Abstract:A non-human primate is a valuable model for investigating the structure and function of the brain. Different from the human brain imaging using radio frequency (RF) head coils, in the present study, on a human whole-body 7 T magnetic resonance imaging system, we used an RF knee coil for monkey brain imaging in vivo due to the smaller size of the macaque’s brain compared to that of a human, and particularly, high-dielectric pads were also utilized in order to improve brain imaging performance. Our experimental results suggest that high-dielectric pads can effectively enhance the B1 field strength and receive sensitivity, leading to a higher flip-angle magnitude, an image signal-to-noise ratio, and tissue contrast, and in the meantime, we did not observe elevated receive array element coupling and receive noise amplification nor apparent magnetic susceptibility-induced artifact or distortion, showing that the pads do not introduce adverse RF interferences in macaque brain imaging at 7 T.

Abstract:Purpose: Low-field MRI has been assumed to be implant-friendly based on limited studies. However, RF-induced heating due to an implant is a complex resonance phenomenon, highly dependent on the implant's configurations and the applied RF frequencies. This study aims to evaluate the RF heating of DBS implants during MRI at low-field strengths compared to higher field 1.5 T MRI. Methods: A commercial deep brain stimulation (DBS) implant was used in full system as well as lead only configurations to evaluate and compare RF heating during MR imaging at 0.55 T and 1.5 T. The transfer function of the device at both configurations was measured and validated at each of the frequencies, which was then used for the in vivo prediction of RF heating for realistic DBS configurations at head, chest and abdomen imaging landmarks. Results: For the lead only case, the RF heating due to the DBS was substantially smaller during imaging at 0.55 T compared to that at 1.5 T. However, for the full DBS system (longer implant), the RF heating at 0.55 T was comparable to and for some cases even higher than that at 1.5 T, reaching a level that poses risk of tissue damage in patients. Conclusions: While RF heating generally tends to be lower at low-field MRI, the case with longer implanted leads demands extra caution, due to the higher possibility of matching resonant condition at low-field-strength frequencies. Thus, specific risk evaluation for each implant and configuration is required rather than assuming that lower field strength imaging is safer.

Pathological Findings Around Deep Brain Stimulation Leads in Ultra-High Field MRI

Abstract:Objective To determine the pathological responses in tissue around the deep brain stimulation (DBS) leads in 1.5 T,3.0 T and 7.0 T magnetic resonance imaging (MRI).Methods Eighteen male Newzealand rabbits were randomly assigned to ultra-high field 7.0 T group (n =6),ultra-high field 3.0 T group (n =6) and the routine 1.5 T group (n =6).DBS devices were implanted into the rabbit brains of each group targeting the left VPN of thalamus.MRI scanning of corresponding field strength was conducted.After 24 hours,the brains were processed for HE staining and transmission electron microscopy (TEM).The results were semiquantified using our scoring system.All data were shown as mean deviation,and processed by one-way ANOVA analysis using SPSS 19.0 software.A P value less than 0.05 was considered statistically significant.Results In HE staining and TEM,the difference between the ultra-high field groups and the routine 1.5 T group was not significant (P ＞ 0.05).Conclusions In this study,the heating of DBS lead in ultra-high field MRI scanning failed to induce more injury than in the routine 1.5 T MRI,indicating that the ultra-high magnitude MRI scanning might not be as dangerous to the patients with DBS devices as we thought.

