Abstract:Objective: To investigate the clinical significance of foramen ovale-related parameters examined by echocardiography in the prenatal diagnosis of hypoplastic left heart.Methods: One hundred and eighty-eight cases of fetal foramen ovale-related parameters were obtained,to analyze its change with the gestational age,and study the relationship between foramen ovale-related parameters and fetuses with hypoplastic left heart.Results: In the 188 cases,165 cases were nomal and 23 cases were abmomal associated with hypoplastic left heart(12 cases were hypoplastic left heart syndrome,8 cases were aortic valve aortic stenosis,3 cases were coarctation of aorta).Normal fetal foramen ovale valve flap diameter was similar to the aortic diameter,and normal fetuses had a smaller ratio of foramen ovale valve flap diameter/aortic root size than fetuses with hypoplastic left heart(P0.01),but had a larger ratio of foramen ovale valve flap diameter/atrial septum size(P0.01).Conclusion: The foramen ovale-related parameters examined by echocardiography has important significance in prenatal diagnosis of hypoplastic left heart.

Abstract:Objective To observe the clinical value of fetal echocardiography in the diagnosis of fetal hypoplastic left heart syndrome (HLHS). Methods Echocardiographic features and clinical data of 18 fetuses prenatally diagnosed with HLHS were analyzed retrospectively. Results The findings of 18 fetal echocardiogram were as followed: ①Eight fetuses were found with aortic and mitral valve atresia and severe left ventricular dysplasia,5 with mitral valve atresia and various degrees of aortic stenosis as well as aortic arch coarctation,3 with thickened mitral valve,aortic stenosis and aortic arch coarctation,while 2 with thickened mitral valve,aortic atresia atreasia and aortic arch coarctation. ②The diameter of aorta was smaller than that of pulmonary artery. The ratio of AO/PA was 0.25—0.40 (0.38±0.18). There was reverse flow from ductus artery to the aortic arch and ascend aortic. ③Intact atrial septum and thickeness appeared in 6 fetuses. No blood shunt was detected in intact atrial septum. Other 12 fetuses presented few left to right shunts. Conclusion Fetal echocardiography may serve as a reliable methods for early diagnosis of fetal HLHS. It is important for the detection during the parental period and the guidance of the clinical treatment.

Abstract:Objective To evaluate the ultrasonic feature and clinical significance of persistent left superior vena cava(PLSVC)in fetal life.Methods Fetal echocardiography was performed in 3368 fetuses.Thirty-one fetuses of PLSVC were confirmed.Results The dilated coronary sinus was observed in 30 of 31 fetuses.Congenital heart defects were presented in 14 of these cases,and extracardiac anomalies were presented in 6 fetuses.Both congenital heart defects and extracardiac anomalies were observed in 4 fetuses.Conclusions PLSVC is always associated with congenital heart defects.The prognosis Of affected fetuses largely depends on whether or not the PLSVC is associated with a congenital heart defect.Prenatal diagnosis of PLSVC can help US plan perinatal counseling and ameliorate the postnatal course.

Abstract:Background: Both hypoplastic left heart syndrome and ventriculo-coronary communication are extremely rare congenital cardiac conditions. Case: The diagnosis of hypoplastic left heart syndrome combined with endocardial fibroelastosis and ventriculo-coronary communication was made in a pregnant woman at 24 weeks of gestation. Interestingly, unlike diastolic flow from the coronary artery to the left ventricle in typical ventriculo-coronary communication, this fetus showed a systolic flow from the left ventricle to the coronary artery. Conclusion: To the best of our knowledge, this is the first study to present only a systolic flow signal in hypoplastic left heart syndrome combined with ventriculo-coronary communication in prenatal ultrasonography.

Abstract:A 29-year-old pregnant woman was referred to our hospital for fetal ultrasonography at 24 weeks of gestation. Her personal and family history was unremarkable. Two-dimensional ultrasound and colour Doppler were performed. The sonographic examination showed the fetus had a right-sided stomach, and the abdominal aorta and inferior vena cava were on the left side of the spine (figure 1). Fetal left ventricle was significantly smaller than the right ventricle, and the atrioventricular septal was completely defected. The aortic arch was located on the right side of the trachea, and the pulmonary artery was narrow. The pulmonary veins flowed into the right atrium. The prenatal ultrasonographic diagnosis was fetal heterotaxy syndrome and complex cardiac malformations. The parents decided to terminate the pregnancy, and the consent for post-mortem autopsy was obtained. At autopsy, organ dissection revealed a midline liver, a right-sided stomach, asplenia, two lobar bronchi and four-lobed lungs with bilateral main bronchi. Cardiovascular dissection showed a stenotic left ventricle, complete atrioventricular septal defect with common atrioventricular valve, right aortic arch with left brachiocephalic trunk, right common carotid artery and right subclavian artery, pulmonary atresia, anomalous draining of the pulmonary veins to the right atrium. The thoracic part of the inferior vena cava was on the right side of the spine, and the abdominal part is turned to the left side of the spine. Abstract:Objective To investigate the antenatal sonographic screen of hypoplastic left heart syndrome(HLHS) by using four-chamber view and three-vessel and trachea view. Methods Routine prenatal ultrasonography was used. Four-chamber view and three-vessel and trachea view were applicated for screening of fetal cardiac anomalies. Results Four cases of HLHS were found during the two years of study span. Three of them were complicated by inner cardiac abnormity (one was ventricular septal defect, another two were double outlet in right ventricule). One fetus accompanied with extra cardiac malformation (Dandy-Walker syndrome and single umbilical artery). Conclusion It is easy to screen fetal cardiac malformation by using four-chamber view and three-vessel and trachea view.
