Abstract:The American Journal of Sports Medicine, Ahead of Print. Background:Running technique and running speed are considered important risk factors for running injuries. Real-time feedback on running technique and running speed by wearables may help reduce injury risk.Purpose:To investigate whether real-time feedback on spatiotemporal metrics and relative speed by commercially available pressure-sensitive insoles would reduce running injuries and improve running performance compared with no real-time feedback.Study Design:Randomized controlled trial; Level of evidence, 1.Methods:A total of 220 recreational runners were randomly assigned into the intervention and control groups. Both groups received pressure-sensitive insoles, but only the intervention group received real-time feedback on spatiotemporal metrics and relative speed. The feedback aimed to reduce loading on the joint/segment estimated to exhibit the highest load. Injury rates were compared between the groups using Cox regressions. Secondary outcomes compared included injury severity, the proportion of runners with multiple injuries, changes in self-reported personal best times and motivation (Behavioral Regulation in Exercise Questionnaire–2), and interest in continuing wearable use after study completion.Results:A total of 160 participants (73%) were included in analyses of the primary outcome. Intention-to-treat analysis showed no significant difference in injury rate between the groups (Hazard ratio [HR], 1.11; P = .70). This was expected, as 53 of 160 (33%) participants ended up in the unassigned group because they used incorrect wearable settings, nullifying any interventional effects. As-treated analysis showed a significantly lower injury rate among participants receiving real-time feedback (HR, 0.53; P = .03). Similarly, the first-time injury severity was significantly lower (–0.43; P = .042). Per-protocol analysis showed no significant differences in injury rates, but the direction favored the intervention group (HR, 0.67; P = .30). There were no significant differences in the proportion of patients with multiple injuries (HR, 0.82; P = .40) or changes in running performance (3.07%; P = .26) and motivation. Also, ~60% of the participants who completed the study showed interest in continuing wearable use.Conclusion:Real-time feedback on spatiotemporal metrics and relative speed provided by commercially available instrumented insoles may reduce the rate and severity of injuries in recreational runners. Feedback did not influence running performance and exercise motivation.Registration:NL8472 (Dutch Trial Register).

Abstract:Running economy, known as the steady-state oxygen consumption at a given submaximal intensity, has been proposed as one of the key factors differentiating East African runners from other running communities around the world. Kenyan runners have dominated middle- and long-distance running events and this phenomenon has been attributed, in part at least, to their exceptional running economy. Despite such speculation, there are no data on running mechanics during real-life situations such as during training or competition. The use of innovative wearable devices together with real-time analysis of data will represent a paradigm shift in the study of running biomechanics and could potentially help explain the outstanding performances of certain athletes. For example, the integration of foot worn inertial sensors into the training and racing of athletes will enable coaches and researchers to investigate foot mechanics (e.g., an accurate set of variables such as pitch and eversion angles, cadence, symmetry, contact and flight times or swing times) during real-life activities and facilitate feedback in real-time. The same technological approach also can be used to help the athlete, coach, sports physician, and sport scientist make better informed decisions in terms of performance and efficacy of interventions, treatments or injury prevention; a kind of "telesport" equivalent to "telemedicine." There also is the opportunity to use this real-time technology to advance broadcasting of sporting events with the transmission of real-time performance metrics and in doing so enhance the level of entertainment, interest, and engagement of enthusiasts in the broadcast and the sport. Such technological advances that are able to unobtrusively augment personal experience and interaction, represent an unprecedented opportunity to transform the world of sport for participants, spectators, and all relevant stakeholders.
Running biomechanics as measured by wearable sensors: effects of speed and surface

