Abstract:Introduction and Aim: In India approximately two million persons are affected by urolithiasis per year and some areas of India have been named as a stone belt. This article reviews information regarding associated risk factors of urolithiasis from Indian perspectives. Methods: The studies were retrieved from eight electronic databases. All the cases of an adult above the age of 18 with urolithiasis were included in this review. The studies conducted in different states of India on human subjects and published between 2010 -2017, which are available in electronic media were included. The quality of studies was assessed by the help of Joanna Briggs Institute Checklist for Systematic Reviews and Research Syntheses. A narrative approach was adopted to analyze the data. Results: The risk factors identified were, less frequency of urination /day, consumption of red meat once a month or more, higher consumption of calcium, sodium, magnesium and phosphorus, high intake of coffee, tea, and sugar, working in high temperatures, lack of physical activity, obesity, and ground water consumption. Habits like smoking and alcohol consumption. Some of the disease conditions like urinary tract infection, renal cyst, horseshoe kidney, atrophic kidney, benign prostatic hyperplasia. Conclusion: The urolithiasis among adults is a public health problem and calls for appropriate action against this.

Epidemiological Trends of Urolithiasis at the Global, Regional, and National Levels: A Population-Based Study

Abstract:Background. Urolithiasis is common worldwide and can predispose to urinary tract infections and renal failure. We aimed to explore the global, regional, and national burden of urolithiasis between 1990 and 2019, stratified by sex, age, and sociodemographic index (SDI). Methods. From 1990 to 2019, data on the number of incident cases of urolithiasis, associated deaths, and disability-adjusted life years (DALYs) were extracted from the 2019 Global Burden of Disease (GBD) study. The trends for the incidence rate, mortality, and DALYs were evaluated using estimated annual percentage changes (EAPCs). Results. The incidence of urolithiasis increased by 48.57%, from 77.78 million incident cases in 1990 to 115.55 million in 2019, while its age-standardized incidence rate (ASIR) decreased. The ASIR increased slightly in the low SDI regions (EAPC = 0.33; 95% confidence interval [CI]: 0.24-0.43), while ASIRs in other SDI regions decreased. The incidence of urolithiasis by age presented a unimodal distribution, with the peak observed in patients aged between 50 years and 70 years. Urolithiasis-related mortality and DALYs also increased over time. Yet, the age-standardized death rate (ASDR) decreased by 2.05% (95% CI, -2.25% to -1.85%) per year, and the annual age-standardized DALY rate decreased by 1.77% (95% CI, -1.92% to -1.63%). The mortality and DALYs increased with age. The incidence, mortality, and DALYs were greater in males than those in females. The burden of urolithiasis showed obvious differences in its regional distribution over the past three decades. Conclusion. From 1990 to 2019, ASIR, ASDR, and age-standardized DALY rate of urolithiasis have decreased. Yet, particularly significant differences exist in the geographic, age, and sex distribution. Thus, medical resources should be rationally allocated and adjusted according to the geographic and demographic distribution of urolithiasis.
Chemical analysis of uroliths: A two-center study of doctors' practice and perspective in South-East, Nigeria

Abstract:Introduction: Urolithiasis is a global disease condition secondary to a variety of factors, and sometimes associated with serious complications. Determination of stone composition is important in delineating causative factors. Knowledge of causative and precipitating factors aids patient management and prevention of recurrence. Objective: The authors' aim was to evaluate the practice and perspective of doctors regarding chemical analysis of stones in management of urolithiasis. Materials and methods: This comparative cross-sectional study was done between December 2016 and May 2017 in two teaching hospitals in South-East, Nigeria. Data were collected using self-administered questionnaires. Results: In all, 88 doctors with mean (standard deviation) age of 37.3 (9.5) years participated in the study. Urinary bladder stones were the most frequently managed, 51 (58.0%); 45 (51.1%) participants do not routinely send stones for chemical analysis. All respondents (100%) agreed that stone analysis is beneficial to patient management. Conclusion: This study showed that in spite of all respondents affirming that chemical analysis of uroliths is beneficial to patient management, more than half of respondents do not routinely send stones for analysis.

