Abstract:Urinary stones pose a significant health concern globally. The incidence rates vary and are influenced by diverse factors. Despite advancements in the treatment modalities, increasing rate of urinary stones necessitates the development of stone prevention strategies and understanding of metabolic risk factors. Objective: To determine the relationship between chemical composition of urinary calculi and evaluation on metabolic disturbance in 1080 Uyghur, Kazakh and Han patients of Xinjiang. Methods: The stone composition of 1080 patients and analysis of 24-h urine and serum biochemistry results of 775 cases was evaluated by infrared spectroscopic analysis and automatic biochemical machine respectively. Results: Among 644 cases of urinary stones, calcium oxalate stones (409 cases) and mixed type stones (436 cases) were predominant. Metabolic disturbances were found in 643 cases (82.97%) out of 775 patients, showing various urinary abnormalities like hyperuricosuria, hyperoxaluria, hyper-phosphaturia, hyper-cystinuria, hypercalciuria, hypomagnesuria, hypocitraturia, and reduced urine volume (<2000ml in 407 cases). Conclusions: Urinary calculi have intimate connection with metabolic evaluation. Metabolic evaluation and composition of urinary calculi have great significance in treatment and prevention of urinary calculi.

Abstract:Purpose: Determine stones composition of the upper urinary tract in the eastern region of Algeria. Methods: Our study focuses on a set of 359 stones of the upper urinary tract collected between January 2007 and December 2012 at hospitals in the eastern region of Algeria and analyzed by Fourier transform infrared spectroscopy. Results: The male/female ratio was only 1.32. Calcium oxalate prevailed in 68.5% of stones and 49.3% of nuclei, mainly as whewellite (51.8% of stones and 37.9% of nuclei vs 16.7% and 11.4% respectively for weddellite). Carbapatite prevailed in 15% of stones and 29.8% of nuclei. The struvite, identified in 11.1% of calculi, prevailed in 3.9% of stones and 3.1% of nuclei. Among purines, uric acid prevailed with frequencies quite close to 8.9% and 7% respectively in the stone and in the nucleus while the ammonium urate prevailed in only 0.3% of stones and 3.3% of nuclei. The cystine frequency was 3.6% in both stone and nucleus. The frequency of stone with umbilication was 26.2%. Whewellite was the main component of umbilicated stones with Randall's plaque. Conclusion: Our results suggest that stones of the urinary tract in the Algerian east region resemble those observed in industrialized countries. Some features such as stones location, the whewellite prevalence, the frequencies of main components in both the stone and the nucleus as well as the formation of stones on renal papilla confirm this trend. Abstract:Urolithiasis has become more prevalent throughout China.Knowing stone composition helps identify their possible underlying etiology of urolithiasis, provide appropriate individualized treatment and prevent stone recurrence.To explore characteristics of urinary stone composition in China, and determine the effects of gender, age, body mass index (BMI), stone localization and geographical region on stone composition.We used Fourier transform infrared spectroscopy to analyse urinary stones from consecutive patients at 46 hospitals in seven geographic areas of China between June 1, 2010 and May 31, 2015.Chi-squared tests and logistic regression analyses were performed to determine associations between stone composition and sex, age, body mass index (BMI), stone location, and geographic region.The most common stone components were calcium oxalate (CaOx; 65.9%), carbapatite (15.6%), urate (12.4%), struvite (2.7%), brushite (1.7%), and cystine (0.9%). CaOx and urate occurred more frequently in males, whereas carbapatite and struvite were more common in females (p<0.01). CaOx and carbapatite stones were more common in those 30-50 and 20-40 years old, respectively than in other groups. Brushite and struvite stones were most common among those younger than 20 and older than 70 years old. Urate stone incidence increased with age. Cystine stone incidence decreased with age. Obese patients were more likely to have urate stones than carbapatite or brushite stones (p<0.01). CaOx, carbapatite, brushite, and cystine stones were more frequently observed in the kidney than other types, whereas urate and struvite stones were more frequent in the bladder (p<0.01). Stone composition varied by geographic region.Stone composition differed significantly in patients grouped by gender, age, BMI, stone localization, and geographic region.
[Urolithiasis in children in West Algeria]

