Abstract:OBJECTIVES: To explore the image characteristics and main types of abnormal branching of fetal pulmonary artery in prenatal echocardiography.MATERIAL AND METHODS: A retrospective analysis of 41 cases diagnosed with abnormal branching of fetal pulmonary artery by prenatal echocardiography was made. The image characteristics of the abnormalities, their combination with intra- or extra-cardiac malformations and chromosomal anomalies were analyzed.RESULTS: The results of prenatal echocardiography showed that, among the 41 cases, 1) 4 cases were with anomalous origin of single pulmonary artery, 8 cases with pulmonary artery agenesis, 9 cases with pulmonary artery sling; 20 cases with crossed pulmonary arteries. 2) 11 cases were complicated with intracardiac malformations and 10 with extracardiac malformations. 3) Only 7 case underwent chromosomal examination and 1 tested abnormal. 4) Pregnancy outcomes: 25 fetuses were born and their abnormalities confirmed by echocardiography (MRI or surgery) to be consistent with prenatal ultrasound diagnosis; 16 cases had their pregnancy terminated due to their combination with other severe malformations, which were confirmed by pathological anatomy after induced abortion.CONCLUSIONS: Prenatal echocardiography can provide detailed images for the diagnosis of abnormal branching of fetal pulmonary artery, which can be complicated by intra- and extracardiac malformations and chromosomal anomalies and should be alerted.

Abstract:Objective To evaluate the ultrasonic feature and clinical significance of persistent left superior vena cava(PLSVC)in fetal life.Methods Fetal echocardiography was performed in 3368 fetuses.Thirty-one fetuses of PLSVC were confirmed.Results The dilated coronary sinus was observed in 30 of 31 fetuses.Congenital heart defects were presented in 14 of these cases,and extracardiac anomalies were presented in 6 fetuses.Both congenital heart defects and extracardiac anomalies were observed in 4 fetuses.Conclusions PLSVC is always associated with congenital heart defects.The prognosis Of affected fetuses largely depends on whether or not the PLSVC is associated with a congenital heart defect.Prenatal diagnosis of PLSVC can help US plan perinatal counseling and ameliorate the postnatal course.

Abstract:A 29-year-old pregnant woman was referred to our hospital for fetal ultrasonography at 24 weeks of gestation. Her personal and family history was unremarkable. Two-dimensional ultrasound and colour Doppler were performed. The sonographic examination showed the fetus had a right-sided stomach, and the abdominal aorta and inferior vena cava were on the left side of the spine (figure 1). Fetal left ventricle was significantly smaller than the right ventricle, and the atrioventricular septal was completely defected. The aortic arch was located on the right side of the trachea, and the pulmonary artery was narrow. The pulmonary veins flowed into the right atrium. The prenatal ultrasonographic diagnosis was fetal heterotaxy syndrome and complex cardiac malformations. The parents decided to terminate the pregnancy, and the consent for post-mortem autopsy was obtained. At autopsy, organ dissection revealed a midline liver, a right-sided stomach, asplenia, two lobar bronchi and four-lobed lungs with bilateral main bronchi. Cardiovascular dissection showed a stenotic left ventricle, complete atrioventricular septal defect with common atrioventricular valve, right aortic arch with left brachiocephalic trunk, right common carotid artery and right subclavian artery, pulmonary atresia, anomalous draining of the pulmonary veins to the right atrium. The thoracic part of the inferior vena cava was on the right side of the spine, and the abdominal part is turned to the left side of the spine. Abstract:Objective To investigate the prenatal echocardiography manifestation of aortopulmonary septal defect (APSD) and clinical significance. Methods Imaging characteristics for 5 fetuses with APSD who underwent fetal echocardiography at our hospital from June 2013 to November 2015 were retrospectively reviewed. All cases were followed up for pregnancy process and clinical results. Results Echocardiographic characteristics of APSD were a defect between the ascending aorta and the trunk of the pulmonary artery in the three vessels view. The defect sizes were ranged from 1.5 to 3.9 mm. The defect location to semilunar valve and /or pulmonary artery bifurcation could be displayed in the left and right ventricular outflow tract views. Color and pulse Doppler showed bidirectional or pulmonary-to-aorta shunt flow pattern. All the five APSD cases were confirmed by autopsy. Conclusions APSD can be diagnosed by fetal echocardiography. Prenatal diagnosis is important for obstetrical management, early surgical intervention and prognosis. Key words: Ultrasonography, prenatal; Echocardiography; Fetus; Aortopulmonary septal defect

Abstract:Objectives Establish a fetal heart anatomical cross-sectional database that correlates with screening transverse ultrasound images suggested by international professional organizations to detect congenital heart defects. Methods Fetuses with suspected congenital heart defects identified using the following cardiac image sequences obtained from transverse slices beginning from the upper abdomen and ending in the upper thorax were the subjects of this study: (1) four-chamber view, (2) left ventricular outflow tract view, (3) three-vessel right ventricular outflow tract view, and (4) the three-vessel tracheal view. A database of digital two-dimensional images of the transverse sweep was created for fetuses with confirmed congenital heart defects. In addition, using four-dimensional ultrasound spatial-temporal image correlation, selected transverse ultrasound images were acquired as part of the database. Ultrasound-detected congenital heart defects were confirmed postnatally from pathological specimens of the heart and lungs using a cross-sectional technique that mirrored the ultrasound images described above. When anatomical specimens were not available, prenatal ultrasound-detected congenital heart defects were confirmed using postnatal echocardiography and/or following surgery. Results The four screening views described in the Methods section identified 160 fetuses that comprised the database. Forty-five datasets consisted of both ultrasound and anatomical cross-sectional images. Thirteen percent (6/45) only had abnormalities of the four-chamber view (eg, endocardial cushion defects). Twenty-four percent (11/45) had abnormalities of the four-chamber view as well as right and left outflow tracts (eg, complex malformations). Of these, 10 of 11 had an abnormal tracheal view. Sixteen percent (7/45) had an abnormal four-chamber view and abnormal right outflow tract (eg, pulmonary stenosis). Thirty-three percent (15/45) had a normal four-chamber view but had abnormal right and left outflow tracts as well as an abnormal tracheal view (eg, tetralogy of Fallot, D-transposition of the great arteries). Conclusions Combining both ultrasound and anatomical imaging may be of assistance in training imagers to recognize cardiovascular pathology when performing the screening examination of the fetal heart.
