Abstract:Power distribution of progressive power lenses provides usable regions based on power distribution analysis. However, recent studies demonstrated that these regions are not always used for certain tasks as predicted. This work determines the concordance between the actual region of lens use and compares it with the theoretically located regions. The pupil position of 26 subjects was recorded using an eye-tracking system (Tobii-Pro-Glasses 3) at distance and near-reading tasks while wearing a general use progressive power lens. Subjects were asked to read aloud a text showed on a screen placed at 5.25 m and 37 cm while looking though the central and lateral regions of the lens. The pupil position was projected onto the back surface of the lens to obtain the actual region of use for each fixation. Results showed that the actual region of use matched with the theoretically located. On average, the concordance between the actual and theoretical regions of use was 85% for a distance-reading task and 73% for a near-reading task. In conclusion, the proposed method effectively located the actual regions of the lens used, revealing how users' posture affects lens usage. This insight enables the design of more customized progressive lenses based on the areas used during vision-based tasks.

Abstract:Purpose: The aim of this study was to develop a methodology, based on profilometer measurements to assess the optical behaviour of Intraocular Lenses (IOls). The "Modulation Transfer Function through-object" (MTF through-object) based on vergence object displacement was calculated for different pupil sizes and pseudophakic eyes. Tilt and decentration were also analysed in a realistic cornea eye model. Methods: For comparison between the different IOLs, an optical quality criterion based on a minimum value the MTF through-object and the recognition of simulated vision optotypes was introduced. Five IOLs were used in this study: Tecnis Eyhance, Mini Well, Tecnis Symfony, Tecnis Synergy and RayOne EMV. Results: The technique was validated with previous methodologies. A general narrowing of the through-object MTF curve compared to the through-focus MTF curve was shown, resulting in greater distances between near and intermediate points and less depth of field around the far peak. The comparison between the IOLs showed that variations in corneal aberrations, pupil size and decentration caused relevant changes in IOL performance. A decrease of the SA produced a hypermetropic shift of the far focus between + 0.3 D and + 0.4 D. Most of IOLs worsen the optical quality as pupil size increased, even the MTF through-object shape changed. Decentration was an important factor in IOL implantation, causing a significant change in MTF through-object shape in most of IOLs. Conclusions: This study highlights the need to evaluate pre-operative patients for corneal aberrations and pupillary size to have the best optical success after cataract surgery in multifocal or extended depth of focus IOLs. The average age was 52.8 years; the interval from injury to development of carcinoma was 45.8 years. Nine of the 12 carcinomas were in the midthoracic segment. Abstract:Purpose: To assess the power profile and optical performance of 2 extended range-of-vision (ERV) intraocular lenses (IOLs), TECNIS Eyhance (ICB00) and LuxSmart, and compare them with their monofocal counterpart lenses with similar platforms and materials: TECNIS 1-piece (ZCB00) and LuxGood, respectively. Setting: Optics and Optometry Faculties of Complutense University (Madrid) and Universitat Politècnica de Catalunya BarcelonaTech (Terrassa) in Spain. Design: Laboratory investigation on optical bench. Methods: For each design, the power distribution and (fourth and sixth-order) spherical aberration (SA) across the lens aperture were measured as well as the optical performance using modulation transfer function-based metrics with through-focus evaluation. 3 nominal powers (+10.00 diopters [D], +20.00 D, and +30.00 D) and 3 pupil sizes (2.0 mm, 3.0 mm, and 4.5 mm) were considered to assess whether the base power of the lens and pupillary dynamics have an influence on the depth-of-focus extension. Results: TECNIS Eyhance and LuxSmart IOLs had different power and SA profiles, but both designs shared a positive add power in their central region in comparison with their monofocal counterparts. LuxSmart had a greater add power while TECNIS Eyhance showed higher peak optical quality but smaller depth of focus. Conclusions: Differences of focus extension between the 2 ERV IOL designs are related to differences of power and SA profile. The nominal base power of the IOLs has little effect on their optical quality. However, pupil dynamics plays a key role since it determines the effective add power and optical performance of the ERV IOLs.

