Abstract:Abstract Ventricular assist devices (VAD) are designed to provide circulatory support to patients suffering from advanced-stage heart failure. While not pulsatile, the advantages of continuous axial flow VADs include a compact size and low mechanical failure rate. However, being compact, they operate at high speed, resulting in adverse effects, such as hemolysis caused by high flow shear and thrombosis formation in stagnant regions are common and threaten the successful use of the device. While state-of-the-art computational fluid dynamics (CFD) is widely used in designing these devices, detailed high-resolution experimental measurements of the flow within them are not readily available in the literature. Such experimental data is crucial for understanding the flow inside the VAD and its interaction with blood cells as well as for validating the CFD predictions. The present study investigates the flow inside a 1:1 exact replica of a VAD – ReliantHeart HeartAssist5®. This 12mm diameter device consists of an inlet guide vane (IGV), a rotor and a stator. However, unlike in the real machine, the rotor is driven by a thin shaft that penetrates through the center of the IGV. Refractive index-matching is used to facilitate optical measurements. Hence, all the blades and housing of the pump are made of transparent acrylic. The working fluid is a mixture of water, sodium iodide and glycerin, which matches the refractive index of acrylic, and the kinematic viscosity of blood. Performance tests have been carried out at speeds ranging from 7000 to 9000 RPM. Trends of the results are consistent with those of the actual machine. While scaled data for the pressure rise across the rotor at different speeds collapse, the total head rise across the entire machine does not. High-resolution 2D PIV measurements with vector spacing of 30μm have been conducted in meridional planes of the rotor passage at several blade orientations. They have been performed at 8000RPM and flowrate of 4.5L/min, consistent with physiological requirements. They show that near the rotor front end, the flow in the tip region is dominated by the tip leakage vortex (TLV), associated blockage effects, and very high turbulence level. The upstream end of this domain remains aligned with the leading edge, implying that blades persistently dissect the remnants of a previous TLVs. Interaction of the hub boundary layer with the blades also generates secondary flows. Downstream of the helical section of the rotor, the axial flow is high near the hub and low in the outer part of the passage due to leakage flow-related blockage. Hub boundary layer separation occurs upstream of the stator, generating a large circumferential vortex that occupies nearly half of the rotor span. The complexity of the flow structure and turbulence in this machine would be a challenge to model.
Left Ventricular Hemodynamics with an Implanted Assist Device: An In Vitro Fluid Dynamics Study

Abstract:Left ventricle assist devices (VADs) aid the heart pumping blood into the systemic circulation and grant the required cardiac output (CO) when the heart itself cannot provide it. However, it is unclear how effective these devices are at restoring not only physiological CO values but also normal intraventricular hemodynamics. In this work, the modified hemodynamics due to a VAD implantation is studied <em>in vitro</em> using an elastic ventricle made of silicone, which is incorporated into a pulse-duplicator setup prescribing a realistic pulsatile flow. Thereafter, a continuous axial pump is connected at the ventricle apex to mimic a VAD and its effect on the ventricular hemodynamics is investigated as a function of the pump flow suction. Using particle image velocimetry (PIV), we observe that the continuous pump flow effectively provides unloading on the ventricle and yields an increased CO. Conversely, the continuous blood suction from the ventricle apex deeply alters the hemodynamics and, in addition, the VAD obstruction in the ventricle behaves as a bluff body that affects the vorticity distribution in the LV thus creating a stagnant region at the ventricle apex. This phenomenon is rationalized by measuring in a modified set-up the benefits on the hemodynamics of a flush-mounted device. Additionally, the suction operated by the VAD reduces the ventricular pressure and yields an increase in the swirling motion around the ventricle axis, in a similar fashion as the bath-tub vortex effect, thus further modifying the intraventricular hemodynamics with respect to healthy conditions.