Abstract:Conformationally flexible analogues of the atypical antipsychotic sertindole (1-[2-[4-[5-chloro -1-(4-fluorophenyl)-1H-indol-3-yl]-4-piperidinyl]ethyl]-2-imidazolidi non e) were synthesized. Replacement of the 4-piperidinyl ring in sertindole by a 2-(methylamino)ethoxy group or a 2-(methylamino)ethyl group (e.g. 1-[2-[2-[5-chloro-1-(4-fluorophenyl)-1H -indol-3-yloxy]ethyl-methylamino]ethyl]-2-imidazolidinone and 1-[3-[[2-[5-chloro-1-(4-fluorophenyl)-1H-indol-3-yl] -ethyl]methylamino]propyl]-2-imidazolidinone results in binding affinities for serotonin 5-HT2A and dopamine D2 receptors, as well as alpha 1 adrenoceptors, which are very similar to those of sertindole. (Methylamino)alkyl groups of other chain lengths, 3-(methylamino)propyloxy groups, and 2-(methylamino)ethylsulfanyl groups do not have such properties. The capability of the 2-(methylamino)ethoxy group and the 2-(methylamino)ethyl group to replace the 4-piperidinyl ring in sertindole is reflected in molecular modeling studies using recently published receptor-interaction models for 5-HT2 and D2 receptors. Structure-affinity investigations concerning the substituents in the indole nucleus and the 2-imidazolidinone ring system in the 2-(methylamino)ethoxy and the 2-(methylamino)ethyl analogues of sertindole have led to high affinity serotonin 5-HT2A receptor antagonists with selectivity versus dopamine D2 receptors and alpha 1 adrenoceptors (e.g. 1-[2-[[2-[6-chloro-1-(4-fluorophenyl) -1H-indol-3-yloxy]ethyl]methylamino]-ethyl]-2-imidazolidinone and 1-[3-[[2-[6-chloro-1-(4-fluorophenyl) -1H-indol-3-yl]ethyl]methylamino]propyl]-2-imidazolidinone). The latter derivative has also high selectivity for 5-HT2A receptors versus serotonin 5-HT2C receptors. Replacement of the basic amino group by nitrogen-containing six-membered rings has led to 5-chloro-1-(4-fluorophenyl)-3-[(4-methylpiperazinyl)-ethoxy]-1H-in dole, which has high affinity for dopamine D2, versus low affinity for serotonin 5-HT2A receptors and alpha 1 adrenoceptors.

Abstract:Deep brain stimulation (DBS) is an effective therapy for treating neurological disorders, while magnetic resonance imaging (MRI) is a valuable method for clinical diagnosis and research. However, DBS systems in clinical MRI are under the influence of displacement force, torque and device vibration, which may be hazardous to patients after DBS implantation surgery. In this study, we evaluated these hazardous effects under 3T MRI. A set of apparatus were designed and built and the corresponding methods for safety assessment were also proposed. Results showed that the displacement force, torque and vibration of the DBS system under 3T MRI were safe. Furthermore, this study demonstrates an example of an MRI compatibility test that can be applied to many other medical instruments.

Abstract:OBJECTIVE The aim of this study was to compare outcomes of direct targeting in deep brain stimulation (DBS) for essential tremor using 7T MRI versus 3T MRI. The authors hypothesized that 7T MRI direct targeting would be noninferior to 3T MRI in early tremor outcomes. METHODS A retrospective study was conducted on patients undergoing unilateral thalamic DBS for essential tremor between 2021 and 2023. Two matched cohorts were assessed, one using 7T MRI and the other using 3T MRI for surgical planning. The primary endpoint was the percentage improvement in the Fahn-Tolosa-Marin Tremor Rating Scale (TRS) scores. Additionally, the authors assessed optimized programming settings and variance in electrode position on postoperative imaging. Demographic and clinical data were compared using the nonparametric Mann-Whitney U-test. The squared Euclidean distance of each contact from the group mean centroid was calculated and averaged across the entire cohort to provide the variance (i.e., the mean squared distance) of electrode contact position. RESULTS A total of 34 patients were analyzed, with 17 in each cohort. There were no significant differences in demographic information or mean surgical dates between the groups. There were no differences in intraoperative target repositioning or adverse events. The 7T group had a significantly greater TRS improvement than the 3T group (64.9% ± 11.4% vs 50.9% ± 16.4%, p = 0.004). Patients in the 7T cohort also had a lower mean stimulation current compared with those in the 3T cohort (2.0 ± 0.8 mA vs 2.7 ± 0.9 mA, p = 0.01). Image evaluation revealed that although the mean electrode position was comparable between 7T and 3T, the 7T electrode positioning was more clustered, indicating a lower variance in the final electrode location. The mean Euclidean distance between the individual electrode tips and the group centroid was significantly less at 7T than at 3T (1.82 ± 0.68 mm vs 2.75 ± 0.81 mm, p = 0.001). CONCLUSIONS Despite concerns for increased artifacts and distortions at 7T, the authors show that these effects can be mitigated with an appropriate workflow, leading to improved surgical outcomes with direct targeting using 7T MRI. Their results suggest similar accuracy but greater precision in targeting with 7T MRI compared with 3T MRI, resulting in lower stimulation currents and improved tremor reduction. Future studies are needed to assess outcomes related to 7T MRI in targeting other subcortical structures.

