Abstract:Objective To investigate the clinical outcomes in the treatment of Osteoporotic Vertebral Compression Fractures with Percutaneous Vertebroplasty(PVP).Methods The clinical results of a group of 21 patients,who had suffered from the Osteoporotic Vertebral Compression Fractures and treated with Percutaneous Vertebroplasty,were compared Fractures and treated with Percutaneous Vertebroplasty,were compared with another group of 19 patients treated by non-operative procedure.Results The duration of hospitalization,the time of pain remission and the out of bed activity were significantly reduced in the PVP.group as compared with those undergone non-operative procedure(P<0.01).On following up from six months to tweleve months,the anterior height of injured vertebral body of all patients were generally recovered obviously(P<0.05),and the Cobb angles were not rectified obviously(P>0.05).Conclusion The PVP is an effective procedure of simple microinjure,with less complication,wide indication and good outcome,and it is essential to master its indication meanwhile.
Percutaneous Curved Vertebroplasty in the Treatment of Thoracolumbar Osteoporotic Vertebral Compression Fractures: a Prospective Study

Abstract:Objective To investigate the clinical effect of percutaneous curved vertebroplasty in the treatment of thoracolum?bar osteoporotic vertebral compression fractures (OVCFs). Methods All of 85 patients with single thoracolumbar vertebral OVCFs who met the admission criteria from January 2017 to July 2018 were divided into three groups according to the random dig?its table method. They were treated with percutaneous curved vertebroplasty, routine unipedicular PVP and routine bipedicular PVP respectively. There were 25 patients in the percutaneous curved vertebroplasty group, 6 males and 19 females; aged 56-80 years, with an average age of 70.6±9.7 years. Fracture vertebral body distribution:T10 2 cases,T11 4 cases,T12 3 cases, L1 9 cases, L2 3 cases, L3 1 case, L4 1 case and L5 2 cases. There were 32 patients in the routine unipedicular PVP group, 6 males and 26 fe? males; aged 58-75 years, with an average age of 69.5±9.3 years. Fracture vertebral body distribution: T10 2 cases, T11 4 cases, T12 5 cases, L1 11 cases, L2 6 cases, L3 1 case, L4 1 case and L5 2 cases. There were 28 patients in the routine bipedicular PVP group, 5 males and 23 females; aged 59-81 years, with an average age of 69.8±8.8 years. Fracture vertebral body distribution: T10 2 cases, T11 4 cases, T12 4 cases, L1 10 cases, L2 4 cases, L3 1 case, L4 1 case and L5 2 cases. The operation time, injected cement volume, in?traoperative blood loss were recorded and analyzed. Preoperative, postoperative 1 week and 3 months visual analogue scale scores and oswestry disability index were adopted to value the clinical improvements. Preoperative, postoperative 1 week and 3 months relative vertebral height and kyphosis correction, and the cement leakage rate were measured and analyzed. Results There was no significant difference in the data of gender, age, VAS scores, ODI and distribution of fracture vertebrae among the three groups (P>0.05), and the baseline data was comparable. The average VAS score in the percutaneous curved vertebroplasty group was 2.3±0.5 at 1 week after surgery, that of the routine unipedicular PVP group was 2.4±0.4 and that of the routine bipe?dicular PVP group was 2.4±0.4; the average ODI in the percutaneous curved vertebroplasty group was 19.8%±3.9%, that of the routine unipedicular PVP group was 20.0%±4.1% and that of the routine bipedicular PVP group was 19.9%±3.8%; they were lower than the preoperative data, which were statistically significant (P<0.001). The average relative vertebral height in the percutaneous curved vertebroplasty group was 48.99%±9.23% at 3 months after surgery, that of the routine unipedicular PVP group was 47.11%±10.12% and that of the routine bipedicular PVP group was 46.71%±11.16%; the average kyphosis cor?rection in the percutaneous curved vertebroplasty group was 6.21%±1.94%, that of the routine unipedicular PVP group was 5.22%±2.07% and that of the routine bipedicular PVP group was 5.97%±2.09%; there was 1 cement leakage case in the per?cutaneous curved vertebroplasty group; those of the routine unipedicular PVP group were 4 cases and those of the routine bipe?dicular PVP group were 6 cases; there was no significant difference among the three groups (P>0.05). Operation time 39.10± 2.00 min vs 38.70±1.70 min, injected cement volume 3.60±0.11 ml vs 3.50±0.13 ml and blood loss 5.10±0.30 ml vs 5.00±0.40 ml of the percutaneous curved vertebroplasty group and the routine unipedicular PVP group were less than those of the routine bipedicular PVP group, which were statistically significant (P<0.05). Conclution Percutaneous curved vertebroplasty could achieve satisfactory clinical outcomes for OVCFs, with advantages of less operation time, less blood loss, limited X?ray expo?sure, less injected cement volume, and more balanced augmentation for stabilization of the affected vertebrae and total verte?bral column.

