (bPVP) for the treatment of osteoporotic vertebral compression fractures (OVCFs) through a systematic review and meta-analysis of the scientific literature. Methods A systematic review of the scientific literature in PubMed, China National Knowledge Infrastructure (CNKI), Wanfang and other databases was conducted in conjunction with different keywords. Nine studies were included; all but 3 were randomised controlled studies and all were prospective or retrospective cohort studies. Results We observed statistically significant differences between the PCVP group and the bPCVP group in terms of postoperative visual analogue scale (VAS) scores (mean difference [MD]: −.08; 95% confidence intervals [CI]: −.15 to .00), bone cement leakage rates (OR = .33; 95%CI: .20 to .54), bone cement injection (MD: −1.52; 95%CI: −1.58 to 1.45), operative times (MD: −16.69; 95%CI: −17.40 to −15.99) and intraoperative fluoroscopies (MD: −8.16; 95%CI: −9.56 to −6.67), with the PCVP group being more dominant. There were no statistical differences in postoperative Oswestry Disability Index (ODI) scores (MD: −.72; 95%CI: −2.11 to .67) and overall bone cement distribution rates (MD: 2.14; 95%CI: .99 to 4.65) between the 2 groups. Conclusions Meta-analysis showed more favourable outcomes in the PCVP group compared to the bPVP group. PCVP might be effective and safe in the treatment of OVCFs because it relieves postoperative patient pain, reduces operative time and cement injection, and decreases the risk of cement leakage and radiation exposure to the surgeon and patient.

Abstract:Objective To compare the safety and efficacy of percutaneous vertebroplasty(PVP) and kyphoplasty(PKP) treatment of painful vertebral compression fracture.Method The study group included 36 patients with painful osteoporotic vertebral compression fracture,20 of which underwent PVP,and 16 of which underwent PKP.The complications and Cobb angles were investigated,and VAS and SF-36 scoring system were used to evaluate the efficacy. One-way ANOVA and t-test were used,a P value less than 0.05 was considered significant.Result: All procedures were completed successfully.There were 8 cement embolus and 2 pulmonary embolism in PVP group,and there were 6 cement embolus and 1 pulmonary embolism in PKP group.Comparison of preoperative and postoperative Cobb angle,VAS and SF-36 scores showed improvement with a statistical significance in both groups(P<0.05).There was no significant difference in the changes of Cobb angles,VAS and SF-36 scores between the two groups.(P>0.05).Conclusion It can not be proved that PKP is superior to PVP in pain relief,kyphosis correction and cement extravasation.

Abstract:Objectives: To compare the clinical efficacy and imaging characteristics of percutaneous vertebroplasty(PVP) with high-viscosity bone cement and percutaneous kyphoplasty with low-viscosity bone cement for osteoporotic vertebral compression fractures (OVCF), and to estimate the clinical value of PVP with high-viscosity bone cement.Methods: 115 patients were diagnosed and treated with one-level OVCF in Peking University Shougang Hospital from June 2015 to December 2016.65 patients were applied by PVP with highviscosity bone cement(group A), and 50 patients were applied by percutaneous kyphoplasty(PKP) with low-viscosity bone cement(group B).The basic clinical information, VAS, ODI, operation time, intraoperative X-ray times, bone cement volume, bone cement leakage, bone cement distribution status, anterior vertebral height and kyphosis(Cobb angle) were collected.SPSS 20.0 was used for statistical analysis.Results: There were significant improvements of VAS, ODI, anterior vertebral height and Cobb angle from preoperatively to postoperatively(P<0.01), there was no significant difference of bone cement leakage between two groups, but the operation time and intraoperative fluoroscopy times of group A were significantly less than those of group B (P<0.01).In group A, there were 6 cases(9.2％) of unilateral diffusion of bone cement in CT, 12 cases(18.5％) of bilateral asymmetric diffusion, 47 cases(73.3％) of bilateral symmetrical diffusion;those three diffusion patterns of group B were 33 cases(66.0％), 7 cases(14.0％), 10 cases(20.0％), respectively;there was a significant difference between the two groups(P<0.01).In group A, there were 3 cases(4.6％) of no endplate diffusion, 16cases(24.6％) of one endplate diffusion and 46 cases(71.0％) of both endplates diffusion;those of group B were 17 cases(34.0％), 22 cases(44.0％), 11 cases(22.0％), respectively;there was a significant difference between the two groups(P<0.01).Conclusions: PVP with high-viscosity bone cement has a good short-term clinical efficacy.It leads to bone cement symmetrical diffusion to the vertebral anterior column and vertebral endplates, which restores the height of vertebral body, improves the kyphosis and reduces the risk of refracture.But the risk of bone cement leakage of this procedure is comparable to that of PKP with low viscosity bone cement.

Abstract: This study provides a baseline examination of teenage sexual relationships in 1991 in a sample drawn from 9 state and independent schools in southwest England; a follow up was conducted in 1992. 1025 students in year 11 and aged 15-16 responded in 1991; the follow up in 1992 showed 315 returning the questionnaire. Information was requested on knowledge of sexually transmitted diseases (STDs), age at first intercourse, sexual intercourse without use of contraception, the frequency of "one night stands" or relationships measured in days, and knowledge of friends who had STDs. Relative risk ratios were calculated for those engaging in sexual activity before and after the age of 16 years. The results showed that in the follow up more girls and persons with more academic interests, as represented by the school subjects taken for the General Certificate of Secondary Education, responded. There were no differences by social class. The average age of the sample was 16.9 years. 54% were sexually active. 84 had sexual intercourse when they were under 16 years and 84 has sexual intercourse between the ages of 16 and the administration of the questionnaire. Those engaging in intercourse prior to the age of 16 were twice as likely to not have used contraception (relative risk [RR] = 1.86, confidence interval [CI] = 1.33 - 2.60 at 95%). They also were more likely to have had sex within a short relationship (RR = 2.24, CI = 1.38 - 3.62). They were 3 times more likely to know a close friend with an STD (RR = 2.97, CI = 1.55 - 5.70). Only 5 of the nonsexually active teenagers knew someone with an STD. The extent of sexual activity was lower than reported from other surveys. The results confirm prior understanding that younger teenagers engage in more sexual risk taking and are less likely to use contraception. The suggestion is that this risk-taking behavior is related to the cognitive development of younger teenagers of the age of first intercourse.
