Abstract:To analyze the biomechanical stability of a redesigned cage, a new lateral plate and the effect of length of cage in CLIF, an L4-L5 finite element model was performed. Six different internal fixation methods were designed and operated under six conditions (Stand-alone CLIF; CLIF with unilateral pedicle screws (CLIF + UPS); CLIF with bilateral pedicle screws (CLIF + BPS); CLIF with lateral plate (CLIF + LP); CLIF with lateral plate and unilateral pedicle screws (CLIF + LP + UPS); CLIF with lateral plate and bilateral pedicle screws (CLIF + LP + BPS)). Ranges of motion (ROM) and stress distribution were evaluated. The effect of the length of cage was analyzed. The ROMs of stand-alone CLIF group and other internal fixation groups were decreased by >90% compared with the intact group. The CLIF + LP + BPS group has the minimum ROM. The CLIF + LP group has smaller ROM than stand-alone group. The stand-alone group has the minimum stress except for extension condition. The CLIF + LP model has less ROM, but a greater stress load was observed in the lateral plate. As for the length of cage, the largest stress is located at the junction between cage and distal end plate, especially in the epiphyseal ring and cortical compact. We conduct a new 'cylinder wall theory' that the cage should be placed to cover the epiphyseal ring. We recommend the length of cage should cover the epiphyseal ring to reduce the subsidence of cage.

Abstract:Abstract Background To analyze the biomechanical stability of the modified LLIF, crenel lateral interbody fusion (CLIF), and compare various methods of instrumentation of CLIF in vitro.Methods Three fresh-frozen cadaveric lumbar spines (L1-S1) were used in our study. The modified CLIF interbody cage was inserted into the L3/4 level in each specimen. Every specimen was tested under 5 conditions: intact group; stand-alone CLIF group; CLIF with lateral plate group (CLIF + LP); CLIF with lateral plate and unilateral pedicle screw group (CLIF + LP + UPS); CLIF with bilateral pedicle screw group (CLIF + BPS).Results The ROM of each CLIF group was significantly reduced when compared with intact group in all directions of loading (p< 0.05). The CLIF + LP + BPS group was the most stable in all directions of loading. CLIF + LP group has less ROM when compared with stand-alone group except for the extension condition. CLIF + BPS group has less ROM than CLIF + LP group in every condition. Conclusions CLIF combine with lateral plate and bilateral pedicel screw is the most stable supplemental fixation, and lateral plate could reduce the ROM under rotation and lateral bending conditions. For patients with good bone quality, stand-alone with or without is a alternative method to achieve a good clinical result.
Biomechanical and Clinical Study of Anterior Transarticular Screw Fixation for Treatment of Traumatic Instability of C1-C2 Joint

Abstract:Objective To evaluate the biomechanical stability of C1-C2 after anterior C1-C2 transarticular screw fixation and assess its curative effect clinically. Methods Eight human cadaveric occipitocervical spine specimens which have been harvested freshly were tested in three modes of loading,flexion-extension, lateral bending, and axial rotation, and four configurations including intact, after an odontoid fracture, and after anterior or posterior C1-C2 transarticular screw fixation. All tests were performed in load and torque control. Clinically, 20 patients who had C1 -C2 traumatic instability were instructed to be operated with anterior C1 -C2 transarticular screw fixation technique, planting cancellated granulate bone into the space between muscular tubercle of arias and odontoid bone. Results Both the anterior and posterior C1-C2 transarticular screw fixation techniques limited motion at the C1-C2 articulation at the tested directions and there were no significant differences between the two techniques. In the 20 patients, one with complete cervical cord injury died of pulmonary infection a month later, and the rest 19 were followed up about 7 months to 3 years (mean 18 months). They all had no trauma on vertebral artery and cervical cord during their cervical operations, and had aflantoaxial synostoses. Conclusion The anterior C1-C2 transarticular screw fixation not only is easy to manipulate and provides the same stability as posterior C1-C2 transarticular screw fixation, but has a less risk of injury on vertebral artery and cervical cord.
Computer Aided Design and Evaluation of New Anatomic Fixation System on Entire Pelvic Model

