Abstract:This study was designed to evaluate and compare the usefulness of clear aligners and conventional appliances on Oral Health-Related Quality of Life (OHRQoL) in pediatric population. Emphasis was placed on the relative benefits and implications of employing clear aligners owing to their escalating prevalence and acceptability. The study participants were divided into four groups: Clear Aligner Group (CAG), Conventional Appliance Group (ConAG), Malocclusion Control Group (MCG), and Normal Control Group (NCG). Parameters including sociodemographic indicators and daily routines were assessed. OHRQoL was evaluated via the Child Perceptions Questionnaire (CPQ). Psychological conditions were assessed through the Depression, Anxiety and Stress Scale (DASS). Statistical differences were found between the four groups regarding CPQ subscales and total scores (p < 0.05). CAG was better than ConAG (p < 0.05) regarding the scores of functional limitations, emotional and social well-being, and total score, however no significant difference was discovered in the oral symptoms scores (p = 0.62). Moreover, all the treatment groups had worse OHRQoL compared to NCG (p < 0.05). Malocclusions and their treatments did not increase the psychological distress as per the DASS results. A novel correlation between the excessive tooth brushing and reduced OHRQoL was also observed (p < 0.05). The study herein emphasized the benefits of clear aligners in children and adolescents with OHRQoL. It was highlighted that the clear aligners had potential and were preferred for the adolescent orthodontic treatment.
Treatment Satisfaction and Quality of Life Changes in Patients Receiving Conventional Fixed Orthodontic Appliances and Clear Aligner Therapy: A Questionnaire Based Cross-sectional Study

Abstract:Introduction: The orthodontic outcome is mainly decided by the objective evaluation by orthodontists with minimal subjective considerations. Although there is increasing patient acceptance of orthodontic aligners for malocclusion treatment, minimal evidence exists regarding their efficacy and adverse effects compared to conventional fixed orthodontic appliances. Aim: To assess patient satisfaction and quality of life changes after these two modalities of orthodontic treatment and to analyse the influencing co-factors. Materials and Methods: A web-based three-part questionnaire survey was done amongst 303 participants who underwent either fixed orthodontic therapy or invisalign treatment. The first part collected the participants’ demographic details and their treatment type. The second part of five questions addressed their preferences, facilitators, and factors preventing them from undergoing orthodontic treatment. The final part of ten questions measured the satisfaction ratio on a five-point Likert scale. Descriptive statistics were computed to provide an overview of responses using frequencies and percentages. Inferential statistics were computed using the chi-square test, and a p-value less than 0.05 was considered statistically significant. Results: This study of 303 orthodontic patients found no significant difference in overall satisfaction between fixed braces and clear aligners (p=0.989), although aligners were favoured for facial appearance, increased treatment costs were a concern for the majority. Female participants (185, 61.1%) constituted the majority of the participants, with 178 (58.7%) aged between 19- 29 years. Clear aligner users experienced more discomfort with additional visits and appointment booking, while fixed braces users had greater concerns about appearance and increased brushing time. Younger patients reported higher satisfaction across several treatment aspects (p=0.013). Men were more satisfied with treatment duration and costs than women. Conclusion: In conclusion, while clear aligners provide aesthetic and hygiene benefits, overall satisfaction was comparable to fixed braces, with cost being a significant factor. Comprehensive consultations are essential to align treatment with patient needs. Future research should involve larger, more diverse samples for validation.

Abstract:Background: Align technology has developed greatly over past few years. Patients tended to prefer clear aligners over conventional brackets because of the superior comfort and esthetics, while the effectiveness of clear aligners was still controversial. The aim of this systematic review was to verify whether the treatment effectiveness of clear aligners was similar to the conventional fixed appliances. Methods: A comprehensive search of the Pubmed, Web of Science, Embase, Scopus, and Cochrane Central Register of Controlled Clinical Trials Register databases for studies published through to August 20, 2018 was conducted. Comparative clinical studies assessing the effectiveness of clear aligners compared with braces were included. Results: Eight papers were included in this study. Two of the included papers were randomized controlled trials and six were cohort studies. Clear aligners might not be as effective as braces in producing adequate occlusal contacts, controlling teeth torque, increasing transverse width and retention. While no statistically significant difference was found between two groups in Objective Grading System score (WMD = 8.38, 95% CI [- 0.17, 16.93]; P = 0.05). On the other hand, patients treated with clear aligners had a statistically significant shorter treatment duration than with braces (WMD = - 6.31, 95% CI [- 8.37, - 4.24]; P < 0.001). Conclusion: Both clear aligners and braces were effective in treating malocclusion. Clear aligners had advantage in segmented movement of teeth and shortened treatment duration, but were not as effective as braces in producing adequate occlusal contacts, controlling teeth torque, and retention.

