Abstract:Purpose: To assess the efficacy and safety of simultaneous mitomycin C application under conjunctival and scleral flaps in patients with repeat trabeculectomy.Methods: A total of 44 patients (44 eyes) with previous failed filtering surgery were randomized to one of two groups. The both-flaps group comprised 22 patients (22 eyes) with trabeculectomy and intraoperative mitomycin C application under conjunctival and scleral flaps, whereas the subconjunctival group comprised 22 patients (22 eyes) with subconjunctival application of mitomycin C. Particular attention was paid to intraocular pressure, postoperative medications, visual acuity, filtering bleb appearance, and complications. The mean follow-up time was 38.18 +/- 12.48 months.Results: The mean preoperative intraocular pressure decreased from 39.1 +/- 7.3 mm Hg to the postoperative level of 15.6 +/- 4.8 mm Hg in the both-flaps group (P = 0.014), and from 39.4 +/- 8.4 to 18.7 +/- 5.8 mm Hg in the subconjunctival group (P = 0.018). There was a statistically significant difference in intraocular pressure at all follow-up times, except at 1 week and 1 month postoperatively. Kaplan-Meier survival analysis showed there was no significant difference in total success rate (complete plus qualified success) between the two group, (P = 0.622, log-rank test). However, the two survival curves for the complete success subgroups (without additional medications) confirmed that mitomycin C applications under both flaps had a higher success rate than subconjunctival application (P = 0.043, log-rank test). No statistically significant difference in medications was present between the two groups, and no severe complications developed in either group.Conclusions: Trabeculectomy augmented with mitomycin C application at both sites could produce a greater lowering of intraocular pressure with low incidence of postoperative complications, and could provide an increased chance of long-term success. The procedure is effective and safe in patients with repeat trabeculectomy.

Abstract:AIM: To evaluate the safety and efficacy of posterior scleral application (a modified technique) of an antimetabolite mitomycin C-soaked sponge in trabeculectomy for patients with glaucoma. METHODS: This retrospective study included 101 patients (115 eyes) with glaucoma (aged 12–83y) who underwent trabeculectomy using a modified mitomycin C-soaked sponge placement method. A piece of 3.5×10 mm2 sponge was placed vertically and posteriorly with the long side perpendicular to the limbus. The mitomycin C concentration and exposure time were 0.2–0.5 mg/mL and 1–5min, respectively. Intraocular pressure, best-corrected visual acuity, and hypotensive medications were recorded at baseline and at the final visit. Complications, interventions required, and bleb morphology were recorded postoperatively. The primary outcome was trabeculectomy safety, including complications and bleb morphology; the secondary outcome was the trabeculectomy success rate. RESULTS: At the final follow-up [median 28mo, range 7–67mo and interquartile range (IQR) 13mo], the qualified (cumulative) success rate was 93.0% and the complete success rate was 60.0%. No bleb-related complications were observed. The mean height, extent, and vascularity grades were 0.6±0.9, 1.1±0.4, and 2.4±0.9, respectively. All Seidel tests were negative. The mean posteriority grade was 0.8±0.4. CONCLUSION: Trabeculectomy with the long side of a mitomycin C-soaked sponge placed perpendicular to the corneal limbus is safe and effective.
Subconjunctival Versus Intrascleral Application of Mitomycin C During Trabeculectomy.

Abstract:Purpose: To compare efficacy and safety between subconjunctival and intrascleral methods of mitomycin C (MMC) application during trabeculectomy Methods: This retrospective study included 165 eyes treated by trabeculectomy and compared clinical outcomes between eyes that received subconjunctival MMC (Group 1, 80 eyes, 48.5%) and eyes that received intrascleral (Group 2, 85 eyes, 51.5%) MMC. Surgical success was defined as an intraocular pressure (IOP) of 6-21 mmHg with or without topical antiglaucoma medication use. Results: Mean IOP values were not significantly different between Group 1 and Group 2 (P > 0.05) until 2 weeks after surgery, while they differed significantly at 1, 3, 6, 12, and 24 months after surgery (P < 0.05). The mean number of antiglaucoma medications was 3.6 ± 0.6 and 3.5 ± 0.7, respectively, before surgery (P = 0.327) and 0.27 ± 0.32 and 0.10 ± 0.29, respectively, at 24 months after surgery (P = 0.001). The surgical success rate at 24 months was 86.25% and 94.12% in Groups 1 and 2, respectively (P = 0.041; log-rank test). Encysted blebs were observed in 17 and 8 patients in Groups 1 and 2 (P = 0.034), respectively, at 24 months. There was no statistically significant difference in the incidence of hypotony, choroidal detachment, conjunctival leakage, cataract and the rate of postoperative procedures between Groups 1 and 2 (P > 0.05). Conclusions: Compared to the subconjunctival method, the intrascleral method of MMC application can increase the long-term success of trabeculectomy without increasing the complication rate.
Application of the Sodium Hyaluronate in Mitomycin C Trabeculectomy

