Abstract:BACKGROUND: The anesthetic side effects of propofol still occur in clinical practice because no reliable monitoring techniques are available. In this regard, continuous monitoring of propofol in breath is a promising method, yet it remains infeasible because there is large variation in the blood/exhaled gas partial pressure ratio (R-BE) in humans. Further evaluations of the influences of breathing-related factors on R-BE would mitigate this variation. METHODS: Correlations were analyzed between breathing-related factors (tidal volume [TV], breath frequency [BF], and minute ventilation [V-M]) and R-BE in 46 patients. Furthermore, a subset of 10 patients underwent pulmonary function tests (PFTs), and the parameters of the PFTs were then compared with the R-BE. We employed a 1-phase exponential decay model to characterize the influence of V-M on R-BE. We also proposed a modified R-BE (R-DCM) that was not affected by the different breathing patterns of the patients. The blood concentration of propofol was predicted from breath monitoring using R-BEM and R-BE. RESULTS: We found a significant negative correlation (R = -0.572; P < .001) between V-M and R-BE (N = 46). No significant correlation was shown between PFTs and R-BE in the subset (N = 10). R-BEM demonstrated a standard Gaussian distribution (mean, 1.000; standard deviation [SD], 0.308). Moreover, the predicted propofol concentrations based on breath monitoring matched well with the measured blood concentrations. The 90% prediction band was limited to within +/- 1 mu g.mL(-1). CONCLUSIONS: The prediction of propofol concentration in blood was more accurate using R-BEM than when using R-BE and could provide reference information for anesthesiologists. Moreover, the present study provided a general approach for assessing the influence of relevant physiological factors and will inform noninvasive and accurate breath assessment of volatile drugs or metabolites in blood.
High-Pressure Photon Ionization Source for TOFMS and Its Application for Online Breath Analysis.

Abstract:Photon ionization mass spectrometry (PI-MS) is a widely used technique for the online detection of trace substances in complex matrices. In this work, a new high-pressure photon ionization (HPPI) ion source based on a vacuum ultraviolet (VUV) Kr lamp was developed for time-of-flight mass spectrometry (TOFMS). The detection sensitivity was improved by elevating the ion source pressure to about 700 Pa. A radio frequency (RF)-only quadrupole was employed as the ion guide system following the HPPI source to achieve high ion transmission efficiency. In-source collision induced dissociation (CID) was conducted for accurate chemical identification by varying the voltage between the ion source and the ion guide. The high humidity of the breath air can promote the detection of some compounds with higher ionization potentials (IPs) that could not be well detected by single photon ionization (SPI) at low pressure. Under 100% relative humidity (37 °C), the limits of detection down to 0.015 ppbv (parts per billion by volume) for aliphatic and aromatic hydrocarbons were obtained. This HPPI-TOFMS system was preliminarily applied for online investigations of the exhaled breath from both healthy nonsmoker and smoker subjects, demonstrating its analytical capacity for complicated gases analysis. Subsequently, several frequently reported VOCs in the breath of healthy volunteers, i.e., acetone, isoprene, 2-butanone, ethanol, acetic acid, and isopropanol, were successfully identified and quantified.
A Non-invasive Monitoring of Propofol Concentration in Blood by a Virtual Surface Acoustic Wave Sensor Array

Abstract:Propofol (2,6-diisopropylphenol) is widely used in total intravenous anesthesia. An unknown drug concentration in blood always leads to some side effects in patients with propofol injection. However, the drug concentration in the blood is hard to be continuously measured since invasive sampling causes a loss of blood at each measurement. Here, we introduced a virtual surface acoustic wave sensor array (VSAWSA) to non-invasively detect the propofol concentration in blood through exhaled gases. Calibration was conducted by a parallel test using gas chromatography and mass spectrometry (GC-MS) with solid-phase micro-extraction (SPME) for preconcentration. The limit of detection of VSAWSA reached 0.15 nmol/L for propofol. Six cases of clinical trials was conducted to compare the exhaled propofol concentrations to the plasma concentrations controlled by target-controlled infusion (TCI). The calibration by GC-MS ensured the feasibility, reliability, and accuracy of the VSAWSA (R = 0.9904, p <0.001). The clinical monitoring data by VSAWSA showed an excellent consistency with TCI.
Simultaneous On-Line Monitoring of Propofol and Sevoflurane in Balanced Anesthesia by Direct Resistive Heating Gas Chromatography.