Abstract:A health care reform for senior citizens took effect in 1992 in Sweden. Shortcomings as regards geriatric rehabilitation were identified and, in 1995, governmental grants were assigned to local developmental projects. The aim of this study was to report the staff and administration perspectives in a geriatric rehabilitation developmental process over a 1-year period. In all, 782 staff and 45 administration respondents took part. The investigation was based on questionnaires and series of interviews. Significant positive changes with respect to collaboration between the authorities involved were demonstrated at the administrative level, while the most positive results at staff level concerned competence development in the group of employees without college education. A positive process as regards geriatric rehabilitation has started in the study district, indicating that governmental grants assigned for the stimulation of developmental activities are beneficial. Official goals for geriatric rehabilitation, points of departure for the organization of rehabilitation services and the content of general and specialized rehabilitation programmes are pinpointed as some of the areas deserving further attention.
Clinical 7T MRI for epilepsy care: Value, patient selection, technical issues, and outlook

Abstract:Ultra-high-field 7.0 Tesla (T) MRI offers substantial gains in signal-to-noise ratio (SNR) over 3T and 1.5T, but for over two decades has remained a research tool, while 3T scanners have achieved widespread clinical use. This much slower translation of 7T relates to daunting technical challenges encountered in ultra-high-field human MR imaging. The recent introduction of United States Food and Drug Administration (FDA)-approved clinical 7T scanners promises to be a watershed for many 7T neuroimaging applications, including epilepsy imaging. The high SNR of 7T allows clinical imaging of fine neuroanatomic detail at unprecedented spatial resolution, helping with detection and differentiation of subtle, potentially treatable lesions undetectable or suboptimally assessed at 3T. The accompanying research paper reports our group's analysis of 7T MRI efficacy in epilepsy treatment planning. Here, we introduce the technical background and clinical approach we currently use, in order to assist clinical epileptologists and neuroimagers contemplating, creating, or referring patients to a clinical 7T epilepsy imaging service. We describe a tiered epilepsy imaging strategy and protocols designed to optimize 7T value and work around signal intensity variation and signal loss artifacts, which remain significant challenges to full exploitation of 7T clinical value. We describe FDA-approved techniques for mitigating these artifacts and briefly outline techniques currently under development, but not yet FDA approved. Finally, we discuss the major issues in 7T patient safety and toleration, outlining their physical causes and effects on workflow, and provide references to more comprehensive technical reviews for readers seeking greater technical detail.

Abstract:The use of 7 Tesla (T) magnetic resonance imaging (MRI) is expanding across medical specialties, particularly, clinical neurosciences and orthopedics. Investigational 7 T MRI has also been performed in cardiology. A limiting factor for expansion of the role of 7 T, irrespective of the body part being imaged, is the sparse testing of biomedical implant compatibility at field strengths >3 T. Implant compatibility can be tested following the American Society for Testing and Materials International guidelines. To assess the current state of cardiovascular implant safety at field strengths >3 T, a systematic search was performed using PubMed, Web of Science, and citation matching. Studies written in English that included at least 1 cardiovascular-related implant and at least 1 safety outcome (deflection angle, torque, or temperature change) were included. Data were extracted for the implant studied, implant composition, deflection angle, torque, and temperature change, and the American Society for Testing and Materials International standards were followed. The Preferred Reporting Items for Systematic Reviews and Meta-Analyses reporting guidelines for scoping reviews were followed. A total of 9 studies were included. A total of 34 cardiovascular-related implants tested ex vivo at 7 T and 91 implants tested ex vivo at 4.7 T were included. The implants included vascular grafts and conduits, vascular access ports, peripheral and coronary stents, caval filters, and artificial valves. A total of 2 grafts, 1 vascular access port, 2 vena cava filters, and 5 stents were identified as incompatible with the 7 T MRI. All incompatible stents were 40 mm in length. Based on the safety outcomes reported, we identify several implants that may be compatible with >3 T MRI. This scoping review seeks to concisely summarize all the cardiovascular-related implants tested for ultrahigh field MRI compatibility to date.
Optimizing the trajectory of deep brain stimulation leads reduces RF heating during MRI at 3 T: Characteristics and clinical translation