Abstract:Background The diverse manifestations of urolithiasis provide very interesting epidemiological data. This has prompted various studies to look into the etiopathogenesis of renal stones, which is believed to be multifactorial, both exogenous and endogenous. VDR Fok1 is a risk factor for renal stone formation and could cause the formation of renal stones through the mechanism of crystal induction and crystallization in the urine. While a few recent studies have shown the role of heavy metals like cadmium and lead in the formation of renal stones, the current knowledge is still insufficient. Methods This case-control prospective study was conducted in Guru Teg Bahadur (GTB) Hospital, a tertiary care facility in Delhi with 30 cases and 30 controls. Patients visiting the department of surgery between November 2011 and April 2013 were enrolled in the study. Cases were defined as patients with renal stones diagnosed on the basis of history and radiological investigations. Controls were selected from the patients admitted to the department of surgery for reasons other than renal stones. The study protocol was approved by the Institutional Ethical Committee of the University College of Medical Sciences, GTB Hospital, Delhi. Written informed consent was obtained from all patients. A structured questionnaire was used to collect data. Metal levels were analyzed by an atomic absorption spectrophotometer (Shimadzu Flame AA-680, Shimadzu Corp., Kyoto, Japan) at Delhi University. The vitamin D receptor gene was measured using genomic DNA. Horizontal agarose gel electrophoresis was used for the quantification of the genomic DNA. Results There were 30 cases and 30 controls in the study. Stress was more prevalent among cases (63%) compared to controls (36%). Nearly 83% of cases had the ff allele of the Vitamin D receptor gene compared to 46% of controls. The median arsenic and lead levels were higher among cases compared to controls. In the unadjusted model of logistic regression, we found stressed patients had three times higher odds of developing renal stones compared to non-stressed patients (OR (95% CI): 2.98 (1.04-8.52); p=0.04). Similarly, patients with higher blood concentrations of arsenic and lead had higher odds of developing renal stones compared to those with lower concentrations. Conclusions There was a definitive role of heavy metals, including lead, cadmium, and arsenic, seen with renal stones. A significant association was seen between the ff allele of VDR polymorphism (Fok1 enzymes) and patients with renal stones. Other parameters, including male and stress factors, seem to have an important role in renal stone formation.
Prevalence of urolithiasis in China: A systematic review and meta‐analysis

Abstract:Background China has a large population and complex geographical environment, which is lead to a large number of urolithiasis patients. However, there is a dearth of comprehensive investigations of urolithiasis prevalence. Consequently, This study aimed to estimate the pooled prevalence , as well as the spatial and temporal distribution, urolithiasis among subjects in China. Methods and Materials We conducted a comprehensive search of both Chinese and English databases to retrieve literature pertaining to the prevalence of urolithiasis in the indigenous Chinese population. A random‐effects meta‐analysis model was employed to calculate the pooled prevalence of urolithiasis. Subgroup analysis were conducted based on factors such as time, region, gender and sample size. Prevalence and spatial distribution maps were created based on provinces and latitude/longitude coordinates. Results 46 studies conducted in 22 provinces across China were included in this meta‐analysis and the pooled prevalence of urolithiasis, kidney stones, ureteral calculi, urethral and bladder stones were 8.1% (95% CI: 5.6–11.1), 7.8% (95% CI: 5.8–10.0),3.2% (95% CI 0.6‐5.7),0.5% (95% CI: 0.1–0.9). Most of the urolithiasis prevalence screening in China was concentrated between 100°E and 120°E, with higher rates observed in low latitude areas. Subgroup analysis of kidney stones revealed that Guangdong (12.7%), Guangxi (10.3%) had the highest prevalence , with the eastern developed area exhibiting higher rates compared to the west. The prevalence of males was higher than females (OR = 1.67, 95% CI: 1.46‐1.92), although the gender gap has significantly reduced since 2006. Moreover, a greater sample size is associated with a decreased prevalence of urolithiasis. Conclusions The prevalence of urolithiasis is increasing in China, and there are noteworthy regional or provincial disparities in their occurrence. It is worth noting that the current number of screening studies in some areas is insufficient. Additional investigations with appropriate sample sizes should be supplemented in time.