Abstract:OBJECTIVE:To evaluate the effectiveness of percutaneous monoplanar screw internal fixation via injured vertebrae for treatment of thoracolumbar fracture. METHODS:Between May 2015 and August 2017, 38 cases of thoracolumbar fractures without neurological symptom were treated with percutaneous monoplanar screw internal fixation via injured vertebrae. There were 22 males and 16 females, aged 25-52 years (mean, 32.5 years). There were 23 cases of AO type A3 and 15 cases of AO type A4. The injured vertebrae located at T 11 in 4 cases, T 12 in 9 cases, L 1 in 11 cases, L 2 in 10 cases, L 3 in 3 cases, and L 4 in 1 case. The mean interval between injury and operation was 4.5 days (range, 3-7 days). The pre- and post-operative degrees of lumbodorsal pain were estimated by the visual analogue scale (VAS) score. The X-ray film, CT three-dimensional reconstruction, and MRI were performed, and the ratio of anterior vertebral body height and sagittal Cobb angle were measured to assess the kyphosis of the fractured area. RESULTS:All operations in 38 patients successfully completed without complications such as dural sac, nerve root, or vascular injury. The operation time was (56.2±3.7) minutes and the intraoperative blood loss was (42.3±3.5) mL. All incisions healed by first intention without redness, swelling, or exudation. All patients were followed up 17-33 months, with an average of 21.5 months. The VAS score at each time point after operation significantly improved when compared with that before operation ( P<0.05), and significantly improved at 3 months and last follow-up when compared with that at 1 week ( P<0.05); there was no significant difference between 3 months and last follow-up ( P>0.05). There was no internal fixator loosening, breakage, or delayed kyphosis in all patients. The ratio of anterior vertebral body height and sagittal Cobb angle significantly improved postoperatively ( P<0.05), and no significant difference was found between the different time points after operation ( P>0.05). CONCLUSION:Percutaneous monoplanar screw internal fixation via injured vertebrae is an easy approach to treat thoracolumbar fracture without neurological symptom, which can effectively restore vertebral body height and correct kyphosis, and avoid long-term segmental kyphosis.
Clinical Effect of PKP in Treatment of Patients with Multi-segmental Osteoporotic Vertebral Compressive Frac-tures

Abstract:Objective To investigate clinical effect and complication of percutaneous kyphoplasty (PKP) in treatment of multi-segmental osteoporotic vertebral compressive fractures ( OVCF) . Methods A total of 73 patients with osteoporotic vertebral compressive fractures OVCF during February 2008 and July 2013 were recruited in this study, which included 60 patients with two-segmental OVCF and 13 patients with three-segmental OVCF, and there were 159 vertebral bodies altogether. All new fractured vertebral bodies were treated with PKP. Conditions of symptom improve-ment were evaluated by Visual analogue score ( VAS) and Oswestry disability index ( ODI) sore before and after surgery. Radiographic outcomes were evaluated using indexes such as vertebral compression degree, restoration degree of vertebral height and kyphosis losing degree in postoperative 1 year. Conditions of distribution and penetrance of bone cement were studied by postoperative computed tomography scan. Results Symptomatic relief rate of patients were 95. 89% (70/73), and postoperative computed tomography scan showed that the total penetrance of bone cement was 31. 51%. Values of VAS and ODI score in postoperative 1 week and 1 year were lower than those before operation(P<0. 01), and values of VAS and ODI score in postoperative 1 year were lower than those in postoperative 1 week (P<0. 01). Conclusion PKP can effectively treat multi-segmental OVCF with satisfactory analgesic effect, but incident rates of bone cement leak-aging and second fracture are high, and therefore anti-osteoporotic medication should be supplemented.

