Abstract:ObjectiveTo compare the accuracy, efficiency, and consistency between experienced and less-experienced professionals using intelligent navigation echocardiography. MethodsIn this prospective study, we enrolled 93 second- and third-trimester fetuses with conotruncal defects (CTD) from July 2017 to February 2018. One or more spatiotemporal image correlation volume data sets were collected per case. The fetuses with CTD were diagnosed by the following two groups of professionals (n = 20 in each) with different experience levels using intelligent navigation echocardiography and two-dimensional ultrasound: group A with 15 years of experience and group B with 1 year of experience. The diagnostic consistency and accuracy of the technologies between the two groups were analyzed. ResultsSatisfactory consistency was noted in the two groups (group A, tau = 0.855, P < 0.05, and group B, = 0.821, P < 0.05), and no significant difference in accuracy ((2 )= 3.218, P > 0.05) in using intelligent navigation echocardiography was reported between the two groups. However, there a significant difference in accuracy (chi (2 )= 0.021, P < 0.05) when using two-dimensional ultrasound was observed between the two groups. ConclusionIntelligent navigation echocardiography was found to be efficient and accurate for the diagnosis of CTD and good consistency existed in the experienced and less-experienced professionals. Intelligent navigation echocardiography was feasible and accurate for the diagnosis of fetal conotruncal defects.

Abstract:Objective To evaluate the accuracy of prenatal diagnosis of fetal conotruncal anomalies with echocardiography. Methods Fetal echocardiography were conducted for all women in their antenatal visits from Jan 2003 to Dec 2004. The echocardiographic diagnosis was compared with cardiac pathology results. Results Among 2063 women who had fetal echocardiographiy performed, conotruncal anomaly was detected in 28 fetuses with the average gestation of 26. 6 (16-40) weeks when the diagnosis was made, including tetralogy of Fallot (n=5), double outlet right ventricle (n = 6) , transposition of the great arteries (n = 5) , truncus arteriosus (n = 9) and ventricular septal defect with suspected overriding aorta (n=1). Four of the 28 fetuses were lost during follow up and the accurate rate of prenatal assessment of the great artery relationship was 75. 0% (18/24). Two fetuses with prenatal diagnosis of truncus arteriosus were reported as having pulmonary atresia with ventricular septal defect at autopsy. An overriding aorta was suspected in the other case, but postnatal echocardiography was diagnosed as perimembranous subaortic defect. Two fetuses with prenatally suspected double outlet right ventricle were diagnosed as having transposition of the great arteries at autopsy. Only one false-negative diagnosis was made in this series (4. 2%). Conclusions Conotruncal anomalies can be correctly diagnosed by fetal echocardiography. A combination of different views is necessary to define the exact spatial relationship of the great arteries. Specific diagnosis remains a challenge in some cases.

Abstract:Objective The aim of this study was to know about the prenatal ultrasound screening and diagnosis of fetal congenital heart disease.Method Fetus who were diagnosed congenital heart disease through prenatal fetal echocardiography or after birth from April 1st 2010 to December 31st 2011 were collected.We studied the mean gestational weeks of prenatal diagnosis,major abnormalities of heart disease and the outcome of fetus.Results 33 cases of congenital heart disease were prenatal diagnosed.9 of them were born alive and 24 of the others had termination of pregnancy.11 ventricular septal defects,2 pulmonary stenosis and 1 pulmonary atresia were discovered after birth.Conclusions We could discover severe congenital heart disease through ultrasound screening during 20-24 gestational weeks and made a prenatal diagnosis by fetal echocardiography.Thus we could reduce neonatal mortality.

