Abstract:Objectives: The aim of the study was to evaluate the curve of Spee (COS) stability in patients treated with continuous archwires with different vertical patterns. Methods: The study sample consisted of 60 patients (28 males, 32 females; mean age 19.8 ± 1.4 years) presenting with COS depth of at least2 mm at baseline. For each subject, lateral cephalograms and dental casts were available before treatment (T1), at the end of orthodontic therapy (T2), and 2 years after the end of treatment (T3). All subjects were divided into three groups according to vertical facial patterns. Cephalometric parameters were used to evaluate the dental movements after treatment. COS depth was measured on digital casts. Mean differences between vertical facial subgroups were contrasted by means of analysis of variance test (P < 0.01). Results: In low-angle subjects, COS levelling occurred through advancement and intrusion of lower incisors, whereas in high-angle patients, the COS was flattened through extrusion and uprighting of lower posterior teeth. In the low-angle group, a significant relapse of lower incisors inclination was observed. Differently, the high-angle group exhibited a greater stability of COS obtained by stable extrusion of posterior teeth. Conclusions: The long-term instability of flared incisors determined the relapse of overbite and COS depth in the low-angle group.

Abstract:Abstract Introduction To guide deep overbite treatment, the effectiveness of clear aligners and the influence of maxillofacial characteristics were assessed in levelling the curve of Spee (COS). Methods Forty-two patients underwent phased treatment using clear aligners. Digital dental models were created before and after treatment following which predicted outcomes, and dental cast measurements were conducted in addition to the analysis of lateral cephalometric and panoramic radiographs. The anticipated COS correction was compared with the actual correction achieved. Using multiple linear regression, associations between levelling effectiveness and clinical indicators were evaluated to establish prediction equations. P = 0.05 indicated statistical significance. Results The levelling effectiveness was 71.8 ± 19.0%. The regression model accounted for 88.5% of the variance in levelling effectiveness ( F -value = 167.209, adjusted R 2 = 0.885, Akaike information criterion [AIC] [E]=-146.94). The levelling effectiveness was related to the vertical skeletal pattern, the angulation of the mandibular incisors to the mandibular plane, the axial inclination of the mandibular anterior teeth, and the pre-treatment COS depth. Levelling effectiveness and other maxillofacial variables were not significantly related. Conclusion Clear aligners effectively level the COS. Patients with a larger Frankfort-mandibular plane angle, a pre-treatment COS depth, and an incisor-mandibular plane angle, more easily achieved COS levelling using clear aligners. Those with smaller mandibular incisor parallelism still achieved effective levelling.

Abstract:Introduction Aligning and leveling is the initial stage of comprehensive fixed orthodontic treatment aimed at minimizing the depth of the curve of Spee (COS). Various techniques exist to decrease the magnitude of the curve. This study investigates the skeletal and dental factors that reduce COS in individuals with minor class II malocclusions receiving nitinol (NiTi) wires with a reverse curve of Spee (RCS). Materials and methods The data for this observational study was collected from a sample of 84 patients who had class II molar relations and were sequentially treated with RCS NiTi wires throughout the initial leveling and aligning phase. All patients with class II molar relationships underwent non-extraction procedures during the leveling phase. The COS was determined using digitalized dental models. Skeletal and dental characteristics that could impact COS were identified and quantified using digital lateral cephalograms and orthopantomograms recorded during the pre-treatment (T1) and post-leveling (T2) stages. After calibrating the radiographs and models, we acquired angular and linear data. The data was categorized based on gender, growth pattern, and initial alignment of the teeth. We analyzed the differences between the groups using an independent t-test and an ANOVA. A paired t-test was used to compare the difference in the dimensional values between (T1) and (T2) points. Following the correlation coefficient tests, the study used stepwise multiple linear regression analysis to assess the predictive value of independent factors on the COS. The results were considered statistically significant at p < 0.05. Results The COS decreased by -1.43 ± 0.68 mm, which is statistically significant (<0.001*). There is no significant difference in COS reduction between the categorical variables. Despite statistically significant differences in the parameters between pre- and post-treatment, the linear correlation between most of the variables and COS reduction ranged from very weak (<0.20) to weak (0.20-0.39). Conclusions The vertical extrusion of lower premolars and molars combined with the intrusion of lower incisors contributed to the reduction of the COS by RCS wires. There is a change in the orientation of the occlusal plane with the flattening of the COS.
Long‐term stability of curve of Spee depth among orthodontically treated patients: A retrospective longitudinal study

