Abstract:Objective To explore the diagnostic value of 3.0T MRI in shoulder impingement syndrome(SIS),and to observe the imaging features and injuring mechanism of SIS.Methods Thirty patients of SIS underwent MRI,and the imaging findings and clinical symptoms were reviewed retrospectively.Results Theanatomical structures and abnormal changes of shoulder were excellently demonstrated with 3.0T MRI.①Direct signs:Supraspinatus tendon illustrated signal and patterns changes in all 30 patients,while supraspinatus tendon full-thickness tears were detected in 6 patients;Partial-thickness tears were found in 13 patients,abnormal and mixed signals on the surface of the muscle tendon were found in 11 patients.②Indirect signs:Subacromial bursa thickening,subacromial-deltoid sac fluid and the joint capsule fluid were signs of supraspinatus tendon tears.③Secondary signs included joint labrum avulsion,the supraspinatus muscle atrophy and the deltoid muscle atrophy and Bankart Lesion.④Etiological signs:Acromino were classified as flat,smoothly curved and hooked.AHI5 mm(7/30)and between 5-10 mm(23/30).Conclusion 3.0T MR can effectively demonstrate imaging features in SIS and provide information to guide the protection and rehabilitation of shoulder function.

Abstract:With current technology a properly conceived imaging strategy can demonstrate instability lesions in the athlete. Plain radiographs can diagnose acute dislocations and assess successful reductions. In addition, plain radiographs can demonstrate Hill-Sachs and, more importantly for instability, osseous Bankart lesions. In the acute setting, conventional MRI nicely demonstrates labral Bankart, ligamentous. and tendonous injuries that result from dislocations and can lead to instability. In the setting of chronic instability, MR arthrography best evaluates these lesions. In the postoperative shoulder, muitislice CT arthrography may be the modality of choice, but further investigation is needed. If large series validate multislice CT arthrography for the evaluation of postoperative instability lesions, this technique may become widely used in athletes and in other populations where recurrent instability is a problem. Other imaging strategies may also find an increasing central role in evaluating shoulder instability lesions. Indirect MR arthrography, for example, may have a role in assessing these lesions in athletes . Another intriguing technology for this application is the development of high field (0.5 Tesla or greater) open magnets. In such a setting, physiological relationships in the shoulder with motion and stress may be evaluated. Such imaging may farther illuminate our understanding of the stable and unstable shoulder. Unfortunately, with all imaging modalities, whether widely used or experimental, outcomes data is Lacking. How do the various imaging modalities and strategies affect patient outcome? The answer is unknown and needs to be answered before a definitive patient work-up for shoulder instability can be established.

Abstract:Abstract Background To evaluate the diagnostic value of MR-derived CT-like images and simulated radiographs compared with conventional radiographs in patients with suspected shoulder pathology. Methods 3 T MRI of the shoulder including a 3D T1-weighted gradient echo sequence was performed in 25 patients (mean age 52.4 ± 18 years, 13 women) with suspected shoulder pathology. Subsequently a cone-beam forward projection algorithm was used to obtain intensity-inverted CT-like images and simulated radiographs. Two radiologists evaluated the simulated images separately and independently using the conventional radiographs as the standard of reference, including measurements of the image quality, acromiohumeral distance, critical shoulder angle, degenerative joint changes and the acromial type. Additionally, the CT-like MR images were evaluated for glenoid defects, subcortical cysts and calcifications. Agreement between the MR-derived simulated radiographs and conventional radiographs was calculated using Cohen’s Kappa. Results Measurements on simulated radiographs and conventional radiographs overall showed a substantial to almost perfect inter- and intra-rater agreement (κ = 0.69–1.00 and κ = 0.65–0.85, respectively). Image quality of the simulated radiographs was rated good to excellent (1.6 ± 0.7 and 1.8 ± 0.6, respectively) by the radiologists. A substantial agreement was found regarding diagnostically relevant features, assessed on Y- and anteroposterior projections (κ = 0.84 and κ = 0.69 for the measurement of the CSA; κ = 0.95 and κ = 0.60 for the measurement of the AHD; κ = 0.77 and κ = 0.77 for grading of the Samilson-Prieto classification; κ = 0.83 and κ = 0.67 for the grading of the Bigliani classification, respectively). Conclusion In this proof-of-concept study, clinically relevant features of the shoulder joint were assessed reliably using MR-derived CT-like images and simulated radiographs with an image quality equivalent to conventional radiographs. MR-derived CT-like images and simulated radiographs may provide useful diagnostic information while reducing the amount of radiation exposure.