Abstract:Design Systematic reviewStudy population Orthodontic patients undergoing treatment with removable thermoplastic retainers compared with conventional fixed orthodontic appliances. The mean age of patients was between 15 and 33 years. Data sources Six electronic databases including: Scopus, Web-of-Science, PubMed, Cochrane, Clinical Trials and Grey Literature were searched in addition to a search of references in eligible studies with no restriction of language. Out of 559 studies, 55 were eligible. Only four articles were included; three non-randomised and one randomised controlled trial (RCT) involving 96-182 patients. The studies included were completed within 2005-2016.Study selection Clinical trials involving orthodontic patients undergoing treatment using thermoplastic removeable aligners, namely Invisalign, in comparison to conventional fixed orthodontic appliances of either self-ligating or tip-edge bracket systems were included.Data extraction and synthesis Two members of the research team independently selected articles which fulfilled the inclusion criteria and compared results prior to data collection. A third party examiner was available to resolve disagreements. The study quality was analysed for presence of bias; three non-randomised studies were evaluated and scored using the Methodological Index for Non-Randomised Studies (MINORS), one study was considered high methodological quality, two were considered moderate. The RCT was assessed using the Cochrane Collaboration Common Scheme for Bias and was deemed to be low risk of bias. Results The results suggest that Invisalign can provide successful alignment, however, it poses difficulties in achieving occlusal contacts, buccolingual inclination and vertical movement of teeth. One study found that Invisalign cases are more susceptible to relapse, which can be explained through the tipping mechanism compared to bodily movement, of conventional fixed appliances. Furthermore some studies show that although using aligners can result in overall shorter treatment time the final occlusion was not favourable. The outcome of the RCT, showed that aligners can be successful in treating class I malocclusions in extraction cases.Conclusions This systematic review found that the use of clear aligners made no significant difference to treatment time compared with conventional appliances. Additionally, removable aligners were effective in crowding cases but had limitations in achieving favourable outcomes in the anterior-posterior position and final occlusion, as well as being more likely to relapse post treatment.

Abstract:Objective To compare the level of oral health-related quality of life (OHRQoL) between patients receiving clear aligners or fixed appliances within one year of follow-up using Oral Health Impact Profile 14 (OHIP-14), a validated self-administered questionnaire. Materials and methods A single-centered, two-arm parallel-group randomized controlled clinical trial was conducted on 36 adult patients (19 females, 17 males; age range: 18 to 25 years) who had severe crowding and require orthodontic treatment with first premolars extraction. The patients were equally and randomly divided into two groups: The clear aligners (CA) group and the fixed appliances (FA) group. OHRQoL was assessed using the OHIP-14 tool at various times during comprehensive orthodontic therapy: baseline (T0), one week (T1), two weeks (T2), one month (T3), 6 months (T4), and 12 months (T5) after starting the active orthodontic treatment. Mann-Whitney U test or Friedman test were used to detect significant differences. The level of significance was set at 5%. Results All of the selected patients entered the statistical analysis stage. There were no significant differences between the CA and FA groups for the psychological discomfort, psychological disability, social disability, and handicap (P˃0.05) at almost all assessment times. For the functional limitation, physical pain, physical disability, and the overall score, there were significant differences between the studied groups (P˂0.05), with the FA group having higher mean scores than the CA group in all of the assessment times. Conclusion Patients' treatment with clear aligners has less impact on OHRQoL than those treated using conventional fixed appliances during the first year of treatment.

