Abstract:Objective To study the toxicity of mitomycin C(MMC) on cultured rabbit corneal endothelial cells,and search its safety during clinic use .Methods Rabbit corneal endothelial cells in culture were treated with different concentration of MMC(10 -6 g·L -1、10 -5 g·L -1、10 -4 g·L -1、10 -3 g·L -1、10 -2 g·L -1、10 -1 g·L -1),and changes in morphology were observed under phase-contrast microscope at different time,and MTT assay was used to detect the proliferation of the cells.Results At the concentrations of 10 -2 g·L -1 and 10 -1 g·L -1,part of cells died in 8 hours and almost all died in 20 hours.Toxic effect on cells was farther confirmed by MTT assay.There was no toxic effect on corneal endothelial cells at 10 -6 g·L -1 and 10 -5 g·L -1(P>0.05),while the proliferation of cells were inhibited at the concentration from 10 -4 g·L -1 to 10 -1 g·L -1.Conclusion The toxicity of MMC to rabbit corneal endothelial cells is concentration-dependent.
Effect of mitomycin C on the corneal endothelial cells of Saudi patients with myopia after transepithelial photorefractive keratectomy: a two-armed cohort study

Abstract:AIM: To study the effect of mitomycin C (MMC) applied during transepithelial photorefractive keratectomy (TPRK) on the corneal endothelium one week (W1) and three months (M3) after surgery and its determinants. METHODS: In this two-armed cohort study conducted in 2022, eyes treated with MMC during TPRK (group 1) were compared with eyes not treated with MMC (group 2). The corneal endothelial cell (EC) count, EC density (ECD; cells/mm2), average (μm2), standard deviation (μm2), coefficient of variation (CV%), ECmax, ECmin, and EC percentage of hexagonality were estimated at W1 and M3. The postoperative changes in the EC count in the two groups were compared and correlated with the other independent variables. RESULTS: Group 1 had 26 eyes, and group 2 had 78 eyes. All TPRK indices were significantly higher for the eyes in group 1 than for those in group 2. The MMC usage was not a significant predictor of the change in ECD (P=0.644), change in CV (P=0.374), and change in the percentage of hexagonality of EC (P=0.164) at W1. However, the use of MMC was a significant predictor of changes in CV (P=0.014) and the change in the percentage of hexagonality of EC (P=0.039) at M3. The duration of laser exposure and the size of the optical zone influenced the correlation of MMC use with the changes in EC indices, postoperatively. CONCLUSION: The use of MMC doesn’t affect ECD, CV, and percentage of hexagonality at W1 if other surgical indices are considered. At M3 after operating myopic eyes by TPRK, MMC significantly influence the CV and percentage of hexagonality. The duration of the laser application and the size of the optical zone should be considered when determining the effect of MMC on the EC indices.

Biological effects of mitomycin C on late corneal haze stromal fibrosis following PRK

Abstract:This review details the current understanding of the mechanism of action and corneal effects of mitomycin C (MMC) for prophylactic prevention of stromal fibrosis after photorefractive keratectomy (PRK), and includes discussion of available information on dosage and exposure time recommended for MMC during PRK. MMC is an alkylating agent, with DNA-crosslinking activity, that inhibits DNA replication and cellular proliferation. It acts as a pro-drug and requires reduction in the tissue to be converted to an active agent capable of DNA alkylation. Although MMC augments the early keratocyte apoptosis wave in the anterior corneal stroma, its most important effect responsible for inhibition of fibrosis in surface ablation procedures such as PRK is via the inhibition of mitosis of myofibroblast precursor cells during the first few weeks after PRK. MMC use is especially useful when treating eyes with higher levels of myopia (≥approximately 6 D), which have shown higher risk of developing fibrosis (also clinically termed late haze). Studies have supported the use of MMC at a concentration of 0.02%, rather than lower doses (such as 0.01% or 0.002%), for optimal reduction of fibrosis after PRK. Exposure times for 0.02% MMC longer than 40 s may be beneficial for moderate to high myopia (≥6D), but shorter exposures times appear to be equally effective for lower levels of myopia. Although MMC treatment may also be beneficial in preventing fibrosis after PRK treatments for hyperopia and astigmatism, more studies are needed. Thus, despite the clinical use of MMC after PRK for nearly twenty years-with limited evidence of harmful effects in the cornea-many decades of experience will be needed to exclude late long-term effects that could be noted after MMC treatment.
The Effect of Mitomycin C on the Corneal Endothelium and Corneal Posterior Surface after Laser Subepithelial Keratomileusis

