Abstract:BackgroundDespite being considered the standard surgical procedure for symptomatic cervical disc disease, anterior cervical decompression and fusion invariably accelerates adjacent segment degeneration. Cervical total disc replacement is a motion-preserving procedure developed as a substitute to fusion. Whether cervical total disc replacement is superior to fusion remains unclear.MethodsWe comprehensively searched PubMed, EMBASE, Medline, and the Cochrane Library in accordance with the inclusion criteria to identify possible studies. The retrieved results were last updated on December 12, 2014. We classified the studies as short-term and midterm follow-up.ResultsNineteen randomized controlled trials involving 4516 cases were identified. Compared with anterior cervical decompression and fusion, cervical total disc replacement had better functional outcomes (neck disability index [NDI], NDI success, neurological success, neck pain scores reported on a numerical rating scale [NRS], visual analog scales scores and overall success), greater segmental motion at the index level, fewer adverse events and fewer secondary surgical procedures at the index and adjacent levels in short-term follow-up (P < 0.05). With midterm follow-up, the cervical total disc replacement group indicated superiority in the NDI, neurological success, pain assessment (NRS), and secondary surgical procedures at the index level (P < 0.05). The Short Form 36 (SF-36) and segmental motion at the adjacent level in the short-term follow-up showed no significant difference between the two procedures, as did the secondary surgical procedure rates at the adjacent level with midterm follow-up (P > 0.05).ConclusionsCervical total disc replacement presented favorable functional outcomes, fewer adverse events, and fewer secondary surgical procedures. The efficacy and safety of cervical total disc replacement are superior to those of fusion. Longer-term, multicenter studies are required for a better evaluation of the long-term efficacy and safety of the two procedures.
Cervical Disc Arthroplasty with ProDisc-C Artificial Disc: 5-Year Radiographic Follow-Up Results.

Abstract:BACKGROUND:Cervical disc arthroplasty is an alternative surgery to standard cervical decompression and fusion for disc degeneration. Different types of cervical disc prosthesis are used in China. The aim of this study was to evaluate the radiographic outcomes of cervical arthroplasty using the ProDisc-C prosthesis.METHODS:Radiographic evaluation, including static and dynamic flexion-extension lateral images, was performed at baseline and at final follow-up.RESULTS:Twenty six patients who had single-level ProDisc-C arthroplasty were followed up for a mean period of 63 months (56-76 months). The range of motion at the operated level was 9.3°±3.7° at baseline and 7.3°±3.5° at final follow-up, with a significant difference (P < 0.05). Seventeen of 26 levels (65.4%) developed heterotopic ossification: three were classified as grade II, 13 were classified as grade III, and 1 as grade IV, according to McAfee's classification. Forty nine adjacent segments were evaluated by lateral X-ray and 18 (36.7%) segments developed adjacent segment degenerations.CONCLUSIONS:ProDisc-C arthroplasty had acceptable radiographic results at 5-year follow-up. The range of motion was preserved. However, more than 60% of the patients developed heterotopic ossification.
Biomechanics of Hybrid Anterior Cervical Fusion and Artificial Disc Replacement in 3-Level Constructs: an in Vitro Investigation.

Abstract:BACKGROUND:The ideal surgical approach for cervical disk disease remains controversial, especially for multilevel cervical disease. The purpose of this study was to investigate the biomechanics of the cervical spine after 3-level hybrid surgery compared with 3-level anterior cervical discectomy and fusion (ACDF). MATERIAL AND METHODS:Eighteen human cadaveric spines (C2-T1) were evaluated under displacement-input protocol. After intact testing, a simulated hybrid construct or fusion construct was created between C3 to C6 and tested in the following 3 conditions: 3-level disc plate disc (3DPD), 3-level plate disc plate (3PDP), and 3-level plate (3P). RESULTS:Compared to intact, almost 65~80% of motion was successfully restricted at C3-C6 fusion levels (p<0.05). 3DPD construct resulted in slight increase at the 3 instrumented levels (p>0.05). 3PDP construct resulted in significant decrease of ROM at C3-C6 levels less than 3P (p<0.05). Both 3DPD and 3PDP caused significant reduction of ROM at the arthrodesis level and produced motion increase at the arthroplasty level. For adjacent levels, 3P resulted in markedly increased contribution of both upper and lower adjacent levels (p<0.05). Significant motion increases lower than 3P were only noted at partly adjacent levels in some conditions for 3DPD and 3PDP (p<0.05). CONCLUSIONS:ACDF eliminated motion within the construct and greatly increased adjacent motion. Artificial cervical disc replacement normalized motion of its segment and adjacent segments. While hybrid conditions failed to restore normal motion within the construct, they significantly normalized motion in adjacent segments compared with the 3-level ACDF condition. The artificial disc in 3-level constructs has biomechanical advantages compared to fusion in normalizing motion.
Long-term efficacy of cervical artificial disc replacement for the treatment of cervical spondylotic myelopathy