Abstract:Radiofrequency (RF) induced tissue heating around deep brain stimulation (DBS) leads is a well-known safety risk during magnetic resonance imaging (MRI), hindering routine protocols for patients. Known factors that contribute to variations in the magnitude of RF heating across patients include the implanted lead's trajectory and its orientation with respect to the MRI electric fields. Currently, there are no consistent requirements for surgically implanting the extracranial portion of the DBS lead. Recent studies have shown that incorporating concentric loops in the extracranial trajectory of the lead can reduce RF heating, but the optimal positioning of the loop is unknown. In this study, we evaluated RF heating of 77 unique lead trajectories to determine how different characteristics of the trajectory affect RF heating during MRI at 3 T. We performed phantom experiments with commercial DBS systems from two manufacturers to determine how consistently modifying the lead trajectory mitigates RF heating. We also presented the first surgical implementation of these modified trajectories in patients. Low-heating trajectories included small concentric loops near the surgical burr hole which were readily implemented during the surgical procedure; these trajectories generated nearly a 2-fold reduction in RF heating compared to unmodified trajectories.Clinical Relevance- Surgically modifying the DBS lead trajectory can be a cost-effective strategy for reducing RF-induced heating during MRI at 3 T.

Abstract:<h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Purpose</h3><p>Deep brain stimulation (DBS) has become a widely performed surgical procedure for patients with medically refractory movement disorders and mental disorders. It is clinically important to set up a MRI protocol to map the brain targets and electrodes of the patients before and after DBS and to understand the imaging artifacts caused by the electrodes.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Methods</h3><p>Five patients with DBS electrodes implanted in the habenula (Hb), fourteen patients with globus pallidus internus (GPi) targeted DBS, three pre-DBS patients and seven healthy controls were included in the study. The MRI protocol consisted of magnetization prepared rapid acquisition gradient echo T1 (MPRAGE T1W), 3D multi-echo gradient recalled echo (ME-GRE) and 2D fast spin echo T2 (FSE T2W) sequences to map the brain targets and electrodes of the patients. Phantom experiments were also run to determine both the artifacts and the susceptibility of the electrodes. Signal to noise ratio (SNR) on T1W, T2W and GRE datasets were measured. The visibility of the brain structures was scored according to the Rose criterion. A detailed analysis of the characteristics of the electrodes in all three sequence types was performed to confirm the reliability of the postoperative MRI approach. In order to understand the signal behavior, we also simulated the corresponding magnitude data using the same imaging parameters as in the phantom sequences.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Results</h3><p>The mean ± inter-subject variability of the SNRs, across the subjects for T1W, T2W, and GRE datasets were 20.1 ± 8.1, 14.9 ± 3.2, and 43.0 ± 7.6, respectively. High resolution MPRAGE T1W and FSE T2W data both showed excellent contrast for the habenula and were complementary to each other. The mean visibility of the habenula in the 25 cases for the MPRAGE T1W data was 5.28 ± 1.11; and the mean visibility in the 20 cases for the FSE T2W data was 5.78 ± 1.30. Quantitative susceptibility mapping (QSM), reconstructed from the ME-GRE sequence, provided sufficient contrast to distinguish the substructures of the globus pallidus. The susceptibilities of the GPi and globus pallidus externa (GPe) were 0.087 ± 0.013 ppm and 0.115 ± 0.015 ppm, respectively. FSE T2W sequences provided the best image quality with smallest image blooming of stimulator leads compared to MPRAGE T1W images and GRE sequence images, the measured diameters of electrodes were 1.91 ± 0.22, 2.77 ± 0.22, and 2.72 ± 0.20 mm, respectively. High resolution, high bandwidth and short TE (TE = 2.6 ms) GRE helped constrain the artifacts to the area of the electrodes and the dipole effect seen in the GRE filtered phase data provided an effective mean to locate the end of the DBS lead.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Conclusion</h3><p>The imaging protocol consisting of MPRAGE T1W, FSE T2W and ME-GRE sequences provided excellent pre- and post-operative visualization of the brain targets and electrodes for patients undergoing DBS treatment. Although the artifacts around the electrodes can be severe, sometimes these same artifacts can be useful in identifying their location.</p>

7T MRI with Post-Processing for the Presurgical Evaluation of Pharmacoresistant Focal Epilepsy.