Abstract:Purpose Lens power profiles can provide valuable insights on the imposed optical defocus and visual experience of contact lens wearers, especially in the context of myopia control. This study measured the refractive power profiles of multifocal soft contact lenses (MFCLs) currently used or that have the potential for use in myopia control using high spatial resolution aberrometry. The instrument's repeatability for determining MFCLs power profiles was also assessed. Method The power profiles of 10 MFCLs of various designs (centre‐distance, centre‐near and extended depth of focus) were measured using the Lambda‐X NIMOEVO, a phase shifting Schlieren‐based device. Power profiles were graphically expressed as measured power at each chord position and the maximum add power was calculated. The repeatability of the NIMOEVO was expressed as the within‐subject standard deviation at each chord position for a subset of five MFCLs. Results The measured distance powers differed from nominal powers for more than half of the MFCLs with a definable distance zone. There were variations in the chord position of the distance and near correction zones, rate of power transitions and calculated maximum add between the MFCLs which did not depend on lens design. For half of the MFCLs, the power profile shape was inconsistent between different nominal back vertex powers of the same design. The repeatability of the NIMOEVO was dependent on the lens design, with designs featuring faster rates of power change exhibiting worse repeatability. Conclusions Significant differences in MFCL power profiles were found which were not adequately represented in labelling. This is likely due to the small number of parameters used to define lens power characteristics. Eye health care practitioners should be aware of potential differences in power profiles between different MFCLs, which will impact the retinal defocus introduced during lens wear and the wearer's visual experience.

Abstract:Objective To investigate the effects of a series of specially designed spectacle lenses on peripheral refraction,peripheral clear vision field and subjective evaluation. Methods Three subjects (4 eyes) aged 22 to 31 years,with a refraction of -2. 0 D were enrolled. They underwent peripheral refraction,peripheral clear vision field measurement and subjective evaluation with 5 consecutive lenses in situ in a randomized manner,including regular aspheric lens,MyoVision lens,Stop lenses with respectively small,medium and large optical zones. Subjective evaluation included the degrees of comfort,clearness and acceptability both at distance and at near. Results All tested eyes had a relatively hyperopic peripheral refraction with any of the five lenses in situ,with the degree of hyperopia increasing towards the periphery. Peripheral refraction is the most hyperopic with regular aspheric lens and Stop lens with large optical zone,followed by Stop lens with medium optical zone. The least peripheral hyperopia was seen in eyes with MyoVision lens and Stop lens with small optical zone. Peripheral clear vision field was largest with regular aspheric lens and Stop lens with large optical zone,moderate with Stop lens with medium optical zone,and smallest with MyoVision lens and Stop lens with small optical zone. Subjective evaluation at distance was the highest with regular aspheric lens, followed by Stop lens with large optical zone and medium optical zone,the lowest with MyoVision lens and Stop lens with small optical zone. Subjective evaluation at near was the highest with regular aspheric lens and Stop lens with large optical zone,followed by Stop lens with medium optical zone and MyoVision lens,the lowest with Stop lens with small optical zone. Conclusions Lenses with different mid-periphery additional designs differed in their effects on peripheral refraction,whose intensity was negatively correlated to adaptability with lenses in situ.
Spatially resolved wavefront aberrations of ophthalmic progressive-power lenses in normal viewing conditions

Abstract:Purpose: To measure the wavefront aberration at different locations in progressive-power lenses (PPL's) isolated and in situ (PPL's plus eye). Methods: A Hartmann-Shack wavefront sensor was used to measure progressive-power lenses and human eyes either independently or in combination. In each selected zone, the lens was placed and tilted accordingly to simulate natural viewing conditions. We measured 21 relevant locations across an isolated PPL (plano lens of power addition of 2 D). In six of the locations, the wavefront aberration of the eye plus PPL were obtained in two ways: (1) by direct measurement of the system and (2) by adding the individual wavefront aberrations of the eye and the lens for each appropriate zone. In every case, we obtained the wavefront aberration as Zernike polynomials expansions, the root mean square error, the point-spread function, and the Strehl ratio. Results: Along the corridor of the PPL, third-order coma and trefoil, and astigmatism were the dominant aberrations. In areas of the PPL outside the corridor, astigmatism increased, whereas other aberrations remained similar to the lens center. Small differences were found between the direct and calculated methods used to obtain the wavefront aberration of the eye with the lens, and the possible sources of errors were discussed. In some lenses zones, the aberrations of the lens may be compensated by the particular aberrations of the eye, yielding improved optical performance over that present in the lens alone. Conclusions: We designed and built a wavefront sensor to perform spatially resolved aberration measurements in ophthalmic lenses, in particular in PPL's, either isolated or in combination with the eye. The aberrations appearing in the PPL were compared with those in normal aged eyes.