Abstract:In balanced anesthesia, sevoflurane and propofol are often used in combination to achieve a better anesthetic effect. However, methods for on-line monitoring of concentrations of the two anesthetics in patients are still rare in clinical. This study proposed a non-invasive method utilizing a fast gas chromatograph combined with a surface acoustic wave sensor (Fast GC-SAW) to simultaneously on-line monitor sevoflurane and propofol in patients' exhaled gas. By using the direct resistive heating capillary column, the single detection time of Fast GC-SAW system was significantly shortened to 90s, as well as the size reduced to (40cm x 30cm x 20 cm). Besides, in the calibration of sevoflurane, Fast GC-SAW system showed a good linear correlation (R-2 = 0.9925, P < 0.01) with gas chromatography-mass spectrometer (GC-MS), which ensured the reliability and accuracy of the Fast GC-SAW system. Finally, clinical exper. iments on patients under balanced anesthesia were conducted. Abstract:Rapid and quantitative determination of blood propofol concentration is important for anesthesiologists to accurately control intraoperative propofol dose, timely monitor physiological statuses of patients and greatly improve the safety of surgery. Herein, a dopant-assisted negative photoionization ion mobility spectrometer with the optimized ionization region structure and the three-way inlet design was developed, increasing the generation ratio of the reactant ions O2-, and improving the ionization efficiency of propofol molecules. Besides, the addition of methanol-anisole solution during injection promoted the neutral desorption of propofol in blood, further improving the detection sensitivity by an order of magnitude, eliminating any sample pretreatment and effectively reducing the single analysis time to less than 1 min compared to the previous article. The dual calibration quantitative method, i.e. the method of calibrating the O2- concentration and the sample concentration changes during the entire process of detecting propofol through the integral value of M·O2- and the maximum signal intensity of O2-, successfully achieved accurate quantification of blood propofol. And the linear calibration curve of propofol was obtained with the range of 0.1 to 15 ng μL-1 and with the limit of detection of 0.03 ng μL-1, which was fulfilled to conduct propofol determination throughout the perioperative period. Finally, this method was applied to clinically measure the blood propofol concentration in patients newly regained consciousness with concentrations ranging from 0.2 ng μL-1 to 3 ng μL-1, and it turned out that the older patient had the lower propofol concentration in blood.

Abstract:Propofol accounts for about 70% market share of intravenous anesthetics in clinical surgery. With further research and application of total intravenous anesthesia, precision closed-loop anesthesia is proved to be important in clinical practice. It is necessary to monitor anesthetic dose, assess anesthesia effect, and evaluate anesthesia prognosis in a rapid and comprehensive way during the closed-loop anesthesia. Due to intravenous injection of anaesthetic, the dififculty to precisely monitor anaesthetic dose in blood has become the main limit for development of precision closed-loop anesthesia. In recent ten years, propofol detection using real-time breath monitoring developed rapidly with novel techniques and methods, which might break through the barrier of the closed-loop anesthesia. This study presents systems and methods using synchronized applications of gas chromatograph (GC) and surface acoustic wave (SAW) sensor for on-line monitoring of propofol in exhaled gas. SPME-GC-MS was used to calibrate to GC-SAW sensor system for verifying feasibility, reliability, detection limit, and linearity. The propofol concentrations in exhaled gas was compared with concentrations in blood calculated by target-controlled infusion (TCI). The results showed a high degree of consistency. In conclusion, GC-SAW system was proved to be a reliable way for clinical monitoring of propofol concentration through patients’exhaled gas. This detection system would reduce the side effects of propofol intravenous anesthesia during surgery.

