Abstract:Background Patients with hypertrophic cardiomyopathy (HCM) are at risk for sudden death and individuals with ≥1 major risk markers are considered for primary prevention implantable cardioverter defibrillators. Guidelines recommend cardiac magnetic resonance imaging (CMR) to identify high-risk imaging features. However, CMR is resource intensive and is not widely accessible worldwide. Objective To develop electrocardiogram (ECG) deep-learning (DL) models for identification of HCM patients with high-risk features. Methods HCM patients evaluated at Tufts Medical Center (N=1,930; Boston, United States) were used to develop ECG-DL models for prediction of high-risk imaging features: systolic dysfunction, massive hypertrophy (≥30mm), apical aneurysm, and extensive late-gadolinium enhancement (LGE). ECG-DL models were externally validated in an HCM cohort from Amrita Hospital (N=233; Kochi, India). Results ECG-DL models reliably identified high-risk features (systolic dysfunction, massive hypertrophy, apical aneurysm, and extensive LGE) during hold-out model testing (c-statistics 0.72, 0.83, 0.93, and 0.76) and external validation (c-statistics 0.71, 0.76, 0.91, and 0.68). A hypothetical screening strategy employing echocardiography combined with ECG-DL guided selective CMR use demonstrated sensitivity of 97% for identifying patients with high-risk features, while reducing the number of recommended CMRs by 61%. Negative predictive value with this screening strategy for absence of high-risk features in patients without ECG-DL recommendation for CMR was 99.5%. Conclusions In HCM, novel ECG-DL models reliably identified patients with high-risk imaging features while offering the potential to reduce CMR testing requirements in under-resourced areas.

Abstract:Objective: Given the psychosocial and ethical burden, patients with hypertrophic cardiomyopathy (HCMs) could benefit from the establishment of genetic probability prior to the test. This study aimed to develop a simple tool to provide genotype prediction for HCMs. Methods: A convolutional neural network (CNN) was built with the 12-lead electrocardiogram (ECG) of 124 HCMs who underwent genetic testing (GT), externally tested by predicting the genotype on another HCMs cohort (n = 54), and compared with the conventional methods (the Mayo and Toronto score). Using a third cohort of HCMs (n = 76), the role of the network in risk stratification was explored by calculating the sudden cardiac death (SCD) risk scorers (HCM risk-SCD) across the predicted genotypes. Score-CAM was employed to provide a visual explanation of the network. Results: Overall, 80 of 178 HCMs (45%) were genotype-positive. Using the 12-lead ECG as input, the network showed an area under the curve (AUC) of 0.89 (95% CI, 0.83-0.96) on the test set, outperforming the Mayo score (0.69 [95% CI, 0.65-0.78], p < 0.001) and the Toronto score (0.69 [95% CI, 0.64-0.75], p < 0.001). The network classified the third cohort into two groups (predicted genotype-negative vs. predicted genotype-positive). Compared with the former, patients predicted genotype-positive had a significantly higher HCM risk-SCD (0.04 & PLUSMN; 0.03 vs. 0.03 & PLUSMN; 0.02, p <0.01). Visualization indicated that the prediction was heavily influenced by the limb lead. Conclusions: The network demonstrated a promising ability in genotype prediction and risk assessment in HCM. Key messages Patients with genotype-positive hypertrophic cardiomyopathy (HCM) have a higher risk of severe heart failure and sudden cardiac death (SCD). A deep learning-derived 12-lead electrocardiogram was developed, outperformed the conventional methods in genotype prediction, and showed a promising future in SCD risk assessment in HCM.

