Abstract:There is increasing clinical requirement for early and accurate ultrasound diagnosis of rotator cuff tears (RCTs). A method based on non-linear statistical analysis was proposed for the detection of RCTs using ultrasound radiofrequency (RF) signals. One hundred fifty-two patients with shoulder pain were first examined with ultrasound and then diagnosed with magnetic resonance imaging (MRI) as the ground truth. By comparison of the region of interest (ROI) with a part of the supraspinatus with no pathologic change part in the same RF signal frame, the relative P-ks value (viz., rP(ks) value) was evaluated to quantify the pathophysiologic changes. The results indicated that the rP(ks) values of all RCTs are <0.7, and the accuracy, sensitivity and specificity of the proposed method can reach 97.5%, 100% and 91.4%, respectively. This computer-aided method was found to perform better diagnostic than the results reported by an experienced radiologist (accuracy = 75.7%, sensitivity = 72.6%, and specificity = 85.7%). The high sensitivity advantage of this method indicates that the prospects for its application in the computer-aided diagnosis of RCTs are good. Breast cancer stem cells are involved in Trastuzumab resistance through the HER2 modulation in 3D culture

Abstract:Breast cancer human cells culture as spheroids develop autophagy and apoptosis, which promotes Trastuzumab resistance in HER2 overexpressing cells. Our aim was to study the association of the hostile environment developed in 3D with the breast cancer stem cells population and the HER2 modulation. Human mammary adenocarcinoma cell lines were cultured as spheroids using the hanging drop method. We generated hypoxia conditions by using a hypoxic chamber and CoCl2 treatment. Breast cancer stem cells were measured with mammosphere assays, the analysis of CD44 + CD24low population by flow cytometry and the pluripotent gene expression by RT‐qPCR. HER2 expression was evaluated by flow cytometry and Western blot. MTS assays were conducted to study cell viability. Hostil environment developed in spheroids, defined by hypoxia and autophagy, modulated the response to Trastuzumab. In HER2+ cells with acquired resistance, we observed an increase in the breast cancer stem cell population. In BT474 spheroids, Trastuzumab induced the acquisition of resistance, along with an increase in breast cancer stem cells. Also, in 3D culture conditions we determined a modulation in the HER2 expression. Moreover, breast cancer stem cells showed enhanced HER2 expression. Finally, cells without HER2 gene amplification cultured as spheroids were sensitive to Trastuzumab, diminishing HER2 expression and cancer stem cells. Our findings show that 3D architecture is able to modulate breast cancer stem cell population and HER2 distribution, modifying the cell response to Trastuzumab.
Possibility of diagnosing rotator cuff tears in areas with scarce medical resources: can non-standard anteroposterior radiographs accurately predict rotator cuff tears?

Abstract:Background Due to the scarcity and high cost of MRI in resource-constrained regions, prompt diagnosis and treatment of rotator cuff tears remain problematic in these areas. Therefore, extensive research has been conducted to predict rotator cuff tears using simple and affordable anteroposterior radiographs. It remains unclear whether non-standard anteroposterior radiographs would have a notable impact on the preciseness of the diagnosis. Method We analyzed patients treated for shoulder pain at hospitals. These patients underwent shoulder joint MRI and standard anteroposterior radiographs, were categorized into those with rotator cuff tears and a control group. We assessed whether the radiographs were standard anteroposterior radiographs using classification criteria from previous studies. Three assessors independently measured the acromiohumeral interval, upwards migration index, acromion index, critical shoulder angle, and double-circle radius ratio in radiographic images. The intraclass correlation coefficient and receiver operating characteristic curves were used to assess measurement reliability and predictive capabilities of each predictive method for rotator cuff tears. Results This study included 102 non-standard radiographs that met the research criteria for the measurement and analysis. The intragroup correlation coefficients for the acromiohumeral interval, upwards migration index, and double-circle radius ratio were above 0.7 (0.77, 0.71, 0.76), while those for the acromion index and critical shoulder angle exceeded 0.8 (0.86 and 0.87). In non-standard radiographs, the double-circle radius ratio reliably predicted rotator cuff tears ( p < 0.05), contrary to the other methods ( p > 0.05). The areas under the receiver operating characteristic curves of the double-circle radius ratio, estimated by the three researchers for rotator cuff tears. Conclusion This study found that non-standard radiographs significantly impaired the diagnostic performance of the acromiohumeral interval, upwards migration index, acromion index, and critical shoulder angle. Only the double-circle radius ratio maintained its predictive power (although this diminished capability may fall short of clinical relevance) and demonstrated high applicability. These findings indicate the need for researchers to prioritize the quality of radiographs and focus on reducing the sensitivity of the prediction method in relation to radiograph quality. The capability exhibited by the double-circle radius ratio warrants further investigation, to facilitate a simplified diagnosis of rotator cuff tears.

