Abstract:Purpose: The objective of this study is to evaluate the in vivo corneal biomechanical response to three laser refractive surgeries. Methods: Two hundred and twenty-seven patients who submitted to transepithelial photorefractive keratectomy (tPRK), femtosecond laser-assisted in-situ keratomileusis (FS-LASIK), or small-incision lenticule extraction (SMILE) were included in this study. All cases were examined with the Corvis ST preoperatively (up to 3 months) and postoperatively at 1, 3, and 6 months, and the differences in the main device parameters were assessed. The three groups were matched in age, gender ratio, corneal thickness, refractive error corrections, optical zone diameter, and intraocular pressure. They were also matched in the preoperative biomechanical metrics provided by the Corvis ST including stiffness parameter at first applanation (SP-A1), integrated inverse radius (IIR), deformation amplitude (DA), and deformation amplitude 2 mm away from apex and the apical deformation (DARatio2mm). Results: The results demonstrated a significant decrease post-operation in SP-A1 and significant increases in IIR, DA, and DARatio2mm ( p < 0.05), all of which indicated reductions in overall corneal stiffness. Inter-procedure comparisons provided evidence that the smallest overall stiffness reduction was in the tPRK group, followed by the SMILE, and then the FS-LASIK group ( p < 0.05). These results remained valid after correction for the change in CCT between pre and 6 months post-operation and for the percentage tissue altered. In all three surgery groups, higher degrees of refractive correction resulted in larger overall stiffness losses based on most of the biomechanical metrics. Conclusion: The corneal biomechanical response to the three surgery procedures varied significantly. With similar corneal thickness loss, the reductions in overall corneal stiffness were the highest in FS-LASIK and the lowest in tPRK.

Abstract:Purpose: To evaluate the change in corneal biomechanics in patients with postoperative ectasia risk when combining two common laser vision correction procedures (tPRK and FS-LASIK) with cross-linking (in tPRK Xtra and FS-LASIK Xtra). Methods: The study included 143 eyes of 143 myopic, astigmatic patients that were divided into non-cross-linked refractive surgery groups (non-Xtra groups, tPRK and FS-LASIK) and cross-linked groups (Xtra groups, tPRK Xtra and FS-LASIK Xtra) according to an ectasia risk scoring system. The eyes were subjected to measurements including the stress-strain index (SSI), the stiffness parameter at first applanation (SP-A1), the integrated inverse radius (IIR), the deformation amplitude at apex (DA), and the ratio of deformation amplitude between apex and 2 mm from apex (DARatio2mm). The measurements were taken preoperatively and at 1, 3, and 6 months postoperatively (pos1m, pos3m, and pos6m). Posterior demarcation line depth from the endothelium (PDLD) and from the ablation surface (DLA) were recorded at pos1m. Results: SP-A1 significantly decreased, while IIR, deformation amplitude, and DARatio2mm increased significantly postoperatively in all four groups (p < 0.01)-all denoting stiffness decreases. In the FS-LASIK group, the changes in IIR, DA, and DARatio2mm were 32.7 ± 15.1%, 12.9 ± 7.1%, and 27.2 ± 12.0% respectively, which were significantly higher (p < 0.05) compared to 20.1 ± 12.8%, 6.4 ± 8.2%, and 19.7 ± 10.4% in the FS-LASIK Xtra group. In the tPRK group, the change in IIR was 27.3 ± 15.5%, significantly larger than 16.9 ± 13.4% in the tPRK Xtra group. The changes of SSI were minimal in the tPRK (-1.5 ± 21.7%, p = 1.000), tPRK Xtra (8.4 ± 17.9%, p = 0.053), and FS-LASIK Xtra (5.6 ± 12.7%, p = 0.634) groups, but was significant in the FS-LASIK group (-12.1 ± 7.9%, p < 0.01). After correcting for baseline biomechanical metrics, preoperative bIOP and the change in central corneal thickness (△CCT) from pre to pos6m, the changes in the IIR in both FS-LASIK and tPRK groups, as well as DA, DARatio2mm and SSI in the FS-LASIK group remained statistically greater than their corresponding Xtra groups (all p < 0.05). Most importantly, after correcting for these covariates, the changes in DARatio2mm in the FS-LASIK Xtra became statistically smaller than in the tPRK Xtra (p = 0.017). Conclusion: The statistical analysis results indicate that tPRK Xtra and FS-LASIK Xtra effectively reduced the biomechanical losses caused by refractive surgery (tPRK and FS-LASIK). The decrease in corneal overall stiffness was greater in FS-LASIK than in tPRK, and the biomechanical enhancement of CXL was also higher following LASIK than after tPRK.
Changes of dry eye parameters after femtosecond laser and conventional LASIK

