Abstract:Objective To compare the corneal hysteresis (CH) and corneal resistance factor (CRF) measured with the Ocular Response Analyzer (ORA) in normal and keratoconic eyes. Methods It was a case-control study. Random selected 96 normal eyes and 46 keratoconic eyes in the same period were included in this study. Normal eyes were divided into 2 groups: high corneal astigmatism (≥3.00 D) and low-to-moderate corneal astigmatism (<3.00 D). Keratoconic eyes were also divided into 3 groups based on Amsler-Krumeich classification: mild (stage Ⅰ), moderate (stage Ⅱ) and severe (stage Ⅲ/Ⅳ). CH and CRF were compared between groups and the areas under ROC curves of the CH and CRF were calculated. Results The mean CH and CRF were (7.1±1.6) mm Hg and (6.3±1.5) mm Hg in keratoconic eyes compared with (10.1±1.3) mm Hg and (10.5±1.6) mm Hg in normal eyes. The difference were statistically significant(t=-11.813, -14.943 ;P<0.001). In normal eyes, there was no difference of CH or CRF between the high corneal astigmatism and low-to- moderate corneal astigmatism (t=0.373,0.095; P>0.05). In keratoconic eyes, there was a significant negative correlation between CH and the keratoconus grade (r=-0.627, P<0.001) and the same relationship was found between CRF and the keratoconus grade (r=-0.587, P<0.001). In multiple linear regression analysis, CH was correlated with central corneal thickness (CCT) and corneal curvature (r=0.320, -0.375;P<0.05) and CRF was correlated with corneal curvature in keratoconic eyes (r=-0.441 ,P<0.01), while they were only correlated with CCT in normal eyes (r=0.367,0.459;P<0.001). The areas under ROC curves of the CH and CRF were 0.9282 and 0.9731 (Z=20.462,38.305 ;P<0.0001), the difference between them was significant (Z =7.134,P=0.008). Conclusions The CH and CRF were significantly lower in keratoconic eyes than in normal eyes, especially on CRF. The long-term follow-up of CH and CRF may provide information for evaluation of progression of keratoconus. They may be included as indicators for detecting keratoconus.

Abstract:·AlM:To compare the corneal biomechanical changes at early stage after femtosecond laser assisted in situ keratomi ( LASlK) and microkeratome LASlK according to different residual corneal stroma thickness/cornea thickness ( RCST/CT) . <br> · METHODS: Ninety -four patients ( 187 eyes ) with femtosecond LASlK ( FS-lK ) and 96 patients ( 192 eyes ) with microkeratome LASlK ( M-lK ) were included in this study. LASlK group was divided into three groups according to the ratio of RCST/CT, which were lK-l group ( RCST/CT<55%) , lK-Ⅱ group ( 55%≤RCST/CT<60%) and lK-Ⅲ group ( RCST/CT≥60%) .Corneal hysteresis ( CH) and corneal resistance factor ( CRF) were measured by reichert ocular response analyzer ( ORA ) at pre -operation, postoperative 1and 3mo. <br> ·RESULTS:Compared with FS-lK and M-lK,there were no statistical difference in CH and CRF ( F=0.44, F=2.56, P=0.51, P=0.11 ) .Based on different RCST/CT, there were statistical differences in CH and CRF between groupⅠand group Ⅱ (F=103.03, 128.48,P<0.05), group Ⅱand groupⅢ ( P<0.05 ) , group Ⅰ and group Ⅲ ( P<0. <br> 05 ) . There were statistical differences in CH and CRF between pre-operation and different postoperative times ( F =576.99, 1162.06, P <0.05 ). CH and CRF of postoperative 1mo and 3mo were significantly smaller than pre-operation ( P<0.05) , but there was no statistical difference between postoperative 1and 3mo (P>0.05). <br> · CONCLUSlON: The biomechanical parameters decreased at early stage after femtosecond LASlK and microkeratome LASlK, but there is no statistical difference between femtosecond LASlK and microkeratome LASlK. The less the ratio of RCST/CT, the more effect on biomechanics.

