Abstract:We evaluated the repositioning accuracy of a commercially available stereotactic whole body immobilization system (BodyFIX, Medical Intelligence, Schwabmuenchen, Germany) in 36 patients treated by hypofractionated stereotactic body radiation therapy. CT data were acquired for positional control of patient and tumor before each fraction of the treatment course. Those control CT datasets were compared with the original treatment planning CT simulation and analyzed with respect to positional misalignment of bony patient anatomy, and the respective position of the treated small lung or liver lesions. We assessed the stereotactic coordinates of distinct bony anatomical landmarks in the original CT and each control dataset. In addition, the target isocenter was recorded in the planning CT simulation dataset. An iterative optimization algorithm was implemented, utilizing a root mean square scoring function to determine the best-fit orientation of subsequent sets of anatomical landmark measurements relative to the original treatment planning CT data set. This allowed for the calculation of the x, y and z-components of translation of the patient's body and the target's center-of-mass for each control CT study, as well as rotation about the principal room axes in the respective CT data sets. In addition to absolute patient/target translation, the total magnitude vector of patient and target misalignment was calculated. A clinical assessment determined whether or not the assigned planning target volume safety margins would have provided the desired target coverage. To this end, each control CT study was co-registered with the original treatment planning study using immobilization system related fiducial markers, and the computed isodose calculation was superimposed. In 109 control setup CT scans available for comparison with their respective treatment planning CT simulation study (2-5 per patient, median 3), anatomical landmark analysis revealed a mean bony landmark translation of -0.4 +/- 3.9 (mean +/- SD), -0.1 +/- 1.6 and 0.3 +/- 3.6 mm in x, y and z-directions, respectively. Bony landmark setup deviations along one or more principal axis larger than 5 mm were observed in 32 control CT studies (29.4%). Body rotations about the x-, y- and z-axis were 0.9 +/- 0.7, 0.8 +/- 0.7 and 1.8 +/- 1.6 degrees, respectively. Assuming a rigid body relationship of target and bony anatomy, the mean computed absolute target translation was 2.9 +/- 3.3, 2.3 +/- 2.5 and 3.2 +/- 2.7 mm in x, y and z-directions, respectively. The median and mean magnitude vector of target isocenter displacement was computed to be 4.9 mm, and 5.7 +/- 3.7 mm. Clinical assessment of PTV/target volume coverage revealed 72 (66.1%), 23 (21.1%), and 14 (12.8%), of excellent (100% isodose coverage), good (>90% isodose coverage), and poor GTV/isodose alignment quality (less than 90% isodose coverage to some aspect of the GTV), respectively. Loss of target volume dose coverage was correlated with translations >5 mm along one or more axes (p<0.0001), rotations >3 degrees about the z-axis (p=0.0007) and body mass index >30 (p<0.0001). The analyzed BodyFIX whole body immobilization system performed favorably compared with other stereotactic body immobilization systems for which peer-reviewed repositioning data exist. While the measured variability in patient and target setup provided clinically acceptable setup accuracy in the vast majority of cases, larger setup deviations were occasional observed. Such deviations constitute a potential for partial target underdosing warranting, in our opinion, a pre-delivery positional assessment procedure (e.g., pre-treatment control CT scan).
Spine Stereotactic Body Radiation Therapy Residual Setup Errors and Intra-Fraction Motion Using the Stereotactic X-Ray Image Guidance Verification System

Abstract:Purpose: To determine the precision of our institution's current immobilization devices for spine SBRT, ultimately leading to recommendations for appropriate planning margins. Methods: We identified 12 patients (25 treatments) with spinal metastasis treated with spine Stereotactic Body Radiation Therapy (SBRT). The Body-FIX system was used as immobilization device for thoracic (T) and lumbar (L) spine lesions. The head and shoulder mask system was used as immobilization device for cervical (C) spine lesions. Initial patient setup used the infrared positioning system with body markers. Stereotactic X-ray imaging was then performed and correction was made if the initial setup error exceeded predetermined institutional tolerances, 1.5 mm for translation and 2° for rotation. Three additional sets of verification X-rays were obtained pre-, mid-, and post-treatment for all treatments. Results: Intrafraction motion regardless of immobilization technique was found to be 1.28 ± 0.57 mm. The mean and standard deviation of the variances along each direction were as follows: Superior-inferior, 0.56 ± 0.39 mm and 0.77 ± 0.52 mm, (p = 0.25); Anterior-posterior, 0.57 ± 0.43 mm and 1.14 ± 0.61 mm, (p = 0.01); Left-right, 0.48 ± 0.34 mm and 0.74 ± 0.40 mm, (p = 0.09) respectively. There was a significantly greater difference in the average 3D variance of the BodyFIX as compared to the head and shoulder mask immobilization system, 1.04 ± 0.46 mm and 1.71 ± 0.52 mm; (p = 0.003) respectively. Conclusions: Overall, our institution's image guidance system using stereotactic X-ray imaging verification provides acceptable localization accuracy as previously defined in the literature. We observed a greater intrafraction motion for the head and shoulder mask as compared with the BodyFIX immobilization system, which may be a result of greater C-spine mobility and/or the suboptimal mask immobilization. Thus, better immobilization techniques for C-spine SBRT are needed to reduce setup error and intrafraction motion. We are currently exploring alternative C-spine immobilization techniques to improve set up accuracy and decrease intrafraction motion during treatment.
Cardiac syndrome X, intraventricular gradients and, beta-blockers.