Abstract:AIM: To compare the changes of dry eye symptoms and signs after femtosecond-assisted laser in situ keratomileusis(LASIK)and conventional LASIK by Keratograph 5M.METHODS: Sixty patients(120 eyes)who underwent corneal refractive surgery from June 2017 to November 2017 were enrolled. Thirty patients(60 eyes)underwent femtosecond-assisted laser LASIK and 30 patients(60 eyes)underwent conventional LASIK. All patients took examinations of Keratograph 5M dry eye related examinations,routine ophthalmological examinations and ocular surface disease index(OSDI)questionnaire before and 1wk, 1mo, 3mo and 6mo after surgery.RESULTS: One week after operation, the OSDI scores of two groups were significantly higher than pre-operation(P<0.01), but 1mo after operation, the two groups recovered to the preoperative level(P>0.05). Noninvasive tear film break-up time(NI-BUT)in the conventional group was shorter 1wk, 1mo and 3mo after operation(P<0.01), while it was shorter in 1wk and 1mo after operation in femtosecond laser group(P<0.01). There was significant difference between conventional group and femtosecond laser group at 3mo after operation in NI-BUT(P<0.01). Tear meniscus hight(TMH)and the thickness of lipid layers of the two groups all decreased in 1wk and 1mo after operation(P<0.05).CONCLUSION: Whether femtosecond-assisted or conventional LASIK will affect tear film stability and cause dry eye symptoms. The degree of influence decreases gradually with the time after operation, but femtosecond laser group can recover more quickly.

Abstract:PURPOSE:To investigate the differences in corneal deformation parameters after femtosecond laser small incision lenticule extraction (SMILE), laser-assisted subepithelial keratomileusis (LASEK), and femtosecond laser-assisted LASIK (FS-LASIK).METHODS:In this retrospective study, 17 eyes of 17 patients after SMILE, 18 eyes of 18 patients after LASEK, and 17 eyes of 17 patients after FS-LASIK were included. Corneal deformation parameters were measured with the CorVis ST tonometer (Oculus Optikgeräte GmbH, Wetzlar, Germany) 3 months postoperatively.RESULTS:The mean value of deformation amplitude of the FS-LASIK group was significantly higher than that of the LASEK group (P = .022). The mean value of applanation time (applanation 1) of the LASEK group was significantly higher than that of the FS-LASIK group (P = .038). No significant difference was detected in the mean values of deformation amplitude and applanation time (applanation 1) (P > .05) between the LASEK and SMILE groups or between the SMILE and FS-LASIK groups. Multiple linear regression model analysis revealed that after adjustment for age and preoperative central corneal thickness and manifest refraction spherical equivalent, the significance of the difference in the mean values of applanation time (applanation 1) and deformation amplitude between the LASEK and FS-LASIK groups were P = .084 and .059, respectively. In all three groups, the values of applanation 1 negatively correlated to those of applanation time (applanation 2) (SMILE: r = −0.577, P = .015; LASEK: r = −0.833, P < .001; FS-LASIK: r = −0.516, P = .034) and deformation amplitude (SMILE: r = −0. 556, P = .021; LASEK: r = −0.877, P < .001; FS-LASIK: r = −0.509, P = .037).CONCLUSIONS:Applanation time (applanation 1) and deformation amplitude (as measured with the CorVis ST tonometer) may be helpful in assessing corneal biomechanical changes after corneal refractive surgery. The relations between these parameters should be discussed in further studies.
Characteristics and Correlative Factors of Posterior Corneal Surface Changes after Laser in Situ Keratomileusis