Abstract:PURPOSE: To evaluate by means of the Ocular Response Analyzer (ORA) the biomechanical changes that follow intracorneal ring segment (ICRS) implantation and to develop a predicting model for the postoperative visual outcome, considering these biomechanical changes and other clinical parameters.METHODS: A total of 45 consecutive keratoconic eyes of 35 patients aged 18 to 55 years and implanted with ICRS were included in this retrospective study. All patients were implanted with KeraRing using femtosecond laser technology. Visual acuity, refraction, corneal topography, and aberrations were evaluated during a 6-month follow-up. Additionally, corneal biomechanical changes were evaluated with the ORA system.RESULTS: No significant changes in corneal hysteresis (CH) and corneal resistance factor (CRF) were observed at 1 month postoperatively (P ≥ 0.39). However, significant changes in these parameters were detected afterward (CH, 3 months, P = 0.03; CRF, 6 months, P = 0.02). Preoperative corneal biomechanical parameters were significantly correlated with postoperative corneal higher-order aberrations at all visits (P ≤ 0.05), although these correlations became stronger at the end of the follow-up. Multiple regression analysis revealed that corrected distance visual acuity at 1 month after surgery was significantly correlated with the preoperative mean keratometry and the preoperative difference between CH and CRF (P < 0.01, adjusted R² of 0.66).CONCLUSIONS: Biomechanical parameters measured with the ORA and mean keratometry are factors allowing a prediction of the postoperative visual outcome in a short term after ICRS implantation in keratoconic eyes. In the long term, corneal biomechanical changes limit the prediction of the ring segment effect.

Characteristics and Correlative Factors of Posterior Corneal Surface Changes after Laser in Situ Keratomileusis

Abstract:OBJECTIVE:To assess the changes of the posterior corneal surface and the factors influencing these changes after laser in situ keratomileusis (LASIK). These factors included the preoperative refraction, preoperative intraocular pressure (IOP), preoperative corneal thickness, spherical equivalent correction and one-week postoperative corneal thickness.METHODS:The posterior corneal radius of curvature for the posterior best fit sphere (PBFS), irregularity of posterior surface, and anterior chamber depth (ACD) were obtained with the scanning slit topography system before and 1 week, 1 month and 3 months after LASIK in 64 eyes of 34 patients with myopic refractive errors of -2.25 to -12.5 diopters. Multiple regression analysis was used to assess the factors that affecting these changes of posterior corneal surface. The difference of radius PBFS and posterior elevation difference (PED) was used as an indicator for the forward shift of posterior corneal surface.RESULTS:The ACD before the surgery and 1 week, 1 month and 3 months after the surgery was (3.2448 +/- 0.2806), (3.2770 +/- 0.2810), (3.2579 +/- 0.2932) and (3.2651 +/- 0.2875) mm, respectively. The PBFS at that time was (6.5095 +/- 0.2177), (6.3731 +/- 0.2127), (6.4257 +/- 0.2358) and (6.4354 +/- 0.2266) mm, respectively. The posterior corneal irregularity at 5 diameters zone was 0.622 +/- 0.142, 0.978 +/- 0.244, 0.884 +/- 0.207 and 0.881 +/- 0.174, respectively The preoperative posterior corneal radius of curvature for PBFS, the irregularity of the posterior surface and the ACD were significantly different from that of postoperative data at each time point (P < 0.01). There was no significant difference in these indexes between 1 month and 3 months postoperatively. Multiple regression analysis showed that factors relevant to the changes of posterior corneal surface were the corrected preoperative refraction (regression coefficient r = 0.0173, P = 0.005) and 1 week postoperative corneal thickness (mm) (regression coefficient r = -1.495, P = 0.001).CONCLUSIONS:The posterior corneal surface after LASIK become steeper and more irregular and remains stable in three months after the surgery. Eyes with thinner corneas and higher myopia requiring more ablation are more predisposed to the changes of posterior corneal surface. The results of the present study should not be used in extremely high degree myopia. A study using a larger group of patients with a longer observation period is required.