Abstract:Objective The curve of Spee is deemed important characteristic of the dentition for a balanced occlusion and distribution of masticatory forces, while orthodontic levelling of an accentuated curve of Spee is generally included as a treatment goal for deepbite correction. However, relapse is often seen and can be problematic. Methods A retrospective longitudinal study of predominantly young patients with a deep curve of Spee, who had been treated orthodontically with 0.018"‐slot Edgewise fixed appliances, was performed. The depth of the curve of Spee was digitally measured before treatment (T1), at debond (T2), and an average of 7 years post‐debond (T3) and analysed statistically at 5%. Results A total of 157 patients were included (56.7% female; 11.6‐year‐old at T1), 16.6% of which were treated with premolars extractions. Non‐extraction treatment reduced the curve of Spee at the first premolar from 1.87 mm (T1) to 0.22 mm (T2), which relapsed 0.12 mm (T3; P = .04). The respective depths for the second premolar were 2.0 mm (T1), reduced to 0.80 mm (T2). No significant relapse was seen for the second premolar (0.08 mm; P > .05) or the first permanent molar (0.06 mm; P > .05). No overall significant differences in absolute relapse were seen between extraction and non‐extraction patients, but premolar extractions were associated with less clinically relevant relapse at the first molar (odds ratio 0.27; 95%‐confidence interval 0.08‐0.88; P = .003). Conclusion Steep curves of Spee can be satisfactorily levelled orthodontically with satisfactory stability in the long term, while premolar extractions might be associated with less relapse.

Abstract:ABSTRACT Objectives To evaluate leveling of the Curve of Wilson (COW) by two different treatment appliances (clear aligners [CA] and continuous archwire fixed appliances [FA]) in a permanent dentition sample of patients. Materials and Methods Digital casts of 40 patients (CA group = 20 patients; FA group = 20 patients) were collected. Angular values for COW, right and left vertical height difference of lower first molars, and linear distance between lower teeth and the WALA ridge were analyzed for pre- (T1), posttreatment (T2) and on final virtual (ClinCheck) models (T2-CC) of the CA group. An unpaired t-test was used to evaluate significant intergroup differences (P < .05), while a paired t-test was used for posttreatment CA intragroup comparison. Results FA group showed better control of second molar crown positions compared to CA group (47-WALA = −0.2 ± 0.1 mm, 37-WALA = −0.6 ± 0.3 mm). No significant difference was detected for linear distance of lower first molars and the WALA ridge or for vertical height difference. CA group showed a greater reduction of distance between lower premolars and the WALA ridge (mean difference: –0.5 mm for both 45-WALA and 35-WALA; mean difference: −0.5 mm for 44-WALA, −0.6 mm for 34-WALA). Predictability for the CA group was high for every measurement (87% Right COW, 89% Left COW, 88% 46 Vertical Diff, 87% 36 Vertical Diff). Conclusions Clear aligner and continuous archwire mechanics were effective in leveling COW. FA was more effective in changing crown position of lower second molars with respect to the WALA ridge, while CA provided a greater distance reduction between lower premolars and WALA ridges compared to FA.

Smile outcome comparison of Invisalign and traditional fixed-appliance treatment: A case-control study

