Abstract:Objective To evaluate the efficacy and safety of Ahmed glaucoma valve implantation versus trabeculectomy in the treatment of refractory glaucoma after scleral buckling.Methods A total of 41 eyes of 41 patients with glaucoma after scleral buckling who accepted Ahmed glaucoma valve implantation or trabeculectomy were enrolled,including 19 patients in the valve group and 22 patients in the trabeculectomy group.The successful rate,intraocular pressure (IOP) reduction,visual acuity,complications and medical therapy were observed for one year.Results The successful rate was 63.16％ in the valve group 12 months after surgery,and 31.82％ in the trabeculectomy group.The success rate in the valve group was significantly higher than that in the trabeculectomy group at 12 months follow-up (x2 =4.027,P =0.0427).IOP decreased from (24.7±2.8) mmHg at baseline to (16.5 ±4.9) mmHg in the valve group 12 months after surgery,and it decreased from (25.5 ± 3.6) mmHg to (19.8 ± 4.3) mmHg in the trabeculectomy group.The IOP in the valve group was significantly lower than that in the trabeculectomy group at 12 months followup (t =2.1102,P =0.0427).There was no significant difference in visual acuity,complications or medical therapy between those two groups.Conclusion Compared with trabeculectomy,Ahmed glaucoma valve implantation is an option with more persistence and same security in the treatment of glaucoma after scleral buckling.
Comparison of Ahmed Glaucoma Valve Implantation and Trabeculectomy for Glaucoma: A Systematic Review and Meta-Analysis

Abstract:ObjectiveTo compare the efficacy and safety of Ahmed glaucoma valve implantation (AGV) with trabeculectomy in the management of glaucoma patients.MethodsA comprehensive literature search (PubMed, Embase, Google, and the Cochrane library) was performed, including a systematic review with meta-analysis of controlled clinical trials comparing AGV versus trabeculectomy. Efficacy estimates were the weighted mean differences (WMDs) for the percentage intraocular pressure reduction (IOPR %) from baseline to end-point, the reduction in glaucoma medications, and the odds ratios (ORs) for complete and qualified success rates. Safety estimates were the relative risks (RRs) for adverse events. All outcomes were reported with a 95% confidence interval (CI). Statistical analysis was performed using the RevMan 5.0 software.ResultsSix controlled clinical trials were included in this meta-analysis. There was no significant difference between the AGV and trabeculectomy in the IOPR% (WMD = -3.04, 95% CI: -8.36-2.26; P = 0.26). The pooled ORs comparing AGV with trabeculectomy were 0.46 (0.22, 0.99) for the complete success rate (P = 0.05) and 0.97 (0.78-1.20) for the quantified success rate (P = 0.76). No significant difference in the reduction in glaucoma medicines was observed (WMD = 0.24; 95% CI: -0.27-0.76; P = 0.35). AGV was found to be associated with a significantly lower frequency of all adverse events (RR = 0.71; 95% CI: 1.14-0.97; p = 0.001) than trabeculectomy, while the most common complications did not differ significantly (all p > 0.05).ConclusionAGV was equivalent to trabeculectomy in reducing the IOP, the number of glaucoma medications, success rates, and rates of the most common complications. However, AGV was associated with a significantly lower frequency of overall adverse events.
Long-term results of Ahmed glaucoma valve implantation with/without intravitreal anti-VEGF injection in neovascular glaucoma patients