Changes in sodium, potassium, and adenosine triphosphate contents of red blood cells in sepsis and septic shock.

Abstract:Sepsis and septic shock were induced in fifteen awake rabbits by the infusion of live Escherichia coli. Sodium K+, and adenosine triphosphate (ATP) concentrations in red blood cells (RBC) and plasma were measured during the control, septic, and septic shock periods. The significant elevations of Na+ content in RBC during sepsis appeared to be primarily a function of increased cell membrane permeability. The significantly decreased intracellular K+ concentrations, accompanying the high Na+ levels of RBC later in shock, were consistent with progressive failure of the energy-dependent transport mechanism (Na+-k+ pump). Hyponatremia and hyperkalemia were apparent in the late stages of sepsis, these alterations reached statistically significant levels in the shock period. The electrolyte derangements associated with sepsis and septic shock could not be related to energy depletion. The continuous significant accumulations of ATP, observed in RBC and plasma, were interpreted as a result of decreased energy utilization and attributed to the diminished active ion transport by the impaired Na+-K+ pump.

Abstract:Fatty infiltration (FI) of the rotator cuff is an important clinical outcome. Quantitative‐based ultrasound methods to estimate FI of the supraspinatus and infraspinatus were compared to MRI fat fractions. Preliminary evidence suggests ultrasound may be used to estimate FI. Future research should compare these modalities across a larger, diverse population. Introduction Fatty infiltration (FI) of the rotator cuff has important clinical implications. Quantitatively estimating FI using ultrasound (US) has considerable benefits for assessing FI in a non‐invasive, accessible manner. This research investigated whether FI of the supraspinatus (SS) and infraspinatus (IS), estimated using US was related to intramuscular fat fractions measured from magnetic resonance images (MRI). Methods Data from 12 healthy young adult participants were used for analysis. US images of the SS and IS were captured using multiple transducer placement techniques from which echogenicity of the muscle region was quantified. Shoulder MRI were captured from which SS and IS were manually segmented and intramuscular fat fractions calculated. Six upper limb strength exertions were performed, resisted by a hand dynamometer. Results IS and SS echogenicity explained a significant amount of variance in MRI fat fractions for certain body positions and transducer techniques. Echogenicity agreement was higher for IS than SS. Significant relationships were identified between strength exertions and both echogenicity and MRI muscle volume, but not MRI fat fraction. Conclusions This research provides preliminary evidence showing that quantitative‐based US methods can be used to estimate MRI calculated fat fractions for the rotator cuff.

Abstract:Background Patients treated for symptomatic rotator cuff tear (RCT) on one shoulder seem to have a higher prevalence of RCT on the contralateral shoulder. Purpose To compare the supraspinatus (SSP) tendon and RC muscle properties on the contralateral shoulder in patients after repair surgery to those healthy individuals using quantitative magnetic resonance imaging (MRI). Material and Methods A total of 23 patients treated for RCT (group A) and 23 healthy controls (group B) were recruited. Constant score, visual analog scale score (VAS), and MRI examinations were conducted. The SSP tendon structural status was graded based on the Zlatkin classification and quantified on ultrashort echo time (UTE)-T2* mapping images. Fatty degeneration of RC muscles was classified according to the Goutallier classification and quantified on T2 mapping. Results The Constant and VAS scores were comparable between groups A and B (all P >0.05). No significant differences were observed in tendon structural status between the two groups ( P >0.05). However, significant differences were established in UTE-T2* values of the SSP tendon on the distal subregion between groups A and B (16.4 ± 2.4 ms vs. 14.8 ± 1.2 ms; P = 0.01). Regarding muscle degeneration, no significant differences were displayed in T2 values and Goutallier classification of RC muscles (all P >0.05). Conclusion Patients with a treated RCT demonstrated inferior SSP tendon in the distal subregion on the contralateral shoulders one year postoperatively compared to that of healthy controls based on quantitative MRI data.