Abstract:Objectives:To evaluate the long-term efficacy of cervical artificial disc replacement in patients with cervical spondylotic myelopathy.Methods:Retrospective analysis of 17 patients who underwent singlelevel Bryan artificial disc replacement for cervical spondylotic myelopathy with more than 10 years follow-up in our hospital between December 2003 and December 2007.The age ranged from 37 to 76 years(mean age,57.2±8.9 years),including 13 males and 4 females.The involved levels were 3 cases of C4/5,9 cases of C5/6,5 cases of C6/7.According to the results of preoperative CT and MRI examination,the patients were divided into two groups,7 patients in group of simply cervical disc herniation and 10 patients in group of combined with osteophyte.The radiographic evaluation indexes included the global and segmental range of motion(ROM),Cobb angle and incidence of high grade(grade 3-4) paravertebral ossification(PO) of operated segment.The clinical indexes included Japanese Orthopaedic Association (JOA) score,neck disability index (NDI) and Odom's standard.The differences of clinical indexes and radiographic evaluation indexes were analyzed between the two groups,and in each group the difference was compared between preoperation and final fellow-up.The continuous variable data were analyzed by t-test.The grading evaluation data were analyzed by Mann-Whitney test.A P＜0.05 was considered statistically significant.Results:The mean age of disc herniation group was 56.1±11.7 years old,including 5 males and 2 females,and 3 cases of C5/6 level,4 cases of C6/7 level.The preoperative segmental ROM was 8.18°±3.70° and 9.07°±4.76° at final follow-up,mobility lost occurred in 1 case(14.3％);the preoperative global ROM was 42.78°±14.89° and 47.34o±6.93° at final follow-up,the preoperative segmental Cobb angle was 2.77°±1.08° and 2.37°±1.48° at final follow-up,segmental kyphosis occurred in 1 case,and the high level PO rate was 14.3％.The radiographic evaluation indexes (global and segmental ROM,and Cobb angle of operated segment) had no difference between preoperation and final follow-up.The mean age of combined osteophyte group was 57.9±7.0 years old,including 8 males and 2 females,and 3 cases of C4/5 level,6 cases of C5/6 level,1 case of C6/7 level.The preoperative segmental ROM was 11.46°±5.19° and 6.41°±4.87° at final follow-up,3 cases (30.0％)occurred mobility lost;the preoperative global ROM was 45.53°±11.89° and 41.60°±13.45° at final follow-up,the preoperative segmental Cobb angle was 2.14°±1.35° and 1.29°±1.33° at final follow-up,segmental kyphosis occurred in 1 case,and the high level PO rate was 40.0％.The segmental ROM and Cobb angle were significantly decreased at final follow-up(P＜0.05).In disc herniation group,the improvement rate of JOA score was (76.73±25.53)％,NDI％ decreased by (12.29±6.26)％,and the satisfaction rate of Odom's standard was 100.0％.In combined osteophyte group,the improvement rate of JOA score was (82.90±19.03)％,NDI％ decreased by (10.00±4.32)％,and the satisfaction rate of Odom's standard was 100.0％.The global and segmental ROM,Cobb angle of operated segment,JOA score,NDI％ and Odom's criteria had no significant difference between the two groups at final follow-up(P＞0.05).Conclusions:Cervical artificial disc replacement has a satisfied long-term clinical efficacy in the treatment of cervical spondylotic myelopathy.The maintenance of ROM at operated level of cervical artificial disc replacement in cervical spondylotic myelopathy patients caused by simply disc herniation is better than those caused by osteophyte.
Single-level Cervical Arthroplasty with ProDisc-C Artificial Disc: 10-Year Follow-Up Results in One Centre.

