Abstract:PurposeHypofractionated radiotherapy of prostate cancer is gaining clinical acceptance given its potential increase in therapeutic ratio and evidence for noninferiority and lack of added late toxicities compared to conventional fractionation. However, concerns have been raised that smaller number of fractions might lead to larger dosimetric influence by interfractional motion. We aim to compare the effect of these variations on hypofractionated pencil beam scanning (PBS) proton therapy and volumetric modulated arc therapy (VMAT) for localized prostate cancer.MethodsWeekly CT images were acquired for 6 patients participating in a randomized clinical trial. PBS plans featuring bilateral (BL) and a combination of lateral and anterior-oblique beams (AOL), and VMAT plans were created. All patients were treated to a conventional 79.2Gy total dose in 44 fractions. For this study, hypofractionated dose to the prostate gland was 51.6Gy in 12 fractions or 36.25Gy in 5 fractions, and 32.8, and 23.1Gy to proximal seminal vesicles, respectively. Patients were simulated with endorectal balloons to aid gland immobilization. Three fiducial markers were implanted for setup guidance. All plans were recomputed on the weekly CT images after aligning with the simulation CT. The entire set of 9 CT images was used for dose recalculation for 12-fraction and only 5 used for the 5-fraction case. Adaptive range adjustments were applied to anterior-oblique beams assuming clinical availability of invivo range verification. Fractional doses were summed using deformable dose accumulation to approximate the delivered dose. Biologically equivalent dose to 2Gy(EQD2) was calculated assuming / of 1.5Gy for prostate and 3Gy for bladder and rectum.ResultsThe median delivered prostate D-98 was 0.13/0.14/0.13Gy(EQD2) smaller than planned for PBS-BL, 0.13/0.27/0.17Gy(EQD2) for PBS-AOL and 0.59/0.66/0.59Gy(EQD2) for VMAT, for 44/12/5 fractions, respectively. The largest D-98 reduction was 1.5 and 3.5Gy(EQD2) for CTV1 and CTV2, respectively. Target dose degradation was comparable for all fractionation schemes within each modality. The maximum increase in rectum D-2 was 0.98Gy(EQD2) for a 5-fraction PBS case.ConclusionsThe robustness of PBS and VMAT were comparable for all patients for the studied fractionations. The delivered target dose generally remained within clinical tolerance and the deviations were relatively minor for both fractionation schemes. The delivered OAR dose stayed in compliance with the RTOG hypofractionation constraints for all cases.
A comparison of two immobilization systems for stereotactic body radiation therapy of lung tumors

Abstract:Purpose: This study aims to compare the efficacy, efficiency and comfort level of two immobilization systems commonly used in lung stereotactic body radiation therapy (SBRT): the Bodyfix and the abdominal compression plate (ACP). Materials and methods: Twenty-four patients undergoing SBRT for medically inoperable stage I lung cancer or pulmonary metastases were entered on this prospective randomized study. All underwent 4DCT simulation with free breathing, the Bodyfix, and the ACP to assess respiratory tumor motion. After CT simulation, patients were randomly assigned to immobilization with either the Bodyfix or the ACP for the actual SBRT treatment. Cone beam CTs (CBCTs) were acquired before and after each treatment to assess intrafraction tumor motion. Setup time and patient comfort were recorded. Results: There were 16 upper lobe, two middle lobe and seven lower-lobe lesions. Both the Bodyfix and the ACP significantly reduced the superior-inferior (SI) and overall respiratory tumor motion compared to free breathing (4.6 and 4.0 vs 5.3mm; 5.3 and 4.7 vs 6.1mm, respectively, p<0.05). The ACP further reduced the SI and overall respiratory tumor motion compared to the Bodyfix (p<0.05). The mean overall intrafraction tumor motion was 2.3mm with the Bodyfix and 2.0mm with the ACP (p>0.05). The ACP was faster to set up and rated more comfortable by patients than the Bodyfix (p<0.05). Conclusions: While there is no significant difference between the Bodyfix and ACP in reducing intrafraction tumor motion, the ACP is more comfortable, faster to set up, and superior to the Bodyfix in reducing SI and overall respiratory tumor motion.
Repositioning accuracy of a commercially available double-vacuum whole body immobilization system for stereotactic body radiation therapy