Standard measures of voltage abnormalities on electrocardiograms (ECGs) have demonstrated high false positive rates and poor correlation of LV thickness. Despite poor testing characteristics, ECGs continue to be part of most adolescent and family screening programs. While recent studies suggest that machine learning based analysis of ECGs may have utility in the identification of HCM in adult patients, there are limited data regarding its effectiveness in the pediatric population.PurposeThe purpose of this study was to develop a deep learning model utilizing raw digital ECG data to accurately distinguish healthy pediatric patients from those with HCM.MethodsECGs from patients < 19 years with HCM were compared to ECGs from age- and sex-matched patients that underwent evaluation with ECG and echocardiogram for chest pain, sports clearance, syncope, or abnormal ECG between January 2011 and March 2021. Patients with a known clinical history of primary causes of left ventricular hypertrophy such as aortic stenosis, systemic hypertension, and glycogen storage disease were excluded from the study. Unsupervised signal processing tools including descriptive statistics and wavelet transformation were used to extract features from 12-lead ECG signals. A cascaded convolutional neural network was developed combining a residual neural network to extract features from waveform 12-lead ECG signals with a 2-layer dense neural network to embed features from signal processing tools. Model performance was evaluated by 10-fold stratified cross validation.ResultsThe analytic sample included 181 patients (72% male; 63% Caucasian). Mean age was 12.3 ± 5.8 years. A cascaded deep learning model classified patients into healthy or HCM categories with a cross validation sensitivity of 77.7% and specificity of 81.6%. The positive predictive value was 82% and AUC of 0.89 (0.84-0.94).ConclusionThis CNN trained with a relatively small number of images was able to accurately identify HCM in pediatric patients from standard 12-lead ECG. Evaluation of its potential utility in larger populations is needed.

Abstract:Background: Hypertrophic cardiomyopathy (HCM) is a leading cause of sudden cardiac death among young people and be detected from ECGs using artificial intelligence. Wearable devices could allow for broad AI-based screening but obtain noisy ECGs. We report the development of a wearable-adapted AI-ECG model that retains performance for detecting HCM in noisy single-lead ECGs. Methods: In 11,883 ECGs obtained within 30 days of transthoracic echocardiograms from Yale New Haven Hospital (2015-2021), we defined HCM as interventricular septal wall thickness (end diastole) (IVSd) > 15 mm and moderate to severe left ventricular diastolic dysfunction (LVDD). Standard and noise-adapted models were trained using a 1:10 age-sex matched dataset. For the noise-adapted model, each ECG was included four times in the training set, augmented each time with random gaussian noise within four distinct frequency ranges emulating different real-world noise sources. We evaluated the performance of the noise-adapted and standard models on an independent set of ECGs with four different real-world noisy artifacts, including noise extracted from a portable device ECG, at multiple signal-to-noise ratios (SNRs). Results: In single-lead ECGs without noise, the noise-adapted model outperformed the standard model, with an AUROC of 0.920 and 0.906, respectively for HCM. With the same ECGs augmented with noise ranging from half (SNR 2) to twice (SNR 0.5) the signal, the noise-adapted model maintained their performance across real-world noise signatures (A). At an SNR of 0.5, the noise adapted model had a significantly greater performance in detecting HCM in ECGs augmented with portable device ECG noise (AUROC 0.917 [0.861-0.974] vs 0.609 [0.398, 0.821]) (B). Conclusions: We developed a model which accurately screens for HCM in noisy single-lead ECGs. This approach represents a more accessible and scalable method for screening for HCM using wearable device ECGs.

Abstract:Circulation, Volume 146, Issue Suppl_1, Page A14817-A14817, November 8, 2022. Introduction:Hypertrophic cardiomyopathy (HCM) has an estimated prevalence of 1/500 but remains underdiagnosed. Echocardiography is the primary diagnostic modality but requires recognition of varied patterns of hypertrophy. Point of care tools to facilitate improved identification of HCM are needed.Methods:We identified 400 study cases of HCM in adults meeting professional society guideline diagnostic criteria from 2000-2020 at Duke University Medical Center. 900 study controls matched on age, sex, year of echo, and ejection fraction were also identified. Multi-vendor echocardiographic examinations performed within 12 months of initial diagnosis were used to train a convolutional neural network (CNN) to identify HCM. The model consisting of a 18-layer deep R(2+1)D residual network was developed on 60% of the data (training), 20% of the data (validation) was used to select model hyperparameters and to monitor for overfitting, and the remaining 20% (test) was used for performance estimation. CNN performance was evaluated by the receiving operating characteristic (ROC) and precision-recall curve and their summaries, namely, area under the ROC (AUROC) and average precision (AP,i.e., average positive predictive value). We report performance characteristics at the study level.Results:Among 400 HCM cases and 900 controls, the median age was 65, with 48.1% male. Hypertension was present in 44.9%, with heart failure in 16.3% and Atrial Fibrillation in 12%. Median EF was 55% with 14.5% having an EF < 50%. Median diastolic dimension was 4.4 cm and moderate or severe MR was present in 8.4%. Study-level AUCROC for the CNN was 0.853 and the average precision was 0.698 (Figure 1).Conclusion:A machine learning CNN is able identify study-wise HCM with an AUC of 0.853. Further work will be necessary to ensure explainability of results and similar multi-institutional performance as a point of care tool for the echocardiographic identification of HCM on diagnostic rates.