Abstract:The aim of this study was to evaluate the long-term clinical and radiographic outcomes of cervical arthroplasty using the ProDisc-C prosthesis. Clinical and radiographic evaluations, including dynamic flexion–extension lateral images, were performed at baseline and at 10-year follow-up. Twenty-seven patients who had single-level ProDisc-C arthroplasty were followed up for a mean period of 123 months. The range of motion at the operated level was 8.9° ± 3.9° at baseline and 6.6° ± 3.5° at final follow-up. Twenty of 27 levels (74%) developed heterotopic ossification. According to McAfee’s classification, one level was classified as grade I, four levels were classified as grade II, 12 levels were classified as grade III and three levels were classified as grade IV. Three patients developed recurrent cervical radiculopathy or myelopathy due to adjacent segment disease and received the reoperations. The reoperations included two cases of cervical arthroplasty at adjacent segments and one case of cervical laminoplasty. ProDisc-C arthroplasty had acceptable clinical and radiographic results at 10-year follow-up. Heterotopic ossification was common after ProDisc-C arthroplasty, which decreased the range of motion. Initial exploration of clinical outcomes of consecutive two-level cervical artificial disc replacement

Abstract:Objective:To evaluate the cervical alignment and clinical outcomes of consecutive two-level cer-vical artificial disc replacement.Method:23 cases with cervical spondylotic myelopathy and/or radiculopathy undergoing consecutive two-level cervical arthroplasty between April 2004 and January 2007 were analyzed retrospectively.Of these patients,17 cases underwent Bryan disc replacement(including 12 cases with malalign-ment of cervical spine preoperatively),6 cases underwent Prodisc-C replacement(including 4 cases with malalignment of cervical spine preoperatively).The range of motion,neutral alignment,Cobb's angle at disease segment,incidence of heterotopic ossification,degeneration at adjacent level and postoperative Odom score,JOA score and VAS score of each patient at 3th month,12th month and final follow-up were reviewed respective-ly.Result:There showed no significant difference with respect to the range of motions,the neutral alignment and the operative segment Cobb's angles between pre-operation and post-operation(P>0.05).The local kypho-sis in the patients with Bryan disc replacement worsened after operations(P <0.05),whereas those with Prodisc-C replacement ameliorate(P<0.05).Also there showed significant difference with repect to the Odom's score,JOA spinal function score and VAS neckpain score between pre-operation and post-operation(P<0.05).2 cases were complicated with heterotopic ossification and lose of the range of motion,1 case was complicated with adjacent segment degeneration.Conclusion:Two-level cervical artificial disc replacement plays its role in maintaining of the cervical spine alignment and motion preservation.Prodisc-C artificial disc is available for patients with cervical malalignment.
The Mid-Term Follow-Up Results of Artificial Disc Replacement for Discogenic Low Back Pain

Abstract:Objective To evaluate the mid-term clinical and radiographic results of artificial disc replacement (ADR) for discogenic low back pain.Methods From July 2004 to July 2007,21 patients with discogenic low back pain,aged from 26 to 67 years,underwent lumbar ADR with the Charité Ⅲ artificial disc at 22 levels and Activ L prosthesis at 2 levels:L4-5 5 cases,L5S1 13 cases,L4-5 and L5S1 3 cases.The diagnosis was proved by discography in all the patients.The clinical and radiographic results were evaluated and compared between pre-,post-operation and finial follow-up.Results All patients were followed up for 4to 7 years (average,5.1 years).There were statistical differences between pre-operation and final follow-up in Oswestry disability index and Visual Analogue Scale for back pain and leg pain.All the prostheses were mobile without dislocation,breakage,subsidence or spontaneous fusion,only a slight scoliosis was noted in one patient.At the ADR levels,the anterior and posterior intervertebral height increased averagely 6.3 mm and 1.9 mm respectively,the lordosis increased 2.9°,and the mean range of motion (ROM) was 4.6°.At the adjacent level proximal to ADR,the intervertebral height and lordosis decreased slightly,and the ROM decreased 2.5°.The total lumbar lordosis increased 7.8°,and the total lumbar ROM increased 2.4°.According to MRI and CT scans,the degeneration of proximal adjacent disc and facet were not evident,however the degeneration of facets at the ADR levels was evident.Conclusion The 5 years results of ADR for discogenic low back pain were satisfactory,with preserved motion at the ADR level,and the degeneration of adjacent level was not evident.However,there was obvious degeneration in facet joints of the ADR level.
A Prospective Randomized Cohort Study On 3d-Printed Artificial Vertebral Body In Single-Level Anterior Cervical Corpectomy For Cervical Spondylotic Myelopathy