Abstract:Abstract Background and Purpose. Shoulder impingement syndrome is a common condition and is often managed with an exercise program. The purpose of this study was to examine an exercise program in patients with shoulder impingement syndrome. Specifically, the purpose was to identify changes that might occur in 3-dimensional scapular kinematics, physical impairments, and functional limitations. Subjects. Fifty-nine patients with impingement syndrome were recruited, and 39 patients successfully completed the 6-week rehabilitation program and follow-up testing. Impingement was defined as having at least 3 of 6 predefined clinical signs or symptoms. Methods. Subjects were assessed before and after a 6-week rehabilitation program and again at 6 months. Pain, satisfaction, and function were measured using the University of Pennsylvania Shoulder Scale. Range of motion, isometric muscle force, and 3-dimensional scapular kinematic data also were collected. Subjects were given a progressive exercise program that included resistive strengthening, stretching, and postural exercises that were done daily at home. Subjects also were given shoulder education related to anatomy, the basic mechanics of impingement, and strategies for reducing load on the shoulder. Each subject attended one physical therapy session per week for a 6-week period, primarily for monitoring and upgrading the exercise program. Pretest and posttest scores were compared using paired t tests and repeated-measures analysis of variance. Results. Passive range of motion increased for both external and internal rotation but not for elevation. Abduction external and internal rotation force all increased. There were no differences in scapular kinematics. Improvements were found for pain, satisfaction, and shoulder function and for Medical Outcomes Study 36-Item Short-Form Health Survey (SF-36) scores related to physical function. At 6-month follow-up, improvements made in pain, satisfaction, and function were maintained. Discussion and Conclusion. The use of this exercise protocol in the management of shoulder impingement syndrome may have a positive impact on patients' impairments and functional limitations. Our findings suggest a relatively simple exercise program combined with patient education may be effective and, therefore, merits study in a larger trial using a control group. Changes in scapular kinematics did not appear to be a primary mechanism underlying improvement in symptoms and function.

Abstract:Objectives : To explore the subacromial motion metrics in patients with and without subacromial impingement syndrome (SIS) and to investigate whether the abnormality was associated with rotator cuff pathologies. Design This cross-sectional observational study used dynamic quantitative ultrasound imaging for shoulder joint assessment. Setting Outpatient rehabilitation clinic. Participants Individuals with SIS on at least one shoulder (n=32) and asymptomatic controls (n=32). Interventions Not applicable. Main Outcome Measures Frame-by-frame, the humeral greater tuberosity against the lateral edge of the acromion was traced to obtain the minimal vertical acromiohumeral distance (AHD). The rotation angle and radius of the humerus were computed using the least-squares curve fitting method. Results Approximately two-thirds of the shoulders with SIS did not have any sonographically identifiable rotator cuff pathologies. There was a consistent trend of non-significantly increased humeral rotation angles in painful shoulders. The generalized estimating equation demonstrated that the decreased minimal vertical AHD was associated with painful subacromial impingement (beta coefficient of -0.123 cm, 95% confidence interval [CI], -0.199 to -0.047). The area under the curve for the minimal vertical AHD to discriminate painful/impinged shoulders ranged from 0.624 to 0.676. The increased rotation angle (beta coefficient of 10.516°; 95% CI, 3.103-17.929) and decreased rotation radius (beta coefficient of -2.903 cm; 95% CI, -5.693 to -0.111) were shown to be significantly related to the presence of supraspinatus tendinopathy. Conclusions Shoulders with SIS were characterized by a decreased minimal vertical AHD during dynamic examination. Abnormal subacromial metrics can develop in patients with mild (or no) rotator cuff pathologies. More prospective cohort studies are warranted to investigate the changes in subacromial motion metrics in populations at risk for painful/impinged shoulders.