Abstract:Severe right heart failure, often overlooked and challenging to manage, has prompted a growing interest in innovative approaches to provide functional support. This study uses experimentation in large porcine models to introduce a novel prototype of a pulsatile mechanical circulatory support device and document its effects when deployed as a right ventricular assist device (RVAD). The pulsatile ventricular assist platform (pVAP), featuring a membrane pump driven by an intra-aortic balloon pump console, actively generates pulsatile flow to propel right ventricular blood into the pulmonary artery. This novel prototype demonstrates promising potential in addressing the challenges of right heart failure management. After preliminary in vitro assessments, the pVAP was tested on seven porcine models in a healthy state and after the induction of right ventricular failure. During the procedure, a set of standard ( ie , standard-of-care) hemodynamic measurements was obtained. Additionally, invasive pressure-volume loop analysis was employed to examine left ventricular hemodynamics. Results indicated that activation of the pVAP during right ventricular failure significantly improved systemic hemodynamics and enhanced left ventricular function. This study sheds light on the potential of the pVAP in managing right heart failure.

Experimental Study on a New Device for Left Ventricular Assist: Dynamic Aortic Valve

Abstract:Objective Prototype of a novel device, previously named "Dynamic Aortic Valve (DAV)", for left ventricular assistance was fabricated and tested to demonstrate its feasibility. Methods An impeller similar to an airscrew is to be placed in the position of aortic valve .The impeller is held by a cylindrical supporting cage locating in the lumen of arising aorta by attaching to the valve annulus via a sewing ring. A permanent magnetic rotor is fitted into the cage and fixed to the impeller to form a “rotor-impeller”. The supporting cage would port the aortic wall thus prevent the rotor from connecting to the wall, allowing the rotor to whirl freely. The alternating magnetic field generated by a extracorporeal device would drive the rotor to rotate and the axial flow of blood from ventricular to the aorta would be produced, resulting in the reduction of the ventricular load. Based on such hypotheses, A prototype of “DAV” was fabricated and tested in a mock circuit. In the present stage, The rotor is 8 mm in diameter and 15 mm in length, the impeller has 3 vanes with a diameter of 18 mm. The supporting cage is 22 mm in outer diameter and 20 mm in length. The alternating magnetic field turning the“"rotor-impeller” was produced by a rotating magnet placed side by side with the “rotor-impeller” at a distance of 15 mm.Result The prototype was fitted into a tube, simulating the aorta, full of water solution of glycerol. When actuated by magnetic force, the “rotor-impeller” of DAV started to rotate, producing a pressure difference and liquid flow in the tube. The flow rate or pressure difference proportioned to rotational speed. At the state of maximal output of hydraulic power, the flow rate reaches 5 L/min against a afterload of 6.65 kPa.Conclusion The maximal pressure differential maintained by the DAV prototype at 12 000 r/min is 10.64 kPa. The results demonstrated the feasibility of the concept of DAV, suggesting that further researches on this concept are justifiable.
CFD analysis of the HVAD's hemodynamic performance and blood damage with insight into gap clearance

Abstract:Mechanical circulatory support using ventricular assist devices has become commonplace in the treatment of patients suffering from advanced stages of heart failure. While blood damage generated by these devices has been evaluated in depth, their hemodynamic performance has been investigated much less. This work presents the analysis of the complete operating map of a left ventricular assist device, in terms of pressure head, power and efficiency. Further investigation into its hemocompatibility is included as well. To achieve these objectives, computational fluid dynamics simulations of a centrifugal blood pump with a wide-blade impeller were performed. Several conditions were considered by varying the rotational speed and volumetric flow rate. Regarding the device's hemocompatibility, blood damage was evaluated by means of the hemolysis index. By relating the hemocompatibility of the device to its hemodynamic performance, the results have demonstrated that the highest hemolysis occurs at low flow rates, corresponding to operating conditions of low efficiency. Both performance and hemocompatibility are affected by the gap clearance. An innovative investigation into the influence of this design parameter has yielded decreased efficiencies and increased hemolysis as the gap clearance is reduced. As a further novelty, pump operating maps were non-dimensionalized to highlight the influence of Reynolds number, which allows their application to any working condition. The pump's operating range places it in the transitional regime between laminar and turbulent, leading to enhanced efficiency for the highest Reynolds number.
Evaluation of the Hemolysis and Fluid Dynamics of a Ventricular Assist Device under the Pulsatile Flow Condition