Abstract:Objective To investigate the clinical effect of mitomycin C (MMC) on the corneal endothelium and corneal posterior surface after laser subepithelial keratomileusis (LASEK).Methods In a continuity prospective double-blind study,0.02％ MMC was used topically for 20 seconds in 33 eyes of 33 patients who had undergone LASEK after excimer laser ablation in the Fudan EENT Hospital.The other eye of these patients without MMC treatment was used as the control.Three and 6 months after surgery,the changes in the curve and elevation of the corneal posterior surface,and the density and variation coefficient of corneal endothelial cells were observed to evaluate the MMC toxicity.Data were analyzed using a paired t test.Results In the MMC-treated eyes,3 and 6 months after the operation,the average corneal endothelium cell density was 3353.3±290.3 cells/mm2 and 3407.6±298.4 cells/mm2; the variation coefficient was 13.36％±6.29％ and 13.48±4.34％.In the control eyes 3 and 6 months after the operation,the average corneal endothelium cell density was 3258.7±284.6 cells/mm2 and 3339.2±213.4 cells/mm2; the variation coefficient was 14.52％±7.85％ and 14.06％±6.50％.The changes in the elevation and curve of the corneal posterior surface in the MMC treated eyes were-0.67±2.26 μm and-0.52±2.67 μm and 0.01±0.20 D and 0.01±0.21 D,3 and 6 months after the operation.The changes in the control eyes were-0.85±2.31 μm and -0.15±3.00 μm and-0.01±0.22 D and 0.05+0.17 D at the respective time points after the operation.There were no significant differences in the above parameters either between preoperative or postoperative data or between these two groups.Conclusion The study showed that the application of 0.02％ mitomycin C in LASEK is safe.
Changes in Total, Anterior, and Posterior Corneal Surface Higher-Order Aberrations after 1.8 Mm Incision and 2.8 Mm Incision Cataract Surgery

Abstract:Purpose: To evaluate changes in corneal higher-order aberrations (HOAs) on the anterior and posterior corneal surfaces after 1.8 mm microincision cataract surgery (MICS) and 2.8 mm small-incision cataract surgery (SICS). Setting: Eye Department, First Affiliated Hospital, School of Medicine, Zhejiang University, China. Design: Prospective case series. Methods: Right eyes of patients had MICS or SICS. The preoperative and 1-week and 3-month postoperative distance visual acuity (CDVA) and dry eye-related indices were determined. The corneal total HOAs and Zernike coefficients (3rd and 4th order) over 4.0 and 6.0 mm zones, corneal volume, central corneal thickness (CCT), and anterior and posterior corneal astigmatism were measured using a Pentacam HR analyzer. Results: The MICS group comprised 126 eyes and the SICS group 70 eyes. The MICS and SICS groups had similar postoperative CDVA; however, the MICS group had quicker recovery of CCT, corneal volume, and corneal astigmatism. Significantly increased anterior corneal total HOAs were observed in the SICS group over a 6.0 mm zone (P < .001). Both groups showed significantly increased posterior corneal total HOAs over both zones (P < .001). Similar changing patterns in individual Zernike terms were observed. The MICS group had quicker recovery of posterior corneal surface coma and trefoil than the SICS group, especially over the 6.0 mm zone. Changes in posterior corneal surface total HOAs were correlated with corneal volume changes (P < .01). Conclusions: The data suggest quicker corneal recovery and less change in total and anterior corneal surface corneal HOAs after MICS. Changes in posterior corneal surface HOAs were more pronounced in both surgical groups.
The Effect of Intraoperative Mitomycin C on Refractive Stability after Laser-Assisted Subepithelial Keratomileusis in Treating High Myopia