Abstract:We evaluated the repositioning accuracy of a commercially available stereotactic whole body immobilization system (BodyFIX, Medical Intelligence, Schwabmuenchen, Germany) in 36 patients treated by hypofractionated stereotactic body radiation therapy. CT data were acquired for positional control of patient and tumor before each fraction of the treatment course. Those control CT datasets were compared with the original treatment planning CT simulation and analyzed with respect to positional misalignment of bony patient anatomy, and the respective position of the treated small lung or liver lesions. We assessed the stereotactic coordinates of distinct bony anatomical landmarks in the original CT and each control dataset. In addition, the target isocenter was recorded in the planning CT simulation dataset. An iterative optimization algorithm was implemented, utilizing a root mean square scoring function to determine the best-fit orientation of subsequent sets of anatomical landmark measurements relative to the original treatment planning CT data set. This allowed for the calculation of the x, y and z-components of translation of the patient's body and the target's center-of-mass for each control CT study, as well as rotation about the principal room axes in the respective CT data sets. In addition to absolute patient/target translation, the total magnitude vector of patient and target misalignment was calculated. A clinical assessment determined whether or not the assigned planning target volume safety margins would have provided the desired target coverage. To this end, each control CT study was co-registered with the original treatment planning study using immobilization system related fiducial markers, and the computed isodose calculation was superimposed. In 109 control setup CT scans available for comparison with their respective treatment planning CT simulation study (2-5 per patient, median 3), anatomical landmark analysis revealed a mean bony landmark translation of -0.4 +/- 3.9 (mean +/- SD), -0.1 +/- 1.6 and 0.3 +/- 3.6 mm in x, y and z-directions, respectively. Bony landmark setup deviations along one or more principal axis larger than 5 mm were observed in 32 control CT studies (29.4%). Body rotations about the x-, y- and z-axis were 0.9 +/- 0.7, 0.8 +/- 0.7 and 1.8 +/- 1.6 degrees, respectively. Assuming a rigid body relationship of target and bony anatomy, the mean computed absolute target translation was 2.9 +/- 3.3, 2.3 +/- 2.5 and 3.2 +/- 2.7 mm in x, y and z-directions, respectively. The median and mean magnitude vector of target isocenter displacement was computed to be 4.9 mm, and 5.7 +/- 3.7 mm. Clinical assessment of PTV/target volume coverage revealed 72 (66.1%), 23 (21.1%), and 14 (12.8%), of excellent (100% isodose coverage), good (>90% isodose coverage), and poor GTV/isodose alignment quality (less than 90% isodose coverage to some aspect of the GTV), respectively. Loss of target volume dose coverage was correlated with translations >5 mm along one or more axes (p<0.0001), rotations >3 degrees about the z-axis (p=0.0007) and body mass index >30 (p<0.0001). The analyzed BodyFIX whole body immobilization system performed favorably compared with other stereotactic body immobilization systems for which peer-reviewed repositioning data exist. While the measured variability in patient and target setup provided clinically acceptable setup accuracy in the vast majority of cases, larger setup deviations were occasional observed. Such deviations constitute a potential for partial target underdosing warranting, in our opinion, a pre-delivery positional assessment procedure (e.g., pre-treatment control CT scan).