Abstract:Abstract Objective This study aimed to compare the pain intensity and impacts on oral health-related quality of life (OHRQoL) between orthodontic patients treated with clear aligners (CAs) and fixed appliances (FAs). Methods A systematic search was conducted up to December 2022 using PubMed, Web of Science, Cochrane Central Register of Controlled Trials, and Embase. Randomized controlled trials (RCTs) and prospective non-randomized controlled trials (non-RCTs) comparing pain intensity or OHRQoL between patients treated with CAs and FAs were included. The risk of bias (RoB) of individual studies was evaluated using the Cochrane RoB tool 2.0 and ROBINS-I tool for RCTs and non-RCTs, respectively. Further, meta-analyses were separately conducted for each included study using the total oral health impact profile (OHIP)-14 and visual analog scale (VAS) scores to evaluate OHRQoL and pain intensity, respectively. Results Overall, 12 studies (5 RCTs and 7 non-RCTs) were included in the study. Subgroup analyses conducted according to the total OHIP-14 scores revealed that patients treated with CAs had higher OHRQoL at 1 week, 1 month, and 6 months of the treatment. Meanwhile, subgroup analyses conducted according to the VAS scores revealed that pain levels were lower in the CA group only at 3 and 4 days of the treatment. Conclusions Patients treated with clear aligners had higher OHRQoL than those treated with fixed appliances during orthodontic treatment. However, OHRQoL appeared to be similar between the two groups at the end of the treatment. Moreover, patients treated with clear aligners experienced lesser pain than those treated with fixed appliances on the third and fourth day after the initial treatment. The difference in pain intensity between the two treatment modalities was not noted at other time points.

Risk of root resorption between Invisalign and fixed orthodontic treatment: A retrospective study

Abstract:Objectives: The aim of our study is to evaluate and compare the presence and severity of external root resorption in patients treated with either clear aligners (Invisalign) versus conventional fixed orthodontic appliances. Material and Methods: A retrospective study was performed on 203 patients, which includes 30 treated with Invisalign and 173 with fixed appliances. 60 cases (30 per group) were matched based on extraction status, treatment duration, gender, age, and malocclusion classification after inclusion criteria were applied. Radiographic evaluations were conducted with standardized periapical radiographs and Panoramic radiographs (OPG). Comparing radiographs taken before and after treatment allowed for the measurement of the extent of external root resorption. Results: In the matched cases, external root resorption was present in 92% of patients in the Invisalign group and 77% of patients in the fixed appliance group. Maxillary incisors were the most afflicted teeth, particularly in situations involving tooth extractions. The greatest percentage of root resorption (100%) was recorded in Class II malocclusion, which was followed by Class I (84%) and Class III (77%). Although Invisalign treatments demonstrated an increased incidence of root resorption, fixed appliances were associated with more severe root reduction. There were not significant distinctions between the groups, according to statistical analysis (p > 0.05). Conclusions: A frequent adverse effect of orthodontic treatments involving both fixed appliances and Invisalign is external root resorption. Root resorption was more common in Invisalign patients, but it was greater in fixed appliance cases. These results highlight the need for more expansive prospective studies to confirm these findings and enable clinicians minimize side effects associated with treatment.
Comparison of Patient Satisfaction between Invisible Appliance and Fixed Orthodontic Appliances - A Systematic Review

Abstract:To compare the patient acceptance between fixed orthodontic appliances and clear aligners. An electronic search was completed in PubMed, Cochrane Database, Web of Science, Scopus, Lilacs, Google Scholar, and Open Grey databases without any restrictions until August 23. All comparative study types contrasting patient perception between clear aligners and fixed appliances were included. After removal of duplicates, exclusion by title and abstract, and reading the full text, only eight articles were included. The studies used an observational questionnaire study design. Of the observational studies, one was retrospective and three prospective, one online survey, and the other three cross sectional. Though the response varied in terms of patient preference and acceptance, it was found that clear aligners have a positive impact with respect to quality of life, tolerance, and improved confidence. However, there is not enough evidence to make a final conclusion about treatment outcomes.

