Abstract:Background: Left Ventricular Systolic Dysfunction (LVSD) affects 5% of adults with available therapies, but most are diagnosed after the onset of symptoms. AI-ECG tools that detect LVSD from clinical ECGs are not suitable for community screening. Wearable devices offer broad access to ECGs but end users rely on PDF outputs of single-lead data, with limited access to underlying signal data. Methods: We simulated wearable PDF outputs from commercial wearable devices using lead I data from clinical 12-lead ECGs during 2015-2021. We selected the subset with echocardiography within 15 days of the ECG to define LVSD, based on LVEF < 40%. We developed a model to detect LVSD from wearable-adapted PDF outputs using an EfficientNet-B3 convolutional neural network with self-supervised pretraining. The model deployment pipeline was optimized to segment and process PDF outputs across all commercially available devices as inputs to the model (Fig A & B). Results: 385,601 ECGs with paired echocardiograms were used for model development. In a held-out test set of 11,611 ECGs from unique patients (8.2% with LVEF < 40%) the model demonstrated high discrimination power for detection of LVSD with an AUROC of 0.88, AUPRC of 0.45, sensitivity 0.88, and specificity 0.72. An ECG suggestive of LVSD portended over 18-fold higher odds of LVSD in the held-out set (OR 18.3, 95% CI, 15.0-22.4). It also represents a digital biomarker in those with a false positive vs a true negative prediction at baseline, with a false positive assessment being associated with 3.6-fold (95% CI, 2.97-4.34) increased risk of developing incident LV systolic dysfunction in the future (Fig C). Conclusions: We developed and validated a deep learning model that can identify LVSD from PDF image outputs of wearable and handheld single-lead devices. This approach broadens access to screening for LVSD to anyone with access to a wearable device with ECG.

Abstract:Background: Current electrocardiogram (ECG) criteria of left ventricular hypertrophy (LVH) have low sensitivity. Deep learning (DL) techniques have been widely used to detect cardiac diseases due to its ability of automatic feature extraction of ECG. However, DL was rarely applied in LVH diagnosis. Our study aimed to construct a DL model for rapid and effective detection of LVH using 12-lead ECG. Methods: We built a DL model based on convolutional neural network-long short-term memory (CNN-LSTM) to detect LVH using 12-lead ECG. The echocardiogram and ECG of 1,863 patients obtained within 1 week after hospital admission were analyzed. Patients were evenly allocated into 3 sets at 3:1:1 ratio: the training set ( n = 1,120), the validation set ( n = 371) and the test set 1 ( n = 372). In addition, we recruited 453 hospitalized patients into the internal test set 2. Different DL model of each subgroup was developed according to gender and relative wall thickness (RWT). Results: The LVH was predicted by the CNN-LSTM model with an area under the curve (AUC) of 0.62 (sensitivity 68%, specificity 57%) in the test set 1, which outperformed Cornell voltage criteria (AUC: 0.57, sensitivity 48%, specificity 72%) and Sokolow-Lyon voltage (AUC: 0.51, sensitivity 14%, specificity 96%). In the internal test set 2, the CNN-LSTM model had a stable performance in predicting LVH with an AUC of 0.59 (sensitivity 65%, specificity 57%). In the subgroup analysis, the CNN-LSTM model predicted LVH by 12-lead ECG with an AUC of 0.66 (sensitivity 72%, specificity 60%) for male patients, which performed better than that for female patients (AUC: 0.59, sensitivity 50%, specificity 71%). Conclusion: Our study established a CNN-LSTM model to diagnose LVH by 12-lead ECG with higher sensitivity than current ECG diagnostic criteria. This CNN-LSTM model may be a simple and effective screening tool of LVH.