Abstract:When the rotary blood pumps are used as ventricular assist devices, the pump flow rate will have a pulsatile component even at a constant impeller rotational speed due to the remaining beating of the natural heart. However, previous studies on the in vitro hemolysis evaluation of a rotary blood pump have always been conducted under steady states and this pulsation was not taken into account. In this study, the hemolysis in a centrifugal blood pump is evaluated under the pulsatile flow condition in vitro. The required time-varying flow rate is obtained by conducting a system simulation of the pump-assisted cardiovascular system, and realized by controlling a pulsation unit in the experiments. The results of our tests indicate a significant increase in hemolysis under the pulsatile flow condition compared with the non-pulsatile condition. To reveal the flow characteristics responsible for the higher hemolysis, transient computational fluid dynamic simulations are then performed. This study suggests that traditional hemolysis evaluation under the steady states may not fully represent the hemolytic performance in the clinical use. For the ventricular assist pumps at the design stage, eliminating the concern about the extra hemolysis under the pulsatile condition will be helpful for the subsequent in vivo experiments.
A Fast Building and Effective Hydraulic Pediatric Mock Circulatory System for the Evaluation of A Left Ventricular Assist Device

Abstract:A mock circulatory system (MCS) has been proven a useful tool in the development of a ventricular assist device. Nowadays a MCS aimed at the evaluation of pediatric blood pumps, which require many different considerations compared with that of adults, has become an urgent need. This article presents the details on how the dynamic process of the left ventricle, which is described in terms of the pressure-volume loop (P-V loop), and the properties of the circulation such as compliance and resistance are simulated by hydraulic elements. A simple control method is introduced to reproduce the physiological afterload and preload sensitivities of the mock ventricle for the first time. Hemodynamic performance of the system is obtained by medical sensors to validate the similarity of the device to the native cardiovascular system. The actual sensitivities of the mock ventricle are obtained intuitively from the changes of the P-V loops. The aortic input impedance of the MCS is also obtained and compared with the data from previous medical reports. At last a pediatric left ventricular assist device (LVAD) prototype is introduced for testing to further verify the effectiveness of the MCS. The experimental results indicate that this pediatric MCS is capable of reproducing basic hemodynamic characteristics of a child in both normal and pathological conditions and it is sufficient for testing a pediatric LVAD. Besides, most components constituting the main hydraulic part of this MCS are inexpensive off-the-shelf products, making the MCS easy and fast to build.

Abstract:Ventricular assist devices have been widely used and accepted to treat patients with end-stage heart failure. The role of VAD is to improve circulatory dysfunction or temporarily maintain the circulatory status of patients. In order to be closer to the medical practice, a multi-Domain model of the left ventricular coupled axial flow artificial heart was considered to study the effect of its hemodynamics on the aorta. Because whether LVAD itself was connected between the left ventricular apex and the ascending aorta by catheter in the loop was not very important for the analysis of simulation results, on the premise of ensuring the multi-Domain simulation, the simulation data of the import and export ends of LVAD were imported to simplify the model. In this paper, the hemodynamic parameters in the ascending aorta, such as blood flow velocity vector, wall shear stress distribution, vorticity current intensity, vorticity flow generation, etc., have been calculated. The numerical conclusion of this study showed the vorticity intensity under LVAD was significantly higher than that under patients' conditions and the overall condition is similar to that of a healthy ventricular spin, which can improve heart failure patients' condition while minimizing other pitfalls. In addition, high velocity blood flow during left ventricular assist surgery is mainly concentrated near the lining of the ascending aorta lumen. What's more, the paper proposes to use Q criterion to determine the generation of vorticity flow. The Q criterion of LVAD is much higher than that of patients with heart failure, and the closer the LVAD is to the wall of the ascending aorta, the greater the Q criterion is. All these are beneficial to the effectiveness of LVAD in the treatment of heart failure patients and provide clinical suggestions for the LVAD implantation in clinical practice.