Abstract:We analyzed a series of 61 stones from children aged 3 to 14 years old using Fourier transform infrared spectroscopy. The calculi were collected from urology departments of the University Hospitals of Oran, Sidi-Bel-Abbès and Mostaganem in West Algeria. This series is the first investigation concerning the composition of stones in children based on infrared analysis. Calculi were more frequent in males (75.4%) and mainly localized in the bladder (55.8%). Upper urinary tract calculi were more frequent in children over the age of 10 years, and the sex ratio was about 1. Calcium oxalate monohydrate was present in 70.5% of stones and was the main component in 50.8% of cases both in whole stones and nuclei. In contrast, calcium oxalate dihydrate was the main component in only 9.8% of calculi although it was present in 75.4% of stones. Ammonium urate was detected in 29.5% of stones and was always the main component of nuclei. Uric acid, observed in 31.1% of calculi, was the major constituent in 14.7% of stones and 19.7% of nuclei. Magnesium ammonium phosphate was observed in 24.6% of stones as a consequence of urinary tract infection by urea-splitting bacteria. Our observations emphasized that the anatomical location of stone and their composition were in accordance with those previously reported in other countries.
Risk Factors of Kidney Stones in Khyber Pakhtunkhwa, Pakistan: A Descriptive Cross-Sectional Study

Abstract:Introduction Renal calculi, commonly known as kidney stones, affect approximately 12% of the global population, often resulting in mild to severe pain and complications such as infection and renal failure. The causes are multifactorial, involving lifestyle factors, genetic predisposition, and various medical conditions. Despite advancements in treatment, the incidence of kidney stones is on the rise, especially in regions like Asia's stone belt, which includes Pakistan as well. This study investigates the risk factors associated with kidney stones in patients at the Hayatabad Medical Complex, Peshawar, Pakistan, with the goal of informing healthcare policymakers and educating patients to reduce the prevalence and economic burden of the disease in the region. Methods Over a four-month period, 173 patients admitted to the department with confirmed urolithiasis were recruited using a consensus sampling technique. Inclusion criteria were adults aged 18 or older residing in Khyber Pakhtunkhwa, Pakistan. Exclusion criteria included pregnancy, incomplete medical records, secondary causes of renal stones, and unwillingness to participate. Data on socio-demographic factors, clinical history, and lifestyle habits were collected through structured interviews. Descriptive analysis was performed using SPSS software version 23 (IBM Inc, Armonk, New York). Results The study findings indicate that kidney stones were notably prevalent among patients aged 31 to 45 years, with 85 cases identified, comprising 49.1% of the study participants. Furthermore, a significant proportion of affected individuals were males, accounting for 94 cases (54.3%). Those with primary schooling or lower education comprised 106 cases (61.3%), and 124 individuals (71.7%) had lower socioeconomic status. Overweight individuals constituted 81 cases (45.8%) of the total. The study also found a considerable prevalence among residents of rural areas, with 128 cases identified (74.0%). Additionally, 104 cases (60.1%) had a personal history of renal stones. Insufficient water intake was prevalent among 122 individuals (70.5%), while daily consumers of more than one cup of tea also exhibited a higher prevalence, with 97 cases identified (56.1%). Lastly, individuals with a high sodium diet were notably affected, with 112 cases (64.7%) recorded. Conclusion In summary, individuals aged 31-45, males, those residing in rural areas, with lower educational and socioeconomic status, insufficient water intake, a diet high in sodium, personal history of kidney stones, and overweight are at higher risk of urolithiasis. Public education on preventive measures is essential to decrease the incidence of kidney stones and enhance quality of life.

Abstract:We introduce a new type of scanning protocols, called segmented protocols, which enable extracting multi-range flow velocity information from a single Spectral OCT data set. The protocols are evaluated using a well defined flow in a glass capillary. As an example of in vivo studies, we demonstrate two- and three-dimensional imaging of the retinal vascular system in the eyes of healthy volunteers. The flow velocity detection is performed using a method of Joint Spectral and Time domain OCT. Velocity ranging is demonstrated in imaging of retinal vasculature in the macular region and in the optic disk area characterized by different flow velocity values. Additionally, an enhanced visualization of retinal capillary network is presented in the close proximity to macula.