Abstract:Objective To evaluate the effect of intraoperative mitomycin C on the stability of refraction after laser-assisted subepithelial keratomileusis (LASEK) in treating high myopia.Methods In this prospective control study,66 eyes of 33 patients were divided into two groups.One eye of each patient was treated with 0.02％ mitomycin C (MMC) during LASEK for myopia (MMC group) while the other eye was not treated (control group).Corneal refraction,intraocular pressure using noncontact tonometry and haze were measured at 3,6,9,12 and 15 weeks after LASEK,and the two groups were compared using paired t test.Results The difference in refraction between the 3rd week and the 6th week in the MMC group was statistically significant (t=2.254,2.175,P＜0.05),while the differences between the other adjacent time points were not statistically significant.Refraction in the MMC treatment group became stable at 9 weeks after surgery.In the control group,the refractive differences between the 3rd and 6th weeks and the 6th and 9th weeks were statistically significant (t=3.315,2.440,2.356,P＜0.05),while the differences between the other times were not significant.Refraction in the control group became stable 12 weeks after surgery.The average course and frequency of using 0.1％ fluorometholone eye drops after surgery in the MMC group were 9 weeks and 2.6 times per day while course and frequency were 12.9 weeks and 2.9 times per day for the control group.Conclusion Intraoperative MMC can shorten the refractive fluctuation time after LASEK for treating high myopia.The effect may reduce the frequency and dosage of corticosteroid eye drops after surgery and avoid the occurrence of steroid-induced ocular hypertension and glaucoma.
[Mutiple Factors Analysis of the Effects of Excimer Laser Photorefractive Keratectomy].

Abstract:PURPOSE:To observe the effect of excimer laser photorefractive keratectomy (PRK) correcting myopia and astigmatism, and to study the related factors.METHOD:PRK was performed with the aim to correct myopia errors from 1.00 to 16.00 diopters on 529 myopia eyes (306 patients). All eyes were divided into two groups: Group I: -1.00-6.00 D(294 eyes); group II: -6.25-16.00 D(235 eyes). All the patients were followed up for 3 to 12 months.RESULTS:At postoperative 12 months the rates of uncorrected visual acuity more than or equal to 10/20 or 20/20 were 97.5%, 77.5% in group I, and 80.3%, 32.8% in group II respectively; 92.5% of eyes were within +/- 1.00 diopter of the desired emmetropia in group I, but 42.6% in group II; The influential factors of changes of postoperative diopters was the severity of corneal haze, methods of removing corneal epithelium, intraocular pressure, the central corneal thickness.CONCLUSION:The excimer laser PRK is regarded to be a very predictable, safe, stable, and effective method to correct myopia up to -6.00 D. Prior to operation, the plan would be disigned according to diopters, ages, corneal thickness intraocular pressure. During operation, the corneal epithelium bad better to be removed by laser.
Effect of Biometric and Surgical Parameters on Tear Meniscus Height after Phacoemulsification

Abstract:Objective To evaluate tear meniscus height(TMH) changes after phacoemulsification with anterior segment optical coherence tomography(AS-OCT),and to discuss the effect of biometric and surgical parameters on tear meniscus height changes.Methods This clinical trial involved 45 patients with age-related cataract(50 eyes) who underwent phacoemulsification.Age,nuclear density,cumulative dissipated energy(CDE),phacotime and TMH were measured preoperatively and 1 month postoperatively.Results The preoperative average of TMH was(0.286±0.059)mm,which were significant differences from one month after operation(0.217 ± 0.026) mm(P0.001).There were positive relationships between the age,nuclear density,phacotime,CDE,axial length and TMH decrease(P0.001,respectively).Conclusion After phacoemulsification,a mild decrease of TMH could be observed.TMH decrease involves in many factors.AS-OCT can be taken as an objective clinical imaging method for the diagnosis of dry eye after phacoemulsification.
Long-Term Effects of Simultaneous Subconjunctival and Subscleral Mitomycin C Application in Repeat Trabeculectomy