Abstract:INTRODUCTION Development of intraventricular gradients (IVG) on exertion seldom occurs in an unselected population. We performed exercise stress echocardiography (SE) in a 23-year-old athlete with a positive treadmill ECG and normal resting echocardiogram, but without angiographically demonstrable coronary disease (cardiac syndrome X), during which he developed an IVG of 102 mmHg and systolic anterior motion (SAM) of the mitral valve. Subsequently, we performed SE in 91 patients, 44 of whom were women, mean age 51.4 +/- 12.1 years (20-72), under similar circumstances--positive treadmill testing and no angiographically demonstrable coronary disease. In 33 (36%) of these, SE disclosed IVG with a mean end-systolic peak of 86 +/- 34 mmHg (30-165), together with mitral valve SAM in 23. The 20 patients who repeated SE under beta-blocker therapy constitute the study population. Seven (35%) of them were women, mean age 50.7 +/- 13.4 years (23 to 72). OBJECTIVE To assess the effect of beta-blockers on the occurrence of IVG and mitral valve SAM in patients with cardiac syndrome X. METHODS We repeated SE in 20 of the 33 patient that developed IVG on exertion, under beta-blocker therapy. They all underwent 2D and Doppler echocardiographic assessment before, during and after treadmill exercise. RESULTS Mean IVG in the 20 patients in the study population was 971 +/- 31 mmHg on the first SE assessment. In the assessment performed under beta-blocker therapy, 11 did not develop IVG, and in 9 the gradient was significantly reduced, to a mean of 46 +/- 13 mmHg. Mean heart rate at peak exercise was 161 +/- 17 bpm in the first SE assessment and 143 +/- 12 bpm under treatment with beta-blockers (p < 0.0001). Fifteen of these patients showed clinical improvement during follow-up, while no change in clinical status was seen in the other five. CONCLUSIONS In patients with cardiac syndrome X and IVG on exertion, treatment with oral beta-blockers prevented the occurrence of IVG or significantly reduced their magnitude. These changes were associated with significant reduction in heart rate at peak exercise and with clinical improvement in 75% of the study population.
A comparison of two immobilization systems for stereotactic body radiation therapy of lung tumors

Abstract:Purpose: This study aims to compare the efficacy, efficiency and comfort level of two immobilization systems commonly used in lung stereotactic body radiation therapy (SBRT): the Bodyfix and the abdominal compression plate (ACP). Materials and methods: Twenty-four patients undergoing SBRT for medically inoperable stage I lung cancer or pulmonary metastases were entered on this prospective randomized study. All underwent 4DCT simulation with free breathing, the Bodyfix, and the ACP to assess respiratory tumor motion. After CT simulation, patients were randomly assigned to immobilization with either the Bodyfix or the ACP for the actual SBRT treatment. Cone beam CTs (CBCTs) were acquired before and after each treatment to assess intrafraction tumor motion. Setup time and patient comfort were recorded. Results: There were 16 upper lobe, two middle lobe and seven lower-lobe lesions. Both the Bodyfix and the ACP significantly reduced the superior-inferior (SI) and overall respiratory tumor motion compared to free breathing (4.6 and 4.0 vs 5.3mm; 5.3 and 4.7 vs 6.1mm, respectively, p<0.05). The ACP further reduced the SI and overall respiratory tumor motion compared to the Bodyfix (p<0.05). The mean overall intrafraction tumor motion was 2.3mm with the Bodyfix and 2.0mm with the ACP (p>0.05). The ACP was faster to set up and rated more comfortable by patients than the Bodyfix (p<0.05). Conclusions: While there is no significant difference between the Bodyfix and ACP in reducing intrafraction tumor motion, the ACP is more comfortable, faster to set up, and superior to the Bodyfix in reducing SI and overall respiratory tumor motion.
Stereotactic body radiotherapy for stage I lung cancer and small lung metastasis: evaluation of an immobilization system for suppression of respiratory tumor movement and preliminary results