Abstract:Running biomechanics research has traditionally occurred in the laboratory, but with the advent of wearable sensors measurement of running biomechanics may shift outside the laboratory. The purpose was to determine if RunScribe™ wearable sensors could detect differences in kinematic, kinetic and spatiotemporal measures during runs at two speeds and on two different surfaces. Fifteen recreational runners (7 males, 8 females; age = 20.0 ± 3.1 years) participated. While wearing sensors on the heels of their shoes, participants completed four 1600 m runs on both track and grass surfaces. On each surface, the first 1600 m was at a self-selected slow speed followed by the second 1600 m at a self-selected fast pace. The sensors quantified several kinetic, kinematic and spatiotemporal measures. Repeated measures ANOVAs compared the effects of surface and speed. The spatiotemporal measures of stride length, cycle time and contact time were predictably affected by increased running speed and increased surface stiffness, as were the kinematic and kinetic measurements of maximum pronation velocity, maximum pronation excursion, impact g, and braking g (p < 0.050). The RunScribe™ sensors identified expected changes in running biomechanics measures at different speeds and on varying surfaces.
Learning effects in over-ground running gait retraining: A six-month follow-up of a quasi-randomized controlled trial

Abstract:This study evaluated learning and recall effects following a feedback-based retraining program. A 6-month follow-up of a quasi-randomized controlled trial was performed with and without recall. Twenty runners were assigned to experimental or control groups and completed a 3-week running program. A body-worn system collected axial tibial acceleration and provided real-time feedback on peak tibial acceleration for six running sessions in an athletic training facility. The experimental group received music-based biofeedback in a faded feedback scheme. The controls received tempo-synchronized music as a placebo for blinding purposes. The peak tibial acceleration and vertical loading rate of the ground reaction force were determined in a lab at baseline and six months following the intervention to assess retention and recall. The impacts of the experimental group substantially decreased at follow-up following a simple verbal recall (i.e., run as at the end of the program): peak tibial acceleration:-32%, p = 0.018; vertical loading rate:-34%, p = 0.006. No statistically significant changes were found regarding the retention of the impact variables. The impact magnitudes did not change over time in the control group. The biofeedback-based intervention did not induce clear learning at follow-up, however, a substantial impact reduction was recallable through simple cueing in the absence of biofeedback.

Footwear-specific biomechanical and energetic responses to 8 weeks of training in advanced footwear technology

Abstract:While the acute effects of advanced footwear technology (AFT) on running biomechanics and efficiency have been extensively studied, the longitudinal effects of AFT use are unknown. The purpose of this study was to investigate the effects of using advanced footwear technology (AFT) versus traditional racing flats during running workouts on relative running economy (RE) and energetic cost (EC), and explore associations between changes in footwear-specific biomechanics and performance. Thirteen competitive runners were randomly assigned Nike Vaporfly Next% 3 (VP) or Nike Rival Waffle 5 (FL) for an 8-week intervention and completed pre- (PRE) and post-intervention (POST) lab testing in both shoes. Weekly training data, including mileage, workouts, and soreness, were collected via questionnaires. Subjects ran two 3-min trials at a self-selected cross-country race pace, with sagittal plane ankle and metatarsophalangeal (MTP) joint kinematics and kinetics recorded and analysed. Subsequently, four 5-min trials were completed at a self-reported submaximal pace, in a randomized order, and shoe-specific RE and EC were calculated and reported as VP% benefit the percentage RE or EC improvement in VP versus FL. Pearson′s correlations were tested between RE outcomes and exploratory biomechanics – measures, and independent samples t-tests tested for group differences in shoe-specific and overall efficiency changes from PRE to POST. VP% benefit increased from PRE to POST in VP trained runners and decreased in FL trained runners. VP trained runners decreased ankle plantarflexion velocity in VP, increased ankle dorsiflexion velocity in VP, and decreased MTP plantarflexion velocity in FL. Correlations revealed several significant associations between metabolic outcome measures, and biomechanical and training variables. Metabolic data support a footwear specificity of training principle (habituation), where training in VP may enhance a runner′s ability to benefit from VP on race day. Associations reveal that this may be due to changes in footwear-specific biomechanics at the ankle and MTP joints. Future research should explore the potential mechanisms through which runners adapt to AFT to maximize their performance benefits.