Abstract:OBJECTIVE:To assess the changes of the posterior corneal surface and the factors influencing these changes after laser in situ keratomileusis (LASIK). These factors included the preoperative refraction, preoperative intraocular pressure (IOP), preoperative corneal thickness, spherical equivalent correction and one-week postoperative corneal thickness.METHODS:The posterior corneal radius of curvature for the posterior best fit sphere (PBFS), irregularity of posterior surface, and anterior chamber depth (ACD) were obtained with the scanning slit topography system before and 1 week, 1 month and 3 months after LASIK in 64 eyes of 34 patients with myopic refractive errors of -2.25 to -12.5 diopters. Multiple regression analysis was used to assess the factors that affecting these changes of posterior corneal surface. The difference of radius PBFS and posterior elevation difference (PED) was used as an indicator for the forward shift of posterior corneal surface.RESULTS:The ACD before the surgery and 1 week, 1 month and 3 months after the surgery was (3.2448 +/- 0.2806), (3.2770 +/- 0.2810), (3.2579 +/- 0.2932) and (3.2651 +/- 0.2875) mm, respectively. The PBFS at that time was (6.5095 +/- 0.2177), (6.3731 +/- 0.2127), (6.4257 +/- 0.2358) and (6.4354 +/- 0.2266) mm, respectively. The posterior corneal irregularity at 5 diameters zone was 0.622 +/- 0.142, 0.978 +/- 0.244, 0.884 +/- 0.207 and 0.881 +/- 0.174, respectively The preoperative posterior corneal radius of curvature for PBFS, the irregularity of the posterior surface and the ACD were significantly different from that of postoperative data at each time point (P < 0.01). There was no significant difference in these indexes between 1 month and 3 months postoperatively. Multiple regression analysis showed that factors relevant to the changes of posterior corneal surface were the corrected preoperative refraction (regression coefficient r = 0.0173, P = 0.005) and 1 week postoperative corneal thickness (mm) (regression coefficient r = -1.495, P = 0.001).CONCLUSIONS:The posterior corneal surface after LASIK become steeper and more irregular and remains stable in three months after the surgery. Eyes with thinner corneas and higher myopia requiring more ablation are more predisposed to the changes of posterior corneal surface. The results of the present study should not be used in extremely high degree myopia. A study using a larger group of patients with a longer observation period is required.

Abstract:Background: Small incision lenticule extraction (SMILE) and femtosecond laser in situ keratomileusis (FS-LASIK) have been extensively studied as the main surgical methods for corneal refractive surgery. However, there is no consensus on whether SMILE is superior to FS-LASIK in corneal biomechanics. Therefore, this systematic review and meta-analysis used the results of ocular response analyzer and corvis ST to explore whether SMILE is superior to FS-LASIK in corneal biomechanics. Methods: The literature was searched in PubMed, EMBASE, and Controlled Trials Register databases. The Cochrane Collaboration's "risk of bias" tool was used to evaluate the quality of the included randomized clinical trials, and the Newcastle-Ottawa Scale was used to evaluate the included non-randomized controlled trials. The results were analyzed using Revman 5.3. Results: Sixteen studies (3 randomized clinical trials and 13 non-randomized controlled trials) were included in this meta-analysis. There was no statistical difference in corneal biomechanics between SMILE and FS-LASIK in corneal hysteresis [mean difference (MD), 0.20; 95% confidence interval (CI): -0.09, 0.49; P = .18] and corneal resistant factor (MD, 0.31; 95% CI: -0.09, 0.71; P = .13), A1 time (MD, -0.02; 95% CI: -0.11, 0.07; P = .66), A1 length (MD, 0.01; 95% CI: -0.01, 0.03; P = .42), A1 velocity (MD, 0.00; 95% CI: -0.01, 0.01; P = .85), A2 velocity (MD, -0.01; 95% CI: -0.11, 0.09; P = .86), HC time (MD, 0.12; 95% CI: -0.13, 0.38; P = .35), The stiffness parameter at first applanation (MD, -7.91; 95% CI: -17.96, 2.14; P = .12), The ratio between the deformation amplitude 2 mm away from apex and the apical deformation (MD, 0.01; 95% CI: -0.26, 0.27; P = .96). Conclusion: A comprehensive assessment of the parameters of ocular response analyzer and corvis ST showed that SMILE is not superior to LASIK in corneal biomechanics 3 months post-surgery.
Effects of Laser In Situ Keratomileusis and Small-Incision Lenticule Extraction on Corneal Biomechanical Behavior: A Finite Element Analysis