Abstract:Aims: Recently, deep learning artificial intelligence (AI) models have been trained to detect cardiovascular conditions, including hypertrophic cardiomyopathy (HCM), from the 12-lead electrocardiogram (ECG). In this external validation study, we sought to assess the performance of an AI-ECG algorithm for detecting HCM in diverse international cohorts. Methods and results: A convolutional neural network-based AI-ECG algorithm was developed previously in a single-centre North American HCM cohort (Mayo Clinic). This algorithm was applied to the raw 12-lead ECG data of patients with HCM and non-HCM controls from three external cohorts (Bern, Switzerland; Oxford, UK; and Seoul, South Korea). The algorithm's ability to distinguish HCM vs. non-HCM status from the ECG alone was examined. A total of 773 patients with HCM and 3867 non-HCM controls were included across three sites in the merged external validation cohort. The HCM study sample comprised 54.6% East Asian, 43.2% White, and 2.2% Black patients. Median AI-ECG probabilities of HCM were 85% for patients with HCM and 0.3% for controls (P < 0.001). Overall, the AI-ECG algorithm had an area under the receiver operating characteristic curve (AUC) of 0.922 [95% confidence interval (CI) 0.910-0.934], with diagnostic accuracy 86.9%, sensitivity 82.8%, and specificity 87.7% for HCM detection. In age- and sex-matched analysis (case-control ratio 1:2), the AUC was 0.921 (95% CI 0.909-0.934) with accuracy 88.5%, sensitivity 82.8%, and specificity 90.4%. Conclusion: The AI-ECG algorithm determined HCM status from the 12-lead ECG with high accuracy in diverse international cohorts, providing evidence for external validity. The value of this algorithm in improving HCM detection in clinical practice and screening settings requires prospective evaluation.

Abstract:Background: Hypertrophic cardiomyopathy (HCM) is associated with significant morbidity and mortality including sudden cardiac death in the young. Its prevalence is estimated to be 1 in 500, although many people are undiagnosed. The ability to screen electrocardiograms (ECGs) for its presence could improve detection and enable earlier diagnosis. Objectives: This study evaluated the accuracy of an artificial intelligence device (Viz HCM) in detecting HCM based on 12-lead ECG. Methods: The device was previously trained using deep learning and provides a binary outcome (HCM suspected or not suspected). This study included 293 HCM-Positive and 2912 HCM-Negative cases, which were selected from three hospitals based on chart review incorporating billing diagnostic codes, cardiac imaging, and ECG features. The device produced an output for 291 (99.3%) HCM-Positive and 2905 (99.8%) HCM-Negative cases. Results: The device identified HCM with sensitivity 68.4% (95% CI: 62.8-73.5%), specificity 99.1% (95% CI: 98.7-99.4%) and area under the curve 0.975 (95% CI: 0.965-0.982). With assumed population prevalence of 0.002 (1 in 500), the positive predictive value was 13.7% (95% CI: 10.1-19.9%) and the negative predictive value was 99.9% (95% CI: 99.9-99.9%). The device demonstrated consistent performance across demographic and technical subgroups. Conclusions: The device identified HCM based on 12-lead ECG with good performance. Coupled with clinical expertise, it has the potential to augment HCM detection and diagnosis.

