Abstract:To evaluate the hemocompatibility of individual components of our pediatric left ventricular assist device (LVAD), we proposed a hemocompatibility assessment platform (HAP) with a magnetic levitated bearing system. The HAP consists of a drive system utilizing a brushless direct current (BLDC) motor, passive magnetic bearings (PMB), and an active magnetically levitated bearing (AMB) to reduce the hemolysis generated by HAP itself. In this study, we designed and evaluated the performance of the AMB by measuring radial and axial displacements of the rotor resulting from radially destabilizing forces as well as the performance of the drive system when rotated at increasing speeds to 1,200 rotations per minute (rpm). The results show that, with radial disturbance, the AMB is capable of maintaining axial stability for the BLDC motor system. The AMB can control up to 1,200 rpm without any contact between the rotor and stator. Future work includes geometry optimization for the AMB structure and increase the capability to control stable high-speed rotation for the entire system. Clinical Relevance- This work furthers the development of the magnetic levitated bearing system for a hemocompatibility assessment platform that will be used to enhance and accelerate the development of adult and pediatric LVADs.
Incidence of Pseudomonas aeruginosa serogroups in drinking water: use of serotyping in the control of water supplies.

Abstract:From 26,568 drinking water samples collected in the years 1977-1981 from deep well supplies in Csongrád county, Hungary, 1269 Pseudomonas aeruginosa strains were isolated. The most frequent serogroups (Lányi-Bergan scheme) were O6 (9.5-69.2%), O1 (5.5-35.5%) and O9 (2.2-36.2%). Predominance of a given serogroup varied with, and was characteristic of geographic areas of the county, water plants and distribution systems. Bacteriological monitoring has shown that serotyping of P. aeruginosa may be useful for evaluating the mechanical condition of the water supply.

Abstract:BackgroundA percutaneous ventricular assist device (pVAD) is an effective method to treat heart failure, but its complications, mainly hemolysis and thrombus formation, cannot be ignored. Accurate evaluation of hemolysis and thrombus formation in pVAD is essential to guide the development of pVAD and reduce the incidence of complications.MethodsThis study optimized the numerical model to predict hemolysis and thrombus formation in pVAD. The hemolysis model is based on the power law function, and the multi-component thrombus prediction model is improved by introducing the von Willebrand factor.ResultsThe error between the numerical simulation and the hydraulic performance experiment is within 5%. The numerical results of hemolysis are in good agreement with those of in vitro experiments. Meanwhile, the thrombus location predicted by the numerical model is the same as that found in the in vivo experiment.ConclusionThe numerical model suggested in this study may therefore accurately assess the possible hemolytic and thrombotic dangers in pVAD, making it an effective tool to support the development of pVAD. A hemolysis model for the pVAD has been improved through previous studies and the in vitro hemolysis experiment. Meanwhile, the improved 11-component thrombus simulation model well predicts the location of thrombus generation in the pVAD. The numerical models help evaluate the hemodynamic performance of the pVAD during the design stage.image
Hemocompatibility Assessment Platform Drive System Design: Trade-off between Motor Performance and Hemolysis

Abstract:Left ventricular assist devices (LVADs) have long been used to treat adults with heart failure, but LVAD options for pediatric patients with heart failure are lacking. Despite the urgent need for long-term, implantable pediatric LVADs, design challenges such as hemolysis, pump thrombosis, and bleeding persist. We have developed a Hemocompatibility Assessment Platform (HAP) to identify blood trauma from individual LVAD components. A HAP would aid in refining pump components before in vivo testing, thereby preventing unnecessary animal sacrifice and reducing development time and cost. So that the HAP does not confound hemolysis data, the HAP drive system consists of an enlarged air-gap motor coupled to a magnetic levitation system. Although it is known that an enlarged air gap motor will have diminished performance, while the larger gap in the motor will cause less blood damage, the trade-offs are not fully characterized. Therefore, in this study we evaluated these trade-offs to determine an optimal rotor diameter for the HAP drive motor. The motor performance was characterized with an experimental method by determining the torque constant for the HAP drive motor with varied rotor diameters. The torque threshold was set as 10 mNm to achieve a nominal current of 3.5A. Hemolysis in the HAP drive motor gap was estimated by calculating scalar shear stress generated in the HAP motor gap analytically and numerically. A design criterion of 30 Pa was selected for scalar shear stress to achieve minimal hemolysis and platelet activation in the HAP drive system.Clinical Relevance- We evaluated a Hemocompatibility Assessment Platform for developing LVAD prototypes that can best balance motor performance and hemocompatibility. This design method can assist with optimizing the drive system during the research stage and illustrates how motor geometry can be tuned to reduce blood trauma.