Abstract:Biomechanical assessments of running typically take place inside motion capture laboratories. However, it is unclear whether data from these in-lab gait assessments are representative of gait during real-world running. This study sought to test how well real-world gait patterns are represented by in-lab gait data in two cohorts of runners equipped with consumer-grade wearable sensors measuring speed, step length, vertical oscillation, stance time, and leg stiffness. Cohort 1 (N = 49) completed an in-lab treadmill run plus five real-world runs of self-selected distances on self-selected courses. Cohort 2 (N = 19) completed a 2.4 km outdoor run on a known course plus five real-world runs of self-selected distances on self-selected courses. The degree to which in-lab gait reflected real-world gait was quantified using univariate overlap and multivariate depth overlap statistics, both for all real-world running and for real-world running on flat, straight segments only. When comparing in-lab and real-world data from the same subject, univariate overlap ranged from 65.7% (leg stiffness) to 95.2% (speed). When considering all gait metrics together, only 32.5% of real-world data were well-represented by in-lab data from the same subject. Pooling in-lab gait data across multiple subjects led to greater distributional overlap between in-lab and real-world data (depth overlap 89.3–90.3%) due to the broader variability in gait seen across (as opposed to within) subjects. Stratifying real-world running to only include flat, straight segments did not meaningfully increase the overlap between in-lab and real-world running (changes of <1%). Individual gait patterns during real-world running, as characterized by consumer-grade wearable sensors, are not well-represented by the same runner's in-lab data. Researchers and clinicians should consider "borrowing" information from a pool of many runners to predict individual gait behavior when using biomechanical data to make clinical or sports performance decisions.

The Correlation between Running Economy and Maximal Oxygen Uptake: Cross-Sectional and Longitudinal Relationships in Highly Trained Distance Runners

Abstract:A positive relationship between running economy and maximal oxygen uptake (V̇O2max) has been postulated in trained athletes, but previous evidence is equivocal and could have been confounded by statistical artefacts. Whether this relationship is preserved in response to running training (changes in running economy and V̇O2max) has yet to be explored. This study examined the relationships of (i) running economy and V̇O2max between runners, and (ii) the changes in running economy and V̇O2max that occur within runners in response to habitual training. 168 trained distance runners (males, n = 98, V̇O2max 73.0 ± 6.3 mL∙kg-1∙min-1; females, n = 70, V̇O2max 65.2 ± 5.9 mL kg-1∙min-1) performed a discontinuous submaximal running test to determine running economy (kcal∙km-1). A continuous incremental treadmill running test to volitional exhaustion was used to determine V̇O2max 54 participants (males, n = 27; females, n = 27) also completed at least one follow up assessment. Partial correlation analysis revealed small positive relationships between running economy and V̇O2max (males r = 0.26, females r = 0.25; P<0.006), in addition to moderate positive relationships between the changes in running economy and V̇O2max in response to habitual training (r = 0.35; P<0.001). In conclusion, the current investigation demonstrates that only a small to moderate relationship exists between running economy and V̇O2max in highly trained distance runners. With >85% of the variance in these parameters unexplained by this relationship, these findings reaffirm that running economy and V̇O2max are primarily determined independently.

Abstract:Understanding the influence of running-induced acute fatigue on the homeostasis of the body is essential to mitigate the adverse effects and optimize positive adaptations to training. Fatigue is a multifactorial phenomenon, which influences biomechanical, physiological, and psychological facets. This work aimed to assess the evolution of these three facets with acute fatigue during a half-marathon. 13 recreational runners were equipped with one inertial measurement unit (IMU) on each foot, one combined global navigation satellite system-IMU-electrocardiogram sensor on the chest, and an Android smartphone equipped with an audio recording application. Spatio-temporal parameters for the running gait, along with the heart rate, its variability and complexity were computed using validated algorithms. Perceived fatigability was assessed using the rating-of-fatigue (ROF) scale at every 10 min of the race. The data was split into eight equal segments, corresponding to at least one ROF value per segment, and only level running parts were retained for analysis. During the race, contact time, duty factor, and trunk anteroposterior acceleration increased, and the foot strike angle and vertical stiffness decreased significantly. Heart rate showed a progressive increase, while the metrics for heart rate variability and complexity decreased during the race. The biomechanical parameters showed a significant alteration even with a small change in perceived fatigue, whereas the heart rate dynamics altered at higher changes. When divided into two groups, the slower runners presented a higher change in heart rate dynamics throughout the race than the faster runners; they both showed similar trends for the gait parameters. When tested for linear and non-linear correlations, heart rate had the highest association with biomechanical parameters, while the trunk anteroposterior acceleration had the lowest association with heart rate dynamics. These results indicate the ability of faster runners to better judge their physiological limits and hint toward a higher sensitivity of perceived fatigue to neuromuscular changes in the running gait. This study highlights measurable influences of acute fatigue, which can be studied only through concurrent measurement of biomechanical, physiological, and psychological facets of running in real-world conditions.