Abstract:Background: This was a prospective randomized cohort study aiming at examining the safety and efficacy of artificial vertebral body (AVB) fabricated by electron beam melting (EBM) in comparison to conventional titanium mesh cage (TMC) used in single-level anterior cervical corpectomy and fusion (SL-ACCF).Methods: Forty patients with cervical spondylotic myelopathy (CSM) underwent SL-ACCF using either the EBM-AVB or the TMC. Patients were evaluated for their demographics, radiological characteristics, neurologic function [using the Japanese Orthopaedic Association (JOA) scale], and health-related quality-of-life (HRQoL) aspects [using the Short Form 36 (SF-36)] before and after the surgery and comparison was made between the two groups both at baseline and the last follow-up. The Student t-text, paired-sample t-text, and Fisher's exact test were used when appropriate to detect any statistical significance at the level of alpha=0.05.Results: Post-operative recovery was uneventful for all patients and no revision surgery was required. There were no significant differences between the EBM-AVB group and the TMC group at baseline. Patients in both groups demonstrated significant improvement in cervical alignment, JOA score, and SF-36 score after the surgery. Six months post-operatively, patients in the EBM-AVB group were found to have significantly less loss of fusion height and lower incidence for severe implant subsidence compared with the TMC group. Patients in the two groups were comparable at the last follow-up regarding their rate of fusion, cervical alignment, JOA recovery rate, SF-36 score, and by Odom's criteria.Conclusions: For CSM patients undergoing SL-ACCF, the EBM-AVB group demonstrated comparable outcomes regarding patient cervical alignment, neurologic function, and HRQoL in comparison with the TMC group. Furthermore, the use of EBM-AVB was associated with decreased loss of the height of the fusion mass and a lower rate for severe implant subsidence.
Comparison of the Mid-Term Follow-Up Results Between Bryan Cervical Artificial Disc Replacement and Anterior Cervical Decompression and Fusion for Cervical Degenerative Disc Disease

Abstract:Objective To evaluate the mid-term tollow-up results of cervical artificial disc replacement (CADR) for cervical degenerative disc disease,and to explore whether it can reduce the occurrence of adjacent segment degeneration (ASD).Methods A prospective comparative study of 93 patients who underwent CADR or anterior cervical decompression and fusion (ACDF) for cervical degenerative disc disease were conducted.All patients were followed up for more than 6 years.The Japanese Orthopaedic Association (JOA) score,neck disability index (NDI),Odom's scale,X-rays and magnetic resonance imaging (MRI) were used to evaluate the clinical and radiologic results.Results Twenty eight patients who underwent CADR and 35 patients who underwent ACDF had complete follow-up data.At final follow-up,the JOA score and NDI improved significantly in both groups.Between the two groups,there was no significant difference in terms of JOA score,NDI and Odom's scale.The sagittal alignment was well maintained in both groups.The total cervical spine range of motion (ROM) had no significant change for the CADR group,whereas,it significantly decreased for the ACDF group.The ROM at the replacement level of CADR patients decreased from 9.5° ± 3.7° before operation to 7.0° ± 3.0° 3 months after operation,and it was maintained to 6.6° ± 4.1° at final follow-up without significant decrease.Lateral radiographs and T2-weighted MRI showed the incidence of ASD in CADR group was significantly lower than that in ACDF group.Conclusion The six-year follow-up results of CADR are basically satisfactory.Compared with ACDF,it could better preserve physiological motion and biomechanics of cervical spine,and reduce the incidence of ASD.
Medium and long term result of the single level cervical disc arthroplasty for cervical spondylosis meylopathy