Abstract:Ion mobility spectrometry (IMS) opened a potential avenue for the rapid detection of organophosphorus pesticides (OPPs), though an improved selectivity of stand-alone IMS was still in high demand. In this study, a stand-alone positive photoionization ion mobility spectrometry (PP-IMS) apparatus was constructed for the rapid detection of OPPs with acetone as dopant. The photoionization of acetone molecules was induced by the ultraviolet irradiation to produce the reactant ions (Ac)2H+, which were employed to ionize the OPPs including fenthion, imidan, phosphamidon, dursban, dimethoate and isocarbophos via the proton transfer reaction. Due to the difference in proton affinity, the tested OPPs exhibited the different dopant-dependent manners. Based on this observation, the switching of dopant concentration was implemented to improve the selectivity of PP-IMS for OPPs detection. For instance, a mixture of fenthion, dursban and dimethoate was tested. By switching the concentration of doped acetone from 0.07 to 2.33 to 19.94mgL-1, the ion peaks of fenthion and dursban were inhibited in succession, achieving the selective detection of dimethoate at last. In addition, another mixture of imidan and phosphamidon was initially detected by PP-IMS with a dose of 0.07mgL-1 acetone, indicating that their ion peaks were severely overlapped; when the concentration of doped acetone was switched to 19.94mgL-1, the inhibition of imidan signals promised the accurate identification of phosphamidon in mixture. Finally, the PP-IMS in combination of switching dopant concentration was applied to detect the mixed fenthion, dursban and dimethoate in Chinese cabbage, demonstrating the applicability of proposed method to real samples.

Abstract:BACKGROUNDAlthough pilot studies have reported that exhaled propofol concentrations can reflect intraoperative plasma propofol concentrations in an individual, the blood/exhaled partial pressure ratio RBE varies between patients, and the relevant factors have not yet been clearly addressed. No efficient method has been reported for the quick evaluation of RBE and its association with inter-individual variables.METHODSWe proposed a novel method that uses a surface acoustic wave (SAW) sensor combined with a fast gas chromatograph (GC) to simultaneously detect propofol concentrations in blood and exhaled gas in 28 patients who were receiving propofol i.v. A two-compartment pharmacokinetic (PK) model was established to simulate propofol concentrations in exhaled gas and blood after a bolus injection. Simulated propofol concentrations for exhaled gas and blood were used in a linear regression model to evaluate RBE.RESULTSThe fast GC-SAW system showed reliability and efficiency for simultaneous quantitative determination of propofol in blood (correlation coefficient R(2)=0.994, P<0.01) and exhaled gas (R(2)=0.991, P<0.01). The evaluation of RBE takes <50 min for a patient. The distribution of RBE in 28 patients showed inter-individual differences in RBE (median 1.27; inter-quartile range 1.07-1.59).CONCLUSIONSFast GC-SAW, which analyses samples in seconds, can perform both rapid monitoring of exhaled propofol concentrations and fast analysis of blood propofol concentrations. The proposed method allows early determination of the coefficient RBE in individuals. Abstract:Propofol is a widely used intravenous anesthetic agent in sedation and general anesthesia. To improve the safety and maintain the depth of anesthesia, it is important to develop a rapid, sensitive, and reliable method to monitor the concentration of propofol in blood during anesthesia continuously. Here, we present a novel strategy based on paper spray ionization-mass spectrometry (PSI-MS) to detect propofol. Samples (in 10 μL) were mixed with methanol as protein precipitation solvent and 2,6-dimethylphenol as internal standard. Protein micro-precipitation was achieved with methanol by vortexing and centrifuging for 5 s each, and propofol was extracted to the supernatant. PSI-MS was performed in negative ionization mode, and MS signal lasted for 1 min. The analysis of a single sample was completed within 2 min. The area ratios of propofol to internal standard were calculated for quantification. Limit of detection of 5.5 ng mL−1 and limit of quantification of 18.2 ng mL−1 were achieved for propofol in whole blood. Calibration curve was linear in the range of 0.02–10 μg mL−1. The developed method was used successfully in monitoring the propofol concentration in 3 patients’ whole blood during anesthesia, showing its further application in controlling and feeding-back target concentration infusion.
The Accuracy of Target-Controlled Infusion of Propofol in Chinese

