As in the past, such times favour the exclusion of the weakest. As resources dwindle, only the clinical meaning, the dynamism and creativity of the nursing teams can ensure the emergence of new solutions for complex care situations.

Abstract:Historical aspects: The idea behind this work is to have an ethical examination of the institutional psychotherapy movement which has long influenced French public psychiatry and which has progressively, since the 80s, been subject to growing doubts. In the first part, institutional psychotherapy is presented. It is a model for theoretical development and practice in psychiatric care. It came into being just following the end of the Second World War at the same time as modern medical ethics. Its principles come on the one hand, from recognition of asylums' pathogenic effects--which led to the crushing of the patient's being--and on the other, through recognition of the uniqueness of each person and the subjectivity of mental suffering. These elements gave rise to creativity within the world of medicine and, in the sector, generated the science of psychiatry which advocated for continuity in care (both inpatient and outpatient) and preventive work directed at the population. This movement called for the use of the institution in its dynamic aspect which promotes exchanges and allows patients to situate or resituate themselves in historic and symbolic dimensions. It privileges a high level of transversality, maximum communication, favouring speaking out loud and responsibility. It requires a permanent analysis of the institutional counter transference (emotional reactions of the caregivers involved, their interrelations and the social and material organization of the institution) which determines the therapeutic action itself. THEORICAL BASIS: In a second part, its theoretical foundations and its practice shall be investigated in light of the guiding concepts of medical ethics (justice, autonomy, beneficence, non-malfeasance). Institutional psychotherapy responds to the need for justice by considering the patient as a whole and by conceiving each patient as being like oneself despite the differences (associated with the mode of hospitalization, the social or diagnostic category). The principle of autonomy lies always at care's horizon, made concrete in practice by notions of habitability, orientation to place and time, references, by activities and by meetings. The principle of beneficence was the basis for round-the-clock intakes, the use of an established theoretical model and clinical practice centred on the patient's words. Institutional analysis attempts not to repeat the alienation felt by the patient, alienation being associated with the treatment environment, and draws from the principle of non-malfeasance. It therefore appears that the approach of institutional psychotherapy conforms with ethical requirements, given its major interest in the subject who aspires to find meaning in life. Current situation: The third part discusses current orientations in care related to the explosion in neuroscience and technology, the promotion of the citizen as an individual, and legal doctrine, budgetary constraints, and new demands made on psychiatry from the social and political domains. The widespread trend towards simplification, swollen with hypotheses from neuroscientific research, is progressively reducing mental illness to target symptoms. The recovery of the notion of citizenship through technological capture and ideological strains in contemporary culture have also affected a suspension of the subject as a thinking and desiring being, and exempted caregivers from considering transferral phenomena, indicated with the appearance of new signifiers: user, stress, plague, network. The new medical-technical jargon of scales, tables, and management participates in the same process of patient objectification and care compartmentalization. In this context, under the cover of science and generally good actions, psychiatry has become biologisized, whilst being diluted from its social aspect, even as it becomes more repressive for patients. Conclusion: This observation leads to the conclusion, in the fourth part, that there is a need for psychiatry, which within its own discipline has a hard time finding sufficient resources, to refocus itself ethically. This enlightenment could come within a multi-disciplinary ethical space, uniting practitioners and psychiatric caregivers, psychoanalysts, philosophers, sociologists, neurobiologists etc. The research should concern the legitimacy of the principles that underlie psychiatric action in its care and preventive aspects.
[Many faces of deinstitutionalization--sociological interpretation]

Abstract:The article summarizes in an international perspective what kind of results psychiatric deinstitutionalization has brought so far: a profound change of size and functions of the psychiatric hospital; better services for people with less severe problems; and the failing of community services to compensate for some of the functions of the former asylums, resulting in trans-institutionalization and/or neglect for many chronic patients. Three different sociological versions to explain the background and typical outcomes of psychiatric deinstitutionalization have been brought forward so far: political economy, professional dominance and post-structuralism. They are confronted with an approach using the concept of medicalisation which offers a more comprehensive understanding of the process.