Abstract:Objective To study the application of sodium hyaluronate (SH)in mitomycin C (MMC) trabeculectomy.Methods 98 cases (116 eyes)were operated with MMC trabeculectomy.SH was injected into the anterior chamber and under the sutured scleral flaps in 52 cases(62 eyes) ,and the other 46 cases(54 eyes) as control group were jnjected into BSS.The postoperative visual acuity, intraocular pressure (IOP), filtration, forming of anterior chamber and the postoperative complications were compared between the two groups. Results The postoperative visual acuity, the forming of anterior chamber or the postoperative complications in the treatment group was better than that in the control group(P<0.01 or P<0.05).The postoperative IOP of the treatment group was(8.3±1.4)mmHg(second day),or(7.5±1.6)mmHg(1week later),or(13.2±2.4) mmHg(half a year later)averagely and that of the control group was(5.6±1.0)mmHg(P<0.05),or(6.8±1.2)mmHg(P>0.05), or(17.6±3.8)mmHg(P<0.05).All cases were followed up for 6 months to 3 years(18 months averagely).The rate of functional filtering bubles was 96.8% in the treatment group,and that in the control groupwas 85.2%(P<0.05).The successful rate was 95.2% and 81.5% respectively in two groups(P<0.05).Conclusins sodium hyaluronate using in MMC trabeculectomy can decrease the postoperative complications and elevate the successful rate.
Safety and Efficacy of Mitomycin C: Augmented Trabeculectomy: Subtenon's Injection versus Sponge Application

Abstract:Objective: The objective of the study was to study the safety and efficacy of subtenon injection of mitomycin C (MMC) versus sponge application of MMC during trabeculectomy. Materials and methods: Thirty-seven patients having primary glaucoma warranting trabeculectomy were enrolled in the study and their forty eyes were alternately allocated into either of the two groups: subtenon injection (ST) of 0.1 mL of 0.01% of MMC or sponge application (SP) of 0.02% of MMC and were operated by a single surgeon and followed for 3 months. The outcome was analyzed primarily based on reduction in intraocular pressure (IOP) and bleb morphology. Results: Similar outcome in terms of complete success (ST - 90% and SP - 85%), qualified success (ST - 5% and SP - 5%), and failure rate (ST - 5% and SP - 10%) was seen at the end of 3 months. The absolute reduction in IOP from the baseline was -10.00 ± 3.67 mmHg (-41.2% ± 12.30) in ST versus -8.90 ± 5.56 mmHg (-35.9% ± 16.1) in the SP group at the end of 3 months. At the end of 3 months, blebs in the ST group had low-to-medium height and in the SP group had low height. Blebs in both the groups were diffuse with mild vascularity. Antiglaucoma medications required postoperatively were 0.20 ± 0.62 versus 0.40 ± 1.10 in the ST and SP group, respectively. The duration of surgery was 19.85 ± 0.75 min in the ST group versus 22.50 ± 0.51 min in the SP group. Conclusion: Subtenon injection of MMC is as efficacious and safe as the conventional sponge application technique.

Abstract:Trabeculectomy with adjunctive use of Mitomycin C (MMC) has been a benchmark for glaucoma filtration surgery for decades. However, there are many variations in the ways that the sponges soaked with MMC are applied during the trabeculectomy surgery. We herein describe our way of placing the MMC-soaked sponges to improve the safety and efficacy of the trabeculectomy. The sponges are placed vertically and posteriorly with the long side of the sponge perpendicular to the limbus, not parallel. This will reduce the size of the conjunctival wound at the limbus to preserve more virgin conjunctiva that can be used for repeated trabeculectomy when needed. This will also facilitate a more posteriorly directed flow of aqueous drainage that, in turn, may increase the success rate of the trabeculectomy. We have obtained encouraging results in our practice, and further large-scale randomized studies seem warranted.
Efficacy and safety of intraoperative injection of mitomycin C during trabeculectomy: a systematic review and meta-analysis