Abstract:Objective To evaluate the accuracy of target-controlled infusion (TCI) of propofol using pharmacokinetic parameters reported by Marsh to predict plasma propofol concentration in Chinese. MethodsTwenty-two ASA I - II patients were divided into two groups: group Y aged 65 yr (n = 11) and group E ages 65 yr ( n = 11). Patients with liver, kidney or cardiovascular diseases were excluded. The patients were premedicated with pethidine 50mg and phenobarhital 0.1 g im. Radial artery and internal jugular vein(IJV) were cannulated. The pharmacokinetic parameters incorporated in the Graseby 3500 pump we used were: V1=228 ml-kg-1 , K10 =0.119 min-1 ,K12=0.112 min-1, K2l=0.055min-1 , K13 =0.0419 min-1 ,K31 =0.0033 min-1. Target concentration was started with 2ug-ml-1 and increased at increment of 1ugml-1 until loss of consciousness. The patient was then intubated. When target concentration of propofol was increased, the concentration of inhalation anesthetic was reduced to maintain hemodynamic stability. When target concentration of propofol was increased, arterial blood sample was taken 1-3 times for determination of plasma propofol concentration measured by HPLC (Agilent 1100) . Then blood samples every 10-15 min. For each sample prediction error(PE) and constancy error(CE) were calculated. For each patient median prediction error(MDPE), median absolute prediction error(MDAPE) . Median absolute constancy error (MDACE) and median constancy error (MDCE) were calculated.ResultsThere was remarkable initial overshot. PE and absolute PE were 63.3 % and 66.2 % in group E and 62.1 % and 62.7% in group Y. CE and absolute CE were -0.3% and 12.7% in group E and 0.6% and 13.5% in group Y. The median value of MDPE ( = the median value of MDAPE) was 78.1 % in group E and 66.1% in group Y. The median value of MDCE was 0.2% (group E) and 0.8% (group Y) and MDACE was 12.5% (group E) and 13.5% (group Y) . The measured concentrations were significantly linearly correlated with the premedicated concentrations. Conclusion TCI system with propofolpharmacokinetic parameters reported by Marsh can lead to initial overshot and underestimate the measured plasma propofol concentration but it can maintain a stable plasma concentration
Exhaled breath is found to be better than blood samples for determining propofol concentrations in the brain tissues of rats

Abstract:The correlation between propofol concentration in exhaled breath (CE) and plasma (CP) has been well-established, but its applicability for estimating the concentration in brain tissues (CB) remains unknown. Given the impracticality of directly sampling human brain tissues, rats are commonly used as a pharmacokinetic model due to their similar drug-metabolizing processes to humans. In this study, we measured CE, CP, and CB in mechanically ventilated rats injected with propofol. Exhaled breath samples from the rats were collected every 20 seconds and analyzed using our team's developed vacuum ultraviolet time-of-flight mass spectrometry (VUV TOF-MS). Additionally, femoral artery blood samples and brain tissue samples at different time points were collected and measured using high-performance liquid chromatography mass spectrometry. The results demonstrated that propofol concentration in exhaled breath exhibited stronger correlations with that in brain tissues compared to plasma levels, suggesting its potential suitability for reflecting anesthetic action sites' concentrations and anesthesia titration. Our study provides valuable animal data supporting future clinical applications.

Continuous Real Time Breath Gas Monitoring in the Clinical Environment by Proton-Transfer-Reaction-Time-of-Flight-Mass Spectrometry

Abstract:Analysis of volatile organic compounds (VOCs) in breath holds great promise for noninvasive diagnostic applications. However, concentrations of VOCs in breath may change quickly, and actual and previous uptakes of exogenous substances, especially in the clinical environment, represent crucial issues. We therefore adapted proton-transfer-reaction-time-of-flight-mass spectrometry for real time breath analysis in the clinical environment. For reasons of medical safety, a 6 m long heated silcosteel transfer line connected to a sterile mouth piece was used for breath sampling from spontaneously breathing volunteers and mechanically ventilated patients. A time resolution of 200 ms was applied. Breath from mechanically ventilated patients was analyzed immediately after cardiac surgery. Breath from 32 members of staff was analyzed in the post anesthetic care unit (PACU). In parallel, room air was measured continuously over 7 days. Detection limits for breath-resolved real time measurements were in the high pptV/low ppbV range. Assignment of signals to alveolar or inspiratory phases was done automatically by a matlab-based algorithm. Quickly and abruptly occurring changes of patients' clinical status could be monitored in terms of breath-to-breath variations of VOC (e.g. isoprene) concentrations. In the PACU, room air concentrations mirrored occupancy. Exhaled concentrations of sevoflurane strongly depended on background concentrations in all participants. In combination with an optimized inlet system, the high time and mass resolution of PTR-ToF-MS provides optimal conditions to trace quick changes of breath VOC profiles and to assess effects from the clinical environment.