Abstract:Objective To determine the efficacy and feasibility of the combination of postural reduction and percutaneous vertebroplasty for traumatic thoracolumbar spine fractures.Methods Forty-eight consecutive patients with single level traumatic thoracolumbar spine fractures were included in this study.There are 36 males and 12 females,with average age of 50 years(range 24-76 years).Patients were treated with postural reduction by extending the operating table,then percutaneous bilateral transpedicular reduction of the endplate was performed,and calcium phosphate cement was injected.The results were quantitatively evaluated,according to the concept of estimated vertebral height loss and kyphotic angle of the vertebral fractures by radiographs.Visual analog scale(VAS)and the affected vertebra shape changes were recorded.Results Patients were followed up for 15-37 months(average 18 months);all patients reported significant alleviation of the pain after treatment.VAS scores was reduced averagely from 6.9±1.6 to 1.7±0.8.the anterior and middle vertebral height was restored and kyphotic abnormality was corrected.During the period of follow up,postoperative outcomes were satisfactory without notable corrected vertebral height loss.Conclusion If the indications are respected,percutaneous vertebroplasty following postural reduction can provide significant pain relief and restore the vertebral height and kyphotic angle in patients with traumatic thoracolumbar spine fractures.

Abstract:OBJECTIVE:To determine the efficacy and feasibility of the combination of postural reduction and percutaneous kyphoplasty for traumatic thoracolumbar spine fractures.METHODS:Thirty-seven patients with single level traumatic thoracolumbar spine fractures were included in this study. Patients were treated with postural reduction and then percutaneous vertebroplasty with Calcium Phosphate Cement (CPC) were performed on the fractured vertebra. The results were quantitatively evaluated, according to the concept of estimated vertebral height loss and kyphotic angle of the vertebral fractures by preoperative and postoperative plain standing lateral radiographs. Visual analog scale (VAS) and the fracture vertebra shape changes were recorded.RESULTS:Patients were followed up for 9 to 24 months (average 13 months), pain was significantly relieved compared with the preoperative, VAS was reduced averagely from 7.6 +/- 2.5 to 1.8 +/- 1.5, the anterior and middle vertebral height was restored and kyphotic angle was corrected. During the period of follow up, outcomes were satisfactory, without notable correction loss.CONCLUSION:If the indications are correctly handled, the combination of postural reduction and percutaneous kyphoplasty for the treatment of traumatic thoracolumbar spine fractures can provide significant pain relieve and restore the vertebral height and kyphotic angle.
Molecular identification of cyanobacteria associated with stromatolites from distinct geographical locations.

Abstract:Modern stromatolites represent a significant resource for studying microbial ecology and evolution. A preliminary investigation was undertaken employing specific genetic probes to characterize the cyanobacteria responsible for stromatolite construction in a range of environments, including microbial mats found in Australia not previously examined with molecular methods. Isolates of cyanobacteria were collected from stromatolites in thermal springs, hypersaline lakes, and oceanic fringes on two continents. A polymerase chain reaction specific for DNA of cyanobacterial 16S rRNA was developed, the resulting products of the DNA amplification reaction were sequenced, and the data were used to infer relatedness between the isolates studied and other members of the cyanobacterial radiation. Complete sequence was generated for the region from position 27 to 408 for 13 strains of cyanobacteria associated with stromatolites. All stromatolite-derived sequences were most closely related to cyanobacteria, as indicated by local sequence alignment. It was possible to correlate genetic identity with morphological nomenclatures and to expand the phylogeny of benthic cyanobacteria. These inferences were also expanded to temporal variation in the dominant resident cyanobacterial species based on sampling of surface and core sinter laminations. Under the methods employed, only one cyanobacterial strain was detected in each sample, suggesting the possible dominance of a specific clonal population of cyanobacteria at any one time in the biota of the samples tested. The data indicate that internal core samples of a stromatolite at least 10 years old can be successfully analyzed by DNA-based methods to identify preserved cyanobacteria.
Short-segment Pedicle Screw Fixation Combined with Polymethyl Methacrylate Vertebroplasty in Treating Thoracolumbar Osteoporotic Compression Fractures