Abstract:Background: In stereotactic body radiotherapy (SBRT) for lung tumors, reducing tumor movement is necessary. In this study, we evaluated changes in tumor movement and percutaneous oxygen saturation (SpO2) levels, and preliminary clinical results of SBRT using the BodyFIX immobilization system. Methods: Between 2004 and 2006, 53 consecutive patients were treated for 55 lesions; 42 were stage I non-small cell lung cancer (NSCLC), 10 were metastatic lung cancers, and 3 were local recurrences of NSCLC. Tumor movement was measured with fluoroscopy under breath holding, free breathing on a couch, and free breathing in the BodyFIX system. SpO2 levels were measured with a finger pulseoximeter under each condition. The delivered dose was 44, 48 or 52 Gy, depending on tumor diameter, in 4 fractions over 10 or 11 days. Results: By using the BodyFIX system, respiratory tumor movements were significantly reduced compared with the free-breathing condition in both craniocaudal and lateral directions, although the amplitude of reduction in the craniocaudal direction was 3 mm or more in only 27% of the patients. The average SpO2 did not decrease by using the system. At 3 years, the local control rate was 80% for all lesions. Overall survival was 76%, cause-specific survival was 92%, and local progression-free survival was 76% at 3 years in primary NSCLC patients. Grade 2 radiation pneumonitis developed in 7 patients. Conclusion: Respiratory tumor movement was modestly suppressed by the BodyFIX system, while the SpO2 level did not decrease. It was considered a simple and effective method for SBRT of lung tumors. Preliminary results were encouraging.
Textured Growth of Strontium Ferrite Thin Films by Sputtering

Abstract:Study of M-type hexagonal ferrite films (BaFe 12 O 19 and SrFe 12 O 19 ) is drawing a lot of interest due to their potential applications. Ba ferrite films with perpendicular anisotropy have been studied due to their projected use in perpendicular recording media, magneto-optic recording media and in microwave/millimeter wave devices. The current interest in depositing these films with in plane anisotropy is for use in high density longitudinal recording media. There have been attempts to deposit these materials, mostly on crystalline substrates though without complete control over the orientation. We have for the first time deposited the sputtered strontium ferrite films and could vary the texture of the film to obtain both perpendicular and in plane anisotropy. It was found that the films prepared at lower rf power when annealed at temperatures ≥ 800 °C showed perpendicular anisotropy while the films prepared with higher rf power after similar annealing showed in plane anisotropy. These films had been prepared without substrate heating during sputter deposition and the as deposited films were X-ray amorphous and were nonmagnetic. However, these 'as deposited' films when annealed, showed different textures depending on the deposition conditions, as if these deposition conditions leave some signature in the films, which eventually decides the texture of these films. The microstructural studies showed that though the 'as deposited' films were X-ray amorphous, oriented microcrystallites form during deposition, which could be controlling the texture at the time of complete crystallization during annealing.
RECENT DEVELOPMENTS IN TREATMENT OF PULMONARY DISEASES.

Abstract:O NE of the most important advances in chest diseases in recent years has been the advent of streptomycin in the treatment of pulmonary tuberculosis. Although its discovery was announced only four years ago, experience with over one thousand patients in Army, Navy, Veterans, and civilian hospitals has clearly demonstrated that streptomycin alters the course of tuberculous infections in a manner unapproached by earlier chemotherapeutic agents.3'7 Exudative infiltrations of recent origin represent the type of pulmonary lesion most favorably influenced by streptomycin. At this stage vascularity remains little impaired, and there is good penetlation of the antibiotic throughout the involved tissues. The most dramatic results are observed in miliary tuberculosis, in which there is frequently complete roentgenologic clearing of the pulmonary infiltrations. Relapses unfortunately occur in the majority of such patients, and a second course of treatment is ineffective because of the appearance of resistant organisms. Another reason for the poor results obtained in miliary tuberculosis is the high incidence of tuberculous meningitis, which may be present when treatment is started, or may develop two or three months later. With tuberculous meningitis the initial response to treatment is also usually good, but later the signs and symptoms recur and death results within a few weeks. Only about 15 per cent of patients with tuberculous meningitis have remained symptom free for as long as a year following therapy with streptomycin. Because of the development of resistant organisms, and the toxic potentialities of the drug, it is generally felt that streptomycin is not indicated at present in the treatment of patients with minimal and moderately advanced pulmonary tuberculosis, since in these stages the response to bed rest and collapse therapy is usually favorable. Streptomycin is also contraindicated in chronic fibroid or fibrocaseous tuberculosis, in which anatomical barriers prevent adequate contact between the infecting organisms and the antimicrobial agent. There may be regression of the infiltration surrounding long-standing fibrocaseous involvement, but improvement is usually slight and temporary. Tuberculous empyema is not benefited by streptomycin. Streptomycin also seems to be of value when used in conjunction with the surgical treatment of tuberculosis, especially resection. In the recent series of Clagett,2 streptomycin appeared to prevent early postoperative spreads following lobectomy and pneumonectomy. Late spreads, however, were not
Residual setup errors in cranial stereotactic radiosurgery without six degree of freedom robotic couch: Frameless versus rigid immobilization systems