Abstract:Despite echocardiography being essential for cardiac assessment, a point-of-care tool for rapid left ventricular ejection fraction (LVEF) evaluation is lacking in current clinical practice, highlighting the need for a widescale, easily deployable method to assess LV function. To develop and validate two artificial intelligence (AI) models that accurately detect reduced LVEF on a single 12-lead ECG using a smartphone, facilitating early identification of patients at risk for heart failure who may benefit from more detailed echocardiographic evaluation. A large collection of ECGs and transthoracic echocardiograms (TTEs) was sourced from our hospital data vault from 2011 to 2021. ECGs were paired to TTEs within a 24h time window and the resulting pairs were randomly assigned to the model development dataset (50%) and validation dataset (50%). Two distinct AI-ECG models were developed: one to detect LVEF≤40% and another for LVEF<50%, derived from the ESC definition of heart failure. Both AI-ECG models were paired with smartphone based ECG digitization technology. Key performance metrics include area under the receiver operating characteristic curve (AUC), sensitivity, specificity, positive predictive value (PPV), negative predictive value (NPV), and F1-score. A total of 1,205,370 ECGs (198,274 patients) and 291,433 TTEs (105,849 patients) were collected and paired, resulting in 109,809 ECG-TTE pairs (56,236 patients). The validation dataset consisted of 25,510 distinct TTE-ECG pairs (25,510 patients). Prevalence of LVEF≤40% and LVEF<50% was 5.4% and 7.9% respectively. The LVEF≤40% model demonstrated an AUC of 0.963 (95% CI: 0.959-0.966), sensitivity 0.924 (95% CI: 0.91-0.937), specificity 0.887 (95% CI: 0.883-0.891), and F1-score of 0.474 (95% CI: 0.457-0.490). PPV and NPV were 0.318 (95% CI: 0.304-0.333) and 0.995 (95% CI: 0.994-0.996) respectively. Performance of the LVEF <50% model shows an AUC of 0.952 (95% CI: 0.947-0.956), with a slightly lower sensitivity of 0.899 (95% CI: 0.886-0.912), specificity of 0.875 (95% CI: 0.871-0.879), PPV of 0.382 (95% CI: 0.368-0.395), NPV of 0.99 (95% CI: 0.989-0.992), and F1-score of 0.536 (0.521-0.55).

Pediatric ECG-Based Deep Learning to Predict Left Ventricular Dysfunction and Remodeling

Abstract:BACKGROUND: Artificial intelligence–enhanced ECG analysis shows promise to detect ventricular dysfunction and remodeling in adult populations. However, its application to pediatric populations remains underexplored. METHODS: A convolutional neural network was trained on paired ECG–echocardiograms (≤2 days apart) from patients ≤18 years of age without major congenital heart disease to detect human expert–classified greater than mild left ventricular (LV) dysfunction, hypertrophy, and dilation (individually and as a composite outcome). Model performance was evaluated on single ECG–echocardiogram pairs per patient at Boston Children’s Hospital and externally at Mount Sinai Hospital using area under the receiver operating characteristic curve (AUROC) and area under the precision-recall curve (AUPRC). RESULTS: The training cohort comprised 92 377 ECG–echocardiogram pairs (46 261 patients; median age, 8.2 years). Test groups included internal testing (12 631 patients; median age, 8.8 years; 4.6% composite outcomes), emergency department (2830 patients; median age, 7.7 years; 10.0% composite outcomes), and external validation (5088 patients; median age, 4.3 years; 6.1% composite outcomes) cohorts. Model performance was similar on internal test and emergency department cohorts, with model predictions of LV hypertrophy outperforming the pediatric cardiologist expert benchmark. Adding age and sex to the model added no benefit to model performance. When using quantitative outcome cutoffs, model performance was similar between internal testing (composite outcome: AUROC, 0.88, AUPRC, 0.43; LV dysfunction: AUROC, 0.92, AUPRC, 0.23; LV hypertrophy: AUROC, 0.88, AUPRC, 0.28; LV dilation: AUROC, 0.91, AUPRC, 0.47) and external validation (composite outcome: AUROC, 0.86, AUPRC, 0.39; LV dysfunction: AUROC, 0.94, AUPRC, 0.32; LV hypertrophy: AUROC, 0.84, AUPRC, 0.25; LV dilation: AUROC, 0.87, AUPRC, 0.33), with composite outcome negative predictive values of 99.0% and 99.2%, respectively. Saliency mapping highlighted ECG components that influenced model predictions (precordial QRS complexes for all outcomes; T waves for LV dysfunction). High-risk ECG features include lateral T-wave inversion (LV dysfunction), deep S waves in V1 and V2 and tall R waves in V5 and V6 (LV hypertrophy), and tall R waves in V4 through V6 (LV dilation). CONCLUSIONS: This externally validated algorithm shows promise to inexpensively screen for LV dysfunction and remodeling in children, which may facilitate improved access to care by democratizing the expertise of pediatric cardiologists.