Abstract:The aim of the study was to analyse variations in patients' perception on oral health, aesthetic acceptance, pain perceptions and comfort levels in patients who has undergone/undergoing orthodontic treatment by means of two types of invisible orthodontic appliances: fixed lingual metal brackets and fixed buccal aesthetic/ceramic brackets. A comparative survey to assess the patient perception after their initial levelling alignment phase was created via Google form and sent to 25 ceramic labially treated and 25 lingually treated patient. Comfort level, pain perception, speech impediment ,quality of life in patients was significantly better in Ceramic brackets as compared to lingual (p=0.001),Aesthetic perception was significantly higher impact on ceramic (p=0.001) as 72% reported no impact on aesthetics with lingual brackets as compared to 4% in the ceramic brackets. Oral hygiene maintenance: The difference on impact of Oral hygiene maintenance was statistically non- significant between ceramic brackets and lingual brackets (p=0.238).Difficulty while having food was statistically non- significantly between ceramic brackets and lingual brackets.(p=0.448). Patients prefer lingual treatment cause of their esthetic nature and their expectations are higher with lingual treatment which itself is not favorable from a clinicians perspective and it is worth noting that speech disturbances may lead to greater social embarrassment than more or less invisible ceramic brackets.

Abstract:Annealing an unsaturated sample of soft amorphous magnetic ribbon or thin film displays a characteristically pinned hysteresis loop. 1,2. In this paper narrow ribbons of a cobalt-rich nonmagnetostrictive metallic glass (dimensiobs 75mmxlrnmx35um) are studied in more detail in order to understand the characteristics of this phenomenon. Figure 1 shows a characteristic hysteresis curve obtained after annealing when the sample is exposed to a moderate magnetic field of a few Oersteds maximum. This loop can be unstable at high maximum fields where it can change to that of a normal sample with high demagnetization field. The model that was used assumed that the anisotropy induced by the anneal followed the direction of the magnetization at the time of the anneal even when the magnetization made a domain wall. This induced helical anisotropy could then trap the wall as it passed the site creating a loop as seen in Fig. 1. The model has now been confirmed by direct observation of the pinning site. Figure 2 shows an example of a direct observation of the domain wall and the pinning site, obtained with the digitally enhanced magnetooptic Kerr microscope adjusted to observe the 1ongitudina.l Kerr effect along the axis prependicular to the ribbon axis 3. A finger shaped domain can be seen on the left with a single domain on the right. Even without a domain wall on the right, a contrast appears at the pinning site indicating a "quasi wall"-a magnetization excursion with a zero angle of total rotation, induced by the local anisotropy.

Abstract:A palladium-catalyzed carbonylative reaction of 1-(2,2-dibromovinyl)-2-alkenylbenzene and carbon monoxide, with phenol or alcohol, is reported, which affords 1-methylene-1H-indene-2-carboxylates in moderate to good yields. This cascade process, which includes carbonylation and Heck reaction, proceeds smoothly with good functional group tolerance and high selectivity.
Smile outcome comparison of Invisalign and traditional fixed-appliance treatment: A case-control study

Abstract:Introduction: The purpose of this study was to evaluate and compare smile treatment outcomes between patients treated with Invisalign clear aligners (Align Technology, Santa Clara, Calif) and those treated with traditional fixed appliances by integrating variables such as lip symmetry, smile index, smile cant, buccal corridors, and gingival display into smile outcome evaluation. Methods: Records from 58 patients, 29 of whom received Invisalign treatment (mean age 19.03 years) and 29 of whom received traditional fixed-appliance treatment (mean age 13.83 years), were compared for their smile outcome. Pretreatment scores, posttreatment scores, and differences between within-group smile score before and after treatment were determined for each group. Fifteen variables comprised the smile outcome, and the data were analyzed with the Mann-Whitney U test and the Wilcoxon t test for 2 dependent samples. Results: Six variables within the fixed-appliance group presented with better smile scores than those within the Invisalign group; buccal corridors (%) (mean difference = 8.42%), buccal corridors (mm) (5.35 mm), smile cant (0.42°), maxillary dental midline (0.21 mm), gingival display (0.56 mm), and smile index (1.09%) for P <0.05. Invisalign performed better on 2 variables that determined maxillary incisor position (1.26 mm) and inclination (2.09°). No significant difference (for P = 0.05) was shown between pretreatment and posttreatment scores for either of the 2 groups. Conclusions: Using the 15 variables in this study, the results suggested that for patients with Class I nonextraction, treatment with traditional fixed appliances changes the patient's smile more than Invisalign treatment, and fixed appliances appear to be more effective in improving the variables that quantify posttreatment smile outcome.

