Abstract:Individuals with transtibial amputation can activate residual limb muscles to volitionally control robotic ankle prostheses for walking and postural control. Most continuous myoelectric ankle prostheses have used a tethered, pneumatic device. The Open Source Leg allows for myoelectric control on an untethered electromechanically actuated ankle. To evaluate continuous proportional myoelectric control on the Open Source Ankle, we recruited five individuals with transtibial amputation. Participants walked over ground with an experimental powered prosthesis and their prescribed passive prosthesis before and after multiple powered device practice sessions. Participants averaged five hours of total walking time. After the final testing session, participants indicated their prosthesis preference via questionnaire. Participants tended to increase peak ankle power after practice (powered W/kg and passive W/kg). Additionally, participants tended to generate greater ankle work with the powered prosthesis compared to their passive device ( .15 J/kg increase). Although work and peak power generation were not statistically different between the two prostheses, participants preferred walking with the prosthesis under myoelectric control compared to the passive prosthesis. These results indicate individuals with transtibial amputation learned to walk with an untethered powered prosthesis under continuous myoelectric control. Four out 5 participants generated larger magnitudes in peak power compared to their passive prosthesis after practice sessions. An additional important finding was participants chose to walk with peak ankle powers about half of what the powered prosthesis was capable of based on mechanical testing.

Abstract:Powered ankle-foot prostheses can often reduce the energy cost of walking by assisting with push-off. However, focus on providing mechanical work may lead to ignoring or exacerbating common issues with chronic pain, irritation, pressure ulcer development, and eventual osteoarthritis in persons with amputation. This paper presents the design and validation of a novel transtibial prosthesis informed by predictive biomechanical simulations of gait which minimize a combination of user effort and interaction loading from the prosthesis socket. From these findings, the device was designed with a non-biomimetic anterior-posterior translation degree of freedom with a 10 cm range of motion which is primarily position-controlled to change the alignment of the prosthetic foot with the residual limb. The system is both mobile and tethered, with the batteries, actuators, and majority of electronics located in a small backpack. Mechanical loads are transmitted through cables to the prosthesis, minimizing the distal mass carriage required. We measured torque and force sensing accuracy, open loop actuator performance, closed loop torque and position control bandwidth, and torque and position tracking error during walking. The system is capable of producing up to 160 N-m of plantarflexion torque and 394 N of AP translation force with a closed loop control bandwidth of about 7 Hz in both degrees of freedom. Torque tracking during walking was accurate within about 10 N-m but position tracking was substantially affected by phase lag, possibly due to cable slack in the bidirectional mechanism. The prototype was capable of replicating our simulated prosthesis dynamics during gait and offers useful insights into the advantages and the practical considerations of using predictive biomechanical simulation as a design tool for wearable robots.

Abstract:Transfemoral amputation is a debilitating condition that leads to long-term mobility restriction and secondary disorders that negatively affect the quality of life of millions of individuals worldwide. Currently available prostheses are not able to restore energetically efficient and functional gait, thus, recently, the alternative strategy to inject energy at the residual hip has been proposed to compensate for the lack of energy of the missing leg. Here, we show that a portable and powered hip exoskeleton assisting both the residual and intact limb induced a reduction of walking energy expenditure in four individuals with above-knee amputation. The reduction of the energy expenditure, quantified using the Physiological Cost Index, was in the range [-10, -17]% for all study participants compared to walking without assistance, and between [-2, -24]% in three out of four study participants compared to walking without the device. Additionally, all study participants were able to walk comfortably and confidently with the hip exoskeleton overground at both their self-selected comfortable and fast speed without any observable alterations in gait stability. The study findings confirm that injecting energy at the hip level is a promising approach for individuals with above-knee amputation. By reducing the energy expenditure of walking and facilitating gait, a hip exoskeleton may extend mobility and improve locomotor training of individuals with above-knee amputation, with several positive implications for their quality of life.

