From the hundreds of documents examined, the USAID/MCHIP team selected 61 that contained information on the impact of new vaccine introduction (NUVI) on immunization programs and, in a few cases, the broader health system. Most of the documents were Post-Introduction Evaluations (PIEs), PIE summaries, meeting reports and presentations, and consultant reports. Most dealt with one NUVI but a few covered multiple country experiences (one summarized 16 PIEs, another seven). Country experiences reviewed were mostly from low-income African countries and concerned introductions of HepB and Hib, although there were several cases from middle-income countries, other continents, and rotavirus, pneumococcal vaccines, and human papillovirus vaccines (RV, PCV, and HPV). To summarize relevant information, MCHIP used a format based on the WHO health systems components, with separate columns for impact on the EPI and health system. Limitations: ►Most documents about EPI, but not about NUVI specifically, failed to mention the effects of NUVI on the EPI or health system; most documents on NUVI focused on the preparation and introduction, or the EPI status shortly after the introduction. ►PIEs and other NUVI assessments report on a particular point in time, which may fail to capture the changing effects of NUVI over time. ►In many cases only PIE summaries from WHO were available rather than full documents, and they sometimes contained contradictory or incomplete information. ►It was difficult to separate the effects of NUVI from the effects of donor funding that often accompanied NUVI. ►Because introductions of PCV, RV, and HPV into developing countries began only a few years ago, there was limited information on their impact, so the review may not

Abstract:During the fall of 1985, 4 Angora goats, from a herd of 40, were examined on a farm in central Michigan. Affected goats were alert but had neurologic deficits consistent with upper and lower moto neuron involvement. Eosinophilic pleocytosis in a cerebrospinal fluid sample from one goat was consistent with cerebrospinal nematodiasis. Parelaphostrongylosis was confirmed in 3 goats by identification of Parelaphostrongylosis tenuis larvae in spinal cord sections. Ivermectin may have influenced the herd's susceptibility to new parasitic infections. Control of parelaphostrongylosis probably is best achieved by removal of susceptible animals from treed swamps coinhabited by white-tailed deer from late summer until after the first killing frost.
Efficacy of Different Root Canal Irrigating Solutions in Removing Smear Layer: A Scanning Electron Microscopic Study

Abstract:Background The aim of the present investigation was to evaluate and compare the efficacy of frequently used chemical agents in terms of their capacity to eliminate the smear layer after instrumentation, as observed via scanning electron microscopy (SEM). Materials and methods Sixty extracted single-rooted mandibular premolar teeth, each with roots 15 mm in length, were used in this study. The teeth were divided into one control group and four study groups, each containing 12 teeth. In Control Group 0, teeth were irrigated with 3 ml of saline only. In Group 1, teeth were irrigated initially with 3% sodium hypochlorite (NaOCl) and then given a final rinse with 3 ml of 17% ethylenediaminetetraacetic acid (EDTA) for one minute. In Group 2, teeth were irrigated with 3% NaOCl and given a final rinse with 3 ml of a mixture of tetracycline, acid, and detergent (MTAD, BIOPURE) for one minute. In Group 3, teeth were irrigated with saline and given a final rinse with 3 ml of 17% EDTA for one minute. In Group 4, teeth were irrigated with saline and given a final rinse with 3 ml of MTAD for one minute. One-half of each tooth was chosen and prepared for scanning electron microscopic (SEM) examination at the cervical, middle, and apical thirds. These were observed at magnifications of up to 1,000 times to check for the presence or absence of a smear layer. The data were analyzed using the Kruskal-Wallis test and post-hoc Dunn's test. Results All of the root canal irrigation protocols exhibited superior efficacy compared to the control group in the elimination of the smear layer. Group 2 (3% NaOCl with MTAD) showed the lowest mean scores, compared to all the groups, followed by Group 1 (3% NaOCl with 17% EDTA). MTAD was more effective than EDTA. The smear layer was effectively removed from the apical third, followed by the middle and coronal thirds of the root. Conclusion Initial irrigation with 3% NaOCl and one-minute final irrigation with 3 ml MTAD was the most effective root irrigant, and particularly indicated in teeth with infection of the apical third.
The Effectiveness of Different Irrigation Techniques on Debris and Smear Layer Removal in Primary Mandibular Second Molars: An In Vitro Study

