Abstract:INTRODUCTION Static bone histomorphometry was applied to existing iliac bone sections originating from a 370-d 5 degrees head-down bed rest experiment. This bed rest experiment is the longest ever to have been conducted. We hypothesized that bed rest would decrease cancellous bone volume fraction and that this effect would be reversed by countermeasures. METHODS Eight healthy male subjects underwent 370 d of 5 degrees head-down bed rest. Three subjects were treated with bisphosphonate (Xidifon, potassium salt of ethane-1-hydroxy-1-disphosphonate, EHDP) combined with an exercise regimen (1-2 h x d(-1)) for the entire study period. Five subjects underwent 120 d of bed rest without countermeasures followed by 250 d of bed rest with the exercise regimen. Transiliac bone biopsies were obtained either at baseline and day 366, or at baseline, day 116, and day 366 at alternating sides of the ileum. Static histomorphometry was performed using a computerized method. RESULTS The 120 d of head-down bed rest without countermeasures resulted in decreased bone volume fraction BV/TV (-6.3%, p = 0.046) and trabecular number (Tb.N; -10.2%, p = 0.080) and increased trabecular separation (Tb.Sp; 14.7%, p = 0.020), whereas the 250 d of subsequent head-down bed rest with exercise treatment prevented further significant deterioration of the histomorphometric measures. DISCUSSION The 120 d of 5 degrees head-down bed rest without countermeasures induced significant deterioration of iliac crest cancellous bone histomorphometric properties. On average, the countermeasures consisting of either bisphosphonate and exercise, or exercise alone were able to either prevent or stop immobilization-induced changes of the iliac cancellous bone structure.

Abstract:Introduction: Heterotopic pregnancy (HP) refers to the simultaneous presence of intrauterine pregnancy (IUP) and ectopic pregnancy, which is very rare but potentially life-threatening. The spontaneous incidence of HP in the general population is 1/30,000. With the widespread use of assisted reproductive technology (ART), the incidence rises to 1/1,000. Aims and Methods. This is a prospective case series looking at the cases of heterotopic pregnancies presenting to the early pregnancy unit (EPU) in a tertiary maternity hospital, from November 2015 to November 2016. The clinical presentation, ultrasound findings, and laparoscopy findings were all documented. The incidence of HP was calculated and compared with the quoted incidence in the literature. Outcomes. Five women with HP presented to the EPU over the course of a year. The first case describes a spontaneous HP with a previous salpingostomy. The second case describes an HP following ovulation induction. The third case describes a spontaneous HP with no known risk factors. The fourth and fifth cases describe heterotopic pregnancies following in vitro fertilisation with more than one embryo. All five cases of HP underwent laparoscopy and salpingectomy with uneventful recovery. The three women who had a viable IUP had no further complications in their pregnancies. Conclusion: Early and accurate diagnosis of HP can be challenging. An early transvaginal ultrasound plays an important role in making the diagnosis in women with risk factors and following ART. A high index of suspicion is required for timely diagnosis and appropriate intervention, especially in spontaneous HP.

Abstract:Background: Coexistence of an intrauterine pregnancy and an ectopic pregnancy is termed a heterotopic pregnancy. The ectopic pregnancy can be tubal, cornual, cervical, ovarian, or abdominal. The incidence of heterotopic pregnancy with natural conception is ∼1:30000. This article reports on a patient with a heterotopic pregnancy after spontaneous conception, which is very rare.Case: A 25-year-old primigravida at 11 weeks of pregnancy presented with an acute abdomen along with vomiting and vaginal bleeding. An examination revealed that she had a tense and tender abdomen. Her uterus was equivalent to 14 weeks' pregnancy. On vaginal examination, tenderness and fullness was present in the left side of her pelvis. A transvaginal ultrasound finding was suggestive of a ruptured left-sided tubal pregnancy with a live intrauterine fetus. The patient underwent emergency laparotomy followed by a left salpingectomy.Results: This patient's postoperative period was uneventful.Conclusions: Early clinical diagnosis should be performed, and a high index of suspicion for heterotopic pregnancy should be maintained in any pregnant woman presenting with an acute abdomen.