Abstract:Introduction: The purpose of this study was to evaluate and compare smile treatment outcomes between patients treated with Invisalign clear aligners (Align Technology, Santa Clara, Calif) and those treated with traditional fixed appliances by integrating variables such as lip symmetry, smile index, smile cant, buccal corridors, and gingival display into smile outcome evaluation. Methods: Records from 58 patients, 29 of whom received Invisalign treatment (mean age 19.03 years) and 29 of whom received traditional fixed-appliance treatment (mean age 13.83 years), were compared for their smile outcome. Pretreatment scores, posttreatment scores, and differences between within-group smile score before and after treatment were determined for each group. Fifteen variables comprised the smile outcome, and the data were analyzed with the Mann-Whitney U test and the Wilcoxon t test for 2 dependent samples. Results: Six variables within the fixed-appliance group presented with better smile scores than those within the Invisalign group; buccal corridors (%) (mean difference = 8.42%), buccal corridors (mm) (5.35 mm), smile cant (0.42°), maxillary dental midline (0.21 mm), gingival display (0.56 mm), and smile index (1.09%) for P <0.05. Invisalign performed better on 2 variables that determined maxillary incisor position (1.26 mm) and inclination (2.09°). No significant difference (for P = 0.05) was shown between pretreatment and posttreatment scores for either of the 2 groups. Conclusions: Using the 15 variables in this study, the results suggested that for patients with Class I nonextraction, treatment with traditional fixed appliances changes the patient's smile more than Invisalign treatment, and fixed appliances appear to be more effective in improving the variables that quantify posttreatment smile outcome.

Abstract:Abstract The objective of this study was to detective the accuracy of model superimposition and automatic analysis for upper and lower dentition width in Invisalign Progress Assessment during the process of clear aligners. 19 cases were included in this study. Pre-treatment dental cast (T0) and post-treatment dental cast after staged treatment (T1) were available for three-dimensional model superimposition. Subsequently, movements of maxillary teeth in the horizontal plane (cross-section) after staged treatment and width of upper and lower dentition were measured by three-dimensional model superimposition in the real world and Invisalign Progress Assessment separately. Consequently, the data collected from these two methods were compared. In Invisalign Progress Assessment, movements of maxillary teeth in the horizontal plane after staged treatment was 2.31 (1.59,3.22) [median (upper quartile, lower quartile)] millimeter (mm), while in three-dimensional model superimposition, the result was 1.79 (1.21,3.03) mm. The difference between the two groups is significant ( P < 0.05). Intercanine width upper, intermolar width upper, intercanine width lower, and intermolar width lower were 36.55 ± 2.76 mm, 56.98 ± 2.62 mm, 28.16 ± 1.85 mm, 53.21 ± 2.72 mm separately in Invisalign Progress Assessment and were 36.48 ± 2.78 mm, 56.89 ± 2.58 mm, 28.05 ± 1.85 mm, 53.16 ± 2.64 mm separately in three-dimensional model analysis, which was no significant difference among these groups ( P > 0.05). The data from Invisalign Progress Assessment was not in parallel with what was achieved from model superimposition with palate as a reference completely. The accuracy of model superimposition in Invisalign Progress Assessment needs further investigation, whereas the accuracy of model analysis in Invisalign Progress Assessment was accurate. Thereby, results from Invisalign Progress Assessment should be interpreted with caution by the orthodontist in the clinic.

Abstract:Background: Maxillary hypoplasia and mandibular asymmetry may be corrected with orthognathic surgery after growth completion. For most stable results, some cases may require segmental Le Fort I osteotomies. Unfortunately, Invisalign's software (6.0 version) still has some inherent limitations in predicting outcomes for complex surgeries. This study explores the potential of aligners, particularly in multiple-piece maxillary osteotomies in both cleft and non-cleft patients. Method: Thirteen patients who underwent pre-surgical treatment with Invisalign were retrospectively matched in terms of diagnosis, surgical procedure, and orthodontic complexity with thirteen patients treated using fixed appliances. Virtual curves following the lower arch were employed to guide the correct pre-surgical positions of the upper teeth with a simple superimposition technique. The amount of impressions required in both groups to achieve satisfactory pre-surgical alignment of the segmented arches was compared. Results: one or no refinement phases were needed in the Invisalign group to reach an acceptable pre-surgical occlusion, while the amount of pre-surgical impressions needed to reach adequate coordination with fixed appliance treatment was slightly higher (p > 0.05). Conclusions: it appears that clear aligner could serve as an effective treatment for individuals necessitating segmental Le Fort I osteotomies when aided by the suggested simple superimposition approach.

