Abstract:Cochlear implants (CIs) have tremendously helped people with severe to profound hearing loss to gain access to sound and oral–verbal communication. However, the electrical stimulus in the cochlea spreads easily and widely, since the perilymph and endolymph (i.e., intracochlear fluids) are essentially electrolytes, leading to an inability to focus stimulation to discrete portions of the auditory nerve, which blurs the neural signal. Here, we characterize the complex transimpedances of human cadaveric cochleas to investigate how electrical stimulus spread is distributed from 10 Hz to 100 kHz. By using electrochemical impedance spectroscopy (EIS), both the resistive and capacitive elements of human cochleas are measured and modeled with an electrical circuit model, identifying spread-induced and spread-independent impedance components. Based on this electrical circuit model, we implement a Laplace transform to simulate the theoretical shapes of the spread signals. The model is validated by experimentally applying the simulated stimulus as a real stimulus to the cochlea and measuring the shapes of the spread signals, with relative errors of <0.6% from the model. Based on this model, we show the relationship between stimulus pulse duration and electrical stimulus spread. This EIS technique to characterize the transimpedances of human cochleas provides a new way to predict the spread signal under an arbitrary electrical stimulus, thus providing preliminary guidance to the design of CI stimuli for different CI users and coding strategies.

Impedance measures for a better understanding of the electrical stimulation of the inner ear

Abstract:The performance of cochlear implant (CI) listeners is limited by several factors among which the lack of spatial selectivity of the electrical stimulation. Recently, many studies have explored the use of multipolar strategies where several electrodes are stimulated simultaneously to focus the electrical field in a restricted region of the cochlea. Objective: These strategies are based on several assumptions concerning the electrical properties of the inner ear that need validation. The first, often implicit, assumption is that the medium is purely resistive and that the current waveforms produced by several electrodes sum linearly. The second assumption relates to the estimation of the contribution of each electrode to the overall electrical field. These individual contributions are usually obtained by stimulating each electrode and measuring the resulting voltage with the other inactive electrodes (i.e. the impedance matrix). However, measuring the voltage on active electrodes (i.e. the diagonal of the matrix) is not straightforward because of the polarization of the electrode-fluid interface. In existing multipolar strategies, the diagonal terms of the matrix are therefore inferred using linear extrapolation from measurements made at neighboring electrodes. Approach: In experiment 1, several impedance measurements were carried out in vitro and in eight CI users using sinusoidal and pulsatile waveforms to test the resistivity and linearity hypotheses. In experiment 2, we used an equivalent electrical model including a constant phase element in order to isolate the polarization component of the contact impedance. Main results: In experiment 1, high-resolution voltage recordings (1.1 MHz sampling) showed the resistivity assumption to be valid at 46.4 kHz, the highest frequency tested. However, these measures also revealed the presence of parasitic capacitive effects at high frequency that could be deleterious to multipolar strategies. Experiment 2 showed that the electrical model provides a better account of the high-resolution impedance measurements than previous approaches in the CI field that used resistor-capacitance circuit models. Significance: These results validate the main hypotheses underlying the use of multipolar stimulation but also suggest possible modifications to their implementation, including the use of an impedance model and the modification of the electrical pulse waveform.
Correlation of early auditory potentials and intracochlear electrode insertion properties: an animal model featuring near real-time monitoring

Abstract:Objective: The goal of this work was to assess electrophysiologic response changes to acoustic stimuli as an intracochlear electrode impacted cochlear structures in an animal model of hearing preservation cochlear implantation. The ultimate goal is to develop efficient procedures for assessing the status of cochlear physiology for intraoperative use. Methods: Sixteen gerbils and 18 ears were tested. A rigid electrode was inserted through a basal turn cochleostomy and directed toward the basilar membrane/osseous spiral lamina complex. We recorded acoustically evoked early auditory potentials including cochlear microphonics (CMs) and compound action potentials (CAPs) to a short stimulation sequence consisting of one stimulus frequency and intensity as the electrode was advanced. A microendoscope was used to visualize the electrode insertion progress and to identify the site of electrode impact. After each experiment, the site of intracochlear trauma was confirmed using whole mount preparations. Results: Electrophysiologic changes correlated well with the degree and location of trauma. We observed four distinct patterns. In addition, the endoscope in conjunction with the short recording sequence allowed for the detection of response changes that were reversible when the electrode was retracted. These cases were associated with less than full-thickness damage on histology. Conclusion: The short recording sequence to obtain acoustically evoked intracochlear potentials and the microendoscope allowed us to detect various levels of cochlear trauma including minor and reversible damage. Recordings of this type are potentially available using current implant technology. Future improvements in the measurements can be expected to improve the efficiency of the recording paradigm to produce a clinically useful tool.