Intersession Repeatability of Diffusion-Tensor Imaging in the Supraspinatus and the Infraspinatus Muscles of Volunteers

Abstract:Background: Quantifying the rotator cuff (RC) muscles' viscoelasticity could provide outcome relevant information in patients with RC tears. MR-elastography requires robust diffusion-tensor imaging (DTI) to account for tissue anisotropy in muscles stiffness computation. Purpose: To assess the repeatability of DTI parameters in the supraspinatus and infraspinatus muscles and to explore DTI tractography conformity with the muscles' anatomy. Study type: Prospective. Subjects: Six healthy volunteers underwent three consecutive shoulder MRI sessions about 10 minutes apart. Field strength/sequence: 3T/T1-vibe Dixon and Spin echo EPI DTI (12 gradient encoding directions, b-values 500 and 800 sec/mm2 ). Assessment: Supraspinatus and infraspinatus muscles were segmented on the T1-vibe Dixon sequence. DTI image quality was assessed using a quantitative threshold based on the signal-to-noise ratio (SNR). The eigenvalues ( λ 1 , λ 2 , λ 3 ), fractional anisotropy (FA) and mean diffusivity were calculated. DTI tractography was visually assessed. Statistical tests: DTI parameters within-subject intersession repeatability was assessed with Bland-Altman analysis and the coefficient of variation (CV). Repeatability was considered good for CV < 10%. Results: The SNR between diffusion-weighted and non-diffusion-weighted images was greater than 3, which aligns with standards for estimating DTI parameters. The FA showed the lowest mean bias (-0.007; 95% confidence interval [CI] -0.031 to 0.018) whereas the λ1 had the highest mean bias (0.146 × 10-3 mm2 /sec; CI -0.034 to 0.326 × 10-3 mm2 /sec). CVs of the DTI parameters varied between 3.5% (FA) and 8.4% (λ3 ) for the supraspinatus and between 3.2% (λ1 ) and 6.8% (λ3 ) for the infraspinatus. Tractography provided muscle fiber representations in three-dimensional space concordant with RC anatomy. Data conclusion: DTI of the supraspinatus and infraspinatus muscles achieved an adequate SNR, allowing the measurement of the DTI metrics with good repeatability, and thus can be used for optimizing stiffness estimation in these anisotropic tissues. Enhancing Preoperative Diagnosis of Subscapular Muscle Injuries with Shoulder MRI-based Multimodal Radiomics