Abstract:A clinical investigation was undertaken to evaluate a Chinese population wearing Surevue disposable contact lens wearers. The patients were divided into three groups and fitted respectively with: (1) 8.8-mm base curve lenses; (2) 8.8-mm base curve lenses for 1 month and then switched to 8.4-mm base curve lenses; and (3) one eye was fitted with an 8.8-mm base curve lens, and the other eye was fitted with an 8.4-mm base curve lens. In addition to an initial visit, follow-up visits occurred at 1 week and 1 month. Assessment included lens centration, lens movement, keratometry, and adverse responses. Vision and comfort were evaluated. One hundred fifty-three subjects were enrolled and completed the study. The results indicated that base curve selection should eventually determined by the clinical fit evaluation. The 8.8-mm base curve Surevue lens was the preferred option for most Chinese wearers.

Abstract:The crystalline lens makes an important contribution to the peripheral refraction of the human eye, which may affect the development and progression of myopia. However, little has been known about the peripheral optical features of the crystalline lens and its impacts on the peripheral ocular refraction. This study aims to investigate the relationship between the structural parameters of the crystalline lens and its peripheral power profile over a wide visual field. The peripheral power profile is defined with respect to the entrance and exit pupil centers along the chief rays. Analysis is performed by three-dimensional ray tracing through the gradient refractive index (GRIN) lens models built from measurement data. It has been found that the vergence of the wavefronts at the entrance and the exit pupil centers of the lens show an approximate linear correlation to each other for each field angle. The exponent parameters of the axial refractive index profile and the axial curvature profile, and the asphericity of the posterior lens surface are found to be the most influential parameters in the peripheral power profiles. The study also shows that there can be significantly different, sometimes unrealistic, power profiles in the homogeneous lens model compared with its corresponding GRIN model with the same external geometry. The theoretical findings on the peripheral lens properties provide a new perspective for both wide-field eye modelling and the design of intraocular lenses to achieve normal peripheral vision.

Abstract:Progressive myopia remains one of the urgent problems in modern ophthalmology. Optical methods for controlling progressive myopia suggest the formation of peripheral myopic defocus, which has a stabilizing effect on the axial length of the eye and, as a result, the degree of myopia. One option for optical control of myopia is defocus soft contact lenses. Purpose : to evaluate the growth of axial length and changes in the spherical equivalent of clinical refraction in users of defocus (bifocal) contact lenses and monofocal glasses expressed in diopters, against the background of the correction of progressive myopia with defocus (bifocal) contact lenses and monofocal glasses. Materials and methods. The study involved 256 patients with bilateral mild to moderate and high myopia in the range from (–)0.5 to (–) 7.75 D in spherical equivalent and an annual progression gradient from 0.65 to 1.5 D in the age categories 7–10, 11–14, 15–18 years old. Depending on the degree of myopia and the method of correction, the patients were divided into main and control groups. For correction in the main groups, defocused (with an addition of +4.0 D) soft contact lenses (SCLs) were used. In the control groups monofocal glasses were used. All patients underwent a standard ophthalmological examination. Clinical refraction data and axial length of the eye were used as criteria for evaluating the results. The follow-up period was 3, 6, 12 months. Results . After 12 months of wearing bifocal soft contact lenses, there was a statistically significant slowdown in the growth of the axial length and the spherical equivalent of refraction Higher rates of progression of myopia were in young children (7–10 years). Myopia at the age of 15–18 years is characterized by a decrease in the rate of progression with any type of optical correction and in most cases does not require the appointment of correction with the formation of peripheral myopic defocus. Conclusion. The results obtained indicate the effectiveness of the use of bifocal SCLs in the control of progressive mild, moderate and high myopia. The main stabilization factor is the formation of peripheral myopic defocus on the retina, followed by a slowdown in the growth of the axial length of the eye and the degree of myopia (spherical equivalent of clinical refraction).
Optical characterisation of two novel myopia control spectacle lenses

