Abstract:To address some challenges facing psychiatrists today we discuss issues of happenstance and fulfilment in psychiatric careers through some of the record and reflections of four psychiatrists since the 1950s. We trace the changes in psychiatry attendant to the transition from the welfare to the neoliberal state and=its contemporary postmodern culture. We highlight the crucial importance of political-cultural as well as technological developments in determining psychiatric service management and provision, and clinical practice and career outcomes. In the light of this impact, in a global era that some highly respected authorities consider in apocalyptic terms, we advocate for the incorporation of training in political awareness and activism in the psychiatric curriculum and practice. We suggest that this is necessary for social justice and patient welfare and that it will help safeguard psychiatric professionalism, conscience and self-esteem.

Abstract:Poetry captures that which lies beyond words. It expresses the rhythms and silences of music reaching areas of experience that exist outside the restrictions of simple prose. In terms of the treatment of poetry in the pages of the IJP and the IRP since their inception, we encounter a commanding story of the history of psychoanalysis itself. In the life of these two journals, we trace a massive shift from "victimisation of literature by psychoanalytic interpreters", as Wilfred Bion described it, to genuine enlightenment. That is, the pages of the journals confirm the rather bleak oversimplifications of many of the early articles, by contrast to the more contemporary psychoanalytic insights into the nature of creativity and the centrality of meaning in psychic and artistic life. The move is towards the aesthetic: towards the relationship between form and content and towards the poetic becoming a vital dimension of understanding the unconscious itself.

Abstract:This paper honours a paediatrician whose career has been patchwork but distinguished, with many facets. It is a light-hearted look at the printed (infinitely preferable to the electronic) word. Although the obvious response is "read everything I have written", this would only drive down the stores of Prozac. Read the scientific literature of course. The oldie reads to stay one jump ahead on the ward round. Meta-analyses and systematic reviews rely on the judgement of others, so should be regarded with suspicion. Do your own research, and do not rely on the thoughts of other people. Read beyond PubMed. "There is more in Heaven and Earth, Horatio, than is dreamed of in your PubMed" (Shakespeare). Read the giants of the past. Richard Asher has so much to teach. What are the seven sins of medicine, and how many have you committed? Asher is top read of the talk. Learn from the mistakes of the past. Your patient comes first. Read how the medical profession covered up the death of Steve Biko and never compromise with patient safety and your integrity. Finally, remember thou art mortal - read a real book for pleasure. Kindle reading must be like making love to an inflatable doll (not that I have done either). Choose what you relax with; for me, PG Wodehouse, Jane Austen, Billy Bunter and Wisden. Who said and why: 'Madam, under similar circumstances I would have kicked the Archangel Gabriel!'.

Abstract:Considered by many the foremost German language literary critic of the first half of the 20 th century, Walter Benjamin remains a star in our contemporary constellation of cultural criticism. His broad range of reference coupled with his dazzling linguistic versatility and radical understanding of technological transformation and its relation to society, continue to offer insights that help us respond better to the 21 century's disorienting pace of innovation and change. Also, to measure the distance we have travelled since his contributions. Here we introduce aspects of his times, life, philosophical formation, and social-historical and critical work, including his use of literary montage and concepts of history, now-time ( Jetztzeit ), non-sensuous similarities, and dialectical images. Benjamin's methodology and discoveries offer penetrating tools to any psychiatry which is truly committed to understanding culture beyond the conservative or descriptive ethnographic study of tradition in order to make sense of the nature and psychopathological impact of accelerating upheavals in our era of international conflict, climate change, social media, artificial intelligence and imperfect and threatened liberal pluralism. This era demands an approach open to co-creation and co-production and we discuss the relevance, including the limitations, of Benjamin's work.