Abstract:Band selection, as a special case of the feature selection problem, tries to remove redundant bands and select a few important bands to represent the whole image cube. This has attracted much attention, since the selected bands provide discriminative information for further applications and reduce the computational burden. Though hyperspectral band selection has gained rapid development in recent years, it is still a challenging task because of the following requirements: 1) an effective model can capture the underlying relations between different high-dimensional spectral bands; 2) a fast and robust measure function can adapt to general hyperspectral tasks; and 3) an efficient search strategy can find the desired selected bands in reasonable computational time. To satisfy these requirements, a multigraph determinantal point process (MDPP) model is proposed to capture the full structure between different bands and efficiently find the optimal band subset in extensive hyperspectral applications. There are three main contributions: 1) graphical model is naturally transferred to address band selection problem by the proposed MDPP; 2) multiple graphs are designed to capture the intrinsic relationships between hyperspectral bands; and 3) mixture DPP is proposed to model the multiple dependencies in the proposed multiple graphs, and offers an efficient search strategy to select the optimal bands. To verify the superiority of the proposed method, experiments have been conducted on three hyperspectral applications, such as hyperspectral classification, anomaly detection, and target detection. The reliability of the proposed method in generic hyperspectral tasks is experimentally proved on four real-world hyperspectral data sets.

Abstract:A mock circulatory system (MCS) has been proven a useful tool in the development of a ventricular assist device. Nowadays a MCS aimed at the evaluation of pediatric blood pumps, which require many different considerations compared with that of adults, has become an urgent need. This article presents the details on how the dynamic process of the left ventricle, which is described in terms of the pressure-volume loop (P-V loop), and the properties of the circulation such as compliance and resistance are simulated by hydraulic elements. A simple control method is introduced to reproduce the physiological afterload and preload sensitivities of the mock ventricle for the first time. Hemodynamic performance of the system is obtained by medical sensors to validate the similarity of the device to the native cardiovascular system. The actual sensitivities of the mock ventricle are obtained intuitively from the changes of the P-V loops. The aortic input impedance of the MCS is also obtained and compared with the data from previous medical reports. At last a pediatric left ventricular assist device (LVAD) prototype is introduced for testing to further verify the effectiveness of the MCS. The experimental results indicate that this pediatric MCS is capable of reproducing basic hemodynamic characteristics of a child in both normal and pathological conditions and it is sufficient for testing a pediatric LVAD. Besides, most components constituting the main hydraulic part of this MCS are inexpensive off-the-shelf products, making the MCS easy and fast to build.
Development of Inspired Therapeutics Pediatric VAD: Benchtop Evaluation of Impeller Performance and Torques for MagLev Motor Design

Abstract:Purpose: Despite the availability of first-generation extracorporeal mechanical circulatory support (MCS) systems that are widely used throughout the world, there is a need for the next generation of smaller, more portable devices (designed without cables and a minimal number of connectors) that can be used in all in-hospital and transport settings to support patients in heart failure. Moreover, a system that can be universally used for all indications for use including cardiopulmonary bypass (CPB), uni- or biventricular support (VAD), extracorporeal membrane oxygenation (ECMO) and respiratory assist that is suitable for use for adult, neonate, and pediatric patients is desirable. Providing a single, well designed, universal technology could reduce the incidence of human errors by limiting the need for training of hospital staff on a single system for a variety of indications throughout the hospital rather than having to train on multiple complex systems. The objective of this manuscript is to describe preliminary research to develop the first prototype pump for use as a ventricular assist device for pediatric patients with the Inspired Universal MCS technology. The Inspired VAD Universal System is an innovative extracorporeal blood pumping system utilizing novel MagLev technology in a single portable integrated motor/controller unit which can power a variety of different disposable pump modules intended for neonate, pediatric, and adult ventricular and respiratory assistance. Methods: A prototype of the Inspired Pediatric VAD was constructed to determine the hemodynamic requirements for pediatric applications. The magnitude/range of hydraulic torque of the internal impeller was quantified. The hydrodynamic performance of the prototype pump was benchmarked using a static mock flow loop model containing a heated blood analogue solution to test the pump over a range of rotational speeds (500-6000 RPM), flow rates (0-3.5 L/min), and pressures (0 to ~ 420 mmHg). The device was initially powered by a shaft-driven DC motor in lieu of a full MagLev design, which was also used to calculate the fluid torque acting on the impeller. Results: The pediatric VAD produced flows as high as 4.27 L/min against a pressure of 127 mmHg at 6000 RPM and the generated pressure and flow values fell within the desired design specifications. Conclusions: The empirically determined performance and torque values establish the requirements for the magnetically levitated motor design to be used in the Inspired Universal MagLev System. This next step in our research and development is to fabricate a fully integrated and functional magnetically levitated pump, motor and controller system that meets the product requirement specifications and achieves a state of readiness for acute ovine animal studies to verify safety and performance of the system.
Development of a prototype magnetically suspended rotor ventricular assist device