Abstract:ABSTRACT Introduction: Orthodontists have been using clear aligners to treat malocclusions, and one potential effect of treatment with orthodontic aligners is the intrusion and/or resists extrusion of the posterior teeth. This “bite-block effect” is primarily anecdotal due to the frequent occurrence of posterior open bites in patients after clear aligner therapy. Objective: The purpose of this study was to compare changes promoted by clear aligners and traditional fixed appliances in cephalometric measurements of the vertical dimension and molar position in adult patients with Class I malocclusion treated with non-extraction. Methods: Pre- and post-treatment lateral cephalometric radiographs of adult patients treated with either clear aligners (n=44) or traditional fixed appliances (n=22) were selected for retrospective analysis. Eight interval measurements and one nominal measurement were evaluated: anterior overbite (OB), mandibular plane angle related to cranial base (SN_MP) and related to Frankfort (FMA), lower molar height (L6H) and upper molar height (U6H), palatal plane to mandibular plane angle (PP_MP), lower facial height (LFH), total facial height (TFH), and posterior open bite (Posterior_OB). A single evaluator traced all cephalographs, and changes in select measures of the vertical dimension were compared within and between groups. Results: OB decreased (1.15 mm) and L6H increased (0.63 mm) in the traditional fixed appliance group. Mandibular plane angles (related to cranial base and to Frankfort) increased (0.43° and 0.53°, respectively) in the clear aligner group, but just FMA showed significant difference between groups (difference of 0.53°). LFH and TFH increased (ranging from 0.52 mm to 0.80 mm) in both groups, with no differences between treatment modality. Presence of visible posterior open bite significantly increased over the course of treatment. OB, FMA and L6H exhibited an interaction between treatment stage (pre- and post-treatment) and modality (clear aligner therapy and traditional fixed appliances), but no interaction among these three variables was found. Conclusions: The evidence does not support the theory that clear aligner therapy produces better vertical dimension control than traditional fixed appliances. Traditional fixed appliance therapy slightly extruded the lower molar, and clear aligner therapy produced a slightly mandibular backward rotation.

Abstract:Individuals with insomnia were exposed to a stressor (speech threat) prior to getting into bed and were instructed to think about the speech and its implications in either images (Image group, n = 14) or verbal thought (Verbal group, n = 17). Participants completed questionnaires about speech anxiety, arousal, and sense of resolution. Measures were taken of subjective (sleep diary estimates) and objective (actigraphy) sleep-onset latency. In the short term, the Image group reported more distress and arousal relative to those in the Verbal group. In the longer term, the Image group estimated that they fell asleep more quickly and, the following morning, reported less anxiety and more comfort about giving the speech compared with the Verbal group.

Abstract:Background: Optimal management of hypodivergent growing patients demands a strict control of vertical dimension and to exploit the growth potential. If a deep bite malocclusion causes a traumatic contact between the upper and lower incisors and affects the facial appearance, an early interceptive treatment is recommended. The aim of this case report is to outline the clinical management of the occlusal plane of a growing Class II division 1 deep bite patient treated with aligners and Class II elastics. Methods: The treatment lasted 11 months and was divided into two phases. Treatment goals included improvement of the soft tissue profile and basal bone relationships through an increase in the mandibular third of the face and a sagittal advancement of the mandible. The correction of the curve of Spee involved intrusion of the mandibular incisors and extrusion of the mandibular premolars. Results: The cephalometric analysis at the end of the treatment displayed significant differences in the skeletal and occlusal pattern along with aesthetic improvements. Conclusion: The final cephalogram showed a consistency between the planned tooth movement and the clinical results. Although definitive recommendations must be withheld until longer follow-up is available, the patient presented here shows that the treatment protocol yielded positive mandibular growth.