Abstract:Summary The recent drive within the UK National Health Service to improve psychosocial care for people with mental illness is both understandable and welcome: evidence-based psychological and social interventions are extremely important in managing psychiatric illness. Nevertheless, the accompanying downgrading of medical aspects of care has resulted in services that often are better suited to offering non-specific psychosocial support, rather than thorough, broad-based diagnostic assessment leading to specific treatments to optimise well-being and functioning. In part, these changes have been politically driven, but they could not have occurred without the collusion, or at least the acquiescence, of psychiatrists. This creeping devaluation of medicine disadvantages patients and is very damaging to both the standing and the understanding of psychiatry in the minds of the public, fellow professionals and the medical students who will be responsible for the specialty's future. On the 200th birthday of psychiatry, it is fitting to reconsider the specialty's core values and renew efforts to use psychiatric skills for the maximum benefit of patients

The medical desertification that is gradually taking hold in certain regions is a cause for concern, as is the growing disaffection of nurses throughout France. It is essential to draw up a map that incorporates survival oases, in order to meet the essential need to provide care anyway, while at the same time perpetuating the process of improving care. Abstract:To address some challenges facing psychiatrists today we discuss issues of happenstance and fulfilment in psychiatric careers through some of the record and reflections of four psychiatrists since the 1950s. We trace the changes in psychiatry attendant to the transition from the welfare to the neoliberal state and=its contemporary postmodern culture. We highlight the crucial importance of political-cultural as well as technological developments in determining psychiatric service management and provision, and clinical practice and career outcomes. In the light of this impact, in a global era that some highly respected authorities consider in apocalyptic terms, we advocate for the incorporation of training in political awareness and activism in the psychiatric curriculum and practice. We suggest that this is necessary for social justice and patient welfare and that it will help safeguard psychiatric professionalism, conscience and self-esteem.
[Changes in the provision of psychiatric services at a psychiatric district hospital]

Abstract:Changes in the provision of psychiatric services within the last decennials are probably best understood if the impact of national socialism on clinical psychiatry is regarded. Many psychiatrists took part in the "Aktion T4" the organised killing of their patients, at least they did not resist campaigns directed to killing or sterilisation patients. After WW II, a period of silence and acceptance of inhuman circumstances in the large mental state hospitals appeared, when in the sixties a new generation of psychiatrists was no longer willing to continue the traditional system of mental health care delivery. The Expert Commission on Mental Health Care reported a comprehensive agenda on reformation of service delivery in 1975, which was influential in the development of alternative structures of psychiatric services. Most mental state hospitals reduced their capacities and parallel to this process smaller units, devoted to principles of community psychiatry, associated to general hospitals were created. Overall, the number of hospital driven beds decreased at about 33 %, but in the large hospitals at two thirds within the last 25 years in Baden-Wurttemberg. This process of deinstitutionalization was accompanied by the development of structures for community care, internal reorganization and modernization, and important steps in budget development, which lead to better treatment opportunities for all kinds of psychiatric institutions. The concrete and historical reality of these circumstances and changes, and possible future directions are exemplified for the psychiatric hospital in Weinsberg.
[Vulnerabilities, ethical responsibility and acceptance of otherness during the first Covid lockdown: A case study of a French Care Centre for Support and Prevention in Addictology]

Abstract:The first Covid-19 lockdown changed medical and care practices. Although positive patients were prioritised and the focus was on the efficiency of the intensive care units, the relational aspect of care is central to the philosophy of French Care Centres for Support and Prevention in Addictology (CSAPA in French). The caregivers were impacted by restrictive aseptic protocols, with an increased risk of patients, that are already very vulnerable, abandoning or not following-up their treatment. Based on the case of Grasse CSAPA, the paper aims (1) to analyse, retrospectively, the ethical tensions triggered by the acceptance of otherness in patients with complex issues and (2) to highlight the inventiveness of the caregivers in their daily practice. The method is philosophical and conceptual and focuses on Levinas's ethics of the "face", because the crisis exacerbated pre-existing ethical tensions concerning intersubjectivity. Caregivers had to face up to a heightened ethical "responsibility for others". However, the crisis was also an opportunity to reinvest in relational care, by allowing them to detach from the primacy of sight: the masks sometimes helped and sometimes hindered care, revealing the shared vulnerability of both caregivers and patients. The health crisis reminds us that vigilance is always necessary with regard to the risks of dehumanising the relationship of care.
[Brief psychiatric hospitalization: a possible way, a strategy to evolve?]