Abstract:Objective To evaluate the clinical efficacy and safety of short-segment pedicle screw fixation combined with vertebroplasty for the treatment of thoracolumbar osteoporotic compression fractures.Methods A retrospective case control study was conducted on 63 patients with fresh thoracic or lumbar osteoporotie compression fractures who were surgically treated from January 2010 to December 2013.There were 26 males and 37 females,with age of 63-87 years [(76.3 ± 5.7) years].According to the surgical method,the patients were assigned to simple vertebroplasty group (Group A),short-segment pedicle screw fixation group (Group B),and short-segment pedicle screw fixation group combined with vertebroplasty group (Group C),with 21 cases in each group.Length of hospital stay,operation time,and blood loss were recorded.The visual analog scale (VAS),anterior vertebral body height,angle of the kyphotic deformity,and complications before operation,immediately after operation,3 months after operation,and at the last follow up were compared among three groups.Complications of each group were recorded.Results The hospital stay,operation time,and blood loss in Group C were significantly higher than those in Group A (P ＜ 0.05),but there were no significant differences between Group B and Group C (P ＞ 0.05),except for a longer operation time in Group C.The pre-operative VAS showed no significant difference among three groups (P ＞ 0.05).However,the mean VAS in Groups A,B and C at the last follow up were 1.0(0.0,2.0)points,1.0(0.0,2.0)points,0.0(0.0,1.0)points,respectively.The VAS in Group C was significantly lower than that in Group B or A (P ＜ 0.05).The mean anterior vertebral body height and angle of the kyphotic deformity in Group C had no significant difference from that in Group A or B before operation and immediately after operation (P ＞ 0.05).At the last follow up,the mean anterior vertebral body height and angle of the kyphotic deformity in Groups A,B and C were (68 ±14.7)％,(72.3 ±9.0)％,(81.5 ±5.6)％ and (10.6 ±3.9)°,(10.7 ±5.0)°,(7.4 ± 5.0) °,respectively.The loss of anterior vertebral body height and angle of the kyphotic deformity correction in Group C were significantly less than that in Group A or B (P ＜ 0.05).Superficial infection was found in Groups B (n =2) and C (n =1),and the infection was cured after antibiotic therapy and dress change.Bone cement leakage was found in Groups A (n =8) and C (n =5),with no nerve compression.Internal fixation failure was seen in Group B (n =3),and the implant was removed directly.Conclusion Short-segment pedicle screw fixation combined with vertebroplasty can effectively reduce the loss of anterior vertebral body height and angle of the kyphotic deformity and hence enhance the clinical efficacy.

Abstract:Abstract Background To investigate the clinical effect of percutaneous pedicle screw fixation (PPSF) combined with percutaneous vertebroplasty (PVP) in the treatment of osteoporotic compression vertebral fracture (OVCF) of the thoracolumbar vertebra with kyphosis. Methods One hundred sixty-six patients before June 2017 were retrospectively analyzed, and patients were divided into PPSF + PVP group A and PVP group B. Operative time, bone mineral density, postoperative bed time, high compression ratio, bone cement leakage rate, and bone cement dose were recorded. Comparison of vertebral anterior edge height, Cobb angle, visual analogue score (VAS), and low back pain dysfunction index (ODI) between the two groups in preoperative, postoperative 3 days, postoperative 6 months, postoperative 12 months, and postoperative 24 months, postoperative complications were observed in the two groups. Results The operation time of group A was longer than that of group B (59.0 ± 8.6 min, 26.6 ± 5.2 min), longer postoperative bed rest time (3.3 ± 0.7 days, 1.2 ± 0.5 days), the differences were statistically significant ( P < 0.01), there was no difference in the amount of bone cement between the two groups (5.4 ± 0.6 ml, 5.3 ± 0.8 ml) ( P > 0.05). The height of the anterior edge and Cobb angle of the two groups recovered significantly in postoperative 3 days. The height of anterior edge (2.7 ± 0.3 cm, 2.6 ± 0.2 cm, 2.5 ± 0.7 cm; 2.3 ± 0.6 cm, 1.7 ± 0.5 cm, 1.6 ± 0.3 cm) and Cobb angle (4.9 ± 2.2, 5.5 ± 2.3, 5.7 ± 2.3; 12.4 ± 3.2, 17.2 ± 2.5, 13.2 ± 2.3) was statistically significant in postoperative 6 months, postoperative 12 months, and postoperative 24 months ( P < 0.01). VAS and ODI scores of postoperative 6 months and 12 months were significantly different between the two groups ( P < 0.05). Postoperative complications in group B were much higher than those in group A. Conclusion The efficacy of PVP alone was not satisfactory, and the rate of complications was high for OVCF patients with severe anterior edge compression with kyphosis. PPSF combined with PVP is recommended, the vertebral height loss was not obvious, the satisfaction was good, and the complication rate was lower during 2 years follow-up.

