Abstract:Objective To explore the clinical significance of prenatal ultrasound examination in diagnosing twin reversed arterial perfusion sequence (TRAP). Methods This was a retrospective study on 5 cases of TRAP sequence conifrmed by prenatal ultrasound and clinical method. The clinical data and ultrasound images were analyzed. Cases were collected at Hubei Maternal and Child Health Hospital from 2001 January to 2013 February. Results Four-iffths of TRAP were not corrected diagnosed and another one was misdiagnosised as asymmetrical conjoined twins in early pregnancy. Five cases of TRAP were correctly diagnosed during the second trimester. Sonographic ifndings of ifve acardia without blood lfow were as follows:(1) All acardias had no blood perfusion in body and umbilical cord, were absence of the fetal head (acephalous) and heart (acaidia), without skin edema, and a linear object resemble as umbilical cord extend from umbilical region of acardia to plcenta could be seen. Four (case 2-5) had lower limbs or dysplasia of lower limbs (two had spine), and one (case 1) had an irregular mass with bone. (2) Four pump twins had no obvious abnormity, while one had two clubfeet (case 2). Three died in uterus, one underwent induced labor due to oligohydramnios and heart dysfunction, and another one survived. Prenatal ultrasound images were consistent with MRI, X-ray examination, autopsy and postnatal neonatal examination results. Conclusions Acardia without blood lfow which was generally misdiagnosed as single gestation in utero can be diagnosed according to the special disformity of acardia and the linear umbilical cord extend from umbilical region to placenta, besides, it still need to strengthen monitoring the pump twin even if there was no blood perfusion to acardia.

Abstract:Background TRAP sequence occurs in monochorionic pregnancies consisting of one normal fetus and a non-viable fetus. The pump twin has an increased risk of developing high-output cardiac failure. Case 32-year-old G4P2012 with TRAP syndrome in current pregnancy presented to triage at 26 weeks with contractions and spotting. She had undergone RFA for selective reduction at another facility. Placental abruption was suspected and patient underwent a cesarean section. Twin A was delivered alive although she subsequently succumbed due to complications of prematurity. Conclusion This case highlights the importance of early detection and consistent prenatal care in the management of TRAP sequence. Further research of interventions associated with improved outcomes should be encouraged.

Abstract:Twin reversed arterial perfusion (TRAP) sequence is a rare complication of monochorionic multiple gestation pregnancies, in which the pump twin provides hemodynamic support to a nonviable co-twin (acardius). Fetal magnetic resonance imaging (MRI) is used to detect pump twin abnormalities, particularly brain ischemia, prior to fetal intervention to interrupt umbilical blood flow to the acardius.

Abstract:Twin reversed arterial perfusion (TRAP) sequence is a rare complication of monochorionic twinning whereby a donor twin perfuses an acardiac twin via aberrant vascular anastomoses. The resulting paradoxical retrograde blood flow supplying the acardiac twin is oxygen-poor, leading to some of the most severe malformations encountered in humans. Though the first descriptions of acardiac twins date back to at least the 16th century, the pathophysiologic processes which underpin the development of TRAP sequence are still being elucidated. Theories on the pathogenesis of TRAP sequence include deficiencies intrinsic to the embryo and primary abnormalities of the placental vasculature. Autopsy studies continue to provide clues to the underlying pathogenesis of TRAP sequence, and the characterization of the spectrum of manifestations that can be observed in acardiac twins. Herein, we present the clinical, autopsy, and molecular findings in a unique case of TRAP sequence. Novel findings include a primitive cloaca-like structure and chromosomal aberrations involving 6q11.1 and 15q25.1.
Natural History of Twin Reversed Arterial Perfusion Sequence and Perinatal Outcomes