Abstract:Purpose: This study aimed to evaluate the efficacy of Ahmed glaucoma valve (AGV) implantation with or without anti-VEGF injections in neovascular glaucoma patients. Materials and methods: This single-center retrospective study assessed NVG patients who underwent AGV implantation with or without anti-VEGF injections. Demographic and clinical data, including ocular findings, intraocular pressure (IOP), visual acuity, and glaucoma medication count, were recorded preoperatively and postoperatively at one day, one month, and one year. The study included 35 patients. Group 1 consisted of 23 patients who received anti-VEGF injections before AGV surgery. Group 2, with 12 patients, had no anti-VEGF injections prior to surgery. Successful surgery was defined as IOP values between 6 and 21mmHg. The primary outcome was a 30% or more reduction in IOP. Results: The groups displayed no significant difference in their demographic or clinical profiles (P>0.05). The visual acuity before and one year after surgery did not differ significantly between the groups. However, IOP values significantly decreased by the end of the one-year follow-up for both groups. No significant differences were found between the groups regarding visual acuity, IOP, or the number of medications during the one-year follow-up (P>0.05). Success rates were 95.7% for Group 1 and 91.7% for Group 2. No significant difference in complications between the groups was observed (P>0.05). Conclusion: Anti-VEGF injections prior to AGV implantation did not significantly impact visual acuity, IOP values, or medication count during the one-year follow-up.

Abstract:PurposeThe aim of this study was to assess the efficacy of Ahmed glaucoma valve (AGV) in eyes with Descemet’s stripping endothelial keratoplasty (DSEK) and glaucoma and evaluate the risk factors for failure.MethodsRetrospective interventional study included 27 eyes (27 subjects) that underwent AGV implantation following DSEK. The main outcome measure was AGV success, defined as IOP between 6 and 21 mmHg with or without topical antiglaucoma medications (AGMs). Secondary outcome measure was graft survival. Graft failure was defined as corneal edema for ≥ 1 month or irreversible graft opacity requiring intervention.ResultsEighteen eyes (66.7%) had anterior chamber tube and 9 eyes (33.3%) had sulcus tube placement. Median follow-up was 1.2 years (interquartile range, IQR, 0.5–2.5 years). Following AGV implantation, the median IOP decreased significantly (p < 0.0001), median number of AGMs reduced significantly (p < 0.001) and median visual acuity was maintained (p = 0.76). Cumulative success probability of AGV was 75.8 ± 10.6% at 1 year, and 75.8 ± 10.6% at 2 years. For 24 clear grafts before AGV, estimates of success post-AGV were 100% at 1 year, 77.8 ± 10% at 2 years and 51.9 ± 16.4% at 3 years. Postoperative tube intervention was a borderline significant risk factor for graft failure (p = 0.05) with hazards ratio of 7.2; however, tube location was not associated with failure.ConclusionsAGV can be considered an effective surgical option for IOP control in eyes with DSEK and glaucoma. However, high risk of long-term graft failure in these eyes needs to be considered specially those eyes needing tube intervention.

Abstract:Purpose: Teaching and training of glaucoma fellows on the technique of Ahmed glaucoma valve (AGV) implantation surgery is very important. The purpose of this study was to describe the clinical outcomes and complications of AGV surgery performed by glaucoma fellows of a tertiary eye center from eastern India. Methods: This was a retrospective study based on electronic medical records. Thirty-five eyes operated with AGV by five glaucoma fellows from January 2016 to November 2020 were included. Best-corrected visual acuity, intraocular pressure (IOP), and number of antiglaucoma medications (AGMs) were the study outcome measures. Results: The mean (±standard deviation) age of the patients was 40.5 (±19.7) years. The follow-up period was 24.37 (±13.01) months. Both complete success (IOP <21 mmHg without additional AGM) and qualified success (IOP 21 mmHg with AGM beyond 3 months) in seven eyes. Despite a high rate of failure of the primary surgery, with the consultants’ intervention, there was statistically significant improvement in vision and IOP in all patients ( P < 0.01) at the last follow-up. Conclusion: AGV implantation is a challenging surgical skill to be acquired by the glaucoma fellows.