Abstract:PURPOSE: To investigate changes in Ocular Response Analyzer parameters after myopic LASIK. METHODS: Thirty myopic patients (60 eyes) undergoing bilateral LASIK were studied prospectively. Corneal hysteresis, corneal resistance factor, Goldmann-correlated intraocular pressure (IOPg), and corneal-compensated IOP (IOPcc) measurements were obtained with the Ocular Response Analyzer before and after LASIK. Central corneal thickness and corneal flap thickness were also measured with optical coherence tomography. RESULTS: Compared with preoperative baseline values, corneal hysteresis, corneal resistance factor, IOPg, IOPcc, and central corneal thickness decreased post-operatively (P<.01). Corneal hysteresis (CH), corneal resistance factor (CRF), IOPg, IOPcc, and central corneal thickness (CCT) increased between 10 days and 3 months (post-hoc, P<.05). (The differences between the preoperative values and those at 3 months postoperatively are denoted by Delta.) The correlation of the ablation depth was stronger for Delta CRF (r=0.457) than for Delta CH (r=0.271). Delta CH was negatively correlated to flap thickness (r=-0.361) and the ratio of flap thickness to preoperative central corneal thickness (r=-0.442) whereas Delta CRF was not. Delta CH was positively correlated with Delta IOPcc, and Delta CRF was positively correlated with Delta IOPg. Delta IOPg was positively correlated with Delta CCT whereas Delta IOPcc was riot. CONCLUSIONS: Corneal biomechanical properties measured by corneal hysteresis and corneal resistance factor were decreased at 10 days after LASIK. Minimal recovery (approximately 10% of the changes) was achieved by 3 months. The changes in biomechanical properties may affect IOP measurement. Differences in the early biomechanical effects of hyperopic and myopic laser in situ keratomileusis

Abstract:Purpose: To compare changes in corneal hysteresis (CH) and the corneal resistance factor (CRF) in myopic and hyperopic laser in situ keratomileusis (LASIK) and evaluate their relationship to the number of photoablative pulses delivered, a surrogate for ablation volume. Setting: Cleveland Clinic Cole Eye Institute, Cleveland, Ohio, USA. Methods: Preoperative and 1-week postoperative Ocular Response Analyzer measurements in eyes that had femtosecond-assisted LASIK were studied retrospectively. Changes in CH and CRF were compared and tested for correlation with the number of excimer laser pulses. Results: Thirteen myopic eyes and 11 hyperopic eyes were evaluated. Preoperative corneal thickness, CH, CRF, programmed correction magnitude, flap thickness, and total number of fixed spot-size photoablative pulses were similar in the 2 groups (P>.1). Decreases in CH and CRF were greater after myopic LASIK than after hyperopic LASIK (P<.005), and changes in CRF were correlated with the number of excimer laser pulses in the myopic group only (r = -0.63, P = .02). Regardless of ablation profile, changes in CH were more strongly correlated with preoperative CH values than with attempted ablation volume. Conclusions: With comparable flap thickness and attempted ablation volumes, myopic photoablation profiles were associated with greater decreases in CRF and CH than hyperopic profiles. Results indicate that preoperative corneal biomechanical status, ablation volume, and the spatial distribution of ablation are important factors that affect corneal resistance and viscous dissipative properties differently. Preferential tissue removal in the natively thicker paracentral cornea in hyperopia may partially account for the rarity of ectasia after hyperopic LASIK.

Abstract:AIM: To compare the difference of biomechanical stability after a femtosecond laser-assisted in situ keratomileusis(FS-LASIK)and laser-assisted subepithelial keratomileusis(LASEK)by ocular response analyzer(ORA). METHODS: This prospective study was conducted at the First Affiliated Hospital of Harbin Medical University, and myopic patients with the equivalent sphere between -2.00 and -5.00 diopters from January 2016 and December 2017 were enrolled. All the subjects were divided into FS-LASIK and LASEK group respectively according to different surgical methods. There were 64 patients(100 eyes)participated in the FS-LASIK group while 53 patients(100 eyes)in the LASEK surgery group. Corneal hysteresis value(CH)and corneal resistance factor value(CRF)were measured pre-, 1mo and 3mo postoperation of all the subjects. And the variance analysis of two groups was repeated to compare the overall difference between the two modes of operation from pre-operation to post operation. RESULTS: The CH and CRF value of the two groups were different before and after operation(P<0.05). The comparison results of repeated measurements showed that the CH value and CRF value of the surgical methods were reduced in 1 and 3mo after operation, respectively, and there was significance(P<0.05). The CH and CRF of FS-LASIK group was lower than LASEK group only in 1mo after the operation(P<0.05). CONCLUSION: Both surgeries could affect the biomechanical stability of cornea. From the point of view of biomechanical stability, LASEK operation is safer than FS-LASIK operation and reduces the possibility of postoperative refractive regression.
Comparison of Corneal Biomechanical Properties Between Post-LASIK Ectasia and Primary Keratoconus