Abstract:Orthopaedic Journal of Sports Medicine, Volume 12, Issue 10, October 2024. Background:Acromioclavicular joint (ACJ) disruption occurs frequently in athletes engaged in contact sports. However, the current understanding of ACJ biomechanics during muscle-driven functional activities and the influence of different treatment approaches (eg, reconstruction surgery vs nonoperative methods) on ACJ kinematics and stability remains limited. The absence of precise in vivo biomechanical measurement modalities for scapular and clavicular kinematics contributes significantly to this lack of understanding.Purposes/Hypothesis:The purposes of this study were to determine whether dynamic stereo x-ray (DSX) imaging can be used to evaluate the in vivo kinematics of the ACJ and to provide preliminary comparative data on ACJ kinematics, range of motion, and isometric strength of surgically reconstructed or nonoperatively treated ACJ shoulders and their uninjured contralateral shoulders. It was hypothesized that ACJ kinematics could be measured successfully using DSX and that surgically and nonoperatively treated shoulders would show abnormal 3-dimensional (3-D) ACJ kinematics compared with the uninjured contralateral.Study Design:Controlled laboratory study.Methods:In this cross-sectional study, 11 participants who had undergone unilateral ACJ reconstruction surgery and 3 patients who received nonoperative treatment were enrolled. ACJ kinematics were assessed during active forward flexion, scaption, and abduction through high-speed DSX imaging, complemented by 3-D bone models obtained via computed tomography (CT) scans. To gauge kinematic differences, a 1-dimensional statistical parametric mapping method was employed, which compared outcomes in the index limb to those in the uninjured counterpart. In addition, the range of motion and isometric strength at various abduction angles were analyzed, employing a repeated-measures analysis of variance to compare the affected and uninjured sides.Results:Leveraging a combination of DSX imaging and patient-specific CT bone models, ACJ kinematics was measured successfully during movements along anatomic planes. Preliminary findings from this investigation revealed no detectable differences between the surgically reconstructed and uninjured sides in ACJ biomechanics, shoulder range of motion, and isometric strength outcomes. However, on average, the nonoperatively treated shoulders demonstrated increased internal rotation, upward rotation, and posterior tilting of the scapula relative to the clavicle (no statistical analyses were performed due to the small sample size).Conclusion:DSX imaging is a promising tool for evaluating potential in vivo kinematic abnormalities in the ACJ during muscle-driven activities, laying the groundwork for further investigations in both ACJ-reconstructed and nonreconstructed patients. This study furnished essential data for conducting power analyses and designing future studies with an adequate sample size to investigate the impact of different treatment approaches on shoulder girdle mechanics.Clinical Relevance:With its potential for accurately characterizing shoulder girdle kinematics post-ACJ injury, DSX imaging can offer valuable insights for future clinical studies, facilitating informed decisions regarding the short- and long-term impacts of treatment choices on shoulder health and function.

Indication for Computed Tomography Scan in Shoulder Instability: Sensitivity and Specificity of Standard Radiographs to Predict Bone Defects After Traumatic Anterior Glenohumeral Instability

Abstract:Background: Quantifying glenohumeral bone loss is key in preoperative surgical planning for a successful Bankart repair. Hypothesis: Simple radiographs can accurately measure bone defects in cases of recurrent shoulder instability. Study Design: Cohort study (diagnosis); Level of evidence, 2. Methods: A true anteroposterior (AP) view, alone and in combination with an axillary view, was used to evaluate the diagnostic properties of radiographs compared with computed tomography (CT) scan, the current gold standard, to predict significant bone defects in 70 patients. Sensitivity, specificity, and positive and negative predictive values were evaluated and compared. Results: Detection of glenoid bone loss on plain film radiographs, with and without axillary view, had a sensitivity of 86% for both views and a specificity of 73% and 64% with and without the axillary view, respectively. For detection of humeral bone loss, the sensitivity was 8% and 17% and the specificity was 98% and 91% with and without the axillary view, respectively. Regular radiographs would have missed 1 instance of significant bone loss on the glenoid side and 20 on the humeral side. Interobserver reliabilities were moderate for glenoid detection (κ = 0.473-0.503) and poor for the humeral side (κ = 0.278-0.336). Conclusion: Regular radiographs showed suboptimal sensitivity, specificity, and reliability. Therefore, CT scan should be considered in the treatment algorithm for accurate quantification of bone loss to prevent high rates of recurrent instability.