Far from idle: Four ways in which growing knowledge of the 'resting' brain is transforming our understanding of the causes of childhood disorder

Abstract:Historians of science continue to debate the importance of individual inspiration and personal creativity as fuel in the engine of scientific progress. While true that, in general, scientific knowledge advances cautiously by careful experimentation, painstaking observation and the gradual accumulation of evidence occasionally a field of enquiry can be revolutionised by a single, perhaps simple, yet inspired and profound insight. Such breakthroughs are most likely to occur when an individual moves outside the intellectual tramlines that normally constrain scientific thinking, leaving them able to look at old evidence in new and original ways. The reception of such original insights by the research community varies considerably, of course. Some insights may be 'too original'--a step too far in what is normally an incremental journey of discovery. Some ideas, enthusiastically accepted initially, may burn out before making any real impression. Other ideas revolutionize a field--producing a cascade of hypotheses and lines of enquiry that lead to new discoveries which permanently change the scientific landscape. The issue of scientific creativity was very much in my mind when reading through the papers slated to appear in the current journal number. One article in particular, by Pannekoeke and colleagues on intrinsic brain organisation in depressed adolescents, initiated a chain of thought that led me to my focus for this editorial. A development that provides perhaps the most compelling recent example of the transformative power of individual inspiration in the field of cognitive neuroscience--a development which is also beginning to have profound implications for models of childhood mental disorders.

Abstract:The paper explores the phenomenology of major depression and suicidal ideation in children, offering a brief overview of relevant findings in psychiatry, with particular reference to statistical evidence, and outlining psychoanalytic models, including Jungian as well as the post-Kleinian and Bionian vertices. Bowlby's seminal contribution to the understanding of mourning processes in relation to Freud's theory is also considered. The case of an eight-year-old boy is examined, with particular attention to a series of drawings that the patient produced over the two years of his psychoanalytic psychotherapy. The therapy was interrupted abruptly by his parents.

Abstract:Hospitalised children do not only suffer from their illness but in addition from the separation from parents and home (deprivation). The possibility for daily visiting and, if need be, simultaneous admission of the mother has greatly improved this situation. Painful investigations can be extremely stressful and always cause anxieties if the children first are not adequately informed and prepared in advance, and second are separated form their mothers. Such mistakes can be avoided if a certain person of the ward's staff is put in charge of a particular child, as a rule a nurse as a substitute for the mother. The nurse's role is not restricted to helping the doctor; her task includes establishing a personal relationship to the child. Mostly she is the person who knows the child best, and is therefore the primary contact person. For the wellbeing of the hospitalised child she is of utmost importance. Doctor, nurse, kindergarten teacher and psychologist should regard themselves as a common "therapeutical team", discussing the child's problems together and taking action in close touch with the parents.

Abstract:The philosophy of Bruner transcends traditional boundaries in the study of the human mind with a new kind of psychology, one that frees the thinking mind from its opposition to feelings and also from the limitations of being considered an ‘inside-the-head’ phenomenon. It is with active engagement with the outside world that a child develops its understanding. In this engagement with the outside world, the developmental construction of thought is actively created through the use of symbols. The cultural context, images, and languages a person experiences are thus considered to be formative in thinking. Opposing the notion of readiness, Bruner believed children to be capable of complex thought, and the dynamics of these developments were guided by meaning-making. The significance of meaning in Psychology was resurrected in his writing. Furthermore, the notion of narrative as constructive in facilitating the organisation and management of mental processes is invaluable. In this article, we bring a commentary on two articles, one that relates to the study of scaffolding of emotion regulation by parents of adolescents and the other on the narrative understanding of selfhood of individuals with autism.

Abstract:Australasian Psychiatry, Ahead of Print. ObjectivePersonal narratives of lived experience with psychiatric illness and distress remain central in the epistemology of mental illness. We provide a commentary on this potential bridging of patient narrative-based epistemology, and medico-scientific epistemology used by psychiatrists used for diagnosis, formulation, prognosis and treatment.ConclusionDiscussion and planning of psychiatric care can be framed by understanding the narrative-based epistemology of a patient's illness as highlighted by five key questions to explore the patient's illness explanatory models. We propose five key questions for the psychiatrist's complementary consideration of medico-scientific epistemology that frame conceptual models of aetiology, pathophysiology, diagnosis, formulation, prognosis and treatment, which are embedded in the predominant socio-cultural environment. These questions assist in bridging patient narrative and medico-scientific explanatory models to facilitate more effective collaborative care planning.