Abstract:Ciprofol (HSK 3486, C14H20O), a novel 2,6-disubstituted phenol derivative similar to propofol, is a new type of intravenous general anaesthetic. We found that the exhaled ciprofol concentration could be measured online by ultraviolet time-of-flight mass spectrometry (UV-TOFMS), which could be used to predict the plasma concentration and anaesthetic effects of ciprofol. In this study, we present the calibration method and validation results of UV-TOFMS for the quantification of ciprofol gas. Using a self-developed gas generator to prepare different concentrations of ciprofol calibration gas, we found a linear correlation between the concentration and intensity of ciprofol from 0 parts per trillion by level (pptv) to 485.85 pptv (R2 = 0.9987). The limit of quantification was 48.59 pptv and the limit of detection was 7.83 pptv. The imprecision was 12.44 % at 97.17 pptv and was 8.96 % at 485.85 pptv. The carry-over duration was 120 seconds. In addition, we performed a continuous infusion of ciprofol in beagles, measured the exhaled concentration of ciprofol by UV-TOFMS, determined the plasma concentration by high-performance liquid chromatography, monitored the anaesthetic effects as reflected by the bispectral index value. The results showed that the exhaled and plasma concentrations of ciprofol were linearly correlated. The exhaled ciprofol concentration correlated well with the anaesthetic effect. The study showed that we could use the UV-TOFMS to provide a continuous measurement of gaseous ciprofol concentration at 20-second intervals.

Breath-by-breath measurement of exhaled ammonia by acetone-modifier positive photoionization ion mobility spectrometry via online dilution and purging sampling

Abstract:Exhaled ammonia (NH3) is an essential noninvasive biomarker for disease diagnosis. In this study, an acetone-modifier positive photoionization ion mobility spectrometry (AM-PIMS) method was developed for accurate qualitative and quantitative analysis of exhaled NH3 with high selectivity and sensitivity. Acetone was introduced into the drift tube along with the drift gas as a modifier, and the characteristic NH3 product ion peak of (C3H6O)4NH4+ (K0 = 1.45 cm2/V·s) was obtained through the ion-molecule reaction with acetone reactant ions (C3H6O)2H+ (K0 = 1.87 cm2/V·s), which significantly increased the peak-to-peak resolution and improved the accuracy of exhaled NH3 qualitative identification. Moreover, the interference of high humidity and the memory effect of NH3 molecules were significantly reduced via online dilution and purging sampling, thus realizing breath-by-breath measurement. As a result, a wide quantitative range of 5.87-140.92 μmol/L with a response time of 40 ms was achieved, and the exhaled NH3 profile could be synchronized with the concentration curve of exhaled CO2. Finally, the analytical capacity of AM-PIMS was demonstrated by measuring the exhaled NH3 of healthy subjects, demonstrating its great potential for clinical disease diagnosis.

Abstract:This study investigated the effectiveness of telephone-delivered cognitive-behavioral therapy (T-CBT) in the management of chronic pain with older military veterans enrolled in VA primary-care clinics. We conducted a randomized clinical trial comparing T-CBT with telephone-delivered pain education (T-EDU). A total of 98 military veterans with chronic pain were enrolled in the study and randomized into one of two treatment conditions. Study participants were recruited from primary-care clinics at an urban VA medical center and affiliated VA community-based outpatient clinics (CBOCs). Pain management outcomes were measured at midtreatment (10 weeks), posttreatment (20 weeks), 3-month follow-up (32 weeks), and 6-month follow-up (46 weeks). No significant differences were found between the two treatment groups on any of the outcome measures. Both treatment groups reported small but significant increases in level of physical and mental health, and reductions in pain and depressive symptoms. Improvements in all primary outcome measures were mediated by reductions in catastrophizing. Telephone-delivered CBT and EDU warrant further study as easily accessible interventions for rural-living older individuals with chronic pain.
Breath analysis-performance and potential of ion mobility spectrometry