Abstract:Aims: The aim of the present in vitro study was to compare the effectiveness of passive ultrasonic irrigation (PUI), sonic irrigation, and mechanic dynamic activation on the removal of debris and smear layer from primary mandibular second molars during pulpectomy. Materials and methods: Mesial roots of 48 primary mandibular second molars were prepared with an R-motion 21 mm file (30/0.04) (FKG Dentaire SA, La Chaux-de-Fonds, Switzerland), irrigated with 1% sodium hypochlorite (NaOCl) and 17% ethylenediaminetetraacetic acid (EDTA), and divided into four groups (n = 24 canals) according to the final irrigation activation technique: control group without activation, PUI with Ultra-X (Eighteeth, Changzhou, China), mechanical activation with XP-endo Finisher (FKG), and sonic irrigation with EQ-S (Meta Biomed, Chungcheongbuk-do, Korea). The roots were split longitudinally and analyzed using scanning electron microscopy (SEM). The presence of debris and smear layer was assessed using a 5-grade scoring scale with 200× and 1000× magnification, respectively. The Kruskal-Wallis and Friedman tests were used for data analysis. Results: The activation of the irrigant significantly improved debris and smear layer removal (p < 0.001). There was no significant difference between Ultra-X, XP-endo Finisher, and EQ-S (p > 0.05). No activation technique was able to completely eliminate debris and smear layer from the root canals of primary mandibular second molars. Conclusions: During pediatric pulpectomy, the irrigation protocol must include activation of the irrigation solutions using either ultrasonic, sonic, or mechanical activation techniques to enhance the removal of debris and smear layer for a better prognosis. Clinical significance: During root canal treatment on primary teeth, the clinician must incorporate an activation technique in the irrigation protocol to enhance the removal of debris and smear layer and increase the success of the treatment.
Evaluation of the Smear Layer Removal Ability of Various Rotary Files with/without Chemical Agents on Primary Teeth: An In Vitro Study

Abstract:Aim: The aim of the current study was to assess the cleaning and smear layer removal efficacy of two different rotary files with or without chemical agents on primary teeth. Materials and methods: For the study, 90 extracted primary maxillary incisors without internal or external resorption and with at least two-thirds of complete roots were chosen. Then, based on the kind of instruments used to clean and shape the canals, they were randomly assigned to three experimental groups, each consisting of 30 teeth. Group-I: The canal was instrumented manually with K-files, Group-II: The canal was instrumented with Kedo-S files, Group-III: The canal was instrumented with Kedo-SG Blue files. After the canals were finally instrumented, 2 mL of QMixTM solution was used to irrigate 15 samples from each group. The samples were subsequently allowed to remain in the canals for 90 seconds in order to eliminate the smear layer. After that a stereomicroscope was used to assess the cleaning effectiveness. Results: With irrigant solution, the highest mean value was found in manual K-files (2.86 ± 0.34), followed by Kedo-S files group (1.34 ± 0.26) and Kedo-SG Blue files (1.28 ± 0.18). Without irrigant solution, the highest mean value was found in manual K-files (2.92 ± 0.22) followed by Kedo-S files group (1.44 ± 0.18) and Kedo-SG Blue files (1.36 ± 0.14). There was a statistically significant difference found at all the three levels. Conclusion: On conclusion, the current study's findings demonstrated that irrigation solution was significantly more effective in cleaning and removing smear layers from pediatric rotary files than manual K-files. Clinical significance: The effectiveness of endodontic therapy depends on a successful chemomechanical preparation. The canals are instrumented using either hand files or rotary instruments; there are several irrigation and instrumentation techniques. In order to completely sterilize the canals, chemical agents are utilized for irrigation during instrumentation. Abstract:Background The smear layer has an adverse effect on the sealing of root canals during obturation, and it is the main reason for the failure of root canal treatment. Root canal irrigation using a conventional irrigation system is ineffective for smear layer removal, especially from the apical third region, where most lateral canals are present. For successful endodontic treatment, the smear layer should be removed from the apical third region using an effective irrigation activation system. Aim This study aimed to compare smear layer removal by conventional irrigation, XP Endo Finisher (XPF), endoactivator (EA), passive ultrasonic activation, and root canal brush using 17% ethylenediaminetetraacetic acid (EDTA) as a chelating solution and 5.25% sodium hypochlorite after chemomechanical preparation, using scanning electron microscopy (SEM). Method A total of 50 extracted human mandibular single canal premolars with mature roots were selected for this study. Samples were decoronated to obtain a standard working length (WL) of 15 mm. Canal patency was achieved using a 10 k file. Samples were sealed with sticky wax to obtain the vapor lock effect. Biomechanical preparation is done till F3. The samples were divided into five groups according to the final irrigation activation protocol: Group 1, control group; group 2, XPF; group 3, EA; group 4, passive ultrasonic irrigation (PUI); and group 5,, root canal brush. Samples were divided into two equal halves longitudinally. Each sample was analyzed for a smear layer under SEM at 2000x magnification. Statistical analysis was done using the one-way Anova "F" test and Tukey's post-hoc test. Results Group 3 showed the least presence of a smear layer, followed by groups 4, 2, 5, and 1. All the groups exhibited highly significant differences between each other (p < 0.001). Group B shows no significant difference with groups C, D, and E. Group C shows no significant difference with groups D and E. Group D shows no significant difference with group E. Conclusion The EA removes the smear layer effectively as compared with other groups. All the irrigation activation system shows the presence of smear layer. No activation systems were able to remove the smear layer completely.

