Abstract:This paper investigates the representation of mental disorder in the discourse of mental health within Indian society through the analysis of Jerry Pinto’s novel Em and the Big Hoom. The novel captures the experience of living with a family member who suffers from a mental disorder, through the eyes of caregivers. The novel also portrays the present-day mental health care condition in India. Pinto describes language as the only medium to highlight the struggle of people suffering from mental disorders. The puns and jokes and the colloquial terms used by Em in this novel are examined to emphasize the interrelation between mental illness and society. Jokes and puns disclose the unconscious of the human mind and decode the repressed thoughts and desires that are there censored by society. Delusions and memories of Em are also investigated. The theory of Psychoanalysis is applied for this purpose. Utterings of a ‘pagli’ (mad) that is immediately discarded by society get attention from her caregivers. Research about literature on mental illness could contribute to bringing real changes in society and laws related to mental health care and eradicate the stigma associated with mental disorders.
'Picture imperfect': the motives and uses of patient photography in the asylum

Abstract:In the nineteenth century, photography became common in psychiatric asylums. Although patient photographs were produced in large numbers, their original purpose and use are unclear. Journals, newspaper archives and Medical Superintendents' notes from the period 1845-1920 were analysed to understand the reasons behind the practice. This revealed: (1) empathic motivation: using photography to understand the mental condition and aid treatment; (2) therapeutic focus on biological processes: using photography to detect biological pathologies or phenotypes; and (3) eugenics: using photography to recognise hereditary insanity, aimed at preventing transmission to future generations. This reveals a conceptual move from empathic intentions and psychosocial understandings to largely biological and genetic explanations, providing context for contemporary psychiatry and the study of heredity.

Abstract:Our understanding of the history of psychiatry has been warped by the way history is remembered – collectively and personally; this is the point of departure from which Wynter, Wallis and Ellis have embarked upon this splendid essay anthology. The introduction provokes something of a liminal space by describing psychiatry as a specialty that is ever existing between 'the terrible past and the ideal future'. They argue that barbarity has become a cultural trope for psychiatry, especially when historical treatments are viewed through modern framing without the scaffolding of contemporaneous societal values. Progress seems to always be in response to wanting to heal this iniquity and provide a change in direction. The authors endeavour to show us that the actuality of psychiatry may exist somewhere between these two opposing poles, and furthermore it is interested in the idea that modern mental health services have arisen from within this in-between space. The essays loosely stick to a theme of memory and remembrance, and are arranged within five sections: governance; practitioners; case books; oral histories; and personal recollections. The various essays look at different aspects of psychiatry through different eras and across the globe, with very distinct voices and forms, but this thematic arrangement provides a sense of cohesion. However, there is no onus on the reader to read the whole collection all at once, with most of the pieces having enough strength to be considered in their own right. We see Britain's colonial footprint in the particularly provocative 'Madness, memory and delusion in late nineteenth-century Barbados', whereby Leonard Smith skilfully introduces readers to the cultural matrix of colonial Barbados, where many people were living in destitution, and then goes on to consider a series of nineteenth-century lunatic asylum case records held in the Barbados National Archive. A re-examination of these material impressions of the past acknowledges the colonial gaze through which patients were described, but also provides a new way of seeing patients and their problems, beyond the hermeneutic trouble entailed by the passage of time. A chapter about Bombay's lunatic asylums around the turn of the twentieth century readily relates to this one, but other works describing topics as far reaching as the remodelling of the Sigmund Freud Museum in Vienna and the nostalgia experienced by staff in the wake of an American psychiatric hospital ensure a significant breadth of content. The book ends with a devastatingly honest reflective essay by Allan Beveridge that focuses sharply upon his experience of psychiatric training in Edinburgh in the 1980s. He shares his personal sense of excitement upon embarking upon his career that gives way to unease as he faces the reality of clinical practice, and the discombobulating effect of competing explanations and opposing views that continue to persist, to a greater or lesser extent, to this day. The book grasps onto the Derridean idea that there is an ever-transforming inheritance of the past in light of changing contexts of the future, and that the key to history lies in how it is remembered. Through an exploration of remembrance, this work generates novel insights, which allow us to recast how we understand previous insights into psychiatry's past. This thought-provoking essay collection will generate enthusiasm for new ways of considering the past and reframing the present through troubling our interpretation of what has been written before. It is certainly worth reading to generate reflections on what the legacy of our own work may be.

