Abstract:The deinstitutionalization policy sought to replace institutional care for populations in need of care and control with prosocial community-based alternatives. U.S. institutional populations, however, have increased since the policy’s inception by 205%. As implemented, with the assistance of advocacy and cost-cutting factions, it has succeeded only in enabling the divestiture of state responsibility for target groups. It sought to prevent unnecessary admission and retention in institutions. As implemented, deinstitutionalization initiated a process that involved a societal shift in the type of institutions and institutional alternatives used to house its target groups, often referred to as trans-institutionalization. For many in need of institutional placements, it has succeeded in preventing all admissions, expanding admissions for others. In seeking to develop community alternatives for housing, treating, and habilitating or rehabilitating its target groups, it has succeeded in establishing a variety of alternative living arrangements and showcase and model programs illustrating what can be done; yet, it has failed to deliver on investments in such programs to serve the majority of its target groups. It has resulted in the abandonment of substantial numbers to homelessness. It has been documented, from political, economic, legal, and social perspectives, how this policy has affected the care and control of populations such as older adults, children, people with mental illness or developmental disabilities, people under correctional-system supervision, and, more recently, individuals without a home. Suggestions for a truer implementation of deinstitutionalization’s initial aspirations are available.

Abstract:The current generation of IP, version 4 (IPv4), has been in use for more than 20 years, since its inception in 1980 and has supported the Internet's rapid growth during that time. IPv4 has proven to be robust, easily implemented and interoperable. This is a tribute to its initial design. However, the current Internet has grown much bigger than was anticipated. There are several problems such as impending exhaustion of the IPv4 address space, configuration complexities, and poor security at the IP level and inadequate QoS support for real-time delivery of data. To address these and other concerns, the Internet engineering task force (IETF) has developed a suite of protocols and standards known as IP version 6 (IPv6). The new features introduced such as auto-address configuration, end to end connectivity, mandatory support for security and mobility pose a great challenge on security for future networks based on IPv6. This paper identifies and examines security threats relating to the new features introduced in IPv6.

Abstract:Gauge theories on q-deformed spaces are constructed using covariant derivatives. For this purpose a ''vielbein'' is introduced, which transforms under gauge transformations. The non-Abelian case is treated by establishing a connection to gauge theories on commutative spaces, i.e. by a Seiberg-Witten map. As an example we consider the Manin plane. Remarks are made concerning the relation between covariant coordinates and covariant derivatives.

Abstract:The concept of deinstitutionalization started in the 1960s in the US to describe closing down or reducing the number of beds in mental hospitals. The same process has been going on in many countries but with different names and in various forms. In Europe, countries like Italy prescribed by law an immediate ban on admitting patients to mental hospitals while in some other European countries psychiatric care was reorganized into a sectorized psychiatry characterized by open psychiatric care. This sectorization has not been studied to the same extent as the radical closures of mental hospitals, even though it entailed major changes in the organization of care. The deinstitutionalization in Sweden is connected to the sectorization of psychiatric care, a protracted process taking years to implement.

Abstract:Institutionalization of people living with mental illness has evolved over the years, especially in the 19th and early 20th century. This has created over crowdedness in various psychiatric institutions, specifically in low and-middle-income countries, including Ghana. The objective of this study is to use Bronfenbrenner's Social-Ecological Model (1979) to propose a process for deinstitutionalizing psychiatric services in Ghana while supporting a transition to community-based mental health care. Using the concept of Bronfenbrenner's Social-Ecological Model, this article explores some benefits and difficulties with the concept of deinstitutionalization with regards to mental health care services in Ghana. Individuals living with mental illness will be more comfortable in various communities in Ghana where education on stigmatization and discrimination is heightened to subsequently prevent it. Education and intervention policies are required to intensify the campaign to win the support of all people in the community. Residents of Ghana should learn to say no to stigma and discrimination among people living with mental illness.