Abstract:Postoperative inflammation and the formation of fibrotic tissue around the intracochlear electrode array are often held responsible for negative outcomes in cochlear implant recipients. Here we test the effectiveness of intracochlear delivery of dexamethasone via a drug-eluting electrode array in reducing fibrotic tissue formation, assessed via measurement of both monopolar and four-point electrode impedance. Adult guinea pigs were bilaterally implanted with a dexamethasone-eluting array (left ear) and a standard non-eluting array (right ear). Arrays were electrically stimulated daily for 4 weeks, commencing 1 week after implantation, and impedance measured both before and after stimulation. Histological assessment of the tissue was made at the end of the 5-week period. The dexamethasone-eluting array did not reduce monopolar (MP1 + 2) electrode impedance over the course of 5 weeks, and no significant difference was observed in fibrotic tissue, new bone growth, or spiral ganglion neuron density between array types. However, four-point impedance, which provides an indication of the local environment at the neural-tissue interface, was significantly lower in the presence of dexamethasone. A strong relationship was seen between four-point and monopolar impedance for individual electrode arrays, with the exception of the standard array after daily electrical stimulation. This group instead showed a significant correlation between the final four-point impedance measure and percentage of fibrous tissue and new bone growth. In conclusion, this study demonstrated that dexamethasone influences four-point electrode impedance as well as the relationship between fibrotic tissue and impedance, and that both outcomes are shaped by daily electrical stimulation. These results suggest a change occurs at the local tissue-electrode interface in the presence of sustained, intracochlear release of dexamethasone.
Tissue-Engineered Cochlear Fibrosis Model Links Complex Impedance to Fibrosis Formation for Cochlear Implant Patients

Abstract:Cochlear implants are a life-changing technology for those with severe sensorineural hearing loss, partially restoring hearing through direct electrical stimulation of the auditory nerve. However, they are known to elicit an immune response resulting in fibrotic tissue formation in the cochlea that is linked to residual hearing loss and suboptimal outcomes. Intracochlear fibrosis is difficult to track without postmortem histology, and no specific electrical marker for fibrosis exists. In this study, a tissue-engineered model of cochlear fibrosis is developed following implant placement to examine the electrical characteristics associated with fibrotic tissue formation around electrodes. The model is characterized using electrochemical impedance spectroscopy and an increase in the resistance and a decrease in capacitance of the tissue using a representative circuit are found. This result informs a new marker of fibrosis progression over time that is extractable from voltage waveform responses, which can be directly measured in cochlear implant patients. This marker is tested in a small sample size of recently implanted cochlear implant patients, showing a significant increase over two postoperative timepoints. Using this system, complex impedance is demonstrated as a marker of fibrosis progression that is directly measurable from cochlear implants to enable real-time tracking of fibrosis formation in patients, creating opportunities for earlier treatment intervention to improve cochlear implant efficacy.

Abstract:In this work, cell adhesion and electroporation effects have been studied through impedance measurements. Chinese Hamster Ovary (CHO-K1) cells have been plated and grown adherent to multi-electrode arrays and then stimulated to obtain the electroporation. We used Electrochemical Impedance Spectroscopy (EIS) measurements to analyze the impedance variation that occurs with cell adhesion and, consequently, to obtain the intrinsic electrical parameters of the electrode-cell interface through a lumped parameter model. The whole model allows to estimate the correction factor β and the sealing resistance Rseal, both dependent on the cell coverage and adhesion. The electroporation has been performed after cell adhesion, using a custom stimulation bench, able to evaluate the cell-electrode coupling and to stimulate an individually addressable site. By performing EIS measurements, before and after the electroporation, cell status, adhesion and membrane resistance can be detected. From measurements, an impedance decrease as a function of pulse amplitude can be noted. The proposed equivalent electrical model allows to evaluate the variation in membrane conductance due to pores formation. Moreover, average pore radius between 0.5 nm and 2.2 nm, increasing with pulse amplitude, was estimated.