Abstract:Objective To investigate the surgical technique and clinical outcome of treatment of acute traumatic thoracolumbar fractures through the approach of para-vertebral muscle sparing by transpedicular vertebroplasty with artificial cement and posterior instrumentation.Methods Thirty-one patients,22 males and 9 females,averaging 46 years of age with single level traumatic thoracolumbar fracture with light or without neurologic deficit were prospectively studied.These patients had 22 compression fractures and 9 burst fractures,located 3 at T11,9 at T12,14 at L1 and 5 at L2.They were treated by short-segment pedicle-screw reduction and fixation and augmentation of the injured vertebra with quick hardening artificial cement through the approach of para-vertebral muscle sparing.Vertebral central and anterior height,kyphotic angle of the vertebral fractures by radiographs and visual analog scale were calculated pre-operatively and post-operatively.Results All 31 patients were operated successfully and were followed for 12-36 months.None neurologic deterioration was observed.The operating time was averaged 95 min and blood loss was 150 mL.All patients reported significant pain alleviation shortly after operation.VAS scores was reduced from 8.97±0.66 to 3.23±0.98 and anterior vertebral body height compressional ratio improved from 44.58±6.71 before surgery to 5.43±1.63 after surgery,whereas the central vertebral body height was improved from 36.42±5.56 to 4.01±0.90; and sustained kypotic abnormality was corrected.Cement was ossified within 3-8 months after operation.There was no instrumentation failure or measurable loss of sagittal curve and vertebral body height correction in any group of patients.Conclusion Posterior instrumentation combing with transpedicular vertebroplasty with artificial cement through the approach of para-vertebral muscle sparing seems to b a feasible option in the management of some thoracolumbar fracture and is less invasive restoring vertebral anterior-middle column strength and decreasing the failure of instrumentation.

Abstract:Object. The purpose of this study was to evaluate the feasibility and safety of minimally invasive pedicle screw fixation combined with percutaneous vertebroplasty (PVP) for treating acute thoracolumbar osteoporotic vertebral compression fracture (VCF) and preventing secondary VCF after PVP. Methods. Twenty patients with a mean age of 73.6 year's (range 65-85 years) who sustained fresh thoracic or lumbar osteoporotic VCFs without neurological deficits underwent minimally invasive pedicle screw fixation combined with PVP. Visual analog scale pain scores were recorded, and the Cobb angles and the central and anterior vertebral body (VB) heights were measured on the lateral radiographs before surgery and immediately, 1 month, 2 months, 3 months, 6 months, 1 year, and 2 years after surgery. Results. The patients were followed up for an average of 26 months (range 24-30 months) after sugery. The visual analog scale score was found to be significantly decreased; from 7.3 +/- 1.3 before surgery to 1.2 +/- 0.7 immediately after surgery and to 0.7 +/- 0.7 (p < 0.001) at the end of follow-up. The Cobb angle was 17.0 degrees +/- 4.3 degrees before surgery and 6.4 degrees +/- 3.6 degrees immediately after surgery. The central VB height that was 44.5% +/- 7.6% before surgery increased to 74.6% +/- 6.4% of the estimated intact central height immediately after surgery (p <0.001). The anterior VB height increased from 50.7% +/- 7.4% before surgery to 82.5% +/- 6.7% of the estimated intact anterior height immediately after surgery (p <0.001). There were no significant changes in the results obtained over the follow-up time period. There was no occurrence of new fracture in surgically treated or adjacent vertebrae in these patients. Conclusions. Minimally invasive pedicle screw fixation combined with PVP is a good choice for the treatment of acute thoracolumbar osteoporotic VCF and can prevent the occurrence of new VCFs after PVP.
The analgesic efficacy of extracorporeal shock wave combined with percutaneous vertebroplasty in the treatment of osteoporotic thoracolumbar compression fractures in postmenopausal women