A Paradigm Shift on Deinstitutionalization and Dementia Care: A Narrative Review

Abstract:This narrative explores the impact of deinstitutionalization policies on the quality of life and care outcomes for individuals with Alzheimer's disease and related dementias. We offer a historical perspective on these policies, their implications on dementia care, and the barriers to deinstitutionalization. The potential benefits of deinstitutionalization, such as improved quality of life and access to community-based support and services, are highlighted. Challenges and controversies surrounding safety, caregiver burden, and resource allocation are also examined. Ethical considerations related to the autonomy and decision-making capacity of people living with dementia are discussed. We present best practices and innovative models in dementia care that balance deinstitutionalization with appropriate care. We further put forth recommendations for future research and policy development in dementia care and deinstitutionalization, emphasizing the need for a balanced approach that respects the autonomy and preferences of people living with dementia while ensuring their safety and well-being.

Abstract:In many European countries, deinstitutionalisation has been an ongoing process over the last few decades. Mental health organisations were transformed to provide support in a more integrated and comprehensive manner, preferably in their own homes in the community. Yet, despite the welcome aspiration of community integration for all, people with complex mental health problems (also termed severe mental illness) have continued to require high levels of support, in inpatient settings and in the community. This group's needs make them highly dependent on their caregivers. The attitudes, knowledge and skills of the staff providing treatment and support is crucial to their recovery. Rehabilitation programmes provide a much-needed framework to guide practitioners and help them organise and focus their recovery-oriented approach. In this editorial, we will provide a non-exhaustive overview of such rehabilitation programmes and interventions to illuminate the wide scope and practical usability of these interventions for this group of people with complex mental health problems.
Old familiar faces: some aspects of the asylum era in Britain, Part II

Abstract:The paper continues a review of clinical conditions and dilemmas which featured in Asylum practice during the years 1850-1950 in Britain. The rise and fall of 'Physiognomy' as a diagnostic aid and as an indicator of quality of asylum care is outlined. The problem of incontinence and the use of the ice cold douche as a therapeutic intervention in this and other forms of 'indolence or perversion' is reviewed from the writings of those who expressed opinions of these subjects at the time. Likewise, Destructive Behaviour, which was dealt with by means of 'strong clothes', presented a familiar problem. As in Part I of this review, the author concludes with a discussion of the causes and consequences of the changing faces of conditions seen in psychiatric hospitals. Viewed from the present, the changing nature of asylum insanity is apt to be forgotten and adverse judgment is apt to be applied to past practices.

Abstract:Die Studie trägt Entwicklungen und lokale Modelle psychiatrischer Rehabilitation der DDR und der UdSSR zusammen. Nach dem Zweiten Weltkrieg konzentrierte sich diese auf die Krankenhäuser selbst. Als wichtige Anregungen erwiesen sich in der DDR die Rodewischer und die Brandenburger Thesen sowie die "Konzeption zur Verbesserung der Betreuung psychisch Kranker nach 1980". Die Durchsicht der DDR-Primärliteratur zeigt, dass oft sowjetische Konzepte rezipiert worden sind. Hier wie dort sah man Etappen-, Kontinuitäts- und Sektorisierungsprinzipien als Voraussetzung für den Erfolg. Übergangseinrichtungen, wie psychoneurologische Dispensaires, arbeitstherapeutische/geschützte Werkstätten, befürsorgte Wohnmöglichkeiten, therapeutische Klubs, Tages- und Nachtkliniken, wurden geschaffen. Ausgelagerte Arbeitstherapie sollte den Übergang in die Arbeitswelt erleichtern. So wurde in der DDR während der 1970er- und 1980er-Jahre ein hoher Beschäftigungsgrad psychisch Kranker in der Wirtschaft erreicht. The study describes developments and local models of psychiatric rehabilitation in the GDR and the USSR. After the Second World War patient care was focused on hospitals. The Rodewisch Theses and the Brandenburg Theses as well as the "Conception to improve the patient care for mentally ill people after 1980" were important suggestions. The GDR primary literature review shows that soviet concepts were often received. The principles of stages, continuity and sectorization set the stage for success here and there. Transition units such as psychoneurological dispensaries, the occupational/protected workshops and assisted residences, therapeutic clubs, day and night hospitals were created. The occupational therapy was intended to facilitate the transition into the normal working. As a consequence, during the 1970s and 1980s in the GDR a high level of employment of mentally ill people in the economy could be achieved. Transcultural Psychiatry: Cultural Difference, Universalism and Social Psychiatry in the Age of Decolonisation