Abstract:PURPOSE. To determine the indications for Ahmed glaucoma valve implantation by analyzing the duration and regimen of hypotensive therapy, and the configuration of previous interventions. METHODS. Retrospective evaluation of the duration and intensity of therapeutic treatment, and the configuration of laser and surgical procedures in a group of 139 patients (153 interventions) who underwent Ahmed glaucoma valve implantation in 2009–2011, and another group of 270 patients (272 interventions) who were treated in 2019–2010. RESULTS . The average age of candidates for implantation of the Ahmed valve increased from 63 y.o. in 2009–2011 to 70 y.o. in 2019–2020. The treatment preceding Ahmed valve implantation had extended in duration from 8.8±1.4 to 11.2±1.1 years leading to an increase in cumulative preservative toxicity from 9 293.8±968.6 to 10 038.1±888.9 μg. The main classes of intraocular pressure (IOP)-lowering drugs were prostaglandin analogues (75.4% in 2009–2011 and 77.1% in 2019–2020), carbonic anhydrase inhibitors (75.4% and 82.6%), and beta-blockers (57.4% and 61.1% accordingly). Laser trabeculoplasty was performed in only 11.8% and 28.7% of cases, respectively. Hypotensive filtering operations preceded valve implantation in 63.4% (2009–2011) and 80.5% of cases (2019–2020), while in the rest of patients the installation of the Ahmed device was the first surgical procedure. Despite treatment, the rate of glaucoma progression to an advanced stage increased from 62.1% in 2009–2011 to 82.7% in 2019–2020. CONCLUSION. With increase in the duration of conservative management of glaucoma its progression continues, while the increased preservative toxicity reduces the effectiveness of conjunctival surgery. In conclusion, earlier switch to surgical methods of IOP normalization involving the use of the Ahmed valve is advisable not only in secondary, but also in primary open-angle glaucoma treated conser-vatively for a long (over 7–8 years) period.
Modified ex-PRESS technique versus Ahmed glaucoma valve as primary glaucoma surgery for hereditary transthyretin amyloidosis glaucoma

Abstract:Background: To compare the efficacy of modified Ex-PRESS technique (ExP) versus Ahmed Glaucoma Valve (AGV) as primary surgery in hereditary transthyretin amyloidosis (ATTRv) secondary glaucoma. Methods: Retrospective study at the national amyloidosis centre. Success was defined as an IOP ≥ 6 mmHg and ≤ 21 mmHg with no need for further glaucoma surgery or laser trabeculoplasty and without loss of light perception at the time of the last follow-up. Secondary outcomes included surgical complications, need for hypotensive drugs, and endothelial cell loss. Patients submitted to previous glaucoma surgery were excluded. Results: We included 180 eyes of 150 patients, 121 in AGV and 59 in ExP group. No significant baseline differences were found between groups. At the time of last follow-up, both groups exhibited significant intraocular pressure (IOP) reduction (p < .001) and number of glaucoma medications (p < .001). Kaplan-Meyer analysis showed higher cumulative probability of success for AGV group (80.1% vs. 41.1%, p < .001) and higher mean time to failure (54.5 vs. 36.9 months, respectively, p < 0.001) at 60 months follow-up. AGV showed lower hazard (HR 0.21, 95% CI [0.111-0.407], p < .001) for failure. Conclusion: AGV and ExP are safe and effective techniques in the treatment of ATTRv secondary glaucoma. However, AGV's efficacy seems to be more durable.
Primary implantation of glaucoma drainage device in secondary glaucoma: Comparison of Aurolab aqueous drainage implant versus Ahmed glaucoma valve

Abstract:Abstract Glaucoma drainage devices are invaluable in the management of secondary/ refractory glaucomas. This study aimed to compare the efficacy and safety of Aurolab Aqueous Drainage Implant (AADI) and the Ahmed Glaucoma Valve (AGV) in filtration-surgery-naïve secondary glaucoma eyes. For this purpose, a retrospective, comparative review was conducted on patients with secondary glaucoma (open and closed) who underwent primary tube procedures, either AADI or AGV. The primary outcome measure was intraocular pressure (IOP), and secondary measures included best-corrected visual acuity (BCVA), number of antiglaucoma medications (AGMs), and complications. This study included 59 eyes in the AADI group with a mean follow-up of 20.3 ± 12.9 months and 61 eyes in the AGV group with a mean follow-up of 19.8 ± 11.8 months. Preoperative IOP, AGM use, and BCVA did not significantly differ between the groups. However, at the last visit, both IOP and AGM use were significantly lower in the AADI group (12.9 ± 3.7 mmHg and 0.6 ± 0.9 vs. 15.7 ± 2.7 and 1.8 ± 1.0 respectively, all p < .001). Moreover, the AADI group exhibited a significantly higher rate of complete success (57.6%) compared to the AGV group (14.7%, p < .001); corresponding qualified success was 91.5% and 80.3%. Serious complication rates were comparable between the two groups. In conclusion, toth AADI and AGV procedures effectively reduced IOP and the need for AGMs. However, the reductions were significantly greater in the AADI group, which also showed a higher rate of complete success. Considering its affordability, AADI could have a substantial positive impact, particularly in resource-constrained settings.