Abstract:PURPOSE Orthognathic surgery for dentofacial deformities is typically preceded and followed by orthodontic treatment. Traditionally, orthodontic hardware is secured to the dentition to allow dental movement and stabilization. Clear-aligner therapy (eg, Invisalign) provides an aesthetic alternative, consisting of a series of transparent trays. Its use has not been described in complex triple-jaw orthognathic surgery. The purpose of this study is to evaluate perioperative outcomes and 3-dimensionally quantify postoperative edema in Invisalign patients undergoing triple-jaw orthognathic procedures, comparing this to patients treated with conventional fixed appliances. The surgical approach to patients with clear-aligners is also outlined. METHODS The authors conducted a retrospective chart review and 3-dimensional morphometric study of Invisalign patients undergoing triple-jaw surgery (LeFort I osteotomy, bilateral sagittal split osteotomy, and genioplasty). An identical assessment of demographically matched patients treated with conventional fixed appliances was performed and compared with the Invisalign group. RESULTS Thirty-three patients, with a mean age of 19.99 years, were included: 13 with Invisalign and 20 with conventional fixed appliances. No significant difference was observed in operating time, concurrent extraction of teeth, fat grafting, duration of hospital stay, diet advancement, and use of narcotic analgesics between the 2 groups. Nine patients had sufficient 3-dimensional images for volumetric analysis (4 with Invisalign and 5 with conventional fixed appliances). Postoperative edema was not significantly different (P = 0.712) when comparing conventional fixed appliances (44.29 ± 23.16 cm) to Invisalign (37.36 ± 31.19 cm). CONCLUSION The present study demonstrates that complex multiple-jaw orthognathic procedures can be successfully performed in Invisalign patients. Perioperative and short-term clinical outcomes are not compromised.

Abstract:AIM The current study aimed to analyse the dentoskeletal effects of the Invisalign mandibular advancement (MA) device in the treatment of skeletal Class II malocclusions. MATERIALS AND METHODS Pre-treatment and post-treatment lateral skull radiographs from patients treated with MA versus TB (Twin-Block Appliance) at the Department of Orthodontics of the University of L'Aquila, Italy, were traced. Eligibility criteria included SNB<78; ANB>4; no previous orthodontic treatments; and vertebral maturation stage (CVM) CS3. Radiographs from patients with craniofacial anomalies, or who underwent extraction treatments, were excluded. Totally, 20 patients were examined, 10 of whom treated with MA and 10 treated with TB. All the radiographs were traced by one expert operator, blind to the groups. A preliminary method error study was performed to exclude intra-operator differences. RESULTS Baseline characteristics of the participants were similar between the groups. Both appliances demonstrated a reduction of SNB and ANB angle, and a decrease in overjet. TB demonstrated a higher efficacy in increasing mandibular dimensions. A significant retroinclination of the upper incisive was observed in the TB group, where a decrease of SNA angles was additionally observed. The resulting differences between the two groups could be attributed to the different design of the appliances. CONCLUSIONS The present data show the effectiveness of both TB and MA in the management of skeletal Class II malocclusions due to mandibular retrusion. But some differences exist in the dentoalveolar effect of the two appliances. MA seems indicated in Class II cases where a control of the upper frontal teeth position is needed.

Abstract:Purpose: The aims of the study were (1) to evaluate the fitting of three different aligners (Invisalign [Align Technology, Santa Clara, CA, USA], CA Clear Aligner [Scheu-Dental, Iserlohn, Germany] and F22 [Sweden&Martina, Due Carrare, Italy]) on anchorage attachments using scanning electron microscopy (SEM), and (2) to analyze the influence of 2 different types of resin used to build attachments on aligner fitting. Methods: Using STL files of a patient, six resin casts were obtained and rectangular attachments were bonded on them. Conventional bulk-fill resin was used to build upper attachments while a flowable resin was used to build the lower ones. Passive aligners were adapted on each cast and then sectioned buccolingually. Microphotographs of the obtained sections were performed using a SEM and then micrometric measurements of aligner fitting on anchorage attachments were recorded. Results: Analyzing the overall fitting of upper arch aligners, Invisalign provided a significantly better fitting with respect to F22 (P = 0.009); differences were not significant when comparing Invisalign with CA Clear Aligner, and CA Clear Aligner with F22. Analyzing the overall fitting of lower arch aligners, F22 provided a significantly better fitting with respect to CA Clear Aligner (P = 0.008) and Invisalign (P = 0.011). The analysis showed a significantly better fitting on upper attachments, built using conventional bulk-fill resin (P = 0.034). Conclusions: Invisalign, CA Clear Aligner and F22 have comparable performance in terms of fitting on anchorage attachments. Conventional bulk-fill resin provides the best fitting on anchorage attachments.