Computational fluid dynamics analysis of blade tip clearances on hemodynamic performance and blood damage in a centrifugal ventricular assist device

Abstract:An important challenge facing the design of turbodynamic ventricular assist devices (VADs) intended for long-term support is the optimization of the flow path geometry to maximize hydraulic performance while minimizing shear-stress-induced hemolysis and thrombosis. For unshrouded centrifugal, mixed-flow and axial-flow blood pumps, the complex flow patterns within the blade tip clearance between the lengthwise upper surface of the rotating impeller blades and the stationary pump housing have a dramatic effect on both the hydrodynamic performance and the blood damage production. Detailed computational fluid dynamics (CFD) analyses were performed in this study to investigate such flow behavior in blade tip clearance region for a centrifugal blood pump representing a scaled-up version of a prototype pediatric VAD. Nominal flow conditions were analyzed at a flow rate of 2.5 L/min and rotor speed of 3000 rpm with three blade tip clearances of 50, 100, and 200 microm. CFD simulations predicted a decrease in the averaged tip leakage flow rate and an increase in pump head and axial thrust with decreasing blade tip clearances from 200 to 50 microm. The predicted hemolysis, however, exhibited a unimodal relationship, having a minimum at 100 microm compared to 50 microm and 200 microm. Experimental data corroborate these predictions. Detailed flow patterns observed in this study revealed interesting fluid dynamic features associated with the blade tip clearances, such as the generation and dissipation of tip leakage vortex and its interaction with the primary flow in the blade-blade passages. Quantitative calculations suggested the existence of an optimal blade tip clearance by which hydraulic efficiency can be maximized and hemolysis minimized.
Systemic and splanchnic hemodynamic changes after liver transplantation for cirrhosis: A long‐term prospective study

Abstract:The effect of orthotopic liver transplantation (OLT) on the systemic and splanchnic hemodynamic alterations of cirrhosis is still largely unknown. The aim of this study was to prospectively investigate the long‐term changes induced by OLT on several hemodynamic parameters. In 28 patients undergoing OLT for cirrhosis, the following parameters were measured before surgery and subsequently at 6‐month intervals (mean follow‐up period, 17 months): cardiac index, mean arterial pressure (MAP), heart rate, total peripheral resistance (TPR), portal vein flow velocity and flow volume, spleen size, and Doppler ultrasound resistance or pulsatility indexes (RI or PI) in the: 1) interlobular renal, 2) superior mesenteric, 3) splenic, and 4) hepatic arteries. The same parameters were measured in 10 healthy controls. After OLT, cardiac index and heart rate significantly decreased (P < .01), while MAP and TPR increased (P < .001), so that any significant difference from controls disappeared. Renal RI progressively decreased, achieving a significant reduction (P < .05) to normal values at the 12th month of follow‐up. Portal flow velocity and hepatic and splenic RI returned to values not significantly different from controls. Portal flow volume increased over normal values after OLT (P < .001), and SMA PI, lower than normal before OLT, did not show any statistically significant increase thereafter. Spleen size decreased significantly, but persisted to be larger than in controls. In conclusion, systemic, renal, and most, but interestingly not all, splanchnic circulatory alterations of cirrhosis are restored to normal after OLT.