Abstract:Background Slipped capital femoral epiphysis (SCFE) is a frequent disorder of the adolescent hip, which may lead to avascular necrosis (AVN) of the femoral head, chondrolysis and early osteoarthritis due to the post-slip deformity of the proximal femur. To warrant the best possible outcome for the affected (and contralateral) hip, early diagnosis and proper treatment are needed. Methods A review of the literature was undertaken to identify today's role of available imaging modalities in the management of SCFE. Summary This review outlines the relevancy of different imaging modalities such as radiography, ultrasound, CT, MRI and bone scintigraphy in the treatment of SCFE patients. While standard radiography is the first-choice imaging modality for patients with suspected SCFE, ultrasound and advanced imaging modalities may aid in surgical planning, diagnosis of complications such as AVN and treatment follow-up.

Abstract:Sports-related articular and periarticular hip injuries are common in athletes. Knowledge of patient complaints and clinical findings are crucial for adequate interpretation of imaging examinations. However, asymptomatic athletes can present abnormal imaging findings, and clinical presentation of hip injuries may be nonspecific. Therefore, a thorough examination of the hip and surrounding soft tissue images is essential.This review describes the intra-articular pathologies encountered in sports activities including labrum or cartilage lesions, associated or not with femoroacetabular hip impingement syndromes, as well as ligament teres injuries, using magnetic resonance imaging (MRI) or computed tomography arthrography. Different causes of snapping hip syndrome (from intra- and extra-articular origins) are also discussed and illustrated. The extra-articular forms of hip impingement syndromes including ischiofemoral and subspine impingement are depicted with MRI and ultrasound. Diagnostic imaging of bone avulsions, greater trochanteric syndrome, athletic pubalgia, and myotendinous injuries is also described.

CO M M EN TA RY The treatment efficacy for manualised, emotionfocused psychotherapies for borderline personality disorder (BPD) is well documented. The accumulation of these studies has provided hope for what was once considered an intractable, lifethreatening condition. Despite these advances, questions about these treatments remain, including the longevity of the outcomes, the extent to which treatments impact multiple domains of functioning, and the degree to which specialised BPD therapies are unique or operate under a common set of therapeutic ingredients. This study has extended prior work by examining the benefits of two specialised BPD treatments (ie, dialectical behaviour therapy and general psychiatric management) over a 3-year period including 2 years of naturalistic follow-up. The equivalence in follow-up outcomes across treatments and primary and secondary outcomes is consistent with the original treatment study. Although not tested directly, the study highlights that effective treatments for BPD require a specialised, comprehensive team approach. Each treatment condition involved manualised interventions, training in suicide management, multiple treatment modalities and weekly supervision meetings. The extent to which each component is necessary, or contributes to the outcomes is unclear. A components analysis for improved feasibility of implementation would be the next logical step in this research. The study also showed a large percentage of sustained unemployment and psychiatric disability, despite achieving over 50% diagnostic remission of BPD in both treatments at 1-year follow-up and 2-year follow-up. The fact that a high percentage of participants were not working or off disability is not surprising given that neither treatment explicitly targeted these outcomes nor was it a hypothesised goal of the original study. It also seems unlikely that symptom remission would be sufficient to compel participants into the workforce without an explicit therapeutic focus on these goals. Instead, the results highlight the continued need for psychotherapeutic care following symptom remission if additional client goals are to be achieved.
Identification of Diagnostic Magnetic Resonance Imaging Findings in 47 Shoulders with Subcoracoid Impingement Syndrome by Comparison with 100 Normal Shoulders.

