Abstract:Heterotopic pregnancy involves the coexistence of both an intrauterine and an extrauterine pregnancy occurring simultaneously. The spontaneous incidence of heterotopic pregnancy in the general population has been estimated to be 1/30,000. This report discusses the case of a 37-year-old female who presented to the emergency department with vaginal bleeding and lower abdominal pain. Upon workup, a transabdominal and transvaginal ultrasound demonstrated a heterotopic pregnancy with an estimated gestational age of seven weeks. The ultrasounds confirmed an intrauterine pregnancy with fetal cardiac activity and a fetal pole along with a right adnexal ectopic pregnancy with fetal cardiac activity and a fetal pole. Additionally, a right paratubal cyst was incidentally found. The patient subsequently underwent exploratory laparotomy with a right salpingectomy for the removal of the right tubal pregnancy and a right paratubal cyst. This case denotes the significance of a carefully performed ultrasound examination, especially in the early weeks of pregnancy. Even when a standard pregnancy is visualized with ultrasound examination, it remains imperative for the examiner to meticulously examine the adnexa and interstitial portion of the fallopian tube. A heterotopic pregnancy has the potential to be life-threatening and can often go undetected, resulting in missed diagnoses.
Early abdominal pregnancy in a spontaneous heterotopic pregnancy: Case report

Abstract:Introduction and significance: Spontaneous heterotopic pregnancies, concurrently occurring intrauterine and ectopic pregnancies, pose a substantial risk to maternal health and are often misdiagnosed. This case report details the challenges in identifying and managing an exceptionally rare case of abdominal pregnancy without assisted reproduction. The patient's initial misdiagnosis underscores the complexities in diagnosis, emphasizing the importance of comprehensive imaging techniques. Case presentation: We present the case of a 36-year-old gravida 5, para 3, with a history of dilation and curettage, experiencing a heterotopic pregnancy involving delayed miscarriage in both uterine and abdominal cavities. Despite presenting symptoms of pelvic pain and abnormal vaginal bleeding, the abdominal pregnancy was initially overlooked in ultrasound examinations. The accurate diagnosis was only achieved post-miscarriage, leading to a timely intervention through laparotomy. Clinical discussion: The absence of identifiable risk factors, except for the patient's history of dilation and curettage, highlights the spontaneous nature of this non-assisted reproduction-related pregnancy. This case emphasizes the challenges in diagnosing and managing spontaneous heterotopic pregnancies, particularly when an abdominal pregnancy is involved. Vigilance and advanced imaging techniques are crucial for early recognition and appropriate intervention. Conclusion: This unique case underscores the difficulties in diagnosing and managing spontaneous heterotopic pregnancies, especially when an abdominal pregnancy is present. Vigilance and advanced imaging are essential to identify rare occurrences like abdominal pregnancies that may go unnoticed in conventional ultrasound examinations. Early recognition and intervention are critical in averting potential life-threatening consequences associated with this uncommon condition.

Abstract:Objectives The coexistence of intrauterine twin pregnancy and ectopic pregnancy (EP), known as heterotopic pregnancy, is a rare but potentially life‐threatening condition. In this study, we aimed to investigate the pregnancy outcomes in women with intrauterine twin pregnancies complicated with EP after assisted reproductive technology. Methods This retrospective study analyzed the medical records of 42 women diagnosed with intrauterine twin pregnancies complicated with EP via ultrasound or surgery at our hospital between January 2005 and December 2020. We collected data on patient general characteristics, high‐risk factors, clinical symptoms, ultrasound features, treatment methods, and pregnancy outcomes. Results Among the 42 included women, 47.6% (20/42) had a history of tubal surgery, while 52.4% (22/42) and 47.6% (20/42) received the transfer of 2 and 3 embryos, respectively. In terms of treatment, 21.4% (9/42) women received expectant management, whereas 78.6% (33/42) underwent surgical treatment, with laparoscopic surgery accounting for 71.4% (30/42). Tracking the pregnancy outcomes revealed a live birth rate of 81.0% (34/42) and a full‐term birth rate of 50.0% (21/42). The preterm birth rate was 31.0% (13/42), with a breakdown of 1 singleton (7.7%, 1/13) and 12 twin births (92.3%, 12/13) among the total 13 premature deliveries. Among the neonates, 33.3% (14/42) were singletons and 47.6% (20/42) twins. Caesarean section accounted for 31 out of 34 deliveries (91.2%). Conclusions Though the incidence of EP in twin pregnancies has declined in the last decade, early diagnosis and proper management are still crucial for favorable outcomes in twin pregnancies with EP.