Abstract:Rationale and objectives: Rotator cuff injury is a common ailment in the musculoskeletal system, with the subscapularis muscle being the largest and most robust muscle of the rotator cuff. The occurrence of subscapularis muscle tears is more frequent than previously reported. The main objective of this research is to harness the power of artificial intelligence to enhance the precision in diagnosing subscapularis muscle injuries via magnetic resonance imaging of the shoulder joint, prior to surgical intervention. This study seeks to integrate advanced artificial intelligence algorithms to analyze magnetic resonance imaging data, aiming to provide more accurate preoperative assessments, which can potentially lead to better surgical outcomes and patient care and promote technological progress in the field of medical imaging analysis. Method: This is a multicenter study that involves 324 patients from a major medical center serving as both the training and testing groups, with an additional 60 patients from two other medical centers comprising the verifying group. The imaging protocol for all these subjects included a series of shoulder magnetic resonance imaging scans: T1-weighted coronal sequences, T2-weighted coronal, axial, and sagittal images. These comprehensive imaging modalities were utilized to thoroughly examine the shoulder joint's anatomical details and to detect any signs of subscapularis muscle damage. To enhance the diagnostic accuracy before surgical procedures, radiomic analysis was employed. This technique involves the extraction of a multitude of quantitative features from the magnetic resonance imaging, which can provide a more nuanced and data-driven approach to identifying subscapularis muscle injuries. The integration of radiomics in this study aims to offer a more precise preoperative assessment, potentially leading to improved surgical planning and patient outcomes. Result: In the course of this study, a comprehensive extraction of 1197 radiomic features was performed for each imaging modality of every patient. The coronal T1-weighted modality, when assessed within the internal verifying cohort, delivered a diagnostic accuracy of 0.766, coupled with an AUC of 0.803. In the case of the T2-weighted modality, the coronal planes exhibited a diagnostic accuracy of 0.781 and an AUC of 0.844. The axial T2-weighted images recorded an accuracy of 0.719 and an AUC of 0.761, while the sagittal T2-weighted images scored an accuracy of 0.766 and an AUC of 0.821. The amalgamation of these imaging techniques through a multimodal strategy markedly enhanced the accuracy to 0.828, with an AUC of 0.916 for the internal verifying group. The diagnostic performance of the coronal T1-weighted modality in the external verifying cohort yielded an accuracy of 0.833, with an area under the curve (AUC) of 0.819. For the T2-weighted modality, the coronal imaging demonstrated an accuracy of 0.767 and an AUC of 0.794. The axial T2-weighted images had an accuracy of 0.783 and an AUC of 0.797, while the sagittal T2-weighted images achieved an accuracy of 0.833 and an AUC of 0.800. When combining the modalities, the multimodal approach significantly improved the accuracy to 0.867, with an AUC of 0.803 in the external verifying group, indicating a robust diagnostic capability. Conclusion: Our study demonstrates that the application of multimodal radiomic techniques to shoulder magnetic resonance imaging significantly enhances the precision of preoperative diagnosis for subscapularis muscle injuries. This approach leverages the comprehensive data provided by various magnetic resonance imaging modalities to offer a more detailed and accurate assessment, which is crucial for surgical planning and patient care.
Diagnostic performance of conventional magnetic resonance imaging for detection and grading of subscapularis tendon tear according to Yoo and Rhee classification system in patients underwent arthroscopic rotator cuff surgery

Abstract:ObjectivesTo assess the diagnostic accuracy of conventional MRI for detecting and grading subscapularis (SSC) tears by applying the Yoo and Rhee classification.Materials and methodsA total of 179 patients who underwent MRI followed by arthroscopic rotator cuff surgery were enrolled. Two musculoskeletal radiologists evaluated the SSC using axial, oblique sagittal, and oblique coronal MRI according to the Yoo and Rhee classification. Using arthroscopic findings as the reference standard, the sensitivity, specificity, accuracy, and interobserver agreement of SSC tears were analyzed.ResultsArthroscopy confirmed that the numbers of type I, IIA, IIB, III, IV, and V tears were 35, 70, 35, 9, 9, and 0, respectively. The sensitivity, specificity, and accuracy of readers 1 and 2 for the detection of tears (type IIA or higher) were 85%, 75%, and 82%, and 89%, 70%, and 83%, respectively, while those for the detection of surgical candidates (type IIB or higher) were 77%, 75%, and 75%, and 77%, 83%, and 82%, respectively. The interobserver agreement for detecting SSC tear presence was substantial (κ = 0.70) for reader 1 vs. reader 2, and those for detecting the surgical candidate group was substantial (κ = 0.68) for reader 1 vs. reader 2. The interobserver agreement for grading SSC tears was excellent (κ = 0.86) for reader 1 vs. reader 2.ConclusionConventional MRI showed 82.5% and 78.5% average accuracy in detecting IIA and IIB or higher tears by applying the Yoo and Rhee classification for the diagnosis of SSC tears with an excellent interobserver agreement in tear grading.

Risk Factors Analysis and Prediction of Rotator Cuff Tears: A Retrospective Study

Abstract:Aim: Rotator cuff tears (RCTs) are a major cause of shoulder pain and disability, affecting millions worldwide. Understanding the risk factors and developing reliable predictive measures for RCTs is essential for early diagnosis, targeted prevention, and effective treatment of this patient population. This study seeks to enhance our understanding by analyzing the acromiohumeral distance (AHD) and Constant-Murley Score (CMS) in patients with and without RCTs, thereby aiding the development of a predictive model aimed at improving clinical outcomes and prevention strategies in rotator cuff pathology. Methods: This retrospective analysis involved 201 patients with shoulder pain, categorized into RCT (n = 72) and no RCTs (N-RCTs, n = 129) groups based on Magnetic Resonance Imaging (MRI) findings. We compared demographics, AHD, CMS, and rotator cuff status between groups and utilized logistic regression for identifying RCT predictors, leading to the development of a multifactorial predictive model. Results: The mean AHD was 6.60 ± 1.12 mm. The RCT group showed a marginally higher AHD than the N-RCT group (p = 0.669). CMS scores were significantly lower in the RCT group (p < 0.001). Dominant side involvement (Odds Ratio (OR) 2.244), type III acromion (OR 6.106), and lower CMS (OR 0.938) significantly correlated with RCTs. The predictive model demonstrated an area under the curve (AUC) of 0.701 for RCT diagnosis. Conclusions: Reduced CMS, dominance of the affected side, and type III acromion emerged as key risk factors for RCTs. Our predictive model, incorporating these factors, holds promise for RCT diagnosis, with future studies needed for further validation.