Abstract:Purpose: To quantify interfractional anatomical variations and their dosimetric impact during the course of fractionated proton therapy (PT) of prostate cancer and to assess the robustness of the current treatment planning techniques.Methods: Simulation and daily in-room CT scans from ten prostate carcinoma patients were analyzed. PT treatment plans (78 Gy in 39 fractions of 2 Gy) were created on the simulation CT, delivering 25 fractions to PTV1 (expanded from prostate and seminal vesicles), followed by 14 boost fractions to PTV2 (expanded from prostate). Plans were subsequently applied to daily CT, with beams aligned to the prostate center in the sagittal plane. For five patients having a sufficiently large daily imaging volume, structure contours were manually drawn, and plans were evaluated for all CT sets. For the other five patients, the plans were evaluated for six selected fractions. The daily CT was matched to the simulation CT through deformable registration. The registration accuracy was validated for each fraction, and the three patients with a large number of accurately registered fractions were used for dose accumulation.Results: In individual fractions, the coverage of the prostate, seminal vesicles, and PTV1 was generally maintained at the corresponding prescription dose. For PTV2, the volume covered by the fractional prescription dose of 2 Gy (i.e., V2) was, on average, reduced by less than 3% compared to the simulation plan. Among the 225 (39 x 5 + 6 x 5) fractions examined, 15 showed a V2 reduction larger than 5%, of which ten were caused by a large variation in rectal gas, and five were due to a prostate shift in the craniocaudal direction. The fractional dose to the anterior rectal wall was found to increase for one patient who had large rectal gas volume in 25 of the 39 fractions, and another who experienced significant prostate volume reduction during the treatment. The fractional bladder dose generally increased with decreasing fullness. In the total accumulated dose for the three patients after excluding a few fractions with inaccurate registration due to a large amount of rectal gas (a condition inconsistent with RTOG protocol), 98.5%, 96.6%, and 98.2% of the PTV2 received the prescription dose of 78 Gy. The V75 and V70 of the anterior rectal wall and bladder both remained within tolerance.Conclusions: The results confirm that the PT planning techniques and dose constraints used at our institution ensure that target coverage to the prescription dose is maintained in the presence of interfractional anatomical variations. Dose coverage in individual fractions can be compromised, and normal tissue dose increased, due to deviations in the bladder and rectal volume compared to the simulation plans or progressive changes in the prostate volume during the treatment. Abstract:Objective : Most contemporary treatment planning systems (TPS) exclude the couch during treatment planning. The present investigation aimed to evaluate the impact of couches and accessories on radiation therapy planning by quantifying the degree of attenuation for two-photon beam energies, 6 MV and 10 MV, at two different field sizes, 5 cm x 5 cm and 10 cm x 10 cm. Methods: An x-ray radiation beam at two energies (6 MV and 10 MV) generated by a linear accelerator (Elekta Synergy). The output dose was measured using a digital parallel plate ionization chamber of the IBA cc13 type (IBA Dosimetry, Germany), and the readings were recorded with a 2944 Farmer Type chamber. Different commercially available couch tops and accessories evaluated in this study. Results: The results show that the highest attenuation values were observed at 1300 gantry angles for both energies and field sizes. The lowest attenuation values were recorded at 1000 gantry angles with 6 MV energy for 5cm x 5cm and 10cm x 10cm field sizes. The breast board with D level demonstrated the highest attenuation of 6.27% and 5.51% for 6 MV and 10 MV energies, respectively. Conclusion: the findings indicate that the degree of attenuation is not uniform across all angles and may exceed tolerable limits, indicating the need for careful consideration during treatment planning to ensure optimal treatment outcomes.

Abstract:This study evaluated the efficiency of stereotactic body radiation therapy of lung (SBRT-Lung) in generating a treatment volume using conventional multiple-phase three-dimensional computed tomography (3D-CT) of a patient immobilized with pneumatic abdominal compression. The institutional protocol for SBRT-Lung using the RapidArc technique relied on a planning target volume (PTV) delineated using 3D-CT and accounted for linear and angular displacement of the tumor during respiratory movements. The efficiency of the institutional protocol was compared with that of a conventional method for PTV delineation based on radiobiological estimates, such as tumor control probability (TCP) and normal tissue complication probability (NTCP), evaluated using dose–volume parameters. Pneumatic abdominal compression improved the TCP by 15%. This novel protocol improved the TCP by 0.5% but reduced the NTCP for lung pneumonitis (0.2%) and rib fracture (1.0%). Beyond the observed variations in the patient’s treatment setup, the institutional protocol yielded a significantly consistent TCP (p < 0.005). The successful clinical outcome of this case study corroborates predictions based on radiobiological evaluation and deserves validation through an increased number of patients.

