Abstract:Objective: This study aimed to investigate the data of adult patients admitted to the only tertiary care center in Somalia with the diagnosis of urolithiasis and to present the first report from this Sub-Saharan African country. Patients and methods: This study was designed as a retrospective single-center study conducted in Somalia Turkiye Training and Research Hospital. Adult patients who received the diagnosis of urolithiasis and who were admitted to the urology department constituted the target population. Reviewed data included demographic parameters, stone features, type of surgical procedure, intraoperative and early postoperative complications, and inpatient mortality. Results: Overall, 3,680 patients were admitted during the study period. Among these, 620 (17%) patients were admitted due to urolithiasis. There was a significant male predominance with a male-to-female ratio of 3.4:1. Urinary bladder was the most common stone location (n=253, 40.8%), followed by the kidney (n=223, 35.9%). The mean stone diameter was 22.41 (5-64); most (39.4%) of the patients had a stone diameter between 20 and 30 mm, while 27.5% had stones with diameters between 10 and 20 mm. Minimally invasive procedures were the primary surgical modality in 52.6% (n=326) of our patients. However, 45.9% (n=285) of the patients underwent open surgery. Conclusions: The rate of adults with urolithiasis is relatively high in Somalia, as in many other African countries, with a significant male predominance. Although open surgery is rarely used for treating adult urolithiasis in industrialized countries, this approach is still commonly used in Somalia, similar to other parts of Africa.
Clinico-pathological prognostic factors of large bowel cancer in Singapore: a multivariate analysis.

Abstract:Clinical and pathological data from 219 cases of large bowel cancer were analysed using the Cox regression model to evaluate their effects on survival. Clinical variables such as sex, change in bowel habit, bleeding per rectum, tumour site, size and duration of symptoms had no significant influence on survival. Individually, age, abdominal distension, Dukes' stage and tumour grade had significant influence on survival. When considered together, Dukes' stage was the only variable selected. Therefore Dukes' stage is the most important prognostic factor in colorectal cancer. The distribution of patients was 25, 18, 26 and 31 percent for Dukes' A, B, C and D respectively. The probabilities of survival at five years were 71, 55, 36 and 16 percent for the respective Duke's stages. These data show that the clinical and pathological variables of patients with colorectal cancer in Singapore and the factors significantly associated with survival are similar to those in western countries.
Exploring the incidence and characteristics of urolithiasis in the central region of Saudi Arabia: Insights from a prominent medical center

Abstract:Introduction: Urolithiasis is a common and recurrent condition with a rising global incidence. Stones typically develop in the upper urinary tract, primarily the kidneys. Various factors such as age, gender, diet, fluid intake, climate, occupation, genetics, and metabolic diseases influence stone formation. Stones can vary in size and location, causing obstruction, urine stasis, and complications such as infection. The prevalence of urolithiasis in Saudi Arabia has significantly increased in recent decades, and the study aims to determine the current prevalence and composition trends of urolithiasis, guide treatment and prevention strategies, as well as understand predictors of occurrence and recurrence. Materials and methods: It is a retrospective cohort study where the data was collected in the time frame of 2015-2021. The study was conducted in the Department of Surgery and the Division of Urology at King Abdulaziz Medical City in Riyadh, Kingdom of Saudi Arabia. Results: The study reveals significant trends in the sociodemographic profile and clinical aspects of urolithiasis patients. With a higher incidence among males (68.5%). Stone compositions predominantly consist of calcium oxalate (67.8%) and uric acid (19.7%), while site distribution shows the left kidney as the most common location (36.5%). Notably, hypertensive patients exhibit a significant association with stone site (P = 0.014). Encouragingly, the majority of patients do not experience reoccurrence (91.6%), and the study demonstrates an increasing recurrence rate with subsequent visits. The relatively shorter hospital stays (55.9% with 1-day stays) indicate efficient management, and this knowledge can aid in optimizing patient care. Conclusion: This study sheds light on the multifaceted nature of urolithiasis by examining various facets. Low recurrence rate of kidney stones offers positive prospects for effective initial management. The shorter hospital stays, suggest advancements in medical practices, enhancing patient convenience and healthcare resource optimization. Investigating the underlying causes behind the observed stone compositions yield insights into potential preventive strategies. Furthermore, extended studies examining the impact of lifestyle modifications and medical interventions on stone recurrence could contribute to refined treatment protocols. These findings can guide healthcare professionals in optimizing patient care, preventive strategies, and future research endeavors.