Abstract:Background: Monitoring the fatty infiltration (FI) process in rotator cuff muscles is of value in establishing a treatment plan and predicting the postoperative prognosis. Quantitative T1 mapping shows promise for evaluating muscle degeneration, while its validity in monitoring rotator cuff muscle FI progression needs further investigation. Purpose: To determine the validity of T1 mapping in monitoring FI progression of rotator cuff muscles. Study Design: Controlled laboratory study. Methods: Sprague-Dawley rats (N = 108) underwent left supraspinatus (SS) and infraspinatus (IS) tenotomy only (TT), suprascapular nerve transection only (NT), or SS and IS tenotomy plus suprascapular nerve transection (TT+NT). Sham surgery on the right shoulder served as the control. The magnetic resonance imaging examination included T1 mapping performed at 12, 16, and 20 weeks postoperation. SS and IS muscles were harvested to quantitatively evaluate FI via direct evaluation (triglyceride quantification assay and histological analysis) at the same predetermined intervals. The correlation of the imaging data with direct evaluation of rotator cuff muscles was analyzed. Results: T1 values were significantly lower in left SS and IS muscles at 12, 16, and 20 weeks postoperation as compared with those on the right side. T1 values of the left SS and IS muscles were continuously decreased in all groups. The TT+NT group had a greater decrease in T1 value than did the TT and NT groups. Triglyceride quantification assay and histological analysis demonstrated significant and progressive FI of the left SS and IS muscles in the 3 groups. The most serious FI changes were observed in the TT+NT group. T1 values were also well correlated with triglyceride contents and area fractions of fat. Conclusion: T1 mapping can be an effective imaging modality for sensitive and quantitative monitoring of FI progression in rotator cuff muscles. Clinical Relevance: The findings of this study provide a tool for researchers to noninvasively and quantitatively monitor the process of muscle degeneration, contributing to the evaluation of surgical indication and postoperative prognosis.