Abstract:Cancer of the endometrium is a neoplasm which essentially strikes postmenopausal women and in which the predominant risk factors are recognized as an excessive imbalance of estrogen and obesity. The survival rate is good in the first stage and is acceptable in the second, while in the third and especially the fourth stage is unfavorable. To verify and evaluate such facts we have carried out a study of survival rates on patients admitted in our Institute surgery division from 1987 to 1992. The results confirm the importance of the stage and histologic grading of the determining variables, while there appears to be more controversy regarding the role of myometrial invasion. In conclusion, our data show that intervention by pelvic-aortic lymphadenectomy contributes to an improvement in survival rates independently of stage.

Abstract:Artificial intelligence (AI) can detect left ventricular systolic dysfunction (LVSD) from electrocardiograms (ECGs). Wearable devices could allow for broad AI-based screening but frequently obtain noisy ECGs. We report a novel strategy that automates the detection of hidden cardiovascular diseases, such as LVSD, adapted for noisy single-lead ECGs obtained on wearable and portable devices. Overall, 385,601 ECGs were used for development of a standard and noise-adapted model. For the noise-adapted model, ECGs were augmented during training with random gaussian noise within four distinct frequency ranges, each emulating real-world noise sources. Both models performed comparably on clean ECGs with an AUROC of 0.90. The noise-adapted model performed significantly better on the same test set augmented with four distinct real-world noise recordings at multiple signal-to-noise ratios (SNRs), including noise isolated from a portable device ECG. The standard and noise-adapted models had an AUROC of 0.72 and 0.87, respectively when evaluated on ECGs augmented with portable ECG device noise at an SNR of 0.5. This approach represents a novel strategy for the development of wearable adapted tools from clinical ECG repositories.

Abstract:Background and Objective Left ventricular hypertrophy (LVH) can impair ejection function and elevate the risk of heart failure. Therefore, early detection through screening is crucial. This study aimed to propose a novel method to enhance LVH detection using 12-lead electrocardiogram (ECG) waveforms with a two-dimensional (2D) convolutional neural network (CNN). Methods Utilizing 42,127 pairs of ECG-transthoracic echocardiogram data, we pre-processed raw data into single-shot images derived from each ECG lead and conducted lead selection to optimize LVH diagnosis. Our proposed one-shot screening method, implemented during pre-processing, enables the superimposition of waveform source data of any length onto a single-frame image, thereby addressing the limitations of the one-dimensional (1D) approach. We developed a deep learning model with a 2D-CNN structure and machine learning models for LVH detection. To assess our method, we also compared our results with conventional ECG criteria and those of a prior study that used a 1D-CNN approach, utilizing the same dataset from the University of Tokyo Hospital for LVH diagnosis. Results For LVH detection, the average area under the receiver operating characteristic curve (AUROC) was 0.916 for the 2D-CNN model, which was significantly higher than that obtained using logistic regression and random forest methods, as well as the two conventional ECG criteria (AUROC of 0.766, 0.790, 0.599, and 0.622, respectively). Incorporating additional metadata, such as ECG measurement data, further improved the average AUROC to 0.921. The model's performance remained stable across two different annotation criteria and demonstrated significant superiority over the performance of the 1D-CNN model used in a previous study (AUROC of 0.807). Conclusions This study introduces a robust and computationally efficient method that outperforms 1D-CNN models utilized in previous studies for LVH detection. Our method can transform waveforms of any length into fixed-size images and leverage the selected lead of the ECG, ensuring adaptability in environments with limited computational resources. The proposed method holds promise for integration into clinical practice as a tool for early diagnosis, potentially enhancing patient outcomes by facilitating earlier treatment and management.