Abstract:Objectives: In hypertrophic cardiomyopathy (HCM), specific ECG abnormalities are observed. Therefore, ECG is a valuable screening tool. Although several studies have reported on estimating the risk of developing fatal arrhythmias from ECG findings, the use of ECG to identify the severity of heart failure (HF) by applying deep learning (DL) methods has not been established. Methods: We assessed whether data-driven machine-learning methods could effectively identify the severity of HF in patients with HCM. A residual neural network-based model was developed using 12-lead ECG data from 218 patients with HCM and 245 patients with non-HCM, categorised them into two (mild-to-moderate and severe) or three (mild, moderate and severe) severities of HF. These severities were defined according to the New York Heart Association functional class and levels of the N-terminal prohormone of brain natriuretic peptide. In addition, the patients were divided into groups according to Kansas City Cardiomyopathy Questionnaire (KCCQ)-12. A transfer learning method was applied to resolve the issue of the low number of target samples. The model was trained in advance using PTB-XL, which is an open ECG dataset. Results: The model trained with our dataset achieved a weighted average F1 score of 0.745 and precision of 0.750 for the mild-to-moderate class samples. Similar results were obtained for grouping based on KCCQ-12. Through data analyses using the Guided Gradient Weighted-Class Activation Map and Integrated Gradients, QRS waves were intensively highlighted among true-positive mild-to-moderate class cases, while the highlighted part was highly variable among true-positive severe class cases. Conclusions: We developed a model for classifying HF severity in patients with HCM using a deep neural network algorithm with 12-lead ECG data. Our findings suggest that applications of this DL algorithm for using 12-lead ECG data may be useful to classify the HF status in patients with HCM.

Abstract:Introduction Misdiagnosis of hypertrophic cardiomyopathy (HCM) is not uncommon among patients with left ventricular hypertrophy (LVH). Purpose We hypothesized that a convolutional neural network (CNN) could be trained to identify HCM patients among individuals with LVH using standard transthoracic echocardiography (TTE). Methods Echocardiographic reports of 121,767 unique patients linked to clinical data from the largest tertiary medical centre in the country (2007-2022) were reviewed. Patients with no septal hypertrophy were excluded. All HCM cases were retrieved from electronic medical records and were adjudicated by the authors. Data was then divided into 3 mutually exclusive cohorts: training (N=7,368), testing (N=2,456) and validation (2,456) cohorts. Deep learning models were used in order to train a model that identifies HCM patients. The model was based solely on parasternal long axis and apical four chamber views. Results Final study population included 12,281 patients who underwent a total of 27,242 echocardiography studies. There were 1535 (12%) patients with HCM. Mean age of study population was 72±14 and 7,943 (65%) were men. Overall, the model obtained 98.1% classification accuracy with an area under the curve of 0.851 in the independent test data (FIGURE) which could translate to sensitivity and specificity of 68% and 99%, respectively with positive and negative predictive values of 50% and 99%, respectively. Conclusions Applying artificial intelligence to the standard TTE can identify HCM patients with a reasonable accuracy. With further evaluation, this tool can be used to assist in HCM diagnosis.

Abstract:The efficacy of convolutional neural network (CNN)-enhanced electrocardiography (ECG) in detecting hypertrophic cardiomyopathy (HCM) and dilated HCM (dHCM) remains uncertain in real-world applications. This retrospective study analyzed data from 19,170 patients (including 140 HCM or dHCM) in the Shinken Database (2010–2017). We evaluated the sensitivity, positive predictive rate (PPR), and F1 score of CNN-enhanced ECG in a ''basic diagnosis'' model (total disease label) and a ''comprehensive diagnosis'' model (including disease subtypes). Using all-lead ECG in the "basic diagnosis" model, we observed a sensitivity of 76%, PPR of 2.9%, and F1 score of 0.056. These metrics improved in cases with a diagnostic probability of ≥ 0.9 and left ventricular hypertrophy (LVH) on ECG: 100% sensitivity, 8.6% PPR, and 0.158 F1 score. The ''comprehensive diagnosis'' model further enhanced these figures to 100%, 13.0%, and 0.230, respectively. Performance was broadly consistent across CNN models using different lead configurations, particularly when including leads viewing the lateral walls. While the precision of CNN models in detecting HCM or dHCM in real-world settings is initially low, it improves by targeting specific patient groups and integrating disease subtype models. The use of ECGs with fewer leads, especially those involving the lateral walls, appears comparably effective.