Abstract:A novel powered ankle-foot prosthesis is designed. The effect of wearing the novel prosthesis and an energy-storage-and-return (ESAR) foot on lower-limb biomechanics is investigated to preliminarily evaluate the design. With necessary auxiliary materials, a non-amputated subject (a rookie at using prostheses) is recruited to walk on level ground with an ESAR and the novel powered prostheses separately. The results of the stride characteristics, the ground reaction force (GRF) components, kinematics, and kinetics in the sagittal plane are compared. Wearing the powered prosthesis has less prolongation of the gait cycle on the unaffected side than wearing the ESAR foot. Wearing ESAR or proposed powered prostheses influences the GRF, kinematics, and kinetics on the affected and unaffected sides to some extent. Thereinto, the knee moment on the affected side is influenced most. Regarding normal walking as the reference, among the total of 15 indexes, the influences of wearing the proposed powered prosthesis on six indexes on the affected side (ankle's/knee's/hip's angles, hip's moment, and Z- and X-axis GRF components) and five indexes on the unaffected side (ankle's/knee's/hip's angles and ankle's/hip's moments) are slighter than those of wearing the ESAR foot. The influences of wearing the powered prosthesis on two indexes on the unaffected side (knee's moment and X-axis GRF component) are similar to those of wearing the ESAR foot. The greatest improvement of wearing the powered prosthesis is to provide further plantarflexion after reaching the origin of the ankle joint before toe-off, which means that the designed powered device can provide further propulsive power for the lifting of the human body's centre of gravity during walking on level ground. The results demonstrate that wearing the novel powered ankle-foot prosthesis benefits the rookie in recovering the normal gait more than wearing the ESAR foot.

Abstract:Over the course of both evolution and development, the human musculoskeletal system has been well shaped for the cushion function of the foot during foot-strike and the impulsive function of the ankle joint during push-off. Nevertheless, an efficient energy interaction between foot structure and ankle joint is still lacking in the human body itself, which may limit the further potential of economical walking. Here we showed the metabolic expenditure of walking can be lessened by an unpowered exoskeleton robot that modulates energy among the foot-ankle complex towards a more effective direction. The unpowered exoskeleton recycles negative mechanical energy of the foot that is normally dissipated in heel-strike, retains the stored energy before mid-stance, and then transfers the energy to the ankle joint to assist the push-off. The modulation process of the exoskeleton consumes no input energy, yet reduces the metabolic cost of walking by % (mean ± s.e.m) for healthy subjects. The electromyography measurements demonstrate the activities of target ankle plantarflexors decreased significantly without added effort for the antagonistic muscle, suggesting the exoskeleton enhanced the subjects' energy efficiency of the foot-ankle complex in a natural manner. Furthermore, the exoskeleton also provides cushion assistance for walking, which leads to significantly decreased activity of the quadriceps muscle during heel-strike. Rather than strengthening the functions of existing biological structures, developing the complementary energy loop that does not exist in the human body itself also shows its potential for gait assistance.

Abstract:Dichloro- and trichloroacetic acids (DCA and TCA) and chloroform are formed during chlorination disinfection of drinking water. The effects of DCA and TCA treatment on CHCl3 toxicity were assessed in these studies. Male and female rats were gavaged with DCA or TCA (0.92 and 2.45 mmol/kg administered 3 times over 24 h). Three hours after the last dose CHCl3 was injected ip (0.75 mg/kg). Male rats experienced some weight loss (15%) and slight increases of ALT and BUN, but there were no effects of either DCA or TCA on any of these responses. In females, CHCl3 increased plasma ALT and this response was greater (up to threefold) in the DCA group, compared to saline controls. Similarly, BUN was increased by CHCl3 and this was more severe (up to threefold) in both the DCA and TCA pretreated groups. These results show that CHCl3 toxicity is increased by DCA and TCA, and this effect is gender-specific, occurring only in females. DCA increases both liver and kidney toxicity, whereas TCA affects only kidney toxicity.

Abstract:After above-knee amputation, the missing biological knee and ankle are replaced with passive prosthetic devices. Passive prostheses are able to dissipate limited amounts of energy using resistive damper systems during "negative energy" tasks like sit-down. However, passive prosthetic knees are not able to provide high levels of resistance at the end of the sit-down movement when the knee is flexed, and users need the most support. Consequently, users are forced to over-compensate with their upper body, residual hip, and intact leg, and/or sit down with a ballistic and uncontrolled movement. Powered prostheses have the potential to solve this problem. Powered prosthetic joints are controlled by motors, which can produce higher levels of resistance at a larger range of joint positions than passive damper systems. Therefore, powered prostheses have the potential to make sitting down more controlled and less difficult for above-knee amputees, improving their functional mobility.