Abstract:Purpose: To quantify the amount of myopic defocus, contrast modulation and other optical characteristics of two novel spectacle lenses (MiYOSMART by Hoya and Stellest by Essilor) with the inclusion of lenslets in their designs were investigated computationally and experimentally. This paper examined the hypothesis that despite the non-coaxial nature of the optics, image degradation will exist due to the fragmented nature of the base optic when imaging through the lens regions populated by lenslets. Methods: Optical power was evaluated by computing wavefront vergence and curvature from wavefront slope measured with the Optocraft aberrometer within 1.0 and 6.0 mm apertures across MiYOSMART hexagons and Stellest rings. Point-spread functions (PSFs) were computed using physical (wave) optics and geometrical ray optics principles, and compared with experimental measurements using a 4f optical system. Simulated retinal images and modulation transfer functions (MTFs) were computed from PSF-derived optical transfer functions (OTFs). Results: Mean lenslet power in MiYOSMART was +3.95 ± 0.10 D through the hexagons and +6.00 ± 0.15 D in Stellest in rings 1-5 and decreased by 0.42 D/ring reaching 3.50 D in the final one. Stellest lenslets included up to -0.015 microns of primary spherical aberration. PSFs and retinal images revealed simultaneous contributions of the base optic and lenslets. MTFs showed a decrease in contrast at low (1-10 c/deg) spatial frequencies (SFs) comparable to 0.25 D of defocus, and retention of diminished levels of contrast at higher SFs. Conclusions: Varying sagittal power and consistent curvature power across the lenslets is an identifying signature of the novel non-coaxial lens design included in both spectacle lenses. Lenslet array structure itself plays a significant role in determining image characteristics. For both lenses, the blur created by the fragmented base optic contributes to the image quality. The reduced MTFs over a wide range of spatial frequencies result in lowered image contrast.
Ray-Tracing Customization in Myopic and Myopic Astigmatism LASIK Treatments for Low and High Order Aberrations Treatment: 2-Year Visual Function and Psychometric Value Outcomes of a Consecutive Case Series

Abstract:Purpose: The safety and long-term efficacy of automated ray-tracing customized myopic and myopic astigmatic femtosecond laser-assisted LASIK. Methods: This consecutive case series retrospective analysis, of 20 subjects (40 eyes) treated with automated raytracing named Wavelight Plus, to include low and high order aberrations based on a three-dimensional custom virtual eye for each case-calculated from interferometry data-obtained from a single diagnostic device that also provides Hartman-Shack Wavefront and Scheimpflug tomography data. We evaluated before and after the customized LASIK procedure: visual acuity, refractive error, high order aberrations, contrast sensitivity, and psychometric post-operative visual function data. Results: At 24 months, the comparison of the pre-operative to the post-operative refractive and visual function value changes in average were: subjective manifest refraction from -4.38 ± 2.54 diopters (D) (range -9.75 to -1.25 D) to +0.11 ± 0.19 D; subjective manifest refractive astigmatism from -0.76 ± 0.91 D (range -2.75 to 0 D) to -0.13 ± 0.16 D, corneal astigmatism from -1.16 ± 0.64 D (range -0.2 to -2.8) to -0.47 ± 0.11 D. 65% of the eyes studied demonstrated an increase of at least one line of vision, while from the same group 38% demonstrated 2 lines of increase. High order aberrations, contrast sensitivity as well as the subjective psychometric input based on the VFQ-25 questionnaire demonstrated actual improvement. Conclusion: This longer-term follow-up, single-arm retrospective consecutive case series documents LASIK treatment customization that appears to be safe and effective in the correction of myopia and myopic astigmatism. Markedly improved objective and subjective visual function post-operatively, underlying the potential importance of simultaneously attempting to correct high order aberrations and improving the spatial alignment of total, measured human eye optics.
Considerations on the Castrop formula for calculation of intraocular lens power

Abstract:Background To explain the concept of the Castrop lens power calculation formula and show the application and results from a large dataset compared to classical formulae. Methods The Castrop vergence formula is based on a pseudophakic model eye with 4 refractive surfaces. This was compared against the SRKT, Hoffer-Q, Holladay1, simplified Haigis with 1 optimized constant and Haigis formula with 3 optimized constants. A large dataset of preoperative biometric values, lens power data and postoperative refraction data was split into training and test sets. The training data were used for formula constant optimization, and the test data for cross-validation. Constant optimization was performed for all formulae using nonlinear optimization, minimising root mean squared prediction error. Results The constants for all formulae were derived with the Levenberg-Marquardt algorithm. Applying these constants to the test data, the Castrop formula showed a slightly better performance compared to the classical formulae in terms of prediction error and absolute prediction error. Using the Castrop formula, the standard deviation of the prediction error was lowest at 0.45 dpt, and 95% of all eyes in the test data were within the limit of 0.9 dpt of prediction error. Conclusion The calculation concept of the Castrop formula and one potential option for optimization of the 3 Castrop formula constants (C, H, and R) are presented. In a large dataset of 1452 data points the performance of the Castrop formula was slightly superior to the respective results of the classical formulae such as SRKT, Hoffer-Q, Holladay1 or Haigis.