Abstract:Through a longstanding collaboration, psychiatrists and anthropologists have assessed the impact of sociocultural context on mental health and elaborated the concept of culture in psychiatry. However, recent developments in ecological anthropology may have untapped potential for cultural psychiatry. This paper aims to uncover how "ecologies" inform patients' and clinicians' experiences, as well as their intersubjective relationships. Drawing on my ethnography with Jerome, a carriage driver who became my patient in a shelter-based psychiatric clinic, and on anthropological work about how psychic life is shaped ecologically, I describe how more-than-human relationality and the affordances of various places—a clinic and a stable—influenced both Jerome's well-being and my perceptions as a clinician. I also explore how these ecologies shaped our different roles, including my dual roles as psychiatrist and ethnographer. In the discussion, I define ecological factors, describe their implications for clinical practice, and suggest how they could be integrated into DSM's cultural formulation.

Abstract:Our understanding of the history of psychiatry has been warped by the way history is remembered – collectively and personally; this is the point of departure from which Wynter, Wallis and Ellis have embarked upon this splendid essay anthology. The introduction provokes something of a liminal space by describing psychiatry as a specialty that is ever existing between 'the terrible past and the ideal future'. They argue that barbarity has become a cultural trope for psychiatry, especially when historical treatments are viewed through modern framing without the scaffolding of contemporaneous societal values. Progress seems to always be in response to wanting to heal this iniquity and provide a change in direction. The authors endeavour to show us that the actuality of psychiatry may exist somewhere between these two opposing poles, and furthermore it is interested in the idea that modern mental health services have arisen from within this in-between space. The essays loosely stick to a theme of memory and remembrance, and are arranged within five sections: governance; practitioners; case books; oral histories; and personal recollections. The various essays look at different aspects of psychiatry through different eras and across the globe, with very distinct voices and forms, but this thematic arrangement provides a sense of cohesion. However, there is no onus on the reader to read the whole collection all at once, with most of the pieces having enough strength to be considered in their own right. We see Britain's colonial footprint in the particularly provocative 'Madness, memory and delusion in late nineteenth-century Barbados', whereby Leonard Smith skilfully introduces readers to the cultural matrix of colonial Barbados, where many people were living in destitution, and then goes on to consider a series of nineteenth-century lunatic asylum case records held in the Barbados National Archive. A re-examination of these material impressions of the past acknowledges the colonial gaze through which patients were described, but also provides a new way of seeing patients and their problems, beyond the hermeneutic trouble entailed by the passage of time. A chapter about Bombay's lunatic asylums around the turn of the twentieth century readily relates to this one, but other works describing topics as far reaching as the remodelling of the Sigmund Freud Museum in Vienna and the nostalgia experienced by staff in the wake of an American psychiatric hospital ensure a significant breadth of content. The book ends with a devastatingly honest reflective essay by Allan Beveridge that focuses sharply upon his experience of psychiatric training in Edinburgh in the 1980s. He shares his personal sense of excitement upon embarking upon his career that gives way to unease as he faces the reality of clinical practice, and the discombobulating effect of competing explanations and opposing views that continue to persist, to a greater or lesser extent, to this day. The book grasps onto the Derridean idea that there is an ever-transforming inheritance of the past in light of changing contexts of the future, and that the key to history lies in how it is remembered. Through an exploration of remembrance, this work generates novel insights, which allow us to recast how we understand previous insights into psychiatry's past. This thought-provoking essay collection will generate enthusiasm for new ways of considering the past and reframing the present through troubling our interpretation of what has been written before. It is certainly worth reading to generate reflections on what the legacy of our own work may be.

[The stakes in psychiatric hospitalization. Is an ethical approach possible?]

Abstract:To pose the question of what is at stake in a psychiatric hospitalisation really means to pose the question of its purpose, its justification and ultimately of its underlying ethics. After having defined ethics in its moral and deontological aspects, we shall broach the psychiatric and medical aspect of its politics and relationship to the social field, questions which have become acute because of the fact that they are directly related to our notion of liberty. Certain points shall be more specifically developed: Whom do we serve? To hospitalise, is a medical act. Hospitalisation is a means of protection against Murder and Incest. Hospitalisation is a Third dimension in a Space and Time experience. Our intervention is a logical outcome of our own conception of symptoms and the healing process. What if ultimately, pathology were but the answer to an unverbalized interrogation? To take into consideration, beyond the determinating factors be they genetical, biological, or social, a psychic reality which we recognize in the patient's transfer and his verbalisation, impell a necessary and imperative change in the perspective of the field of conventional psychiatry. To allow oneself to be interrogated by this "Other" discourse, and to assume all the consequences, both theoretical and practical, this is the passing from medical ethics to what might be called analytical ethics.