Abstract:The process of disinstitutionalization combined with the economic reality is responsible for the great upheaval in taking care of psychiatric patients. The repercussions are worldwide, national, and local concerning the Philippe Pinel Psychiatric Hospital (Amiens, Somme) place of this work. So the psychiatrists of this institution have to do with the following datas: a reduction of the admissions between 1991 et 1998 (around 1,5%) and a provided reduction of the hospitalization capacities upper to 40% for the following two years! Then the connection with these two figures requires the development of new therapeutic strategies, with the existing means. In this peculiar context, a study has been carried on over 2 years: 49 psychiatric patients who benefited from a brief hospitalization (less than 48 hours) have been followed up. The interest proceeds from the high frequency of the type of clinical situation which concerns 12,5% of the admissions in the studied psychiatric department. In the same time, a pilot group of 49 patients has been drawn lots among all the admissions during the same time: patients who benefited from a more traditional hospitalization (about twelve days), with strictly a same psychiatric diagnosis as in the first group, using the ICD 10 classification. The emphasis was focussed on the patient's psychiatric curing process into the 2 groups; we have compared the item rehospitalizations in a psychiatric hospital (through the number of rehospitalization, the number of days of rehospitalization, and the necessity - or not - of a rehospitalization) with the object of estimating the benefit, the inefficiency, or even the negligence of proposing a brief hospitalization. We have also studied socio-environmental datas, antecedents and effective psychiatric follow-up into the two groups. Concerning the diagnosis, mental disorders related to alcohol abuse (F10) are the most frequent (49%) into the group brief hospitalization , which diverges from the usual results taking account hospitalizations in psychiatric services. Then we have found personality disorders (14,3%), schizophrenia (12%), adaptation disorders (10,2%), anxiety (8,2%) and opiated abuse (4,3%). By that very fact, the pilot group allows exactly the same diagnosis. There's no significant difference concerning number of hospitalizations or number of days of rehospitalization; there is even a tendency in favour of patients who have been hospitalized less than 2 days: an other hospitalization is not as frequent as in the pilot group, without any exacerbation of their pathology (no less sight of patients, same number of death). It's important to emphasize that this benefit isn't related to less severe pathology for the first group: there can be a comparison between psychiatric antecedents and seniority of mental troubles. By another way, socio-environmental datas (age, sex, social and family positions) are homogenous. Lastly, circumstances of the hospitalization - inclusion between the groups are similar: same origin of the patients, who have required themselves their admission (it means the knowledge of the psychiatric hospital, distinctly one or more previous hospitalizations). The contacts with the entourage of the patients have been managed in the same way with the same results, and medical follow-up after having left hospital were identical. So we come to the conclusion that in spite of the apparent slight of this strategy, there can be a comparison between this kind of aid and a more traditional hospitalization, in many clinical situations, all the more when the emphasis is laid on the patients psychiatric curing process. Consequently a brief hospitalization can be considered as a possible therapeutic strategy. Two facts command attention now: we must clearly define the type of patients who have really benefited of this brief hospitalization, with the object of being able to plan this strategy. By another way, it seems that a brief hospitalization, just like any hospitalization, is one part of our patients curing process for the two groups. Therefore, the choice of a psychiatric hospitalization becomes a debatable point, through the treatment of a psychiatric emergency.

Abstract:Nearly a decade since the first experiment in France of the so-called "announcement policy" a concerted effort, the pooling of skills of health professionals from different backgrounds, have had noticeable positive effects on the daily practice. This article attempts at analyzing the question that was causing the application of former patients and their main complaint: the lack of communication and its direct consequences, anguish and loneliness. Subsequently, the announcement policy, this multifaceted approach, had consisted of creating for the patient what can be called "a good enough environment able to hold". It is therefore not a plan but a disposal (if we are willing to agree on the fact that these are the qualities of the therapeutic setting that count and not the format by itself). Considering the ipseity of each patient, the uniqueness of each particular situation, of each social background, the need for harmonization of the announcement policy is questionable. Harmonization does not mean uniformity. The quality of this policy must be adapted closer to each case. However, it must be based on the basic principles and might also take into account the dimension of anguish. These fundamental principles are directly derived from the principles of medical humanism, those that have brought meaning to scientific progress.

Committed to teamwork, they play their part in the multi-disciplinary weave of containing and structuring that the psychiatric institution confers on the most fragile patients.