Epidemiological Trends of Urolithiasis at the Global, Regional, and National Levels: A Population-Based Study

Abstract:Background. Urolithiasis is common worldwide and can predispose to urinary tract infections and renal failure. We aimed to explore the global, regional, and national burden of urolithiasis between 1990 and 2019, stratified by sex, age, and sociodemographic index (SDI). Methods. From 1990 to 2019, data on the number of incident cases of urolithiasis, associated deaths, and disability-adjusted life years (DALYs) were extracted from the 2019 Global Burden of Disease (GBD) study. The trends for the incidence rate, mortality, and DALYs were evaluated using estimated annual percentage changes (EAPCs). Results. The incidence of urolithiasis increased by 48.57%, from 77.78 million incident cases in 1990 to 115.55 million in 2019, while its age-standardized incidence rate (ASIR) decreased. The ASIR increased slightly in the low SDI regions (EAPC = 0.33; 95% confidence interval [CI]: 0.24-0.43), while ASIRs in other SDI regions decreased. The incidence of urolithiasis by age presented a unimodal distribution, with the peak observed in patients aged between 50 years and 70 years. Urolithiasis-related mortality and DALYs also increased over time. Yet, the age-standardized death rate (ASDR) decreased by 2.05% (95% CI, -2.25% to -1.85%) per year, and the annual age-standardized DALY rate decreased by 1.77% (95% CI, -1.92% to -1.63%). The mortality and DALYs increased with age. The incidence, mortality, and DALYs were greater in males than those in females. The burden of urolithiasis showed obvious differences in its regional distribution over the past three decades. Conclusion. From 1990 to 2019, ASIR, ASDR, and age-standardized DALY rate of urolithiasis have decreased. Yet, particularly significant differences exist in the geographic, age, and sex distribution. Thus, medical resources should be rationally allocated and adjusted according to the geographic and demographic distribution of urolithiasis.

Abstract:Background: The urinary tract stone disease is a common problem. Dual energy CT with advanced post processing techniques gives anatomical information as size, site of the stone as well as its chemical composition which alter the clinical management. Objective: This study was aimed at studying the role of dual energy computed tomography in characterization of renal stones. Patients and methods: This study was carried out at the private radiology center throughout the period from January 2020 to January 2021 included 48 patients with renal stones. In this study, renal stones were more common in men (66.7 %) than in women (30.3%). The age of our selected patients ranged from 24 to 55 years with a mean of 42 years. The most affected age group was from 24-44 years (24 cases) represented 50% of the cases. Results: The average age of the studied group was (42.1±7.6), the most common presentation symptoms was loin pain (75.0%) followed by hematuria (18.8 %) then nausea& vomiting (4.2%) and fever (2.0%).Dual energy CT had 88.9% ability to truly detect uric from non-uric acid stones with (97.9%) accuracy, (100.0%) ability to exclude non uric stones (specificity), (100.0%) predictive value positive and (97.5%) predictive value negative with (50.0%) prevalence and (35.0%) likelihood positive ratio. Conclusion: It could be concluded that dual energy CT has been shown to be effective for characterizing chemic composition of the urinary stones, dual energy CT not well established enough to be used in clinical practice, but it will replace helical non contrast CT as the standard imaging modality for urinary calculi.