Abstract:A continuous flow centrifugal blood pump with magnetically suspended impeller has been designed, constructed, and tested. The system can be functionally divided into three subsystem designs: 1) centrifugal pump and flow paths, 2) magnetic bearings, and 3) brushless DC motor. The centrifugal pump is a Francis vane type design with a designed operating point of 6 L/min flow and 100 mmHg pressure rise at 2,300 RPM. Peak hydraulic efficiency is over 50%. The magnetic bearing system is an all active design with five axes of control. Rotor position sensors were developed as part of the system to provide feedback to a proportional-integral-derivative controller. The motor is a sensorless brushless DC motor. Back electromotive force voltage generated by the motor is used to provide commutation for the motor. No slots are employed in the motor design in order to reduce the radial force that the bearings must generate. Tests pumping blood in vitro were very encouraging; an index of hemolysis of 0.0086 +/- 0.0012 was measured. Further design refinement is needed to reduce power dissipation and size of the device. The concept of using magnetic bearings in a blood pump shows promise in a long-term implantable blood pump.
A magnetic bearing system for a continuous ventricular assist device

Abstract:This article describes a prototype continuous flow ventricular assist device (CFVAD3) supported in magnetic bearings. The VAD is a small centrifugal four bladed pump. The pump's geometry is explained. The CFVAD3 is the first compact VAD completely supported in magnetic bearings. The magnetic bearings are composed of an inlet side actuator divided into eight pole sets, and an outlet side actuator, also divided into eight pole sets. The pump operating performance was tested and found to be within the design flow rate of up to 9 L/min, and head up to 170 mm Hg for human circulatory support. Magnetic bearing operation out of center positions under various operating orientations were measured and found to be < 1/6 of the bearing clearance, well within specifications. The expected magnetic bearing power loss has been calculated at approximately 6.5 watts.
Computational characterization of flow and blood damage potential of the new maglev CH-VAD pump versus the HVAD and HeartMate II pumps

Abstract:Left ventricular assist devices are routinely used to treat patients with advanced heart failure as a bridge to transplant or a destination therapy. However, non-physiological shear stress generated by left ventricular assist devices damages blood cells. The continued development of novel left ventricular assist devices is essential to improve the left ventricular assist device therapy for heart failure patients. The CH-VAD is a new maglev centrifugal left ventricular assist device. In this study, the CH-VAD pump was numerically analyzed and compared with the HVAD and HeartMate II pumps under two clinically relevant conditions (flow: 4.5 L/min, pressure head: normal ~80 and hypertension ~120 mmHg). The velocity and shear stress fields, washout, and hemolysis index of the three pumps were assessed with computational fluid dynamics analysis. Under the same condition, the CH-VAD hemolysis index was two times lower than the HVAD and HeartMate II pumps; the CH-VAD had the least percentage volume with shear stress larger than 100 Pa (i.e. normal condition: 0.4% vs HVAD 1.0%, and HeartMate II 2.9%). Under the normal condition, more than 98% was washed out of the three pumps within 0.4 s. The washout times were slightly shorter under the hypertension condition for the three pumps. No regions inside the CH-VAD or HVAD had extremely long residential time, while areas near the straightener of the HeartMate II pump had long residential time (>4 s) indicating elevated risks of thrombosis. The computational fluid dynamics results suggested that the CH-VAD pump has a better hemolytic biocompatibility than the HVAD and HeartMate II pumps under the normal and hypertension conditions.