Abstract:an observation; diagnoses recorded on death certificates were never part of our original hypothesis and cannot be used in the inferential process. We do not agree with Gross et al. that it is scientifically valid to include a long-term outcome (death with a cancer-related diagnosis) in a predictive model based on the perioperative physical status of the patient. Dr. Roth’s suggestion that blood transfusion and extracorporeal circulation may have synergistic adverse effects is not unreasonable. Certainly, this is one explanation for the observation that cardiac surgery is the only clinical setting where adverse transfusion-related immunomodulation effects have been conclusively demonstrated. Our study data does not help to clarify this suggestion, however, as most of our transfused patients were exposed to extracorporeal circulation. None of this is any reason to promote the use of blood. The short-term risks are compelling, as are the huge costs and the ever diminishing resource. However, we should not be misinforming patients who have survived more than 2 months after coronary artery grafts that they have a serious risk of premature death as a result of a moderate transfusion of blood products.

Abstract:Urolithiasis has become more prevalent throughout China.Knowing stone composition helps identify their possible underlying etiology of urolithiasis, provide appropriate individualized treatment and prevent stone recurrence.To explore characteristics of urinary stone composition in China, and determine the effects of gender, age, body mass index (BMI), stone localization and geographical region on stone composition.We used Fourier transform infrared spectroscopy to analyse urinary stones from consecutive patients at 46 hospitals in seven geographic areas of China between June 1, 2010 and May 31, 2015.Chi-squared tests and logistic regression analyses were performed to determine associations between stone composition and sex, age, body mass index (BMI), stone location, and geographic region.The most common stone components were calcium oxalate (CaOx; 65.9%), carbapatite (15.6%), urate (12.4%), struvite (2.7%), brushite (1.7%), and cystine (0.9%). CaOx and urate occurred more frequently in males, whereas carbapatite and struvite were more common in females (p<0.01). CaOx and carbapatite stones were more common in those 30-50 and 20-40 years old, respectively than in other groups. Brushite and struvite stones were most common among those younger than 20 and older than 70 years old. Urate stone incidence increased with age. Cystine stone incidence decreased with age. Obese patients were more likely to have urate stones than carbapatite or brushite stones (p<0.01). CaOx, carbapatite, brushite, and cystine stones were more frequently observed in the kidney than other types, whereas urate and struvite stones were more frequent in the bladder (p<0.01). Stone composition varied by geographic region.Stone composition differed significantly in patients grouped by gender, age, BMI, stone localization, and geographic region.

Abstract:Objective To obtain the prevalence and distribution of urolithiasis among adult population in China. Methods From May 2013 to July 2014, a multi-stage random cluster sampling was used to investigate the prevalence of urolithiasis and distribution among adult population in 7 provinces/Municipalities in China. In total, 11 urban communities and 19 rural villages were selected. All residents 18 years of age or older in the selected clusters were underwent urinary ultrasound graph, interview of questionnaire, urine analysis and blood examinations. Urolithiasis diagnostic criteria: spots ≥4 mm in diameter was found in urinary system screen. Results A total of 12 570 sampling subjects were selected to participant the study. 9 310 participants completed the urinary ultrasound graph, with an average of (51.34±14.26)years old. The total response rate was 74.07%. There were 3 792(40.73%) male and 5 518(59.27%) female. 605 participants were found with urolithiasis. The overall prevalence was 6.50%. The adjusted prevalence was 6.06% (man, 6.85%; woman, 5.25%). The adjusted prevalence in Guangdong, Chongqing, Heilongjiang, Hunan, Shanghai, Gansu and Shanxi was 11.63%, 11.29%, 8.13%, 5.98%, 4.78%, 1.86% and 0.14% respectively. The prevalence of urolithiasis was significant higher in male(7.65% vs. 5.71%, P<0.01), rural areas (7.96% vs. 4.92%, P<0.01) , and southern China(8.85% vs. 2.85%, P<0. 01). The proportion of upper urinary stone, single stone and stones less than 10 mm in diameter was 99.5%(602/605), 61%(369/605) and 87.11% (527/605) among these 605 patients. Conclusions Urolithiasis is prevalent in adult population of China. The prevalence was significant higher in male, rural areas, and southern China. Exploring the incidence and characteristics of urolithiasis in the central region of Saudi Arabia: Insights from a prominent medical center