Abstract:Introduction. The objective of our study was to evaluate the performance of detailed fetal echocardiography by skilled obstetric physician sonologists in the diagnosis of congenital heart disease (CHD) in a Chinese population. Material and methods. This investigation included a multicenter prospective cohort of 10 259 pregnant women attending 10 regional tertiary hospitals in China. The inclusion criteria were singleton pregnancy and gestational age from 18 to <= 28 weeks. Women with multiple pregnancies were excluded. A detailed fetal echocardiography was performed by trained physicians with at least 3 years of experience. The primary outcome measures included sensitivity, specificity, and positive and negative likelihood ratios of detailed fetal echocardiography in prenatal detection of CHD. Results. The sensitivity and specificity of fetal echocardiography in detecting any CHD were 33.9 and 99.8%, respectively, in the low-risk population, and 68.8 and 99.4%, respectively, in the high-risk population. For detecting major CHDs, fetal echocardiography had a high sensitivity and specificity, and satisfactory positive and negative likelihood ratios in both the low-risk population (88.2, 100%, 6947.7, and 0.118, respectively) and high-risk population (100, 99.9%, 833.3, and <0.0001, respectively). The sensitivity and likelihood ratios were substantially lower for detecting minor CHDs in both populations. Conclusions. Detailed fetal echocardiography performed by skilled physicians had high detection rate for major CHD in both low-risk and high-risk populations. However, its value for detecting minor CHD was limited. The incorporation of fetal echocardiography with multiple cardiac views into routine ultrasound screening may improve the detection rate of fetal major CHD and facilitate appropriate parental counseling.

Abstract:Objective This study aimed to investigate the genetic etiologies of fetuses with single atria and/or ventricle (SA or/and SV) using different genetic detection methods in a Chinese prenatal cohort. Methods In this retrospective study, the various genetic results of 44 fetuses with SA and/or SV were analyzed. All 44 cases were tested by chromosomal microarray analysis (CMA) and karyotyping simultaneously, and 8 underwent whole exome sequencing (WES). Data on the pregnancy outcomes and neonatal prognoses were collected from medical records and postnatal follow-up. Results The whole cohort of 44 fetuses included 14 SA cases (31.8%), 12 SV cases (27.3%), and 18 SA and SV cases (40.9%). A total of 9 pathogenic genetic results were detected by conventional karyotyping, CMA and trio-WES, indicating an overall detection rate of 20.5% (9/44). Six pathogenic chromosomal abnormalities were identified by CMA among the 44 cases, showing a detection rate of 13.6% (6/44). Two microdeletions being missed by karyotyping were diagnosed by CMA, showing an additional diagnostic yield of 4.5% for CMA in present cohort(2/44). Three pathogenic variants in two fetuses were identified by WES, indicating an incremental diagnostic yield of 4.5%(2/44) for WES in fetuses with SA or/and SV. Conclusion In this study, WES achieved an additional diagnostic yield of 4.5% in fetuses with SA or/and SV. WES is valuable for fetal prognosis assessment and could add diagnostic value for fetuses with SA and/or SV when CMA is negative. It would be a valuable technique for the identification of underlying pathogenic variants in prenatal cohorts.
EP06.06: Analysis of Copy Number Variations in Fetal Conotruncal Heart Defects Using Low‐coverage Whole Genome Sequencing

Abstract:The aim of the study is to explore copy number variations (CNVs) in the fetuses with conotruncal heart defects (CTD). A total of 31 fetuses diagnosed with CTD by fetal echocardiography and verified by postmortem examination were prospectively enrolled for genetic analysis of CNVs in Beijing Anzhen Hospital from January 2013 to September 2015. According to the recommendation of American College of Medical Genetics and Genomics (ACMG), we classified the clinical significance of the CNVs into 5 categories: 1) pathogenic, 2) likely pathogenic, 3) unknown, 4) likely benign and 5) benign. The genotype–phenotype correlation of CTD was also analysed. In this cohort (n = 31), we identified that 64.5% (20/31) of the subjects had CNVs, including 6 with definite pathogenic CNVs (2 with trisomy18, 1 with trisomy 21, 2 with 22q11.2 deletion and 1 with 17p12p11.2 deletion), 2 cases with potentially pathogenic CNVs (1 with 16p13.11 duplication and 1 with novel 2p23.1p25.2 duplication), 11 with unknown, and 7 with likely benign and benign clinical significance. Low-coverage whole genome sequencing identifies a high prevalence of CNVs in fetuses with CTDs. Further investigation of the CNVs with unknown clinical significance may lead to discovery of new causative CNVs, improved understanding of the mechanisms involved in CTDs and more precise diagnosis, prognosis and counselling of CTDs.