Abstract:In the mid-twentieth century, in the aftermath of WWII and the Nazi atrocities and in the midst of decolonisation, a new discipline of transcultural psychiatry was being established and institutionalised. This was part and parcel of a global political project in the course of which Western psychiatry attempted to leave behind its colonial legacies and entanglements, and lay the foundation for a more inclusive, egalitarian communication between Western and non-Western concepts of mental illness and healing. In this period, the infrastructure of post-colonial global and transcultural psychiatry was set up, and leading psychiatric figures across the world embarked on identifying, debating and sometimes critiquing the universal psychological characteristics and psychopathological mechanisms supposedly shared among all cultures and civilisations. The article will explore how this psychiatric, social and cultural search for a new definition of 'common humanity' was influenced and shaped by the concurrent global rise of social psychiatry. In the early phases of transcultural psychiatry, a large number of psychiatrists were very keen to determine how cultural and social environments shaped the basic traits of human psychology, and 'psy' practitioners and anthropologist from all over the world sought to re-define the relationship between culture, race and individual psyche. Most of them worked within the universalist framework, which posited that cultural differences merely formed a veneer of symptoms and expressions while the universal core of mental illness remained the same across all cultures. The article will argue that, even in this context, which explicitly challenged the hierarchical and racist paradigms of colonial psychiatry, the founding generations of transcultural psychiatrists from Western Europe and North America tended to conceive of broader environmental determinants of mental health and pathology in the decolonising world in fairly reductionist terms-focusing almost exclusively on 'cultural difference' and cultural, racial and ethnic 'traditions', essentialising and reifying them in the process, and failing to establish some common sociological or economic categories of analysis of Western and non-Western 'mentalities'. On the other hand, it was African and Asian psychiatrists as well as Marxist psychiatrists from Eastern Europe who insisted on applying those broader social psychiatry concepts-such as social class, occupation, socio-economic change, political and group pressures and relations etc.-which were quickly becoming central to mental health research in the West but were largely missing from Western psychiatrists' engagement with the decolonising world. In this way, some of the leading non-Western psychiatrists relied on social psychiatry to establish the limits of psychiatric universalism, and challenge some of its Eurocentric and essentialising tendencies. Even though they still subscribed to the predominant universalist framework, these practitioners invoked social psychiatry to draw attention to universalism's internal incoherence, and sought to revise the lingering evolutionary thinking in transcultural psychiatry. They also contributed to re-imagining cross-cultural encounters and exchanges as potentially creative and progressive (whereas early Western transcultural psychiatry primarily viewed the cross-cultural through the prism of pathogenic and traumatic 'cultural clash'). Therefore, the article will explore the complex politics of the shifting and overlapping definitions of 'social' and 'cultural' factors in mid-twentieth century transcultural psychiatry, and aims to recover the revolutionary voices of non-Western psychiatrists and their contributions to the global re-drawing of the boundaries of humanity in the second half of the twentieth century.
The closing of forensic psychiatric hospitals in Italy: Determinants, current status and future perspectives. A scoping review