Abstract:Background: Differentiating among cardiac diseases associated with left ventricular hypertrophy (LVH) informs diagnosis and clinical care. Objective: To evaluate if artificial intelligence-enabled analysis of the 12-lead electrocardiogram (ECG) facilitates automated detection and classification of LVH. Methods: We used a pretrained convolutional neural network to derive numerical representations of 12-lead ECG waveforms from patients in a multi-institutional healthcare system who had cardiac diseases associated with LVH (n = 50,709), including cardiac amyloidosis (n = 304), hypertrophic cardiomyopathy (n = 1056), hypertension (n = 20,802), aortic stenosis (n = 446), and other causes (n = 4766). We then regressed LVH etiologies relative to no LVH on age, sex, and the numerical 12-lead representations using logistic regression ("LVH-Net"). To assess deep learning model performance on single-lead data analogous to mobile ECGs, we also developed 2 single-lead deep learning models by training models on lead I ("LVH-Net Lead I") or lead II ("LVH-Net Lead II") from the 12-lead ECG. We compared the performance of the LVH-Net models to alternative models fit on (1) age, sex, and standard ECG measures, and (2) clinical ECG-based rules for diagnosing LVH. Results: The areas under the receiver operator characteristic curve of LVH-Net by specific LVH etiology were cardiac amyloidosis 0.95 [95% CI, 0.93-0.97], hypertrophic cardiomyopathy 0.92 [95% CI, 0.90-0.94], aortic stenosis LVH 0.90 [95% CI, 0.88-0.92], hypertensive LVH 0.76 [95% CI, 0.76-0.77], and other LVH 0.69 [95% CI 0.68-0.71]. The single-lead models also discriminated LVH etiologies well. Machine Learning Methods for Identifying Atrial Fibrillation Cases and Their Predictors in Patients With Hypertrophic Cardiomyopathy: The HCM-AF-Risk Model

Abstract:Hypertrophic cardiomyopathy (HCM) patients have a high incidence of atrial fibrillation (AF) and increased stroke risk, even with low risk of congestive heart failure, hypertension, age, diabetes, previous stroke/transient ischemic attack scores. Hence, there is a need to understand the pathophysiology of AF and stroke in HCM. In this retrospective study, we develop and apply a data-driven, machine learning based method to identify AF cases, and clinical and imaging features associated with AF, using electronic health record data. HCM patients with documented paroxysmal/persistent/permanent AF (n = 191) were considered AF cases, and the remaining patients in sinus rhythm (n = 640) were tagged as No-AF. We evaluated 93 clinical variables and the most informative variables useful for distinguishing AF from No-AF cases were selected based on the 2-sample t test and the information gain criterion. We identified 18 highly informative variables that are positively (n = 11) and negatively (n = 7) correlated with AF in HCM. Next, patient records were represented via these 18 variables. Data imbalance resulting from the relatively low number of AF cases was addressed via a combination of oversampling and under-sampling strategies. We trained and tested multiple classifiers under this sampling approach, showing effective classification. Specifically, an ensemble of logistic regression and naive Bayes classifiers, trained based on the 18 variables and corrected for data imbalance, proved most effective for separating AF from No-AF cases (sensitivity = 0.74, specificity = 0.70, C-index = 0.80). Our model is the first machine learning based method for identification of AF cases in HCM. This model demonstrates good performance, addresses data imbalance, and suggests that AF is associated with a more severe cardiac HCM phenotype.