Abstract:This paper presented a special computer aided procedure to design a new sacroliliac anatomic bar-plate internal fixation system, and evaluated its biomechanical properties on an accurate patient-specific finite element model of entire pelvis, compared with two conventional internal fixation methods. Based on virtual anatomical measure of 30 digital pelvic models reconstructed from CT, an anatomic plate was designed according to the complicated structure of the outer table of the posterior ilium, and was integrated into the complete fixation system. Then, an ad hoc semi-automatic mesh generator was employed to construct a patient-specific finite element model of whole pelvis, including elaborate sacroiliac joints, important pelvic ligaments, and interpubic disc, as well as position-dependent cortical thickness and trabecular bone elastic modulus. Following, one side of sacroiliac joint related ligaments were deleted to simulate a complete unilateral sacroiliac joint disruption. Then the new anatomic fixation system was integrated to fix the fracture, and two comparing models including iliosacral screw fixation and front reconstruction plate fixation were also generated. Finally, all models were simulated under same loading conditions. The results demonstrated that the mechanical stability of the new anatomic fixation system was superior, with obviously improved stress distribution and little displacement, which implied an effective internal fixation method for potential clinical application.

Abstract:Pelvic ring injuries are severe, with an estimated frequency of 3% of all fractures. [ 1 ] Unstable posterior pelvic ring injuries commonly involve disruption of the sacrum, sacroiliac joint, ilium, or a combination of these injuries, [ 2 ] mostly leading to vertical and rotational instability of the pelvic ring. These fractures typically suffer from high-energy traumas, such as traffic accidents and falls from heights, [ 3 ] and are associated with high rates of morbidity and mortality. [ 4 ] Due to destruction of the complex posterior pelvic structures, conservative treatment is usually not an effective therapeutic method. Surgical treatment for unstable pelvic ring injuries has become the gold standard in recent years. [ 5 ] However, the therapeutic outcomes of unstable pelvic ring injuries are far from satisfactory, including chronic pelvic pain and restricted activities of daily living. [ 6 ] Therefore, it remains a great challenge for orthopedic surgeons to choose an appropriate fixation method for the treatment of unstable pelvic ring injuries.

Biomechanical Comparison of Fixation Systems in Posterior Wall Fracture of Acetabular by Finite Element Analysis

Abstract:Background: The use of reconstruction plates and lag screws has been recommended for fractures to the posterior wall of the acetabulum. However, little information about the rigidity of recommended forms of fracture fixation is available. This study aimed to evaluate the biomechanical difference among the fixation systems.Methods: A posterior wall fracture, which is represented by softer elements with lower elastic modulus, was created along an arc of 40-90 degrees about the acetabular rim. Three different fixation systems: screws alone, reconstruction plate with screws, reconstruction plate with T-shaped plates were used to fix the posterior wall fractures to the acetabulum.Results: All three fixation system can be used to obtain good functional outcomes. The reconstruction plate with T-shaped plates was beneficial to increasing the effective stiffness, decreasing the stress concentration and enhancing the rigidity of fracture fixation. So this fixation system served an ideal result in the analysis.Conclusion: Theoretically, the reconstruction plate with T-shaped plates system may reduce many of the risks and limitations compared to the other fixation systems. This fixation system may result in a clinical benefit.

Abstract:Objective: To determine the best method for anterior internal fixation by comparing the biomechanical characteristics of various anterior fixation methods for vertically unstable sacroiliac dislocations.Methods: Eight pelves with Tile C fracture with vertical loading were measured separately. Three‐ and two‐hole anterior double plates were fixed at different angles and compared with sacroiliac screws.Results: For two‐hole anterior double plates, with increasing angle, axial stability increases while lateral stability decreases. For three‐hole plates with an angle of 60°, axial stability is clearly better when placed horizontally, while lateral stability shows no obvious differences.Conclusion: For fixing sacroiliac dislocations, three‐hole anterior double plates placed at about 60° produce excellent stability, which would be greatly increased if screws were added on the iliac side.

