Abstract:OBJECTIVE:To evaluate the efficacy and safety of reoperation of residual myopia because of regression, undercorrection with or without coexisting severe corneal haze after excimer laser photorefractive keratectomy (PRK).METHODS:A VISK 20/20 excimer laser was used to treat myopia of -1.00 to -16.50 diopters (spherical equivalent) at corneal plane. 35 patients (51 eyes) who had residual myopia because of regression, undercorrection with or without coexisting severe corneal haze after initial PRK were retreated by using PRK and phototherapeutic keratectomy.RESULTS:At 12 months after retreatment, the rates of uncorrected visual acuity more than or equal to 10/20 or 20/20 were 82.6% and 39.1% respectively, 100.0% and 50.0% in myopia up to -6.00 D, 73.3% and 33.3% in myopia of -6.25 to -16.50 D respectively. 86.3% of eyes were within +/- 1.00 diopters of desired emmetropia. Corneal haze after retreatment was not greater than that observed after initial procedures.CONCLUSIONS:The residual myopia because of regression, undercorrection with or without severe corneal haze after initial PRK can be successfully retreated. Although the therapeutic result of retreatment of PRK is less successful than that of the initial procedure, retreatment appears to enhance it.

Abstract:Purpose. To assess refractive and visual outcomes and patient satisfaction of wavefront-guided photorefractive keratectomy (PRK) in eyes with myopia and compound myopic astigmatism, with the ablation profile derived from a new Hartmann-Shack aberrometer. Methods. In this retrospective study, 662 eyes that underwent wavefront-guided PRK with a treatment profile derived from a new generation Hartmann-Shack aberrometer (iDesign aberrometer, Abbott Medical Optics, Inc., Santa Ana, CA) were analyzed. The preoperative manifest sphere ranged from -0.25 to -10.75 D, and preoperative manifest cylinder was between 0.00 and -5.25 D. Refractive and visual outcomes, vector analysis of the change in refractive cylinder, and patient satisfaction were evaluated. Results. At 3 months, 91.1% of eyes had manifest spherical equivalent within 0.50 D. The percentage of eyes achieving uncorrected distance visual acuity 20/20 or better was 89.4% monocularly and 96.5% binocularly. The mean correction ratio of refractive cylinder was 1.02 ± 0.43, and the mean error of angle was 0.00 ± 14.86° at 3 months postoperatively. Self-reported scores for optical side effects, such as starburst, glare, halo, ghosting, and double vision, were low. Conclusion. The use of a new Hartmann-Shack aberrometer for wavefront-guided photorefractive keratectomy resulted in high predictability, efficacy, and patient satisfaction.

Abstract:Purpose: To examine patient-reported outcomes and higher order aberrations following topography-guided laser-assisted in situ keratomileusis (LASIK). Methods: This was a prospective, nonrandomized observational study at a single academic center. Sixty eyes from 30 patients underwent bilateral topography-guided femtosecond LASIK for correction of myopia using the VisuMax 500 femtosecond laser (Zeiss; Oberkochen, Germany) and Allegretto Wave Eye-Q 400 Hz Excimer Laser (Alcon/Wavelight; Erlangen, Germany) with the Contoura topography system (Alcon; Geneva, Switzerland) for topography-modified refraction. The main outcomes of this study were higher order aberrations (HOAs) and results from the Patient-Reported Outcomes with LASIK questionnaire. Results: There was a small, significant increase in HOA root mean square, spherical aberration, and coma at 1, 3, 6, and 12 months following topography-guided LASIK (all P < 0.05), but no change in trefoil. In addition, self-reported worry related to vision (P < 0.001) and ability to perform activities (P < 0.001) significantly improved after surgery. The prevalence of double images, glare, halos, or starbursts decreased from 73% preoperatively to 56%, and no participants reported "very" or "extremely" bothersome visual symptoms after 12 months. Dry eye symptoms per Ocular Surface Disease Index score decreased significantly at 6 (P = 0.01) and 12 (P = 0.002) months after surgery. There was a 100% satisfaction rate with visual outcomes and duration of time to improvement in vision following the procedure. Conclusions: Although there was an increase in HOAs following topography-guided LASIK, there were significant improvements in the presence of double images, glare, halos, and starbursts and vision-related quality-of-life metrics. Overall satisfaction rates in this study were high.
A Long-Term Outcome of Photorefractive Keratectomy for Myopia