Abstract:Heterotopic pregnancies are a rare phenomenon defined by the simultaneous occurrence of intrauterine and extrauterine pregnancy. The incidence of heterotopic pregnancy occurring through natural fertilization is low but is increased by risk factors such as assisted reproductive techniques or pelvic inflammatory disease increase. We report the case of a 36-year-old female patient in the 6th week of pregnancy who presented to the emergency unit with severe acute lower abdominal pain. Laboratory chemistry and sonography revealed a suspected heterotopic pregnancy. The patient was admitted for observation. The sonographic reevaluation on the next day confirmed the suspicion of extrauterine gravidity with intact intrauterine gravidity with additional decreasing hemoglobin and hematoperitoneum, so that laparoscopy was indicated. Intraoperatively, the mass on the left ovary was removed without complications. The patient could be discharged quickly postoperatively after a complication-free course and gave birth to a healthy child by spontaneous partus in the 38th week of gestation. Due to their rarity, there is limited research related to heterotopic pregnancies, and most scientific articles are case studies. Diagnostically, the most important thing in clinical practice is to think about the possibility of EUG even if there is evidence of an intact IUG. Transvaginal sonography is of particular importance in diagnostics. The performance of prospective cohort studies is desirable for the evidence-based diagnosis and therapy of affected patients in the future. Heterotope Schwangerschaften sind ein seltenes Phänomen, welches durch das gleichzeitige Auftreten einer intrauterinen und extrauterinen Schwangerschaft definiert wird. Die Inzidenz durch natürliche Befruchtung auftretender heterotoper Schwangerschaften ist niedrig, wird jedoch durch Risikofaktoren, wie assistierte Reproduktionstechniken oder Entzündungen des Beckens, erhöht. Wir berichten über den Fall einer 36-jährigen Patientin in der 6. SSW, die sich mit akuten Unterbauchschmerzen in der Notaufnahme vorstellte. Laborchemisch und sonographisch stellte sich bei der Patientin der V.a. eine heterotope Schwangerschaft mit Lokalisation am Ovar. In der sonographischen Reevaluation erhärtete sich am Folgetag bei sinkendem Hämoglobin und Hämatoperitoneum der V.a. eine heterotope Gravidität, sodass die Indikation zur Laparoskopie gestellt wurde. Intraoperativ konnte die Raumforderung am linken Ovar entfernt werden. Die Patientin konnte nach komplikationslosem postoperativem Verlauf rasch entlassen werden und brachte in der 38. SSW per Spontanpartus ein gesundes Kind auf die Welt. Aufgrund ihrer Seltenheit gibt es wenige große Studien in Bezug auf heterotope Schwangerschaften, die meisten wissenschaftlichen Artikel sind Fallstudien. Diagnostisch ist im klinischen Alltag vor allem wichtig bei Nachweis einer intakten IUG an die Möglichkeit einer EUG zu denken. Besondere Bedeutung hat in der Diagnostik die transvaginale Sonographie. Die Durchführung prospektiver Kohortenstudien ist für die evidenzbasierte Diagnostik und Therapie der betroffenen Patientinnen in Zukunft wünschenswert. Abstract:Introduction: Heterotopic pregnancy, defined as simultaneous intrauterine and ectopic pregnancy, is a rare and potentially life-threatening condition. The incidence of heterotopic pregnancy has significantly increased in the modern era, primarily due to use of assisted reproductive technology. Heterotopic pregnancy in the absence of risk factors is uncommon. The symptoms of heterotopic pregnancy are similar to those of ectopic pregnancy, primarily abdominal pain and vaginal bleeding. Case report: We report a case of heterotopic pregnancy occurring in the absence of risk factors and presenting with primary symptoms of chest pain and shortness of breath. Conclusion: While uncommon, heterotopic pregnancy may occur in patients without risk factors and may present with atypical symptoms such as chest pain and shortness of breath.