Abstract:Urolithiasis has become more prevalent throughout China.Knowing stone composition helps identify their possible underlying etiology of urolithiasis, provide appropriate individualized treatment and prevent stone recurrence.To explore characteristics of urinary stone composition in China, and determine the effects of gender, age, body mass index (BMI), stone localization and geographical region on stone composition.We used Fourier transform infrared spectroscopy to analyse urinary stones from consecutive patients at 46 hospitals in seven geographic areas of China between June 1, 2010 and May 31, 2015.Chi-squared tests and logistic regression analyses were performed to determine associations between stone composition and sex, age, body mass index (BMI), stone location, and geographic region.The most common stone components were calcium oxalate (CaOx; 65.9%), carbapatite (15.6%), urate (12.4%), struvite (2.7%), brushite (1.7%), and cystine (0.9%). CaOx and urate occurred more frequently in males, whereas carbapatite and struvite were more common in females (p<0.01). CaOx and carbapatite stones were more common in those 30-50 and 20-40 years old, respectively than in other groups. Brushite and struvite stones were most common among those younger than 20 and older than 70 years old. Urate stone incidence increased with age. Cystine stone incidence decreased with age. Obese patients were more likely to have urate stones than carbapatite or brushite stones (p<0.01). CaOx, carbapatite, brushite, and cystine stones were more frequently observed in the kidney than other types, whereas urate and struvite stones were more frequent in the bladder (p<0.01). Stone composition varied by geographic region.Stone composition differed significantly in patients grouped by gender, age, BMI, stone localization, and geographic region.

Abstract:Kidney stone disease, prevalent since ancient times, affects around 12% of the global population, with a higher incidence in males. Recurrence is common, and the formation of stones is influenced by genetic, metabolic, environmental, and structural factors. Ultrasonography, as the initial diagnostic tool, is noninvasive and radiation-free, making it suitable for initial evaluations. However, for more precise diagnosis, especially in complex cases, CT scans, including Dual Energy CT (DECT), are preferred due to their higher accuracy. This review also delves into the historical evolution of diagnostic methods, from the once-popular Intravenous Urography (IVU) to the advanced DECT, which offers detailed insights into stone composition. While each imaging modality has its advantages and limitations, CT scans remain the gold standard for their comprehensive analysis capability. The choice of imaging technique is influenced by factors such as stone size, patient characteristics, and clinical context. This review underscores the importance of personalized strategies in selecting the most suitable imaging modality for effective diagnosis and management of kidney stone disease.

Abstract:Our aim was to determine the current trend of endourology in the management of upper urinary tract calculi in Africa reference centres. We conducted an online multiple-choice questionnaire survey involving 46 centres from 27 countries using a structured well-designed Google Form (®) questionnaire. The questionnaires were distributed to the head of service through their emails. The questions collected demographic data about the centre, the epidemiology of urolithiasis, diagnostic means and management of upper urolithiasis, especially access to endourology procedures and their practices. Descriptive analyses were performed. The participation rate was 77.9%. Urinary lithiasis was one of the three main pathologies encountered in 42/46 centres. 33 centres had easy access to CT scanners and 34 had operating theatres equipped with endo-urological surgery equipment. Of these 34 centres, 30 perform endourology for the management of upper urinary tract stones. Rigid ureteroscopy is the main technique used by the centres. It is the only endourology technique used for stone management by 12 centres (40%). 7/30 (23.3%) have the option of performing rigid ureteroscopy, flexible ureteroscopy and percutaneous nephrolithotomy. The frequency of procedures varies widely, with 43.3% rarely performing endourological surgery. Seventeen centres have their operating theatre equipped with a fluoroscope and 6/42 centres have extracorporeal lithotripsy. Open surgery is still used in 29/42 centres (69.1%). Laparoscopy is available in 50% of centres, but none reported performing laparoscopic lithotomy. In Africa, urinary lithiasis plays an important role in the activities of referral centres. Modern management techniques are used to varying degrees (not all centres have them) and with very variable frequency. Open surgery is still widely performed as a management. Rigid ureteroscopy is the main endourological technique. It is essential to develop the practice of modern urology in Africa, mainly endourology.