Abstract:Left ventricular assist devices are routinely used to treat patients with advanced heart failure as a bridge to transplant or a destination therapy. However, non-physiological shear stress generated by left ventricular assist devices damages blood cells. The continued development of novel left ventricular assist devices is essential to improve the left ventricular assist device therapy for heart failure patients. The CH-VAD is a new maglev centrifugal left ventricular assist device. In this study, the CH-VAD pump was numerically analyzed and compared with the HVAD and HeartMate II pumps under two clinically relevant conditions (flow: 4.5 L/min, pressure head: normal ~80 and hypertension ~120 mmHg). The velocity and shear stress fields, washout, and hemolysis index of the three pumps were assessed with computational fluid dynamics analysis. Under the same condition, the CH-VAD hemolysis index was two times lower than the HVAD and HeartMate II pumps; the CH-VAD had the least percentage volume with shear stress larger than 100 Pa (i.e. normal condition: 0.4% vs HVAD 1.0%, and HeartMate II 2.9%). Under the normal condition, more than 98% was washed out of the three pumps within 0.4 s. The washout times were slightly shorter under the hypertension condition for the three pumps. No regions inside the CH-VAD or HVAD had extremely long residential time, while areas near the straightener of the HeartMate II pump had long residential time (>4 s) indicating elevated risks of thrombosis. The computational fluid dynamics results suggested that the CH-VAD pump has a better hemolytic biocompatibility than the HVAD and HeartMate II pumps under the normal and hypertension conditions.

Abstract:Ventricular assist devices (LVADs) are increasingly recognized for supporting blood circulation in heart failure patients who are non-transplant eligible. Because of its volume, the traditional pulsatile device is not easy to implant intracorporeally. Continuous flow LVADs (CF-LVADs) reduce arterial pulsatility and only offer continuous flow, which is different from physiological flow, and may cause long-term complications in the cardiovascular system. The aim of this study was to design a new pulsatile assist device that overcomes this disadvantage, and to test this device in the cardiovascular system. Firstly, the input and output characteristics of the new device were tested in a simple cardiovascular mock system. A detailed mathematical model was established by fitting the experimental data. Secondly, the model was tested in four pathological cases, and was simulated and coupled with a fifth-order cardiovascular system and a new device model using Matlab software. Using assistance of the new device, we demonstrated that the left ventricle pressure, aortic pressure, and aortic flow of heart failure patients improved to the levels of a healthy individual. Especially, in state IV level heart failure patients, the systolic blood pressure increased from 81.34 mmHg to 132.1 mmHg, whereas the diastolic blood pressure increased from 54.28 mmHg to 78.7 mmHg. Cardiac output increased from 3.21 L/min to 5.16 L/min. The newly-developed assist device not only provided a physiological flow that was similar to healthy individuals, but also effectively improved the ability of the pathological ventricular volume. Finally, the effects of the new device on other hemodynamic parameters are discussed.
Improving hydraulic performance of the left atrial assist device using computational fluid dynamics

Abstract:Background: The left atrial assist device (LAAD) is a novel continuous-flow pump designed to treat patients with heart failure with preserved ejection fraction, a growing type of heart failure, but with limited device-treatment options. The LAAD is implanted in the mitral plane and pumps blood from the left atrium into the left ventricle. The purpose of this study was to refine the initial design of the LAAD, using results from computational fluid dynamics (CFD) analyses to inform changes that could improve hydraulic performance and flow patterns within the LAAD. Methods: The initial design and three variations were simulated, exploring changes to the primary impeller blades, the housing shape, and the number, size, and curvature of the diffuser vanes. Several pump rotational speeds and flow rates spanning the intended range of use were modeled. Results: Guided by the insight gained from each design iteration, the final design incorporated impeller blades with improved alignment relative to the incoming flow and wider, more curved diffuser vanes that better aligned with the approaching flow from the volute. These design adjustments reduced flow separation within the impeller and diffuser regions. In vitro testing confirmed the CFD predicted improvement in the hydraulic performance of the revised LAAD flow path design. Conclusions: The CFD results from this study provided insight into the key pump design-related parameters that can be adjusted to improve the LAAD's hydraulic performance and internal flow patterns. This work also provided a foundation for future studies assessing the LAAD's biocompatibility under clinical conditions.
Numerical solution for blood flow in a centrifugal ventricular assist device