Deep overbite reduction in adolescent patients treated with Invisalign: A retrospective analysis

Abstract:Introduction: Data regarding the efficacy of deep overbite correction with clear aligner therapy in adolescent patients are lacking. This study aimed to investigate the efficacy of deep overbite reduction in adolescent patients treated with Invisalign (Align Technology, San Jose, Calif) aligners. Methods: This retrospective study investigated a sample of 102 patients aged 10-17 years from a large database. All patients had completed wear of a prescribed series of Invisalign aligners with planned overbite reduction on a nonextraction basis. The predicted, pretreatment, and posttreatment overbite measurement values were obtained from Align Technology's ClinCheck digital facility. Linear modeling, with Bland-Altman plots, was used to compare the effect of age, the use of bite ramps, and the adoption of the eighth-generational (G8) protocol on the difference between predicted and achieved overbite measurements. Results: A total of 102 patients with a mean ± standard deviation age of 13.94 ± 1.53 years satisfied the selection criteria. A mean ± standard deviation of 41.38 ± 30.43% of the predicted overbite reduction was achieved. There were no differences in the predictability of overbite reduction between patients who were aged <14 years or those aged 14-17 years (P = 0.73), between patients prescribed and not prescribed bite ramps (P = 0.25), or between patients prescribed the G8 protocol and not prescribed the G8 protocol (P = 0.65). Conclusions: The average achieved overbite was less than half of what was planned by the orthodontists via the ClinCheck facility. The age grouping, the provision of bite ramps, and the G8 protocol appear to have little influence on the efficacy of overbite reduction in adolescent patients.
Comparative evaluation of efficacy of two Class II correctors in treatment of skeletal Class II malocclusion: A multicenter randomized controlled trial

Abstract:Background: This article aims to compare clinical efficacy of newly introduced PowerScope 2 appliance with Forsus FRD in the treatment of Skeletal Class II malocclusion. Methods: This randomized controlled trial studied 40 patients at two centers (20 patients at each center, divided into two groups of 10 patients each as Group 1: Forsus FRD and Group 2: PowerScope 2 appliance. The skeletal, dental, soft tissue, and airway changes were noted at T0 (pretreatment), T1 (preappliance therapy), and T2 (postappliance therapy). Acoustic Pharyngometry (Eccovision® Acoustic Pharyngometer) was done to ascertain changes in mean airway volume and area with both modalities noninvasively. Treatment duration and chairside time in appliance installation and removal were noted. Patient comfort was compared using a customized questionnaire survey. Results: A significant change was observed in skeletal, dental, soft tissue, and airway parameters after treatment with both modalities (p-value <0.05 for all) with no significant intergroup difference (p-value >0.05). Chairside time spent in appliance insertion was significantly lower with PowerScope 2 appliance (10.33 min) as compared to the Forsus FRD appliance (18.28 min) (p-value <0.05). Visual Analogue Scale scores for parameters such as problems in mastication, problems in speech, and problems in oral hygiene maintenance were significantly lower with PowerScope 2 appliance as compared to the Forsus FRD appliance (p-value <0.05). Conclusions: Although both modalities are effective in the management of Class II malocclusion, the PowerScope 2 appliance scores better in terms of lesser chairside time in appliance installation and better patient comfort.

Abstract:For two given ordinal scales in a measurement process, the present paper investigates how an indistinguishability operator evaluated according to one of these ordinal scales can be converted to another indistinguishability operator w.r.t. the other ordinal scale, and establishes the mathematical base of these conversions under the framework of measurement theory [Krantz, D.H., Luce, R.D., Suppes, P., Tversky, A. (1971) Foundations of Measurement, Vol. 1 (Academic Press, San Diego)]. Additionally, this work exposes the rudimentary facts behind the studies in [“Fuzzy Numbers and Equality Relations”, Proc.