Abstract:A new centrifugal continuous flow ventricular assist device, the CFVAD III, which is fully magnetic bearing suspended, has been developed. It has only one moving part (the impeller), has no contact (magnetic suspension), is compact, and has minimal heating. A centrifugal impeller of 2 inch outer diameter is driven by a permanent magnet brushless DC motor. This paper discusses the design, construction, testing, and performance of the magnetic bearings in the unit. The magnetic suspension consists of an inlet side magnetic bearing and an outlet side magnetic bearing, each divided into 8 pole segments to control axial and radial displacements as well as angular displacements. The magnetic actuators are composed of several different materials to minimize size and weight while having sufficient load capacity to support the forces on the impeller. Flux levels in the range of 0.1 T are employed in the magnetic bearings. Self sensing electronic circuits (without physical sensors) are employed to determine the impellar position and provide the feedback control signal needed for the magnetic bearing control loops. The sensors provide position sensitivity of approximately 0.025 mm. A decentralized 5 axis controller has been developed using modal control techniques. Proportional integral derivative controls are used for each axis to levitate the magnetically supported impeller.
CFD-Based Flow Channel Optimization and Performance Prediction for a Conical Axial Maglev Blood Pump

Abstract:Ventricular assist devices or total artificial hearts can be used to save patients with heart failure when there are no donors available for heart transplantation. Blood pumps are integral parts of such devices, but traditional axial flow blood pumps have several shortcomings. In particular, they cause hemolysis and thrombosis due to the mechanical contact and wear of the bearings, and they cause blood stagnation due to the separation of the front and rear guide wheel hubs and the impeller hub. By contrast, the implantable axial flow, maglev blood pump has the characteristics of no mechanical contact, no lubrication, low temperature rise, low hemolysis, and less thrombosis. Extensive studies of axial flow, maglev blood pumps have shown that these pumps can function in laminar flow, transitional flow, and turbulent flow, and the working state and performance of such pumps are determined by their support mechanisms and flow channel. Computational fluid dynamics (CFD) is an effective tool for understanding the physical and mechanical characteristics of the blood pump by accurately and effectively revealing the internal flow field, pressure-flow curve, and shear force distribution of the blood pump. In this study, magnetic levitation supports were used to reduce damages to the blood and increase the service life of the blood pump, and a conical impeller hub was used to reduce the speed, volume, and power consumption of the blood pump, thereby facilitating implantation. CFD numerical simulation was then carried out to optimize the structural parameters of the conical axial maglev blood pump, predict the hemolysis performance of the blood pump, and match the flow channel and impeller structure. An extracorporeal circulation simulation platform was designed to test whether the hydraulic characteristics of the blood pump met the physiological requirements. The results showed that the total pressure distribution in the blood pump was reasonable after optimization, with a uniform pressure gradient, and the hemolysis performance was improved.
Numerical solution for blood flow in a centrifugal ventricular assist device

Abstract:A very small centrifugal pump, fully supported by magnetic bearings, is being developed for use as a ventricular assist device to be implanted in humans. In this paper, we apply computational fluid dynamics to model the blood flow to aid in the design of the ventricular assist device. The flow of blood through the pump has been modeled using computational fluid dynamics (CFD) software that is commercially available from AEA Technology, UK. The flow regions modeled in version 3 of the Continuous Flow Ventricular Assist Device (CF3) are the fully shrouded four bladed impeller and the two clearance regions around the impeller that are bounded by the pump hub and shroud. This paper describes the geometry and computational grids developed for the flow regions, and the equations of motion for the blood flow are developed. The overall numerically-evaluated flow rates and head rise have similar trends to the flow parameters experimentally measured, indicating that future pump designs can be effectively modeled numerically before being constructed and tested. Numerical solutions are presented and compared with experimentally-obtained overall pump performance results. These solutions are used to predict shear stress levels to be experienced by the blood flowing through the pump, and it is predicted that hemolysis will be insignificant. The solutions also indicate no regions of flow stagnation that can be a source of thrombosis in pumps. The calculations provide a viable design method to achieve improved efficiency in future versions of this pump.

Abstract:In this paper, adaptive proportional derivative (APD) parameter control was proposed to solve the problem of high power consumption caused by the unclear mechanism of liquid disturbance during the lifting-up of magnetic bearing in left ventricular assist devices. A mathematical model was derived that describes how the rotor operates in liquid filling. The disturbance caused by the liquid in the lifting-up process was analyzed, and an adaptive control system was developed to improve dynamic performance and reduce power consumption. The experimental results show that APD control requires a shorter rise time without overshoot of rotor displacement compared to traditional fixed configurations. When using the APD controller, the peak current dropped by 8%. The duration in which the current is greater than 1A was reduced by 10.2 ms, and the average current also dropped by 34%.