Abstract:Background: To explore the analgesic efficacy of extracorporeal shock wave (ESW) combined with percutaneous vertebroplasty (PVP) after reduction in overextension position in the treatment of osteoporotic thoracolumbar compression fractures in postmenopausal women. Methods: The data of postmenopausal women with osteoporotic thoracolumbar compression fracture admitted in our department from January 2017 to October 2019 were analyzed retrospectively. They were divided into groups of unipedicular percutaneous kyphoplasty (U-PKP n = 21), bipedicular PKP (B-PKP n = 20), and ESW combined with PVP after reduction in overextension position (EP-PVP n = 18). The improvement of pain and vertebral height in three groups was compared. Results: Postoperative compression rate and Cobb angle of vertebral fractures in the three groups were all lower than those before surgery, and the differences between pre-operation and post-operation were statistically significant (P < 0.05). The visual analog scale (VAS) and Oswestry dysfunction index (ODI) scores of the three groups decreased significantly after the operation (P < 0.05). The ODI scores of the EP-PVP group in the third months after the operation were significantly improved compared with the other two groups, and the difference was statistically significant (P < 0.05). Conclusions: In our small-sample study, all three treatment schemes can treat osteoporotic compression fracture of thoracolumbar vertebrae in postmenopausal women, relieve pain, and improve quality of life. ESW combined with PVP after reduction in overextension position could achieve a good vertebral reduction rate and improve kyphosis, and may reduce the application of analgesic drugs.
The Minimally Invasive Pedicle Screw Fixation with Percutaneous Vertebroplasty Augmentation in the Surgical Treatment of Thoracolumbar Osteoporosis Fracture

Abstract:[Objective]To evaluate the outcomes of the treatment of acute thoracolumbar osteoporosis fractures by minimally invasive pedicle screw fixation with percutaneous vertebroplasty.[Method]From 2008.7 to 2011.1,one hundred and three patients with an average age of 71.6 years,who sustained thoracolumbar fresh fractures without neurologic deficit were treated with 3 following methods:group 1,percutaneous kyphoplasty,group 2,pedicle screw fixation with vertebroplasty through open approach,group 3,minimally invasive pedicle screw fixation with percutaneous kyphoplasty.Preoperative and postoperative visual analog scale pain scores were recorded in the average follow-up of 16.9 months.The vertebral height loss and kyphotic angle of the vertebral compression fractures were evaluated by plain lateral radiographs.[Result] Pain scores improved significantly in all 3 groups after operation(P0.001),and VAS of group 3 was lowest at 6 months postoperatively(P0.05).Vertebral heights increased and kyphosis angles decreased significantly postoperatively in all groups(P0.001),and more heights were restored in group 2,3 than in group 1 after opeartions(P0.001).Restoration of vertebral heights(P0.001)and correction of kyphosis angles(P0.05)significantly decreased at 1 month postoperatively in group 1.[Conclusion]Minimally invasive pedicle screw fixation with percutaneous vertebroplasty is a good choice for acute thoracolumbar osteoporosis fractures.