Abstract:Purpose: To assess the efficacy and safety of simultaneous mitomycin C application under conjunctival and scleral flaps in patients with repeat trabeculectomy.Methods: A total of 44 patients (44 eyes) with previous failed filtering surgery were randomized to one of two groups. The both-flaps group comprised 22 patients (22 eyes) with trabeculectomy and intraoperative mitomycin C application under conjunctival and scleral flaps, whereas the subconjunctival group comprised 22 patients (22 eyes) with subconjunctival application of mitomycin C. Particular attention was paid to intraocular pressure, postoperative medications, visual acuity, filtering bleb appearance, and complications. The mean follow-up time was 38.18 +/- 12.48 months.Results: The mean preoperative intraocular pressure decreased from 39.1 +/- 7.3 mm Hg to the postoperative level of 15.6 +/- 4.8 mm Hg in the both-flaps group (P = 0.014), and from 39.4 +/- 8.4 to 18.7 +/- 5.8 mm Hg in the subconjunctival group (P = 0.018). There was a statistically significant difference in intraocular pressure at all follow-up times, except at 1 week and 1 month postoperatively. Kaplan-Meier survival analysis showed there was no significant difference in total success rate (complete plus qualified success) between the two group, (P = 0.622, log-rank test). However, the two survival curves for the complete success subgroups (without additional medications) confirmed that mitomycin C applications under both flaps had a higher success rate than subconjunctival application (P = 0.043, log-rank test). No statistically significant difference in medications was present between the two groups, and no severe complications developed in either group.Conclusions: Trabeculectomy augmented with mitomycin C application at both sites could produce a greater lowering of intraocular pressure with low incidence of postoperative complications, and could provide an increased chance of long-term success. The procedure is effective and safe in patients with repeat trabeculectomy.
Application of the Sodium Hyaluronate in Mitomycin C Trabeculectomy

Abstract:Objective To study the application of sodium hyaluronate (SH)in mitomycin C (MMC) trabeculectomy.Methods 98 cases (116 eyes)were operated with MMC trabeculectomy.SH was injected into the anterior chamber and under the sutured scleral flaps in 52 cases(62 eyes) ,and the other 46 cases(54 eyes) as control group were jnjected into BSS.The postoperative visual acuity, intraocular pressure (IOP), filtration, forming of anterior chamber and the postoperative complications were compared between the two groups. Results The postoperative visual acuity, the forming of anterior chamber or the postoperative complications in the treatment group was better than that in the control group(P<0.01 or P<0.05).The postoperative IOP of the treatment group was(8.3±1.4)mmHg(second day),or(7.5±1.6)mmHg(1week later),or(13.2±2.4) mmHg(half a year later)averagely and that of the control group was(5.6±1.0)mmHg(P<0.05),or(6.8±1.2)mmHg(P>0.05), or(17.6±3.8)mmHg(P<0.05).All cases were followed up for 6 months to 3 years(18 months averagely).The rate of functional filtering bubles was 96.8% in the treatment group,and that in the control groupwas 85.2%(P<0.05).The successful rate was 95.2% and 81.5% respectively in two groups(P<0.05).Conclusins sodium hyaluronate using in MMC trabeculectomy can decrease the postoperative complications and elevate the successful rate.
Early elastic and viscoelastic corneal biomechanical changes after photorefractive keratectomy and small incision lenticule extraction