Abstract:Background: We aimed to evaluate the diagnostic yield of seven-tesla (7T) magnetic resonance imaging (MRI) with post-processing of three-dimensional (3D) T1-weighted (T1W) images by the morphometric analysis program (MAP) in epilepsy surgical candidates whose 3T MRI results were inconclusive or negative. Methods: We recruited 35 patients with pharmacoresistant focal epilepsy. A multidisciplinary team including an experienced neuroradiologist evaluated their seizure semiology, video-electroencephalography data, 3T MRI and post-processing results, and co-registered FDG-PET. Eleven patients had suspicious lesions on 3T MRI and the other 24 patients were strictly MRI-negative. 7T MRI evaluation was then performed to aid clinical decision. Among patients with pathologically proven focal cortical dysplasia (FCD) type II, signs of FCD were retrospectively evaluated in each MRI sequence (T1W, T2W, and FLAIR), and positive rates were analyzed in each MAP feature map (junction, extension, and thickness). Results: 7T MRI evaluation confirmed the lesion in nine of the 11 (81.8%) patients with suspicious lesions on 3T MRI. It also revealed new lesions in four of the 24 (16.7%) strictly MRI-negative patients. Histopathology showed FCD type II in 11 of the 13 (84.6%) 7T MRI-positive cases. Unexpectedly, three of the four newly identified FCD lesions were located in the posterior quadrant. Blurred gray-white boundary was the most frequently observed sign of FCD, appearing on 7T T1W image in all cases and on T2W and FLAIR images in only about half cases. The 7T junction map successfully detected FCD (10/11) in more cases than the extension (1/11) and thickness (0/11) maps. The 3D T1W images at 7T exhibited superior cerebral gray-white matter contrast, more obviously blurred gray-white boundary of FCD, and larger and brighter positive zones in post-processing than 3T T1W images. Conclusion: 7T MRI with post-processing can enhance the detection of subtle epileptogenic lesions for MRI-negative epilepsy and may optimize surgical strategies for patients with focal epilepsy.

Abstract:There are currently more than 50 ultra-high field (7 T and above) MRI systems installed around the world. The vast majority of these perform studies on humans, whether healthy volunteers, volunteers for clinical research, or actual patients. The increased magnetic field strength potentially raises new safety concerns in the areas of the effects of the static magnetic field itself, increased power deposition due to the higher operating frequency, and acoustic noise. There are currently very different operational modes in different institutions, with policies effectively set by local ethics committees. This article summarizes the current legal and practical status regarding safety of ultra-high field MRI.

Abstract:The combination of electroencephalography (EEG) and functional magnetic resonance imaging (fMRI) at ultra-high field (7 Tesla) offers unprecedented possibilities to probe human brain function non-invasively with high coverage, millisecond temporal precision and sub-millimeter spatial precision, unraveling cortical layers and small subcortical structures. Unfortunately, this technique has remained largely inaccessible at 7T, due to prohibitive cross-modal interference effects and physical constraints. Here, we developed a first-of-its-kind EEG-fMRI acquisition framework on a clinical 7T system combining key improvements from previous works: compact EEG transmission chain to reduce artifact incidence, reference sensors for artifact correction, and adapted leads for compatibility with a dense radiofrequency receive-array allowing state-of-the-art fMRI sensitivity and acceleration. Two implementations were tested: one using an EEG cap adapted in-house, and another using a recently-designed prototype from an industrial manufacturer, intended to be further developed into a commercial product accessible to the broader community. A comprehensive evaluation in humans showed that simultaneous acquisitions, including with sub-millimeter fMRI resolution, could be conducted without detectable safety issues or major practical constraints. The EEG exerted relatively mild perturbations on fMRI quality (6-11% loss in temporal SNR), without measurably affecting the detection of resting-state networks and visual responses. The artifacts induced on EEG could be corrected to a degree where the spatial, spectral and temporal characteristics were comparable to outside recordings, and hallmark features such as resting-state alpha and eyes-closing alpha modulation could be clearly detected. Altogether, these findings indicate excellent prospects for neuroimaging applications, that can leverage the unique possibilities achievable at 7T.