[Which way ahead for psychiatry? Identity, competence and cooperation--challenges of the 21st century]

Abstract:In the past decades some disadvantageous changes have been occurred in almost each field of medicine, especially neurology and psychiatry. Medical students and young doctors are less and less able to utilize their neurological knowledge in the everyday clinical practice and the number of doctors specialized in neurosciences is decreasing. Recent problems of psychiatry originate from special development, controversial process of detachment, particular societal and cultural roots, differences from medical model, and problems of identity, self-determination and competence, which may cause many difficulties in clinical practice. The author - near the review of the theoretical background - mentions several practical topics as well, for example: the difficulties of diagnosing, and classification of mental disorders, the diagnostic and treating problems being in the background of psychopathological symptoms. In the article it is emphasized that the present situation results in several serious problems for the psychiatrists like emotional overburden, somatic and mental disorders and burnout, moreover psychiatry became one of the least popular medical professions, which causes huge difficulties in the aftergrowth. Furthermore the author emphasises, that this profession has to actualize the humanistic view in clinical practice based on the holistic bio-psycho-socio-spiritual approach. The (neuro)scientific and consultative psychiatric viewpoints, the constructive intra- and interdisciplinary communication, the more effective value-based advocacy and the modern, evidence-based, interactive and personal education together have a key role in the solution of the above mentioned problems.

Abstract:Sir Alexander Morison's The Physiognomy of Mental Diseases and the original art work that formed the basis of the book have not had the scholarly attention they deserve. The published book and the commissioned portraits have not been studied in any detail. Historians have tended to offer cursory assessments that have reflected their own preconceived ideas rather than properly engaging with the material. This is a pity because Morison's work is a rich source that tells us much about the history of psychiatry. The pictures and text give us a glimpse into the world of the asylum and that of the patient. Although we see the patient through the eyes of the artist and Dr Morison, they do emerge as individuals. The accompanying texts reflect the psychiatric approach of the time and reveal contemporary notions of diagnosis, aetiology and treatment. Morison's work can also be located in the history of ideas about physiognomy. He himself was particularly influenced by Jean-Étienne-Dominique Esquirol, and Morison's work, in turn, influenced WAF Browne. These papers will outline Morison's career and consider in detail his book on The Physiognomy of Mental Diseases.
Dementia praecox to schizophrenia: the first 100 years

Abstract:The historical roots of dementia praecox and schizophrenia are described in the context of current nosology and continuing controversies surrounding this nosology. Relevant books and journal articles were reviewed. The information was obtained through computer searches and cross-references from previously published papers. If English translations of foreign language articles were available, they were used; if not, the cross-references were consulted. The psychoses have existed as diagnostic categories from ancient times although their names have changed. Initially, these disorders were considered diseases of the brain, a concept that was swept aside in the United States, influenced by European-derived psychodynamic theories. American clinicians and investigators simply accepted these theories, showing little interest in testing their underlying principles. In contrast, a narrower Kraepelinian approach was adopted outside the United States, and attempts were made to refine its nosology. Because current data supports a central nervous system aetiology for schizophrenia, the concept of dementia praecox warrants resurrection. The authors suggest abandoning the term schizophrenia in favour of the more broad and generic term dementia praecox. Replacing 'schizophrenia' with 'dementia praecox' in the 21st century will facilitate further research and help clarify the nosology of various brain disorders currently included in the schizophrenias.
Old familiar faces: some aspects of the asylum era in Britain, Part II