The Biomechanics of Competitive Male Runners in Three Marathon Racing Shoes: A Randomized Crossover Study

Abstract:Background: We have shown that a prototype marathon racing shoe reduced the metabolic cost of running for all 18 participants in our sample by an average of 4%, compared to two well-established racing shoes. Gross measures of biomechanics showed minor differences and could not explain the metabolic savings. Objective: To explain the metabolic savings by comparing the mechanics of the shoes, leg, and foot joints during the stance phase of running. Methods: Ten male competitive runners, who habitually rearfoot strike ran three 5-min trials in prototype shoes (NP) and two established marathon shoes, the Nike Zoom Streak 6 (NS) and the adidas adizero Adios BOOST 2 (AB), at 16 km/h. We measured ground reaction forces and 3D kinematics of the lower limbs. Results: Hip and knee joint mechanics were similar between the shoes, but peak ankle extensor moment was smaller in NP versus AB shoes. Negative and positive work rates at the ankle were lower in NP shoes versus the other shoes. Dorsiflexion and negative work at the metatarsophalangeal (MTP) joint were reduced in the NP shoes versus the other shoes. Substantial mechanical energy was stored/returned in compressing the NP midsole foam, but not in bending the carbon-fiber plate. Conclusion: The metabolic savings of the NP shoes appear to be due to: (1) superior energy storage in the midsole foam, (2) the clever lever effects of the carbon-fiber plate on the ankle joint mechanics, and (3) the stiffening effects of the plate on the MTP joint.
Habituation Does Not Change Running Economy in Advanced Footwear Technology

Abstract:Purpose: This study aimed to compare running economy across habituated and nonhabituated advanced footwear technology (AFT) in trained long-distance runners. Methods: A total of 16 participants completed up to six 5-minute trials in 1 to 3 pairs of their own habituated shoes and 3 different and standardized AFTs at individual marathon pace. We measured oxygen uptake and carbon dioxide production and expressed running economy as oxygen uptake (in milliliters oxygen per kilogram per minute), oxygen cost of transport (oxygen per kilogram per minute), energetic cost (in watts per kilogram), and energetic cost of transport (in joules per kilogram per kilometer). We used linear mixed-effect models to evaluate differences. Relative shoe weight and shoe mileage (distance worn during running) were covariates. Results: Forty-eight standardized and 29 individual AFT conditions were measured (mileage 117.0 [128.8] km, range 0–522 km; 25 habituated 135.7 [129.2] km, range 20–522 km; 4 nonhabituated 0 [0] km, range 0–0 km). Rating of perceived exertion, blood [La], and respiratory exchange ratio ranged from 9 to 15, 1.11 to 4.54 mmol/L, and 0.76 to 1.01. There was no effect for habituation on energetic cost of transport (thabituation = −.232, P = .409, b = −0.006; 95% CI, −0.058 to 0.046) or other running economy metrics. Neither shoe weight nor shoe mileage had an effect. Conclusions: Our results suggest that habituation to AFTs does not result in greater benefits in the use of AFTs. This means that implementation in training may not be needed, even if we cannot rule out any other possible benefits of habituation at this stage, such as adaptation of the musculoskeletal system.