Abstract:Purpose: Trabeculectomy, a primary surgical treatment for glaucoma, often employs mitomycin C (MMC) to reduce scar formation and improve surgical outcomes. However, the optimal application method of MMC, whether by injection or sponge, remains a subject of debate. This meta-analysis aims to compare injectable and sponge-based MMC application in terms of efficacy and safety, focusing on various clinical outcomes in glaucoma patients. Methods: A comprehensive literature search of Scopus, MEDLINE, EMBASE, Ovid, Chinese biomedical literature database, China National Knowledge Infrastructure, and Cochrane Library was done for eligible studies that report data of glaucoma patients who were administered MMC by injection or sponge application during trabeculectomy. Outcomes of interest included intraocular pressure (IOP) reduction, bleb appearance grading (height, extent, vascularity), use of anti-glaucoma medications, and rates of complete success, qualified success, and failure. Data were reported as weighted mean differences (WMD) or odds ratios (OR) with confidence intervals (CI). The random-effects inverse-variance model with DerSimonian-Laird estimate of tau2 was employed, with continuity correction applied where necessary. Results: A total of 15 studies with 1276 participants were included. The meta-analysis revealed no significant difference in IOP reduction between patients treated by MMC injection and sponge application (WMD = - 0.434). Significant differences were observed in bleb appearance grading scores for height (WMD = - 0.170) and extent (WMD = 0.174), with substantial heterogeneity. The use of anti-glaucoma medications was significantly lower in the injection group (WMD = - 0.274). However, there were no significant differences in the rates of complete success, qualified success, and failure. The study demonstrated moderate to high heterogeneity across various outcomes. Conclusion: This meta-analysis indicated that while both injection and sponge methods of MMC application during trabeculectomy were equally effective for IOP reduction, they differ in their impact on bleb morphology and postoperative medication requirement. The findings highlight the need for individualized treatment approaches in glaucoma surgery, taking into account the specific needs and characteristics of each patient.
Surgical outcomes of trabeculectomy augmented with sub-Tenon injected mitomycin C

Abstract:Background: Recently, trabeculectomy with mitomycin C (MMC) where MMC is applied by injection into the Tenon layer has attracted close attention. However, the data on efficacy and safety of this technique is still limited and more clinical studies are needed. Therefore, the work is aimed at comprehensive evaluation of the effectiveness of trabeculectomy using MMC applied by intra-Tenon injection. Methods: A set of 50 eyes in 50 patients underwent trabeculectomy using MMC at concentration of 0.4 mg/ml in a total volume of 0.05 ml. The primary end point was to control intraocular pressure (IOP) on postoperative days 1, 8, 30 and 90 and subsequently at 6 and 12 months after surgery. The secondary end point was to evaluate the changes in various corneal parameters prior to and 90 days after surgical procedure. Results: The mean preoperative IOP was 32.34 ± 9.45 mmHg. After surgery, the mean IOP significantly decreased to 17.52 ± 4.58 mmHg at the 90-day follow-up, and to 18.14 ± 3.74 and 19.30 ± 3.82 mmHg at 6 and 12 months after the procedure, respectively. The mean BCVA values remained unchanged compared to baseline (0.77 ± 0.23) to the 90-day follow-up (0.80 ± 0.23). The mean number of anti-glaucoma medications significantly reduced from 3.50 ± 0.74 to 0.58 ± 1.03 postoperatively. Similarly, the mean corneal hysteresis and ACD of the eye as well as CECD were significantly changed postoperatively. Conclusions: Trabeculectomy using MMC applied by injection is a safe and effective surgical method for the treatment of primary and secondary forms of open-angle glaucoma. It has a significant hypotonising effect and allows a complete discontinuation of antiglaucoma drugs (Tab. 3, Fig. 3, Ref. 58).
CSM enhances the filtration bleb survival in rabbit model of experimental glaucoma surgery.