[Feasibility of behavioural interventions for tobacco smoking cessation in routine general practice].

Abstract:The aims of this study were: 1) to evaluate a training course on smoking cessation techniques offered to general practitioners of the RMB Local Health Unit (Rome, Italy) 2) to evaluate the feasibility of delivering brief counseling interventions during routine primary care practice and 3) to evaluate factors associated with modification of smoking habits. In total, 166 GPs participated in the training courses. Of these 60 GPs (36.1%) voluntarily agreed to take part in the study. A total of 3280 patients were enrolled and given a brief counseling intervention ("ask-advise"). Six months later the same patients were asked to complete a questionnaire, 24% responded. Approximately 24% of those who responded had quit smoking and over 43% had either reduced the number of cigarettes or attempted to quit. Of the subjects who modified their smoking behaviour (quit, attempted to quit, or reduced the numbers of cigarettes smoked) following the "ask-advise" intervention, 10% had also used drug therapy, while approximately 4% did so only with their doctor's assistance. Smoking cessation interventions incorporated into routine consultations in general practice are feasible and relevant from a public health perspective.

Abstract:Background: Left ventricular systolic dysfunction (LVSD) characterized by a reduced left ventricular ejection fraction (LVEF) is associated with adverse patient outcomes. We aimed to build a deep neural network (DNN)-based model using standard 12-lead electrocardiogram (ECG) to screen for LVSD and stratify patient prognosis. Methods: This retrospective chart review study was conducted using data from consecutive adults who underwent ECG examinations at Chang Gung Memorial Hospital in Taiwan between October 2007 and December 2019. DNN models were developed to recognize LVSD, defined as LVEF <40%, using original ECG signals or transformed images from 190,359 patients with paired ECG and echocardiogram within 14 days. The 190,359 patients were divided into a training set of 133,225 and a validation set of 57,134. The accuracy of recognizing LVSD and subsequent mortality predictions were tested using ECGs from 190,316 patients with paired data. Of these 190,316 patients, we further selected 49,564 patients with multiple echocardiographic data to predict LVSD incidence. We additionally used data from 1,194,982 patients who underwent ECG only to assess mortality prognostication. External validation was performed using data of 91,425 patients from Tri-Service General Hospital, Taiwan. Results: The mean age of patients in the testing dataset was 63.7 ± 16.3 years (46.3% women), and 8,216 patients (4.3%) had LVSD. The median follow-up period was 3.9 years (interquartile range 1.5–7.9 years). The area under the receiver-operating characteristic curve (AUROC), sensitivity, and specificity of the signal-based DNN (DNN-signal) to identify LVSD were 0.95, 0.91, and 0.86, respectively. DNN signal-predicted LVSD was associated with age- and sex-adjusted hazard ratios (HRs) of 2.57 (95% confidence interval [CI], 2.53–2.62) for all-cause mortality and 6.09 (5.83–6.37) for cardiovascular mortality. In patients with multiple echocardiograms, a positive DNN prediction in patients with preserved LVEF was associated with an adjusted HR (95% CI) of 8.33 (7.71 to 9.00) for incident LVSD. Signal- and image-based DNNs performed equally well in the primary and additional datasets. Conclusion: Using DNNs, ECG becomes a low-cost, clinically feasible tool to screen LVSD and facilitate accurate prognostication.