Abstract:A very small centrifugal pump, fully supported by magnetic bearings, is being developed for use as a ventricular assist device to be implanted in humans. In this paper, we apply computational fluid dynamics to model the blood flow to aid in the design of the ventricular assist device. The flow of blood through the pump has been modeled using computational fluid dynamics (CFD) software that is commercially available from AEA Technology, UK. The flow regions modeled in version 3 of the Continuous Flow Ventricular Assist Device (CF3) are the fully shrouded four bladed impeller and the two clearance regions around the impeller that are bounded by the pump hub and shroud. This paper describes the geometry and computational grids developed for the flow regions, and the equations of motion for the blood flow are developed. The overall numerically-evaluated flow rates and head rise have similar trends to the flow parameters experimentally measured, indicating that future pump designs can be effectively modeled numerically before being constructed and tested. Numerical solutions are presented and compared with experimentally-obtained overall pump performance results. These solutions are used to predict shear stress levels to be experienced by the blood flowing through the pump, and it is predicted that hemolysis will be insignificant. The solutions also indicate no regions of flow stagnation that can be a source of thrombosis in pumps. The calculations provide a viable design method to achieve improved efficiency in future versions of this pump.

Abstract:A novel pulsatile assist device, intra-ventricular assist device, was proposed to address various disadvantages existing in conventional pulsatile assist device, such as the large size, accessories and reduced pulsatility. The assist device was designed, fabricated and implanted into the sac from left ventricular apex in a home-designed mock circulatory system. In vitro test was carried out and results demonstrated that the response time did not vary with the heart rate, and co-pulsatiled synchronously with native heart by electrocardiograph. The key parameter, stroke volume of proposed device was precisely measured under different afterloads (60, 80, 100, and 120 mmHg), drive pressure (from 90 to 300 mmHg at 30 mmHg intervals), and heart rate (45-150 beats per minute). The measurement results revealed that the output characteristics of device, stroke volume increased with increasing drive pressure but decreased with increasing peripheral resistance, were consistent with the native heart. The proposed pump was then coupled with mock system that was set to a heart failure mode and the circulatory responses were tested. Results showed that the device improved left ventricular pressure from 106 to 158 mmHg, and stroke volume from 25.5 to 44 ml at 90 bpm.
Enhancing the implantation of mechanical circulatory support devices using computational simulations

Abstract:Introduction: Patients with end-stage heart failure (HF) may need mechanical circulatory support such as a left ventricular assist device (LVAD). However, there are a range of complications associated with LVAD including aortic regurgitation (AR) and thrombus formation. This study assesses whether the risk of developing aortic conditions can be minimised by optimising LVAD implantation technique. Methods: In this work, we evaluate the aortic flow patterns produced under different geometrical parameters for the anastomosis of the outflow graft (OG) to the aorta using computational fluid dynamics (CFD). A three-dimensional aortic model is created and the HeartMate III OG positioning is simulated by modifying (i) the distance from the anatomic ventriculo-arterial junction (AVJ) to the OG, (ii) the cardinal position around the aorta, and (iii) the angle between the aorta and the OG. The continuous LVAD flow and the remnant native cardiac cycle are used as inlet boundaries and the three-element Windkessel model is applied at the pressure outlets. Results: The analysis quantifies the impact of OG positioning on different haemodynamic parameters, including velocity, wall shear stress (WSS), pressure, vorticity and turbulent kinetic energy (TKE). We find that WSS on the aortic root (AoR) is around two times lower when the OG is attached to the coronal side of the aorta using an angle of 45° ± 10° at a distance of 55 mm. Discussion: The results show that the OG placement may significantly influence the haemodynamic patterns, demonstrating the potential application of CFD for optimising OG positioning to minimise the risk of cardiovascular complications after LVAD implantation.