Abstract:Background: Hypertrophic cardiomyopathy (HCM) is a widely distributed, but clinically heterogeneous genetic heart disease, affects approximately 20 million people worldwide. Nowadays, HCM is treatable with the advancement of medical interventions. However, due to occult clinical presentations and a lack of easy, inexpensive, and widely popularized screening approaches in the general population, 80–90% HCM patients are not clinically identifiable, which brings certain safety hazards could have been prevented. The majority HCM patients showed abnormal and diverse electrocardiogram (ECG) presentations, it is unclear which ECG parameters are the most efficient for HCM screening. Objective: We aimed to develop a pragmatic prediction model based on the most common ECG features to screen for HCM. Methods: Between April 1st and September 30th, 2020, 423 consecutive subjects from the International Cooperation Center for Hypertrophic Cardiomyopathy of Xijing Hospital [172 HCM patients, 251 participants without left ventricular hypertrophy (non-HCM)] were prospectively included in the training cohort. Between January 4th and February 30th, 2021, 163 participants from the same center were included in the temporal internal validation cohort (62 HCM patients, 101 non-HCM participants). External validation was performed using retrospectively collected ECG data from Xijing Hospital (3,232 HCM ECG samples from January 1st, 2000, to March 31st, 2020; 95,184 non-HCM ECG samples from January 1st to December 31st, 2020). The C-statistic was used to measure the discriminative ability of the model. Results: Among 30 ECG features examined, all except abnormal Q wave significantly differed between the HCM patients and non-HCM comparators. After several independent feature selection approaches and model evaluation, we included only two ECG features, T wave inversion (TWI) and the amplitude of S wave in lead V1 (SV1), in the HCM prediction model. The model showed a clearly useful discriminative performance (C-statistic > 0.75) in the training [C-statistic 0.857 (0.818–0.896)], and temporal validation cohorts [C-statistic 0.871 (0.812–0.930)]. In the external validation cohort, the C-statistic of the model was 0.833 [0.825–0.841]. A browser-based calculator was generated accordingly. Conclusion: The pragmatic model established using only TWI and SV1 may be helpful for predicting the probability of HCM and shows promise for use in population-based HCM screening.

An Ensemble Deep Learning Algorithm for Structural Heart Disease Screening Using Electrocardiographic Images: PRESENT SHD

Abstract:Background: Identifying structural heart diseases (SHDs) early can change the course of the disease, but their diagnosis requires cardiac imaging, which is limited in accessibility. Objective: To leverage 12-lead ECG images for automated detection and prediction of multiple SHDs using a novel deep learning model. Methods: We developed a series of convolutional neural network models for detecting a range of individual SHDs from images of ECGs with SHDs defined by transthoracic echocardiograms (TTEs) performed within 30 days of the ECG at the Yale New Haven Hospital (YNHH). SHDs were defined based on TTEs with LV ejection fraction <40%, moderate-to-severe left-sided valvular disease (aortic/mitral stenosis or regurgitation), or severe left ventricular hypertrophy (IVSd > 1.5cm and diastolic dysfunction). We developed an ensemble XGBoost model, PRESENT-SHD, as a composite screen across all SHDs. We validated PRESENT-SHD at 4 US hospitals and a prospective population-based cohort study, the Brazilian Longitudinal Study of Adult Health (ELSA-Brasil), with concurrent protocolized ECGs and TTEs. We also used PRESENT-SHD for risk stratification of new-onset SHD or heart failure (HF) in clinical cohorts and the population-based UK Biobank (UKB). Results: The models were developed using 261,228 ECGs from 93,693 YNHH patients and evaluated on a single ECG from 11,023 individuals at YNHH (19% with SHD), 44,591 across external hospitals (20-27% with SHD), and 3,014 in the ELSA-Brasil (3% with SHD). In the held-out test set, PRESENT-SHD demonstrated an AUROC of 0.886 (0.877-894), sensitivity of 89%, and specificity of 66%. At hospital-based sites, PRESENT-SHD had AUROCs ranging from 0.854-0.900, with sensitivities and specificities of 93-96% and 51-56%, respectively. The model generalized well to ELSA-Brasil (AUROC, 0.853 [0.811-0.897], sensitivity 88%, specificity 62%). PRESENT-SHD performance was consistent across demographic subgroups. A positive PRESENT-SHD screen portended a 2- to 4-fold higher risk of new-onset SHD/HF, independent of demographics, comorbidities, and the competing risk of death across clinical sites and UKB, with high predictive discrimination. Conclusion: We developed and validated PRESENT-SHD, an AI-ECG tool identifying a range of SHD using images of 12-lead ECGs, representing a robust, scalable, and accessible modality for automated SHD screening and risk stratification.
Deep learning algorithm to improve hypertrophic cardiomyopathy mutation prediction using cardiac cine images