Abstract:Objective: To analyze the indications and results of prenatal cytogenetic screening in patients from a specialized center in Xiamen City, China, to provide a reference database for prenatal diagnosis. Materials and Methods: This retrospective, observational study included 7,400 pregnant women who underwent chorionic villous sampling (CVS), amniocentesis or cordocentesis at the Women and Children's Hospital, School of Medicine, Xiamen University, China, over a 10-year period (2008-2018). Clinical data and the results of the cytogenetic analysis were assessed. Results: Fetal chromosomal aberrations were observed in 335 of 7,400 (4.5%) cases, with trisomy 21 the most common aberration (87/335, 26%). A high risk on maternal serum screening was the indication for cytogenetic analysis in 36 of the 87 fetuses with trisomy 21 (41.4%). Abnormal fetal ultrasonographic findings were the clinical indication for cytogenetic testing in more than half of fetuses with abnormal karyotypes (181/335, 54%). Conclusions: Prenatal cytogenetic analysis is useful for the prenatal diagnosis of birth defects in Xiamen City, and may prevent termination of potentially healthy fetuses.

Abstract:Abstract Background Birth defects are responsible for approximately 7% of neonatal deaths worldwide by World Health Organization in 2004. Many methods have been utilized for examining the congenital anomalies in fetuses. This study aims to investigate the efficiency of simultaneous CNV-seq and whole-exome sequencing (WES) in the diagnosis of fetal anomaly based on a large Chinese cohort. Methods In this cohort study, 1800 pregnant women with singleton fetus in Hubei Province were recruited from 2018 to 2020 for prenatal ultrasonic screening. Those with fetal structural anomalies were transferred to the Maternal and Child Health Hospital of Hubei Province through a referral network in Hubei, China. After multidisciplinary consultation and decision on fetal outcome, products of conception (POC) samples were obtained. Simultaneous CNV-seq and WES was conducted to identify the fetal anomalies that can compress initial DNA and turnaround time of reports. Results In total, 959 couples were finally eligible for the enrollment. A total of 227 trios were identified with a causative alteration (CNV or variant), among which 191 (84.14%) were de novo. Double diagnosis of pathogenic CNVs and variants have been identified in 10 fetuses. The diagnostic yield of multisystem anomalies was significantly higher than single system anomalies (32.28% vs. 22.36%, P = 0.0183). The diagnostic rate of fetuses with consistent intra- and extra-uterine phenotypes (172/684) was significantly higher than the rate of these with inconsistent phenotypes (17/116, P = 0.0130). Conclusions Simultaneous CNV-seq and WES analysis contributed to fetal anomaly diagnosis and played a vital role in elucidating complex anomalies with compound causes.

Abstract:This study aimed to assess the efficiency of CNV-seq and WES in detecting genetic cause of congenital heart disease (CHDs) in prenatal diagnoses and to compare CNV detection rate between isolated and non-isolated CHD cases. We conducted a retrospective study of 118 Chinese fetuses diagnosed with CHD by prenatal ultrasound. Participants underwent CNV-seq and, if necessary, WES to detect chromosomal and single nucleotide variations. The overall detection rate for pathogenic or likely pathogenic chromosomal abnormalities was 16.9%, including 7.6% aneuploidies and 9.3% pathogenic/likely pathogenic copy number variations (CNVs), predominantly 22q11.2 deletion syndrome (54.4%). The sensitivity and specificity of CNV-Seq for detecting P/Lp CNVs were 95% and 100%, respectively. CNV-Seq offered a 6.7% improvement in detecting chromosomal abnormalities over karyotyping. WES further identified significant single nucleotide and small indel variations contributing to CHD in genes such as TMEM67, PLD1, ANKRD11, and PNKP, enhancing diagnostic yield by 14.8% in cases negative for CNVs. Non-isolated CHD cases exhibited higher rates of detectable chromosomal abnormalities compared to isolated cases (32.4% vs. 9.9%, p = 0.005), underlining the genetic complexity of these conditions. The combined use of CNV-seq and WES provides a comprehensive approach to prenatal genetic testing for CHDs, unveiling significant genetic cause that could impact clinical management and parental decision-making. This study supports the integration of these advanced genomic technologies in routine prenatal diagnostics to increase detection diagnostic yields of causal genetic variants associated with CHDs.