Abstract:Stereotactic ablative radiotherapy (SABR) has become a key technique in management of spine metastases. With improved control over treatment plan dosimetry, there is a greater need for accurate patient positioning to guarantee agreement between the treatment plan and delivered dose. With serious potential complications such as fracture and myelopathy, the margins of error in SABR of the spine are minimal. In this study, we assessed the performance of two patient immobilization setups in SABR for spinal metastases. First, a Type‐S head and shoulders mask (CQ Medical, Avondale, PA), and second, the BPL1 setup, which includes a wing board, vacuum bag, and the Respiratory Belt for the Body Pro‐Lok ONE (CQ Medical, Avondale, PA). Immobilization was assessed using image‐guided intrafraction repositioning shifts. Required planning target volume (PTV) margins were calculated based on repositioning data for 172 treated fractions using 2 standard deviation (2SD) and analytic approaches. Overall, 91.7% and 74.1% of fractions treated had total 3D repositioning shifts ≤3.0 mm using the Type‐S and BPL1 setups, respectively. In the thoracic spine, 43.2% and 46.5% of fractions had shifts ≤1.5 mm for the respective setups. Suggested margins were under 3.5 mm in all directions and use cases. In the posterior‐anterior direction, the BPL1 setup had a 0.6 mm smaller suggested margin for the thoracic spine compared to the Type‐S setup, at 1.4 mm, calculated using the analytic approach. Both the Type‐S and BPL1 setups are effective for immobilization in spine SABR. The Type‐S demonstrated superior immobilization in the upper spine and remains the clinical standard for cervical and upper thoracic spine positioning. The BPL1 setup showed effective immobilization in use cases treating the mid‐to lower thoracic spine and lumbar spine and remains our clinical standard for those use cases. Results additionally demonstrate feasibility of potential PTV margin reduction.

Comparing immobilisation devices in gynaecological external beam radiotherapy: improving inter-fraction reproducibility of pelvic tilt

Abstract:Introduction: The aim was to determine which immobilisation device improved inter-fraction reproducibly of pelvic tilt and required the least pre-treatment setup and planning interventions. Methods: Sixteen patients were retrospectively reviewed, eight immobilised using the BodyFIX system (BodyFIX®, Elekta, Stockholm, Sweden) and eight using the Butterfly Board (BB) (Bionix Radiation Therapy, Toledo, OH, USA). The daily pre-treatment images were reviewed to assess setup variations between each patient and groups for pelvic tilt, pubic symphysis, sacral promontory and the fifth lumbar spine (L5). Results: Compared with the planning CT, pelvic tilt for most patients was within ±2° using the BodyFIX and ± 4° for the BB. The Butterfly Board had a slightly higher variance both for patient-to-patient (standard deviation of the systematic error) and day-to-day error (standard deviation of the random error). Variance in position between individual patients and the two stabilisation devices were minimal in the anterior-posterior (AP) and superior-inferior (SI) direction for the pubic symphysis, sacral promontory and L5 spine. Re-imaged fractions due to pelvic tilt reduced by about half when BodyFIX was used (39.1% BB, 19.4% BodyFIX). One patient treated with the BB required a re-scan for pelvic tilt. Three patients required a re-scan for body contour variations (two using BodyFIX and one with the BB). Conclusions: BodyFIX resulted in a more accurate inter-fraction setup and efficient treatment and is used as the standard stabilisation for gynaecological patients at our centre. It reduced the pelvic tilt variance and reduced the need for re-imaging pre-treatment by half.
A Clinical Monte Carlo Dosimetric Evaluation in Accelerated Partial Breast Electronic Brachytherapy