Abstract:BACKGROUND:Use biomechanical technique to access the strain distribution and stability of Contour anti-protrusion cage with or without the whole circumference acetabular cup fixation by the transacetabular screws.METHODS:Pelvic specimens from 10 male cadavers were used for the biomechanical test. The models of type C bone defect were copied according to the AAOS classification. Group 1: the contour reconstruction ring was fixed only by flange screws; Group 2: the contour reconstruction ring was fixed both by flange screws and transacetabular screws in dome. Under load in Heel Strike, Midstance, and Toe off phase, the transverse and longitudinal strains were measured in both the superior measure site near the root of the iliac wing and the inferior measure site near the root of the ischium. The relative displacement between the anterior inferior spine and the root of ischium below acetabular component was also measured.FINDINGS:Compared with Group 1, the transverse and longitudinal pressure strain of Group 2 on the superior measure site decreased, and measure sites also showed reductions on the inferior under load under three different gait phases. The result of mean relative displacement showed transacetabular screws fixation couldn't provide significant improvement of displacement prevention.INTERPRETATION:Biomechanical test showed that under the premise of a satisfied bone allograft and well-fixed flange screws in iliac and ischium, the satisfactory initial stability of the reconstruction cup can be achieved, but could not be improved by transacetabular screw technique. However, the max strain in root part of the flange will be reduced using transacetabular screws in ilium, pubis and ischium together, and the strain distribution will also be improved.

Abstract:To compare the biomechanical properties of several anterior pelvic ring external fixators with two new configurations in the treatment of Tile C pelvic fractures, in order to evaluate the effectiveness of the new configurations and provide a reference for their clinical application. A finite element model of a Tile C pelvic ring injury (unilateral longitudinal sacral fracture and ipsilateral pubic fracture) was constructed. The pelvis was fixed with iliac crest external fixator (IC), anterior inferior iliac spine external fixator (AIIS), combination of IC and AIIS, combination of anterior superior iliac spine external fixator (ASIS) and AIIS, and S1 sacroiliac screw in 5 types of models. The stability indices of the anterior and posterior pelvic rings under vertical longitudinal load, left–right compression load and anterior–posterior shear load were quantified and compared. In the simulated bipedal standing position, the results of the vertical displacement of the midpoint on the upper surface of the sacrum are consistent with the displacement of the posterior rotation angle, and the order from largest to smallest is IC, AIIS, ASIS + AIIS, IC + AIIS and S1 screw. The longitudinal displacement of IC is greater than that of the other models. The displacements of ASIS + AIIS and IC + AIIS are similar and the latter is smaller. In the simulated semi-recumbent position, the vertical displacement and posterior rotation angle displacement of the midpoint on the upper surface of the sacrum are also consistent, ranking from large to small: IC, AIIS, ASIS + AIIS, IC + AIIS and S1 screw. Under the simulated left–right compression load state, the lateral displacements of the highest point of the lateral sacral fracture end are consistent with the highest point of the lateral pubic fracture end, and the order from large to small is S1 screw, IC, AIIS, ASIS + AIIS and IC + AIIS, among which the displacements of S1 screw and IC are larger, and the displacements of ASIS + AIIS and IC + AIIS are similar and smaller than those of other models. The displacements of IC + AIIS are smaller than those of ASIS + AIIS. Under the simulated anterior–posterior shear load condition, the posterior displacements of the highest point of the lateral sacral fracture end and the highest point of the lateral pubic fracture end are also consistent, ranking from large to small: IC, AIIS, ASIS + AIIS, IC + AIIS and S1 screw. Among them, the displacements of IC and AIIS are larger. The displacements of ASIS + AIIS and IC + AIIS are similar and the latter are smaller. For the unstable pelvic injury represented by Tile C pelvic fracture, the biomechanical various stabilities of the combination of IC and AIIS are superior to those of the external fixators of conventional configurations. The biomechanical stabilities of the combination of ASIS and AIIS are also better than those of the external fixators of conventional configurations, and slightly worse than those of the combination of IC and AIIS. Compared with sacroiliac screw and conventional external fixators, the lateral stabilities of IC + AIIS and ASIS + AIIS are particularly prominent.