Abstract:Objective To evaluate the long-term outcomes of excimer laser myopic photorefractive keratectomy (PRK).Methods This retrospective study mainly measured patients' visual acuity,refractive predictability more than 8 years (8.78± 1.54 yrs) after PRK.The total 23 patients (45 eyes)were divided into 3 groups:low myopia (＜-3D),moderate myopia (≥-3D and ＜-6D),high myopia (≥-6D).Results Postoperative UCVA ≥20/20 was noted in 7 eyes (100％) of low myopia,14 eyes (51.85％) of moderate myopia.5 eyes (18.52％) in moderate myopia and 3 eyes (30％) in high myopia lost ≥l-line of BCVA.And 8 eyes (100％),17 eyes (62.96％),3 eyes (30％) were within ± 1D of attempted correction respectively.The mean RMS of high order aberration was significant different among the three groups postoperatively.One eye (3.7％) in moderate myopia,two eyes (20％) in high myopia had haze grade of 0.5 and haze grade of 1 was noted in two eyes in high myopia.Conclusions PRK has a satisfactory effect for low to moderate myopia but less satisfactory result for high myopia.Corneal haze can still exist in the long term,especially in high myopia.
Retrospective Comparison of Visual Outcomes After KAMRA Corneal Inlay Implantation With Simultaneous PRK or LASIK

Abstract:Purpose: To report a retrospective study of simultaneous LASIK versus photorefractive keratectomy (PRK) with accompanying small-aperture cornea inlay implantation (KAMRA; AcuFocus, Inc., Irvine, CA) in treating presbyopia. Methods: Simultaneous LASIK/inlay and simultaneous PRK/inlay was performed on 79 and 47 patients, respectively. Follow-up examinations were conducted at 1, 3, and 6 months postoperatively. The main outcome measures were safety, efficacy, predictability, and stability with primary emphasis on monocular uncorrected near visual acuity (UNVA). Results: Both groups met U.S. Food and Drug Administration criteria for efficacy with 95% and 55% of the LASIK/inlay group and 83% and 52% of the PRK/inlay group having a monocular UNVA of 20/40 (J5) and 20/25 (J2), respectively, at 6-month follow-up. Ninety-two percent of the LASIK/inlay group and 95% of the PRK/inlay group had a UDVA of 20/40 or better at 6 months. Two eyes lost one line of corrected distance visual acuity (CDVA). Mild hyperopic shift was noted in both groups at 6 months. Conclusions: Simultaneous PRK/inlay and LASIK/inlay meet the U.S. Food and Drug Administration standards for efficacy and safety based on 6-month preliminary results and have similar outcomes to emmetropic eyes. Abstract:Two mutants of the tylosin-producing Streptomyces fradiae defective in the biosynthesis of the macrolide antibiotic tylosin were isolated from colonies derived from regenerated protoplasts. Both strains were unable to carry out any of at least seven tylosin biosynthetic steps and were sensitive to tylosin. One strain, JS82, was also more sensitive to chloramphenicol (Cm), mitomycin C (Mc), hygromycin B (Hm) and kanamycin (Km) than its parent strain. The other strain, JS87, was also more sensitive to Cm than wild type but expressed normal levels of resistance to Mc and Hm. Both strains expressed genetic instabilities associated with auxotrophy or expression of antibiotic resistance. Since the genetic instabilities were not due to defective error-free or error-prone DNA repair, they appear to be due to genetic rearrangements associated with the deletion or amplification of sequences linked to and perhaps encompassing tylosin biosynthesis genes.
Changes in the Higher Order Ocular Aberrations and Central Corneal Thickness After T-PRK and Fs-LASIK

Abstract:INTRODUCTION: Refractive surgery procedures, transepithelial photorefractive keratectomy (T-PRK), and femtosecond laser in situ keratomileusis (Fs-LASIK) are regarded as safe and efficacious methods for correcting myopia and myopic astigmatism. These two methods do not have many differences in results when treating spherical myopia while differences exist in the treatment of astigmatism correction. Vector analysis presents a powerful tool to show the real differences between these two methods regarding high ocular aberrations and central corneal thickness of treated eyes.AIM: The aim of the study is to investigate changes in higher order ocular aberrations (HOAs) and central corneal thickness (CCT) following treatment of myopia and myopic astigmatism above -5.00DS and up to -2.00DC after either T-PRK or Fs-LASIK.METHODS: Patients (30 eyes per group) underwent T-PRK (group I) or Fs-LASIK (group II) procedure using the Schwind Amaris 750S laser. HOAs (3mm&5mm pupil) and CCT were measured objectively at pre-, 1,3 & 6 months postop in each case.RESULTS: Key results at 6 months were: i) mean values of trefoil (5mm pupil) were 0.092μm (sd,0.055,95% CI 0.072 to 0.112) & 0.126μm (sd,0.078,95% CI 0.098 to 0.154) in group I, and 0.088μm (sd,0.058,95% CI 0.067 to 0.109) & 0.064μm (sd,0.034,95% CI 0.052 to 0.076) in group II (P=0.001 at 6 months); ii) Changes in CTT (ΔCTT) and best spherical equivalent correction (ΔBSE) was significant in group II (ΔCCT=-26.55[ΔBSE]-14.06,R=0.486,P=0.006) but not in group I (p=0.034).CONCLUSIONS: After T-PRK trefoil is worse than Fs-LASIK. The predictability of corneal changes is better following Fs-LASIK.