A simple electrical circuit model for impedance spectroscopy with cochlear implant electrodes

Abstract:Although cochlear implants are an established method of restoring hearing, they can have limitations such as increasing current spread and decreasing frequency resolution due to tissue growth around the electrode array. Impedance measurements in cochlear implants have become a versatile tool for intra- and post-operative diagnosis of cochlear implant state. However, most clinical devices use current pulse stimulation already available in the implants and analyze the voltage response in the time-domain and spread along the cochlea. To use the full potential of impedance spectroscopy in differentiating cell types, measurement over an extended frequency range is required. This study presents a simple electrical equivalent circuit for impedance spectroscopy with cochlear implants in a 2-pole configuration. The electrical equivalent circuit describes the electrical properties of the cochlear implant electrode and its electrochemical behavior at the electrode-electrolyte interface by comparing two non-linear bilayer models, Cole-Cole and Schwan-Faraday. The model is validated for four cochlear implant electrodes from four different manufacturers (MED-EL FlexSoft, AB HiFocus SlimJ, Oticon EVO, Cochlear Nucleus CI622) characterized by impedance spectroscopy between 5 Hz and 13 MHz. In the future, this electrical equivalent circuit may help to extract parameters for differentiating cell types around the cochlear implant electrode from an impedance spectroscopic measurement.

Abstract:Cochlear implantation (CI) is now widely used to provide auditory rehabilitation to individuals having severe to profound sensorineural hearing loss (SNHL). However, CI can lead to electrode insertion trauma (EIT) that can cause damage to sensory cells in the inner ear resulting in loss of residual hearing. Even with soft surgical techniques where there is minimal macroscopic damage, we can still observe the generation of molecular events that may initiate programmed cell death via various mechanisms such as oxidative stress, the release of pro-inflammatory cytokines, and activation of the caspase pathway. In addition, individuals with CI may be exposed to noise trauma (NT) due to occupation and leisure activities that may affect their hearing ability. Recently, there has been an increased interest in the auditory community to determine the efficacy of drug-eluting electrodes for the protection of residual hearing. The objective of this study is to determine the effect of NT on implanted cochlea as well as the otoprotective efficacy of dexamethasone eluting electrode to implanted cochlea exposed to NT in a guinea pig model of CI. Animals were divided into five groups: EIT with dexamethasone eluting electrode exposed to NT; EIT exposed to NT; NT only; EIT only and naïve animals (control group). The hearing thresholds were determined by auditory brainstem recordings (ABRs). The cochlea was harvested and analyzed for transcript levels of inflammation, apoptosis and fibrosis genes. We observed that threshold shifts were significantly higher in EIT, NT or EIT + NT groups compared to naive animals at all the tested frequencies. The dexamethasone eluting electrode led to a significant decrease in hearing threshold shifts in implanted animals exposed to NT. Proapoptotic tumor necrosis factor-α [TNF-α, TNF-α receptor 1a (TNFαR1a)] and pro-fibrotic transforming growth factor β1 (TGFβ) genes were more than two-fold up-regulated following EIT and EIT + NT compared to the control group. The use of dexamethasone releasing electrode significantly decreased the transcript levels of pro-apoptotic and pro-fibrotic genes. The dexamethasone releasing electrode has shown promising results for hearing protection in implanted animals exposed to NT. The results of this study suggest that dexamethasone releasing electrode holds great potential in developing effective treatment modalities for NT in the implanted cochlea.

Abstract:Objective. To evaluate the electrochemical properties, biological response, and surface characterization of an electrodeposited Platinum-Iridium (Pt-Ir) electrode coating on cochlear implants subjected to chronic stimulation in vivo. Approach. Electrochemical impedance spectroscopy (EIS), charge storage capacity (CSC), charge injection limit (CIL), and voltage transient (VT) impedance were measured bench-top before and after implant and in vivo. Coated Pt-Ir and uncoated Pt electrode arrays were implanted into cochlea of normal hearing rats and stimulated for ~4 h d, 5 d week−1 for 5 weeks at levels within the normal clinical range. Neural function was monitored using electrically-evoked auditory brainstem responses. After explant, the electrode surfaces were assessed, and cochleae examined histologically. Main results. When measured on bench-top before and after stimulation, Pt-Ir coated electrodes had significantly lower VT impedance (p < 0.001) and significantly higher CSC (p < 0.001) and CIL (p < 0.001) compared to uncoated Pt electrodes. In vivo, the CSC and CIL of Pt-Ir were significantly higher than Pt throughout the implantation period (p= 0.047 and p< 0.001, respectively); however, the VT impedance (p= 0.3) was not. There was no difference in foreign body response between material cohorts, although cochleae implanted with coated electrodes contained small deposits of Pt-Ir. There was no evidence of increased neural loss or loss of neural function in either group. Surface examination revealed no Pt corrosion on any electrodes. Significance. Electrodeposited Pt-Ir electrodes demonstrated significant improvements in electrochemical performance on the bench-top and in vivo compared to uncoated Pt. Neural function and tissue response to Pt-Ir electrodes were not different from uncoated Pt, despite small deposits of Pt-Ir in the tissue capsule. Electrodeposited Pt-Ir coatings offer promise as an improved electrode coating for active neural prostheses.