Comparison of Tenon duplication with dura mater covering technique for Ahmed glaucoma valve implantation

Abstract:Purpose: To compare the efficacy and complications of Tenon duplication with dura mater covering technique for Ahmed glaucoma valve (AGV) implantation. Methods: This retrospective study included 44 refractory glaucoma patients (44 eyes) who underwent AGV implantation from 2017 to 2020 in the Ophthalmology Clinic of Eskişehir Osmangazi University Hospital and attended regular postoperative follow-ups. The patients were divided based on whether they underwent Tenon duplication technique (group 1: n = 20) or dura mater covering technique (group 2: n = 24) during surgery. The patients’ age, gender, systemic diseases, glaucoma type, pre-op intraocular pressure (IOP), and ocular surgeries were recorded. The groups were compared for IOP level control, early and late complications, postoperative antiglaucomatous medication requirements, glaucoma surgery requirements, presence of postoperative hypertensive phase (HP), and surgical success which was defined as an IOP ≥5 and ≤21 mmHg, with or without antiglaucoma medication. Results: By the end of the mean follow-up (22.6 ± 10.6 months), the success rates were 95% (group 1) and 96% (group 2). The groups showed no differences in postoperative complications, postoperative antiglaucomatous drugs’ onset time, additional glaucoma surgery, need for needling, presence of HP at 6 months postoperatively, and relationship between the glaucoma type and success rates ( P values: 0.86, 0.9, 0.48, 0.12, 0.36, and 0.8, respectively). The IO P values at the last follow-up were 15.2 ± 4.1 in group 1 and 14.7 ± 4.8 in group 2. The IOP reduction rates showed no significant differences. Conclusion: Since success and complications are similar in both Tenon duplication and dura mater covering technique, unique grafting materials may not be needed in AGV implantation surgery, except in special cases.

Abstract:Precis: Our study highlights the long-term success of trabeculectomy or AGV surgery in subjects with glaucoma secondary to ICE syndrome. However, many of ICE syndrome cases may need multiple glaucoma surgeries to achieve controlled IOP, with/without concomitant corneal graft surgery. Purpose: To evaluate the long-term outcome of trabeculectomy and Ahmed glaucoma valve (AGV) implantation in iridocorneal endothelial syndrome (ICE). Methods: Patients with glaucoma secondary to ICE syndrome who underwent either trabeculectomy or AGV surgery with intraoperative adjunctive MMC from 2009 to 2020 were included in this study. All patients were followed for at least 6 months after initial surgery. The main outcome measures were intraocular pressure (IOP), number of IOP lowering medications and surgical success. Surgical success was defined as complete according to the levels of IOP ( < 18) and at least 20% reduction from preoperative IOP without medications and qualified as complete success but with medications, where the number of medications was less than preoperative numbers. Cumulative success was the sum of the qualified and complete success. Results: Twenty-nine eyes of 29 patients were included. Trabeculectomy was done in 13 patients (group A, 44.8%) and 16 patients underwent AGV surgery (group B, 55.2%). The median age was 50(42-56.50) and 47(36.75-52.75) years in groups A and B, respectively ( P value=0.10). All patients completed at least 2 years of follow up. Mean IOP was not significantly different between groups preoperatively ( P =0.70) and the effect of the type of surgery on IOP was not statistically significant at multiple follow-up time points (repeated measures ANOVA, P =0.44). The mean IOP decreased from 35.76±6.36 mmHg preoperatively to 16.00 + 3.10 in group A and from 36.12 + 8.11 mmHg to 17.00 + 3.75 in group B ( P =0.449) at year 2 of follow-up. The effect of the type of surgery was not significant on the total number of IOP lowering medications used throughout the study (repeated measures ANOVA, P =0.81). Kaplan-Meier analysis show complete success in 14 patients (48.3%), 11 patients (37.9%) and 7 patients (24.1%) at 6-month, 1 year and 2 years follow-up, respectively. Cumulative success rate was 95% at two years follow-up for all patients. Conclusions: In 2 year follow up, trabeculectomy or AGV could significantly reduce the IOP in glaucoma patients secondary to ICE syndrome.