Abstract:Background: This retrospective chart review compared the higher-order aberrations (HOAs) among photorefractive keratectomy (PRK), laser-assisted in situ keratomileusis (LASIK), and small incision lenticule extraction (SMILE) alongside changes in spherical equivalent (SEQ) and corneal shape (Q-value). Methods: Analyzing 371 myopic eyes, including 154 LASIK, 173 PRK, and 44 SMILE cases, Pentacam imaging was utilized pre-operatively and at one-year post-operative visits. Results: All procedures resulted in 100% of patients achieving an uncorrected distance visual acuity (UDVA) of 20/40 or better, with 87% of LASIK and PRK, and 91% of SMILE patients having 20/20 or better. Significant increases in HOAs were observed across all procedures (p < 0.05), correlating positively with SEQ and Q-value changes (LASIK (0.686, p < 0.05), followed by PRK (0.4503, p < 0.05), and SMILE (0.386, p < 0.05)). Vertical coma and spherical aberration (SA) were the primary factors for heightened aberration magnitude among the procedures (p < 0.05), with the largest contribution in SMILE, which is likely attributed to the centration at the corneal apex. Notably, PRK showed insignificant changes in vertical coma (−0.197 μm ± 0.0168 to −0.192 μm ± 0.0198, p = 0.78), with an increase in oblique trefoil (p < 0.05). Conclusions: These findings underscore differences in HOAs among PRK, LASIK, and SMILE, helping to guide clinicians.

Advances in Transepithelial Photorefractive Keratectomy versus Laser-Assisted In Situ Keratomileusis

Abstract:(1) Background: Laser-assisted refractive surgery is a safe and effective surgical correction of refractive error. For most patients, both the newer Trans-PRK and the established LASIK technique can produce the required surgical correction, sparking the question of which technique should be opted for. (2) Methods: The study prospectively evaluated 121 patients (230 eyes) for at least one month postoperatively; 66 patients (126 eyes) and 45 patients (85 eyes) returned for 6 months and 1 year follow-up. (3) Results: No statistical difference was recorded at 1 week or 1 month post-operation. At 6 months, a difference was found for spherical diopters (Trans-PRK −0.0476 ± 0.7012 versus FS-LASIK +0.425 ± 0.874, p = 0.004) and spherical equivalent (Trans-PRK −0.1994 ± 0.0294 versus FS-LASIK +0.225 ± 0.646, p = 0.025) but not for CYL D (Trans-PRK −0.3036 ± 0.5251 versus FS-LASIK −0.4 ± 0.820, p = 0.499). Uncorrected visual acuity was better for Trans-PRK 6 months post-operation (UCVA logMAR 0.02523 versus 0.0768 logMAR; p = 0.015 logMAR). At 1-year, Trans-PRK was favored for spherical diopters (Trans-PRK −0.0294 ± 0.6493 versus FS-LASIK +0.646 ± 0.909, p < 0.001) and spherical equivalent (Trans-PRK −0.218 ± 0.784 versus FS-LASIK 0.372 ± 1.08, p = 0.007). Overall speed in visual recovery, variance of results and surgically induced astigmatism were in favor of Trans-PRK. (4) Conclusions: The study reported improvements for Trans-PRK patients, with both techniques found to be safe and effective.

Abstract:Purpose: The study of change of the epithelium of the cornea in two points after Photo Refractive Keratectomy (PRK): at the peak of the cornea (ETpeak) and in the middle of the cornea (ETcenter). The study of the change of Lower Order Aberrations (LOAs) and Higher Order Aberrations (HOAs), Contrast Sensitivity (CS) and Corrected Distance Visual Acuity (CDVA) and the relationship of these parameters with the change of ETpeak and ETcenter after PRK. Materials and Methods: Twenty-seven patients (5 women and 22 men) with an average age of 27 years (age range 23-36) underwent PRK. For two years all 27 patients had stable refraction. The refractive objective was emmetropia. Results: After PRK, a statistically significant difference was found between pre-surgery and post-surgery measurements of ETpeak and ETcenter with in relation to LOAs, HOAs, CS and CDVA. As well as between CS and HOA (Z33). Conclusion: In conclusion, after PRK, the change in ETcenter and ETpeak is associated with the change in CDVA and the changes in LOAs and HOAs. Also, the change in CDVA is related to SC, LOAs and HOAS. Finally, the change in the ETcenter appears to be related to the change in the CS.
Safety and efficacy of a laser refractive procedure in eyes with previous keratoplasty: systematic review and meta-analysis