Abstract:BACKGROUND The aim of this study was to identify the diagnostic magnetic resonance imaging (MRI) findings in 47 shoulders with subcoracoid impingement syndrome by comparison with 100 normal shoulders. MATERIAL AND METHODS The subcoracoid impingement syndrome group consisted of 47 shoulders with subcoracoid impingement syndrome and the normal group consisted of 100 normal shoulders. The MRI parameters - coracoids-humeral distance (CHD), coracoid index (CI), height of the lesser tuberosity (HLT), coracoid obliquity (CO), coracoglenoid angle (CGA), coracohumeral angle (CHA), width of the subscapular tendon (WST), and contact distance between subscapular tendon and coracoid process (CD) - were compared between the subcoracoid impingement syndrome group and the normal group. The areas under the curves (AUCs) from the receiver operating characteristic (ROC) for single MRI parameters were recorded, in which the MRI parameters with AUC exceeding 0.70 were included in the analysis of combined parameters. Comparisons of ROC were made among single parameters and combined parameters. RESULTS For diagnosing subcoracoid impingement syndrome by using single MRI parameters (CHD, CI, HLT, CGA, CHA, WST, and CD), the AUCs were 0.963, 0.806, 0.745, 0.691, 0.613, 0.685, and 0.614, respectively, of which CHD had the largest AUC. CHD, CI, and HLT (AUC exceeding 0.70) were included in the study of the combined parameters. The AUC of combined CHD and HLT showed a significantly larger AUC than that of CHD (0.986 vs 0.963, P=0.036), and showed no significant difference compared with that of combined CHD, CI, and HLT (0.986 vs 0.987, P=0.882). CONCLUSIONS Measurement of the coracoid-humeral distance and height of the lesser tuberosity were key MRI diagnostic findings for subcoracoid impingement syndrome.

Abstract:Shoulder arthroplasty has become a standard surgical procedure for treating a variety of complex shoulder disorders, including those with degenerative and traumatic aetiologies. The ever-improving success rates of shoulder arthroplasty could be attributed to advancements in endoprosthesis design, improvements in the biomechanics of endoprosthetic components, and improvements in surgical techniques. It improves patient outcomes and helps restore shoulder joint function and mobility. Imaging plays a vital role by enabling surgeons to plan arthroplasty procedures, help guide endoprosthesis placement, and monitor postoperative outcomes. In addition, imaging plays a role in assessing the residual bone stock and status of rotator cuff integrity and in correcting the placement of prosthetic components to restore shoulder mobility. CT-guided navigation aids surgeons by helping them choose appropriate components for implants and ensuring that implants are placed optimally during surgery. It can lead to better surgical results with reduced patient morbidity and a longer duration of prosthetic stability. After surgery, it is crucial to use imaging techniques to detect issues such as periprosthetic loosening, infections, or fractures to start effective management strategies to enhance patient recovery. This article aims to provide orthopaedic surgeons and radiologists with knowledge on the imaging methods used in shoulder arthroplasty and their role in presurgical planning, intraoperative guidance and postoperative assessment. In this study, we aimed to investigate the rationale behind utilising various types of shoulder replacements: total shoulder replacement (TSA), reverse total shoulder arthroplasty (RTSA), and hemiarthroplasty; methods, their respective advantages and limitations; and outcomes. Our objective is to comprehensively analyse the procedures mentioned above and highlight their unique features and benefits to facilitate a better understanding of these approaches. Additionally, we will discuss how these imaging techniques help identify issues such as loose components, fractures around the implant site, joint instability and infections.
Quantitative Analysis of Dynamic Subacromial Ultrasonography: Reliability and Influencing Factors

Abstract:Objective: Current imaging methods used to examine patients with subacromial impingement syndrome (SIS) are limited by their semi-quantitative nature and their capability of capturing dynamic movements. This study aimed to develop a quantitative analytic model to assess subacromial motions using dynamic ultrasound and to examine their reliability and potential influencing factors. Method: We included 48 healthy volunteers and examined their subacromial motions with dynamic ultrasound imaging. The parameters were the minimal vertical acromiohumeral distance, rotation radius, and degrees of the humeral head. The generalized estimating equation (GEE) was used to investigate the impact of different shoulder laterality, postures, and motion phases on the outcome. Result: Using the data of the minimal vertical acromiohumeral distance, the intra-rater and inter-rater reliabilities (intra-class correlation coefficient) were determined as 0.94 and 0.88, respectively. In the GEE analysis, a decrease in the minimal vertical acromiohumeral distance was associated with the abduction phase and full-can posture, with a beta coefficient of −0.02 cm [95% confidence interval (CI), −0.03 to −0.01] and −0.07 cm (95% CI, −0.11 to −0.02), respectively. The abduction phase led to a decrease in the radius of humeral rotation and an increase in the angle of humeral rotation, with a beta coefficient of −1.28 cm (95% CI, −2.16 to −0.40) and 6.60° (95% CI, 3.54–9.67), respectively. A significant negative correlation was observed between the rotation angle and radius of the humeral head and between the rotation angle and the minimal vertical acromiohumeral distance. Conclusion: Quantitative analysis of dynamic ultrasound imaging enables the delineation of subacromial motion with good reliability. The vertical acromiohumeral distance is the lowest in the abduction phase and full-can posture, and the rotation angle of the humeral head has the potential to serve as a new parameter for the evaluation of SIS.