Abstract:Purpose and objectives: This IRB-approved study was to compare the residual inter-fractional setup errors and intra-fractional motion of patients treated with cranial stereotactic radiosurgery without a 6 degree of freedom (DoF) couch. We evaluated both frameless non-invasive vacuum-suction immobilization (Aktina PinPoint) and TALON rigid screw immobilization. Materials and methods: Twenty consecutive patients treated by Varian TrueBeam STX or Tomotherapy were selected for data collection. The dose and number of fractions received by each patient ranged from 18 Gy in 1 fraction (SRS) to 25 Gy in 5 fractions (SRT). Twelve patients were immobilized using PinPoint, a frameless suction system (Aktina Medical, New York) and eight patients were immobilized using the TALON rigid screw system. Customized head cushions were used for all patients. Six Atkina patients received pre- and post-treatment cone-beam CT (CBCT) to evaluate the intra-fractional motion of the Aktina system. The intra-fractional motion with the TALON rigid screw system has been reported to be negligible and was not repeated in this study. All patients received pre-treatment CBCT or megavoltage CT (MVCT) to assess inter-fractional setup accuracy. Shifts to the final treatment position were determined based on matching bony anatomy in the pre-treatment setup CT and the planning CT. Setup CT and planning CT were registered retrospectively based on bony anatomy using image registration software to quantify rotational and translational errors. Results: For the frameless Aktina system, mean and standard deviation of the intra-fractional motion were -0.5 ± 0.7 mm (lateral), 0.1 ± 0.9 mm (vertical), -0.5 ± 0.6 mm (longitudinal), -0.04 ± 0.18°(pitch), -0.1 ± 0.23°(yaw), and -0.03 ± 0.17°(roll) indicating negligible intra-fractional motion. Inter-fractional rotation errors were -0.10 ± 0.25° (pitch), -0.08 ± 0.16° (yaw), and -0.20 ± 0.41° (roll) for TALON rigid screw immobilization versus 0.20 ± 0.69° (pitch), 0.34 ± 0.56° (yaw), 0.35 ± 0.82° (roll) for frameless vacuum-suction immobilization showing that the rigid immobilization setup is more reproducible than the frameless immobilization. Without rotational correction by a 6 DoF couch, residual registration error exists and increases with distance from the image fusion center. In a 3D vector space, a tumor located 5 cm from the center of image fusion would require a 0.9 mm margin with the TALON system and a 2.1 mm margin with Aktina. Conclusions: With image-guided radiotherapy, translational setup errors can be corrected by image registration between pre-treatment setup CT and planning CT. However, rotational errors cannot be accounted for without a 6 DoF couch. Our study showed that the frameless Aktina immobilization system provided negligible intra-fractional motion. The inter-fractional rotation setup error using Aktina was larger than rigid immobilization with the TALON system. To treat a single lesion far from the center of image registration or for multiple lesions in a single plan, additional margin may be needed to account for the uncorrectable rotational setup errors.
Companison of Dose Distribution and Setup Error of Two Different Positions and Immobilization Techniques in Breast Cancer Radiotherapy

Abstract:Objective To compare the dose distribution and setup error of angled breast board position ( ABB) with plain breast board position ( PBB) in breast cancer radiotherapy. Methods Twenty consecutive postoperative left breast cancer patients in the First Affiliated Hospital, Zhejiang University were enrolled from March 2017 to January 2018. All cases were assigned into the ABB and PBB groups according to positions and there were 10 cases in each groups respectively. The target volume, heart and lung structures were defined on the CT images of the localized scan. The plan was designed using the field in field ( FIF) technique to compare the dosimetric parameters of the PTV, lung, and heart treatments, and the setup errors for the two different positions. Results The ipsilateral lung V20[ABB:(11.2 ±3.2)％, PBB:(15.9 ±5.3)％, t= -2.47,P< 0.05], and V30[ABB:(9.8 ±1.5)％, PBB:(12.9 ±2.2)％, t= -4.46,P<0.05] were both statistically significant for the two different position and immobilization. Heart dose V25[ABB: (1.9 ± 0.2)％, PBB: (2.8 ± 0.4)％, t = -8.28, P <0.05], V30 [ABB:(1.8 ±0.1)％, PBB: (2.7 ± 0.3)％, t = -8.34,P < 0.05], and Dmean of heart [ABB: (3.0 ±0.5)Gy, PBB:(5.3 ± 1.2)Gy, t=5.58,P<0.05] were all statistically significant for the two different positions. The translational errors of ABB and PBB on LR, SI, and AP were ( 3.23 ± 2.63 ) , ( 5.42 ± 3.22), (4.58 ± 2.30) mm, and (2.35 ± 1.22), (2.17 ± 1.29), (2.27 ± 1.58) mm, respectively. The rotation errors of pitch(θ), yaw(Φ) and roll(ψ) for ABB and PBB were (1.60 ± 0.56)°, (3.40 ± 1.65)°, (2.50 ± 1.72)°, and (1.37 ± 0.43)°, (1.79 ± 0.71)°, (2.06 ± 0.63)°, respectively. Meanwhile, the in-and out-SI, anterior and posterior AP, yaw rotation error (Φ) were also statistically significant(t=3.06, 2.80, 3.33,P<0.05). Conclusions There is no statistically significant difference in the tumor target between the two position and immobilization techniques. However, the ABB is better than the PBB in normal tissue sparing while the setup accuracy of PBB is better than the ABB.
Efficacy of a thermoplastic mask and pneumatic abdominal compression device for immobilization in stereotactic ablative radiotherapy of spine metastases