Modified Pedicle Screw-Rod Versus Anterior Subcutaneous Internal Pelvic Fixation for Unstable Pelvic Anterior Ring Fracture: A Retrospective Study and Finite Element Analysis

Abstract:Objectives: This study compared the stability and clinical outcomes of modified pedicle screw-rod fixation (MPSRF) and anterior subcutaneous internal pelvic fixation (INFIX) for the treatment of anterior pelvic ring fractures using Tornetta and Matta grading system and finite element analyses.Methods: In a retrospective review of a consecutive patient series, 63 patients with Orthopaedic Trauma Association (OTA)/Arbeitsgemeinschaft für Osteosynthesefragen (AO) type B or C pelvic ring fractures were treated by MPRSF (n=30) or INFIX (n=33). The main outcome measure were Majeed score, incidence of complications and adverse outcomes, and fixation stability was evaluated by finite element analysis (FEA).Results: two groups did not differ in terms of injury severity score, OTA classification, cause of injury, and time to pelvic surgery. However, the MPSRF group had a higher satisfactory rate according to the Tornetta and Matta grading system than the INFIX group (73.33% vs 63.63%) as well as a higher Majeed score (81.5±10.4 vs 76.3±11.2), which was statistically significant at 6 months’ post-surgery. FEA showed that MPRSF was stiffer and more stable than INFIX and had a lower risk of implant failure.Conclusions: Both MPSRF and INFIX have acceptable biomechanical stability for the treatment of unstable pelvic anterior ring fractures. However, MPRSF has better fixation stability and lower risk of implant failure, which can lead to better clinical outcomes.
Biomechanical comparison of three minimally invasive fixations for unilateral pubic rami fractures

Abstract:Background: To compare the mechanical characteristics of a percutaneous superior pubic intramedullary screw, percutaneous bridging plate and percutaneous screw-rod system of the anterior ring for the treatment of unilateral vertical pubic fractures to provide a reference for clinical application. Methods: A finite element model of pelvic anterior ring injury (unilateral vertical pubic fracture) was produced. The fractures were fixed with a percutaneous superior pubic intramedullary screw, percutaneous bridging plate and percutaneous screw-rod system of the anterior ring and their combinations in 5 types of models. The fracture stabilities under vertical, bilateral and anterior-posterior load were quantified and compared based on the displacement of the hip joints' midpoint as quantificational index of fracture stability. Results: In the condition of bilateral and anterior-posterior load, the vertical, bilateral and anterior-posterior displacements of the hip joints' midpoint of different models were significantly different respectively. In general, the displacements of the 5 pelvic anterior ring fixations were ranked from maximum to minimum as follows: bridging plate, pelvic anterior screw-rod system, combination of bridging plate and pelvic anterior screw-rod system, superior pubic intramedullary screw and combination of superior pubic intramedullary screw and pelvic anterior screw-rod system. Conclusion: For the fixation in unilateral pubic fractures of pelvic ring injury, the percutaneous superior pubic intramedullary screw is optimal, the percutaneous pelvic anterior screw-rod system is the second choice, and percutaneous bridging plate ranks the third. The percutaneous pelvic anterior screw-rod system can significantly increase fixation stability of the percutaneous superior pubic intramedullary screw and the percutaneous bridging plate.
Finite element analysis of a novel anatomical plate in posterolateral plateau fractures