Abstract:Clear aligner therapy has emerged as a popular alternative to traditional fixed orthodontic treatment, particularly among adult patients seeking aesthetic and comfortable options. These aligners, made of clear thermoformed plastic, offer advantages such as improved aesthetics, comfort, oral hygiene, and periodontal health compared to fixed appliances. Recent studies have shown that clear aligners can be equally effective as fixed appliances, if not more so, for treating mild to moderate malocclusions, with shorter treatment durations, fewer appointments, and reduced emergency visits. While clear aligners may not be as effective for complex cases requiring additional techniques, advancements in technology have expanded their applicability, allowing for the incorporation of methods used in traditional braces. However, more extensive research is needed to fully understand their impact on oral health and the oral microbiome, spanning all treatment phases from initiation to maintenance. Additionally, various brands of clear aligners with different materials and designs have entered the market, offering alternatives to the widely known Invisalign™, though scientific literature on these alternatives remains limited. This article discusses about the efficacy of clear aligners as compared to fixed appliances in orthodontic treatment.

Treatment effectiveness of young adults using clear aligners versus buccal fixed appliances in class I malocclusion with first premolar extraction using the ABO-Objective Grading System: A randomized controlled clinical trial

Abstract:Objective: To compare the effectiveness of the clear aligners with the traditional fixed appliances in the treatment of premolars extraction complex cases using the American Board of Orthodontics Objective Grading System (ABO-OGS). Material and methods: A single-centre, 2-parallel groups RCT with two arms. Forty severe crowding patients (14 males, 26 females; mean age: 21.40±2.42) who required four first premolars extraction were included and randomly allocated into two treatment groups: clear aligners therapy group (CAT), and fixed appliances therapy group (FAT). Cases complexities were measured on pre-treatment records using the Discrepancy index (DI). Post-treatment records were evaluated using the American Board of Orthodontics Objective Grading System (ABO-OGS). Two sample t-tests and Fisher's Exact tests were used to test for significant differences between the two groups. The statistical significance was set at P < 0.006 using Bonferroni's correction. Results: For the DI, the mean scores were 32.25 (± 4.33) in the CAT group and 33 (± 7.92) in the FAT group. In the CAT group, the total OGS score ranged between 6-33 points with an average of 17.50(± 7.41), whereas the total score in the FAT group went between 4-30 points with an average of 12.89 (± 6.31) with no significant differences between the two groups (P=0.05). When comparison of the successful cases between the two groups was made, 11 cases received passing scores, and 9 cases received failing scores in the CAT group. Whereas in the FAT group, 17 cases received passing scores, and 3 received a failing score. No statistically significant differences were found in the passing rates between of the CAT and FAT groups (P = 0.421). Conclusions: According to the ABO-OGS total scores, there was no significant difference between the clear aligners and fixed appliances in the treatment of class I severe crowding cases with first premolars extraction in young adults. There were no differences between the two techniques in the OGS components scores except for the occlusal contacts, which were significantly better with the fixed appliances. When comparing the number of successful and failed cases between the two groups, no significant differences were noted, with the fixed appliances having a 30% higher success rate than the clear aligners, which must be considered clinically when choosing between these two techniques in the complex orthodontic cases treatment.

Abstract:Background and Objectives: To compare the oral-health-related quality of life (OHRQoL) outcomes between patients treated with modified Ni-Ti spring-based alignment appliances or conventional fixed appliances using the Oral Health Impact Profile 14 (OHIP-14), as well as the levels of satisfaction with the appliance appearance, treatment progress, and outcomes. Materials and Methods: Thirty-six patients (11 males, 25 females) were randomly divided into two groups: either the modified aligner appliance with Ni-Ti springs group (MAA) or the traditional fixed appliances group (FA). The allocation ratio was 1:1, and the randomization process was carried out by an independent investigator not involved in this study. Mild crowding cases were included in this study. The OHRQoL of patients was evaluated using the short-form Oral Health Impact Profile (OHIP-14) at five time points: before the treatment commencement (T0); 2 weeks (T1), 1 month (T2), and 2 months (T3) after the treatment initiation; and post-treatment (T4). The visual analog scale (VAS) was used to evaluate the patient satisfaction. Blinding was performed only during the outcomes assessment. Results: This randomized controlled trial had no dropouts, and the demographic characteristics of the groups were comparable. The MAA group experienced significantly greater functional limitations compared with the FA group at all three evaluation time points (T1, T2, and T3), as evidenced by the statistically significant p-values (p = 0.004, p = 0.001, and p < 0.001, respectively). The psychological disability in the MAA group was significantly lower than in the FA group at both T2 (p = 0.005) and T3 (p = 0.003). The patient satisfaction with the appliance appearance was significantly higher in the MAA group than in the FA group (p = 0.002). Conclusions: The OHRQoL improved in both the modified aligner appliance with Ni-Ti springs and fixed appliance groups after the treatment. Moreover, the functional limitations during the treatment were less severe in the FA group, while the psychological disability was lower, and the patient satisfaction with the appliance appearance was higher in the MAA group.
Cephalometric evaluation of changes in vertical dimension and molar position in adult non-extraction treatment with clear aligners and traditional fixed appliances