Abstract:Objective To study the effect of different irrigation regimens and methods of activation on smear layer removal in root canals.Methods Thirty-two mandibular premolar roots were divided into 4 groups,with 8 teeth in each.In group A,the teeth were instrumented and irrigated by lateral opening syringe.In group B,irrigation was done using passive ultrasonic irrigation (PUI) system.In group C RinsEndo root canal irrigation system was used and in group D,Vibringe sonic irrigation system.Scanning electron microscope evaluation was done for assessment of smear layer removal in the coronal,middle,and apical thirds.Results In each group,there was a smear layer covering the dentinal tubule in apical thirds,and the score was higher than that in the coronal and middle (P＜0.05).The scores of B,C and D groups were lower than those in group A (P ＜ 0.05).Among A,B,C,D groups in the area of the root canal wall,difference of smear layer score was not statistically significant.Conclusion Better removal of smear layer of the middle and apical thirds of root canal can be achieved by using RinsEndo,Vibringe and PUI system than by lateral opening syringe.
Role of Sympathetic Efferent Nerves in Blood Pressure Regulation and in Hypertension

Abstract:This article presents some aspects of the role of sympathetic efferent nerves in the regulation of blood pressure in humans. Lessons have been learned from disorders that cause either sympathetic underactivity or overactivity. In chronic autonomic failure, pressor stimuli (mental arithmetic, isometric exercise, or cold) are unable to raise blood pressure, whereas stimuli that normally activate sympathetic efferent nerves to maintain blood pressure (head-up tilt, exercise, and food ingestion) can cause marked hypotension. Recognition of specific defects, such as the inability to synthesize norepinephrine in isolated dopamine β-hydroxylase deficiency, suggests that sympathetic nerves may influence blood pressure regulation through nonadrenergic mechanisms (dopamine, neuropeptides, and purines). Tetraplegic patients with high cervical cord transection also have sympathetic impairment and postural hypotension, but this is less of a clinical problem because of compensatory hormonal and other mechanisms. Tetraplegic patients are unique as they also may have severe paroxysmal hypertension because of increased spinal sympathetic reflex activity. The pathophysiological mechanisms responsible for this exaggerated response include changes in postsynaptic adrenergic receptor numbers and their sensitivity, the actions of nonadrenergic cotransmitters, and the lack of sympathoneural pathways from the brain that are severed by the lesion. Finally, the putative role of the sympathetic nervous system in hypertension with unilateral renal artery stenosis, which initially is humorally mediated, is discussed. The centrally acting sympatholytic agent clonidine is effective in lowering blood pressure in renovascular hypertension independently of humoral factors when multiple agents have failed. The pathophysiological basis of altered cardiovascular control in these various disorders along with the effects of drugs with known mechanisms have advanced our understanding of the role in humans of sympathetic efferent nerves in blood pressure regulation and in hypertension.
Effectiveness of three auxiliary irrigation methods for cleaning the internal walls of root canals of curved isolated teeth