Abstract:Myopia, which is the result of the uncoordinated development of the eyeball, has become a major public health focus worldwide. Laser in situ keratomileusis (LASIK) and small-incision lenticule extraction (SMILE) have been successfully used in modern corneal refractive surgery. However, there are still controversies about postoperative results of LASIK and SMILE. In this study, a three-dimensional finite element model of the cornea was constructed based on the elevation and pachymetry data of a female volunteer. Surgical parameters, magnitudes of myopic correction, and intraocular pressure (IOP) were varied. Furthermore, an iterative algorithm was applied to retrieve the free-stress state of the intact corneal model, LASIK model, and SMILE model. To better evaluate the differences between LASIK and SMILE procedures, the displacement and Von Mises stress on the anterior and posterior corneal surface along the x- and y -axes were analyzed. Results for the zero-pressure model showed larger displacement compared to the image-based corneal model, suggesting that the initial corneal pre-stress stiffens the response of the cornea, both in the intact cornea and under refractive surgery. In addition, the displacement on the corneal surface in LASIK (both zero-pressure and image-based model) was obviously higher than that of the SMILE model. In contrast, SMILE increased Von Mises stress in the corneal cap and reduced Von Mises stress in the residual stromal bed compared with the LASIK model. However, the maximum Von Mises stress in the SMILE model was still smaller than that of the LASIK model. Moreover, the displacement and Von Mises stress on the residual stromal bed increased linearly with IOP. Overall, LASIK and SMILE refractive surgery could change biomechanical behaviors of the cornea. Compared to LASIK refractive surgery, SMILE may present a lower risk of ectasia. Creating a corneal cap rather than a corneal flap may have an advantage in improving corneal biomechanical stability.

Abstract:Purpose: To compare early changes in the corneal biomechanical parameters after photorefractive keratectomy (PRK) and small incision lenticule extraction (SMILE) and their correlations with corneal shape parameters. Methods: One hundred twenty four eyes received myopic PRK and SMILE for similar amounts of myopia. Corneal tomography with Pentacam HR, biomechanical parameters using Corvis ST, and Ocular Response Analyzer (ORA) were evaluated before and 2 weeks after surgery. The change in each parameter was compared between groups, while the difference in central corneal thickness and cornea-compensated intraocular pressure measured before and after surgery were considered as covariates. Results: A significant reduction was seen in the corneal stiffness parameter at first applanation, and an increase in deformation amplitude ratio (DAR), and integrated inverse radius (IIR) in both groups after surgery (p < 0.001) Changes in DAR, and IIR were significantly greater in the SMILE than in the PRK group (p < 0.001) Corneal hysteresis (CH) and corneal resistance factor (CRF) decreased in both SMILE and PRK groups after surgery, (p < 0.001) with no statistically significant difference between groups (p > 0.05) Among new Corvis ST parameters, DAR showed a significant correlation with changes in Ambrosio relational thickness in both groups (p < 0.05). Conclusions: Both techniques caused significant changes in corneal biomechanics in the early postoperative period, with greater elastic changes in the SMILE group compared to the PRK group, likely due to lower tension in the SMILE cap and thinner residual stromal bed in SMILE. There were no differences in viscoelastic changes between them, so the lower CH may reflect the volume of tissue removed.

Abstract:PURPOSE: To evaluate theoretical biomechanical advantages of human corneas treated with small incision lenticule extraction (SMILE) compared with femtosecond laser-assisted LASIK (FS-LASIK) Patients and methods: In a prospective, comparative, non-randomized, consecutive case series patients with moderate to high myopia and/or astigmatism underwent corneal refractive surgery. Patients either received standard FS-LASIK or SMILE. Preoperatively and up to 3 months postoperatively data were analyzed including tomography with Pentacam HR, Goldmann tonometry, non-contact tonometry, and deformability of the cornea using an ultra-high-speed camera (Corvis ST). Data were analyzed and p < 0.05 was considered as statistically significant (t-test).RESULTS: Seventy-three patients (128 eyes) were treated. Forty-eight eyes of 29 patients underwent FS-LASIK and 80 eyes of 44 patients underwent SMILE. Preoperative spherical equivalent (SE) was -3.23 ± 1.64 D (FS-LASIK group) and -4.83 ± 1.63 D (SMILE group) (p < 0.0001). Almost all parameters obtained by Corvis ST between preoperative and postoperative measurements showed significant changes after refractive surgery. Significant changes were seen between both groups postoperatively. However, matched pair subgroup analysis (n = 69; 26 eyes FS-LASIK; 43 eyes SMILE) of eyes with initially equal pachymetry, intraocular pressure, SE, and difference of pre- to postoperative pachymetry (p>0.05), showed no significant changes in parameters measured with Corvis ST (p>0.05).CONCLUSIONS: Corneal biomechanical parameters measured preoperatively with Corvis ST showed significant differences postoperatively in total and in both groups. In subgroup analysis with homogenous groups, FS-LASIK showed no significant changes in biomechanical data measured with Corvis ST compared with SMILE.