Abstract:Background Patients with hypertrophic cardiomyopathy (HCM) are at risk for sudden death and individuals with ≥1 major risk markers are considered for primary prevention implantable cardioverter defibrillators. Guidelines recommend cardiac magnetic resonance imaging (CMR) to identify high-risk imaging features. However, CMR is resource intensive and is not widely accessible worldwide. Objective To develop electrocardiogram (ECG) deep-learning (DL) models for identification of HCM patients with high-risk features. Methods HCM patients evaluated at Tufts Medical Center (N=1,930; Boston, United States) were used to develop ECG-DL models for prediction of high-risk imaging features: systolic dysfunction, massive hypertrophy (≥30mm), apical aneurysm, and extensive late-gadolinium enhancement (LGE). ECG-DL models were externally validated in an HCM cohort from Amrita Hospital (N=233; Kochi, India). Results ECG-DL models reliably identified high-risk features (systolic dysfunction, massive hypertrophy, apical aneurysm, and extensive LGE) during hold-out model testing (c-statistics 0.72, 0.83, 0.93, and 0.76) and external validation (c-statistics 0.71, 0.76, 0.91, and 0.68). A hypothetical screening strategy employing echocardiography combined with ECG-DL guided selective CMR use demonstrated sensitivity of 97% for identifying patients with high-risk features, while reducing the number of recommended CMRs by 61%. Negative predictive value with this screening strategy for absence of high-risk features in patients without ECG-DL recommendation for CMR was 99.5%. Conclusions In HCM, novel ECG-DL models reliably identified patients with high-risk imaging features while offering the potential to reduce CMR testing requirements in under-resourced areas.

Abstract:Heart failure (HF) is an escalating global health concern, worsened by an aging population and limitations in traditional diagnostic methods like electrocardiograms (ECG). The advent of deep learning has shown promise for utilizing 12-lead ECG models for the early detection of left ventricular systolic dysfunction (LVSD), a crucial HF indicator. This study validates the AiTiALVSD, an AI/machine learning-enabled Software as a Medical Device, for its effectiveness, transparency, and robustness in detecting LVSD. Conducted at Mediplex Sejong Hospital in the Republic of Korea, this retrospective single-center cohort study involved patients suspected of LVSD. The AiTiALVSD model, which is based on a deep learning algorithm, was assessed against echocardiography findings. To improve model transparency, the study utilized Testing with Concept Activation Vectors (TCAV) and included clustering analysis and robustness tests against ECG noise and lead reversals. The study involved 688 participants and found AiTiALVSD to have a high diagnostic performance, with an AUROC of 0.919. There was a significant correlation between AiTiALVSD scores and left ventricular ejection fraction values, confirming the model's predictive accuracy. TCAV analysis showed the model's alignment with medical knowledge, establishing its clinical plausibility. Despite its robustness to ECG artifacts, there was a noted decrease in specificity in the presence of ECG noise. AiTiALVSD's high diagnostic accuracy, transparency, and resilience to common ECG discrepancies underscore its potential for early LVSD detection in clinical settings. This study highlights the importance of transparency and robustness in AI/ML-based diagnostics, setting a new benchmark in cardiac care.

Abstract:Background: We previously developed and validated an AI-based ECG analysis tool (ECG Buddy) in a Korean population. This study aims to validate its performance in a U.S. population, specifically assessing its LV Dysfunction Score and LVEF-ECG feature for predicting LVEF <40%, using NT-ProBNP as a comparator. Methods: We identified emergency department (ED) visits from the MIMIC-IV dataset with information on LVEF <40% or ≥40%, along with matched 12-lead ECG data recorded within 48 hours of the ED visit. The performance of ECG Buddy's LV Dysfunction Score and LVEF-ECG feature was compared with NT-ProBNP using Receiver Operating Characteristic - Area Under the Curve (ROC-AUC) analysis. Results: A total of 22,599 ED visits were analyzed. The LV Dysfunction Score had an AUC of 0.905 (95% CI: 0.899 - 0.910), with a sensitivity of 85.4% and specificity of 80.8%. The LVEF-ECG feature had an AUC of 0.908 (95% CI: 0.902 - 0.913), sensitivity 83.5%, and specificity 83.0%. NT-ProBNP had an AUC of 0.740 (95% CI: 0.727 - 0.752), with a sensitivity of 74.8% and specificity of 62.0%. The ECG-based predictors demonstrated superior diagnostic performance compared to NT-ProBNP (all p<0.001). In the Sinus Rhythm subgroup, the LV Dysfunction Score achieved an AUC of 0.913, and LVEF-ECG had an AUC of 0.917, both outperforming NT-ProBNP (0.748, 95% CI: 0.732 - 0.763, all p<0.001). Conclusion: ECG Buddy demonstrated superior accuracy compared to NT-ProBNP in predicting LV systolic dysfunction, validating its utility in a U.S. ED population.
Deep Learning-based 12-Lead Electrocardiogram for Low Left Ventricular Ejection Fraction Detection in Patients