Abstract:Purpose. To compare the corneal biomechanical properties between post-LASIK ectasia and primary keratoconus. Methods. A total of 42 eyes of 42 patients with matching age and central corneal thickness (CCT) were divided into two groups according to diagnosis of post-LASIK ectasia (PLE group; n = 21; age range: 22–47 years) and primary keratoconus (KC group; n = 21; age range: 21–47 years). The corneal biomechanical properties were assessed using Scheimpflug-based technology (Corvis ST; Oculus Optikgeräte, Wetzlar, Germany). The paired t-test and linear regression analysis were performed. Results. The PLE group had significantly higher mean stiffness parameter at the first applanation (SP-A1; 76.65 ± 21.66 vs 52.72 ± 13.65, p≤0.001) and mean stress-strain index (SSI) (SSI: 0.78 ± 0.16 versus 0.64 ± 0.12, p=0.001) than the KC group. SP-A1 was positively correlated with CCT in the PLE group (Pearson’s r = 0.816, p≤0.001), but not in the KC group (Pearson’s r = −0.014, p=0.952). No statistical correlation was observed between SSI and CCT in either group (Pearson’s r = 0.292, p=0.199, and Pearson’s r = 0.004, p=0.985, respectively). Conclusions. In our case series, KC manifested more severe than PLE in biomechanical properties. Since SSI measurements were independent of corneal thickness, it can be used for corneal biomechanical assessment.

Abstract:Soft-switching PWM combined three-level (TL) converters are very suitable for high input voltage with wide range applications due to the reduced voltage stress on power switches and reduced filter requirements. The converters include two transformers and have two operation modes, which provide possibilities to optimize the performance of the converters. This paper proposes some optimization strategies for the combined TL converters based on the control strategies and turns-ratios to achieve a better performance. Two prototype converters have been tested for the verification of the theoretical analysis.

Abstract:Background Aberrations of the eye are the difference between two surfaces: the ideal and the actual wave front. Higher order aberrations are a relatively small component, comprising about 10% of the eye’s total aberrations. High order aberrations increase with age and minor symmetry exists between the right and the left eyes. Aim and objectives To Compare corneal high order aberrations in post laser-assisted in situ keratomileusis with corneal high order aberrations in post Photorefractive keratectomy in myope. Subjects and methods this study was designed as a prospective randomized intervention study that conducted on 40 myopic eyes. The eyes divided into two randomized groups. Group A: 20 myopic eyes corrected with laser-assisted in situ keratomileusis and Group B: 20 myopic eyes corrected with Photorefractive keratectomy. Cases were collected for LASIK and PRK at operative theater in Beni-Suef University Hospital and clear vision laser center in Cairo, from May 2019 till January 2022. Result There was clinically significant difference between pre- and post-operative visual acuity, vertical trefoil and vertical coma of LASIK group and there was clinically significant between pre- and post-operative visual acuity, vertical trefoil and vertical coma of PRK group. But there was no significant difference between LASIK and PRK in visual acuity, spherical aberration, higher order 5 mm and 6 mm, vertical coma and Vertical trefoil. Conclusion There is non-significant increase in higher order aberration and spherical aberration post LASIK and post PRK with lesser amount in PRK.

Abstract:Objective To observe the relationship between the change of corneal biomechanical parameters and the measurement of intraocular pressure after LASIK.Design Prospective case series.Participants Eighty-one patients (162 eyes)with myopia were performed LASIK from Janurary to December in 2014 in the First Affiliated Hospital of Harbin Medical University.Methods The corneal hysteresis (CH),corneal resistance factor (CRF),Goldmann-correlated IOP value (IOPg) and corneal-compensated intraocular pressure (IOPcc) were examined with ocular response analyzer (ORA) before and after LASIK.The intraocular pressures were measure using non-contact tonometer (NCT) as well.Main Outcome Measures NCT,IOPcc,IOPg,CH,CRF before and after LASIK.Results Postoperative 3 months,CH,CRF,IOPcc,IOPg,and NCT measured values decreased significantly compared with those preoperatively.Thecomparison among IOPcc,IOPg and NCT postoperatively shows the significant distinction (P＜0.05).There is a negative correlation between △CH,△CRF and △IOPcc,△IOPcc-IOPg and △IOPcc-△NCT.There is a positive correlation between △CH,△CRF and △ IOPg and △ NCT (P＜0.05).Conclusion The corneal biomechanics and the measurement of intraocular pressure after LASIK were lower than preoperatively.There is a correlation between the decline of the measurement of intraocular pressure and the decrement of CH and CRF.The level of intraocular pressure drop may be affected by corneal biomechanical properties.
Early elastic and viscoelastic corneal biomechanical changes after photorefractive keratectomy and small incision lenticule extraction