Abstract:Stereotactic ablative radiotherapy (SABR) has become a key technique in management of spine metastases. With improved control over treatment plan dosimetry, there is a greater need for accurate patient positioning to guarantee agreement between the treatment plan and delivered dose. With serious potential complications such as fracture and myelopathy, the margins of error in SABR of the spine are minimal. In this study, we assessed the performance of two patient immobilization setups in SABR for spinal metastases. First, a Type‐S head and shoulders mask (CQ Medical, Avondale, PA), and second, the BPL1 setup, which includes a wing board, vacuum bag, and the Respiratory Belt for the Body Pro‐Lok ONE (CQ Medical, Avondale, PA). Immobilization was assessed using image‐guided intrafraction repositioning shifts. Required planning target volume (PTV) margins were calculated based on repositioning data for 172 treated fractions using 2 standard deviation (2SD) and analytic approaches. Overall, 91.7% and 74.1% of fractions treated had total 3D repositioning shifts ≤3.0 mm using the Type‐S and BPL1 setups, respectively. In the thoracic spine, 43.2% and 46.5% of fractions had shifts ≤1.5 mm for the respective setups. Suggested margins were under 3.5 mm in all directions and use cases. In the posterior‐anterior direction, the BPL1 setup had a 0.6 mm smaller suggested margin for the thoracic spine compared to the Type‐S setup, at 1.4 mm, calculated using the analytic approach. Both the Type‐S and BPL1 setups are effective for immobilization in spine SABR. The Type‐S demonstrated superior immobilization in the upper spine and remains the clinical standard for cervical and upper thoracic spine positioning. The BPL1 setup showed effective immobilization in use cases treating the mid‐to lower thoracic spine and lumbar spine and remains our clinical standard for those use cases. Results additionally demonstrate feasibility of potential PTV margin reduction.

Abstract:Objective: The aim of this study is to analyze which tracking modality is more suitable for stereotactic body radiosurgery of lumbosacral spinal tumors by comparing prone and supine patient treatment setup. Methods: Eighteen patients with lumbosacral spinal tumors were selected. CT simulation was performed in the supine position (fixed with a vacuum cushion) and prone position (fixed with a thermoplastic mask and prone plate), respectively. The plans in the supine and prone positions were designed using the xsight spine tracking (XST) and xsight spine prone tracking (XSPT) modalities, respectively. The dose-volume histogram (DVH) parameters, namely, V 100% , D 95% , D mean , conformity index (CI), and heterogeneity index (HI) in planning target volume (PTV), as well as D max , D 0.1cc , D 1cc , and D 5cc in the cauda equina and bowel were recorded. The supine plans were simulation plans and were not used for treatment, which were only used to record the alignment errors. The spinal tracking correction errors (alignment error) and correlation errors of the synchrony respiratory model in the prone position were recorded during the treatment. After treatment, the simulation plan of the supine position was implemented and the spinal tracking correction errors were recorded. The parameters of correction error and DVH parameters for the two positions were analyzed using the paired t -test to compare the difference in positioning accuracy and dose distribution. In addition, the correlation errors of the synchrony respiratory model in the prone position were analyzed to evaluate the prediction accuracy of the synchrony model. Results: For patient setup, the correction error of the supine position in interior/posterior was (0.18 ± 0.16) mm and the prone position was (0.31 ± 0.26) mm ( P < 0.05). The correction error of the supine position in inferior/superior was (0.27 ± 0.24) mm, and the prone position was (0.5 ± 0.4) mm ( P< 0.05). The average correlation errors of the synchrony model for left/right, inferior/superior, and anterior/posterior in the prone position were (0.21 ± 0.11) mm, (0.41 ± 0.38) mm, and (0.68 ± 0.42) mm, respectively. For the dose distribution, compared with prone plans, the average CI in supine plans was increased by 4.5% ( P < 0.05). There was no significant difference in HI, PTV V 100%, D 95% , and D mean between the prone and supine plans. Compared with supine plans, average D 1cc and D 5cc for the cauda equina was significantly decreased by 4.7 and 15.3% in the prone plan ( P< 0.05). For the bowel, average D max , D 0.1cc , D 1cc , and D 5cc were reduced by 8.0, 7.7, 5.2, and 26.6% in prone plans ( P < 0.05) compared with supine plans. Conclusion: Compared with the supine setup, the prone setup combined with XSPT modality for the lumbosacral spinal stereotactic body radiosurgery can spare the bowel and cauda equina of the middle and low dose irradiation, and decrease the number of beams and monitor units.