Abstract:Objectives: To investigate the medium and long term clinical outcome of the single level cervical disc arthroplasty(CDA) vs anterior cervical decompression and fusion(ACDF) in a randomized controlled study.Methods: The medium and long term clinical outcome of the single level Bryan prosthesis CDA(experimental group) and conventional ACDF(controlled group) was reviewed retrospectively.All the patients participated in a randomized controlled prospective study from 2003 to 2005,the inclusive criteria for cervical disc arthroplasty was used in 80 cases,and 32 patients was in CDA group and 35 patients in ACDF group finished the clinical and imaging follow-up(range,7-9 years).The range of movement(ROM) in operation segement was determined by dynamic X-ray.The heterotopic ossification around the prosthesis was determined according to McAfee classification,the adjacent segment degeneration was verified according to Miyazaki classification on MRI T2 weighted images,and the symptom improvement was determined by NDI and VAS score.Results: The postoperative follow-up averaged 8.8 years,ranging from 7.2 to 9.6 years.No prosthesis displacement occurred in CDA group.Range of motion in CDA group averaged 6.35°±1.45°,ranging from 1.20° to 8.20°.The heterotopic ossification around the prosthesis was noted in 8 of 32(25%) segments,which induced loss of motion in 2 cases.3 patients underwent the revision surgery due to 1 cranial adjacent segment degeneration and canal stenosis caused by ossification next to adjacent segment in 2 cases.The adjacent disc degeneration was noted as deterioration of 1 grade in 22 and 2 grade in 8 of the rest 63 segments,but remained asymptomatic.3 patients underwent revision surgery in ACDF group,with cranial disc degeneration and neurological deficit in 2 and spinal cord compression due to caudal disc degeneration in 1.The adjacent disc degeneration was noted as deterioration of 1 grade in 34 and 2 grade in 15 of the rest 67 segments,but remained asymptomatic.The NDI,VAS score for neck and upper limb at final follow-up in CDA group was 16.83±3.12,1.17±0.41 and 1.96±0.51 respectively;while in ACDF group,it was 17.21±3.53,1.23±0.35 and 1.86±0.62 respectively.There was significant improvement compared with pre-operation,but no statistical difference between 2 groups(P>0.05).Conclusions: No significant difference of medium and long term outcome exists between cervical disc arthroplasty and ACDF,while the former can maintain the ROM of involved segment as well as preventing ASD,which is optional for single level cervical spondylosis meylopathy.

Abstract:Objective: To evaluate the long-term efficacy of cervical artificial disc replacement for patients with cervical disc herniation and degenerative cervical canal stenosis. Methods: Total of sixty-eight patients underwent single-level Bryan artificial disc replacement in Beijing Jishuitan Hospital from December 2003 to December 2007 with a minimum 10-year follow-up were retrospectively analyzed. There were 43 males and 25 females with a mean age of (46±8) years. According to preoperative CT and MRI, the patients were divided into two groups: 27 patients in cervical disc herniation group and 41 patients in degenerative cervical canal stenosis group. The evaluation indexes before surgery and at last follow-up were compared between two groups. The clinical indexes included Japanese Orthopaedic Association (JOA) score, neck disability index (NDI) and Odom's grade; and the radiological indexes included the global and segmental range of motion (ROM), Cobb's angle at operated level. The continuous variable data were analyzed by independent sample t test. Results: In cervical disc herniation group, the improvement rate of JOA score was 83%±22%, NDI% decreased by 14%±9%, and Odom's grade was excellent in 17 patients, good in 10 patients. In degenerative cervical canal stenosis group, the improvement rate of JOA was 68%±34%, NDI% decreased by 11%±7%, and Odom's grade was excellent in 19 patients, good in 18 patients, fair in 4 patients. The segmental ROM was 10°±4° and 7°±6° in cervical disc herniation and degenerative cervical canal stenosis group at last follow-up (t=2.284, P=0.026). The global ROM was 50°±9° and 44°±14° in cervical disc herniation and degenerative cervical canal stenosis group at last follow-up (t=2.112, P=0.038). Conclusions: Cervical artificial disc replacement has a favorable long-term efficacy in treating cervical degenerative diseases. The postoperative global and segmental ROM in patients with cervical disc herniation are better than those in patients with degenerative cervical canal stenosis.

Abstract:STUDY DESIGN:Prospective, randomized, controlled, multicenter clinical trial. OBJECTIVE:To compare outcomes of cervical disc arthroplasty with those of anterior cervical decompression and fusion (ACDF) in a Chinese population. SUMMARY OF BACKGROUND DATA:Cervical disc arthroplasty has been found to be superior to ACDF for maintaining range of motion (ROM) at the index spinal segment and possibly will avoid abnormal stress to adjacent segments. METHODS:A total of 120 patients from 3 large hospitals in China were randomly assigned to treatment with cervical disc arthroplasty (n = 60) using the BRYAN prosthesis or ACDF (n = 60) and were observed postoperatively for 24 months. RESULTS:The 2 groups had similar preoperative demographics and baseline characteristics including ROM, neck disability index, and visual analogue scale for neck and arm pain. The total disc replacement (TDR) group had a significantly longer operation time than the ACDF group (P < 0.001). Outcome data obtained after 24 months revealed a significant difference between the groups in mean change from baseline in ROM at the index level (P < 0.001); ROM was maintained in the TDR group but reduced in the ACDF group. There were no significant between-group differences in the baseline changes in neck disability index or visual analogue scale scores for pain. One patient in the TDR group and 4 patients in the ACDF group required reoperations. CONCLUSION:At 24 months after surgery, the cervical disc prosthesis yielded good clinical results while maintaining ROM at the index level. Cervical disc arthroplasty appears to be a viable alternative to ACDF.
Biomechanics of Artificial Disc Replacements Adjacent to a 2-Level Fusion in 4-Level Hybrid Constructs: an in Vitro Investigation