Abstract:The paper continues a review of clinical conditions and dilemmas which featured in Asylum practice during the years 1850-1950 in Britain. The rise and fall of 'Physiognomy' as a diagnostic aid and as an indicator of quality of asylum care is outlined. The problem of incontinence and the use of the ice cold douche as a therapeutic intervention in this and other forms of 'indolence or perversion' is reviewed from the writings of those who expressed opinions of these subjects at the time. Likewise, Destructive Behaviour, which was dealt with by means of 'strong clothes', presented a familiar problem. As in Part I of this review, the author concludes with a discussion of the causes and consequences of the changing faces of conditions seen in psychiatric hospitals. Viewed from the present, the changing nature of asylum insanity is apt to be forgotten and adverse judgment is apt to be applied to past practices.
Naming psychiatry: apropos earliest use of the term by Karl Friedrich Burdach (1800)

Abstract:The term psychiatry (Psychiatrie) was first used in 1800, in the early work of Leipzig Romantic natural philosopher and later neuroanatomist Karl Friedrich Burdach; it was a recherché reference to medical animism. This little-known instance of neologism by a young ambitious author invites a brief lexicological study of psychiatry as a specialty in search of its place among the medical specialties, methods and applications. The European historical lexicology of psychiatry recalls the philosophical commentary tradition on Aristotle's De Anima, eventually (c. 1525) honoured with the mononym psychologia. The battle for the soul's science was superseded by the increasingly diverse theoretical, empirical, forensic and literary-humanitarian interests in mental medicine during the second half of the eighteenth century.

Abstract:This article aims to offer clinical descriptions and philosophical interpretations of the ontological nature of depressive experiences trying to address issues related to the contemporary diagnostic paradigm and values-based practices which shape clinical decisions. At the core of this contribution there is the idea that the variety of depressive experiences might be understood acknowledging the qualitative difference in the ontology underlying each form of depression. We argue that there is a fundamental difference between melancholia as a disease and those melancholic traits which characterizes human existence as such: while the former indicates a qualitative alteration of the global human experience, constituting psychopathological experience, the latter describes a style of disproportion of existence not pathological per se at all. Moreover, we defend the hypothesis that melancholia (in a medical sense) is a distinct unity of mental alteration, and should not be conflated with the multivarious kind of experiences merged under the name of depression, the variety of which may be understood as disorders of the personal development. It is in this context that the leading element of anthropological disproportions impact the role of pre-reflective and transcendental structures, polarizing the self and the world in a three-way topography: 1. depression as an excessive symmetric proportion between self and world; 2. depression as a disproportion between the self and the world (detriment of the self); 3. depressions as impoverishment of the transcendental value of the world.

Presently, is given another description from the stylus of the Tamil Pallava king, Mahendravarman of the 6th century CE from his farcical drama of Mattavilasa Prahasana.

Abstract:Work at the intersection of philosophy and psychiatry has an extensive and influential history, and has received increased attention recently, with the emergence of professional associations and a growing literature. In this paper, we review key advances in work on philosophy and psychiatry, and their related clinical implications. First, in understanding and categorizing mental disorder, both naturalist and normativist considerations are now viewed as important – psychiatric constructs necessitate a consideration of both facts and values. At a conceptual level, this integrative view encourages moving away from strict scientism to soft naturalism, while in clinical practice this facilitates both evidence‐based and values‐based mental health care. Second, in considering the nature of psychiatric science, there is now increasing emphasis on a pluralist approach, including ontological, explanatory and value pluralism. Conceptually, a pluralist approach acknowledges the multi‐level causal interactions that give rise to psychopathology, while clinically it emphasizes the importance of a broad range of "difference‐makers", as well as a consideration of "lived experience" in both research and practice. Third, in considering a range of questions about the brain‐mind, and how both somatic and psychic factors contribute to the development and maintenance of mental disorders, conceptual and empirical work on embodied cognition provides an increasingly valuable approach. Viewing the brain‐mind as embodied, embedded and enactive offers a conceptual approach to the mind‐body problem that facilitates the clinical integration of advances in both cognitive‐affective neuroscience and phenomenological psychopathology.