Abstract:Electronic brachytherapy has been recently utilized in accelerated partial breast irradiation. Deficient tissue inhomogeneity correction in the dose calculation draws physicians' attentions. In this study, a clinical Monte Carlo based dose evaluation was assessed to demonstrate these dosimetric impacts. The dosimetric characteristics of Xoft S700 Axxent X-ray source (Xoft, Fremont, CA) were first evaluated using EGS4 Monte Carlo code to verify the calculation model and source parameter accuracy. A whole body female photographic phantom with defined structures derived from National Library of Medicine's Visible Human Project utilizing all clinical parameters was then used in the dose calculation. A 5cm diameter spherical balloon was inserted in the right breast of the phantom. The volume expanded 1cm from the balloon surface was designed to define as planning target volume (PTV). The prescribed dose per fraction was 3.4 Gy to 5 user-selected points (anterior, posterior, right lateral, superior and inferior) of the PTV. Dose, dose volume histograms of the PTV and normal tissues, isodose distributions in axial, sagittal and coronal planes crossing the center of the balloon were evaluated. Using measured 50 kVp source spectrum provided by Xoft Inc, the simulated radial dose function agrees well with the published data within 3%. After corrected for vender addressed 6% balloon attenuation, the PTV dose reported from the commercial treatment planning system is within 1% compared to our calculation result. The simulation shows 100% PTV covered by 90.6% of the prescription dose with excellent dose uniformity. When accounted for the tissue heterogeneity, changes in the isodose distribution were clearly observed. The two dimensional dose distributions show high skin and rib doses due to photoelectric effect with reasonably acceptable doses to all other surrounding normal tissues. Our preliminary EGS4 Monte Carlo simulation study demonstrates the dosimetric impact in the electronic brachytherapy, especially on the surrounding normal tissues, when tissue inhomogeneity consideration is taken into account.

Abstract:INTRODUCTION Development of intraventricular gradients (IVG) on exertion seldom occurs in an unselected population. We performed exercise stress echocardiography (SE) in a 23-year-old athlete with a positive treadmill ECG and normal resting echocardiogram, but without angiographically demonstrable coronary disease (cardiac syndrome X), during which he developed an IVG of 102 mmHg and systolic anterior motion (SAM) of the mitral valve. Subsequently, we performed SE in 91 patients, 44 of whom were women, mean age 51.4 +/- 12.1 years (20-72), under similar circumstances--positive treadmill testing and no angiographically demonstrable coronary disease. In 33 (36%) of these, SE disclosed IVG with a mean end-systolic peak of 86 +/- 34 mmHg (30-165), together with mitral valve SAM in 23. The 20 patients who repeated SE under beta-blocker therapy constitute the study population. Seven (35%) of them were women, mean age 50.7 +/- 13.4 years (23 to 72). OBJECTIVE To assess the effect of beta-blockers on the occurrence of IVG and mitral valve SAM in patients with cardiac syndrome X. METHODS We repeated SE in 20 of the 33 patient that developed IVG on exertion, under beta-blocker therapy. They all underwent 2D and Doppler echocardiographic assessment before, during and after treadmill exercise. RESULTS Mean IVG in the 20 patients in the study population was 971 +/- 31 mmHg on the first SE assessment. In the assessment performed under beta-blocker therapy, 11 did not develop IVG, and in 9 the gradient was significantly reduced, to a mean of 46 +/- 13 mmHg. Mean heart rate at peak exercise was 161 +/- 17 bpm in the first SE assessment and 143 +/- 12 bpm under treatment with beta-blockers (p < 0.0001). Fifteen of these patients showed clinical improvement during follow-up, while no change in clinical status was seen in the other five. CONCLUSIONS In patients with cardiac syndrome X and IVG on exertion, treatment with oral beta-blockers prevented the occurrence of IVG or significantly reduced their magnitude. These changes were associated with significant reduction in heart rate at peak exercise and with clinical improvement in 75% of the study population.
Positioning accuracy for lung stereotactic body radiotherapy patients determined by on-treatment cone-beam CT imaging

Abstract:Stereotactic body radiotherapy for early stage non-small cell lung cancer is an emerging treatment option in the UK. Since relatively few high-dose ablative fractions are delivered to a small target volume, the consequences of a geometric miss are potentially severe. This paper presents the results of treatment delivery set-up data collected using Elekta Synergy (Elekta, Crawley, UK) cone-beam CT imaging for 17 patients immobilised using the Bodyfix system (Medical Intelligence, Schwabmuenchen, Germany). Images were acquired on the linear accelerator at initial patient treatment set-up, following any position correction adjustments, and post-treatment. These were matched to the localisation CT scan using the Elekta XVI software. In total, 71 fractions were analysed for patient set-up errors. The mean vector error at initial set-up was calculated as 5.3 ± 2.7 mm, which was significantly reduced to 1.4 ± 0.7 mm following image guided correction. Post-treatment the corresponding value was 2.1 ± 1.2 mm. The use of the Bodyfix abdominal compression plate on 5 patients to reduce the range of tumour excursion during respiration produced mean longitudinal set-up corrections of -4.4 ± 4.5 mm compared with -0.7 ± 2.6 mm without compression for the remaining 12 patients. The use of abdominal compression led to a greater variation in set-up errors and a shift in the mean value.