Abstract:BACKGROUND The ideal procedure for multilevel cervical degenerative disc diseases remains controversial. Recent studies on hybrid surgery combining anterior cervical discectomy and fusion (ACDF) and artificial cervical disc replacement (ACDR) for 2-level and 3-level constructs have been reported in the literature. The purpose of this study was to estimate the biomechanics of 3 kinds of 4-level hybrid constructs, which are more likely to be used clinically compared to 4-level arthrodesis. MATERIAL AND METHODS Eighteen human cadaveric spines (C2-T1) were evaluated in different testing conditions: intact, with 3 kinds of 4-level hybrid constructs (hybrid C3-4 ACDR+C4-6 ACDF+C6-7ACDR; hybrid C3-5ACDF+C5-6ACDR+C6-7ACDR; hybrid C3-4ACDR+C4-5ACDR+C5-7ACDF); and 4-level fusion. RESULTS Four-level fusion resulted in significant decrease in the C3-C7 ROM compared with the intact spine. The 3 different 4-level hybrid treatment groups caused only slight change at the instrumented levels compared to intact except for flexion. At the adjacent levels, 4-level fusion resulted in significant increase of contribution of both upper and lower adjacent levels. However, for the 3 hybrid constructs, significant changes of motion increase far lower than 4P at adjacent levels were only noted in partial loading conditions. No destabilizing effect or hypermobility were observed in any 4-level hybrid construct. CONCLUSIONS Four-level fusion significantly eliminated motion within the construct and increased motion at the adjacent segments. For all 3 different 4-level hybrid constructs, ACDR normalized motion of the index segment and adjacent segments with no significant hypermobility. Compared with the 4-level ACDF condition, the artificial discs in 4-level hybrid constructs had biomechanical advantages compared to fusion in normalizing adjacent level motion.

Abstract:ObjectivesTo design and fabricate a 3D-printed cervical cage composite of polylactic acid (PLA)/nano-sized and β-tricalcium phosphate (β-TCP).ResultsCAD analysis provided a useful platform to design the preliminary cage. In vitro cell culture and in vivo animal results showed promising results in the biocompatibility of the constructs. Endplate matching evaluation showed better matching degree of 3D-printed cages than those of conventional cages. Biomechanical evaluation showed better mechanical properties of 3D-printed cages than those of conventional cages.ConclusionThe novel 3D printed PLA/pβ-TCP cage showed good application potential, indicating a novel, feasible, and inexpensive method to manufacture cervical fusion cages.

Artificial Cervical Disc Replacement in Cervical Spondylosis

Abstract:Objective Anterior cervical fusion has been a routine method for the surgical treatment of cervical spondylosis, but concerns for the possibility of acceleration of adjacentsegment disease after fusion have been growing. The artificial cervical disc replacement provides the opportunity to preserve motion after neural decompression while providing stability, which opens a new pathway for the surgical therapy of cervical spondylosis. Method We summarized the results of 16 patients of cervical herniated disc treated by artificial cervical disc replacement after cervical microdecompression and arthroplasty from December 2005 to March 2008. The average followup time was 17 momths. Results All patients got improved neurological function postoperatively. The cervical flextion and extention X - film showed good curvature and motion. 1 patient developed temporary hoarseness and recovered within 1 week. There was no other complications. Conclusions Cervical microdecompression with Bryan disc replacement is a good choice for adequate patient of cervical degenerative disc disease.