Abstract:Health promotion is a set of strategies for positively influencing health through a range of individual, community-based, and population interventions. Despite international recognition that gender is a primary determinant of health and that gender roles can negatively affect health, the health promotion field has not yet articulated how to integrate gender theoretically or practically into its vision. For example, interventions often fail to critically consider women's or men's diverse social locations, gender-based power relations, or sex-based differences in health status. Yet without such analyses, interventions can result in the accommodation or exploitation of gender relations that disadvantage women and compromise their health. In this paper, we seek to ignite an agenda for health promotion for women. We discuss the need for a conceptual framework that includes a sex-gender-diversity analysis and critically considers 'what counts' as health promotion to guide the development and implementation of evidence-based practice. We also consider how innovative knowledge translation practices, technology developments and action research can advance this agenda in ways that foster the participation of a wide range of stakeholders.

Abstract:Abstract Ventricular assist devices (VAD) are designed to provide circulatory support to patients suffering from advanced-stage heart failure. While not pulsatile, the advantages of continuous axial flow VADs include a compact size and low mechanical failure rate. However, being compact, they operate at high speed, resulting in adverse effects, such as hemolysis caused by high flow shear and thrombosis formation in stagnant regions are common and threaten the successful use of the device. While state-of-the-art computational fluid dynamics (CFD) is widely used in designing these devices, detailed high-resolution experimental measurements of the flow within them are not readily available in the literature. Such experimental data is crucial for understanding the flow inside the VAD and its interaction with blood cells as well as for validating the CFD predictions. The present study investigates the flow inside a 1:1 exact replica of a VAD – ReliantHeart HeartAssist5®. This 12mm diameter device consists of an inlet guide vane (IGV), a rotor and a stator. However, unlike in the real machine, the rotor is driven by a thin shaft that penetrates through the center of the IGV. Refractive index-matching is used to facilitate optical measurements. Hence, all the blades and housing of the pump are made of transparent acrylic. The working fluid is a mixture of water, sodium iodide and glycerin, which matches the refractive index of acrylic, and the kinematic viscosity of blood. Performance tests have been carried out at speeds ranging from 7000 to 9000 RPM. Trends of the results are consistent with those of the actual machine. While scaled data for the pressure rise across the rotor at different speeds collapse, the total head rise across the entire machine does not. High-resolution 2D PIV measurements with vector spacing of 30μm have been conducted in meridional planes of the rotor passage at several blade orientations. They have been performed at 8000RPM and flowrate of 4.5L/min, consistent with physiological requirements. They show that near the rotor front end, the flow in the tip region is dominated by the tip leakage vortex (TLV), associated blockage effects, and very high turbulence level. The upstream end of this domain remains aligned with the leading edge, implying that blades persistently dissect the remnants of a previous TLVs. Interaction of the hub boundary layer with the blades also generates secondary flows. Downstream of the helical section of the rotor, the axial flow is high near the hub and low in the outer part of the passage due to leakage flow-related blockage. Hub boundary layer separation occurs upstream of the stator, generating a large circumferential vortex that occupies nearly half of the rotor span. The complexity of the flow structure and turbulence in this machine would be a challenge to model.
Comparison and experimental validation of fluid dynamic numerical models for a clinical ventricular assist device

Abstract:With the recent advances in computer technology, computational fluid dynamics (CFDs) has become an important tool to design and improve blood-contacting artificial organs, and to study the device-induced blood damage. Commercial CFD software packages are readily available, and multiple CFD models are provided by CFD software developers. However, the best approach of using CFD effectively to characterize fluid flow and to predict blood damage in these medical devices remains debatable. This study aimed to compare these CFD models and provide useful information on the accuracy of each model in modeling blood flow in circulatory assist devices. The laminar and five turbulence models (Spalart-Allmaras, k-ε (k-epsilon), k-ω (k-omega), SST [Menter's Shear Stress Transport], and Reynolds Stress) were implemented to predict blood flow in a clinically used circulatory assist device, the CentriMag centrifugal blood pump. In parallel, a transparent replica of the CentriMag pump was constructed and selected views of the flow fields were measured with digital particle image velocimetry (DPIV). CFD results were compared with the DPIV experimental results. Compared with the experiment, all the selected CFD models predicted the flow pattern fairly well except the area of the outlet. However, quantitatively, the laminar model results were the most deviated from the experimental data. On the other hand, k-ε renormalization group theory models and Reynolds Stress model are the most accurate. In conclusion, for the circulatory assist devices, turbulence models provide more accurate results than the laminar model. Among the selected turbulence models, k-ε and Reynolds Stress Method models are recommended.