Abstract:Objectives Studies that assess magnetic resonance imaging (MRI) induced radiofrequency (RF) heating of the tissue in the presence of an active electronic implant are mostly performed in horizontal, closed-bore scanners. Vertical, open-bore MRI systems have a 90{\deg} rotated magnet and generate a fundamentally different RF field distribution in the body, yet little is known about the RF heating of deep brain stimulation (DBS) systems in this class of scanners. Here, we investigated whether RF heating of DBS devices was significantly different in a vertical, open-bore MRI scanner compared to a horizontal, closed-bore MRI scanner. Materials and Methods In this phantom study, RF heating around the lead of a commercial DBS system implanted in an anthropomorphic phantom was evaluated in a 1.2 T vertical open-bore scanner (Oasis, Fujifilm Healthcare) and a 1.5 T horizontal closed-bore scanner (Aera, Siemens Healthineers). DBS devices were implanted following 30 realistic lead trajectories. Electromagnetic simulations were performed to assess the specific absorption rate (SAR) of RF energy around leads with different internal structures. Results When controlling for B1+rms, temperature increase around the lead-tip was significantly lower at the vertical scanner compared to the horizontal scanner (p-value=9.1x10-7). Electromagnetic simulations demonstrated up to a 14-fold reduction in the maximum 0.1g-averaged SAR deposited in the tissue surrounding the lead-tip in a vertical scanner compared to a horizontal scanner for leads with straight and helical internal wires. Conclusions RF experiments and electromagnetic simulations demonstrated consistently lower RF heating and power deposition around the DBS lead-tip at the vertical scanner compared to the horizontal scanner. Our simulation results suggest that this trend in heating may potentially extend to leads from other manufacturers.

Abstract:Background: Business executives’ lives have become a never-ending race against time, technology, and targets. This race creates tension, which leads to dissatisfaction and frustration and eventually manifests itself as psychological and physiological stress with mental and emotional drain. This modern lifestyle intensifies the stress leading to “excessive tension” and consequent deterioration in “executive efficiency”. Objective: To assess emotional intelligent quotient (EQ) in managers undergoing yoga-based Self Management of Excessive Tension (SMET) program. Materials and Methods: 72 managers participated in this study which is of “single group pre-post design.” The EQ test developed by Prof N. K. Chadha from University of Delhi was administered as pre and post the 5 days of SMET program. Statistical Analysis: Means, standard deviations, Kolmogorov-Smirnov test, and Wilcoxon signed rank test were used to analyze the data. Results: The data analysis showed 72.02% significant increase (P<0.001) in EQ. Conclusion: The results suggest that participation in a SMET program was associated with improvement in EQ and may have implications for “executive efficiency.”

Abstract:Background and Objectives: Epileptogenic lesions in focal epilepsy can be subtle or undetected on conventional brain MRI. Ultra-high field (7T) MRI offers higher spatial resolution, contrast and signal-to-noise ratio compared to conventional imaging systems and has shown promise in the pre-surgical evaluation of adult focal epilepsy. However, the utility of ultra-high field MRI in paediatric focal epilepsy, where malformations of cortical development are more common, is unclear. This study compared 7T to conventional 3T MRI in children with epilepsy by comparing: (i) scan tolerability; (ii) radiological image quality; (iii) lesion yield. Materials and Methods: Children with drug-resistant focal epilepsy and healthy controls were recruited prospectively and imaged at both 3T and 7T. Safety and tolerability during scanning was assessed via a questionnaire. Image quality was evaluated by an expert paediatric neuroradiologist and estimated quantitatively by comparing cortical thickness between field strengths. To assess lesion detection yield of 7T MRI, a multi-disciplinary team jointly reviewed patients' images. Results: 41 patients (8-17 years, mean=12.6 years, 22 male) and 22 healthy controls (8-17 years, mean=11.7 years, 15 male) were recruited. All children completed the scan, with no significant adverse events. Higher discomfort due to dizziness was reported at 7T (p=0.02), with side-effects more frequently noted in younger children (p=0.02). However, both field strengths were generally well-tolerated and side-effects were transient. 7T images had increased inhomogeneity and artefacts compared to those obtained at 3T. Cortical thickness measurements were significantly thinner at 7T (p<0.001). 8/26 (31%) patients had new lesions identified at 7T which were not identified at 3T, influencing the surgical management in 4/26 (15%). Discussion: 7T MRI in children with epilepsy is feasible, well-tolerated and is associated with a 31% improvement in lesion detection rates.