Abstract:Purpose: To compare early changes in the corneal biomechanical parameters after photorefractive keratectomy (PRK) and small incision lenticule extraction (SMILE) and their correlations with corneal shape parameters. Methods: One hundred twenty four eyes received myopic PRK and SMILE for similar amounts of myopia. Corneal tomography with Pentacam HR, biomechanical parameters using Corvis ST, and Ocular Response Analyzer (ORA) were evaluated before and 2 weeks after surgery. The change in each parameter was compared between groups, while the difference in central corneal thickness and cornea-compensated intraocular pressure measured before and after surgery were considered as covariates. Results: A significant reduction was seen in the corneal stiffness parameter at first applanation, and an increase in deformation amplitude ratio (DAR), and integrated inverse radius (IIR) in both groups after surgery (p < 0.001) Changes in DAR, and IIR were significantly greater in the SMILE than in the PRK group (p < 0.001) Corneal hysteresis (CH) and corneal resistance factor (CRF) decreased in both SMILE and PRK groups after surgery, (p < 0.001) with no statistically significant difference between groups (p > 0.05) Among new Corvis ST parameters, DAR showed a significant correlation with changes in Ambrosio relational thickness in both groups (p < 0.05). Conclusions: Both techniques caused significant changes in corneal biomechanics in the early postoperative period, with greater elastic changes in the SMILE group compared to the PRK group, likely due to lower tension in the SMILE cap and thinner residual stromal bed in SMILE. There were no differences in viscoelastic changes between them, so the lower CH may reflect the volume of tissue removed.

Abstract:Purpose: To compare the postoperative corneal hysteresis (CH) and corneal resistance factor (CRF) of eyes implanted with a small aperture corneal inlay versus fellow eyes. Methods: Medical records of patients who underwent small aperture corneal inlay (KAMRA; AcuFocus, Inc., Irvine, CA) implantation were retrospectively reviewed. There were two groups: the implanted and non-implanted. Main outcome measures were CH, CRF, Goldmann-correlated intraocular pressure (IOPg), corneal-compensated IOP (IOPcc), and Goldmann applanation tonometry measurements performed preoperatively and at postoperative week 1 and months 1, 3, and 6. Results: The study included 68 eyes of 34 patients. CH was higher in the implanted group when compared with the non-implanted group at postoperative week 1 (12.2 ± 3.1 vs 10.9 ± 1.7 mm Hg; P = .007) and month 1 (12.3 ± 2.5 vs 10.9 ± 1.8 mm Hg; P = .001). CRF was higher in the implanted group when compared with the non-implanted group at postoperative week 1 (11.9 ± 2.9 vs 10.7 ± 1.6 mm Hg; P = .003) and month 1 (12.5 ± 2.5 vs 10.4 ± 1.8 mm Hg; P < .001). IOPcc was higher in the implanted group when compared with the non-implanted group at only the first postoperative month (17.1 ± 3.5 vs 14.3 ± 2.9 mm Hg; P < .001). There were no statistically significant differences in Goldmann applanation tonometry or IOPg measurements between the two groups (P > .05). At the 3-month postoperative visit, all parameters had returned to baseline and there was no change at the 6-month visit. Conclusions: Implantation of the KAMRA corneal inlay does not induce a permanent change in CH or CRF. A transient increase in both was seen in the early postoperative period.
Angle Kappa measurement pre-keratorefractive and post-keratorefractive surgery using Orbscan 3