Abstract:Purpose: The aim of this study is to investigate whether CS-g-MMCs conjugate (CSM) could be a new agent to prevent the post-operative fibrosis in a rabbit model of experimental glaucoma filtration surgery. Materials and methods: In a randomized, controlled, masked-observer study, 40 New Zealand White rabbits underwent trabeculectomy in the right eyes and randomly received subconjunctival injection of phosphate buffered saline, chitosan (CS), CSM (100 mu g/ml), CSM (200 mu g/ml) or Mitomycin C (100 mu g/ml). Bleb characteristics and anterior chamber depth were evaluated by slit-lamp examination. The animals were killed on day 14 and 28. Histopathology and immunohistochemistry were performed to determine the amount of the scarring and fibrosis. Ocular toxicity was assessed by histopathology and electron microscope. Results: We found that the five groups were similar with respect to intraocular pressure and anterior chamber depth. The medians for survival days were: 5.5, 8, 17.5, 28 and 16 in the PBS, CS, CSM100, CSM200 and MMC groups, respectively. Both the CSM200 and the MMC group showed a significantly larger bleb area than the CSM100, CS and the PBS group. Less scarring was seen on day 14 and 28 in CSM200 and MMC group than in the PBS, CS and CSM100 group by histology and immunohistochemistry assessment. No damages were found in the rabbit eyes in each group. Conclusions: Subconjunctival injection of CSM postoperatively can improve the filtration bleb survival in the rabbit model. It can be a safe and effective antimetabolite in glaucoma surgery.

Abstract:目的丝裂霉素C能抑制翼状胬肉切除术后的复发,对胬肉切除术术中应用丝裂霉素C与术后应用的结果进行比较,评估两种方法的有效性和安全性.材料与方法将原发性翼状胬肉患者随机分为两组,术中应用0.02%丝裂霉素C组和术后用0.02%丝裂霉素C组,观察两组患者术后复发率和并发症情况.结果共有91例患者103只眼纳入本研究,其中术中用丝裂霉素C组50只眼中3眼复发(6.0%),术后用丝裂霉素C组53只眼中4眼复发(7.5%),两组间差别无统计学意义(p＞0.05);随访期间两组患者均没有出现严重的并发症.结论翼状胬肉切除术联合术中应用丝裂霉素C与术后应用丝裂霉素C都是治疗翼状胬肉有效的方法,而术中应用丝裂霉素C可能更为安全。

Abstract:Objective To investigate the long-term changes of tear film and ocular surface epithelium after trabeculectomy and the influential factors, especially the effects of mitomycin-C(MMC)and bleb shape on ocular surface of glaucoma patients.Methods One hundred and three patients (139 eyes)underwent trabeculectomy more than 6 months ago were included.Routine ocular examinations and ocular surface examinations,including Schirraer I test (S 1 t),break-up time (BUT),fluorescein staining,rose bengal staining,and eonjunctival impression cytology (CIC), were conducted.The precidence of thin-wall bleb in different MMC groups was analyzed.The effects of intraoperative use of MMC and postoperative bleb shape on the tear film and ocular surface epithelium of the patients were analyzed.Results Compared with normal controls,the BUT value was significantly lowerened,ocular surface vital staining was increased,with marked conjunetival epithelial squamous metaplasia of CIC in glaucoma patients.The preeidenee of thin-wall bleb was significantly higher in high-concentration MMC group than low-concentration MMC group and blank control group.The goblet cell density(GCD)of conjunctival epithelium was significantly related to the intranperative use of MMC and postoperative bleb shape.Conclusions Dry eye conditions existed in the glaucoma patients long after trabeculectomy.Intraoperative use of high-eoneentration-MMC may increase the precidenee of thin-wall bleb after trabeculectomy.Decrease of conjunctival GCD in the long term after the operation was related to use of MMC and the elevated conjunctival bleb.
Outcomes and Risk Factors for Failure of Trabeculectomy in Glaucomatous Patients in Southwest China: a 325 Eyes Analysis