Abstract:Legged locomotion in humans is governed by natural dynamics of the human body and neural control. One mechanism that is assumed to contribute to the high efficiency of human walking is the impulsive ankle push-off, which potentially powers the swing leg catapult. However, the mechanics of the human lower leg with its complex muscle-tendon units spanning over single and multiple joints is not yet understood. Legged robots allow testing the interaction between complex leg mechanics, control, and environment in real-world walking gait. We developed a 0.49m tall, 2.2kg anthropomorphic bipedal robot with Soleus and Gastrocnemius muscle-tendon units represented by linear springs, acting as mono- and biarticular elastic structures around the robot's ankle and knee joints. We tested the influence of three Soleus and Gastrocnemius spring-tendon configurations on the ankle power curves, the coordination of the ankle and knee joint movements, the total cost of transport, and walking speed. We controlled the robot with a feed-forward central pattern generator, leading to walking speeds between 0.35m/s and 0.57m/s at 1.0Hz locomotion frequency, at 0.35m leg length. We found differences between all three configurations; the Soleus spring-tendon modulates the robot's speed and energy efficiency likely by ankle power amplification, while the Gastrocnemius spring-tendon changes the movement coordination between ankle and knee joints during push-off.

Abstract:Powered prostheses aim to mimic the missing biological limb with controllers that are finely tuned to replicate the nominal gait pattern of non-amputee individuals. Unfortunately, this control approach poses a problem with real-world ambulation, which includes tasks such as crossing over obstacles, where the prosthesis trajectory must be modified to provide adequate foot clearance and ensure timely foot placement. Here, we show an indirect volitional control approach that enables prosthesis users to walk at different speeds while smoothly and continuously crossing over obstacles of different sizes without explicit classification of the environment. At the high level, the proposed controller relies on a heuristic algorithm to continuously change the maximum knee flexion angle and the swing duration in harmony with the user's residual limb. At the low level, minimum-jerk planning is used to continuously adapt the swing trajectory while maximizing smoothness. Experiments with three individuals with above-knee amputation show that the proposed control approach allows for volitional control of foot clearance, which is necessary to negotiate environmental barriers. Our study suggests that a powered prosthesis controller with intrinsic, volitional adaptability may provide prosthesis users with functionality that is not currently available, facilitating real-world ambulation.

Abstract:Stroke disease leads to a partial or complete disability affecting muscle strength and functional mobility. Early rehabilitation sessions might induce neuroplasticity and restore the affected function or structure of the patients. Robotic rehabilitation minimizes the burden on therapists by providing repetitive and regularly monitored therapies. Commercial exoskeletons have been found to assist hip and knee motion. For instance, unilateral exoskeletons have the potential to become an effective training system for patients with hemiparesis. However, these robotic devices leave the ankle joint unassisted, essential in gait for body propulsion and weight-bearing. This article evaluates the effects of the robotic ankle orthosis T-FLEX during cooperative assistance with the AGoRA unilateral lower-limb exoskeleton (hip and knee actuation). This study involves nine subjects, measuring muscle activity and gait parameters such as stance and swing times. The results showed a reduction in muscle activity in the Biceps Femoris of 50%, Lateral Gastrocnemius of 59% and Tibialis Anterior of 35% when adding T-FLEX to the AGoRA unilateral lower-limb exoskeleton. No differences were found in gait parameters. Nevertheless, stability is preserved when comparing the two legs. Future works should focus on evaluating the devices in ground tests in healthy subjects and pathological patients.
Effects of toe stiffness on ankle kinetics in a robotic transtibial prosthesis during level-ground walking

Abstract:Robotic transtibial prostheses are gaining popularity, as they significantly improve amputees’ locomotion. Existing studies mainly concentrate on the importance of ankle joints in these prostheses. However, there are few studies that focus on the effects of prosthetic toe joints. Different from human feet, most prosthetic feet are single-segment structures without toe joints. In order to develop more human-like prostheses with ankle and toe joints, it is of great importance to investigate the effects of toe stiffness on ankle kinetics, as it contributes to the design and control optimizations. In this paper, we analyze the effects of toe stiffness on ankle kinetics in a robotic transtibial prosthesis we have built recently. Dynamic models of the prosthesis’ ankle and toe joints are built. The imposed plantar pressures and objective joint angles of these models are obtained from locomotion experiments on able-bodied subjects. Simulation results indicate when the toe joint is fixed, the ankle energy consumption in one gait cycle is almost 1.2 times of that when the toe joint is driven to track the able-bodied toe motion. Experiments of the prosthesis prototype are carried out on an amputee subject with a transtibial amputation. The results show that compared with the fixed toe joint either the active toe joint or the passive toe joint can reduce the ankle output moment and power consumption. This means adding a toe joint to the foot can reduce the designing requirements of the ankle joint. In addition, the active toe joint can help the prosthesis perform more human-like behaviors compared with the passive toe joint. On the other hand, the passive toe joint can simplify the prosthesis structures and reduce the total weight compared with the active toe joint.
Comparing optimized exoskeleton assistance of the hip, knee, and ankle in single and multi-joint configurations