Abstract:Post-keratoplasty (KP) patients' visual acuity can be severely limited by ametropia. Irregular astigmatisms are frequently encountered in these patients and is commonly associated with high degrees of hyperopia or myopia. This systematic review investigates the safety and efficacy of laser refractive surgery for post KP vision correction. 31 studies with 683 participants (732 eyes) enrolled in this review. Mean astigmatism improved significantly (mean difference [MD] = -2.70, 95% CI, -3.13 to -2.28, P < .0001). As well as Mean spherical equivalent (MD = -3.35, 95% CI, -3.92 to -2.78, P < .0001). From 586 participants 5.8% lost 2 or more lines of corrected distance visual acuity after treatment. The proportion of eyes with 20/40 uncorrected distance visual acuity or better was reported and the percentage was 46.79% overall. Laser refractive procedures (laser in situ keratomileusis [LASIK] or photorefractive keratectomy [PRK] or transepithelial PRK) on eyes with corneal transplantation were found to be relatively safe and effective. Our systematic review shows there is improvement in all outcomes. Main adverse effects were haze for PRK and epithelial ingrowth for LASIK.

Improvement after Topography-Guided Photorefractive Keratectomy with Corneal Cross-Linking in Patients with Keratoconus

Abstract:Purpose: To investigate if topography-guided photorefractive keratectomy (TGPRK) alleviates headache, particularly headache attributed to refractive errors (HARE) in keratoconus. Methods: Patients diagnosed with keratoconus undergoing TGPRK for refractive correction were included. Best spectacle corrected visual acuity (BSCVA) using the logMAR scale and refractive error were measured. Patients answered a questionnaire exploring headaches, characteristics, treatment, and the Headache Impact Test (HIT-6) before and 6 months after the surgery. Results: 40 patients were included. Preoperatively, 24 patients (60%) met criteria for headaches: five for migraine, 14 for HARE, and five for tension-type headache (TTH). Patients with headaches preoperatively were more likely to require bilateral TGPRK, and the mean sphere and cylindrical power were higher. Postoperatively, 15 out of the 24 patients of the headache group experienced complete resolution of headaches, and only nine patients met diagnostic criteria for headaches: two for migraine, six for HARE, and one for TTH. The number of headaches reduced from 4.4 ± 2.4 to 0.5 ± 0.7 days/week (p < 0.001). Headache duration decreased from 108.5 ± 100.7 min to 34.4 ± 63.5 min (p = 0.002). Postoperatively, the consumption of analgesia decreased. The HIT-6 revealed an improvement in the quality-of-life post-procedure (p < 0.001). Conclusions: Surgical correction of irregular astigmatism in patients with keratoconus can alleviate or resolve headaches in a large proportion of patients, resulting in an improvement in their quality of life. Physicians should consider keratoconus in patients fitting criteria for HARE not alleviated by spectacle correction and suboptimal vision in glasses.

[A case of ovarian malignant Sertoli-Leidig cell tumor treated with CBDCA, etoposide and epirubicin chemotherapy].

Abstract:A 49-year-old female presented with a low abdominal tumor. Before operation, she had neither evidence of androgen excess nor abnormal tumor marker values, but US, CT and MRI findings strongly suggested the possibility of a malignant ovarian tumor. Her operative findings were as follows: a goose egg-sized main tumor in the low abdomen, with a walnut-sized tumor in the right side, which grew around the right ureter, causing right non-functional kidney. Pathological examination revealed her tumor was a very rare type of malignant Sertoli-Leidig cell tumor with pelvic lymph nodes metastases. Most patients with this disease usually have good prognoses, but with metastasis or recurrence, no therapy is as effective as in epithelial ovarian cancer. In this case, we selected a new combination chemotherapy of CBDCA, Etoposide and Epirubicin, considering current changes in the chemotherapy for ovarian germ cell tumors and Sertoli-Leidig cell tumors of testis. Now, 1 year and 4 months after operation, she has no evidence of recurrence or metastasis. This study proposes a new, presumably more effective chemotherapy for an ovarian malignant Sertoli-Leidig cell tumor.