Abstract:AIM: To evaluate the outcomes and elucidate the failure factors for trabeculectomy with mitomycin C (MMC) in Southwest Chinese patients. METHODS: A retrospective correlational study was conducted on the glaucomatous patients who underwent initial trabeculectomy with MMC in Southwest Hospital and had been followed up for 1-3y. A complete success for surgery is defined as a postoperative intraocular pressure (IOP) >5 and ≤21 mm Hg and 20% reduction of IOP compared to preoperative, without IOP-lowering medications. A qualified success for surgery is defined as the abovementioned postoperative IOP with or without IOP-lowering medications. The primary outcomes were IOP, the number of IOP-lowering medications, and cumulative success rate. The secondary outcomes included best corrected visual acuity (BCVA), mean deviation (MD) of visual field, major complications, and risk factors for surgical failure. RESULTS: A total of 325 eyes of 261 glaucomatous patients had been included in our study. Both the mean IOP and the number of IOP-lowering medications were significantly decreased from 32.9±12.0 to 16.4±5.7 mm Hg (P<0.0001) and 3.0±0.9 to 0.9±1.0 (P<0.0001), respectively, at the last visit. The cumulative complete success rate and qualified success rate were 77.8% and 92.0% at 1-year follow-up, and 47.2% and 77.7% at 3-year follow up. There were no significant differences in surgical outcomes between primary angle-closure glaucoma (PACG) and primary open angle glaucoma (POAG). In PACG patients, the success rates of trabeculectomy were comparable with those of phacotrabeculectomy. Hypertension (HR=1.904, P=0.011), encapsulated bleb (HR=2.756, P<0.001), and more preoperative topical medications (HR=2.475, P=0.008) were risk factors for surgical failure. CONCLUSION: The qualified success rate of trabeculectomy with MMC in glaucomatous patients in the cohort is 92.0% at 1-year, and 77.7% at 3-year follow up. Hypertension, encapsulated bleb, and more preoperative topical medications are associated with surgical failure.

Abstract:Objective:To investigate the effect of two-site phacotrabeculectomy with adjustable suture in cataract and glaucoma patients.Methods:Eighty-four patients(92 eyes)with cataract and glaucoma were included in this retrospective study.In two-site group,38 patients(42 eyes)accepted two sites phacotrabeculectomy with adjustable suture and MMC.In one-site group,46 patients(50 eyes)accepted one site phacotrabeculectomy with MMC.The change of vision,IOP,the success rate,the type of filtering bleb at 6 month after surgery and the complication after surgery between the two groups were analyzed.Results:There was significant difference between the two groups in the change of IOP.There was significant difference between the two groups in complication at 3 d and 1 W、1 m and 6 m post-operatively,but not at 1 d post-operatively.Conclusion:The IOP could be controlled effectively by two sites phacotrabeculectomy with adjustable suture introduced in our study.

Abstract:We assessed whether mitomycin-C (MMC) has different antifibrotic mechanisms in trabeculectomy wound healing.We identified 2 concentrations of MMC as "low-dose" by using WST-1 assay, Lactic dehydrogenase assay, and fluorescence-activated cell sorting flow cytometry. Senescence-associated β-galactosidase (SA-β-gal) and fibrotic gene expression was examined through immunocytochemistry, flow cytometry, real-time quantitative reverse transcription polymerase chain reaction, Western blotting, zymography, and modified scratch assay in vitro. In vivo, 0.1 mL of MMC or normal saline was injected to Tenon's capsule before trabeculectomy in a rabbit model. SA-β-gal expression, apoptotic cell death, and collagen deposition in sites treated and not treated with MMC were evaluated using terminal dUTP nick end labeling assay and histochemical staining. Bleb function and intraocular pressure (IOP) levels were examined 3, 7, 14, 21, 28, and 35 days after trabeculectomy.In vitro, human Tenon's fibroblast (HTF) senescence was confirmed by observing cell morphologic change, SA-β-gal accumulation, formation of senescence-associated heterochromatin, increased p16INK4a and p21CIP1/WAF1 expression, lower percentage of Ki-67-positive cells, and decreased COL1A1 release. Increased expression of α-SMA, COL1A1, and Smad2 signaling in TGF-β1-induced stress fibers were passivated in senescent HTFs. In addition, cellular migration enhanced by TGF-β1was inactivated. In vivo, histological examination indicated increased SA-β-gal accumulation, lower apoptosis ratios, and looser collagen deposition in sites treated with 0.2 μM MMC. Low-dose MMC-induced cellular senescence prolonged trabeculectomy bleb survival and reduced IOP levels in a rabbit model.Low-dose MMC-induced cellular senescence is involved in the antifibrotic mechanism of trabeculectomy wound healing.