Comparison of FP-7 and S-2 Ahmed Glaucoma Valve Implantation in Refractory Glaucoma Patients for Short-Term Follow-Up.

Abstract:BACKGROUND:Ahmed glaucoma valves (AGV) has been used for decades, but there is no detailed report about the efficacy of AGV in Chinese glaucoma patients. This study aimed to compare the intraocular pressure (IOP) lowering efficacy and side effects of S-2 polypropylene and PF-7 silicone AGV implantation in Chinese refractory glaucoma patients.METHODS:Patients were divided into S-2 model AGV group and FP-7 model AGV group. The complete and qualified surgical success rate, change of IOP, number of anti-glaucoma medications used and postoperative complications were recorded and analyzed.RESULTS:Average follow-up time was comparable between two groups. IOP was reduced from (37.9 ± 12.7) mmHg preoperatively to (17.3 ± 5.3) mmHg at the last follow-up in S-2 group and reduced from (39.9 ± 14.4) mmHg to (17.7 ± 4.9) mmHg in FP-7 group. Anti-glaucoma medications were reduced from 3.8 ± 0.2 to 1.5 ± 0.2 in S-2 group, and 3.5 ± 0.2 to 0.7 ± 0.2 in FP-7 groups. The cumulative success rates were comparable in two groups, which were 61.2% and 72.1% in S-2 group and FP-7 group respectively. When IOP reduction criteria was used, complete success rates were 30.6% and 51.2% for S-2 and FP-7 groups, and qualified success rates were 86.1% and 92.7% separately. In both groups, the major complication was hypotony, and the previous trabeculectomy of patients was the major risk factor for surgery failure.CONCLUSIONS:In this short-term retrospective study, S-2 AGV is showed at least as effective as FP-7 AGV in IOP reduction, but associated with higher rate of complications. Previous trabeculectomy is a principle risk factor for AGV implantation failure. These clinical outcomes are important for converting use of the FP-7 silicon AGV in Chinese refractory glaucoma patients.

Abstract:Glaucoma drainage devices have an established position in the surgical treatment of glaucoma, and moreover their use has grown rapidly in recent years. The aim of the study is to discuss the technique and complications of Ahmed glaucoma valve implantation – one of the most frequently used filtering systems in the treatment of difficult cases of glaucoma. Ahmed glaucoma valve implantation is an effective and relatively safe surgical procedure. The effectiveness of the method, according to various literature reports, is estimated at approximately 45–64% in a 5-year follow-up. Traditionally, Ahmed glaucoma valve implantation has been reserved for the treatment of glaucoma refractory to multiple filtration surgical procedures. Currently, this procedure is recommended for patients with risk factors for failure of trabeculectomy with antimetabolites. The current literature shows the potential role of Ahmed glaucoma valve implantation also as a primary surgical option in selected glaucoma cases, for example in patients with a higher risk of infection.