Abstract:Background: Reduced left ventricular ejection fraction (LVEF) initiates heart failure, and promptly identifying low ejection fraction is crucial for managing progression and averting mortality. In this study we developed an artificial intelligence-enabled electrocardiogram (AI-ECG) algorithm to identify patients with low ejection fraction and predict LVEF values. Methods: The electrocardiogram data were used as input, and the algorithm generated the probability of the patient suffering a low ejection fraction and estimated the LVEF value. A 5-year follow-up study on a group of individuals who initially had normal LVEF values was also performed. Furthermore, external validation of the algorithm performance was conducted using the Medical Information Mart for Intensive Care-IV database. Results: The algorithm's performance on the test set yielded an area under the curve value of 0.965 for detecting LVEF ≤ 50%. The algorithm had an accuracy of 92.8%, sensitivity of 88.8%, and specificity of 92.9%. For LVEF regression, the method showed a mean absolute error of 5.28 (95% confidence interval, 5.23-5.33) for the testing set. Additionally, the algorithm obtained an area under the curve value of 0.848 and a mean absolute error value of 9.56 during external validation. Patients with false positive results had a significantly greater likelihood of developing a low ejection fraction compared with patients who received true negative results (26.2% vs 2.0%; P < 0.0001). Conclusions: The AI-ECG algorithm is capable of identifying low ejection fraction in patients with high accuracy. The AI-ECG algorithm is an efficient, prompt, and cost-effective screening tool for early heart failure.
An Ensemble Deep Learning Algorithm for Structural Heart Disease Screening Using Electrocardiographic Images: PRESENT SHD

Abstract:Background: Identifying structural heart diseases (SHDs) early can change the course of the disease, but their diagnosis requires cardiac imaging, which is limited in accessibility. Objective: To leverage 12-lead ECG images for automated detection and prediction of multiple SHDs using a novel deep learning model. Methods: We developed a series of convolutional neural network models for detecting a range of individual SHDs from images of ECGs with SHDs defined by transthoracic echocardiograms (TTEs) performed within 30 days of the ECG at the Yale New Haven Hospital (YNHH). SHDs were defined based on TTEs with LV ejection fraction <40%, moderate-to-severe left-sided valvular disease (aortic/mitral stenosis or regurgitation), or severe left ventricular hypertrophy (IVSd > 1.5cm and diastolic dysfunction). We developed an ensemble XGBoost model, PRESENT-SHD, as a composite screen across all SHDs. We validated PRESENT-SHD at 4 US hospitals and a prospective population-based cohort study, the Brazilian Longitudinal Study of Adult Health (ELSA-Brasil), with concurrent protocolized ECGs and TTEs. We also used PRESENT-SHD for risk stratification of new-onset SHD or heart failure (HF) in clinical cohorts and the population-based UK Biobank (UKB). Results: The models were developed using 261,228 ECGs from 93,693 YNHH patients and evaluated on a single ECG from 11,023 individuals at YNHH (19% with SHD), 44,591 across external hospitals (20-27% with SHD), and 3,014 in the ELSA-Brasil (3% with SHD). In the held-out test set, PRESENT-SHD demonstrated an AUROC of 0.886 (0.877-894), sensitivity of 89%, and specificity of 66%. At hospital-based sites, PRESENT-SHD had AUROCs ranging from 0.854-0.900, with sensitivities and specificities of 93-96% and 51-56%, respectively. The model generalized well to ELSA-Brasil (AUROC, 0.853 [0.811-0.897], sensitivity 88%, specificity 62%). PRESENT-SHD performance was consistent across demographic subgroups. A positive PRESENT-SHD screen portended a 2- to 4-fold higher risk of new-onset SHD/HF, independent of demographics, comorbidities, and the competing risk of death across clinical sites and UKB, with high predictive discrimination. Conclusion: We developed and validated PRESENT-SHD, an AI-ECG tool identifying a range of SHD using images of 12-lead ECGs, representing a robust, scalable, and accessible modality for automated SHD screening and risk stratification.