Aqueous shunts with mitomycin C versus aqueous shunts alone for glaucoma

Abstract:Background: Glaucoma affects more than 70 million people worldwide, with about 10% being bilaterally blind, making it the leading cause of irreversible blindness globally. In patients with advanced glaucoma or those who have failed medical treatment without achieving adequate intraocular pressure (IOP) control, trabeculectomy (glaucoma filtration surgery where an ostium is created into the anterior chamber from underneath a partial thickness scleral flap to allow for aqueous flow out of the anterior chamber intointo the subconjunctival space forming a filtering bleb) and aqueous shunt surgery for more complex and refractory cases remain the mainstay therapies. Proliferation of fibrous tissue around an implanted aqueous shunt may block the diffusion of aqueous humour. Mitomycin C (MMC) is one of two commonly used adjunct antifibrotic agents used during aqueous shunt surgery to prevent proliferation of fibrous tissue. However, the effectiveness and safety of the use of intraoperative MMC during aqueous shunt surgery has not been established. Objectives: To evaluate the effectiveness and safety of MMC versus no MMC used during aqueous shunt surgery for reducing IOP in primary and secondary glaucoma. Search methods: We searched the Cochrane Central Register of Controlled Trials (CENTRAL) (which contains the Cochrane Eyes and Vision Trials Register) (2018, Issue 2); Ovid MEDLINE; Embase.com; PubMed; Latin American and Caribbean Health Sciences Literature Database (LILACS); ClinicalTrials.gov and the World Health Organization (WHO) International Clinical Trials Registry Platform (ICTRP). We did not use any date or language restrictions in the electronic search for trials. We last searched the electronic databases on 13 February 2018. Selection criteria: We included randomized controlled trials (RCTs) in which one group of participants received MMC during aqueous shunt surgery and another group did not. We did not exclude studies based on outcomes. Data collection and analysis: Two review authors independently reviewed titles and abstracts from the literature searches. We obtained full-text reports of potentially relevant studies and assessed them for inclusion. Two review authors independently extracted data related to study characteristics, risk of bias, and outcomes. We used standard methodological procedures expected by Cochrane. Main results: We included five RCTs, with a total of 333 eyes with glaucoma randomized, and identified two ongoing trials. All included trials examined the effect of MMC versus no MMC when used during aqueous shunt surgery for glaucoma. The trials included participants with different types of uncontrolled glaucoma. One study was conducted in China, one in Saudi Arabia, two in the USA, and one study was a multicenter study conducted in Brazil, Canada, Scotland, and USA. We assessed all trials as having overall unclear risk of bias due to incomplete reporting of study methods and outcomes; two of the five trials were reported only as conference abstracts.None of the included trials reported mean decrease from baseline in IOP; however, all five trials reported mean IOP at 12 months post-surgery. At 12 months, the effect of MMC on mean IOP compared with no MMC was unclear based on a meta-analysis of trials (mean difference -0.12 mmHg, 95% CI -2.16 to 2.41; low-certainty evidence). Two trial did not report sufficient information to include in meta-analysis, but reported that mean IOP was lower in the MMC group compared with the no MMC group at 12 months.None of the included trials reported mean change from baseline in visual acuity; however, one trial reported lower mean LogMAR values (better vision) in the MMC group than in the no MMC group at 12 months post-surgery. None of the included studies reported the proportion of participants with stable best-corrected visual acuity. Three trials reported that loss of vision was not significantly different between groups (no data available for meta-analysis).None of the included studies reported the proportion of participants with a postoperative hypertensive phase, which is defined as IOP > 21 mmHg within 3 months after surgery. Two trials reported adverse events (choroidal effusion, corneal edema, flat anterior chamber, and retinal detachment); however, due to small numbers of events and sample sizes, no clear difference between MMC and placebo groups was observed. Authors' conclusions: We found insufficient evidence in this review to suggest MMC provides any postoperative benefit for glaucoma patients who undergo aqueous shunt surgery. Data across all five included trials were sparse and the reporting of study methods required to assess bias was inadequate. Future RCTs of this intervention should report methods in sufficient detail to permit assessment of potential bias and estimate target sample sizes based on clinically meaningful effect sizes.
A Long-Term Study with Stringent Success Criteria