Abstract:Introduction: Urolithiasis is a common and recurrent condition with a rising global incidence. Stones typically develop in the upper urinary tract, primarily the kidneys. Various factors such as age, gender, diet, fluid intake, climate, occupation, genetics, and metabolic diseases influence stone formation. Stones can vary in size and location, causing obstruction, urine stasis, and complications such as infection. The prevalence of urolithiasis in Saudi Arabia has significantly increased in recent decades, and the study aims to determine the current prevalence and composition trends of urolithiasis, guide treatment and prevention strategies, as well as understand predictors of occurrence and recurrence. Materials and methods: It is a retrospective cohort study where the data was collected in the time frame of 2015-2021. The study was conducted in the Department of Surgery and the Division of Urology at King Abdulaziz Medical City in Riyadh, Kingdom of Saudi Arabia. Results: The study reveals significant trends in the sociodemographic profile and clinical aspects of urolithiasis patients. With a higher incidence among males (68.5%). Stone compositions predominantly consist of calcium oxalate (67.8%) and uric acid (19.7%), while site distribution shows the left kidney as the most common location (36.5%). Notably, hypertensive patients exhibit a significant association with stone site (P = 0.014). Encouragingly, the majority of patients do not experience reoccurrence (91.6%), and the study demonstrates an increasing recurrence rate with subsequent visits. The relatively shorter hospital stays (55.9% with 1-day stays) indicate efficient management, and this knowledge can aid in optimizing patient care. Conclusion: This study sheds light on the multifaceted nature of urolithiasis by examining various facets. Low recurrence rate of kidney stones offers positive prospects for effective initial management. The shorter hospital stays, suggest advancements in medical practices, enhancing patient convenience and healthcare resource optimization. Investigating the underlying causes behind the observed stone compositions yield insights into potential preventive strategies. Furthermore, extended studies examining the impact of lifestyle modifications and medical interventions on stone recurrence could contribute to refined treatment protocols. These findings can guide healthcare professionals in optimizing patient care, preventive strategies, and future research endeavors.

Abstract:Clinical and pathological data from 219 cases of large bowel cancer were analysed using the Cox regression model to evaluate their effects on survival. Clinical variables such as sex, change in bowel habit, bleeding per rectum, tumour site, size and duration of symptoms had no significant influence on survival. Individually, age, abdominal distension, Dukes' stage and tumour grade had significant influence on survival. When considered together, Dukes' stage was the only variable selected. Therefore Dukes' stage is the most important prognostic factor in colorectal cancer. The distribution of patients was 25, 18, 26 and 31 percent for Dukes' A, B, C and D respectively. The probabilities of survival at five years were 71, 55, 36 and 16 percent for the respective Duke's stages. These data show that the clinical and pathological variables of patients with colorectal cancer in Singapore and the factors significantly associated with survival are similar to those in western countries.