Abstract:Background Urolithiasis represents one of the most common urogenital disorders. Extracorporeal shock-wave lithotripsy (ESWL) has changed the management of urolithiasis. ESWL has many complications, including cardiac dysrhythmia, bacteriuria, bleeding, steinstrasse, renal colic, and urosepsis. Multidetector computed tomography (MDCT) allows reliable detection and quantification of post-ESWL complications. Aims The aim of this work was to evaluate post-ESWL sequel by CT and to rule out complications. Patients and methods In total, 30 patients with renal stones received treatment with ESWL. Patients came after undergoing ESWL procedure and suffering from clinical complaints to the Department of Diagnostic Radiology in Tanta University Hospital examination center. Results All patients included in our study were evaluated by MDCT to detect complications after ESWL. We found that 10 cases had multiple stone fragments along the course of the ureter that was called steinstrasse. Urinary-tract infections in six (20%) cases. Two cases had signs of inflammation with steinstrasse. Six cases had renal hematoma. We found a significant value. No significant value between the incidence of complications and systolic blood pressure and also diastolic blood pressure. We found significant differences between the incidence of complications and high fasting blood sugar, especially urinary-tract infection. MDCT-diagnosed pyelonephritis has high white blood cell count in urine more than 100. Red blood cell significantly increased in urine analysis of cases with renal hematoma and pyelonephritis. Conclusion MDCT is considered the most accurate noninvasive imaging tool not only for diagnosis of urinary-tract stone but also detection of complications after ESWL.

Abstract:an observation; diagnoses recorded on death certificates were never part of our original hypothesis and cannot be used in the inferential process. We do not agree with Gross et al. that it is scientifically valid to include a long-term outcome (death with a cancer-related diagnosis) in a predictive model based on the perioperative physical status of the patient. Dr. Roth’s suggestion that blood transfusion and extracorporeal circulation may have synergistic adverse effects is not unreasonable. Certainly, this is one explanation for the observation that cardiac surgery is the only clinical setting where adverse transfusion-related immunomodulation effects have been conclusively demonstrated. Our study data does not help to clarify this suggestion, however, as most of our transfused patients were exposed to extracorporeal circulation. None of this is any reason to promote the use of blood. The short-term risks are compelling, as are the huge costs and the ever diminishing resource. However, we should not be misinforming patients who have survived more than 2 months after coronary artery grafts that they have a serious risk of premature death as a result of a moderate transfusion of blood products.

Abstract:The materials balance condition is a fundamental adding up condition, which essentially says that i?½what goes in must come outi?½. In this paper we argue that a number of the recently developed methods of incorporating pollution measures into standard productive efficiency models are inconsistent with this fundamental condition. We propose an alternative method that involves the incorporation of materials balance information into the production model in a similar manner to which price information is normally incorporated. This produces a new environmental efficiency measure that can be decomposed into technical and allocative components, in a similar manner to the conventional cost efficiency decomposition. The approach is illustrated with the case of phosphorus emission on Belgian pig-finishing farms, using data envelopment analysis (DEA). Our results indicate that a substantial proportion of nutrient pollution on these farms can be abated in a cost reducing manner.

Abstract:Introduction: Non-contrast computed tomography (CT) is the gold-standard diagnostic test for urolithiasis. Little is published regarding which information needs to be included in the report for it to be most useful to the healthcare team for efficient triage and high-quality patient care. This study aimed to assess the quality and variability of CT scan reporting at a single Canadian tertiary academic medical center. Methods: We completed a retrospective review of 100 consecutive renal colic CT scans. Descriptive statistics were used to report the frequency with which specific elements commonly used by urologists to triage and treat patients were included in radiology reports. Results: Our sample had a mean age of 51.4±13.1 years. Stone size was universally reported for obstructing stones but was less frequently reported for non-obstructing stones (100% vs. 86.8%). A similar trend was observed for the exact stone number (100% vs. 93.4%). Non-obstructing stones were more likely than obstructing stones to be reported in one dimension (77.5% vs. 47%). Obstructing stones were reported in three dimensions 27% of the time. CT reports commonly include the presence or absence of hydronephrosis status (98%) but are less likely to include renal size (32%) and periureteral stranding (16%). Hounsfield units (HU) were reported in 3% of the reports, but skin-to-stone distance (SSD) and radiation dose were never reported. Conclusions: Reports routinely included assessments of stone size, location, and number (although not uniformly). HU, SSD, and radiation dose were rarely reported. This provides insight into opportunities for standardized reporting to optimize knowledge transfer that may result in clinical efficiency and improved quality of patient care.