Abstract:Objectives: This study aimed to compare the effectiveness of ultrasound and acoustic and laser cleaning of curved root canals. Methods: A total of 92 molars with independent root canals with a curvature of 20°-40° were prepared and standardized at 04 25# and stained with gentian violet solution for 72 h. Among them, 52 were randomly divi-ded into four groups for final rinsing (n=13): NI group, PUI group, EDDY group, and PIPS group. Ten samples in each group were cut horizontally along the long axis perpendicular to the root and divided into curved upper, curved, and apical segments. Images were taken with a stereomicroscope and Image J measurements were taken to calculate the depth of rinse penetration. The remaining three samples from each group were split along the long axis of the dentin, photographed by scanning electron microscope to record the dentin tubule exposure and staining layer, and scored for staining layer by double-blind method. SPSS 26.0 software was used to perform statistical analysis and select the best flushing method. An extra 40 samples were randomly divided into four groups for detection of flushing fluid penetration depth (n=10): 10, 20, 30, and 40 s. Results: In the upper part, the mean depth of infiltration was not significantly different between the experimental and control groups (P>0.05). The PIPS group had a significantly lower smear layer score than the control group and the EDDY group (P<0.01). In the curved segment, the mean depth of infiltration was significantly greater in the PUI group than in the control group (P<0.05); the tarnish layer score was lower in each experimental group than in the control group. At the top, the mean depth of infiltration was greater in the PUI and PIPS groups than in the control group (P<0.05), and the smear layer score was lower in the PIPS group than in the other groups (P<0.05). After the time was changed, the depth of infiltration of PUI increased only in the apical segment as the flushing time increased. Conclusions: The PUI and PIPS methods facilitate the penetration of irrigation solution into the dentin canal in curved root canals, especially in the apical segment. The PIPS technique is effective in removing the smear layer in curved root canals.
In Vitro Efficacy of Apical Negative Pressure Irrigation in Primary Teeth: A Smear Layer Analysis

Abstract:Aim and background: This study aimed to assess the efficacy of root canal debris removal in primary teeth through irrigation using the Endo-Irrigator Plus (CWAIS). Materials and methods: A total of 25 primary mandibular first molars were divided into five groups, with five teeth in each group. After determining the working length and performing biomechanical preparation, groups I and II were subjected to hand and rotary instrumentation (ProTaper, Dentsply, United States of America) with conventional syringe irrigation with positive pressure, while groups III and IV were instrumented using hand and rotary instrumentation with positive and negative pressure irrigation employing the Endo-Irrigator Plus (CWAIS) system. Sodium hypochlorite (5.25%) and ethylenediaminetetraacetic acid (EDTA) (17%) were used as irrigants. Group V served as the control group, undergoing only root canal access opening. The removal of the smear layer was analyzed at the cervical, middle, and apical thirds of the canal using environmental scanning electron microscopy (SEM). Statistical significance was set at p < 0.05. Results: Statistically significant differences were observed among all groups at the cervical, middle, and apical levels (p < 0.05). Groups III and IV exhibited significantly lower smear scores at all levels, particularly in the apical third, as observed through SEM examination. Conclusion: The use of the Endo-Irrigator Plus (CWAIS) system demonstrated superior debridement of intracanal debris in primary teeth compared to the positive pressure irrigation method. Abstract:ABSTRACT Aim: The purpose of this study is to evaluate and compare the efficacy of 17% EDTA, 18% etidronic acid, and 0.2% chitosan nanoparticle solution in smear layer removal using SEM image analysis. Methods: Thirty freshly extracted mandibular premolars were used. After biomechanical preparation, the samples were divided into Group I (17% EDTA), Group II (18% etidronic acid), and Group III (0.2% chitosan nanoparticle solution) containing 10 samples each. Longitudinal sectioning of the samples was done. The samples were observed under SEM at apical, middle, and coronal levels. The images were scored according to the criteria by Hullsman. Statistical analysis was done, with the significance level set at P < 0.05, and performed with SPSS 16.0 statistical package for Windows. Results: Smear layer removal at coronal, middle, and apical thirds was more effective when final irrigation was performed using 0.2% chitosan solution, followed by 17% EDTA. At the apical third, all the irrigants showed poor smear layer removing property, but chitosan showed comparatively better results. Conclusion: 0.2% chitosan nanoparticle solution was more effective in removing the smear layer when compared to 17% EDTA and 18% etidronic acid irrigants.