Abstract:The effectors of cell death in allograft rejection are poorly understood. Oxygen derived free radicals (ODFR) may participate in graft destruction. We examined the impact of the antioxidants ascorbic acid (AA) and α-tocopherol (AT) with low dose CsA on rat cardiac allograft survival. Lewis rats that had undergone het-erotopic abdominal cardiac transplantation with Wis-tar-Furth allografts (day 0) were divided into 6 groups. Group 1 was the control group; groups 2 and 3 received AA (1200 mg/kg), and groups 4 and 5 received AT (800 IU/kg) by gavage daily until rejection. Groups 3, 5, and 6 were given CsA (2.5 mg/kg i.m.) days 1–15. Allograft rejection times (in days) were 7.7+1, 10.3+1.5 (P<0.01 vs. group 1), 37.1±6.4 (P<0.01 vs. group 1, P=0.0004 vs. group 6), 9.0±1.4, 26.5±3.6 (P<0.01 vs. group 1, P<0.03 vs. group 6), and 20±4.9 (P<0.01 vs. group 1) for groups 1, 2, 3, 4, 5, and 6. To assess the impact of AA on ODFR production, chemiluminescence was performed on zymosan-activated Lewis whole blood from control rats and rats administered AA. AA significantly decreased peak chemiluminescence (JP<0.05) as compared with nontreated rats indicating effective ODFR scavenging. To determine whether AA and AT inhibit lymphocyte stimulation, mixed lymphocyte response testing was performed with irradiated Wistar-Furth lymphocytes as stimulator cells for Lew responder cells from rats treated as groups 3, 5, and 6. CsA significantly suppressed (P<.05) proliferation as compared with untreated controls. Neither AA nor AT enhance CsA's immunosuppressive effect by mixed lymphocyte response testing. In summary, prolongation of allograft survival with antioxidants AA and AT does not result from abrogation of lymphocyte responsiveness or alteration in CsA bioavailability. Rather, these data suggest that ODFR are involved in allograft destruction and support a role for effective antioxidant therapy in the treatment of allograft rejection.
Analysis on Chemical Composition of Urinary Calculi and Evaluation on Metabolic Disturbance in 1080 Uyghur, Kazakh and Han Nationality Patients of Xinjiang

Abstract:Urinary stones pose a significant health concern globally. The incidence rates vary and are influenced by diverse factors. Despite advancements in the treatment modalities, increasing rate of urinary stones necessitates the development of stone prevention strategies and understanding of metabolic risk factors. Objective: To determine the relationship between chemical composition of urinary calculi and evaluation on metabolic disturbance in 1080 Uyghur, Kazakh and Han patients of Xinjiang. Methods: The stone composition of 1080 patients and analysis of 24-h urine and serum biochemistry results of 775 cases was evaluated by infrared spectroscopic analysis and automatic biochemical machine respectively. Results: Among 644 cases of urinary stones, calcium oxalate stones (409 cases) and mixed type stones (436 cases) were predominant. Metabolic disturbances were found in 643 cases (82.97%) out of 775 patients, showing various urinary abnormalities like hyperuricosuria, hyperoxaluria, hyper-phosphaturia, hyper-cystinuria, hypercalciuria, hypomagnesuria, hypocitraturia, and reduced urine volume (<2000ml in 407 cases). Conclusions: Urinary calculi have intimate connection with metabolic evaluation. Abstract:Purpose – The purpose of this paper is to present an intelligent mobile based emergency response system (MERS) framework, a text information extraction and aggregation algorithm to integrate information from multiple sources in the MERS system, and an ontology‐supported case‐based reasoning system for the MERS system.Design/methodology/approach – The paper explains the components of information extraction and aggregation process, and a CBR‐Ontology approach for the MERS system.Findings – The result of this study will offer a new opportunity to the interaction between government, citizens, responders, and other non‐government agencies in emergency situations, and therefore improve the services of the government in an emergency situation.Originality/value – The paper indicates the need for usage of mobile technologies to assist the government to get information and make decisions in responding to disasters anytime and anywhere.