Abstract:Purpose: To compare early changes in the corneal biomechanical parameters after photorefractive keratectomy (PRK) and small incision lenticule extraction (SMILE) and their correlations with corneal shape parameters. Methods: One hundred twenty four eyes received myopic PRK and SMILE for similar amounts of myopia. Corneal tomography with Pentacam HR, biomechanical parameters using Corvis ST, and Ocular Response Analyzer (ORA) were evaluated before and 2 weeks after surgery. The change in each parameter was compared between groups, while the difference in central corneal thickness and cornea-compensated intraocular pressure measured before and after surgery were considered as covariates. Results: A significant reduction was seen in the corneal stiffness parameter at first applanation, and an increase in deformation amplitude ratio (DAR), and integrated inverse radius (IIR) in both groups after surgery (p < 0.001) Changes in DAR, and IIR were significantly greater in the SMILE than in the PRK group (p < 0.001) Corneal hysteresis (CH) and corneal resistance factor (CRF) decreased in both SMILE and PRK groups after surgery, (p < 0.001) with no statistically significant difference between groups (p > 0.05) Among new Corvis ST parameters, DAR showed a significant correlation with changes in Ambrosio relational thickness in both groups (p < 0.05). Conclusions: Both techniques caused significant changes in corneal biomechanics in the early postoperative period, with greater elastic changes in the SMILE group compared to the PRK group, likely due to lower tension in the SMILE cap and thinner residual stromal bed in SMILE. There were no differences in viscoelastic changes between them, so the lower CH may reflect the volume of tissue removed.

Abstract:PURPOSE: To compare tear cytokines and clinical outcomes between off-flap and on-flap epi-LASIK eyes and explore the possible mechanism for the clinical differences.METHODS: This double-masked, randomized study enrolled 18 myopic patients who underwent off-flap epi-LASIK with mitomycin C (MMC) in 1 eye and on-flap epi-LASIK with MMC in the contralateral eye. Tears were collected from each eye preoperatively and 2 hours, 1 day, and 5 days postoperatively. Concentrations of multiple tear cytokines were measured by a multiplex immunobead assay. Uncorrected distance visual acuity (UDVA), refraction, haze scores, pain scores, and percentage of corneal epithelial healing were evaluated.RESULTS: Compared with the on-flap group, the off-flap group had outcomes of better UDVA and higher percentages of epithelial healing at 5 days after surgery (P<.001) and lower levels of haze at 1 month after surgery (P=.049). Preoperatively, no significant differences were noted in the release rate of all tear cytokines between groups. At 2 hours postoperatively, the release rate of tear basic fibroblast growth factor (bFGF), platelet-derived growth factor-BB (PDGF-BB), interleukin-8 (IL-8), and tumor necrosis factor alpha (TNF-alpha) in the off-flap group were significantly lower than those in the on-flap group (P=.011,.017,.048, and .041, respectively).CONCLUSIONS: Off-flap epi-LASIK with MMC offers faster corneal epithelial healing and visual recovery, and temporary less haze than on-flap epi-LASIK with MMC. The lower tear levels of bFGF, PDGF-BB, IL-8, and TNF-alpha in the off-flap group 2 hours after surgery may suggest a possible mechanism for the clinical differences. Biomechanical Effects of tPRK, FS-LASIK, and SMILE on the Cornea

Abstract:Purpose: The objective of this study is to evaluate the in vivo corneal biomechanical response to three laser refractive surgeries. Methods: Two hundred and twenty-seven patients who submitted to transepithelial photorefractive keratectomy (tPRK), femtosecond laser-assisted in-situ keratomileusis (FS-LASIK), or small-incision lenticule extraction (SMILE) were included in this study. All cases were examined with the Corvis ST preoperatively (up to 3 months) and postoperatively at 1, 3, and 6 months, and the differences in the main device parameters were assessed. The three groups were matched in age, gender ratio, corneal thickness, refractive error corrections, optical zone diameter, and intraocular pressure. They were also matched in the preoperative biomechanical metrics provided by the Corvis ST including stiffness parameter at first applanation (SP-A1), integrated inverse radius (IIR), deformation amplitude (DA), and deformation amplitude 2 mm away from apex and the apical deformation (DARatio2mm). Results: The results demonstrated a significant decrease post-operation in SP-A1 and significant increases in IIR, DA, and DARatio2mm ( p < 0.05), all of which indicated reductions in overall corneal stiffness. Inter-procedure comparisons provided evidence that the smallest overall stiffness reduction was in the tPRK group, followed by the SMILE, and then the FS-LASIK group ( p < 0.05). These results remained valid after correction for the change in CCT between pre and 6 months post-operation and for the percentage tissue altered. In all three surgery groups, higher degrees of refractive correction resulted in larger overall stiffness losses based on most of the biomechanical metrics. Conclusion: The corneal biomechanical response to the three surgery procedures varied significantly. With similar corneal thickness loss, the reductions in overall corneal stiffness were the highest in FS-LASIK and the lowest in tPRK.