Abstract:This article describes the rules of motion of a human cervical disc after a 3-level hybrid construct.7cadaveric cervical spines(C2-T1)were initially tested in intact condition.Then,all patients had a 1-level(C4-C5)total disc replacement(TDR)followed by a 2-level(C3-C4and C5-C6segments)anterior cervical discectomy and fusion(ACDF).The movement differences among the 3-level hybrid construct,the intact disc and the 1-level TDR were compared with the 3-level hybrid construct and previous 3-level fusion data(dynamic and static plating systems).The comparisons show that the 3-level hybrid construct performs better than3-level fusion.
Long-term Outcomes of Cervical Artificial Disc Replacement for Patients with Degenerative Cervical Canal Stenosis

Abstract:Objective To evaluate the long-term efficacy of cervical artificial disc replacement for patients with degenerative cervical canal stenosis.Methods All of 43 patienta underwent single level Bryan cervical artificial disc replacement for degenerative cervical canal stenosis were retrospectively analyzed with a minimum 10 years follow-up in our hospital between December 2003 and December 2007.There were 28 males and 15 females with an average age of 56.7 ± 8.0 years which ranged from 37 to 76 including 1 case of C3.4 level,11 xases of ~ level,26 cases of C~ level and 5 cases of C6,7 level.According to the symptoms and location of compression on radiograph,the patients were divided into 3 groups:15 patients in radiculopathy group,10 patients in myelopathy group and 18 patients in myeloradiculo p~hy group.Radidogical evaluation indexes include global range of motion (ROM),segmental ROM,segmental Cobb angle.Clinical evaluation indexes including Japanese Orthopaedic Association (JOA) score,neck disability index (NDI) and Odom's criteria.The variable data were analyzed by one-way ANOVA,the grading data were analyzed by Kruskal-Wallis H test.The preoperative and postoperative data were analyzed by paired t test.Results At the last follow-up,the Cobb angle of the operative segment was 1.29°±1.34°in myelopathy group(t=4.606,P=0.001),0.71°±2.20°in radiculopathy group (t=2.355,P=0.034),and 0.69°±2.12° in myeloradiculopathy group (t=3.312,P=0.004),which was significantly lower than that before operation.At the last follow-up,ROM of the operative segment in myelopathy group was 6.41 o ±4.87°,which was significantly lower than that before operation (11.46° ±5.19°,t=3.589,P＜ 0.05),and there was no significant difference in the other two groups.There was no significant difference in other imaging parameters among the three groups.JOA scores at the last follow-up were 16.33±0.75 in radiculopathy group (t=5.857,P＜ 0.001),16.00±1.05 in myelopathy group(t=8.337,P＜ 0.001) and 14.78±1.69 in radiculopathy group (t=4.045,P＜ 0.001);NDI were 13.07％±5.90％ in radiculopathy group (t=7.097,P＜ 0.001),12.60％±4.22％ in myelopathy group (t=7.319,P＜ 0.001) and 23.11％±14.18％ in radiculopathy group (t=4.229,P＜ 0.001),which were all significantly improved than those before operation.The excellent and good rate of Odom's criteria (H=2.719,2.411,P＜ 0.05),JOA (LAD-t=3.770,4.080,P＜0,05) and NDI(LAD-t=2.850,2.643,P＜ 0.05) at the last follow-up of the patients in radiculopathy group and myelopathy group were better than those of patients in myeloradiculopathy group (P＜ 0.05).Conclusion Cervical artificial disc replacement has a satisfied long-term clinical efficacy with degenerative cervical canal stenosis.The efficacy of myelopathy and radiculopathy were better than myeloradiculopathy.

Abstract:BACKGROUND:Anterior cervical discectomy and fusion has been considered the gold standard for treatment of symptomatic degenerative disc disease within the cervical spine.However,fusion may result in progressive degeneration of the adjacent segments.Artificial disc replacement seems to be promising,segmental motion and stability are preserved while the spinal canal has been enlarged,and the intervertebral disc biological mechanics has been preserved,but not leads to progressive degeneration of the adjacent segments.OBJECTIVE:To observe the functional outcome and kinematics after the Prodisc artificial neck intervertebral disc replacement.METHODS:A total of 55 cases (69 intervertebral discs),29 males and 26 females,aged 48 (31-76) years were selected,including 14 with bi-segmental injury.There were 32 cases of cervical spondylotic myelopathy,9 of cervical spondylotic radiculopathy,and 14 of mixed type cervical spondylosis.Prodisc artificial neck intervertebral disc replacement was performed using anterior or oblique incision.Prospective data JOA score and kinematic measures were collected before surgery and at 3,6,12,and 24 months after surgery.Range of motion was determined by independent radiologic assessment of flexion-extension radiographs.RESULTS AND CONCLUSION:The 55 patients were followed-up,including 41 undergoing single segmental disc replacement and 14 undergoing bi-segmental disc replacement.They were followed up for 22 (56-48) months postoperatively.JOA scores displayed improved cervical function by 56%.Range of motion of sagittal and coronal planes were similar to those prior to operation compared with preoperatively (P=0.45,0.74),and the range of motion and stability were maintained as adjacent segments.Results showed that the artificial disc replacement maintained range of motion of original intervertebral disc,accelerated patient recovery,and slightly affected adjacent intervertebral disc.