Abstract:ObjectivesThe high variability of hypertrophic cardiomyopathy (HCM) genetic phenotypes has prompted the establishment of risk-stratification systems that predict the risk of a positive genetic mutation based on clinical and echocardiographic profiles. This study aims to improve mutation-risk prediction by extracting cardiovascular magnetic resonance (CMR) morphological features using a deep learning algorithm.MethodsWe recruited 198 HCM patients (48% men, aged 47 ± 13 years) and divided them into training (147 cases) and test (51 cases) sets based on different genetic testing institutions and CMR scan dates (2012, 2013, respectively). All patients underwent CMR examinations, HCM genetic testing, and an assessment of established genotype scores (Mayo Clinic score I, Mayo Clinic score II, and Toronto score). A deep learning (DL) model was developed to classify the HCM genotypes, based on a nonenhanced four-chamber view of cine images.ResultsThe areas under the curve (AUCs) for the test set were Mayo Clinic score I (AUC: 0.64, sensitivity: 64.29%, specificity: 47.83%), Mayo Clinic score II (AUC: 0.70, sensitivity: 64.29%, specificity: 65.22%), Toronto score (AUC: 0.74, sensitivity: 75.00%, specificity: 56.52%), and DL model (AUC: 0.80, sensitivity: 85.71%, specificity: 69.57%). The combination of the DL and the Toronto score resulted in a significantly higher predictive performance (AUC = 0.84, sensitivity: 83.33%, specificity: 78.26%), compared with Mayo I (p = 006), Mayo II (p = 022), and Toronto score (p = 0.029).ConclusionsThe combination of the DL model, based on nonenhanced cine CMR images and the Toronto score yielded significantly higher diagnostic performance in detecting HCM mutations.Key Points• Deep learning method could enable the extraction of image features from cine images.• Deep learning method based on cine images performed better than established scores in identifying HCM patients with positive genotypes.• The combination of the deep learning method based on cine images and the Toronto score could further improve the performance of the identification of HCM patients with positive genotypes.

Abstract:Clinical risk stratification for sudden cardiac death (SCD) in hypertrophic cardiomyopathy (HC) employs rules derived from American College of Cardiology Foundation/American Heart Association (ACCF/AHA) guidelines or the HCM Risk-SCD model (C-index of 0.69), which utilize a few clinical variables. We assessed whether data-driven machine learning methods that consider a wider range of variables can effectively identify HC patients with ventricular arrhythmias (VAr) that lead to SCD. We scanned the electronic health records of 711 HC patients for sustained ventricular tachycardia or ventricular fibrillation. Patients with ventricular tachycardia or ventricular fibrillation (n = 61) were tagged as VAr cases and the remaining (n = 650) as non-VAr. The 2-sample t test and information gain criterion were used to identify the most informative clinical variables that distinguish VAr from non-VAr; patient records were reduced to include only these variables. Data imbalance stemming from low number of VAr cases was addressed by applying a combination of over- and under-sampling strategies.We trained and tested multiple classifiers under this sampling approach, showing effective classification. We evaluated 93 clinical variables, of which 22 proved predictive of VAr. The ensemble of logistic regression and naive Bayes classifiers, trained based on these 22 variables and corrected for data imbalance, was most effective in separating VAr from non-VAr cases (sensitivity = 0.73, specificity = 0.76, C-index = 0.83). Our method (HCM-VAr-Risk Model) identified 12 new predictors of VAr, in addition to 10 established SCD predictors. In conclusion, this is the first application of machine learning for identifying HC patients with VAr, using clinical attributes.

