We believe that some concerns should be addressed to avoid weakening the strength of this message.First, the study excludes patients with stage 2 and stage 3 hypertension; this population comprises a large proportion of those with metabolic syndrome. After the exclusion of patients with stage 2 or 3 hypertension and diabetes, is it enough to reveal the real relation between metabolic syndrome and blood pressure response to sodium intake?Second, three blood pressure measurements taken every morning during the observation period do not capture the totality of information available. Metabolic syndrome and blood pressure response to sodium – Authors' replyWe agree that our study might slightly underestimate the association between metabolic syndrome and salt sensitivity of blood pressure. To avoid the potential adverse effects of a high sodium diet, we excluded patients with stage 2 hypertension (blood pressure ≥160/100 mm Hg) from our intervention study. Since hypertensive patients are more likely to have metabolic syndrome and salt sensitivity, the exclusion of stage 2 hypertensive patients is likely to bias the association towards the null. On the other hand, our study showed that the association between metabolic syndrome and salt sensitivity was independent of hypertension because this association existed after exclusion of all hypertensive patients (blood pressure ≥140/90 mm Hg).
