Response to Letter to the Editor on "Antibiotic dip and irrigation solutions confer increased antimicrobial efficacy of inflatable penile prosthesis hydrophilic surfaces compared with 0.05% chlorhexidine gluconate"

Abstract:We stand by the findings from our lab which independently identified without bias that 0.05% chlorhexidine gluconate (CHG) (Irrisept, Irrimax, Lawrenceville, GA) and normal saline (NS) irrigation both remove dip solutions from the hydrophilic penile prosthesis surface, and that vancomycin/gentamicin (VG) is the best irrigation and dip combination that we tested. 1 These in vitro findings regarding 0.05% CHG are concerning and should be taken seriously. Clinical data evaluating the safety and efficacy of 0.05% CHG and the hydrophilic surface will be available soon and I encourage the writer who has the largest clinical experience with 0.05% CHG to share their clinical results as well. Our in vitro data and the upcoming clinical data will guide how best to utilize dip/irrigations solutions to preserve the antimicrobial character of the hydrophilic implant surface. The hydrophilic surface from Coloplast (Minneapolis, MN) was introduced 22 years ago and has helped to reduce infection in thousands of men by eluting antibiotics into the surgical space for 24-48 hours. 2 It is crucial to ensure that any proposed irrigation/dip solution does not interfere with the protection offered by the hydrophilic surface. Use of 0.05% CHG in prosthetic urology has increased significantly over the past few years; however, no clinical study exists which demonstrates its safety and efficacy with the hydrophilic surface. Precedence does exist that irrigation solutions increase the risk for penile prosthesis infection. 3 In the interim, we can only rely on in vitro studies to help guide best use recommendations. 1 , 4 , 5