Abstract:Background: Wearable and portable devices with ECG capabilities have the potential to enable broad AI-based screening for clinical disorders, but models trained on single-lead signals are limited by challenges accessing the raw signal data from proprietary systems. Additionally, wearable ECGs have traditionally been limited to diagnosing only a few rhythm disorders. Methods: From 385,601 clinical ECGs performed at Yale during 2015-2021, we extracted lead I data, and simulated wearable PDF outputs from commercial wearable devices. Gold-standard diagnosis statements from cardiologists accompanying these ECGs were processed and used to generate 41 clinical labels. We developed a multilabel model to predict these labels from wearable-adapted PDF outputs using an EfficientNet-B3 convolutional neural network with self-supervised pretraining. The model deployment pipeline was optimized to segment and process PDF outputs across all commercially available devices as inputs to the model (Fig A & B). Results: In a held-out test set of 11,611 ECGs, the model accurately detected a diverse set of disorders. For the rhythm disorders sinus tachycardia, sinus bradycardia, atrial fibrillation, atrial flutter, premature atrial contraction, and premature ventricular contraction, the model had AUROCs of 0.99, 0.98, 0.98, 0.93, 0.96 and 0.95 respectively. For the conduction disorders left bundle branch blocks, right bundle branch blocks, 1 st and 2 nd degree AV blocks, the model had AUROCs of 0.98, 0.97, 0.96 and 0.95, respectively. Finally, for key disorders such as STEMI, LVH, and low limb-lead voltage, the model had AUROCs of 0.81, 0.86, and 0.77, respectively (Fig C). Conclusions: We developed and validated a deep learning model to identify conduction disorders and arrhythmias from PDF image outputs of wearable and handheld single-lead devices. This approach allows for automated and easily accessible insights into wearable and portable ECGs at the point of care.

Abstract:Background Artificial intelligence–enhanced electrocardiogram (AI-ECG) analysis shows promise to detect biventricular pathophysiology. However, AI-ECG analysis remains underexplored in congenital heart disease (CHD). Objectives The purpose of this study was to develop and externally validate an AI-ECG model to predict cardiovascular magnetic resonance (CMR)-defined biventricular dysfunction/dilation in patients with CHD. Methods We trained (80%) and tested (20%) a convolutional neural network on paired ECG-CMRs (≤30 days apart) from patients with and without CHD to detect left ventricular (LV) dysfunction (ejection fraction ≤40%), RV dysfunction (ejection fraction ≤35%), and LV and RV dilation (end-diastolic volume z-score ≥4). Performance was assessed during internal testing and external validation on an outside health care system using area under receiver-operating curve (AUROC) and area under precision recall curve. Results The internal and external cohorts comprised 8,584 ECG-CMR pairs (n = 4,941; median CMR age 20.7 years) and 909 ECG-CMR pairs (n = 746; median CMR age 25.4 years), respectively. Model performance was similar for internal testing (AUROC: LV dysfunction 0.87; LV dilation 0.86; RV dysfunction 0.88; RV dilation 0.81) and external validation (AUROC: LV dysfunction 0.89; LV dilation 0.83; RV dysfunction 0.82; RV dilation 0.80). Model performance was lowest in functionally single ventricle patients. Tetralogy of Fallot patients predicted to be at high risk of ventricular dysfunction had lower survival ( P < 0.001). Model explainability via saliency mapping revealed that lateral precordial leads influence all outcome predictions, with high-risk features including QRS widening and T-wave inversions for RV dysfunction/dilation. Conclusions AI-ECG shows promise to predict biventricular dysfunction/dilation, which may help inform CMR timing in CHD.