Abstract:Abstract Sepsis and necrotizing enterocolitis (NEC) cause significant morbidity and mortality in the newborn. Their ill effects persist in spite of appropriate and effective antibiotic therapy. Lactoferrin as an adjunct to antibiotics in the treatment of sepsis or NEC in the newborn may improve the clinical outcomes by enhancing the host defense and modulating the inflammatory response. This review focuses on the various aspects of lactoferrin use in the newborn.

Abstract:Aim The aim of this study was to evaluate the effect of two different concentrations of chitosan irrigation with two rotary systems (Revo-S and One Shape) on smear layer removal during root canal treatment using scanning electron microscope. Materials and methods Forty freshly extracted human premolar with single canal and nearly straight roots were collected. Teeth were decoronated to obtain a standardized root length of nearly 15 ± 1 mm. Samples were randomly divided into four main groups ( n = 10) according to rotary preparation system and final chitosan irrigation concentration used. Group I: Revo-S rotary system and 5 ml of 0.2% chitosan solution for 3 min as final irrigation. Group II: One Shape single file and 5 ml of 0.2% chitosan solution for 3 min as final irrigation. Group III: Revo-S rotary system and 5 ml of 0.37% chitosan solution for 3 min as final irrigation. Group IV: One Shape single file and 5 ml 0.37% of chitosan solution for 3 min as final irrigation. During preparation and after change of each succeeding file, root canal irrigated with freshly prepared 5 ml of 2.5% sodium hypochlorite solution. Then, rinsed with 5 ml of distilled water. Finally, root canals were irrigated with final chitosan irrigation concentration as mentioned in groups. Samples were split longitudinally and examined by scanning electron microscope analysis. Root canal cleanliness was qualitatively assessed at the coronal, middle, and apical regions of root halves. Kruskal–Wallis test was used to compare between final irrigation solutions at each section and Mann–Whitney U test was used for pair-wise comparisons between groups whenever indicated. The significance level was set at P value less than or equal to 0.05. Results It showed that there were no statistical significant differences between groups for smear layer covering the dentinal surfaces. When comparing different root levels, there were no statistical significant differences between coronal versus middle levels but between coronal versus apical and middle versus apical statistical significant differences were found. Conclusions Smear layer can be removed effectively in all tested groups from coronal and middle levels compared to apical level. Using multiple file system (Revo-S files) is better than single file system (One Shape file) in removal of the smear layer of root canal. Increasing the concentration of chitosan to 0.37% promoted removal of the smear layer better than 0.2% of chitosan.

After traditional endodontic cavities preparation, root canals were prepared to size #35 with a 0.04 taper. The final irrigating techniques were as follows: (I) manual dynamic activation (MDA), (II) ultrasonically activated irrigation (UAI), (III) sonically activated irrigation (SAI), (IV) PIPS, and (V) SWEEPS. Fifty teeth were randomly divided into five groups mentioned above. After root canal preparation, the roots were cleaved longitudinally. The dentine surfaces were photographed from the coronal, middle, and apical third of the root by scanning electron microscopy operated at a low vacuum. Two examiners separately graded each image according to the remained smear layer situations.Results: PIPS and SWEEPS groups showed fewer smear layers remaining than the others in the middle and the apical third (P < 0.05) of the root canal. In contrast, in the coronal third, five groups showed no significant difference (P > 0.05).Conclusion: PIPS and SWEEPS showed superior smear layer clearing efficiency than traditional irrigating techniques in curved root canals.