Abstract:Introduction: This work presents a method to treat stereotactic body radiation therapy (SBRT) for pancreatic cancer on a magnetic resonance-guided linear accelerator (MR-linac) using daily adaptation, real-time motion monitoring, and abdominal compression. Methods: The motion management and treatment planning process involves a magnetic resonance imaging (MRI) simulation with cine and 3D images, a computed tomography (CT) simulation with a breath-hold CT and a 4DCT, pre-treatment verification and planning MRI, and intrafraction MRI cine images. Results: The results from 26 patients were included in this work. Our motion management process results in consistent motion analysis on the CT simulation, MRI simulation, and each treatment fraction. The liver dome was found to be an overestimate of tumor superior/inferior (SI) motion for most patients. Adding compression reduced SI liver dome motion by 6.2 mm on average. Clinical outcomes are similar to those observed in the literature. Conclusions: In this work, we demonstrate how pancreatic SBRT can be successfully treated on an MR-linac using abdominal compression. This allows for an increased duty cycle compared to gating and/or breath-hold techniques.
SU‐FF‐T‐405: Impact of Tissue Heterogeneities Upon Dose Delivery to Lung Lesions and Comparison of BrainSCAN Treatment Planning System Dose Calculations

Abstract:Purpose: Tissue heterogeneity changes result in a build down/build up of the recoil electron fluence at heterogeneity interfaces. The use of stereotactic radiosurgery beams on small lung lesions can compromise the dose delivery to lesions ranging from 6 mm up through 30 mm in diameter, since the dimensions of the lesions are comparable to the range of the secondary electrons in lung and air. Dose perturbations are evaluated for a range of lesion diameters in lung equivalent phantom material. Method and Materials: Lesion diameters of 6.4, 12.5, 19.7, and 25.4 mm of unit density were inserted into lung equivalent material. The dose distributions were measured with film densitometry and calculated with the dose calculation engine of the ADAC Pinnacle system. The Pinnacle treatment planning system uses the adaptive convolution superposition algorithm, which rigorously reproduces x‐ray beam attenuation, and the electron fluence changes at tissue heterogeneity interfaces. Results: The physical density of the lung equivalent phantom material is 0.30 g/cm 3 in contrast to water and normal tissue at 1.0 g/cm 3 . The spherical tumor phantoms were irradiated with beam margins of 0, 7, 10, and 25 mm. Dose volume histogram analysis was performed to develop a DVH surface map of the dose delivery to each lesion as a function of margin size. It was determined that a 7 mm margin for all lesion diameters delivers a minimum tumor dose of 94 % of the prescribed isocenter dose for all lesion diameters. The DVH analysis and film dosimetry were also compared with the dose calculations of the BrainSCAN planning system. Conclusion: These measurements and calculations confirm the clinical feasibility of stereotactic irradiations of lung lesions with minimal compromise of the tumor dose. Quantification of Dosimetry Improvement With or Without Patient Surface Guidance