Abstract:Background: An ectopic pregnancy (EP) is a type of conception in which, the fertilized egg is lodged outside the uterine cavity. Twin ectopic pregnancies are a rarity, and the reported cases of twin tubal pregnancies are a handful to date.Case Report: We report a case of a 35 years old patient who presented to the emergency with the complaint of lower abdominal pain and intermittent vaginal discharge. She was diagnosed with twin tubal alive gestation, underwent exploratory laparotomy and right salpingectomy.Conclusion: Ectopic pregnancy can occur even in the absence of known risk factors. Its incidence is on the rise. It is a leading cause of first-trimester maternal deaths and can be easily diagnosed with Beta HCG levels and transvaginal ultrasound.

Abstract:RATIONALE:Ectopic pregnancy (EP) is a condition in which a fertilized oocyte implants outside the uterine. Spontaneous bilateral tubal pregnancies are the rarest form of ectopic and are considered spontaneous when no fertility treatments are involved. There are few prior cases. Here I report a case of spontaneous bilateral tubal pregnancy.PATIENT CONCERNS:A 24-year-old female with long lasting vaginal bleeding of 29 days duration.DIAGNOSES:Transvaginal ultrasound observed small complex masses on both adnexal sides without intrauterine gestational sac, and the hCG level was 21438 IU/L. But diagnosing based only on imaging findings and blood test result was difficult.INTERVENTIONS:Operative laparoscopy with salpingectomy bilaterally.OUTCOMES:Villi were clearly shown in both resected tubes. A spontaneous bilateral tubal pregnancy was confirmed finally and the patient recovered uneventfully.LESSONS:Women of reproductive age with irregular menstruation should get access to diagnosis and appropriate treatment without delay. Salpingotomy should also be considered if the patient had a strong wish for future pregnancy. Developments of diagnosis and management of such condition are strongly called for.

An Early Extra Peritoneal Ectopic Pregnancy Successfully Treated With Laparoscopy: A Case Report

Abstract:Abdominal pregnancy is very rare. Compared to normal and ectopic pregnancies, it has a higher maternal mortality, perinatal mortality and morbidity. We report the case of a 32-year-old G4P1 female of extra peritoneal ectopic pregnancy. There was a past history of tubal pregnancy on the right side in Dec 2012 and she was treated completely by an abdominal salpingectomy. The patient was again diagnosed as ectopic pregnancy due to the largely increased beta-HCG, while the location of the sac was still not found. Transverse CT of the abdomen showed a low density circular shadow next to abdominal aorta at the fourth lumbar level. The patient was diagnosed with an abdominal pregnancy by ultrasound and then cured by laparoscopy. The combination of ultrasound and beta-HCG plays an important role in diagnosing ectopic pregnancy. And laparoscopy cured the abdominal pregnancy successfully, when the effect of MTX in intramuscular injection on ectopic pregnancy was not significant.
Conjoined Twins in a Spontaneous Monochorionic Triplet Pregnancy: A Case Report and Literature Review.