Abstract:Introduction: Italy is the only country in the world to have closed forensic psychiatric hospitals and converted to fully-residential services. The international interest around this reform has not been matched by research. This scoping review aims to report the determinants of the reform, the most updated information on how the system operates, its benefits and its challenges. We further aim to discuss the implications for policy, research and practice. Methods: 1. Selection of relevant sources through electronic search on four databases, Google, relevant printed materials and personal communication with practitioners currently working in REMS. 2. Study quality monitoring. 3. Data extraction onto NVivo 4. Data synthesis through content analysis. Results: 43 papers were selected for inclusion in our review. Two main themes were identified: 1. Historical chronology of the closure of forensic psychiatric hospitals; 2. The current model of residential forensic psychiatric care. Conclusions: The closing down of Italian forensic psychiatric hospitals represented a fundamental step for human rights. Further work is required to improve the current service, including potential reforming of the penal code, improved referral/admission processes and consistent monitoring to reduce service inequality across regions. Further research is crucial to test the effectiveness of the Italian model of care against traditional ones.
The mentally ill and social exclusion: a critical examination of the use of seclusion from the patient's perspective

Abstract:Nurses in psychiatric settings have an important role to play in the application of seclusion, a measure that continues to be a frequently used intervention for the management of disturbing patient behaviours. Albeit a controversial measure, isolating patients remains a common institutional practice that has received widespread attention from a political, ethical, legal, and clinical standpoint. Although there is an abundance of scientific work on the subject, few studies have examined the experience of patients being confined. In order to improve the quality of nursing care surrounding this measure it appeared essential to obtain data on patients' perspectives, information deemed valuable in orienting nursing interventions. This qualitative study, guided by a phenomenological research design, aimed at describing and gaining a better understanding of patients with a severe and persistent psychiatric disorder who were placed in a seclusion room while hospitalized on a closed psychiatric unit. Using a semi-structured, non-directive interview format, a total of six patients participated in this study. Content analysis of participants' narratives yielded three main themes that appeared to be central to their experience of seclusion: their experience of seclusion on an emotional level, their perception of this intervention, and how they coped during their stay in the seclusion room. Major findings emerging from this nursing study centred on the following dimensions: patients' perceptions of seclusion as a punitive measure and a modality for social control and, the experience of seclusion serving as an intensification of already existing feelings of exclusion, rejection, abandonment, and isolation. In addition the findings also suggest that it is not seclusion per se that impacts on their negative perception and negative emotional experience but rather the lack of nurse-patient contact during the seclusion experience. Furthermore, whether patients coped by regressing, acting out, or taking on a more compliant stance, they appeared to be motivated by a need to connect with staff. This points to the importance of the relational aspects of nursing care when applying this restrictive measure. A need for modifying the institutional culture surrounding seclusion and transforming nursing practices are discussed as are future research endeavours.

Abstract:Die Studie zeigt in den Grundvorstellungen des Psychiaters Karl Peter Kisker sowie des Psychiaters Klaus Weise und Philosophen Achim Thom das gemeinsame Anliegen ab den 1960ern, psychisch Kranken zu mehr Selbstbestimmung und gesellschaftlicher Teilhabe zu verhelfen und die Psychiatrieversorgung zu humanisieren. Aus der Gesamtschau wird zunächst die theoretische Konzeption von Kisker dargestellt, der ein Vertreter der westdeutschen Sozialpsychiatriebewegung war und eine phänomenologisch-anthropologische Prägung aufwies. In Gegenüberstellung wird der ostdeutsche Ansatz von Weise und Thom erläutert, der an die phänomenologisch-anthropologische Subjektorientiertheit anknüpfte, aber mit der Einbeziehung einer marxistischen Auffassung eine sozialistische Sozialpsychiatrie begründete. Im Vergleich werden trotz vieler Übereinstimmungen die unterschiedlichen Gewichtungen der beiden Ansätze deutlich, die soziale Dimension des psychischen Krankseins zu verstehen. The study explores the common effort of social psychiatrists in Eastern and Western Germany to help people suffering from mental health issues to gain more self-determination and social participation and to make mental health care more humane from the 1960s onwards. At the same time, it provides a contrastive analysis of the social psychiatric concepts developed by the psychiatrists Karl Peter Kisker, Klaus Weise and the philosopher Achim Thom. A thorough analysis of literature reveals differences in the theoretical approaches in the East and West. Kisker, who was a representative of the West German social psychiatric movement, had a phenomenological-anthropological background. By contrast, Weise and Thom even though following the same subject orientation, established a socialist social psychiatry clearly integrating Marxist views into their concept. This contrastive also elaborates common viewpoints in understanding the social dimensions of mental health conditions in the two concepts. Crime, punishment, and detention in secure psychiatric hospital