Abstract:Background:Rotator cuff injury is one of the most common tendon injuries in adults. Rotator cuff injury has a high incidence of 22% in 65 years old and can increase to over 62% in 80 years old. With the rapid development of rotator cuff surgery technology, the expenditure of medical systems in various countries is increasing year by year, and the disease itself can bring great pain to patients with obvious pain at night, affecting daily work and life. Therefore, early and accurate diagnosis of rotator cuff injury is of great significance. Preoperative imaging examination of the nature and size of rotator cuff injury is of great value to the selection of treatment and prognosis. Magnetic resonance imaging (MRI) has become one of the most commonly used examination methods for preoperative diagnosis of rotator cuff injury due to its non-invasive, good tissue illumination and accurate information of muscle atrophy, degree of fat infiltration and labial injury. It has been widely used in clinical diagnosis and treatment. Some studies have shown that arthroscopic measurement of rotator cuff tear has high accuracy and can be used as the gold standard for detection. Previous studies in our department have shown that MRI has good specificity and sensitivity for rotator cuff injury, and is an effective auxiliary examination method. As far as we know, there is no systematic quantitative analysis of the consistency between rotator cuff tears of different degrees in MRI and specific values measured under arthroscopy. The choice of treatment and prognosis of rotator cuff tear vary with the degree of rotator cuff tear. Therefore, it is of great significance to accurately determine the value of rotator cuff tear before surgery for the selection of correct treatment and improvement of healing rate and postoperative function.Objective:To determine the accuracy of the MRI for detection and measurement of the size of rotator cuff tears, including full-thickness tear and partial-thickness tear, with arthroscopic findings used as the standard.Methods:From December 2016 to November 2017, 212 patients were selected from the department of orthopaedics and traumatology in our hospital, MRI examination and intraoperative diagnosis were performed in 205 cases, including 112 cases with full-thickness tear and 93 cases with partial tear. For full-thickness tears, the anteroposterior diameters and medial-lateral diameters of the tears were measured under MRI and arthroscopy, and Bland-Altman test was used for consistency analysis. For partial tears, the depth of the tear was measured under MRI and arthroscopy, and the newe classification was made according to Ellman classification. The consistency of the two classifications were compared by Kappa test.Results:For the full-thickness tear, the mean difference of the tear length between the two methods was -0.4 mm, and the standard deviation was 2.2 mm. 95% conformance boundaries are (-4.8, 4.0) mm. 96.4% of the points were within the range of LOA. For the tear width group, the mean difference was -0.1 mm, and the standard deviation was 2.3 mm. The calculated 95% consistency limit was (-4.6, 4.4) mm. 95.5% of the points were located within the LOA range. For 93 patients in the partial tear group, 29 patients were classified as Ellman I, 47 patients as Ellman II, and 17 patients as Ellman III. The tearing depth was measured according to Ellman classification, with 30 patients classified as Ellman I, 45 patients as Ellman II, and 18 patients as Ellman III. After Kappa consistency test, the standard deviation was 0.06 and 95% CI was 0.620-0.856. The weighed Kappa value was 0.738.Conclusion:For full-layer tears, MRI and arthroscopy have a high degree of consistency in the measured values of medial-lateral diameters and anteroposterior diameters. It can accurately predict the degree of tear quantitatively. The measurement of partial tear degree also has good consistency, but does not reach the ideal limit. The results of MRI examination cannot be used as the only standard for the diagnosis and classification of partial tears. In clinical practice, the results of other examinations should be integrated to make an accurate judgment.
Risk Factor Analysis for Predicting the Onset of Rotator Cuff Calcific Tendinitis Based on Artificial Intelligence

Abstract:Background. Symptomatic rotator cuff calcific tendinitis (RCCT) is a common shoulder disorder, and approaches combined with artificial intelligence greatly facilitate the development of clinical practice. Current scarce knowledge of the onset suggests that clinicians may need to explore this disease thoroughly. Methods. Clinical data were retrospectively collected from subjects diagnosed with RCCT at our institution within the period 2008 to 2020. A standardized questionnaire related to shoulder symptoms was completed in all cases, and standardized radiographs of both shoulders were extracted using a human-computer interactive electronic medical system (EMS) to clarify the clinical diagnosis of symptomatic RCCT. Based on the exclusion of asymptomatic subjects, risk factors in the baseline characteristics significantly associated with the onset of symptomatic RCCT were assessed via stepwise logistic regression analysis. Results. Of the 1,967 consecutive subjects referred to our academic institution for shoulder discomfort, 237 were diagnosed with symptomatic RCCT (12.05%). The proportion of women and the prevalence of clinical comorbidities were significantly higher in the RCCT cohort than those in the non-RCCT cohort. Stepwise logistic regression analysis confirmed that female gender, hyperlipidemia, diabetes mellitus, and hypothyroidism were independent risk factors for the entire cohort. Stratified by gender, the study found a partial overlap of risk factors contributing to morbidity in men and women. Diagnosis of hyperlipidemia, diabetes mellitus, and hypothyroidism in male cases and diabetes mellitus in female cases were significantly associated with symptomatic RCCT. Conclusion. Independent predictors of symptomatic RCCT are female, hyperlipidemia, diabetes mellitus, and hypothyroidism. Men diagnosed with hyperlipidemia, diabetes mellitus, and hypothyroidism are at high risk for symptomatic RCCT, while more medical attention is required for women with diabetes mellitus. Artificial intelligence offers pioneering innovations in the diagnosis and treatment of musculoskeletal disorders, and careful assessment through individualized risk stratification can help predict onset and targeted early stage treatment.