Abstract:Rationale: Conjoined twins are a rare complication of monochorionic pregnancies and an extremely rare condition in spontaneous triplet pregnancies. We report a case of conjoined twins in a spontaneous monochorionic triplet pregnancy. The conjoined twins might have suffered from twin anemia-polycythemia sequence, which was reported to be extremely rare. Patient concerns: A 26-year-old woman conceived spontaneously with an obstetric history of invasive mole 4 years ago. Diagnoses: We initially misdiagnosed her as having monochorionic triamniotic triplets at 10 weeks of gestation. However, we confirmed conjoint twins with the monochorionic diamniotic triplet pregnancy at 12 weeks of gestation and classified them as omphalopagus. Interventions: As the woman decided to continue the pregnancy, regular and careful antenatal care was conducted. Outcomes: Unexpectedly, she had a stillbirth 3 weeks later and had to terminate the pregnancy at 15 weeks of gestation. After abortion, the diagnosis of omphalopagus was confirmed in the induced fetuses. Moreover, the skin colors of the conjoined twins were different: one was plethoric, and the other was pale. Additionally, the parents agreed to examine the chromosome of the fetuses, and the results were normal. Conclusion: Dichorionic triplet and monochorionic triplet pregnancies have a poorer prognosis than trichorionic triplet pregnancies. Surgery is the main therapy for conjoined twins; however, most conjoined twins in triplet pregnancies cannot survive, including omphalopagus twins. The conjoined twins may have suffered from twin anemia-polycythemia sequence, which could probably not be diagnosed intrauterine. Transvaginal probe and 3-dimensional ultrasound may be helpful for clarifying the diagnosis in early pregnancy.

Abstract:Heterotopic pregnancies are a rare phenomenon defined by the simultaneous occurrence of intrauterine and extrauterine pregnancy. The incidence of heterotopic pregnancy occurring through natural fertilization is low but is increased by risk factors such as assisted reproductive techniques or pelvic inflammatory disease increase. We report the case of a 36-year-old female patient in the 6th week of pregnancy who presented to the emergency unit with severe acute lower abdominal pain. Laboratory chemistry and sonography revealed a suspected heterotopic pregnancy. The patient was admitted for observation. The sonographic reevaluation on the next day confirmed the suspicion of extrauterine gravidity with intact intrauterine gravidity with additional decreasing hemoglobin and hematoperitoneum, so that laparoscopy was indicated. Intraoperatively, the mass on the left ovary was removed without complications. The patient could be discharged quickly postoperatively after a complication-free course and gave birth to a healthy child by spontaneous partus in the 38th week of gestation. Due to their rarity, there is limited research related to heterotopic pregnancies, and most scientific articles are case studies. Diagnostically, the most important thing in clinical practice is to think about the possibility of EUG even if there is evidence of an intact IUG. Transvaginal sonography is of particular importance in diagnostics. The performance of prospective cohort studies is desirable for the evidence-based diagnosis and therapy of affected patients in the future. Heterotope Schwangerschaften sind ein seltenes Phänomen, welches durch das gleichzeitige Auftreten einer intrauterinen und extrauterinen Schwangerschaft definiert wird. Die Inzidenz durch natürliche Befruchtung auftretender heterotoper Schwangerschaften ist niedrig, wird jedoch durch Risikofaktoren, wie assistierte Reproduktionstechniken oder Entzündungen des Beckens, erhöht. Wir berichten über den Fall einer 36-jährigen Patientin in der 6. SSW, die sich mit akuten Unterbauchschmerzen in der Notaufnahme vorstellte. Laborchemisch und sonographisch stellte sich bei der Patientin der V.a. eine heterotope Schwangerschaft mit Lokalisation am Ovar. In der sonographischen Reevaluation erhärtete sich am Folgetag bei sinkendem Hämoglobin und Hämatoperitoneum der V.a. eine heterotope Gravidität, sodass die Indikation zur Laparoskopie gestellt wurde. Intraoperativ konnte die Raumforderung am linken Ovar entfernt werden. Die Patientin konnte nach komplikationslosem postoperativem Verlauf rasch entlassen werden und brachte in der 38. SSW per Spontanpartus ein gesundes Kind auf die Welt. Aufgrund ihrer Seltenheit gibt es wenige große Studien in Bezug auf heterotope Schwangerschaften, die meisten wissenschaftlichen Artikel sind Fallstudien. Diagnostisch ist im klinischen Alltag vor allem wichtig bei Nachweis einer intakten IUG an die Möglichkeit einer EUG zu denken. Besondere Bedeutung hat in der Diagnostik die transvaginale Sonographie. Die Durchführung prospektiver Kohortenstudien ist für die evidenzbasierte Diagnostik und Therapie der betroffenen Patientinnen in Zukunft wünschenswert. Monochorionic quadramniotic and triamniotic pregnancies following single embryo transfers: two case reports and a review of the literature