Abstract:Purpose: Noncoplanar beams and arcs are routinely used to improve dosimetry for intracranial cases, but their application for extracranial cases has been hampered by the risk of collision. This has led to conservative beam selection whose impact on plan dosimetry has not been previously studied. Methods and materials: A full-body 3-dimensional patient surface was acquired using optical cameras for a single lung patient at the time of computed tomography simulation. Eight stereotactic body radiation therapy (SBRT) plans were created for the patient, with varying degrees of noncoplanarity and deliverability. The plans included volumetric modulated arc therapy and intensity modulated radiation therapy (IMRT) plans ranging from simple, coplanar arcs to multiple noncoplanar arcs and IMRT beams. A total of 70 fields were created across the 8 plans, of which 21 fields were undeliverable with a 5-cm buffer. Organs-at-risk (OARs) metrics including R50, Dmax 2 cm from the PTV, lung V20, and chest wall V30 were evaluated. Five expert SBRT dosimetrists from 5 institutions evaluated field deliverability, with or without the guidance of the clearance map. Results: In the dosimetry evaluation, a clear trend in increasing dosimetric compactness and OAR sparing is observed with increasing plan noncoplanarity. R50, Dmax 2 cm, lung V20, and chest wall V30 decreased 41%, 39%, 43%, and 57%, respectively, from plan 1 (2 coplanar partial arcs) to plan 8 (19 noncoplanar IMRT beams). In the observer tests, the expert dosimetrists' ability to accurately discern beam deliverability because of collision significantly increases with the clearance map. The errors in predicting colliding fields were eliminated using the whole-body surface and clearance map, and the user was able to select fields based on plan quality and patient comfort instead of being overly conservative. Conclusion: The study shows that incorporating a personalized, whole-body clearance map in the treatment planning workflow can facilitate the adoption of noncoplanar beams or arcs that benefit the SBRT plan dosimetry.
Impact of belly board immobilization devices and body mass factor on setup displacement using daily cone‐beam CT in rectal cancer radiotherapy

Abstract:Objective The objective of this study is to evaluate the impact of different belly board and daily changes in patient's body‐mass factor (BMF) on setup displacement in radiotherapy for rectal cancer. Methods Twenty‐five patients were immobilized using the thermoplastic mask with belly board (TM‐BB), and 30 used the vacuum bag cushion with belly board (VBC‐BB), performing daily cone‐beam computed tomography (CBCT) scans 625 times and 750 times, respectively. Daily pretreatment CBCT scans were registered to the planned CT images for BMF change determination and setup displacement measurement. Independent t‐tests compared setup displacement between the two groups in left‐right (LR), superior‐inferior (SI), and anterior‐posterior (AP) directions, as well as the BMF changes. The impact of daily BMF changes on setup displacement was evaluated using multivariate logistic regression and 10‐fold cross‐validation. Results The setup displacement for TM‐BB in the LR, SI, and AP directions were 0.31 ± 0.25, 0.58 ± 0.40, and 0.19 ± 0.18 cm, respectively, while VBC‐BB showed 0.19 ± 0.15, 0.26 ± 0.22, and 0.36 ± 0.29 cm in the corresponding directions, respectively. Margins of planning target volume (PTV) for TM‐BB were 8, 10, and 6 mm in LR, SI, and AP directions, while VBC‐BB showed margins of 5,7, and 8 mm, respectively. The daily BMF changes for both groups were ranked in descending order as follows: sacral rotation angle (RS), hip lateral diameter (HLD), and hip anterior‐posterior diameter (HAPD). HAPD was the main factor affecting setup displacement in both the AP and SI directions in TM‐BB, while RS was the primary factor for setup displacement in the AP direction in VBC‐BB. Conclusion Compared with TM‐BB, VBC‐BB had a larger AP displacement but smaller in LR and SI displacement. Daily changes in BMF have distinct effects on setup displacement in different immobilization devices. Image‐guided radiation therapy (IGRT) is highly recommended and BMF changes should be given consideration during radiotherapy.

Abstract:Background: Stereotactic body radiation therapy is an advanced technique, which delivers ablative doses to lung lesions. Target verification is done either by orthogonal x-rays or cone beam CT. This study was undertaken to compare these two verification methods. Aim: To evaluate the efficacy of ExacTrac and Cone Beam Computed Tomography (CBCT) for target repositioning while delivering Stereotactic Body Radiation Therapy (SBRT) for lung lesions and derive the population-based margin. Materials and methods: All patients who had undergone SBRT for lung lesions from February to September 2009 were involved. Patients were immobilized using the BodyFix double vacuum immobilization system, indexed to the computed tomography (CT) simulator and treatment machine. Four-dimensional (3-D) scan was done to generate internal target volume (ITV) and a free breathing CT scan for planning was done on the BrainLab iPlan 4.1 software. During treatment, patient's position was verified using ExacTrac and CBCT. The resulting vertical, lateral, and longitudinal shifts were noted. The random and systematic error were calculated and the margin recipe derived using the Van Herk formula. Results: Sixteen patients had undergone SBRT for lung tumors from February to September 2009. Data from eight patients who had undergone 34 sessions of SBRT was analyzed. The systematic error for lateral, longitudinal, and vertical shifts for ExacTrac and CBCT were 3.68, 4.27, 3.5 mm and 0.53, 0.38, 0.70 mm, respectively. The random error were 1.10, 1.51, 1.96 mm and 0.32, 0.81, 0.59 mm. The lateral, longitudinal and vertical Van Herk margin recipe for ExacTrac were 9.98, 11.72, 10.18 mm, respectively, and for CBCT was 2.17, 1.53,1.55 mm. Conclusions: The systematic and random errors for CBCT were significantly lesser as compared to the errors with Exactrac.