Abstract:Abstract Exoskeletons that assist the hip, knee, and ankle joints have begun to improve human mobility, particularly by reducing the metabolic cost of walking. However, direct comparisons of optimal assistance of these joints, or their combinations, have not yet been possible. Assisting multiple joints may be more beneficial than the sum of individual effects, because muscles often span multiple joints, or less effective, because single-joint assistance can indirectly aid other joints. In this study, we used a hip–knee–ankle exoskeleton emulator paired with human-in-the-loop optimization to find single-joint, two-joint, and whole-leg assistance that maximally reduced the metabolic cost of walking. Hip-only and ankle-only assistance reduced the metabolic cost of walking by 26 and 30% relative to walking in the device unassisted, confirming that both joints are good targets for assistance ( N = 3). Knee-only assistance reduced the metabolic cost of walking by 13%, demonstrating that effective knee assistance is possible ( N = 3). Two-joint assistance reduced the metabolic cost of walking by between 33 and 42%, with the largest improvements coming from hip-ankle assistance ( N = 3). Assisting all three joints reduced the metabolic cost of walking by 50%, showing that at least half of the metabolic energy expended during walking can be saved through exoskeleton assistance ( N = 4). Changes in kinematics and muscle activity indicate that single-joint assistance indirectly assisted muscles at other joints, such that the improvement from whole-leg assistance was smaller than the sum of its single-joint parts. Exoskeletons can assist the entire limb for maximum effect, but a single well-chosen joint can be more efficient when considering additional factors such as weight and cost.

Abstract:Powered knee prostheses promise to improve the mobility of transfemoral amputees by imitating the biomechanics of the missing knee joint. Unfortunately, the heavy weight and short battery life severely limit the application of powered prostheses. Here, we present a lightweight powered knee prosthesis, which employs a series elastic actuator (SEA) integrating a high torque density external rotor motor, a ball screw and a slider-crank mechanism. A genetic algorithm (GA) is used to optimize the slider-crank mechanism's key parameters and improve the prosthesis's compactness. Combined with the hardware, a hybrid controller is proposed, which comprises impedance control in the stance phase with position control during the pre-swing and swing phases. The controller was applied to the powered prosthesis, and its capability to provide level walking functionality was evaluated on four above-knee amputees. The data obtained from these experimental trials indicated that the prosthesis could replicate the key biomechanical functions of a biological knee in the sagittal plane while the weight was lighter than most powered knee prostheses. In addition, the vertical ground reaction force (GRF) was quantitatively analyzed to validate that the powered knee prosthesis could improve wearing comfort with early-stance knee flexion.
Gait Event Detection with Proprioceptive Force Sensing in a Powered Knee-Ankle Prosthesis: Validation over Walking Speeds and Slopes

Abstract:Many powered prosthetic devices use load cells to detect ground interaction forces and gait events. These sensors introduce additional weight and cost in the device. Recent proprioceptive actuators enable an algebraic relationship between actuator torques and ground contact forces. This paper presents a proprioceptive force sensing paradigm which estimates ground reaction forces as a solution to detect gait events without a load cell. A floating body dynamic model is obtained with constraints at the center of pressure representing foot-ground interaction. Constraint forces are derived to estimate ground reaction forces and subsequently timing of gait events. A treadmill experiment is conducted with a powered knee-ankle prosthesis used by an able-bodied subject walking at various speeds and slopes. Results show accurate gait event timing, with pooled data showing heel strike detection lagging by only 6.7 ± 7.2 ms and toe off detection leading by 30.4 ± 11.0 ms compared to values obtained from the load cell. These results establish proof of concept for predicting gait events without a load cell in powered prostheses with proprioceptive actuators.
Preliminary Evaluation Of Gait Assistance During Treadmill Walking With A Light-Weight Bionic Knee Exoskeleton