Abstract:Background: The aim was to evaluate the long-term outcome and efficacy of primary trabeculectomy with adjunctive mitomycin c (MMC) for treating glaucoma. Methods: We examined the medical records of 286 eyes that underwent trabeculectomy between 2008 and 2009 at the University Eye Hospital in Freiburg, Germany. Preoperative and follow-up data were collected, including intraocular pressure (IOP) measurements, surgical glaucoma interventions, and prescribed glaucoma medication. The first success criterion was defined as IOP ≤ 15 mmHg with no use of pressure-lowering medication by the patient, the second criterion was defined as the absence of surgical revision, and the third criterion as no further IOP-lowering surgery excluding early revisions following trabeculectomy. Statistical analyses comprised Cox regression and Kaplan–Meier survival estimations. Results: The mean follow-up duration was 1841 days (5 years). The mean preoperative IOP was 26.1 mmHg. Evaluating the success criteria at the time of average follow-up yielded a success rate of only 25% for the first criterion but 80% for both the second and third success criteria. Conclusions: The findings suggest that trabeculectomy with adjunctive MMC can be an effective procedure for permanently lowering IOP. However, surgical revisions and/or further glaucoma surgeries might still be needed. The long-term success rate is lower in comparison to previous research, which may be explained by the stricter success criteria in our study.

Efficacy of treatment for pterygium by autologous conjunctival transplantation and mitomycin C

Abstract:Background: The recurrence of pterygium after surgery is high. A variety of adjunctive treatment approaches have been developed to improve the clinical efficacy and many related articles have been published. This study aimed to determine the risk for postoperative pterygium recurrence comparing autologous conjunctival transplantation (ACT) versus mitomycin C (MMC). Methods: Relevant literature published until December 2010 in Medline, Embase, Cochrane, Cochrane library, Science Citation Index, and Google Scholar were searched. Qualified random clinical trial (RCT) studies on the comparison of recurrence rate of pterygium after ACT and MMC treatment were included in this study. Results: Eight RCTs with 663 eyes entered the final analysis. The recurrent rate of pterygium was 8.7% (30/343) for ACT and 18.75% (60/320) for MMC. Using fixed-effect meta analysis, we found that the recurrence was significantly lower after ACT than MMC treatment (odds ratio (OR) = 0.40, 95% confidence index (CI), 0.25 - 0.63, P < 0.0001). In sensitivity analyses, we employed random-effects model and excluded studies of low quality or studies in which MMC was administrated after the operation. All the sensitivity analyses confirmed that ACT led to lower recurrence rates than MMC. Sub-group analysis revealed that the recurrence rate was 20.2% (20/99) and 27.65% (26/94) for conjunctival autograft (CA) and MMC respectively, and no significant difference in the recurrence rate was detected (OR = 0.65, 95%CI 0.33 - 1.28, P = 0.22). However, we found that conjunctival limbal autograft (CLA) had lower recurrence rate than MMC (OR = 0.26, 95%CI 0.14 - 0.48, P = 0.0001). Conclusion: CLA has better therapy efficacy against the recurrence of pterygium than MMC.
The Effect of Mitomycin C on the Corneal Endothelium and Corneal Posterior Surface after Laser Subepithelial Keratomileusis

Abstract:Objective To investigate the clinical effect of mitomycin C (MMC) on the corneal endothelium and corneal posterior surface after laser subepithelial keratomileusis (LASEK).Methods In a continuity prospective double-blind study,0.02％ MMC was used topically for 20 seconds in 33 eyes of 33 patients who had undergone LASEK after excimer laser ablation in the Fudan EENT Hospital.The other eye of these patients without MMC treatment was used as the control.Three and 6 months after surgery,the changes in the curve and elevation of the corneal posterior surface,and the density and variation coefficient of corneal endothelial cells were observed to evaluate the MMC toxicity.Data were analyzed using a paired t test.Results In the MMC-treated eyes,3 and 6 months after the operation,the average corneal endothelium cell density was 3353.3±290.3 cells/mm2 and 3407.6±298.4 cells/mm2; the variation coefficient was 13.36％±6.29％ and 13.48±4.34％.In the control eyes 3 and 6 months after the operation,the average corneal endothelium cell density was 3258.7±284.6 cells/mm2 and 3339.2±213.4 cells/mm2; the variation coefficient was 14.52％±7.85％ and 14.06％±6.50％.The changes in the elevation and curve of the corneal posterior surface in the MMC treated eyes were-0.67±2.26 μm and-0.52±2.67 μm and 0.01±0.20 D and 0.01±0.21 D,3 and 6 months after the operation.The changes in the control eyes were-0.85±2.31 μm and -0.15±3.00 μm and-0.01±0.22 D and 0.05+0.17 D at the respective time points after the operation.There were no significant differences in the above parameters either between preoperative or postoperative data or between these two groups.Conclusion The study showed that the application of 0.02％ mitomycin C in LASEK is safe.