Abstract:ABSTRACT Introduction: Orthodontists have been using clear aligners to treat malocclusions, and one potential effect of treatment with orthodontic aligners is the intrusion and/or resists extrusion of the posterior teeth. This “bite-block effect” is primarily anecdotal due to the frequent occurrence of posterior open bites in patients after clear aligner therapy. Objective: The purpose of this study was to compare changes promoted by clear aligners and traditional fixed appliances in cephalometric measurements of the vertical dimension and molar position in adult patients with Class I malocclusion treated with non-extraction. Methods: Pre- and post-treatment lateral cephalometric radiographs of adult patients treated with either clear aligners (n=44) or traditional fixed appliances (n=22) were selected for retrospective analysis. Eight interval measurements and one nominal measurement were evaluated: anterior overbite (OB), mandibular plane angle related to cranial base (SN_MP) and related to Frankfort (FMA), lower molar height (L6H) and upper molar height (U6H), palatal plane to mandibular plane angle (PP_MP), lower facial height (LFH), total facial height (TFH), and posterior open bite (Posterior_OB). A single evaluator traced all cephalographs, and changes in select measures of the vertical dimension were compared within and between groups. Results: OB decreased (1.15 mm) and L6H increased (0.63 mm) in the traditional fixed appliance group. Mandibular plane angles (related to cranial base and to Frankfort) increased (0.43° and 0.53°, respectively) in the clear aligner group, but just FMA showed significant difference between groups (difference of 0.53°). LFH and TFH increased (ranging from 0.52 mm to 0.80 mm) in both groups, with no differences between treatment modality. Presence of visible posterior open bite significantly increased over the course of treatment. OB, FMA and L6H exhibited an interaction between treatment stage (pre- and post-treatment) and modality (clear aligner therapy and traditional fixed appliances), but no interaction among these three variables was found. Conclusions: The evidence does not support the theory that clear aligner therapy produces better vertical dimension control than traditional fixed appliances. Traditional fixed appliance therapy slightly extruded the lower molar, and clear aligner therapy produced a slightly mandibular backward rotation.
Occlusal contact changes in patients treated with clear aligners: A retrospective evaluation using digital dental models

Abstract:Introduction This study used digital intraoral scans to evaluate how clear aligner treatment affects occlusal contacts and to determine the influence of sex and age on contact changes. Results were compared with contact changes that occur during fixed appliance therapy. Methods Patients included in this study were treated in a University setting and private practice. Inclusion criteria were a Class I malocclusion treated nonextraction with clear aligners and the presence of pretreatment and posttreatment digital intraoral scans. Scans were imported into specialized software, and occlusal contacts were analyzed. The effects of age and sex on contact changes during clear aligner treatment were determined. Changes in occlusal contacts were compared with changes that occur during nonextraction treatment of patients with a Class I relationship using fixed edgewise appliances. Results A total of 45 clear aligner patients fit the eligibility criteria. Clear aligner treatment reduced the percentage of tight, near, and approximating contacts, whereas the percentage of open and no contacts increased. These changes in occlusal contacts were greater for the older age group studied. Genderinfluenced occlusal contact changes in the anterior dentition only where the decrease in near contacts and increase in open contacts were greater for males. These results for patients treated with clear aligners were similar to those for patients treated with fixed appliances; both treatment modalities reduced close occlusal contacts at the time active treatment was completed. Conclusions These results indicated that when clear aligners or fixed appliances are used to treat a Class I malocclusion, the resulting occlusion immediately after debonding is not as "tight" as it was at pretreatment.