Abstract:In this paper, we present the preliminary evaluation of gait assistance during treadmill walking with a powered bionic knee exoskeleton (BioKEX) which is intended for gait rehabilitation of patients with knee joint movement disorder. The BioKEX is a unilateral and light-weight wearable exoskeleton. The anthropomorphic joint design is that the tibia rolls on the femur during knee flexion/extention implemented by gear meshing mechanisms. We applied a finite-state machine based impedance control approach for exoskeleton control. The desired joint torque for the locomotion mode is controlled by finite-state machines, which modulate the joint impedance according to the phase of gait. In the experiments, we firstly verified the effect of assistance performance through the torque given by the exoskeleton during the gait cycle. Then, we performed energetics experiments and found human wearing BioKEX under assistance mode had a lower metabolic cost than wearing same weight load (an average reduction is approximately 6.21 % (p < 0.05). It implies that the assistance torque provide by the knee exoskeleton can compensate the energy consumption caused by exoskeleton intrinsic weight generating load for proximal joint. Therefore, the BioKEX system provides knee joint assistance for gait requirement, but not increases metabolic cost in spite of an extra weight (exoskeleton weight) on human.

Abstract:Most existing transtibial prostheses are energetically passive. Their ankle joints are either rigid or rotatable in a limited range, and their feet are single-segment structures without toe joints. Amputees using these passive prostheses exhibit nonsymmetrical gait patterns, consume more metabolic energy, and walk at lower speeds compared with able-bodied individuals. In this paper, we design and construct a powered transtibial prosthesis with stiffness adaptable ankle and toe joints, which are driven by adapted series-elastic actuators, to improve the walking performance of the amputees. Mechanical models of both joints are built to help analyze joints' capabilities of adjusting stiffness. In actual control of the prosthesis, we utilize a linearized trajectory control method to adjust the stiffness of both joints. To evaluate the performance of the proposed prosthesis, experiments are carried out on an amputee with a unilateral transtibial amputation. Experimental results indicate that both ankle and toe angles of the proposed prosthesis are close to those of the sound limb, and the vertical ground reaction force of the prosthetic side is similar to that of the sound side. Compared with a commercial passive prosthesis, the proposed prosthesis can help the amputee obtain more natural and symmetrical walking gaits.

Abstract:Reducing energy consumption during walking is a critical goal for transtibial amputees. The study presents the evaluation of a semi-active prosthesis with five transtibial amputees. The prosthesis has a low-power actuator integrated in parallel into an energy-storing-and-releasing foot. The actuator is controlled to compress the foot during the stance phase, supplementing the natural compression due to the user's dynamic interaction with the ground, particularly during the ankle dorsiflexion phase, and to release the energy stored in the foot during the push-off phase, to enhance propulsion. The control strategy is adaptive to the user's gait patterns and speed. The clinical protocol to evaluate the system included treadmill and overground walking tasks. The results showed that walking with the semi-active prosthesis reduced the Physiological Cost Index of transtibial amputees by up to 16% compared to walking using the subjects' proprietary prosthesis. No significant alterations were observed in the spatiotemporal gait parameters of the participants, indicating the module's compatibility with users' natural walking patterns. These findings highlight the potential of the mechatronic actuator in effectively reducing energy expenditure during walking for transtibial amputees. The proposed prosthesis may bring a positive impact on the quality of life, mobility, and functional performance of individuals with transtibial amputation.

Abstract:Recovering the ability to stand and walk independently can have numerous health benefits for people with spinal cord injury (SCI). Wearable exoskeletons are being considered as a promising alternative to conventional knee-ankle-foot orthoses (KAFOs) for gait training and assisting functional mobility. However, comparisons between these two types of devices in terms of gait biomechanics and energetics have been limited. Through a randomized, crossover clinical trial, this study compared the use of a knee-powered lower limb exoskeleton (the ABLE Exoskeleton) against passive orthoses, which are the current standard of care for verticalization and gait ambulation outside the clinical setting in people with SCI. Ten patients with SCI completed a 10-session gait training program with each device followed by user satisfaction questionnaires. Walking with the ABLE Exoskeleton improved gait kinematics compared to the KAFOs, providing a more physiological gait pattern with less compensatory movements (38% reduction of circumduction, 25% increase of step length, 29% improvement in weight shifting). However, participants did not exhibit significantly better results in walking performance for the standard clinical tests (Timed Up and Go, 10-m Walk Test, and 6-min Walk Test), nor significant reductions in energy consumption. These results suggest that providing powered assistance only on the knee joints is not enough to significantly reduce the energy consumption required by people with SCI to walk compared to passive orthoses. Active assistance on the hip or ankle joints seems necessary to achieve this outcome.