Abstract:Background/purpose Establishing effective irrigation methods is warranted to ensure the predictability of minimally invasive root canal instrumentation. This study aimed to compare the smear layer removal efficacy of different irrigation techniques in root canals instrumented with TruNatomy nickel-titanium rotary instruments. Materials and methods Experiment 1: Extracted human mandibular incisors were instrumented using TruNatomy Shaping Files up to Small (#20/0.04), Prime (#26/0.04), or Medium (#36/0.03) (n = 17, each) and irrigated with EDTA, NaOCl, and again with EDTA using syringe irrigation (SI). Experiment 2: Extracted mandibular incisors were instrumented using TruNatomy Small and irrigated with EDTA and NaOCl as in Experiment 1, using (1) conventional laser-activated irrigation (LAI) with an erbium-doped yttrium aluminum garnet laser unit at 30 mJ/10 Hz (LAI 30/10) or 70 mJ/20 Hz (LAI 70/20), (2) photon-induced photoacoustic streaming (PIPS) (20 mJ/15 Hz), (3) ultrasonic-activated irrigation, or (4) SI (n = 13 each). Five additional canals served as negative controls (no irrigation). The smear layer on the canal wall was scored under scanning electron microscopy. Results In Experiment 1, the TruNatomy Small group exhibited significantly higher smear layer scores compared to the other groups in the apical and middle thirds. In Experiment 2, the LAI 70/20 and PIPS groups demonstrated significantly lower smear layer scores compared to the LAI 30/10 and SI groups in the apical third. Conclusion Conservative instrumentation using the TruNatomy Small reduced the cleaning efficacy of irrigation. However, PIPS performed satisfactory for smear layer removal without injury to the root canal wall.

Abstract:The purpose of this study was to evaluate the effect of different irrigating solutions on smear layer removal and dentinal tubule opening on root canal surfaces after post space preparation and to study whether additional ultrasonic irrigation has any effect on smear layer removal. Forty-eight anterior teeth were treated endodontically. After post space preparation, they were assigned to six groups: group 1, EDTA; group 2, EDTA with ultrasonic activation; group 3, sodium hypochlorite (NaOCl); group 4, NaOCl with ultrasonic activation; group 5, sodium chloride (NaCl); and group 6, NaCl with ultrasonic activation. Specimens were examined under a field-emission scanning electron micro;cope and scored for debris removal and dentinal tubule opening at the coronal, middle, and apical thirds of the root canal. The results showed that EDTA performed significantly better than NaCl and NaOCl in smear layer removal and dentinal tubule opening. Additional ultrasonic irrigation did not improve smear layer removal significantly. (J Endod 2009;35:583-586)

Abstract:Balloon catheter de-endothelialization of abdominal aorta of cholesterol-fed rabbits was performed. As a result, partly endothelialized neointima was formed, consisting of smooth muscle cells and macrophages. Neutral lipids stored more intensely in non-endothelialized part of neointima. Preferential intracellular lipid accumulation in this area was observed while predominantly extracellular lipid accumulation was found in endothelium-covered part of neointima. It is supposed that endothelium inhibits intracellular lipid accumulation and at the same time stimulates an extracellular one, but as a whole it reduces the density of lipid inclusions in neointima.

Abstract:Aim To evaluate the influence of different preparation tapers on the reduction in planktonic bacteria and biofilms of Enterococcus faecalis and Candida albicans in the apical third (4 mm) of the mesial roots of mandibular molars, correlating decontamination with canal shape. Methodology After microtomography analysis for morphological standardization of the canals, 48 mandibular molar roots, each containing two canals (96 canals), were contaminated with E. faecalis and C. albicans and divided into four groups (n = 11) for canal instrumentation using ProDesign Logic 2 files with different tapers G (.03): # 25.03; G (.04): # 25.04; G (.05): # 25.05; and G (.06): # 25.06 and irrigation with 2.5% sodium hypochlorite. Four roots were examined under a scanning electron microscope (SEM) to qualitatively assess biofilm formation. Eight roots were used as the negative control group (samples were not contaminated). Bacteriological samples were taken exclusively from the apical third of the roots before and after chemical–mechanical preparation and bacterial counts were determined (CFU/mL). The final micro‐CT scan was used to quantify the volume variation and unprepared canal area in the apical third. Statistical analysis was performed using the Kruskal–Wallis, Student–Newman–Keuls and Wilcoxon tests for analysis of microbiological data. anova and the Tukey or Games–Howell test were used for analysis of micro‐CT data and Spearman's test for correlations (α = 5%). Results All groups showed a significant reduction in bacteria (p .05). Conclusion The different preparation tapers influenced root canal shaping in the apical third but did not improve decontamination in this region.