Abstract:Purpose The purpose of this study is to report two cases of monozygotic quadruplet and triplet pregnancies following single embryo transfer (ET). Methods A 29-year-old woman and a 34-year-old woman underwent ART treatment in two affiliated University based ART units. The first woman underwent ICSI with day 3 embryo biopsy for pre-implantation genetic diagnosis (PGD) followed by day 4 transfer, which resulted in a monochorionic quadramniotic (MCQA) quadruplet pregnancy. The second woman underwent conventional IVF with transfer of a single blastocyst, which resulted in a monochorionic triamniotic (MCTA) triplet pregnancy. Results The first patient underwent successful selective foetal reduction at 16 + 3 and 17 + 4 weeks of gestation. Two healthy twin girls were delivered by elective caesarean section at 35 + 6 weeks of gestation. The second patient underwent successful selective foetal reduction at 14 + 1 weeks of gestation. The remaining monochorionic diamniotic (MCDA) twins are well at the time of writing this article. Conclusions To our knowledge, these cases represent the first case of viable MCQA pregnancy following single ET in the world and the third case of a viable MCTA pregnancy following conventional IVF with single ET. Several factors including blastocyst stage transfer and zona pellucida manipulation have been thought to contribute to monozygotic twinning in the context of ART. These two cases add to the growing literature of monozygotic multiple pregnancies following ART.
Heterotopic Pregnancy: Case Series and Review of Diagnosis and Management

Abstract:Introduction: Heterotopic pregnancy (HP) refers to the simultaneous presence of intrauterine pregnancy (IUP) and ectopic pregnancy, which is very rare but potentially life-threatening. The spontaneous incidence of HP in the general population is 1/30,000. With the widespread use of assisted reproductive technology (ART), the incidence rises to 1/1,000. Aims and Methods. This is a prospective case series looking at the cases of heterotopic pregnancies presenting to the early pregnancy unit (EPU) in a tertiary maternity hospital, from November 2015 to November 2016. The clinical presentation, ultrasound findings, and laparoscopy findings were all documented. The incidence of HP was calculated and compared with the quoted incidence in the literature. Outcomes. Five women with HP presented to the EPU over the course of a year. The first case describes a spontaneous HP with a previous salpingostomy. The second case describes an HP following ovulation induction. The third case describes a spontaneous HP with no known risk factors. The fourth and fifth cases describe heterotopic pregnancies following in vitro fertilisation with more than one embryo. All five cases of HP underwent laparoscopy and salpingectomy with uneventful recovery. The three women who had a viable IUP had no further complications in their pregnancies. Conclusion: Early and accurate diagnosis of HP can be challenging. An early transvaginal ultrasound plays an important role in making the diagnosis in women with risk factors and following ART. A high index of suspicion is required for timely diagnosis and appropriate intervention, especially in spontaneous HP.