Abstract:OBJECTIVE:To evaluate the efficacy and safety of FP-7 Ahmed glaucoma valves (AGV) implantation in neurovascular glaucoma (NVG) as the first choice of surgery.METHODS:This retrospective, comparative case series study collected a total of 36 eyes of 36 patients with neurovascular glaucoma who underwent AGV implantation in Zhongshan Ophthalmic Center from January 2009 to June 2010. Change of intraocular pressure (IOP), the best corrected visual acuity, numbers of anti-glaucoma medication, success rate and postoperative complications were followed up at day 1, week 1, month 1, and every 3 months after surgery. Complete success of surgery was rated as reduction of IOP ≥ 30% without medication and those who failed to meet criteria was rated as partial success. Data were analyzed by paired Student t-test for IOP, rank sum test for paired non-parametric numbers of medication, and repeated measures analysis of variance for comparison of IOP between different time points using SPSS 13.0.RESULTS:Compared with pre-operation, IOP was significant (F = 9.26, P < 0.05) decreased after surgery with FP-7 AGV implantation (39.5 ± 9.7) mm Hg (1 mm Hg = 0.133 kPa) vs (9.2 ± 8.9), (11.8 ± 3.8), (13.7 ± 4.8), (16.9 ± 5.3), (16.9 ± 6.8) mm Hg at day 1, week 1, month 1, month 3 and the last following-up of post-operation, respectively. The numbers of anti-glaucoma medication were significantly (Z = 6.764, P < 0.05) reduced from 4.0 (1-6) of pre-operation to 1.0 (1-3) of post-operation. At the last following up, the complete success rate after FP-7 AGV implantation was 80.6%, and qualified success rate was 91.7%. The postoperative complications including occlusion of the drainage tube, exposure of the drainage tube, shallow anterior chamber and encapsulated cystic blebs around the plate were controlled with additional treatment.CONCLUSIONS:The clinical outcome indicated that the implantation of FP-7 AGV has a stable IOP lowering effect and fewer complications, which can be considered as one of the first choices for management of NVG.

Abstract:Background: Neovascular glaucoma (NVG) is a secondary glaucoma with a poor visual prognosis. Trabeculectomy with antifibrotic agents, glaucoma drainage devices (GDDs), and cyclo-destructive procedures are recommended in patients who are refractory to medical management. However, due to the poor success rate of conventional trabeculectomy and the higher cost of GDDs, alternative procedures need to be looked at. Purpose: To compare the surgical outcomes and economic aspects of a newly developed polypropylene suture bed-based modified trabeculectomy to Ahmed glaucoma valve (AGV) implantation for NVG. Methods: It was a prospective interventional study conducted at a tertiary care center between 2018 and 2020. Consecutive patients with NVG with a minimum follow-up of 18 months were included. Surgical outcomes are mainly based on intraocular pressure (IOP) control and the cost of surgery. Results: Sixty eyes were included out of which 40 (60.6%) underwent modified trabeculectomy and 20 (33.7%) underwent AGV. At the final follow-up, no significant difference ( P < 0.05) was found between the surgical outcomes of both groups. The complete success rate (IOP < 21 mm Hg without antiglaucoma medications) was 60 and 65% while the qualified success rate (IOP < 21 mm Hg with antiglaucoma medications) was 30 and 25% in modified trabeculectomy and AGV groups, respectively, at final follow-up. The cost of surgery was significantly higher in the AGV group ( P < 0.0001). Conclusion: Modified trabeculectomy as described might be a better alternative for NVG eyes.

Abstract:Purpose Uveitis can lead to secondary glaucoma, a condition with challenging management that can carry irreversible visual loss. Filtering surgery has demonstrated a higher failure rate, increased incidence of postoperative complications and reinterventions in uveitic patients. There is no consensus on the optimal surgical approach for uveitic glaucoma (UG) due to limited data comparing various intraocular pressure (IOP)-lowering surgeries. This retrospective cohort aims to assess the clinical outcomes of trabeculectomy (TBT), non-penetrating deep sclerectomy (NPDS), Ex-PRESS shunt and Ahmed glaucoma valve, providing additional insights into the long-term IOP control and safety of filtering surgeries in UG.