Abstract:Objective To evaluate the superiority of magnetic resonance imaging (MRI) and three-dimensional (3D) ultrasound in diagnosing rotator cuff tears (RCT) by comparing with the shoulder arthroscopy. Methods From August 2012 to January 2014, 46 patients with RCT used MRI and 3D ultrasound exam before operation, compared all result with each presentation of shoulder arthroscopy, in order to evaluate the accuracy of MRI and 3D ultrasound in diagnosing RCT. Results Among 46 patients, the diagnosis was 40 with full thickness RCT and 6 with partial thickness RCT by MRI; 37 patients with full thickness RCT and 9 patients with partial thickness RCT by 3D ultrasound; 39 patients with full thickness RCT and 7 patients with partial thickness RCT by shoulder arthroscopy. The sensibility, specificity and accuracy of MRI in diagnosing full thickness RCT was 97.4%, 71.4% and 93.4%;the sensibility, specificity and accuracy of 3D ultrasound was 92.3%, 85.7% and 91.3%. There was no significant difference between MRI and 3D ultrasound in diagnosing full thicknes RCT (P<0.05), but the specificity of MRI in diagnosing partial thickness RCT was better than 3D ultrasound (P <0.05). Conclusion Both 3D ultrasound and MRI has relatively high accuracy and specificity in diagnosing full thickness RCT, but MRI is better than 3D ultrasound in diagnosing partial thickness RCT.
Shoulder MRI-based Radiomics for Diagnosis and Severity Staging Assessment of Surgically Treated Supraspinatus Tendon Tears

Abstract:To develop and validate MRI-based radiomics models capable of evaluating supraspinatus tendon tears within the shoulder joints by using arthroscopy as the reference standard. A total of 432 patients (332 in the training set and 100 in the external validation set) with intact supraspinatus tendon (n = 202) and supraspinatus tendon tear (n = 230, 130 full-thickness tears and 100 partial-thickness tears) were enrolled. Radiomics features were extracted from fat-saturated T2-weighted coronal images. Two radiomics signature models for detecting supraspinatus tendon abnormalities (tear or not), and stage lesion severity (full- or partial-thickness tear) and radiomics scores (Rad-score), were constructed and calculated using multivariate logistic regression analysis. The diagnostic performance of the two models was validated using ROC curves on the training and validation datasets. For the radiomics model of no tears or tears, thirteen features from MR images were used to build the radiomics signature with an AUC value of 0.98 in the training set, 0.97 in the internal validation set, and 0.98 in the external validation set. For the radiomics model of full- or partial-thickness tears, thirteen features from MR images were used to build the radiomics signature with an AUC value of 0.79 in the training set, 0.69 in the internal validation set, and 0.77 in the external validation set. The proposed radiomics models in this study can accurately rule out supraspinatus tendon tears and are capable of assessing the severity staging of tears with moderate accuracy based on shoulder MR images. • The radiomics model of no tears or tears achieved a high overall accuracy of 93.6
Detection of erosions and fat metaplasia of the sacroiliac joints in patients with suspected sacroiliitis using a chemical shift-encoded sequence (IDEAL-IQ)

Abstract:Purpose To evaluate the performance of a chemical shift-encoded sequence called IDEAL-IQ for detecting sacroiliac joint (SIJ) erosions and fat metaplasia compared to T1-weighted fast spin echo (T1 FSE) using qualitative and quantitative analysis. Method Thirty-four patients with suspicion of sacroiliitis who underwent both MRI and CT were included. Each SIJ was divided into four quadrants for analysis. For qualitative analysis, the diagnostic performance of IDEAL-IQ and T1 FSE for erosions were compared by the McNemar test, using CT as the gold standard. Cochran's Q and McNemar tests were used to determine differences in structural changes detected by different imaging methods. For quantitative analysis, two-sample t test and receiver operating characteristic (ROC) analysis were used for the analysis of histogram parameters of proton density fat fraction (PDFF). Results Diagnostic sensitivity and accuracy of IDEAL-IQ were greater than T1 FSE for erosions (all P < 0.05). IDEAL-IQ and CT detected more erosions than T1 FSE (all P < 0.05). IDEAL-IQ did not statistically significantly differ from T1 FSE for the detection of fat metaplasia ( P = 0.678). All histogram parameters were different between groups with and without fat metaplasia (all P < 0.05) and could distinguish the two groups (all P < 0.05). PDFF 75th was the most effective histogram parameter. Conclusion IDEAL-IQ detects SIJ erosions with better accuracy than T1 FSE and is similar to T1 FSE for detection of fat metaplasia, enabling further quantitative analysis of the latter via histogram analysis.