Abstract:The human aspartyl beta-hydroxylase (HAAH) is a highly conserved enzyme that hydroxylates epidermal growth factor-like domains in transformation-associated proteins. Previous studies showed that the gene of HAAH was overexpressed in many human malignancies. In the present study, the HAAH-specific single-chain variable fragment (ScFv) antibody was produced in recombinant Escherichia coli. The variable regions of the genes of the heavy chain (VH) and light chain (VL) cloned from the hybridoma cells G3/F11 were connected with a flexible linker using an overlap extension polymerase chain reaction. Nucleotide sequence analysis revealed that the anti-HAAH VH was a member of the VH V gene family and the VL gene belonged to the Vκ gene family VI subgroup. Extensive efforts to express the functional ScFv antibody in E. coli have been made by using two different prokaryotic expression vectors-pHEN1 and pET-16b-to compare the expression level and solubility of the antibody. The recombinant pHEN1/E1-anti-HAAH vector could express soluble ScFv, although the yield was only 7.8% of the total cellular protein. However, the pET-16b/E2-anti-HAAH vector produced the ScFv as inclusion bodies inside the host cytoplasm, although the expression level of the antibody was quite high (28.5% of the total cellular protein). Soluble ScFv antibody produced by pHEN1/E1-anti-HAAH was characterized for its antigen-binding characteristics. Its antigen affinity as antibody was measured by indirect enzyme linked immunosorbent assay analysis and proved to have high binding activity to the antigen HAAH.

Abstract:Purpose – The purpose of this paper is to present an intelligent mobile based emergency response system (MERS) framework, a text information extraction and aggregation algorithm to integrate information from multiple sources in the MERS system, and an ontology‐supported case‐based reasoning system for the MERS system.Design/methodology/approach – The paper explains the components of information extraction and aggregation process, and a CBR‐Ontology approach for the MERS system.Findings – The result of this study will offer a new opportunity to the interaction between government, citizens, responders, and other non‐government agencies in emergency situations, and therefore improve the services of the government in an emergency situation.Originality/value – The paper indicates the need for usage of mobile technologies to assist the government to get information and make decisions in responding to disasters anytime and anywhere.

Abstract:Introduction: Urolithiasis is a common and recurrent condition with a rising global incidence. Stones typically develop in the upper urinary tract, primarily the kidneys. Various factors such as age, gender, diet, fluid intake, climate, occupation, genetics, and metabolic diseases influence stone formation. Stones can vary in size and location, causing obstruction, urine stasis, and complications such as infection. The prevalence of urolithiasis in Saudi Arabia has significantly increased in recent decades, and the study aims to determine the current prevalence and composition trends of urolithiasis, guide treatment and prevention strategies, as well as understand predictors of occurrence and recurrence. Materials and methods: It is a retrospective cohort study where the data was collected in the time frame of 2015-2021. The study was conducted in the Department of Surgery and the Division of Urology at King Abdulaziz Medical City in Riyadh, Kingdom of Saudi Arabia. Results: The study reveals significant trends in the sociodemographic profile and clinical aspects of urolithiasis patients. With a higher incidence among males (68.5%). Stone compositions predominantly consist of calcium oxalate (67.8%) and uric acid (19.7%), while site distribution shows the left kidney as the most common location (36.5%). Notably, hypertensive patients exhibit a significant association with stone site (P = 0.014). Encouragingly, the majority of patients do not experience reoccurrence (91.6%), and the study demonstrates an increasing recurrence rate with subsequent visits. The relatively shorter hospital stays (55.9% with 1-day stays) indicate efficient management, and this knowledge can aid in optimizing patient care. Conclusion: This study sheds light on the multifaceted nature of urolithiasis by examining various facets. Low recurrence rate of kidney stones offers positive prospects for effective initial management. The shorter hospital stays, suggest advancements in medical practices, enhancing patient convenience and healthcare resource optimization. Investigating the underlying causes behind the observed stone compositions yield insights into potential preventive strategies. Furthermore, extended studies examining the impact of lifestyle modifications and medical interventions on stone recurrence could contribute to refined treatment protocols. These findings can guide healthcare professionals in optimizing patient care, preventive strategies, and future research endeavors.
Composition of urinary calculi in infants: a report from an endemic country