Abstract:Objective:To evaluate the efficacy and safety of minimally invasive pedicle screw fixation combined with percutaneous vertebroplasty(PVP) with calcium phosphate bone cement in the treatment of acute thoracolumbar burst fracture . Methods:Twenty‐three patients ,who suffered from single fresh thoracolumbar burst fracture without neurologic deficits and were classified as A3 fracture located between T11~L2 ,underwent the procedure of minimally invasive pedicle screw fixation combined with PVP with calcium phosphate bone cement .Duration of operation and blood loss ,injected volume of bone cement and length of stay ,were recorded .Lumbago and backache scores assessed by visual analogue scale (VAS) were recorded before surgery ,immediately after surgery ,as well as 1 month ,2 months ,3 months ,6 months ,1 year and 2 years after surgery . Vertebral compression rate and restoring rate ,Cobb angle and correction rate of vertebral kyphosis ,were measured at each time point and compared among the time points .Results:The average injected volume of calcium phosphate bone cement was 4 .9(3 .8~6 .4)mL .The mean duration of operation was 92(75~120)min .The average blood loss was 52(40~75)mL .The mean length of stay was 5 .6(4~ 7) d and the average follow‐up period was 26 months (24 ~ 29) .The VAS significantly decreased from 8 .8 ± 1 .2 before surgery to 1 .8 ± 0 .6 immediately after surgery and 0 .4 ± 0 .5 at 2 years after surgery ,and the differences were statistically significant(P< 0 .001) .The compression rate of vertebral body height decreased from(50 .4 ± 7 .2)% before surgery to (6 .2 ± 1 .5)% immediately after surgery and(6 .9 ± 1 .4)% at 2 years after surgery ,and the differences were statistically significant(P<0 .001) .The Cobb angle decreased from (22 .6 ± 1 .8)° before surgery to (3 .2 ± 1 .7)° immediately after surgery and (5 .1 ± 1 .5)° at 2 years after surgery ,and the differences were statistically significant(P<0 .001) .No significant decrease in both kyphosis correction and vertebral body height restoration was observed during the follow‐up .There was no loosen screw or broken screw .Conclusions:Minimally invasive pedicle screw fixation combined with PVP with calcium phosphate bone cement is a safe and efficacious method for the treatment of thoracolumbar vertebral burst fracture .
The study of percutaneous vertebroplasty for the treatment of osteoporosis vertebral compression fracture of multi-vertebral in elderly patients

Abstract:[Objective]To discuss the effect of percutaneous vertebroplasty used in the treatment of osteoporosis vertebral compression fracture of multi-vertebral in elderly patients.[Method]From April of 2010 to April of 2012,a retrospective study was carried out,there were 41 patients who were clarified a diagnosis of osteoporosis vertebral compression fracture of mult-ivertebral,with a mean age of 75.8 years(range,59 to 97 years),who were operated for two segments(4 cases),three segments(29 cases),four segments(7 cases),five segments(1 cases).The anterior height,posterior height of the vertebrae body and kyphotic angle were measured preoperatively and postoperatively.Clinical result were evaluated with Visual Analogue Scale(VAS) and Oswestry Disability Index(ODI).[Result]Low back pain disappeared nearly in all 41 cases.Patients can be ambulatory in 24 hours postoperatively,and vertebral body height was restored,Cobb angle improved significantly(P0.05) after the surgery,arranging 0.5° to 5.7°(average,2.83°).All cases had been followed up for average 24.8 months,(from 3 to 35 months).Between preoperative and postoperative VAS and ODI was significantly different(P0.05).[Conclusion]Percutaneous vertebroplasty used in treatment of multi-vertebral osteoporosis vertebral compression fracture in elderly patients by percutaneous minimally invasive techniques according to our characters has satisfactory clinical efficacy.

Abstract:Abstract Purpose Percutaneous vertebroplasty (PVP) is a routine operation for the treatment of osteoporotic lumbar compression fractures (OLCFs). As is well known, unilateral puncture is a common method. However, with the conventional transpedicular approach (CTPA), the cement may be asymmetrically dispersed, so some surgeons use the transverse process root-pedicle approach (TPRPA). The objective of this study was to compare the clinical results and bone cement distribution of PVP for OLCF with unilateral TPRPA and CTPA to identify the advantages and disadvantages of the two surgical options. Patients and methods From January 2016 to June 2019, seventy-two elderly patients who underwent unilateral PVP for single-level OLCF were retrospectively reviewed. Operation time, injection amount and type of bone cement distribution, and bone cement leakage and surgical complications were recorded. The visual analog scale (VAS) scores and Oswestry disability index (ODI) scores were used to evaluate the clinical results. All patients were followed up for more than 12 months, and the assessment was based primarily on clinical and radiological outcomes. Results There were significant differences in the surgical time and the volume and the type of bone cement distribution and the lost of operative vertebra height between the two groups. However, there was no significant difference in bone cement leakage. Moreover, there were no significant differences in VAS and ODI between the two groups at 2 days and 12 months after the operation. Conclusions Unilateral TPRPA and CTPA are practical and feasible methods in PVP for the treatment of OLCF, and they have similar clinical effects. However, TPRPA has the advantages of a better distribution of bone cement and a shorter operation time and a better maintenance effect of injured vertebra height, without increasing the rate of bone cement leakage.