Abstract:Purpose The use of volumetric modulated arc therapy (VMAT), simultaneous integrated boost (SIB), and hypofractionated regimen requires adequate patient setup accuracy to achieve an optimal outcome. The purpose of this study was to assess the setup accuracy of patients receiving left‐sided breast cancer radiotherapy using deep inspiration breath‐hold technique (DIBH) and surface guided radiotherapy (SGRT) and to calculate the corresponding setup margins. Methods The patient setup accuracy between and within radiotherapy fractions was measured by comparing the 6DOF shifts made by the SGRT system AlignRT with the shifts made by kV‐CBCT. Three hundred and three radiotherapy fractions of 23 left‐sided breast cancer patients using DIBH and SGRT were used for the analysis. All patients received pre‐treatment DIBH training and visual feedback during DIBH. An analysis of variance (ANOVA) was used to test patient setup differences for statistical significance. Abstract:Introduction This study aimed to evaluate the setup accuracy of the new shim mask with mouth bite compared to the standard full brain mask in stereotactic radiosurgery (SRS) and radiotherapy (SRT) treatments for brain metastases or tumors. Method A combined retrospective and prospective design was employed, involving 40 patients treated at our center. Patients previously treated using standard head masks formed the retrospective cohort, while those treated with the Shim mask and mouth bite formed the prospective cohort. Daily cone-beam computed tomography (CBCT) scans were obtained before each treatment session to ensure patient setup accuracy. Key metrics included absolute shifts in translational and rotational directions, the number of repeat CBCTs, and the time interval between CBCTs. Results The Shim mask significantly reduced the mean setup errors in the lateral translation (p=0.022) from 0.17 cm (SD=0.10) to 0.10 cm (SD=0.10), and in X-axis rotation (p=0.030) from 0.79° (SD=0.43) to 0.47° (SD=0.47). By considering cutoff points of 1 mm in translational and 1° in rotational directions, the Shim mask was significantly more accurate in the lateral direction (p=0.004). Moreover, while 70% of patients in the standard group required repeat CBCT scans, none in the Shim group did, resulting in an average time saving of 10.4 minutes per patient. Conclusion The Shim mask with mouth bite offers enhanced immobilization accuracy in SRT/SRS treatments, leading to time and potential cost savings by reducing the need for repeat CBCT scans. This underscores the importance of adopting innovative immobilization techniques to optimize patient outcomes.

Abstract:Objective To compare the setup accuracy between Catalyst HD and skin markers in stereotactic body radiotherapy (SBRT) of lung cancer.Methods A total of 24 cases treated with SBRT were selected and all patients were fixed with vacuum pad in the supine position.Patients in group A were positioned by Catalyst HD and those in group B were positioned by shin markers.All patients were matched with the CT images after CBCT scan by rigid registration and the setup errors in six directions (x-,y-,z-axis,Rtn,Pitch and Roll) were obtained.Results The mean ±SD in group A and B in the six directions were as follows:(0.13±0.12) cm,(0.25± 0.19) cm;(0.26±0.15) cm,(0.13±0.11) cm;(0.23±0.19) cm,(0.35±0.29) cm;(0.43°±0.40°),(0.80°±0.69°);(0.48°±0.47°),(0.79°±0.64°);(0.62°±0.60°) and (0.88°±0.70°),respectively.Except the x-axis data in group B,all the data in the six directions were not normally distributed.The obtained data significantly differed between two groups (all P＜0.05).The out-of-tolerance errors (＞0.5 cm/2°) also significantly differed between two groups (P＜0.05).Conclusions The setup errors of Catalyst HD are less than those of the skin markers (except the y-axis).The setup accuracy of Catalyst HD is superior to that of traditional skin markers,which is worthy of application in clinical practice.