Abstract:Objective:To investigate the natural history and perinatal outcomes of twin reverse arterial perfusion(TRAP) sequence.Methods:A retrospective study was performed in 15 TRAP cases which underwent conservative management between July 2008 and December 2011.The pregnancy outcomes including preterm labor,abortion and average gestational age at delivery as well as the abdominal circumference(AC) ratio,umbilical artery pulsatile index(UA-PI) ratio of acardiac twin and pump twin were analyzed.12-month follow-up was performed for the pump twin after birth.Results:Among the 15 TRAP cases,11 cases were acardius acephalus,4 cases were acardius anceps.10 cases(67.7%) were misdiagnosed as intrauterine death(3 cases),edema(2 cases) and neck lymph aquatumor(5 cases)of one twin by first trimester ultrasonography.For pump twin,2 cases with polyhydramnios(1 case had induced abortion and 1 case had spontaneous abortion),2 cases with fetal heart failure(1 case had preterm delivery and 1 case terminated pregnancy for congenital heart disease),1 case with polyhydramnios as well as fetal heart failure had spontaneous abortion.For acardiac twin,2 cases(13.3%) had spontaneously stopped reversed arterial blood flow,among which 1 case had intrauterine fetal death at 37 gestational weeks,1 case has stable ongoing pregnancy ;8 cases(53.3%) had rare blood flow(UA-PI ratio<0.7),among which 2 cases had spontaneous abortion,1 case had induced abortion,3 cases had three healthy infants after 1 year follow-up and 2 cases are in stable ongoing pregnancy;all other 5 cases(33.3%) with rich blood flow terminated pregnancy.Among 10 acardiac twin(66.7%) with rapid growth(AC ratio >50%),7 cases terminated pregnancy and 1 case had spontaneous abortion.Conclusions:Small acardiac twin and rare blood flow show less risk for the pump twin and better perinatal outcomes.The rapid growth of acardiac twin and the appearance of congestive heart failure of the pump twin or edema syndrome are suggested as poor prognostic factors.

Abstract:Twin reversed arterial perfusion (TRAP) sequence is rare in monochorionic twin pregnancies. TRAP sequence is distinct from other multifetal pregnancies in that one of the twins has normal anatomy while the other twin has a varied amount of characteristic abnormal features. In the literature, mortality is reported 100% in the abnormal twin. We report 1 case of TRAP sequence at our institution in which the diagnosis of TRAP sequence was missed in the first trimester at another hospital. The patient, a 33-year-old G1P0A0, did not have any follow-up after her first scan until the routine second-trimester ultrasound at our institution. Both the radiologist and the sonographer did not appreciate the differential diagnosis of TRAP sequence in their clinical decision-making. The TRAP diagnosis was established after the ultrasound performed at the fetal assessment unit in our hospital. Radiofrequency ablation (RFA) procedure was performed to give the normal twin a chance to survive, but unfortunately, the prognosis was poor in this case. We conclude that diagnosing a TRAP sequence is very important early in the pregnancy for a positive outcome in the normal twin. A robust collaboration among radiologists and obstetricians is vital for the best outcome of the normal twin.
Ultrasound imaging of acardia twin before and after blood spontaneously blocked and pump twin′s outcome analysis in twin reversed arterial perfusion sequence

Abstract:Objective To discuss the ultrasound imaging of acardia twin before and after spontaneously blood blocked in twin reversed arterial perfusion sequence(TRAP),and to analyze the influence factor for the prognosis of pump twin.Methods Seven TRAP with pump twins and acardia′s blood flow blocked were diagnosed by US and autopsy in Hubei Women and Children′s Hospital between 2001 January to 2012 September.The ultrasound images,clinical data and pump twin' outcome were analyzed.Results Ultrasound images showed skin edema,acardia,spine and lower limbs in seven acardia cases before blood blocked,among which 4 were acardius acephalus,3 were acardius anceps.Single umbilical artery were detected in 7 cases with reversed umbilical artery perfusion toward fetus.Four cases had rare blood flow(UA-PI ratio < 0.7.Five acardia cases had slower growing rate than the pump twin.Ultrasound images showed no growing,no blood flow in the acardia,the acardia twin became vagued in the second and third trimester,and finally developed into a paper fetus.A linear umbilical cord extend from umbilical region to placenta were found in 7 cases whose blood flow were spontaneously blocked.Pump twin′s outcome:3 pump twins survived,3 died in uterus,and 1 was induced labor due to hydrocephalus and cardiac failure.The most important factors that affected the prognosis of pump twin included: faster growing in acardia twin with less blood supply,abnormal brain and heart function in pump twin,chromosome abnormalities,abnormal amniotic fluid and cord entanglement.Conclusions Less blood flow and slow growth speed prompt in acardia twin suggested spontaneously acardia blood block.It is mandatory to monitor the pump twin after the spontaneous block of acardiac's blood flow.