Abstract:Objective: This study aims to analyze the biomechanical characteristics of posterolateral plateau fractures fixed by a novel anatomical plate using finite element analysis. Methods: A three-dimensional digital model of the full length of right tibiofibula was obtained by CT scanning. A posterolateral tibial plateau fracture model was then created. The acquired fracture model was assembled with 4 groups of internal fixations: Group A, novel anatomical plate; Group B, straight buttress plate; Group C, oblique T-shaped locking plate; Group D, two lag screws. Axial loads of 500, 1,000 and 1,500 N perpendicular to the horizontal plane were used to simulate the stress on the lateral plateau of a 65 kg person standing, walking and fast running. Results: Vertical displacements of the posterolateral fragments in each of the four groups gradually increased under loads from 500 N to 1,500 N. The maximum displacement of the fracture fragment in four groups were all located on the lateral side of the proximal part, and the displacement gradually decreased from the proximal part to the distal end. The maximum displacement values under the axial load of 1,500 N was in the following order: novel anatomical plate (1.2365 mm) < oblique T-shaped locking plate (1.314 mm) < two lag screws (1.3747 mm) < straight buttress plate (1.3932 mm). As the axial load increased, the stress value of the different internal fixation models gradually increased. The stress behavior of the same internal fixation model under different loads was similar. The maximum stress value under the axial load of 1,500 N was in the following order: novel anatomical plate (114.63 MPa) < oblique T-shaped locking plate (277.17 MPa) < two lag screws (236.75 MPa) < straight buttress plate (136.2 MPa). Conclusion: The patients with posterolateral plateau fractures fixed with a novel anatomical plate in standing, walking and fast running can achieve satisfactory biomechanical results, which lays the foundation for future applications. At the same time, clinical fracture types are often diverse and accompanied by damage to the soft tissue. Therefore, the ideal surgical approach and appropriate internal fixation must be selected based on the patient's injury condition.
A new technique for percutaneous screw fixation for treating FFP IIIa and IIIb fragility fractures of the pelvis

Abstract:To determine the presence of a consistent osseous corridor from the lateral-posterior aspect of the anterior inferior iliac spine to the sacral wing that could be used for safe trans percutaneous screw fixation for pelvic fragility fractures of the iliac wing and fracture dislocations of the sacroiliac joint (FFP types IIIa and IIIb). Computed tomography (CT) scans were obtained from 100 patients and imported to Mimics software for 3D reconstruction. Then, a cylinder was drawn to imitate the modified LC-II screw and adjusted to a maximum radius and length to obtain the feasible region. Thirteen parameters of the osseous corridor of the modified LC-II screw were measured. Differences between sex groups were compared, and significant statistical correlations were carefully studied to determine potentially important clinical relationships. The records of patients with FFP type IIIa and IIIb fragility fractures of the pelvis were extracted from our hospital. Patients who underwent modified LC-II screw fixation, LC-II screw fixation or reconstruction plate fixation were included. Patients' operative characteristics and complications were recorded at follow-up. Fracture reduction quality was assessed using the Matta standard. Functional outcomes were evaluated using the Majeed grading system. The mean maximum diameters of the osseous corridors of the modified LC-II screw in males and females were 12.73 and 10.83 mm, respectively. The mean maximum lengths of the osseous corridors of the modified LC-II screw in males and females were 96.37 and 93.37 mm, respectively. In the treatment of patients with FFP IIIa and FFP IIIb fractures, the group of treatment by the modified LC-II screws fixation was shown significantly shorter operative time and fewer intraoperative blood loss in comparison to that by the reconstruction plates. In the present study, all the males and females had a complete osseous corridor of the modified LC-II screw. The clinical results of the patients who were treated with modified LC-II screw fixation suggest that the novel method has a good preliminary outcome.

Abstract:Abstract There are various internal fixation methods in treating lumbosacral spinal tuberculosis. The study compared the stability and stress distribution in surrounding tissues/implants, such as discs, endplates and screw-rod internal fixation system, etc. when applying three different lumbar internal fixation methods to treat lumbosacral spinal tuberculosis. A finite element model was constructed and validated. The spinal stability was restored using three methods: a titanium cage with lateral double screw-rod fixation (group 1), autologous bone with posterior double screw-rod fixation (group 2), and a titanium cage with posterior double screw-rod fixation (group 3). For comparison, group 4 represented the intact L3-S1 spine. Finally, a load was applied, and the ranges of motion and Von Mises stresses in the cortical endplates, screw-rod internal fixation system and cortical bone around the screws in the different groups were recorded and analyzed. All six ranges of motion (flexion, extension, left/right lateral bending, left/right rotation) of the surgical segment were substantially lower in groups 1 (0.53° ~ 1.41°), 2 (0.68° ~ 1.54°) and 3 (0.55° ~ 0.64°) than in group 4 (4.48° ~ 10.12°). The maximum stress in the screw-rod internal fixation system was clearly higher in group 2 than in groups 1 and 3 under flexion, left/right lateral bending, and left/right rotation. However, in extension, group 1 had the highest maximum stress in the screw-rod internal fixation system. Group 2 had the lowest peak stresses in the cortical endplates in all directions. The peak stresses in the cortical bone around the screws were higher in group 1 and group 2 than in group 3 in all directions. Thus, titanium cage with posterior double screw-rod fixation has more advantages in immediate reconstruction of lumbosacral spinal stability and prevention of screw loosening.