Abstract:Introduction The growing demand on European echocardiography labs, driven by the rise in structural heart disease (SHD), is further complicated by a shortage of skilled sonographers, necessitating resource optimisation strategies. Electrocardiogram (ECG)-based deep learning models may assist in streamlining the echocardiography workflow. Purpose This study sought to develop a deep learning model capable of identifying patients at very low risk of SHD in a population of de novo patients referred to the cardiology outpatient clinic. Methods We identified all adult patients who underwent a 12-lead ECG and echocardiogram at a Dutch academic teaching hospital between 2011 and August 2023. Echocardiogram outcomes were extracted from reports, including left ventricular systolic dysfunction (LVSD), left ventricular dilatation (LVD), aortic stenosis (AS), aortic regurgitation (AR), mitral regurgitation (MR), and tricuspid regurgitation (TR), from which a composite outcome label was derived. A cohort of patients newly referred to our cardiology outpatient clinic with a primary complaint of chest pain, dyspnea, palpitations, dizziness, a murmur, or an abnormal ECG was held out as a test set, while the remaining dataset was split patient-wise into training (90%) and validation (10%) sets. A convolutional neural network was trained using only the ECG tracing as input and the composite label as outcome. Results We included 92,210 ECG-echo pairs from 44,785 patients (19,717 female) with 25,543 (27.7%) abnormal ECG-echo pairs. Among 2724 newly referred patients in the test set, 210/1389 males (15.1%) and 201/1335 females (15.1%) were diagnosed with SHD. In the test set, the model for predicting composite outcome demonstrated an area-under-the-curve, negative predictive value, and specificity of 0.74 (95% confidence interval [CI] 0.71-0.79), 0.94 (95% CI 0.92-0.95), and 0.47 (95% CI 0.45-0.49), respectively. Conclusion This study demonstrates the feasibility of employing deep learning models based solely on electrocardiogram (ECG) data to identify patients at very low risk of structural heart disease (SHD). Further validation and prospective studies are warranted to confirm the generalizability and clinical utility of our findings in diverse patient populations and healthcare settings.

Abstract:Introduction: Left ventricular systolic dysfunction (LVSD) is associated with poor health outcomes. Previous study has demonstrated the effectiveness of electromechanical activation time (EMAT) derived from phonocardiography (PCG) and electrocardiography (ECG) signals in identifying LVSD. However, high throughput and well-performed algorithm is needed in automatically detecting LVSD. Hypothesis: An artificial intelligence (AI) algorithm was developed and validated in detecting LVSD. Methods: Using ECG and PCG data collected by wearable patch from 1020 admitted patients in Ruijin hospital, we trained an AI algorithm to detect the LVSD, which was determined by ejection fraction <50% from echocardiogram. A separate 590 patients were taken as independent test set. The AI algorithm followed a two-step detection structure, where the first step is to use LSTM learning to quantify heartbeat and heartbeat interval in ECG, and the second step is to apply convolution neural network (CNN) in estimating optimal EMAT cutoff points in detecting LVSD. To overcome the challenge of multi-modal signal training, we develop an alignment-based contrastive learning approach that generates shared feature space embeddings for ECG and PCG signals. Results: Among training and test sets, 18.2% and 26.1% were LVSD, respectively. The AI algorithm yields an AUROC of 0.92 in detecting LVSD with sensitivity of 85.63%, and specificity of 79.39%. Among the test set, model yielded an AUROC of 0.86, sensitivity of 81.29%, and specificity of 77.13%. Conclusions: A wearable device incorporated with a high performed AI algorithm can be a viable, convenient, cost-effective tool in screening LVSD.