Abstract:ABSTRACT Objectives: The objective of the study is to analyze the difference in target dose distributions between Acuros XB (AXB) and collapsed cone convolution (CCC)/superposition and the impact of the tumor locations in clinical cases of stereotactic ablative body radiotherapy (SABR) for lung cancer. Materials and Methods: Ninety-six patients underwent SABR for lung cancers Kyushu University Hospital from 2014 to 2017. We recalculated clinical plans originally calculated by AXB using CCC with the identical monitor units (MUs) and beam arrangements. We calculated the following dosimetric parameters: maximum dose (Dmax), minimum dose (Dmin), homogeneity index (HI), conformity index (CI), and D95 of the planning target volume (PTV). We investigated the difference between the results of two calculations and examined the impact of tumor location. Moreover, we determined the target central dose using a thorax phantom and assessed the calculation accuracy of the two algorithms for each fraction. Results: CCC significantly overestimated the dose to PTV, compared to AXB ( P < 0.05). The mean differences of Dmax, Dmin, and D95 were 1.17, 1.95, and 1.85 Gy, respectively. The mean differences of HI and CI were 0.02 and − 0.06. Dmin, HI, and D95 had significant correlations with the tumor location, and the difference was greater when the PTV was included the chest wall ( P < 0.05). The discrepancy between the calculated and irradiated dose was 2.48% for CCC, whereas it was 0.14% for AXB. Conclusions: We demonstrated that CCC significantly overestimated the dose to PTV relative to AXB in clinical cases of lung SABR.

Abstract:Purpose To evaluate the intra‐fractional tumor motion in lung stereotactic body radiotherapy (SBRT) with deep inspiration breath‐hold (DIBH), and to investigate the adequacy of the current planning target volume (PTV) margins. Methods Twenty‐eight lung SBRT patients with DIBH were selected in this study. Among the lesions, twenty‐three were at right or left lower lobe, two at right middle lobe, and three at right or left upper lobe. Post‐treatment gated cone‐beam computed tomography (CBCT) was acquired to quantify the intra‐fractional tumor shift at each treatment. These obtained shifts were then used to calculate the required PTV margin, which was compared with the current applied margin of 5 mm margin in anterior‐posterior (AP) and right‐left (RL) directions and 8 mm in superior‐inferior (SI) direction. The beam delivery time was prolonged with DIBH. The actual beam delivery time with DIBH (Tbeam_DIBH) was compared with the beam delivery time without DIBH (Tbeam_wo_DIBH) for the corresponding SBRT plan. Results A total of 113 treatments were analyzed. At six treatments (5.3%), the shifts exceeded the tolerance defined by the current PTV margin. The average shifts were 0.0 ± 1.9 mm, 0.1±1.5 mm, and ‐0.5 ± 3.7 mm in AP, RL, and SI directions, respectively. The required PTV margins were determined to be 4.5, 3.9, and 7.4 mm in AP, RL, and SI directions, respectively. The average Tbeam_wo_DIBH and Tbeam_DIBH were 2.4 ± 0.4 min and 3.6 ± 1.5 min, respectively. The average treatment slot for lung SBRT with DIBH was 25.3 ± 7.9 min. Conclusion Intra‐fractional tumor motion is the predominant source of treatment uncertainties in CBCT‐guided lung SBRT with DIBH. The required PTV margin should be determined based on data specific to each institute, considering different techniques and populations. Our data indicate that our current applied PTV margin is adequate, and it is possible to reduce further in the RL direction. The time increase of Tbeam_DIBH, relative to the treatment slot, is not clinically significant.