Abstract:Objective To quantify the setup errors measured with orthogonal megavoltage X-ray film in breast cancer patients immobilized in breast bracket.To probe into the margins from clinical target volume (CTV) to planning target volume (PTV) in 3 directions.Methods Repeat orthogonal megavoltage X-ray films were acquired for routine of fine setup verification in 29 breast cancer patients after conserving surgery,17 received whole breast radiation and 12 received both whole breast and supraclavicular node radiation.All patients were immobilized in the supine position with both arms raised over their heads,using a personalized α-cradle (breast bracket).Registrations of the bony anatomy for megavoltage X-ray films to digitally reconstructed-radiographs from the planning CT were compared.Systematic and random setup errors were quantified,and the differences between groups with and without supraclavicular node radiation were compared by independent two-sample t-test.Results The mean set up deviation,systematic error and random error were (0.9±3.1) mm,(0.7±3.0) mm,(1.2±2.1) mmand3.1 mm,3.0 mm,2.1 mm and 2.7 mm,3.3 mm,3.5 mm in the mediolateral,superoinferior and anteroposterior dimensions.There is no difference of set up deviation between patients received supraclavicular node radiation and those did not (t =0.02,0.20,0.20,P =0.98,0.85,0.85).The margins from CTV to PTV were 9.6 mm,9.8 mm,7.7 mm in mediolateral,superoinferior and anteroposterior directions.Conclusion For whole breast irradiation patients immobilized with breast bracket,the margins from CTV to PTV were recommended as 9.6 mm,9.8mm,7.7 mm in mediolateral,superoinferior and anteroposterior directions.
Workflow and error analyses of patient setup based on open-face mask immobilization combined with AlignRT for head tumor radiotherapy

Abstract:Objective:To propose a markless patient setup workflow based on the optical surface monitoring system (AlignRT) and open-face mask immobilization for whole-course head tumor radiotherapy, assess the setup time and repositioning frequency of the proposed workflow, and conduct a comparative analysis of the differences, correlation, and consistency of the setup errors of the AlignRT and cone beam CT (CBCT) systems.Methods:A retrospective analysis was conducted for the data on the errors of 132 fractionated setup based on open-face mask immobilization of 33 head tumor patients. AlignRT-guided markless patient setup workflow was applied throughout the radiotherapy. Meanwhile, the body structures automatically generated by the treatment planning system were used as body references. The 6-degree-of-freedom (6DoF) setup errors (lateral, vertical, longitudinal, rotation, pitch, roll, and yaw directions), setup time, and repositioning frequency of the AlignRT and CBCT systems were recorded and analyzed. The Wilcoxon and Spearman analyses were used to statistically assess the differences and correlation of the setup errors of the two systems. Moreover, the Bland-Altman analysis was employed to evaluate the consistency of the two systems.Results:The 6DoF setup errors of CBCT were within the clinical tolerance (linear motions: -0.30 to 0.30 cm; rotational motions: -2.0° to 2.0°). The setup time and repositioning frequency of CBCT were (98 ± 31) s and 1.51% (2/132), respectively. There was no significant difference in setup errors between the two systems except those in x-axis ( Z = -3.11, P= 0.002), y-axis ( Z = -7.40, P<0.001), and Pitch ( Z= -4.48, P<0.001). There was a significant positive correlation between the setup errors along lateral ( rs = 0.47, P<0.001) and vertical ( rs = 0.29, P = 0.001) directions, rotation (Rtn; rs = 0.47, P<0.001), pitch (Pitch; rs = 0.28, P = 0.001) and roll (Roll; rs = 0.45, P<0.001) of the two systems. The 95% limits of agreement (95% LoA) of 6DoF setup errors were -0.12 to 0.09 cm, -0.07 to 0.17 cm, -0.19 to 0.20 cm, -1.0° to 0.9 °, -1.0° to 1.5°, and -0.9° to 1.0°, respectively. The 95% confidence interval (95% CI) of 95% LoA was -0.14 to 0.11 cm, -0.09 to 0.19 cm, -0.23 to 0.23 cm, -1.2° to 1.1°, -1.2° to 1.7°, and-1.0° to 1.1°, respectively, all of which were within the permissible error ranges. The 6DoF setup error difference of 3.41% (27/792< 5%) was beyond the 95% LoA. The maximum absolute differences of 6DoF setup errors within the 95% LoA were 0.12, 0.16, 0.19 cm, 0.9°, 1.5°, and 1.0°, respectively. Conclusions:The proposed markless setup workflow based on AlignRT combined with open-face mask immobilization for whole-course head tumor radiotherapy exhibits reasonable agreement and consistency with the patient setup using CBCT, with acceptable clinical efficiency. It can be applied to the first radiotherapy and the real-time monitoring of therapy to improve the safety and thus is of value in clinical applications.