Abstract:Background: Wearable devices capable of ECG acquisition can help scale screening of structural heart disease (SHD) if diagnoses can be made with a single lead ECG. Algorithms for portable and wearable ECG require resilience to noisy acquisition but can enable detecting SHD in a community setting before symptom onset, ideal for public health management. Methods: Using Yale EHR data (2015-2021), we developed an artificial intelligence model to detect the presence of left ventricular systolic dysfunction, severe left-sided valvular disease, or hypertrophic cardiomyopathy using lead I of an ECG, which is commonly captured by wearable devices. We used 563,668 ECGs paired with echocardiograms obtained within 30 days of each other from 131,804 patients to develop the model. The dataset was split into training, validation, and test sets (85:5:10). For individual disease labels, we trained 6 convolutional neural network models using ECG signals augmented with random Gaussian real-world noise within 4 distinct frequency ranges and at 4 signal-to-noise ratios. Then, we included their prediction probabilities and age and sex as inputs to an XGBoost model to predict the composite SHD (Fig). Results: In the development set, the prevalence of composite SHD was 13%, encompassing left ventricular systolic dysfunction (9%), severe valvular disease (4%), and hypertrophic cardiomyopathy (1%). In the held-out test set of 13,181 patients (65±17 years, 49% women, 14% Black), AUROC and AUPRC of the ensemble model for prediction of SHD were 0.92 (0.90-0.93) and 0.61 (0.54-0.68), respectively (Fig). With optimized Youden's index, the model has a sensitivity of 0.83, specificity of 0.87, PPV of 0.41, and NPV of 0.98 to detect SHD. Conclusion: A novel wearable-adapted model demonstrated promising performance for detecting multiple SHD using single-lead ECG. This can enable community screening for SHD and improve patient outcomes through early initiation of guideline-directed therapies.

Abstract:Background: The cumulative burden of hypertrophic cardiomyopathy (HCM) is significant, with a noteworthy percentage (10%-15%) of patients with HCM per year experiencing major adverse cardiovascular events (MACEs). A current risk stratification scheme for HCM had only limited accuracy in predicting sudden cardiac death (SCD) and failed to account for a broader spectrum of adverse cardiovascular events and cardiac magnetic resonance (CMR) parameters. Objectives: This study sought to develop and evaluate a machine learning (ML) framework that integrates CMR imaging and clinical characteristics to predict MACEs in patients with HCM. Methods: A total of 758 patients with HCM (67% male; age 49 ± 14 years) who were admitted between 2010 and 2017 from 4 medical centers were included. The ML model was built on the internal discovery cohort (533 patients with HCM, admitted to Fuwai Hospital, Beijing, China) by using the light gradient-boosting machine and internally evaluated using cross-validation. The external test cohort consisted of 225 patients with HCM from 3 medical centers. A total of 14 CMR imaging features (strain and late gadolinium enhancement [LGE]) and 23 clinical variables were evaluated and used to inform the ML model. MACEs included a composite of arrhythmic events, SCD, heart failure, and atrial fibrillation-related stroke. Results: MACEs occurred in 191 (25%) patients over a median follow-up period of 109.0 months (Q1-Q3: 73.0-118.8 months). Our ML model achieved areas under the curve (AUCs) of 0.830 and 0.812 (internally and externally, respectively). The model outperformed the classic HCM Risk-SCD model, with significant improvement (P < 0.001) of 22.7% in the AUC. Using the cubic spline analysis, the study showed that the extent of LGE and the impairment of global radial strain (GRS) and global circumferential strain (GCS) were nonlinearly correlated with MACEs: an elevated risk of adverse cardiovascular events was observed when these parameters reached the high enough second tertiles (11.6% for LGE, 25.8% for GRS, -17.3% for GCS). Conclusions: ML-empowered risk stratification using CMR and clinical features enabled accurate MACE prediction beyond the classic HCM Risk-SCD model. In addition, the nonlinear correlation between CMR features (LGE and left ventricular pressure gradient) and MACEs uncovered in this study provides valuable insights for the clinical assessment and management of HCM.