Abstract:<h3 class="Heading">Background</h3><p class="Para">The aim of this study was to compare the postoperative corneal biomechanical properties between small incision lenticule extraction (SMILE) and other corneal refractive surgeries.</p><h3 class="Heading">Methods</h3><p class="Para">A systematic review and meta-analysis were conducted. Articles from January 2005, to April 2019, were identified searching PubMed, EMBASE, Web of Science, and International Clinical Trials Registry Platform. Studies that compared SMILE with other corneal refractive surgeries on adult myopia patients and evaluated corneal biomechanics were included. Multiple effect sizes in each study were combined. Random-effects model was conducted in the meta-analysis.</p><h3 class="Heading">Results</h3><p class="Para">Twenty-two studies were included: 5 randomized controlled trials (RCTs), 9 prospective and 6 retrospective cohort studies, and 2 cross-sectional studies. Using the combined effect of corneal hysteresis (CH) and corneal resistance factor (CRF), which were obtained from ocular response analyzer (ORA), the pooled Hedges' g of SMILE versus femtosecond laser-assisted in situ keratomileusis (FS-LASIK) was 0.41 (95% CI, 0.00 to 0.81; <em class="EmphasisTypeItalic">p</em> = 0.049; I<sup>2</sup> = 78%), versus LASIK was 1.31 (95% CI, 0.54 to 2.08; <em class="EmphasisTypeItalic">p</em> < 0.001; I<sup>2</sup> = 77%), versus femtosecond lenticule extraction (FLEX) was − 0.01 (95% CI, − 0.31 to 0.30; <em class="EmphasisTypeItalic">p</em> = 0.972; I<sup>2</sup> = 20%), and versus the group of photorefractive keratectomy (PRK) and laser-assisted sub-epithelial keratectomy (LASEK) was − 0.26 (95% CI, − 0.67 to 0.16; <em class="EmphasisTypeItalic">p</em> = 0.230; I<sup>2</sup> = 54%). The summary score of Corvis ST (CST) after SMILE was comparable to FS-LASIK/LASIK with the pooled Hedges' g = − 0.05 (95% CI, − 0.24 to 0.14; <em class="EmphasisTypeItalic">p</em> = 0.612, I<sup>2</sup> = 55%).</p><h3 class="Heading">Conclusions</h3><p class="Para">In terms of preserving corneal biomechanical strength after surgeries, SMILE was superior to either FS-LASIK or LASIK, while comparable to FLEX or PRK/LASEK group based on the results from ORA. More studies are needed to apply CST on evaluating corneal biomechanics after refractive surgeries.</p>