Abstract:Background: This study examined electrical bioimpedance as a biomarker for intracochlear tissue changes after cochlear implant surgery, comparing monopolar, three-point, and four-point impedance measurements over time and evaluating different measurement systems and approaches. Methods: Impedance measurements were obtained from 21 participants during surgery and at four postoperative stages. Monopolar impedances were recorded using the Bionic Ear Data Collection System (BEDCS) and the Active Insertion Monitoring (AIM) system. Three- and four-point impedances were recorded directly using BEDCS, and indirect three-point impedances were additionally derived from Electrical Field Imaging matrices recorded using BEDCS or AIM. Results: There was an 11% relative error between monopolar measurements from BEDCS and AIM and a 25% discrepancy between direct and indirect three-point measurements. Despite this, direct and indirect measurements from both systems were useful for tracking postoperative impedance shifts. Three- and four-point measurements showed a strong relationship both during and after surgery. Our results suggest that three- and four-point measurements are more specific than monopolar impedances in capturing localized tissue changes. Conclusions: Three- and four-point impedance measurements are potential markers of intracochlear tissue changes over time. While direct three-point impedance measurements offer higher accuracy, indirect measurements provide a feasible alternative for monitoring intracochlear changes in clinical settings lacking the option of direct measurements.

Abstract:Objectives: The growing adoption of cochlear implants (CIs) necessitates understanding the factors influencing long-term performance and improved outcomes. This work investigated the long-term effect of early activation of CIs on electrode impedance in a large sample of CI users at different time points. Methods: A retrospective study on 915 ears from CI patients who were implanted between 2015 and 2020. According to their CI audio processor activation time, the patients were categorized into early activation (activated 1 day after surgery, n = 481) and classical activation (activated 4 weeks after surgery, n = 434) groups. Then, the impact of the activation times on the electrode impedance values, along the electrode array contacts, at different time points up to two years was studied and analyzed. Results: The early activation group demonstrated lower impedance values across all the electrode array sections compared to the classical activation at 1 month, 1 year, and 2 years post-implantation. At 1 month, early activation was associated with a reduction of 0.34 kΩ, 0.46 kΩ, and 0.37 kΩ in the apical, middle, and basal sections, respectively. These differences persisted at subsequent intervals. Conclusions: Early activation leads to sustained reductions in the electrode impedance compared to classical activation (CA), suggesting that earlier activation might positively affect long-term CI outcomes.

Abstract:Cochlear implants (CI) represent incredible devices that restore hearing perception for those with moderate to profound sensorineural hearing loss. However, the ability of a CI to restore complex auditory function is limited by the number of perceptually independent spectral channels provided. A major contributor to this limitation is the physical gap between the CI electrodes and the target spiral ganglion neurons (SGNs). In order for CI electrodes to stimulate SGNs more precisely, and thus better approximate natural hearing, new methodologies need to be developed to decrease this gap, (i.e., transitioning CIs from a far-field to near-field device). In this review, strategies aimed at improving the neural-electrode interface are discussed in terms of the magnitude of impact they could have and the work needed to implement them. Ongoing research suggests current clinical efforts to limit the CI-related immune response holds great potential for improving device performance. This could eradicate the dense, fibrous capsule surrounding the electrode and enhance preservation of natural cochlear architecture, including SGNs. In the long term, however, optimized future devices will likely need to induce and guide the outgrowth of the peripheral process of SGNs to be in closer proximity to the CI electrode in order to better approximate natural hearing. This research is in its infancy; it remains to be seen which strategies (surface patterning, small molecule release, hydrogel coating, etc.) will be enable this approach. Additionally, these efforts aimed at optimizing CI function will likely translate to other neural prostheses, which face similar issues.