Abstract:Medicine, Science and the Law, Ahead of Print. Many legal jurisdictions offer some form of hospital diversion and disposal as an alternative to incarceration in prison for mentally disordered offenders. Such diversion is commonly understood as offering a non-punitive alternative in terms of sentencing decisions. However, complete loss of responsibility with respect to acts of violence is rare and indicative of extreme degrees of mental disorder. This raises challenges for sentencers when considering disposal options. From the perspective of the patient and healthcare providers while hospital may be framed as non-punitive, it still involves marked loss of freedom and rights. In this essay, it is argued that failure to acknowledge the punitive element, inherent in hospital detention, risks its repression, and a false dichotomy being established with prison being seen as solely punitive and hospital as solely therapeutic. It is suggested that this division is unhelpful, even potentially harmful, and that a synthesis as solution to this dialectic opposition may be generative in terms of therapeutic work in hospitals, clarification of the role of hospitals in terms of criminal justice disposal, and greater transparency in relation to multi-agency working and the social circumstances of patients detained in secure hospitals. Further work to understand this process is suggested with a particular emphasis being placed on the experience of specific groups of patients, such as women, who may find themselves in a notably precarious state within secure care.

Abstract:De-institutionalization (DI) is framed and recognized internationally as a lever for the realisation of inclusion for citizens with disabilities. In international research, policy and practice, DI is predominantly framed as the dismantling of institutional care to pursue inclusive living, housing, and working in the community. We can hereby, among others, refer to the checklist recently developed by the European Expert Group on the Transition from Institutional to Community-based Care (2019) or the Guidelines on Deinstitutionalization as described by the UN Committee on the Rights of Persons with Disabilities (2022). The renewed emphasis on DI is also promoted in Flanders (the Dutch-speaking region of Belgium), which has a very slow response to DI compared to a.o. the Nordic countries. Although DI is widely promoted, there is no consensus as to how to conceptualise and address DI. Quite recently, sharp critiques are formulated as ambiguities and complexities can be seen both in th goals and implementation process (https://www.ohchr.org/sites/default/files/documents/hrbodies/crpd/cfi-deinstitutionalization/submissions/2022-07-06/Academic_W_Ghent_Uni-06-07-2022.docx). Research shows that persons with disabilities who are physically integrated in the community do not necessarily have the experience of belonging in society; that repressive institutional cultures continue to come into play when people with disabilities live in the community; that there is a risk of not having access to (specialised) care and support; and that they risk lacking social and financial benefits or meaningful interactions and relationships in society. This phenomenon is referred to in international research as 'institutionalisation at home', 'community institutions', ‘extitutions’ or 'asylums without walls'. DI thus refers not only to the location and nature of the architectural and spatial embedding, but also to an institutional culture/professionalism and autonomy. Therefore, there is an urgency in Flanders to support innovation in policy and practice and to gather knowledge on conceptions and strategies of DI. In that spirit, the Flemish Agency for People with Disabilities provided funds to establish the Academic Workplace De-Institutionalization (AWDI) at Ghent University. In this lecture we provide an insight in this unique collaboration between practice, policy and academia to tackle the challenges mentioned before, to inspire policy and generate new insights with regard to DI. In the coming years, the AWDI is committed to connecting the experiential knowledge of people with disabilities, their networks, professionals, policymakers but also architects and urban planners to stimulate innovation in the field and policy and create qualitative living environments. The AWDI will do so by conducting a variety of action research projects, setting up learning networks in which all relevant voices are involved and building up and disseminating knowledge. Thus, this lecture refers to the central theme “Engaging everybody: caring neighbourhoods and compassionate communities” “but it also tackles the topics described under "Change management implementation: macro, meso and micro level”.