Abstract:Background: To compare the corneal biomechanical changes after small incision lenticule extraction (SMILE) and femtosecond laser-assisted in situ keratomileusis (FS-LASIK) with the same programmed optical zone (POZ) and similar refractive correction in patients with high myopia.Methods: In this prospective comparative study of the contralateral eye, 50 patients with high myopia with the same POZ and similar refractive correction who underwent SMILE in one eye and FS-LASIK in the other eye. Corneal biomechanical parameters and central corneal thickness (CCT) were measured using a Corvis ST II. All the patients were evaluated during follow-up visits beyond one year. Additionally, the corneal volume (CV) of the 10-mm diameter region was measured using a Pentacam.Results: Ambrosio relational thickness to the horizontal profile (ARTh) and stiffness parameter A1 (SP-A1) decreased significantly after SMILE and FS-LASIK, whereas deformation amplitude ratio 2.0 mm (DA ratio 2.0 mm) and integrated radius (IR) increased significantly in both groups. The ARTh and SP-A1 were greater after SMILE than those after FS-LASIK at all the follow-up visits. In addition, there were greater amounts of CCT and CV after SMILE compared with that after FS-LASIK. Moreover, a positive correlation was found between ARTh and SP-A1 and postoperative CCT, while a negative correlation was found between IR and DA ratio 2.0 mm and postoperative CCT. A moderate correlation was observed between SP-A1 and CV after both SMILE and FS-LASIK, whereas there were no relationships between CV and ARTh, IR, or DA ratio 2.0 mm.Conclusions: SMILE had greater CCT, CV, ARTh, and SP-A1 than FS-LASIK in high myopia with the same POZ and similar refractive correction. Abstract:·AlM:To compare the corneal biomechanical changes at early stage after femtosecond laser assisted in situ keratomi ( LASlK) and microkeratome LASlK according to different residual corneal stroma thickness/cornea thickness ( RCST/CT) . <br> · METHODS: Ninety -four patients ( 187 eyes ) with femtosecond LASlK ( FS-lK ) and 96 patients ( 192 eyes ) with microkeratome LASlK ( M-lK ) were included in this study. LASlK group was divided into three groups according to the ratio of RCST/CT, which were lK-l group ( RCST/CT<55%) , lK-Ⅱ group ( 55%≤RCST/CT<60%) and lK-Ⅲ group ( RCST/CT≥60%) .Corneal hysteresis ( CH) and corneal resistance factor ( CRF) were measured by reichert ocular response analyzer ( ORA ) at pre -operation, postoperative 1and 3mo. <br> ·RESULTS:Compared with FS-lK and M-lK,there were no statistical difference in CH and CRF ( F=0.44, F=2.56, P=0.51, P=0.11 ) .Based on different RCST/CT, there were statistical differences in CH and CRF between groupⅠand group Ⅱ (F=103.03, 128.48,P<0.05), group Ⅱand groupⅢ ( P<0.05 ) , group Ⅰ and group Ⅲ ( P<0. <br> 05 ) . There were statistical differences in CH and CRF between pre-operation and different postoperative times ( F =576.99, 1162.06, P <0.05 ). CH and CRF of postoperative 1mo and 3mo were significantly smaller than pre-operation ( P<0.05) , but there was no statistical difference between postoperative 1and 3mo (P>0.05). <br> · CONCLUSlON: The biomechanical parameters decreased at early stage after femtosecond LASlK and microkeratome LASlK, but there is no statistical difference between femtosecond LASlK and microkeratome LASlK. The less the ratio of RCST/CT, the more effect on biomechanics.
Changes in corneal biomechanics and posterior corneal surface elevation after FS-LASIK

Abstract:Aim: To investigate the changes in corneal biomechanics and posterior corneal surface elevation after femtosecond laser-assisted in situ keratomileusis (FS-LASIK). Methods: Totally 197 eyes of 100 patients who underwent the FS-LASIK from April 2022 to November 2022 were included. They were divided into three groups according to the ratio of residual corneal stroma thickness/corneal thickness (RCST/CT): Group I (50%≤RCST/CT<55%, 63 eyes of 32 patients), Group II (55%≤RCST/CT<60%, 67 eyes of 34 patients), and Group III (RCST/CT≥60%, 67 eyes of 34 patients). The intraocular pressure (IOP), corneal compensated IOP (IOPcc), corneal hysteresis (CH) and corneal resistance factor (CRF) were measured immediately, 1, and 3mo postoperatively by ocular response analyzer (ORA) and the posterior elevation difference (PED) was measured by Pentacam. Results: After operation, IOP, CH, CRF, and PED were statistically different among the three groups (F=12.99, 31.148, 23.998, all P<0.0001). There was no statistically significant difference in IOPcc among the three groups (F=0.603, P>0.05). The IOP, IOPcc, CH, and CRF were statistical changed after surgery (F=699.635, 104.125, 308.474, 640.145, all P<0.0001). The PED of Group I was significantly higher than that of Group II (P<0.05), and Group II was significantly higher than that of Group III (P<0.05). The PED value of 3mo after surgery decreased in each group compared with 1mo after surgery, but there was no statistical difference (Group I: t=0.82, P=0.41; Group II: t=0.17, P=0.87; Group III: t=1.35, P=0.18). The correlation analysis of corneal biomechanical parameter changes with PED at 1mo and 3mo after surgery showed that ΔIOP, ΔIOPcc, ΔCH, and ΔCRF were not correlated with PED value in three groups (P>0.05). Conclusion: The smaller the RCST/CT, the greater effect on corneal biomechanics and posterior surface elevation. There is no correlation between changes in corneal biomechanics and posterior corneal surface elevation in the range of RCST/CT≥50%.
Short term changes in corneal stress-strain index and other corneal biomechanical parameters post-laser in situ keratomileusis