Abstract:Psychiatric nursing stands at a crossroads, in danger of losing both its identity and standing within the larger body of nursing. Enrollment in graduate programs is at an all time low and many traditional employment opportunities for psychiatric nurses are disappearing. Many explanations have been proposed to account for the crisis in psychiatric nursing practice. Although many of the identified external forces have had real impact on psychiatric nursing, these factors have impacted all of nursing. Yet our Nurse Practitioner (NP) peers are thriving, with proliferation of NP programs with unprecedented enrollment for this nationally accepted and understood role. The psychiatric nursing crisis may be most directly related to the reality that we as a professional group have thus far failed to adequately respond to external realities that have dramatically altered the environment in which psychiatric nursing occurs. This article argues for reframing the discipline of psychiatric nursing, accomplished as a national consensus building process, and including 4 critical components: (1) reconceptualization of what constitutes the core of psychiatric nursing content and represents the epistemological heart of the profession; (2) identification of the critical clinical competencies that reflect the core content, represent the role and scope of psychiatric nursing, and that match current and anticipated practice realities; (3) identification and standardization of measurable clinical outcomes, predicated on both content and competencies, which will allow psychiatric nurses to measure, in meaningful ways, the impact of their practice on patients' health; and (4) establishment of a research agenda that will allow psychiatric nursing to expand its unique knowledge base.

Psychiatrists' experiences of involuntary care in South Africa: dilemmas for practice in challenging contexts

Abstract:Providing for people with psychosocial conditions in crisis is a complex and controversial endeavour that has gained significant attention over the past decade. This increased focus is driven by global calls to reduce coercion, including by the United Nations Committee on the Rights of Persons with Disabilities, who interpret Article 12 of the United Nations Convention on the Rights of Persons with Disabilities in General Comment 1 to advocate for the replacement of substituted decision-making with supported decision-making. Psychiatrists occupy a central role in determining how to care for and respond to individuals with psychosocial conditions in crisis in the midst of these debates. They must protect the rights of people with psychosocial conditions in crisis and provide appropriate support within challenging and dynamic contexts. This responsibility includes promoting the autonomy of people with psychosocial condition while ensuring their long-term health, safety and well-being.In this study, we conducted a phenomenological analysis with a sample of nine psychiatrists in South Africa to explore their experiences with involuntary care and the complex dilemmas they face in delivering healthcare to individuals with mental health conditions. Our findings indicate that psychiatrists encounter significant challenges in preserving patient autonomy, particularly within the resource-limited context of South Africa. Pervasive stigma and insufficient support infrastructure complicate efforts to prioritise autonomy. At the same time, professionals must address the critical need to ensure the long-term safety and well-being of their patients. The absence of involuntary care can exacerbate a person's vulnerability to community stigma and inadequate community support, posing severe risks to their welfare. Balancing between protecting a person's autonomy and addressing the limitations of support structures creates a complex predicament for mental health professionals, often resulting in feelings of isolation and moral distress among psychiatrists.
[Intercultural Opening in Health Care Promotes Solidarity]

Abstract:Aim: Due to the increase in socio-cultural diversity, the importance of intercultural opening processes is also increasing in the German health care system. The present study examines on the basis of 4 case studies to what extent an organisationally designed intercultural opening (IKÖ) has an effect on the system and social integration level. On the basis of the empirical results, practice-oriented recommendations can be derived. Methods: Using socio-scientific qualitative methods, a participating observation was carried out on 21 days in 2 hospitals and 2 outpatient care services in NRW. The institutions examined differed in terms of the degree of intercultural openness. The observations were systematically recorded and evaluated using the documentary method with MAXQDA inductive category-based evaluation. The approach is based on Bourdieu's field-habitus theory 7 and the fields of observation were analyzed on a system and social integration level 8 according to Lockwood. Results: The elaborated system of categories consists of 4 main categories (methodology, work organisation, communication, care and medical system) and 918 sub-categories. An intercultural opening of the organisation in the form of an intercultural team and measures offered for intercultural opening are accompanied by a higher degree of solidarity. Conclusion: The intercultural opening process in the health care system must be anchored at the system and socialintegration level in order to promote more efficient care processes and solidarity. Since IKÖ involves additional time, financial and personnel resources, a strategic decision, on the one hand, and a consciously controlled opening process from above, on the other hand, is needed on the part of the management. This enables the employees to participate in shaping the traditional organisational culture in a negotiation process.

