Abstract:Clark and colleagues' study "Race and Ethnicity, Gender, and Promotion of Physicians in Academic Medicine" 1 provides a comprehensive analysis of how race, ethnicity, and gender are associated with academic career trajectories of US medical school graduates over a 40-year period. This study builds on prior work documenting racial and ethnic disparities in promotion 2 by introducing an intersectional focus, examining how race and ethnicity and gender together shape promotion outcomes. Using data from 673 576 graduates (1979-2019), Clark and colleagues 1 examined disparities in appointments and promotion through 5 academic ranks, from instructor to department chair, using Cox proportional hazards models. The study highlights that while Asian, Black (only graduates before 2000), and White women were more likely than White men to secure entry-level academic positions as instructors, they were significantly less likely to be promoted to associate and full professor. 1 Black and Hispanic women had the lowest likelihood to advance to higher academic ranks. 1 Notably, among those graduating after 2000, Black, Hispanic, and Indigenous women and men had lower likelihoods of promotion to even entry-level academic positions, in addition to persistent disparities in promotion to higher academic ranks. 1 These findings underscore the persistent challenges that underrepresented faculty face in advancing the ranks of academia, raising critical questions about institutional practices and the need for intentional strategies to address these inequities. The findings in the study by Clark and colleagues 1 challenge the assumption that lack of diversity in academic medicine is due to a lack of interest from minoritized groups. For physicians who belong to minorized racial and ethnic groups, barriers to promotion are not just administrative hurdles but have far-reaching impacts on their professional lives. The failure to advance in academic medicine has a cumulative effect for these faculty, influencing career satisfaction, leadership opportunities, compensation, productivity, and retention. 3 -5 This is particularly concerning given that odds of retention of underrepresented faculty in academic medicine are estimated to be as much as 50% lower than those of White faculty, 3 likely driven by systemic bias, disparities in promotion and receipt of federal funding, lack of mentorship, and minority and gratitude taxes. 4 ,5 These challenges disproportionately affect underrepresented women faculty, who face the compounded burden of cultural taxation, often overburdened with diversity tasks while feeling both invisible and hypervisible in predominantly White academic spaces. 6 Their intersecting identities further create what is known as a minority woman tax, which further limits productivity and career progression, 6 underscoring the need for future studies of trends in promotion to consider additional intersections, such as sexual minority, gender minority, and first-generation status. While most findings are consistent with prior racial and ethnic disparities documented in the medical literature, the study investigators 1 do note a surprising finding regarding promotion to chair. Although Black men were less likely than White men to be promoted to associate professor from assistant professor and to full professor from associate professor, Clark et al 1 report that Black men were more likely to be promoted to the position of chair than White men. A similar finding is noted for Hispanic men who graduated medical school after 2000. 1 These results seem counterintuitive and deserve further contextualization and examination. Notably, Clark et al 1 observed that Black and Hispanic men experience inequitable promotion rates at every stage of the academic pathway, except for promotion to chair. This may suggest that while institutions prioritize diversity at the most senior and visible rank, systemic barriers persist throughout the earlier stages of the academic pipeline. Such patterns could mask deeper inequities, creating a facade of diversity and inclusion without ensuring equitable promotion opportunities at all levels. It is important to note that only 85 Black men and just 67 Hispanic men were appointed to positions of chair during the 40-year study timeframe, compared with 1627 White men. 1 Although not reported in the article, it would be useful to better understand what percentage of chair appointments for Black and Hispanic men were at historically Black medical schools and medical schools in Puerto Rico.