Abstract:Aim of the study Measurement of the mean angle Kappa pre-keratorefractive and post-keratorefractive surgery utilizing Orbscan 3. Patients and methods This retrospective study was conducted on patients who had refractive surgeries, group A included patients who underwent Lasik and group B included patients who underwent Photo Refractive Keratectomy (PRK). Mean age from 18 to 45 years. Preoperative, 1-week, and 1-month postoperative assessments included detailed ocular examination, slit-scanning topography done by Orbscan 3, and total ocular aberrations by Zywave 3 were done. Results The study comprised 146 eyes of 75 individuals. Preoperative data demonstrated that angle Kappa mean in Lasik group (group A) was 2.38±1.54 at 147.05±96.94, in PRK group (group B) was 2.31±1.17 at 137.67±85.7. Paired mean differences of the two groups between preoperative data and 1-week postoperative show no statistically significant difference in central corneal thickness (CCT) nor angle Kappa values. Paired mean differences of the two groups between preoperative data and 1-month postoperative show no statistically significant difference regarding CCT nor angle Kappa values. Preoperative, 1-week postoperative, and 1-month postoperative correlation of angle Kappa to refractive error, spherical equivalent, average K-reading, CCT and axial length, showed that both groups had a positive correlation with axial length. Conclusion Keratorefractive surgeries do not affect angle Kappa or its intercept measurements.
Corneal biomechanics after small incision lenticule extraction and femtosecond laser in situ keratomileusis: A systematic review and meta-analysis

Abstract:Background: Small incision lenticule extraction (SMILE) and femtosecond laser in situ keratomileusis (FS-LASIK) have been extensively studied as the main surgical methods for corneal refractive surgery. However, there is no consensus on whether SMILE is superior to FS-LASIK in corneal biomechanics. Therefore, this systematic review and meta-analysis used the results of ocular response analyzer and corvis ST to explore whether SMILE is superior to FS-LASIK in corneal biomechanics. Methods: The literature was searched in PubMed, EMBASE, and Controlled Trials Register databases. The Cochrane Collaboration's "risk of bias" tool was used to evaluate the quality of the included randomized clinical trials, and the Newcastle-Ottawa Scale was used to evaluate the included non-randomized controlled trials. The results were analyzed using Revman 5.3. Results: Sixteen studies (3 randomized clinical trials and 13 non-randomized controlled trials) were included in this meta-analysis. There was no statistical difference in corneal biomechanics between SMILE and FS-LASIK in corneal hysteresis [mean difference (MD), 0.20; 95% confidence interval (CI): -0.09, 0.49; P = .18] and corneal resistant factor (MD, 0.31; 95% CI: -0.09, 0.71; P = .13), A1 time (MD, -0.02; 95% CI: -0.11, 0.07; P = .66), A1 length (MD, 0.01; 95% CI: -0.01, 0.03; P = .42), A1 velocity (MD, 0.00; 95% CI: -0.01, 0.01; P = .85), A2 velocity (MD, -0.01; 95% CI: -0.11, 0.09; P = .86), HC time (MD, 0.12; 95% CI: -0.13, 0.38; P = .35), The stiffness parameter at first applanation (MD, -7.91; 95% CI: -17.96, 2.14; P = .12), The ratio between the deformation amplitude 2 mm away from apex and the apical deformation (MD, 0.01; 95% CI: -0.26, 0.27; P = .96). Conclusion: A comprehensive assessment of the parameters of ocular response analyzer and corvis ST showed that SMILE is not superior to LASIK in corneal biomechanics 3 months post-surgery.
Evaluation of Changes in Human Corneas After Femtosecond Laser-Assisted LASIK and Small-Incision Lenticule Extraction (SMILE) Using Non-Contact Tonometry and Ultra-High-Speed Camera (Corvis ST)