Abstract:PURPOSE Orthognathic surgery for dentofacial deformities is typically preceded and followed by orthodontic treatment. Traditionally, orthodontic hardware is secured to the dentition to allow dental movement and stabilization. Clear-aligner therapy (eg, Invisalign) provides an aesthetic alternative, consisting of a series of transparent trays. Its use has not been described in complex triple-jaw orthognathic surgery. The purpose of this study is to evaluate perioperative outcomes and 3-dimensionally quantify postoperative edema in Invisalign patients undergoing triple-jaw orthognathic procedures, comparing this to patients treated with conventional fixed appliances. The surgical approach to patients with clear-aligners is also outlined. METHODS The authors conducted a retrospective chart review and 3-dimensional morphometric study of Invisalign patients undergoing triple-jaw surgery (LeFort I osteotomy, bilateral sagittal split osteotomy, and genioplasty). An identical assessment of demographically matched patients treated with conventional fixed appliances was performed and compared with the Invisalign group. RESULTS Thirty-three patients, with a mean age of 19.99 years, were included: 13 with Invisalign and 20 with conventional fixed appliances. No significant difference was observed in operating time, concurrent extraction of teeth, fat grafting, duration of hospital stay, diet advancement, and use of narcotic analgesics between the 2 groups. Nine patients had sufficient 3-dimensional images for volumetric analysis (4 with Invisalign and 5 with conventional fixed appliances). Postoperative edema was not significantly different (P = 0.712) when comparing conventional fixed appliances (44.29 ± 23.16 cm) to Invisalign (37.36 ± 31.19 cm). CONCLUSION The present study demonstrates that complex multiple-jaw orthognathic procedures can be successfully performed in Invisalign patients. Perioperative and short-term clinical outcomes are not compromised.

Abstract:We aimed to compare the pain discomfort levels between clear aligners and fixed appliances at multiple time points. Four electronic databases (Pubmed, Medline, CENTRAL and Scholar) were searched up to May 2020. There were no year or language restrictions. Randomized clinical trials and case–control studies comparing pain perception through pain visual analog scale (VAS) in patients treated with clear aligners and with fixed appliances were included. Risk of bias within and across studies was assessed using Cochrane tool and Newcastle–Ottawa Scale (NOS) approach. Random-effects meta-analysis were conducted. VAS score and analgesic consumption were collected. Random-effects meta-analyses were used to synthesize available data. Following the review protocol, five articles met the inclusion criteria and were included, with a total of 273 participants (177 females, 96 males). Overall, clear aligners were associated with significantly less pain than fixed appliances during the first seven days of orthodontic treatment. Patients treated with clear aligners experience less pain discomfort than those treated with fixed appliances and consume less analgesics, with SORT A recommendation.

Abstract:Torque control of maxillary incisors is very important in maxillary protrusion patients with first premolars extraction, but the efficacy of maxillary incisor retraction of clear aligners is still controversial now. This retrospective study was aimed to compare the retraction effects between Damon Q and Invisalign (R) appliances in patients with first premolar extractions. 59 patients (33 cases with Damon Q and 26 cases with Invisalign (R)) with first premolar extraction were selected in this study. Subsequently, patients of each group were allocated into three subgroups according to the pretreatment value of U1-NA (degrees). The retraction effects of maxillary incisors and upper lips were accessed by the variations of cephalometric, overbite and overjet measurements. Treatment duration with Invisalign (R) (31.4 +/- 6.4 months) was longer than Damon Q (27.7 +/- 6.3 months) (P = .03). The angular measurements, U1-NA (degrees) and U1-SN (degrees) showed more lingual crown inclinations in Invisalign (R) group than Damon Q group (P <= .04). When evaluating linear measurements, the retractions of the maxillary incisors and upper lip positions showed no significant differences (P >= .13). Invisalign (R) group also showed more lingual crown retractions and labial root deviations compared to Damon Q group in subgroup III (P <= .037). As regards to the molar relationship, Invisalign (R) displayed less Class I molar relationship than Damon Q group. The increased overbite of anterior incisors was also showed in the Invisalign (R) treatment group (P <= .047). Invisalign (R) was not sufficiently effective in retracting maxillary incisors compared with Damon Q appliances. Invisalign (R) led to more lingual inclination movement and increased overbite.