Abstract:Objective To analyze the prevalence rate,diagnosis,treatment,prognosis of heterotopic pregnancy(HP) after in vitro fertilization and embryo transfer(IVF-ET).Methods The clinical data of 14 patients diagnosed HP after IVF-ET were reviewed.Results Eight cases had history of tubal or chronic pelvic inflammation.One case had chronic pelvic inflammation and bicornuate uterus.Three cases had chronic pelvic inflammation and oligozoo-spermia.One case had azoospermia.One case had oligo-ovulation and oligozoo-spermia.All of them were diagnosed by sonography before surgery.Ten patients had salpingectomy by laparoscopy and one by laparotomy.Two patients of corneal pregnancy had laparoscopy and laparotomy respectively.one had salpingectomy in other hospital.After surgery,9 cases had ongoing intrauterinc pregnancy,5 cases were inevitable abortion.Conclusions Previous tubal or pelvic inflammation disease are the most common risk factors for HP.The sonographic examination is reliable in the diagnosis of HP.Early treatment can improve the prognosis obviously.Laparoscopic salpingectomy is still considered to be the best method for the treatment of HP.
The C-Type Lectin Receptor DCIR Is Crucial for the Development of Experimental Cerebral Malaria

Abstract:Cerebral malaria (CM) is the most severe complication of malaria. The murine Plasmodium berghei ANKA (PbA) infection model has helped to identify crucial players in the pathogenesis of CM. However, the role of pattern recognition receptors in innate immunity to CM induction is still poorly understood. C-type lectin receptors (CLRs) represent a family of pattern recognition receptors that recognize carbohydrate structures on pathogens and self-Ags often in a Ca2+-dependent manner. In this study, we investigated the role of the CLR dendritic cell immunoreceptor (DCIR) in the genesis of CM. Using the murine PbA infection, we show in this article that DCIR is essential for the development of CM. Although PbA infection led to 80% CM in wild-type C57BL/6 mice, DCIR-deficient mice were highly protected with only 15% CM development. In accordance with the reduced CM incidence in DCIR−/− mice, CD8+ T cell sequestration was markedly reduced in brains of PbA-infected DCIR−/− mice, which was accompanied by reduced brain inflammation. Reduced T cell sequestration in the brain was caused by decreased TNF-α levels in sera, as well as a modulated activation of CD4+ and CD8+ T cells in spleen of PbA-infected DCIR−/− mice. This study indicates that DCIR is critically involved in CM induction, thus highlighting the importance of this CLR in innate immunity during malaria infection.
Clinical Analysis of 50 Patients with Heterotopic Pregnancy after Ovulation Induction or Embryo Transfer.

Abstract:Objectives: The purpose of this study was to evaluate the clinical characteristics, prenatal diagnosis, and management of patients with heterotopic pregnancy after ovulation induction or embryo transfer. Methods: This was a retrospective study of fifty cases with heterotopic pregnancy, in which the fertilization way, type and number of embryo transferred, gestational age, clinical presentation and outcome of intrauterine, ultrasound presentation and site of ectopic pregnancy, bilateral fallopian tube and treatment were evaluated. Results: Six patients had spontaneous pregnancy and two had artificial insemination after ovulation induction. Sixteen had fresh and nineteen had frozen embryo transfer with seven patients unrecorded its embryo type and number. The average days from transplantation (or intercourse/insemination) to diagnosing heterotopic pregnancy was thirty-seven with the earliest eighteen and the latest more than 70 days. Although the most common presentation was vaginal bleeding or abdominal pain, more than 21% was found by ultrasound and rare individuals even presented with gastrointestinal symptom which may imply ruptured EP and hemorrhagic shock. Giving proper treatment (surgery or local drug injection), the majority of them had a successful intrauterine pregnancy with only seven miscarried. Conclusions: Ovulation induction or embryo transfer increased the risk of HP greatly and clinician should raise high suspicious during the whole first trimester. Although the most ectopic site was ampullary, other sites such as cornual, cervical, abdominal especially interstitial or tubal stumps should also be assessed by ultrasound even in patients with bilateral salpingectomy or tubal obstructed. Repeated ultrasound tests 2 weeks after the diagnosis of intrauterine pregnancy with heart beating was very necessary to find the missed ones in suspicious patients. Compared with surgery, embryo suction with or without proper local drug injection would be more advisable for patients with cervical, cornual, or interstitial pregnancy in order to reserve the intrauterine pregnancy.