Abstract:The author traces the prestigious history of the Burghölzli, cradle of Swiss psychiatry. He also describes the outstanding role played by Zurich as the city in which integration took place between the developing psychiatry, psycho-analysis, psychodynamics and biological research. The author shows how psychiatry, which had been faced with the mechanistric and philosophical dilemma of an epoch, could disengage from the dangers of a reductive approach thanks to Freud's influence, as well as the knowledge produced by biological and psychopathological discoveries. A new equilibration was, thus, found and a multidimensional model came into being. Today psychiatrists may choose to become either clinicians, or psychoanalysts, or devote themselves to research. Contract has become essential as well as the recognition of these different domains, this in order to improve the understanding of mental disorder.

Abstract:Summary The recent drive within the UK National Health Service to improve psychosocial care for people with mental illness is both understandable and welcome: evidence-based psychological and social interventions are extremely important in managing psychiatric illness. Nevertheless, the accompanying downgrading of medical aspects of care has resulted in services that often are better suited to offering non-specific psychosocial support, rather than thorough, broad-based diagnostic assessment leading to specific treatments to optimise well-being and functioning. In part, these changes have been politically driven, but they could not have occurred without the collusion, or at least the acquiescence, of psychiatrists. This creeping devaluation of medicine disadvantages patients and is very damaging to both the standing and the understanding of psychiatry in the minds of the public, fellow professionals and the medical students who will be responsible for the specialty's future. On the 200th birthday of psychiatry, it is fitting to reconsider the specialty's core values and renew efforts to use psychiatric skills for the maximum benefit of patients

The genealogy of major depression: symptoms and signs of melancholia from 1880 to 1900

Abstract:How deep are the historical roots of our concept of major depression (MD)? I showed previously that psychiatric textbooks published in 1900–1960 commonly described 18 characteristic depressive symptoms/signs that substantially but incompletely overlapped with the current DSM (Diagnostic and Statistical Manual of Mental Disorders) MD criteria. I here expand that inquiry to the key years of 1880–1900 during which our major diagnostic categories of manic-depressive illness (MDI) and dementia praecox were developed. I review the symptoms of depression/melancholia in 28 psychiatric textbooks and 8 other relevant documents from this period including monographs, reviews and the first portrayal of melancholia Kraepelin in 1883. Descriptions of melancholia in the late nineteenth and twentieth century textbooks closely resembled each other, both reporting a mean of 12.4 characteristic symptoms, and emphasizing core features of mood change and alterations in cognitive content and psychomotor behavior. The detailed monographs, reviews and the early description of Kraepelin were more thorough, reporting a mean of 16.6 of these characteristic symptoms. These nineteenth century texts often contained phenomenologically rich descriptions of changes in mood and cognition, loss of interest and anhedonia and emphasized several features not in DSM including changes in volition/motivation, posture/facial expression and derealization/depersonalization. In the early nineteenth century, melancholia was often defined primarily by delusions or as the initial phase of a unitary psychosis transitioning to mania and then dementia. By 1880, the concept of depression as an independent mood disorder with characteristic symptoms/signs and a good prognosis had stabilized. Kraepelin incorporated this syndrome into his diagnostic concept of MDI, changing its name to ‘Depressive States’, but did not alter its underlying nature or clinical description.

Abstract:This article contributes to the history of psychiatry by examining the practice of institutional and community psychiatry in early-twentieth-century Queensland. The source material for this article emerged from a larger project on the history of suicide and a review of the publications and lectures of the state's leading psychiatrist, John Bostock. The paper comes to several conclusions. First, asylums responded to the diversity of illnesses by making case-based judgments about the duration of treatment and the possibility of paroles. Many suicidal patients were not locked up for long periods if their ailments showed promise of alleviation. Second, we suggest that in the interwar period private practice was vibrant and worked interactively with traditional asylum committal. Third, even in rural areas there was awareness by the 1930s of urban-based alternatives to the asylums. Finally, we found evidence of attentive families who pursued several avenues of care for their loved-ones. Ultimately, the findings point to a complex mixed marketplace in psychiatric care during this time period.