Abstract:Background and aims: We wished to document the pattern of institutional care for dependent elderly people during a period of altered provision of care, namely a shift from National Health Service long-term hospital care provision to residential and nursing home provision in the private sector. Methods: We systematically studied all institutionalized people aged 65+ years in a single geographic area in 1989, 1992, 1995 and 1999. Subjects’ physical and mental function was assessed using the Barthel Index and the Abbreviated Mental Test. Descriptive and statistical comparisons between years and care facilities were made. Results: There were large changes in facilities providing for care between 1989 and 1999, with an 84% increase in nursing home placements and an 89% decrease in hospital admissions. Smaller reductions in residential and psychogeriatric placements were observed together with a large increase in Elderly Mentally Infirm (EMI) placements. We found significant increases in the physical dependency and mental impairment of those receiving nursing, psychogeriatric and long- term hospital care. Residential homes are now caring for people who are significantly less physically dependent, and there have been no changes in the dependency level of residents in EMI homes. Conclusions: There has been an overall mean reduction from 4.3 to 3.9% in the percentage of elderly people aged 65 and over receiving institutional care between 1989 and 1999. This trend has included a radical shift in care from hospital- based provision to nursing home care and undoubtedly an increase in support for people wishing to remain in their own homes. The increased dependency found in institutional care is believed to be due to the higher threshold now necessary for provision of care, which may be due to fiscal constraints.

Abstract:In this article we report on the findings of a two-part project investigating contemporary issues in sexuality researchers' interaction with journalists. The goal of the project was to explore best practices and suggest curricular and training initiatives for sexuality researchers and journalists that would enhance the accurate dissemination of sexuality research results in the media. We present the results of a survey of a convenience sample of 94 sexuality researchers about their experiences and concerns regarding media coverage and a summary of the main themes that emerged from an invitational conference of sexuality researchers and journalists. In addition, we present some preliminary recommendations for training and best practices. Topics assessed include reporting accuracy; sex researchers' comfort with various topics, media, and journalists; researchers' perceptions of the purpose and content of articles; concern about the impact of media coverage; and training for sexuality researchers.

Abstract:Here we report the results of a randomised multicenter phase III clinical trial which assesses the therapeutic efficacy and tolerability of a chemotherapy protocol CEMP (cyclophosphamide, etoposide, mitoxantrone and prednisone) in adult and elderly patients with advanced intermediate and high-grade NHL. Between October 1991 and October 1995, 139 patients, aged 55 to 79 years, with diffuse intermediate and high-grade lymphoma, were enrolled. A considerable percentage of patients had clinically aggressive disease: 32.4% had systemic symptoms, 79% had stage III or IV disease, 33.8% had bone marrow involvement, 46% had splenic involvement and 42.5% had increased values of serum lactate dehydrogenate. Complete remission was achieved in 70 of the 139 patients (51.9%) and PR in 12 (16.6%) with an overall response of 68.5%. The overall response survival rate at 6 years was 39%, whereas DFS rate was 48.7% and PFS rate was 28.5%. At four years 49% of the patients were still in CR. Dividing the patients in two groups, under and over 65 years of age, we obtained the same results as far as overall response is concerned. No toxic deaths occured, neither cardiac, renal nor liver complications happened. CEMP regimen is an effective and safe protocol with good results in elderly people, well comparable to those achieved in younger ones.