Abstract:Heterotopic pregnancy is defined by the coexistence of intrauterine and ectopic pregnancy. In our case report, a 41-year-old woman with a positive pregnancy test visited our hospital outpatient department for the first time. She was diagnosed with a viable heterotopic pregnancy, for which she underwent laparotomy and uterine curettage. Our clinical case has a prevalence of one in every 30.000 pregnancies, since it was a spontaneous conception and there was no family history of multiple pregnancies. Risk factors were medical history of a previous ectopic pregnancy, pelvic operations and her age being over forty. Despite her good recovery from the first operation, she underwent a second laparotomy with the aim to treat an obstructive ileus due to adhesions.

Abstract:Electrophoresis is among the most important modern physiochemical methods; however, the important early experiments in this field were performed in the first years of the 19th century. Reuss, 1 in 1807, noted the electro-osmosis of water through clay by the passage of electric current. Porrett, 2 in 1876, observed the migration of water through bladder under the influence of galvanic current and also extended the experiments to filter paper impregnated with egg albumen. To date, there are no cases of heterotopic pregnancy associated with amniotic embolism described in the literature. Therefore, we report the case of a pancreatic heterotopic pregnancy, which led to amniotic embolism and an unfavorable maternal outcome.

Abstract:Objectives:To investigate the clinical manifestations,diagnosis,therapy and outcomes of heter-otopic pregnancy after in vitro fertilization and embryo transfer (IVF -ET).Methods:3 cases of heterotopic preg-nancy after IVF -ET were analyzed retrospectively between January 2001 and December 2013 in Peking University First Hospital.Results:The 1st patient received emergency laparotomy bilateral tubal resection for the broken of sac caused by ruptured ectopic pregnancy.And the pregnancy continues until found abortion without any obvious incen-tive after 16 weeks.The 2nd one found intrauterine fetal demise when admitted into the hospital.And the ultrasound found ectopic gestational sac,then the patient received curettage and laparoscopic tubal resection.The last patient was found heterotopic pregnancy by transvaginal ultrasound and was received salpingectomy (right tube removed) with laparoscopic.Now the intrauterine gestational sac continued to develop.Conclusion:It is difficult to diagnose at the early stage of heterotopic pregnancy for the shortage of typical clinical manifestations.Clinicians should pay more attention to it and take timely treatment based on surgery,so as to protect patients,their fertility and embryo.

Abstract:Heterotopic pregnancy refers to the coexistence of intrauterine and ectopic pregnancies. The incidence of this rare disease significantly increased with application of assistant reproductive technology. The ectopic pregnancies usually occur in the fallopian tube, cervix, ovary, abdomen and previous cesarean scar. Because of the atypical clinical manifestations and the interference of intrauterine pregnancy, many cases could be missed diagnosis, misdiagnosed and delayed treatment, which always leads to serious complications. The treatment options for heterotopic pregnancy include expectant treatment, fetal reduction, surgery, and so on. The treatment should be individualized, based on patient′s preference, ectopic pregnancy site, clinical symptoms and signs. At the same time, it is still the treatment challenges how to avoid the impact of the treatment procedure of ectopic pregnancy on intrauterine pregnancy, for those patients who want to keep their intrauterine pregnancies.