Abstract:Ion mobility spectrometry is a fast and sensitive analytical method for the detection of gas phase analytes in the ppb(v)-ppt(v) range under ambient conditions (pressure and temperature). Ion mobility spectrometers coupled with rapid pre-separation like multi-capillary columns (MCC/IMS) are suitable for the selective characterization of complex and humid mixtures. Recently, MCC/IMS have been applied to analyses of human breath for early diagnosis as well as medication and therapy control. The complete procedure of breath analyses including evaluation and interpretation of the data obtained is demonstrated for the first time on exhaled breath after the consumption of a particular candy as an example. An MCC/IMS equipped with a β-radiation source ((63)Ni) requires 5 to 10 min for a complete analysis of exhaled breath. Retention time and reduced ion mobility of the detected signals are compared to an analyte database for the identification of the related analytes. These findings were successfully validated by gas-chromatographic mass spectroscopy of the headspace of the candy via solid-phase micro-extraction and of breath samples on Tenax adsorption tubes. Furthermore, signal height of particular analyte signals as a measure for their concentration was used to monitor the concentration development with time. This exemplary investigation demonstrates that MCC/IMS is a powerful and rapid non-invasive tool for human breath analyses. The method can be used for medical applications (diagnosis, therapy control, metabolic profiling) as well as for a general determination of the metabolic state of a subject (medication, nutrition, fasting). The demonstrated procedure is independent of whether the analytes detected in breath are caused by nutrition or medication or whether they are metabolite characteristics for a particular disease. Therefore, it can directly be transferred to any relevant peak pattern.
Nebulization Swab Assisted Photoionization Tandem Miniaturized Ion Trap Mass Spectrometry for On-Site Analysis of Nonvolatile Compounds.

Abstract:Nonvolatile compounds usually have a high molecular weight and exhibit a high boiling point, which poses great challenges to the ionization method of MS. Ambient ionization sources can efficiently analyze the nonvolatile compounds without complex pretreatment, but they generally require special media such as heating devices, laser optical devices, or corona needles. Acoustic nebulization assisted photoionization (ANPI) is a potential method for the analysis of nonvolatile compounds that uses nebulization as a prerequisite for photoionization and introduces many advantages of PI, including excellent ionization efficiency, a high yield of molecular ions, and simplified spectrum interpretation. However, the ANPI source can be limited in on-site applications by the complexity of the analytical devices and the high cost of the nebulization chip. To address this issue, in this paper, we explored cheap and commercially piezoelectric materials used in a mist sprayer and fabricated a nebulization swab assisted photoionization (NSAP) as an ambient ionization source. Some useful results are presented: numerical simulation was introduced successfully for optimizing the aerosol distribution in the NSAP source; nonvolatile muscle relaxants, drugs of abuse, antibiotics, phthalates, and cholesterol were detected mostly as their protonated molecular ions while some special acetone/water cluster ions were detected. In addition, the LOD for most of the target analytes ranged from 10.0 to 50.0 pg with RSD ≤ 9%. Finally, this method is implemented for Chinese baijiu spiked with phthalates. The experimental data shows the capability of a NSAP source in high sensitivity and on-site analysis of the nonvolatile compounds.

Abstract:Sevoflurane and propofol are often used in combination to achieve a better anesthetic effect in balanced anesthesia. However, methods for on-line monitoring drug concentrations of the two anesthetics in patients are still scarce in clinic. This study proposed a non-invasive method utilizing a fast gas chromatograph (Fast GC) combined with a surface acoustic wave (SAW) sensor to simultaneously on-line monitor sevoflurane and propofol in patients' exhaled gas. Using the direct resistive heating capillary column, the analytical cycle time of the Fast GC-SAW system was significantly shortened to 90s, as well as the size was reduced.