Effects of longitudinal bending stiffness and midsole foam on running energetics

Abstract:Background: Advanced footwear technology has become commonplace in the competitive running world. However, a systematic exploration of the effects of each of the primary components has not yet been presented. Purpose: To quantify running economy and step parameters in four different shoe conditions: PEBA shoes with plate, PEBA shoes without plate, EVA shoes with plate and EVA shoes without plate. Methods: Participants ran at 14 km/h (n = 8 males) or 12 km/h (n = 6 females) on a treadmill. The shoe order was randomly assigned for the four shoe conditions, where the participants wore each shoe twice in a mirrored order. Results: There was a significant reduction in metabolic power (1.0%) associated with the presence of a plate, as well as a similar reduction based on foam type (1.0%). However, there was no significant interaction between presence of plate and foam type. Adding a plate significantly reduced metabolic power for EVA by 1.3% but not for PEBA. PEBA foam significantly reduced metabolic power by 1.3% in shoes without a plate, but not for the shoes with a plate. Step frequency and contact time were similar between shoes and not correlated to running economy improvements. Conclusions: Starting from a baseline condition with traditional foam without a plate, either adding a plate or using PEBA foam improved running economy with a similar amount (1.3%). Adding the alternate second feature non-significantly improved running economy with an additional 0.6%. The benefit of both technologies combined (1.9%) was smaller than the sum of its parts (1.3% each).

Revealing the Mutual Information between Body-Worn Sensors and Metabolic Cost in Running

Abstract:Running power is a popular measure to gauge objective intensity. It has recently been shown, though, that foot-worn sensors alone cannot reflect variations in the exerted energy that stems from changes in the running economy. In order to support long-term improvement in running, these changes need to be taken into account. We propose leveraging the presence of two additional sensors worn by the most ambitious recreational runners for improved measurement: a watch and a heart rate chest strap. Using these accelerometers, which are already present and distributed over the athlete's body, carries more information about metabolic demand than a single foot-worn sensor. In this work, we demonstrate the mutual information between acceleration data and the metabolic demand of running by leveraging the information bottleneck of a constrained convolutional neural network. We perform lab measurements on 29 ambitious recreational runners (age = 28 ± 7 years, weekly running distance = 50 ± 25 km, V˙O2max = 60.3 ± 7.4 mL · min-1·kg-1). We show that information about the metabolic demand of running is contained in kinetic data. Additionally, we prove that the combination of three sensors (foot, torso, and lower arm) carries significantly more information than a single foot-worn sensor. We advocate for the development of running power systems that incorporate the sensors in watches and chest straps to improve the validity of running power and, thereby, long-term training planning.

A Comparison of the Energetic Cost of Running in Marathon Racing Shoes

Abstract:Background: Reducing the energetic cost of running seems the most feasible path to a sub-2-hour marathon. Footwear mass, cushioning, and bending stiffness each affect the energetic cost of running. Recently, prototype running shoes were developed that combine a new highly compliant and resilient midsole material with a stiff embedded plate. Objective: The aim of this study was to determine if, and to what extent, these newly developed running shoes reduce the energetic cost of running compared with established marathon racing shoes. Methods: 18 high-caliber athletes ran six 5-min trials (three shoes × two replicates) in prototype shoes (NP), and two established marathon shoes (NS and AB) during three separate sessions: 14, 16, and 18 km/h. We measured submaximal oxygen uptake and carbon dioxide production during minutes 3-5 and averaged energetic cost (W/kg) for the two trials in each shoe model. Results: Compared with the established racing shoes, the new shoes reduced the energetic cost of running in all 18 subjects tested. Averaged across all three velocities, the energetic cost for running in the NP shoes (16.45 ± 0.89 W/kg; mean ± SD) was 4.16 and 4.01% lower than in the NS and AB shoes, when shoe mass was matched (17.16 ± 0.92 and 17.14 ± 0.97 W/kg, respectively, both p < 0.001). The observed percent changes were independent of running velocity (14-18 km/h). Conclusion: The prototype shoes lowered the energetic cost of running by 4% on average. We predict that with these shoes, top athletes could run substantially faster and achieve the first sub-2-hour marathon.