Abstract:Abstract Background To compare the clinical outcomes of transepithelial photorefractive keratectomy (TPRK) with femtosecond laser-assisted in situ keratomileusis (FS-LASIK) for correction of high myopia. Methods In this prospective, non-randomised, cohort study, 85 eyes of 46 patients treated with TPRK and 80 eyes of 42 patients treated with FS-LASIK were included. All eyes were highly myopic (spherical equivalent refraction <− 6.00 diopters). Both TPRK and FS-LASIK were performed by Schwind Amaris 750S excimer laser. Visual acuity, refraction, corneal high order aberration (HOA) and other variables were analyzed before and at 1, 3, 6, 12 months after surgery. Results At 12 months after surgery, uncorrected logMAR distance visual acuity (UDVA) in the TPRK and FS-LASIK groups was − 0.04 ± 0.04 and − 0.01 ± 0.08, respectively ( P = 0.039). Corrected logMAR distance visual acuity (CDVA) was − 0.06 ± 0.05 and − 0.04 ± 0.05 in both groups ( P = 0.621). For UDVA, 86% of eyes in the TPRK group and 80% in the FS-LASIK group remained unchanged or improved one or more logMAR lines ( P = 0.314), compared to preoperative CDVA. For CDVA, 97% of eyes in the TPRK group and 90% in the FS-LASIK group remained unchanged or improved one or more lines ( P = 0.096), compared to preoperative CDVA. Spherical equivalent refraction was − 0.05 ± 0.39 and − 0.26 ± 0.47 in both groups ( P = 0.030). 87% of eyes in the TPRK group and 73% in the FS-LASIK group achieved ±0.50 D target refraction ( P = 0.019). All 85 eyes (100%) in the TPRK group and 75 eyes (92%) in the FS-LASIK group were within ±1.00 D of target ( P = 0.003). Root mean square (RMS) of corneal total HOA and vertical coma in the TPRK group were lower compared with the FS-LASIK group ( P < 0.001 for both variables). Conclusions TPRK and FS-LASIK showed good safety, efficacy and predictability for correction of high myopia. Clinical outcomes of TPRK were slightly better than FS-LASIK.

Abstract:Proinflammatory cytokines may be important in the pathogenesis of human immunodeficiency virus type 1 (HIV-1) disease. Tenidap decreases interleukin (IL)-6, IL-1, and tumor necrosis factor (TNF) production by peripheral blood mononuclear cells and decreases IL-6 plasma levels in rheumatoid arthritis patients. In this randomized double-blind study, 43 HIV-1-infected patients received tenidap (120 mg) or placebo daily for 6 weeks and then crossed over to the alternative therapy for an additional 6 weeks. Mean entry CD4 cell count was 140/microL. Analyses were performed on cytokines, acute-phase proteins, virus load, and CD4 cell counts. With the exception of small differences in plasma TNF levels, tenidap had no significant effect on these indices. Significant correlations of plasma IL-6 and TNF levels with HIV-1 RNA were noted. Six patients discontinued tenidap due to rash. The effects of tenidap in HIV-1 infection contrast to results in arthritis patients, in whom tenidap decreased plasma levels of IL-6 and acute-phase proteins.

Abstract:Aim of the study Measurement of the mean angle Kappa pre-keratorefractive and post-keratorefractive surgery utilizing Orbscan 3. Patients and methods This retrospective study was conducted on patients who had refractive surgeries, group A included patients who underwent Lasik and group B included patients who underwent Photo Refractive Keratectomy (PRK). Mean age from 18 to 45 years. Preoperative, 1-week, and 1-month postoperative assessments included detailed ocular examination, slit-scanning topography done by Orbscan 3, and total ocular aberrations by Zywave 3 were done. Results The study comprised 146 eyes of 75 individuals. Preoperative data demonstrated that angle Kappa mean in Lasik group (group A) was 2.38±1.54 at 147.05±96.94, in PRK group (group B) was 2.31±1.17 at 137.67±85.7. Paired mean differences of the two groups between preoperative data and 1-week postoperative show no statistically significant difference in central corneal thickness (CCT) nor angle Kappa values. Paired mean differences of the two groups between preoperative data and 1-month postoperative show no statistically significant difference regarding CCT nor angle Kappa values. Preoperative, 1-week postoperative, and 1-month postoperative correlation of angle Kappa to refractive error, spherical equivalent, average K-reading, CCT and axial length, showed that both groups had a positive correlation with axial length. Conclusion Keratorefractive surgeries do not affect angle Kappa or its intercept measurements.