Reducing the foreign body response on human cochlear implants and their materials in vivo with photografted zwitterionic hydrogel coatings

Abstract:The foreign body response to implanted materials often complicates the functionality of sensitive biomedical devices. For cochlear implants, this response can reduce device performance, battery life and preservation of residual acoustic hearing. As a permanent and passive solution to the foreign body response, this work investigates ultra-low-fouling poly(carboxybetaine methacrylate) (pCBMA) thin film hydrogels that are simultaneously photo-grafted and photo-polymerized onto polydimethylsiloxane (PDMS). The cellular anti-fouling properties of these coatings are robustly maintained even after six-months subcutaneous incubation and over a broad range of cross-linker compositions. On pCBMA-coated PDMS sheets implanted subcutaneously, capsule thickness and inflammation are reduced significantly in comparison to uncoated PDMS or coatings of polymerized poly(ethylene glycol dimethacrylate) (pPEGDMA). Further, capsule thickness is reduced over a wide range of pCBMA cross-linker compositions. On cochlear implant electrode arrays implanted subcutaneously for one year, the coating bridges over the exposed platinum electrodes and dramatically reduces the capsule thickness over the entire implant. Coated cochlear implant electrode arrays could therefore lead to persistent improved performance and reduced risk of residual hearing loss. More generally, the in vivo anti-fibrotic properties of pCBMA coatings also demonstrate potential to mitigate the fibrotic response on a variety of sensing/stimulating implants. STATEMENT OF SIGNIFICANCE: This article presents, for the first time, evidence of the in vivo anti-fibrotic effect of zwitterionic hydrogel thin films photografted to polydimethylsiloxane (PDMS) and human cochlear implant arrays. The hydrogel coating shows no evidence of degradation or loss of function after long-term implantation. The coating process enables full coverage of the electrode array. The coating reduces fibrotic capsule thickness 50-70% over a broad range of cross-link densities for implantations from six weeks to one year.

Abstract:she admitted that she swallowed large quantities of the laxative Dorbanex in the belief that she had severe constipation. Witnesses had noted yellow streaks around the mouth, which came from this drug. She was advised to stop taking Dorbanex and to rely on another laxative. This she agreed to do after many exhortations. Eventually, after some months, the blue colour of the skin faded and its normal colour was restored.
The Intraoperative Application of Neural Response Telemetry with the Nucleus CI24M Cochlear Implant

Abstract:OBJECTIVE:To test device integrity and objective auditory reaction for cochlear implant patients intraoperatively.METHODS:Our protocol for intraoperative testing of the implant device includes device electrode impedance test and neural response telemetry (NRT), which measures the electrically evoked auditory nerve compound action potentials (ECAP).RESULTS:We completed electrode integrity tests and NRT intraoperatively on 40 patients with the Nucleus CI24M cochlear implant. We have measured normal implant function on all 40 patients and obtained ECAP results from 39 patients. Out of 33 patients with normal inner ear, typical ECAPs were recorded in 195 electrodes in all 198 testing electrodes(98.5%). In 7 patients with inner ear Mondini dysphasia, affirmative ECAP waveforms were recorded in 26 electrodes in all 42 testing electrodes. The basal electrode ECAP threshold was higher than that of the epical one.CONCLUSIONS:The intraoperative measurements of device function and NRT are quite simple, quick and with high accuracy. Inner ear malformation is an important factor that may affect NRT results.

Abstract:Abstract Background Cochlear‐implant electrode arrays (EAs) are currently inserted with limited feedback, and impedance sensing has recently shown promise for EA localisation. Methods We investigate the use of impedance sensing to infer the progression of an EA during insertion. Results We show that the access resistance component of bipolar impedance sensing can detect when a straight EA reaches key anatomical locations in a plastic cochlea and when each electrode contact enters/exits the cochlea. We also demonstrate that dual‐sided electrode contacts can provide useful proximity information and show the real‐time relationship between impedance and wall proximity in a cadaveric cochlea for the first time. Conclusion The access resistance component of bipolar impedance sensing has high potential for estimating positioning information of EAs relative to anatomy during insertion. Main limitations of this work include using saline as a surrogate for human perilymph in ex vivo models and using only one type of EA.