Abstract:Pediatric urolithiasis remains endemic in low-resource countries where infants constitute 17-40% of all children with urolithiasis. This study reports socio-economic factors, medical history and chemical composition of urinary stones in 976 infants of up to 2 years of age. Between 1992 and 2016, 976 infants presented to our institute with 1038 stones. Chemical composition of stones was analyzed by Fourier transformation infrared spectroscopy (FTIR). The mean age of infants was 19.5 ± 5.74 months with a M:F ratio of 5.5:1. Half (50%) of the infants were rural dwellers, 90% belonged to low socio-economic class and 70% were malnourished. A history of chronic diarrhoea was reported in 13% and urinary tract infections in 5%. The anatomic location was bladder in 487 (46.9%), kidney in 246 (23.6%), ureter in 142 (13.6%), urethra in 2 (0.2%) and 161 (15.5%) were passed spontaneously. Overall frequency of compounds in stones showed ammonium acid urate (AAU) in 772 (74.37%), calcium oxalate (CaOx) in 410 (39.5%). Uric acid (UA) in 119 (11.46%), calcium phosphate apatite (CaP) in 96 (9.25%), magnesium ammonium phosphate (Struvite) in 45 (4.34%), cystine in 12 (1.16%) and xanthine in 40 (3.85%). Frequency of compounds was similar in genders. Infants of age 0-6 months had higher frequency of UA (28%), CaOx (50%) and low AAU (61%) as compared to 11, 39 and 75%, respectively, in 7-24 months (p < 0.049) (p < 0.002) (p < 0.001). Nucleus of stones showed pure AAU in 63 and 43% on the surface due to addition of CaOx, struvite and CaP. Our study has shown that AAU is a major component of stones in infants where the main risk factors are poverty, malnutrition, diarrheal diseases and dehydration.

Abstract:The materials balance condition is a fundamental adding up condition, which essentially says that i?½what goes in must come outi?½. In this paper we argue that a number of the recently developed methods of incorporating pollution measures into standard productive efficiency models are inconsistent with this fundamental condition. We propose an alternative method that involves the incorporation of materials balance information into the production model in a similar manner to which price information is normally incorporated. This produces a new environmental efficiency measure that can be decomposed into technical and allocative components, in a similar manner to the conventional cost efficiency decomposition. The approach is illustrated with the case of phosphorus emission on Belgian pig-finishing farms, using data envelopment analysis (DEA). Our results indicate that a substantial proportion of nutrient pollution on these farms can be abated in a cost reducing manner.

Abstract:Urolithiasis is a common urinary tract disease with several etiologic factors and is a public health burden worldwide. Studies have revealed that geographic and gender differences characterize its incidence. In addition, urolithiasis is characterized by high morbidity and recurrence rates. According to the stone formation mechanism, stones can be categorized as infectious or metabolic. Identifying stone components can benefit patients by depicting potential metabolic risk factors while helping physicians determine appropriate stone treatment and prevention strategies. The current study aimed to analyze the stone composition characteristics in 3637 cases with upper urinary tract stones from a single center in northern China while clarifying the relationship with gender, age, and time.

Abstract:Objective: There has not been a study that analyzed the composition of urinary stones from patients in the eastern Shandong region of China. Thus, we explored the composition of urinary stones in the eastern Shandong region of China and discuss the clinical significance of the findings. Methods: A total of 3684 urinary calculi from the eastern Shandong region were collected in our study. Compositions of the stones were analyzed using an Automatic Infrared Spectrum Analysis System (type LIIR-20). The results were verified through manual analysis of the spectrograms, which was accompanied by polarizing microscopy and chemical analysis if necessary. Results: Among the 3684 specimens, there were 1767 single-component stones and 1917 mixed-component stones. According to the main components of the stones, the stones were divided into the following types: calcium oxalate monohydrate stones (1779, 48.3%), anhydrous uric acid stones (1105, 30.0%), carbonate apatite stones (590, 16.0%), ammonium magnesium phosphate hexahydrate stones (143, 3.9%), calcium oxalate dihydrate stones (36, 1.0%), and cystine stones (31, 0.8%). Conclusions: There was a relatively high rate of uric acid stones in patients from the eastern Shandong region of China.

Abstract:Determine the epidemiological characteristics of urolithiasis in the South region of Tunisia and the impact of age and sex on stone composition. We conducted a retrospective study including patient records whose urinary lithiasis was analyzed within the biochemistry department of CHU Habib Bourguiba of Sfax (2011-2020). Stone analysis was performed using a stereomicroscope and infrared spectroscopy. A total of 1127 stones were analyzed. The sex ratio was 2,6. Renal Colic pain was the most common symptom (48,3%). The most frequent localization of the stones (84.6%) was the upper urinary tract. Whewellite was the most common component (64.1%). The study of stone component according to age showed a decrease in the frequency of weddellite (p = 0,024) and an increase in the frequency of uric acid stones with age (p < 0,001). Whewellite was more frequent in men (p = 0.022) and, notably in our series, uric acid was significantly more frequent in women (p < 0.001). The epidemiological profile of urolithiasis in south of Tunisia is similar to that observed in industrialized countries.
