Abstract:Mandibular discontinuity defects are most often due to tumor resection and trauma which affects a variety of oral function. Reconstruction plates were popularly applied to bridge mandibular defects after tumor resection. The esthetics and continuity of the mandible have been rehabilitated successfully. However, the function is difficult to reconstruct. Furthermore, skin or mucosal perforation, plate fracture and loss of retention screw were the serious complications resulting in reconstruction failure. For these reasons other alternative has been explored. In this paper, a novel mandibular prosthesis with perfect contour supporting bone chip and dental implant has been designed. The biomechanical effect of the novel prosthesis has been analyzed using the three-dimensional finite element method (FEM). The results showed that the novel mandibular prosthesis filled with bone chip and with heights of 20mm, dental abutment diameter of 4mm (1mm surrounding width), transversal hole space of 3mm and longitudinal space of 2mm, connection interface angel of 45 degree and boss-screw retention could reconstruct the defective mandible perfectly. The stress level in prosthesis-bone system was reasonable. The bone chip filled in the scaffold was also in the normal stress level, which kept it active and was to be useful for combination of bone and prosthesis. And the boss-screw retention was reliable and ensured smooth connection between prosthesis and normal bone segments.