Abstract:Ventricular assist devices (VADs) are used to assist the heart function of patients with advanced heart failure. Computational fluid dynamics in VADs are widely applied in the development and optimization, for example, to evaluate blood damage. For these simulations, the pulsating operating conditions, in which the VAD operates, should be included accurately. Therefore, this study aims to evaluate the flow in a VAD by interacting with patient-specific cardiovascular systems of heart failure patients. A numeric method will be presented, which includes a patient-specific cardiovascular system model that is bidirectionally coupled with a three-dimensional (3D) flow simulation of the HeartMate 3. The cardiovascular system is represented by a lumped parameter model. Three heart failure patients are considered, based on clinical data from end-stage heart failure patients. Various parameters of the cardiovascular system and the VAD are analyzed, for example, flow rates, pressures, VAD heads, and efficiencies. A further important parameter is the blood damage potential of the VAD, which varies significantly among different patients. Moreover, the predicted blood damage fluctuates within a single heartbeat. The increase in blood damage is evaluated based on the operating conditions. Both, overload and especially partial load conditions during the pulsating operation result in elevated blood damage.

Abstract:Objective To explore the in-vivo hemodynamic and hemolysis effect of a newly designed axial continuousflow ventricular assist device(VAD) in swine.Methods Under general anesthesia,each of 5 swine [weight (40.0 ± 5.2)kg] was implanted with the axial continuous-flow VAD into the apex of left heart ventricle,and the outflow graft was anastomosised to descending aorta.Results All of the axial continuous-flow VAD were implanted successfully with post-operative survival rate 100％.All 5 animals survived over one week.There was a positive correlation between pump speed and assistance effect.The mean left ventricular systolic pressure was (131.6 ± 28.0) mmHg(1 mmHg =0.133 kPa).While the axial continuous-flow VAD was working,left ventricular end diastolic pressure decreased,along with mean intraventricular pressure declined.Peripheral hemodynamics was stable and peripheral blood pressure was not remarkably different from the pressure preoperation.Daily urine volume was in normal range within 1 week post operation.Free hemoglobin in plasma was slightly elevated on the surgery day,and gradually dropped to normal level within 1 week.International Normalized Ratio(INR) was maintained between 2.0-2.5 with oral adminiatration of warfarin of 3 mg/day.There was no thrombosis existing in VAD at autopsy.Conclusion The application of the axial pump with hydrodynamic-magnetically levitated impeller in animal experiment can provide stable hemodynamics,advanced heart unloaded effect,favorable peripheral perfusion,and blood compatibility is satisfactory.
Development of the Baylor/NASA axial flow ventricular assist device: in vitro performance and systematic hemolysis test results

Abstract:Our newly developed axial flow pump consists of a flow tube, an internal rotating impeller, and a fixed flow stator (we call the stator) behind the impeller. This pump produces a flow of 3 to 8 L/min against 50 to 150 mm Hg pressure difference, respectively, in the range of 10,000 to 16,000 rpm. An axial flow pump that will be used as a ventricular assist device (VAD) has to have low hemolytic and good antithrombogenic characteristics. This paper will show how to decrease the hemolytic properties of this axial flow pump systematically using a test matrix. The test variables evaluated were impeller blade tip geometry, impeller flow tube clearance (radial clearance), impeller stator clearance (axial clearance), impeller blade number, stator blade number, and impeller length. All in vitro hemolysis tests were performed at 5.0 L/min against 100 mm Hg pressure difference using a total of 83 bags of fresh bovine blood. The results were as follows: the impeller blade tip geometry did not significantly effect hemolysis, a 0.005-inch and a 0.009-inch radial clearance were significantly (p < 0.01 or 0.001) less hemolytic than the other clearances, a 0.075-inch axial clearance was significantly (p < 0.05) more hemolytic than the other clearances, two- and six-bladed impellers were significantly (p < 0.01 and 0.02, respectively) less hemolytic than a four-bladed impeller, a five-bladed stator was significantly (p < 0.05 or 0.01) less hemolytic than the other stators, and the impeller length did not make a significant difference. Currently, the best index of hemolysis is 0.031 +/- 0.018 g/100 L, and using parameters from these results, implantable devices are being fabricated.