Abstract:Purpose: To report the short-term changes in a corneal stress-strain index (SSI) and other corneal biomechanical parameters post-laser in situ keratomileusis (LASIK) surgery. Methods: A retrospective study was conducted at a tertiary eye care center wherein patients who had undergone LASIK (microkeratome blade and femtosecond bladeless LASIK) between July and December 2019 were enrolled. Patients of age group 20-40 years, best-corrected visual acuity of 20/20, intraocular pressure (IOP) <22 mmHg, pre-LASIK pachymetry >500 microns, and corneal astigmatism ≤3.00 D were included. Subjects with a prior history of refractive surgery, any other ocular or systemic disease, poor-quality scans, intraoperative complications, and missing data were excluded. Corneal biomechanical properties including SSI were analyzed using Corvis ST and compared using the Paired T-test for each group separately at pre-LASIK, and 1-month post-operatively. Results: Overall, 202 eyes were reviewed, and 79 eyes fulfilled the inclusion criteria. Forty-three and 36 eyes had undergone Microkeratome Blade LASIK (Group I) and Femto LASIK (Group II), respectively. Overall, 29 and 26 corneal biomechanical parameters out of 33 changed significantly post-Microkeratome Blade LASIK and Femto LASIK, respectively. Statistically significant changes were noted in all the parameters at A1, maximum and Vinciguerra screening parameters (P < 0.001), however, no changes were noted in SSI in both the groups when compared with the pre-surgery data. Conclusion: Though the reduction in SSI was not statistically significant, other biomechanical parameters showed significant biomechanical changes pre- and post-LASIK surgeries in both the groups. However, a long-term study with a larger sample size would be required to understand the changes and stability in SSI post-refractive surgery.

Abstract:Purpose: To investigate the influence of different corneal refractive surgeries on dynamic visual acuity (DVA), and explore its potential influence factors. Methods: This was a prospective non-randomized study. Adult myopic patients undergoing bilateral laser-assisted sub-epithelial keratomileusis (LASEK), femtosecond laser-assisted in situ keratomileusis (FS-LASIK), or small incision lenticule extraction (SMILE) with Plano refraction target were enrolled. Uncorrected and corrected distance visual acuity (UDVA/CDVA), manifest refraction and binocular optotype-moving DVA of 40 and 80 degrees per second (dps) were evaluated pre-operatively and post-operatively up to 3 months. Results: The study included 264 eyes of 132 subjects, with an average age of 27.0 ± 6.7 years, and females accounted for 59% of the participants. Significant improvement was observed at the 3-month visit for 40 dps (SMILE, P = 0.001; LASEK, P = 0.006; FS-LASIK, P = 0.010) and 80 dps (SMILE, P = 0.011; LASEK, P = 0.025; FS-LASIK, P = 0.012) DVA. Adjusting for pre-operative DVA, there was no significant difference in DVA among groups at 3 months post-operatively ( P > 0.05 for multiple comparisons). Overall, multiple linear models demonstrated that post-operative DVA at 3 months was correlated with pre-operative DVA (40 dps, β = 0.349, P = 0.001; 80 dps, β = 0.447, P < 0.001), pre-operative spherical equivalent (40 dps, β = 0.311, P = 0.003; 80 dps, β = 0.261, P = 0.009) and post-operative UDVA (40 dps, β = −0.224, P = 0.024; 80 dps, β = −0.188, P = 0.05). Conclusion: Dynamic visual acuity at 3 months post-operatively of the three corneal refractive surgeries was better than that before the surgery in adult myopic patients, and there was no significant difference among different surgical techniques. Post-operative DVA at 3 months was found correlated with pre-operative DVA, pre-operative SE, and post-operative UDVA. With further improvement, DVA could be a promising functional visual indicator for myopic patients undergoing refractive surgeries.