Abstract:Introduction: Ectopic or extrauterine pregnancy is a life-threatening medical condition in which a fertilized egg is implanted elsewhere than in the uterine cavity, commonly in the fallopian tubes. However, it can also occur in the peritoneal cavity, ovaries, and cervix. There are several etiological factors contributing to ectopic pregnancies, such as a history of pelvic inflammatory diseases, habitual abortions, and miscarriages, previous ectopic pregnancies, fertility procedures, as well as increased maternal age that has been correlated with a higher risk of carrying an ectopic pregnancy. Often, the first alerting sign in any ectopic pregnancy is a pain in the pelvic region, along with vaginal bleeding usually spotted between the 6th and 11th gestational week. Other symptoms include vomiting, nausea, tachycardia, and in severe cases hypovolemic shock due to internal bleeding induced by the rupture of the tubes. The key to diagnosing an ectopic pregnancy is transvaginal ultrasound, alongside the measurement of beta human chorionic gonadotropin hormone (beta-hCG) concentrations, which are usually found to be low in such cases. The management of ectopic pregnancy is either medical or surgical. Case report: We are reporting the case of a 32-year-old pregnant woman, who presented to the emergency department for diffuse abdominal pain and recurrent episodes of vomiting and diarrhea of one-week duration. The patient was stable on admission, but later on, she showed a rapid shift in her vital signs. Blood tests were obtained, and pelvic ultrasound was done, followed by a Computed Tomography (CT) scan that revealed the presence of a right cornual ectopic pregnancy. A rapid surgical intervention was carried out to save the patient’s life and alleviate the pain. Conclusion: Ectopic pregnancy is an urgent condition that needs to be treated immediately to avoid its fatal consequences. Several risk factors stand behind it, and a usual history and physical gynecological examinations are insufficient to detect it, hormone screening tests additionally to ultrasounds are highly recommended to decrease the incidence of mortality. A proper follow-up with a specialist could avoid the risk of an ectopic pregnancy as well as its undesirable outcomes.

Abstract:Objective: To investigate the clinical features, diagnosis and treatments of heterotopic pregnancy (HP) and demonstrate the outcomes of HP after the surgical treatment of ectopic pregnancies. Methods: A retrospective analysis was performed on 144 cases of HP in Women's Hospital, School of Medicine, Zhejiang University from January 2003 to December 2016. Results: (1) Clinical features of HP:the average age of patients was (30.8±3.8) years old, body mass index (BMI) was (21.9±2.9) kg/m(2), and was diagnosed at (6.5±1.3) weeks gestational age. Four patients (2.8%) were naturally pregnant, 10 patients (6.9%) received ovulation induction, and 130 patients (90.3%) had received infertility treatments. Fifty-one patients (35.4%) had no clinical symptoms, and 93 patients (64.6%) had clinical symptoms included vaginal hemorrhage,abdominal pain and hemorrhagic shock caused by intraperitoneal hemorrhage. The location of ectopic pregnancy was most common in the fallopian tubes (59.0%, 85/144) and the interstitial part (33.3%,48/144) . (2) Ectopic pregnancy treatment of HP:thirteen patients underwent expectation treatment, and the remaining 131 cases underwent surgical treatment, including laparoscopy (n=56) , laprotomy (n=52) , and fetal reduction (n=23) . Among the 131 patients underwent surgery, intrauterine pregnancy were found inevitable abortion in 6 cases preoperatively. The total early abortion rate after surgery was 14.4% (18/125) ; the second operation rate was 3.1% (4/131) . (3) Intrauterine pregnancy outcome of HP: 120 intrauterine fetal were survival, the total live birth rate was 83.3% (120/144) .One hundred and seven intrauterine fetal were survival after operation and the live birth rate after operation was 85.6% (107/125) . Twenty-nine cases were premature delivery and the premature delivery rate was 24.2% (29/120) . There was no significant differences between tubal HP and interstitial HP group in the preterm birth rate [25.8% (16/62) vs 26.3% (10/38) ; χ(2)=0.003, P>0.05]. Cesarean section rate of delivery in interstitial HP group was significantly higher than that in tubal HP group [97.4% (37/38) vs 59.7% (37/62) ], and the difference was statistically significant (χ(2)=17.400, P<0.05) . Conclusions: The clinical manifestations of HP are diversified, combining of high risk factors, clinical symptoms and ultrasonography could improve the accuracy of diagnosis. Different method has been used to treat HP, such as laparoscopic or laparotomy and fetal reduction, and there are varying degrees of failure rate and postoperative abortion rate. We should consider carefully to adopt expectant management. Through individualized treatment, most HP could get good perinatal outcomes.