Abstract:The concept of deinstitutionalization started in the 1960s in the US to describe closing down or reducing the number of beds in mental hospitals. The same process has been going on in many countries but with different names and in various forms. In Europe, countries like Italy prescribed by law an immediate ban on admitting patients to mental hospitals while in some other European countries psychiatric care was reorganized into a sectorized psychiatry characterized by open psychiatric care. This sectorization has not been studied to the same extent as the radical closures of mental hospitals, even though it entailed major changes in the organization of care. The deinstitutionalization in Sweden is connected to the sectorization of psychiatric care, a protracted process taking years to implement.

[The stakes in psychiatric hospitalization. Is an ethical approach possible?]

Abstract:To pose the question of what is at stake in a psychiatric hospitalisation really means to pose the question of its purpose, its justification and ultimately of its underlying ethics. After having defined ethics in its moral and deontological aspects, we shall broach the psychiatric and medical aspect of its politics and relationship to the social field, questions which have become acute because of the fact that they are directly related to our notion of liberty. Certain points shall be more specifically developed: Whom do we serve? To hospitalise, is a medical act. Hospitalisation is a means of protection against Murder and Incest. Hospitalisation is a Third dimension in a Space and Time experience. Our intervention is a logical outcome of our own conception of symptoms and the healing process. What if ultimately, pathology were but the answer to an unverbalized interrogation? To take into consideration, beyond the determinating factors be they genetical, biological, or social, a psychic reality which we recognize in the patient's transfer and his verbalisation, impell a necessary and imperative change in the perspective of the field of conventional psychiatry. To allow oneself to be interrogated by this "Other" discourse, and to assume all the consequences, both theoretical and practical, this is the passing from medical ethics to what might be called analytical ethics.
Contemporary approaches in mental health rehabilitation

Abstract:In many European countries, deinstitutionalisation has been an ongoing process over the last few decades. Mental health organisations were transformed to provide support in a more integrated and comprehensive manner, preferably in their own homes in the community. Yet, despite the welcome aspiration of community integration for all, people with complex mental health problems (also termed severe mental illness) have continued to require high levels of support, in inpatient settings and in the community. This group's needs make them highly dependent on their caregivers. The attitudes, knowledge and skills of the staff providing treatment and support is crucial to their recovery. Rehabilitation programmes provide a much-needed framework to guide practitioners and help them organise and focus their recovery-oriented approach. In this editorial, we will provide a non-exhaustive overview of such rehabilitation programmes and interventions to illuminate the wide scope and practical usability of these interventions for this group of people with complex mental health problems.
Mental health care in Italy: Basaglia's ashes in the wind of the crisis of the last decade

Abstract:Background and aims: The purpose is to highlight the legal and ethical principles that inspired the reform of mental health care in Italy, the only country to have closed its psychiatric hospitals. The article will also try to verify some macro-indicators of the quality of care and discuss the crisis that the mental health care system in Italy is experiencing. Methods: Narrative review. Results: The principal changes in the legislation on mental health care in Italy assumed an important role in the evolution of morals and common sense of the civil society of that country. We describe three critical points: first, the differences in implementation in the different Italian regions; second, the progressive lack of resources that cannot be totally attributed to the economic crisis and which has compromised application of the law; and finally, the scarce attention given to measurement of change with scientific methods. Conclusion: Italy created a revolutionary approach to mental health care in a historical framework in which it produced impressive cultural expressions in many fields. At that time, people were accustomed to 'believing and doing' rather than questioning results and producing research, and this led to underestimating the importance of a scientific approach. With its economic and cultural crisis, Italy has lost creativity as well as interest in mental health, which has been guiltily neglected. Any future humanitarian approach to mental health must take the Italian experience into account, but must not forget that verification is the basis for any transformation in health care culture.

Abstract:This conceptual article examines the organisational crisis in England's National Health Service in light of the recently launched model of policing called Right Care Right Person introduced to reduce police hours spent dealing with mental health crisis calls. It is a move that has come with concerns for health services because these newly created gaps alongside the existing ones pose challenges around funding and timescales in implementing the new model. It is a curious case of organisational paradox that diverting mentally ill persons into health services and 'decriminalising' those whose health conditions bring them to the attention of the justice system, has raised concerns in the health sector about access to adequate mental health services unless an arm of the justice system is involved. Given the similarities in health and legal systems in the Anglo-Western world, this English model has international implications about organisational paradoxes in health systems.