Glaucoma valve devices for refractory glaucoma following vitrectomy

Abstract:To evaluate the surgical effectiveness of glaucoma valve devices for refractory glaucoma following vitrectomy surgery.Sixteen eyes of 15 patients suffering from secondary glaucoma following vitrectomy were treated with glaucoma valve implantation, all of the eyes were in critical condition, and the intraocular pressure failed to control after the silicone oil removal and/or with glaucoma medication. Among them, 13 eyes underwent Ahmed valve implantation and 3 eyes underwent Krupin disk implantation.The mean follow-up was 18.5 months. The total success rate was 76.9% at 3 months, 70.0% at 6 months and 66.7% at one year or longer. The postoperative complications included transient hyphema, early postoperative hypotony, choroidal detachment, shallow anterior chamber and obstruction of the tube tip.The glaucoma valve device is a safe and effective method for refractory glaucoma following vitrectomy, especially for the patients who have a certain level of visual acuity.
Ahmed glaucoma valve implantation for neovascular glaucoma after vitrectomy for proliferative diabetic retinopathy

Abstract:Purpose: To evaluate the safety and efficacy of Ahmed Glaucoma Valve implantation (AGVI) for the management of neovascular glaucoma (NVG) associated with proliferative diabetic retinopathy (PDR) in the vitrectomized eyes. Patients and methods: We reviewed the medical records of patients with NVG associated with PDR who underwent AGVI for intraocular pressure (IOP) control and compared the surgical outcome according to vitrectomy history. The main outcome measures were: postoperative IOP control, visual acuity, and complications. Success was defined as an IOP of ≤21 mm Hg and ≥6 mm Hg, without further glaucoma surgery or loss of light perception and devastating complications. Results: A total of 73 patients (73 eyes) were included: 42 patients with vitrectomy history before AGVI (vitrectomized group) and 31 patients without vitrectomy history (nonvitrectomized group). The cumulative probabilities of success after AGVI were 89.9% and 83.8% after 1 year, 74.8% and 74.7% after 2 years, and 62.5% and 68.5% after 3 years for the vitrectomized group and the nonvitrectomized group, respectively (P=0.9309). Cox proportional hazards regression showed the intraocular silicone oil tamponade as a risk factor for the surgical failure (odds ratio=4.543, P=0.047). Final visual acuity improved or stabilized in 33 patients (78.6%) in the vitrectomized group and 18 patients (58.1%) in the nonvitrectomized group. Complications were comparable between the groups, but surgical interventions were needed for 5 patients (11.9%) in the vitrectomized group. Conclusion: Despite some complications that necessitate surgical intervention, the AGVI is a safe and effective procedure that enables successful IOP control and vision preservation in patients with NVG associated with vitrectomy for the PDR.
Outcomes of a Second Ahmed Glaucoma Implant With Mitomycin-C in Pediatric Glaucoma After Initial Valve Failure

Abstract:Prcis: After the failure of a glaucoma drainage device in children, implantation of a second Ahmed glaucoma valve with mitomycin-C allows a significant reduction of intraocular pressure and number of medications with good medium-term and long-term survival. Introduction: The effectiveness of glaucoma drainage devices (GDD) is limited in time. There is little literature regarding the optimal management strategy after failure of a GDD in pediatric glaucoma. Purpose: To report the outcomes of Ahmed glaucoma valve implantation (AGV) with mitomycin-C (MMC) after failure of a GDD in children. Methods: Retrospective chart review of patients with a history of at least 1 GDD receiving an AGV implantation with MMC between 2000 and 2019. We defined complete success as an IOP of 5-21 mm Hg without glaucoma medication and qualified success as a final IOP of 5-21 mm Hg with one or more glaucoma medication, without loss of vision. Results: Twenty-one patients (22 eyes) met the inclusion criteria. The intraocular pressure and number of medications were significantly reduced. The probability of complete success was 47% [95% CI: 29%-75%] at 2 years and 34% [95% CI: 18%-65%] at 4 years. The probability of qualified success was 74% [95% CI: 56%-97%] at 2 years, and 54% [95% CI: 34%-84%] at 5 years. Failure happened in 10 eyes after a mean time of 4.3 years ±3.6 (6 mo to 15 y), 5 of which (23%) for severe complications or loss of vision. Discussion: This study of implantation of a second valve with MMC reports a significant decrease in IOP and medications with medium-term and long-term success rates close to those reported for first valve implantation, although with a high risk of complications.