Modified Internal Fixator for Anterior Pelvic Ring Fractures Versus Conventional Two-Screw Fixation

Abstract:The present study aims to evaluate the short-term clinical effects and complications of modified three-screw fixation and conventional two-screw fixation for treating anterior pelvic ring fractures. A retrospective study of 51 patients with type B fractures was performed. 25 patients (modified group) were treated with modified three-screw fixation and the other 26 patients (conventional group) with conventional two-screw fixation. Outcome measures included operation time, intraoperative blood loss, hospital stays, postoperative complications and the Majeed score at postoperatively 2 months, 3 months, 1 year and the time of implant removal. The mean operative times and mean blood loss for modified three-screw fixation versus conventional two-screw fixation bilateral were 54.8 ± 10.7 min versus 32.3 ± 9.9 min, and 153.3 mL versus 550.0 mL (p < 0.001), respectively. However, the Majeed score was better in modified group at postoperatively 2 months (75.6 ± 9.5 vs. 69.7 ± 8.3, p = 0.008) and 3 months (80.3 ± 10.7 vs. 75.1 ± 11.9, p = 0.014). There was no statistical difference between two groups at the time of implant removal (82.1 ± 9.3 vs. 80.9 ± 8.8, p = 0.272) and postoperatively 1 year (83.5 ± 7.8 vs 82.6 ± 8.2, p = 0.723). No patients experienced surgical wound infection, deep vein thrombosis, delayed union or nonunion, implant loosening or rupture. One patient complained of tardive unilateral thigh pain at postoperatively 4 months in conventional group. Both modified three-screw fixation and conventional two-screw fixation could ultimately afford satisfactory clinical and radiological outcomes with less complication for anterior pelvic ring fractures. The modified three-screw fixation might have better biomechanical strength and faster pelvic rehabilitation.

Abstract:Background: This study aimed to compare the clinical outcomes and complications between two minimally invasive surgical techniques: percutaneous transiliac plate fixation and iliosacral (IS) screw fixation for the treatment of Tile C-type pelvic bone fractures. Methods: We retrospectively reviewed the data of 77 consecutive patients with Tile C pelvic ring injuries who underwent either percutaneous transiliac plate fixation or IS screw fixation in a single academic center between November 2007 and January 2018. We recorded patients' demographics, surgery-related data, and postoperative surgical outcomes and compared the incidence of complications and revision surgery rates between the two groups. Results: Overall, 14 patients were included in the plate group, while 63 were included in the IS screw fixation group. No significant differences were observed in the patients' demographics between the two groups except for a longer interval from injury to surgery (13.5 days vs. 5.4 days, P = 0.001). Both groups acquired fracture union in all cases. There was one case of infection requiring surgical debridement in the plating group. Notably, nerve injury (n = 3) and implant loosening (n = 5) occurred in the IS screw group, but the difference was not significant. Conclusions: Both percutaneous posterior transiliac plating and IS screw fixation in patients with Tile C-type pelvic bone fractures showed good results. We recommend IS screw fixation as the primary treatment and propose posterior plating as treatment for sacral dysmorphism and bilateral sacral alar fractures in patients with spinopelvic dissociation. Independent volumetric internal fixation reduces posterior column acetabular fracture site motion as compared to plate/screw construct: A biomechanical analysis