Abstract:BACKGROUND: Despite the increasing popularity of endurance running competitions among adolescent runners, there is currently limited information regarding expected biomechanical changes across the duration of a long-distance running event, and the relationship between young runners' biomechanics and running performance. Wearable technology offers an ecological means to continuously assess runners' biomechanical data during outdoor running competitions.RESEARCH QUESTION: Do adolescent athletes adopt changes in sensor-derived biomechanics throughout a marathon race, and are there relationships between race performance and biomechanical features among young marathoners?METHODS: Fourteen high-school aged runners (9 M, 5 F; age: 16 ± 1 years, height: 170.8 ± 7.5 cm; mass: 63.6 ± 9.4 kg) wore lace-mounted sensors to record step-by-step biomechanics during a marathon race. Official race segment completion times were extracted across 5 race segments (5-K, 15-K, Half Marathon [21.1-K], 35-K, Marathon [42.2-K]). Within-participant repeated measures of covariance (pace) were conducted to assess changes in biomechanics across the race, with Bonferroni post-hoc comparisons. Pearson's r correlations were performed to assess the relationship between race finish times and biomechanics.RESULTS: Pace was significantly slower (p-range: 0.002-0.005), contact times significantly longer, and stride lengths significantly shorter in the final segment compared to middle segments (p-range: 0.003-0.004). The rate of shock accumulation was significantly higher in the final race segment compared to the first three segments (p-range: 0.001-0.002). Moderate relationships existed between finish times and pace (r = -0.63), stride length (r = -0.62), and contact time (r = 0.51).SIGNIFICANCE: Adolescent runners altered their gait patterns in the final marathon segment compared to earlier segments. Spatiotemporal measures were moderately correlated with race finish times, suggesting a link between faster run pace, increased stride lengths, and reduced contact time for improved running performance during an endurance race.

Abstract:BACKGROUND: Quantitative gait analysis is essential for evaluating walking and running patterns for markers of pathology, injury, or other gait characteristics. It is expected that the portability, affordability, and applicability of wearable devices to many different populations will have contributed advancements in understanding the real-world gait patterns of walkers and runners. Therefore, the purpose of this systematic review was to identify how wearable devices are being used for gait analysis in out-of-lab settings.METHODS: A systematic search was conducted in the following scientific databases: PubMed, Medline, CINAHL, EMBASE, and SportDiscus. Each of the included articles was assessed using a custom quality assessment. Information was extracted from each included article regarding the participants, protocol, sensor(s), and analysis.RESULTS: A total of 61 articles were reviewed: 47 involved gait analysis during walking, 13 involved gait analysis during running, and one involved both walking and running. Most studies performed adequately on measures of reporting, and external and internal validity, but did not provide a sufficient description of power. Small, unobtrusive wearable devices have been used in retrospective studies, producing unique measures of gait quality. Walking, but not running, studies have begun to use wearable devices for gait analysis among large numbers of participants in their natural environment.CONCLUSIONS: Despite the advantages provided by the portability and accessibility of wearable devices, more studies monitoring gait over long periods of time, among large numbers of participants, and in natural walking and running environments are needed to analyze real-world gait patterns, and would facilitate prospective, subject-specific, and subgroup investigations. The development of wearables-specific metrics for gait analysis provide insights regarding the quality of gait that cannot be determined using traditional components of in-lab gait analyses. However, guidelines for the usability of wearable devices and the validity of wearables-based measurements of gait quality need to be established.

Abstract:Running is one of the most popular sports with hundreds of millions of participants worldwide. Good running form is the key to fast, efficient, and injury-free running. Existing kinematic analysis technologies, such as high-speed camera systems, are expensive, difficult to operate, and exclusive to sports physiology laboratories and elite athletes. Miniature MEMS-based motion sensors enable portable high-precision kinematic analysis, but suffer from high energy consumption hence short battery lifetime, especially for continued online analysis for running. This paper presents Gazelle, a wearable online analysis system for running that is compact, lightweight, accurate, and highly energy efficient; intended for runners of all levels. To enable long-term maintenance-free mobile analysis for running, Sparse Adaptive Sensing (SAS) is proposed, which selectively identifies the best sampling points to maintain high accuracy while greatly reducing sensing and analysis energy overheads. Experimental results demonstrate 97.7 percent accuracy with 76.9 to 99 percent reduced energy consumption (83.6 percent average reduction under real-world testing)-a one-order-of-magnitude improvement over existing solutions. SAS enables $>$ 200 days of continuous high-precision operation using only a coin-cell battery. Since 2014, Gazelle has been used by over 100 elite and recreational runners during daily training and at top-level races like the Kona Ironman World Championships and New York Marathon.