Abstract:Abstract Purpose We compared the setup errors determined by an optical imaging system (OSIS) in women who received breast‐conserving surgery (BCS) followed by whole‐breast radiotherapy (WBRT) with those from cone‐beam computed tomography (CBCT) carried out routinely. Methods We compared 130 setup errors in 10 patients undergoing WBRT following BCS by analyzing the translational and rotational couch shifts via CBCT and OSIS. Patients were treated with intensity‐modulated radiotherapy (IMRT). The patient outline extracted from the planning reference Computed tomography (CT) was used as the reference for OSIS and CBCT alignment during treatment. We detected the setup uncertainty using CBCT and OSIS at the first five fractionations of RT and then twice a week. Results The absolute translational setup error (mean ± Standard deviation (SD)) in x (lateral), y (longitudinal), and z (vertical) axes detected by the OSIS was 0.14 ± 0.18, 0.15 ± 0.14, and 0.13 ± 0.13 cm, respectively. The rotational setup error (mean ± SD) in Rx (pitch), Ry (roll), and Rz (yaw) axes was 0.77 ± 0.54, 0.76 ± 0.61, and 1.23 ± 0.95, respectively. Significant difference is observed only in one direction (Rx, p = 0.03) in the paired setup errors obtaining from OSIS and CBCT, without significant differences in five directions. Conclusion OSIS is a repeatable and reliable system that can be used to detect misalignments with accuracy, which is capable of supplementing CBCT for WBRT after BCS. We believe that an OSIS may be easier to use, quicker, and reduce overall dose as this method of patient alignment does not require ionizing radiation.

Abstract:Purpose: Tissue heterogeneity changes result in a build down/build up of the recoil electron fluence at heterogeneity interfaces. The use of stereotactic radiosurgery beams on small lung lesions can compromise the dose delivery to lesions ranging from 6 mm up through 30 mm in diameter, since the dimensions of the lesions are comparable to the range of the secondary electrons in lung and air. Dose perturbations are evaluated for a range of lesion diameters in lung equivalent phantom material. Method and Materials: Lesion diameters of 6.4, 12.5, 19.7, and 25.4 mm of unit density were inserted into lung equivalent material. The dose distributions were measured with film densitometry and calculated with the dose calculation engine of the ADAC Pinnacle system. The Pinnacle treatment planning system uses the adaptive convolution superposition algorithm, which rigorously reproduces x‐ray beam attenuation, and the electron fluence changes at tissue heterogeneity interfaces. Results: The physical density of the lung equivalent phantom material is 0.30 g/cm 3 in contrast to water and normal tissue at 1.0 g/cm 3 . The spherical tumor phantoms were irradiated with beam margins of 0, 7, 10, and 25 mm. Dose volume histogram analysis was performed to develop a DVH surface map of the dose delivery to each lesion as a function of margin size. It was determined that a 7 mm margin for all lesion diameters delivers a minimum tumor dose of 94 % of the prescribed isocenter dose for all lesion diameters. The DVH analysis and film dosimetry were also compared with the dose calculations of the BrainSCAN planning system. Conclusion: These measurements and calculations confirm the clinical feasibility of stereotactic irradiations of lung lesions with minimal compromise of the tumor dose. Abstract:Objective To analyze the application value of two postural fixation techniques.(styrofoam combined with head mask and fixed headrest combined with head mask) in intracranial tumor radiotherapy via cone beam computed tomography (CBCT). Methods This study included 104 patients with intracranial tumors undergoing radiotherapy. The patients were divided into two groups: Group A (54 cases with styrofoam fixation) and Group B (50 cases with fixed headrest fixation). The positional deviation in 3D space between the two groups was compared using CBCT. The set-up errors were expressed as median (25th percentile, 75th percentile)or M(p25, p75) since the set-up errors in all directions were not normally distributed,The Mann-Whitney U test was performed. Results The age and gender of patients in the two groups were not significantly different. The set-up errors of A in lateral (X), longitudinal (Y), vertical (Z), and yaw(Rtn) axes were 1.0 (0,1) mm, 1.0 (0,1) mm, 1.0 (0,2) mm, and 0.4 (0.1, 0.8) degrees, respectively while the set-up errors of B were 1.0 (0,1) mm, 1.0 (1,2) mm, 1.0 (0,2) mm, and 0.5 (0.15,0.9) degrees, respectively. Moreover, patients in the styrofoam group had significantly smaller set-up errors in the Y-axis than patients in the headrest group (p=0.001). However, set-up errors in the X, Z, and Rtn axes were not significantly different between the two groups. The expansion boundaries of the target area in the X, Y, and Z directions were 1.77 mm, 2.45 mm, and 2.47 mm, respectively. The outer expansion boundaries of the headrest group were 2.03 mm, 3.88 mm, and 2.57 mm in X, Y, and Z directions, respectively. The set-up times of groups A and B were (32.71 ± 5.21) seconds and (46.57 ± 6.68) seconds, respectively (p=0.014). Patients in group A had significantly better comfort satisfaction than patients in group B (p=0.001). Conclusion Styrofoam plus head thermoplastic mask body fixation technique has a higher positional accuracy in intracranial tumor radiotherapy than headrest plus head thermoplastic mask fixation. Besides, styrofoam plus head thermoplastic mask body fixation technique is associated with improved positioning efficiency, and better comfort than headrest plus head thermoplastic mask fixation, and thus can be effectively applied for intracranial tumor radiotherapy positioning.