Abstract:Introduction and Aim: In India approximately two million persons are affected by urolithiasis per year and some areas of India have been named as a stone belt. This article reviews information regarding associated risk factors of urolithiasis from Indian perspectives. Methods: The studies were retrieved from eight electronic databases. All the cases of an adult above the age of 18 with urolithiasis were included in this review. The studies conducted in different states of India on human subjects and published between 2010 -2017, which are available in electronic media were included. The quality of studies was assessed by the help of Joanna Briggs Institute Checklist for Systematic Reviews and Research Syntheses. A narrative approach was adopted to analyze the data. Results: The risk factors identified were, less frequency of urination /day, consumption of red meat once a month or more, higher consumption of calcium, sodium, magnesium and phosphorus, high intake of coffee, tea, and sugar, working in high temperatures, lack of physical activity, obesity, and ground water consumption. Habits like smoking and alcohol consumption. Some of the disease conditions like urinary tract infection, renal cyst, horseshoe kidney, atrophic kidney, benign prostatic hyperplasia. Conclusion: The urolithiasis among adults is a public health problem and calls for appropriate action against this.

Abstract:Purpose – The purpose of this paper is to present an intelligent mobile based emergency response system (MERS) framework, a text information extraction and aggregation algorithm to integrate information from multiple sources in the MERS system, and an ontology‐supported case‐based reasoning system for the MERS system.Design/methodology/approach – The paper explains the components of information extraction and aggregation process, and a CBR‐Ontology approach for the MERS system.Findings – The result of this study will offer a new opportunity to the interaction between government, citizens, responders, and other non‐government agencies in emergency situations, and therefore improve the services of the government in an emergency situation.Originality/value – The paper indicates the need for usage of mobile technologies to assist the government to get information and make decisions in responding to disasters anytime and anywhere.

Abstract:BackgroundPediatric urinary stone disease (USD) is a costly medical problem. This study aims to assess the clinical characteristics and outcomes of common and rare causes of pediatric USD.MethodsA retrospective descriptive cohort study included all children < 13 years of age with confirmed USD admitted to the Children’s University Hospital in Damascus, Syria, from January 2013 to December 2019. The study sample was divided into two groups based on etiologies: common and rare causes groups.ResultsWe evaluated 235 patients; 147 of them were males, and the male-to-female ratio was 1.7:1. The common causes group consisted of 203 patients (mean age 3.52 ± 3.66 years) and mainly included metabolic disorders (45.5%) and anatomical abnormalities (22.3%), while the rare causes group included 32 cases (mean age 4.93 ± 4.08 years), 12 patients with uric acid stones (37.5%), 7 patients with cystinuria (21.9%), and primary hyperoxaluria in 5 patients (15.6%). In addition, 39.6% of study patients were born to consanguineous marriages. Sixty-two patients developed AKI, and eleven patients had chronic kidney disease (CKD). Patients with rare causes were more likely to have AKI, CKD, bilateral stones, and recurrent stones (P-value < 0.05). Stone analysis was performed on 83 patients, and the main stone types were calcium oxalate (34.9%), uric acid (14.4%), and struvite stones (12%). Surgery was the most performed treatment in 101 patients (56.7%).ConclusionPatients with rare causes of pediatric USD are at a higher risk for severe complications and require early diagnosis and management. The high rate of uric acid stones in our society requires further evaluation for possible underlying causes.Graphical abstractA higher resolution version of the Graphical abstract is available as Supplementary information

Abstract:Determine the epidemiological characteristics of urolithiasis in the South region of Tunisia and the impact of age and sex on stone composition. We conducted a retrospective study including patient records whose urinary lithiasis was analyzed within the biochemistry department of CHU Habib Bourguiba of Sfax (2011-2020). Stone analysis was performed using a stereomicroscope and infrared spectroscopy. A total of 1127 stones were analyzed. The sex ratio was 2,6. Renal Colic pain was the most common symptom (48,3%). The most frequent localization of the stones (84.6%) was the upper urinary tract. Whewellite was the most common component (64.1%). The study of stone component according to age showed a decrease in the frequency of weddellite (p = 0,024) and an increase in the frequency of uric acid stones with age (p < 0,001). Whewellite was more frequent in men (p = 0.022) and, notably in our series, uric acid was significantly more frequent in women (p < 0.001). The epidemiological profile of urolithiasis in south of Tunisia is similar to that observed in industrialized countries.