Abstract:BACKGROUND: 7T cardiac magnetic resonance imaging (MRI) introduces several advantages, as well as some limitations, compared to lower-field imaging. The capabilities of ultra-high field (UHF) MRI have not been fully exploited in cardiac functional imaging.AIM: To optimize 7T cardiac MRI functional imaging without the need for conducting B1 shimming or subject-specific tuning, which improves scan efficiency. In this study, we provide results from phantom and <i>in vivo</i> scans using a multi-channel transceiver modular coil.METHODS: We investigated the effects of adding a dielectric pad at different locations next to the imaged region of interest on improving image quality in subjects with different body habitus. We also investigated the effects of adjusting the imaging flip angle in cine and tagging sequences on improving image quality, B1 field homogeneity, signal-to-noise ratio (SNR), blood-myocardium contrast-to-noise ratio (CNR), and tagging persistence throughout the cardiac cycle.RESULTS: The results showed the capability of achieving improved image quality with high spatial resolution (0.75 mm × 0.75 mm × 2 mm), high temporal resolution (20 ms), and increased tagging persistence (for up to 1200 ms cardiac cycle duration) at 7T cardiac MRI after adjusting scan set-up and imaging parameters. Adjusting the imaging flip angle was essential for achieving optimal SNR and myocardium-to-blood CNR. Placing a dielectric pad at the anterior left position of the chest resulted in improved B1 homogeneity compared to other positions, especially in subjects with small chest size.CONCLUSION: Improved regional and global cardiac functional imaging can be achieved at 7T MRI through simple scan set-up adjustment and imaging parameter optimization, which would allow for more streamlined and efficient UHF cardiac MRI.
Development and testing of implanted carbon electrodes for electromagnetic field mapping during neuromodulation

Abstract:Purpose: Deep brain stimulation electrodes composed of carbon fibers were tested as a means of administering and imaging magnetic resonance electrical impedance tomography (MREIT) currents. Artifacts and heating properties of custom carbon-fiber deep brain stimulation (DBS) electrodes were compared with those produced with standard DBS electrodes. Methods: Electrodes were constructed from multiple strands of 7-μm carbon-fiber stock. The insulated carbon electrodes were matched to DBS electrode diameter and contact areas. Images of DBS and carbon electrodes were collected with and without current flow and were compared in terms of artifact and thermal effects in phantoms or tissue samples in 7T imaging conditions. Effects on magnetic flux density and current density distributions were also assessed. Results: Carbon electrodes produced magnitude artifacts with smaller FWHM values compared to the magnitude artifacts around DBS electrodes in spin echo and gradient echo imaging protocols. DBS electrodes appeared 269% larger than actual size in gradient echo images, in sharp contrast to the negligible artifact observed in diameter-matched carbon electrodes. As expected, larger temperature changes were observed near DBS electrodes during extended RF excitations compared with carbon electrodes in the same phantom. Magnitudes and distribution of magnetic flux density and current density reconstructions were comparable for carbon and DBS electrodes. Conclusion: Carbon electrodes may offer a safer, MR-compatible method for administering neuromodulation currents. Use of carbon-fiber electrodes should allow imaging of structures close to electrodes, potentially allowing better targeting, electrode position revision, and the facilitation of functional imaging near electrodes during neuromodulation.