Abstract:AIM: To compare the difference of biomechanical stability after a femtosecond laser-assisted in situ keratomileusis(FS-LASIK)and laser-assisted subepithelial keratomileusis(LASEK)by ocular response analyzer(ORA). METHODS: This prospective study was conducted at the First Affiliated Hospital of Harbin Medical University, and myopic patients with the equivalent sphere between -2.00 and -5.00 diopters from January 2016 and December 2017 were enrolled. All the subjects were divided into FS-LASIK and LASEK group respectively according to different surgical methods. There were 64 patients(100 eyes)participated in the FS-LASIK group while 53 patients(100 eyes)in the LASEK surgery group. Corneal hysteresis value(CH)and corneal resistance factor value(CRF)were measured pre-, 1mo and 3mo postoperation of all the subjects. And the variance analysis of two groups was repeated to compare the overall difference between the two modes of operation from pre-operation to post operation. RESULTS: The CH and CRF value of the two groups were different before and after operation(P<0.05). The comparison results of repeated measurements showed that the CH value and CRF value of the surgical methods were reduced in 1 and 3mo after operation, respectively, and there was significance(P<0.05). The CH and CRF of FS-LASIK group was lower than LASEK group only in 1mo after the operation(P<0.05). CONCLUSION: Both surgeries could affect the biomechanical stability of cornea. From the point of view of biomechanical stability, LASEK operation is safer than FS-LASIK operation and reduces the possibility of postoperative refractive regression.

Abstract:Purpose: To evaluate corneal reinnervation, and the corresponding corneal sensitivity and keratocyte density after small incision lenticule extraction (SMILE) and femtosecond laser in situ keratomileusis (FS-LASIK).Methods: In this prospective, non-randomized observational study, 18 patients (32 eyes) received SMILE surgery, and 22 patients (42 eyes) received FS-LASIK surgery to correct myopia. The corneal subbasal nerve density and microscopic morphological changes in corneal architecture were evaluated by confocal microscopy prior to surgery and at 1 week, 1 month, 3 months, and 6 months after surgery. A correlation analysis was performed between subbasal corneal nerve density and the corresponding keratocyte density and corneal sensitivity.Results: The decrease in subbasal nerve density was less severe in SMILE-treated eyes than in FS-LASIK-treated eyes at 1 week (P=0.0147), 1 month (P=0.0243), and 3 months (P=0.0498), but no difference was detected at the 6-month visit (P=0.5277). The subbasal nerve density correlated positively with central corneal sensitivity in both groups (r=0.416, P < 0.0001, and r=0.2567, P=0.0038 for SMILE group and FS-LASIK group, respectively). The SMILE-treated eyes have a lower risk of developing peripheral empty space with epithelial cells filling in (P=0.0005).Conclusions: The decrease in subbasal nerve fiber density was less severe in the SMILE group than the FS-LASIK group in the first 3 months following the surgeries. The subbasal nerve density was correlated with central corneal sensitivity.
Differences in the early biomechanical effects of hyperopic and myopic laser in situ keratomileusis

Abstract:Purpose: To compare changes in corneal hysteresis (CH) and the corneal resistance factor (CRF) in myopic and hyperopic laser in situ keratomileusis (LASIK) and evaluate their relationship to the number of photoablative pulses delivered, a surrogate for ablation volume. Setting: Cleveland Clinic Cole Eye Institute, Cleveland, Ohio, USA. Methods: Preoperative and 1-week postoperative Ocular Response Analyzer measurements in eyes that had femtosecond-assisted LASIK were studied retrospectively. Changes in CH and CRF were compared and tested for correlation with the number of excimer laser pulses. Results: Thirteen myopic eyes and 11 hyperopic eyes were evaluated. Preoperative corneal thickness, CH, CRF, programmed correction magnitude, flap thickness, and total number of fixed spot-size photoablative pulses were similar in the 2 groups (P>.1). Decreases in CH and CRF were greater after myopic LASIK than after hyperopic LASIK (P<.005), and changes in CRF were correlated with the number of excimer laser pulses in the myopic group only (r = -0.63, P = .02). Regardless of ablation profile, changes in CH were more strongly correlated with preoperative CH values than with attempted ablation volume. Conclusions: With comparable flap thickness and attempted ablation volumes, myopic photoablation profiles were associated with greater decreases in CRF and CH than hyperopic profiles. Results indicate that preoperative corneal biomechanical status, ablation volume, and the spatial distribution of ablation are important factors that affect corneal resistance and viscous dissipative properties differently. Preferential tissue removal in the natively thicker paracentral cornea in hyperopia may partially account for the rarity of ectasia after hyperopic LASIK.