Abstract:Evaluation of the clinical performance of ultra-low-dose computed tomography (CT) images of the shoulder joint on image-based diagnosis and three-dimensional (3D) printing surgical planning. A total of 93 patients with displaced shoulder fractures were randomly divided into standard-dose, low-dose, and ultra-low-dose groups. Three-dimensional printing models of all patients' shoulder joints were fabricated. The subjective image quality and 3D-printing model were evaluated by two senior orthopedic surgeons who were blinded to any scanning setting. A 3-point scale system was used to quantitatively assess the image quality and 3D printing model, where more than 2 points meant adequate level for clinical application. Compared with the standard dose protocol, ultra-low-dose technique reduced the radiation dose by 99.29% without loss of key image quality of fracture pattern. Regarding the subjective image quality, the assessment scores for groups of standard, low, and ultra-low doses were 3.00, 2.76, 2.00 points on scapula and humerus, and 3.00, 2.73, 2.44 points on clavicle. Scores of the three groups for the assessment of 3D printing models were 3.00, 2.80, 1.34 on scapula and humerus, and 3.00, 2.90, 2.06 on clavicle. In the ultra-low-dose group, 24 out of 33 (72.7%) 3D printing models of scapula and humerus received lower than 2 points of the evaluation score, while nearly 94% of the clavicle models reached the adequate level. An ultra-low-dose protocol is adequate for the diagnosis of either displaced or non-displaced fractures of the shoulder joint even though minor flaws of images are present. Three-dimensional printing models of shoulder joints created from ultra-low-dose CT scans can be used for surgical planning at specific bone like the clavicle but perform insufficiently in the overall surgical planning for shoulder injuries due to the significant geometric flaws.

Deciphering acute shoulder trauma with normal initial X-ray: Contributions of ultrasonography and MRI

Abstract:Introduction: Shoulder trauma with normal radiographs is a frequent emergency presentation. The predominance of pain makes clinical examination unreliable, and complementary imaging is often needed for diagnosis. The aim of the present study was to assess the contribution of ultrasonography (US) and magnetic resonance imaging (MRI) in deciphering such trauma. The study hypothesis was that MRI can confirm most diagnoses and reveal occult lesions. Methods: 48 patients with a median age of 33 years (IQR, 22-48.75), admitted to the Emergency Department between February and September 2016, were prospectively included. All had normal X-ray and underwent emergency and deferred emergency (D+7-10) clinical examination, and US and MRI at D+7-14. The same examinations were repeated to establish diagnosis and initiate treatment. MRI was considered as gold-standard for definite diagnosis. Results: Diagnoses comprised: 14 shoulder contusions; 13 rotator cuff tears; 8 acromioclavicular sprains; 6 fractures: 3 greater tuberosity, 2 humeral head, 1 lateral quarter of the clavicle; 3 subluxations or anterior dislocations with spontaneous reduction; 1 posterior dislocation with spontaneous reduction; 1 case of resorption of calcification in the pectoralis major tendon; 1 case of coracobrachialis strain; and 1 partial pectoralis major detachment. Concordance with final diagnosis was 42% (20/48) on initial clinical examination and 52% (25/48) on reassessment. On US, concordance was 71% (34/48), with 82% positive predictive value for cuff tear and 0% for instability. MRI was contributive in all cases, revealing 48 abnormal signals in 41 patients, thus establishing 48 diagnoses. Conclusion: Except in precise diagnostic situations such as rotator cuff tear, US is not a contributive examination, completely overlooking shoulder instability and bone contusion. MRI should play an increasing role in deciphering these traumas. Level of evidence: II; prospective cohort study.