Abstract:In the course of the psychiatrist-patient relationship, and especially during a psychotherapy, an image sometimes appears that is greeted by the therapist with a sense of surprise and rediscovery. Such an image has the quality of condensing many of the scattered elements of the patient's life and giving them a form that makes it possible to recognise something stable and permanent in the functioning of the patient's subjectivity. I propose to call these images, which have a valence of enlightenment, clarification and condensation of scattered elements, structuring images. I have used the term 'epiphany' to define these moments of enlightenment, which are of great help in psychiatric and psychotherapeutic work, contributing to a new construction of the history and relationships of the patient's life.

Toward a philosophical structure for psychiatry

Abstract:This article, which seeks to sketch a coherent conceptual and philosophical framework for psychiatry, confronts two major questions: how do mind and brain interrelate, and how can we integrate the multiple explanatory perspectives of psychiatric illness? Eight propositions are proposed and defended: 1) psychiatry is irrevocably grounded in mental, first-person experiences; 2) Cartesian substance dualism is false; 3) epiphenomenalism is false; 4) both brain-->mind and mind-->brain causality are real; 5) psychiatric disorders are etiologically complex, and no more "spirochete-like" discoveries will be made that explain their origins in simple terms; 6) explanatory pluralism is preferable to monistic explanatory approaches, especially biological reductionism; 7) psychiatry must move beyond a prescientific "battle of paradigms" to embrace complexity and support empirically rigorous and pluralistic explanatory models; 8) psychiatry should strive for "patchy reductionism" with the goal of "piecemeal integration" in trying to explain complex etiological pathways to illness bit by bit.

Abstract:Psychiatry has the historical opportunity to shape the future of mental health care, medicine, and society. The term New Brave Psychiatry refers to challenges to psychiatry in our volatile, uncertain, complex and ambiguous (VUCA) world. The present and the future of psychiatry lie on the recognizing these challenges and resolving them. Transdisciplinary multiperspective integrative view on psychiatry (TIP) involving creative, person-centered narrative psychopharmacotherapy (CP-CNP) and empathization-based cognitive-emotional-behavioral therapy (EB-CEBT) as an art and practice of the empathic learning organization are presented in this article as well as the culture of empathy as a cornerstone of good clinical practice and mental health. We should all be driven by the vision of psychiatry and other mental health disciplines for a better world and promotion of mental health for all as a part of the Global Enlightenment 2.0 for Sustainability of the Earth System.

Abstract:Background/Objectives: Hospitalization can be stressful for children due to the influence of unfamiliar environments, separation from family, and interactions with healthcare professionals. This study aimed to explore children's hospitalization experiences from a child-centered care perspective to develop interventions that better meet their emotional and psychological needs. Methods: This qualitative study employed Husserl's descriptive phenomenology to explore hospitalization experiences among children aged 9-13 years. Conducted at a primary health center in Spain from October 2022 to June 2023, the study used purposive sampling to select participants hospitalized within the past six months. Data were collected through in-depth interviews and children's self-created drawings, analyzed using thematic analysis with ATLAS.ti software. Results: A total of 10 school-age children (five boys and five girls) were interviewed. Three main themes emerged: (1) Emotions and Feelings-children described fear, anxiety, and loneliness during their hospital stay; (2) Experiences of Pain and Discomfort-participants reported physical pain and discomfort; (3) Interactions with Medical Staff-children expressed a need for more detailed explanations and clearer communication. Conclusions: The findings highlight the importance of family-centered care and improved communication between healthcare providers and children. Strategies like art therapy and flexible visiting hours could provide better emotional support. Tailored communication to children's developmental levels is crucial. Integrating these strategies into clinical practice could enhance the well-being of hospitalized children. Future research should focus on evaluating these interventions to improve pediatric care.