Abstract:Aims & objectives: To establish whether a suprapectineal pelvic reconstruction plate and posterior column screw (P&S) construct or a single 6.5-mm cannulated posterior column screw (PCS) construct demonstrates greater mechanical stability for fixation of acetabulum fractures involving the posterior column (PC). We hypothesized that the PCS construct would result in less fracture site motion. Materials & methods: Twelve fourth-generation composite hemipelvi were utilized, 6 for each construct. The P&S construct consisted of a suprapectineal pelvic reconstruction plate with two 3.5-mm posterior column screws crossing the fracture site in lag-by-technique fashion and two screws anchoring the plate to the sciatic buttress. The PCS construct consisted of a single 6.5-mm partially threaded cannulated screw placed in an antegrade fashion. Both fixation models were cyclically loaded at 0.5 cycles/second at 400N and 800N, first in a sit-to-stand position that is expected during recovery, and subsequently in a squat-to-stand position to test overload conditions. Results: Under sit-to-stand loading, the PCS construct resulted in less motion at the fracture site than the P&S construct (0.06 ± 0.02 mm vs 0.1 ± 0.02 mm at 400N, p = 0.02; 0.13 ± 0.03 mm vs 0.19 ± 0.04 mm at 800N, p = 0.03). The PCS construct also demonstrated less fracture site motion under squat-to-stand loading (0.22 ± 0.13 mm vs 1.9 ± 0.5 mm at 400N, p = < 0.001; 1.48 ± 0.44 mm vs 4.77 ± 0.3 mm at 800N, p = < 0.001). At 800 N, half of the repairs failed during squat-to-stand loading (2 PCS, 4 P&S). Conclusion: Fixation of the posterior column of the acetabulum with a 6.5-mm cannulated screw demonstrated comparable fracture motion upon loading compared to the plate and screw construct.
Biomedical analysis of four fixation systems in treatment of type II traumatic spondylolisthesis of the axis: a finite element analysis

Abstract:This study aimed to compare the properties and safety of self-designed plates in type II traumatic spondylolisthesis of the axis with those of traditional devices via finite element (FE) analysis. We constructed a hangman's fracture FE model from the occipital bone (C0) level to the C3 level. Then, FE models were constructed for the following four fixation systems: an anterior cervical L-shaped plate with four vertebral screws (4-ACLP), or six screws (6-ACLP), an anterior cervical orion plate (ACOP), and a posterior fixation system. A preloaded compressive force of 50 N and a moment of 1.5 N·m were applied to each model under six working conditions. The mobility of the C2/3 segment decreased significantly in four fixation models. In the Mises stress cloud diagram, 4-ACLP showed a better stress distribution in both the bone graft and fixation system than 6-ACLP and ACOP. The resultant force of 4-ACLP was lower but higher than ACOP in axial force. Additionally, the cage in the 4-ACLP configuration experienced the highest stress in the six working conditions. Hence, this novel self-designed plate has the potential to mitigate the operational difficulties, provide sufficient stability, reduce the risk of plate or screw fractures, and improve bone fusion.

Abstract:One of the traditional surgical approaches for treating cervical spondylosis is anterior cervical discectomy and fusion (ACDF). The conventional titanium plate fixation method and the Zero-P internal fixation method are now the two most used anterior internal fixing techniques. Although these two fixation procedures have been clinically validated in part, their systematic theoretical analysis is still insufficient, and there are several flaws in the application comparison of double-segments cervical spine. The finite element simulation approach is used in this work to perform surgical simulation and stress analysis on two internal fixation methods. Firstly, we established a finite element model of the C3-C7 segments of the lower cervical spine and completed the validity verification of the model. Secondly, a finite element model of the internal fixation surgical method between Zero-P and traditional titanium plates was established. Finally, the cervical spine mobility and the stress of the internal fixation system under different postoperative movements were calculated. The results demonstrate that following typical titanium plate surgery, there is stress concentration in the intervertebral disc endplate, but the simulation results after Zero-P surgery are rather stable. The theoretical life of Zero-P fastening screws is rather limited, and typical titanium plates are prone to fatigue failure around the nail holes. The breadth of the Zero-P cage and the fusion site modification during surgery have a direct influence on the contact between the cervical spine and the instrument following surgery, which is also a major cause of edge fatigue failure.