Abstract:RATIONALE:The recently increased rate of heterotopic pregnancies (HPs) has been largely attributed to the increased use of assisted reproduction technologies (ARTs). HP is a rare and potentially life-threatening condition. It is unusual in natural conception cycles, occurring in 1:10,000 to 1:50,000 pregnancies. However, with the increased use of ART such as in vitro fertilization and embryo transfer (IVF-ET), the incidence has risen to 0.5-1%.PATIENT CONCERNS:Case 1 was a 28-year-old woman who presented to our center complaining of a sudden onset of right-side lower abdominal pain with a small amount of vaginal bleeding. She had undergone IVF-ET and received a thawed embryo transfer with two embryos 23 days previously. She had a history of right salpingectomy for an ectopic pregnancy during the downregulation of her ovulatory cycle 1 year ago. Case 2 was a 25-year-old woman who presented to our center complaining of a sudden onset of right-side lower abdominal pain. She had also undergone thawed embryo transfer following IVF-ET with two embryos 35 days previously. She had a history of right salpingectomy for an ectopic pregnancy 1.5 years previously.DIAGNOSES:Both patients were diagnosed as having a heterotopic pregnancy.INTERVENTIONS:Patient 1 underwent emergency laparoscopy; patient 2 underwent emergency laparotomy and both were treated medically to prevent abortion of the intrauterine pregnancies.OUTCOMES:Patient 1 had an incomplete abortion and underwent uterine curettage on the day 10 after the operation. Patient 2 experienced no further complications during pregnancy and a healthy baby girl was born at the 38th gestational week.LESSONS:Reproductive physicians need to pay more attention to patients who have received more than one embryo at transfer, especially those with a history of salpingectomy.

Abstract:Background. Triplet pregnancy with complete hydatidiform mole and coexisting twin fetuses is extremely rare with an unknown incidence. Case. Here, we present a case report of a pregnancy with twin fetuses and concurrent hydatidiform mole that resulted in the preterm delivery of one viable baby, the unfortunate intrauterine demise of the other twin, and successful treatment of gestational trophoblastic neoplasia in the postpartum period. Conclusion. This case highlights several important questions that arise for women who choose to carry a multiple gestation pregnancy with complete hydatidiform mole and describes complications that can occur. It is imperative to accurately assess risks and counsel individuals who elect to carry these pregnancies to provide the best possible outcomes.
A Triamniotic/monochorionic Triplet Pregnancy Established after Day 2 Single Embryo Transfer with in Vitro Fertilization/embryo Transfer (IVF-ET)

Abstract:OBJECTIVE: To present the first known case of a viable triamniotic/monochorionic triplet pregnancy case after single embryo transfer.DESIGN: Case Report.MATERIALS AND METHODS: A 35-year-old Gravida 1 Para 1 with approximately one year of secondary infertility. Her husband has a history of mild male factor. Her first baby was conceived on the fifth cycle of controlled ovarian hyperstimulation (COH) with intrauterine insemination (IUI). She generally had generated one to three mature follicles at the time of HCG trigger with aggressive doses of gonadotropins (300 to 450 IU daily). The patient breastfed for one year and then began to pursue fertility in 2004. She had an elevated day 3 FSH (12 mIu/mL) in September, 2006. She completed two additional cycles of COH/IUI cycles prior to proceeding to IVF/ Intracytoplasmic Sperm Injection (ICSI) treatment. She received 7195 IU of gonadotropins with a GnRH antagonist protocol. On October 12, 2006 three ova were retrieved. Post wash semen sample revealed 14 million spermatozoa/mL, with 35% progressive motility, with < 5% normal morphology. ICSI was performed on two metaphase-2 oocytes (the third oocyte was atretic). One two-pronuclear embryo was observed 18hrs post-ICSI. The embryo was cultured in Life Global medium with 5% Serum Substitute Supplement (SSS, Irvine Scientific, Cat #99193) and 3% Human Serum Albumin (HSA, Irvine Scientific, Cat#9988). One four-cell good quality embryo was transferred following assisted hatching (with acid Tyrode's solution) 47 hours post ova retrieval.RESULTS: The patient's serum hCG level was 120 mIU/ml on 10/26/2006, twelve days after embryo transfer. On 12/04/2006, at 9 weeks 3 days gestation, a transvaginal ultrasound confirmed a triamniotic/monochorionic triplet pregnancy, each with fetal cardiac activity. The patient subsequently delivered monozygotic female triplets on May 20, 2007 at 33 weeks and 3 days gestation via cesarean section. All three infants were eventually discharged in good condition and have had no identifiable neurological or physical abnormalities.CONCLUSIONS: To our knowledge, this is the first known case of viable triamniotic/monochorionic triplet pregnancy following a single embryo transfer in IVF-ET.