Abstract:Abstract Purpose: Investigating impact of formula constants on predicted refraction and limitations of constant optimization for classical and modern intraocular lens (IOL) power calculation formulae. Setting: Tertiary Care Center. Design: Retrospective single centre consecutive case series. Methods: Analysis based on a dataset of 888 eyes before and after cataract surgery with IOL implantation (Hoya Vivinex). Spherical equivalent refraction predSEQ was predicted using IOLMaster 700 (IOLM) data, IOL power, and formula constants from IOLCon (https://iolcon.org). The formula prediction error (PE) was derived as predSEQ minus achieved spherical equivalent refraction for the SRKT, Hoffer Q, Holladay, Haigis, and Castrop formulae. The gradient of predSEQ (gradSEQ) as a measure for the impact of the constant on refraction was calculated and used for constant optimization. Results: using initial formula constants the mean PE was -0.1782±0.4450/-0.1814±0.4159/-0.1702±0.4207/-0.1211±0.3740/-0.1912±0.3449 dpt for the SRKT/Hoffer Q/Holladay/Haigis/Castrop formula. All gradSEQ (except gradSEQ for the Castrop R) decay with axial length because of interaction with the effective lens position (ELP). Constant optimization for a zero mean PE (SD: 0.4410/0.4307/0.4272/0.3742/0.3436 dpt) results in a change in the PE trend over axial length in all formulae where the constant acts directly on the ELP. Conclusion: With IOL power calculation formulae where the constant(s) act directly on the ELP, a change in the constant(s) always changes the trend of the PE according to gradSEQ. Formulae where at least one constant does not act on the ELP have more flexibility to zero the mean or median PE without coupling to a PE trend error over axial length.

The Effect of Corneal Power Distribution on Axial Elongation in Children Using Three Different Orthokeratology Lens Designs

Abstract:SIGNIFICANCE:This study will enhance our understanding of the effects of orthokeratology lens design on corneal profile, the results of which may be useful in developing future orthokeratology lens designs. PURPOSE:This study aimed to evaluate the effect of lens design on corneal power distribution after orthokeratology using mathematical methods. METHODS:Sixty-five subjects were enrolled in this prospective study and assigned to four groups: Euclid with 6.2-mm back optic zone diameter (aged <14 years), Euclid with 6.2-mm back optic zone diameter (aged ≥14 years), double tear reservoir lens with 5.0-mm back optic zone diameter, and double tear reservoir lens with 6.0-mm back optic zone diameter. Manifest refraction and corneal topography were checked at baseline and 1 day, 1 week, 2 weeks, and 1 month after lens wear. Relative corneal refractive power change was calculated by a polynomial function and a monomial function. The maximum relative corneal refractive power change (Ymax) and the corresponding distance from the corneal center (Xmax) were analyzed. Relative corneal refractive power change over time and between groups was compared using repeated-measures analysis of variance. RESULTS:Refractive reduction and central corneal flattening were seen at all follow-up visits after orthokeratology lens wear, being fastest in the 5.0-mm back optic zone diameter group (P < .001). The cornea steepened in an aspheric way toward the midperiphery and peaked at approximately 2 to 3 mm off the apex. Overall, Ymax was not different among the four groups, but Xmax was smallest in the 5.0-mm back optic zone diameter group (P < .001). At 1/2 Xmax, relative corneal refractive power change of the 5.0-mm back optic zone diameter design was significantly higher than that of the other three groups (P < .001). The power exponent of the monomial of the 5.0-mm back optic zone diameter design was greater than that of the other three groups (P < .001). CONCLUSIONS:An orthokeratology lens design with smaller back optic zone diameter might yield a faster myopic reduction and a smaller aspheric treatment zone.

Abstract:Abstract The process of lens shape change in the eye to alter focussing (accommodation) is still not fully understood. Modelling approaches have been used to complement experimental findings in order to determine how constituents in the accommodative process influence the shape change of the lens. An unexplored factor in modelling is the role of the modelling software on the results of simulated shape change. Finite element models were constructed in both Abaqus and Ansys software using biological parameters from measurements of shape and refractive index of two 35-year-old lenses. The effect of zonular insertion on simulated shape change was tested on both 35-year-old lens models and with both types of software. Comparative analysis of shape change, optical power, and stress distributions showed that lens shape and zonular insertion positions affect the results of simulated shape change and that Abaqus and Ansys show differences in their respective models. The effect of the software package used needs to be taken into account when constructing finite element models and deriving conclusions.

Abstract:The spatially resolved wavefront aberrations of four types of ophthalmic lens are measured with a custom-built apparatus based on a Hartmann-Shack wavefront sensor and specially designed positioning stage. The wavefront aberrations of the progressive addition lenses (Pairs) are compared. The results show that the distribution depends much on the design philosophy, although the average values of root mean square in the entire measurement areas have no significant difference. It is feasible to evaluate the optical performance through the wavefront analysis of PALs, but how to meet the customized visual needs of patients and how to minimize the unwanted aberrations in some special zones are important points that should be taken into account.