Complications of mitomycin C therapy in 100 eyes with ocular surface neoplasia

Abstract:Aim: To determine the complications associated with mitomycin C (MMC) in the treatment of ocular surface neoplasia. Methods: A retrospective and consecutive study of 100 eyes in 91 patients with ocular surface neoplasia treated with MMC in a single centre between November 1998 and January 2005. Outcome measures included complications of MMC and the treatment required for these complications. Results: One to three 7 day cycles of topical MMC 0.04% four times a day were given to 59 eyes with localised corneal-conjunctival intraepithelial neoplasia (CIN), 19 eyes with diffuse CIN, six eyes with recurrent CIN, one eye with ocular surface squamous cell carcinoma, three eyes with primary acquired melanosis (PAM) with atypia, nine eyes with conjunctival malignant melanoma (MM), two eyes with sebaceous carcinoma with pagetoid spread, and one eye with recurrent atypical fibroxanthoma. Nine patients had bilateral CIN. 31 (34%) cases developed an allergic reaction to MMC and 14 (14%) eyes had epiphora secondary to punctal stenosis at a mean follow up period of 26.5 months. Conclusion: In the largest study looking at complications of topical MMC in the treatment of ocular surface neoplasia, allergic reaction and punctal stenosis are relatively common. Serious complications were not observed suggesting the safe use of MMC in mid-term follow up.

Abstract:Purpose: To evaluate the efficacy of mitomycin C in reducing reformation of adhesions following surgery for restrictive strabismus. Methods: Sixteen patients with restrictive strabismus resulting from trauma, retinal detachment surgery, and multiple squint surgeries were randomized to either a mitomycin group (8 patients) or a control group (8 patients). Mitomycin C (0.2 mg/mL) was applied intraoperatively for 5 minutes after release of adhesions. Range of passive ductions was scored for comparison. Postoperative follow-up ranged from 3-18 months. Results: Compared to controls, the mitomycin C group had better range of passive ductions. There were no complications related to the cornea, intraocular pressure, uveitis, or muscle disinsertion in the mitomycin C group. Conclusion: Intraoperative application of mitomycin C (0.2 mg/mL for 5 minutes) is a safe and effective adjunct to surgery in the treatment of restrictive strabismus.
Intraoperative Mitomycin-C Use after the Excision Papillae in Vernal Keratoconjunctivitis

Abstract:Objective To observe the efficacy and safety of excision of cobblestone-like papillae combined with 0.4 mg/ml mitomycin-C(MMC)use in patients with vernal keratoconjunctivits.Methods 10 cases of vernal keratoconjunctivits enrolled in this study.All patients had cobblestone-like papillae on the upper tarsal conjunctiva.Papillary excision combined with intraoperative use of 0.4 mg/ml MMC for 2 minutes to the upper palpebral conjunctiva.Postoperative observation included clinical symptoms and signs.Follow-up ranged from 2 to 12 months.Results The clinical symptoms and signs of all patients got significanly better.Some thin Papillae regrowth was observed in 4 of 20 eyes(20%)after surgery.No adverse effects of MMC use were observed.Conclusions Excision of cobblestone-like papillae combined with MMC use on the upper palpebral conjunctiva in patients with VKC was safe and effactive.