Abstract:PURPOSE: To evaluate theoretical biomechanical advantages of human corneas treated with small incision lenticule extraction (SMILE) compared with femtosecond laser-assisted LASIK (FS-LASIK) Patients and methods: In a prospective, comparative, non-randomized, consecutive case series patients with moderate to high myopia and/or astigmatism underwent corneal refractive surgery. Patients either received standard FS-LASIK or SMILE. Preoperatively and up to 3 months postoperatively data were analyzed including tomography with Pentacam HR, Goldmann tonometry, non-contact tonometry, and deformability of the cornea using an ultra-high-speed camera (Corvis ST). Data were analyzed and p < 0.05 was considered as statistically significant (t-test).RESULTS: Seventy-three patients (128 eyes) were treated. Forty-eight eyes of 29 patients underwent FS-LASIK and 80 eyes of 44 patients underwent SMILE. Preoperative spherical equivalent (SE) was -3.23 ± 1.64 D (FS-LASIK group) and -4.83 ± 1.63 D (SMILE group) (p < 0.0001). Almost all parameters obtained by Corvis ST between preoperative and postoperative measurements showed significant changes after refractive surgery. Significant changes were seen between both groups postoperatively. However, matched pair subgroup analysis (n = 69; 26 eyes FS-LASIK; 43 eyes SMILE) of eyes with initially equal pachymetry, intraocular pressure, SE, and difference of pre- to postoperative pachymetry (p>0.05), showed no significant changes in parameters measured with Corvis ST (p>0.05).CONCLUSIONS: Corneal biomechanical parameters measured preoperatively with Corvis ST showed significant differences postoperatively in total and in both groups. In subgroup analysis with homogenous groups, FS-LASIK showed no significant changes in biomechanical data measured with Corvis ST compared with SMILE.

Abstract:Purpose: The objective of this study is to evaluate the in vivo corneal biomechanical response to three laser refractive surgeries. Methods: Two hundred and twenty-seven patients who submitted to transepithelial photorefractive keratectomy (tPRK), femtosecond laser-assisted in-situ keratomileusis (FS-LASIK), or small-incision lenticule extraction (SMILE) were included in this study. All cases were examined with the Corvis ST preoperatively (up to 3 months) and postoperatively at 1, 3, and 6 months, and the differences in the main device parameters were assessed. The three groups were matched in age, gender ratio, corneal thickness, refractive error corrections, optical zone diameter, and intraocular pressure. They were also matched in the preoperative biomechanical metrics provided by the Corvis ST including stiffness parameter at first applanation (SP-A1), integrated inverse radius (IIR), deformation amplitude (DA), and deformation amplitude 2 mm away from apex and the apical deformation (DARatio2mm). Results: The results demonstrated a significant decrease post-operation in SP-A1 and significant increases in IIR, DA, and DARatio2mm ( p < 0.05), all of which indicated reductions in overall corneal stiffness. Inter-procedure comparisons provided evidence that the smallest overall stiffness reduction was in the tPRK group, followed by the SMILE, and then the FS-LASIK group ( p < 0.05). These results remained valid after correction for the change in CCT between pre and 6 months post-operation and for the percentage tissue altered. In all three surgery groups, higher degrees of refractive correction resulted in larger overall stiffness losses based on most of the biomechanical metrics. Conclusion: The corneal biomechanical response to the three surgery procedures varied significantly. With similar corneal thickness loss, the reductions in overall corneal stiffness were the highest in FS-LASIK and the lowest in tPRK.

Abstract:PurposeTo compare posterior corneal elevation (PCE) changes and corneal biomechanical changes post fematosecond laser-assisted laser in situ keratomileusis (FS-LASIK) and small incision lenticule extraction (SMILE) procedures using the Pentacam system and the Ocular Response Analyzer (ORA).DesignRetrospective observational case series study.Method106 patients with bilateral myopia who underwent either FS-LASIK (56 patients) or SMILE (50 patients) were reviewed, only the right eye was used in the analysis. Inclusion criteria include a spherical equivalent of −6.00DS and completion of 12 months follow up. The main outcome evaluated was change in PCE at 3 months, 6 months and 12 months post-operation using the Pentacam system. Corneal biomechanical parameters were evaluated at 6 and 12 months with the ORA.ResultsPCE change at 3 and 6 months were not significantly different between the two procedures (p=0.064, p=0.109 respectively). At 12 months, PCE change was greater in FS-LASIK than SMILE (p=0.048). One-way ANOVA showed that for either procedure, the change in PCE did not differ at 3, 6 or 12 months post operation. CH and CRF values were reduced after both procedures, with FS-LASIK demonstrating a greater reduction in CRF than SMILE at 6 and 12 months (p=0.037 and p=0.001). Both CH and CRF reduction correlated with PCE increase at 6 and 12 months.ConclusionFS-LASIK demonstrated a greater increase in PCE than SMILE only at 12 months, as well as a greater reduction of CRF than SMILE. There were no significant differences in PCE change over time within either group.