Abstract:Objective To prospectively compare the clinical outcomes of Bryan artificial cervical disc replacement with anterior cervical decompression and fusion(ACDF) in treating single-level cervical spondylotic myelopathy(CSM) or radiculopathy.Methods From August 2006 to February 2009,59 consecutive patients with single-level CSM or radiculopathy were randomly assigned to receive a treatment with Bryan artificial cervical disc replacement(n=27) or ACDF(n=32).Operation time,blood loss and hospital stay were compared between the 2 groups.Range of motion(ROM) of index level and the whole cervical spine were measured on dynamic radiographs.Visual analog scale(VAS) for neck and arm pain,neck disability index(NDI),and radiological measurement were accessed within 2 groups at 6 weeks,and 3,6,12 and 24 months after surgery,respectively.Results There were no statistically significant differences in operation time,blood loss and hospital stay between the 2 groups(P>0.05).VAS for neck and arm pain and NDI were statistically significantly different between 2 groups at 6 weeks and 3 months postoperatively(P<0.05),and not at other time points(P>0.05).However,all the data were significantly different from those before surgery in both groups(P<0.05).ROM of index level was maintained in normal range in Bryan group and had no change in ACDF group.Compared with preoperative motion,ROM of the whole cervical spine was not significantly changed at all the time points in Bryan group(P>0.05),but was significantly different at 6 weeks and 3 months after surgery in ACDF group(P<0.05),which returned to normal gradually in 6 months postoperatively and later.Conclusion This mid-term follow-up indicates that Bryan artificial cervical disc replacement maintains normal ROM of index level,and obtains similar therapeutic effect with that of ACDF.What's more,Bryan replacement is superior to ACDF in regard to short-term clinical outcomes.

Abstract:Objective: To evaluate the safety and effectiveness of applying self-stabilizing zero-profile three-dimensional (3D) printed artificial vertebral bodies in anterior cervical corpectomy and fusion (ACCF) for cervical spondylotic myelopathy. Methods: A retrospective analysis was conducted on 37 patients diagnosed with cervical spondylotic myelopathy who underwent single-level ACCF using either self-stabilizing zero-profile 3D-printed artificial vertebral bodies ( n=15, treatment group) or conventional 3D-printed artificial vertebral bodies with titanium plates ( n=22, control group) between January 2022 and February 2023. There was no significant difference in age, gender, lesion segment, disease duration, and preoperative Japanese Orthopedic Association (JOA) score between the two groups ( P>0.05). Operation time, intraoperative bleeding volume, hospitalization costs, JOA score and improvement rate, incidence of postoperative prosthesis subsidence, and interbody fusion were recorded and compared between the two groups. Results: Compared with the control group, the treatment group had significantly shorter operation time and lower hospitalization costs ( P<0.05); there was no significant difference in intraoperative bleeding volume between the two groups ( P>0.05). All patients were followed up, with a follow-up period of 6-21 months in the treatment group (mean, 13.7 months) and 6-19 months in the control group (mean, 12.7 months). No dysphagia occurred in the treatment group, while 5 cases occurred in the control group, with a significant difference in the incidence of dysphagia between the two groups ( P<0.05). At 12 months after operation, both groups showed improvement in JOA scores compared to preoperative scores, with significant differences ( P<0.05); however, there was no significant difference in the JOA scores and improvement rate between the two groups ( P>0.05). Radiographic examinations showed the interbody fusion in both groups, and the difference in the time of interbody fusion was not significant ( P>0.05). At last follow-up, 2 cases in the treatment group and 3 cases in the control group experienced prosthesis subsidence, with no significant difference in the incidence of prosthesis subsidence ( P>0.05). There was no implant displacement or plate-screw fracture during follow-up. Conclusion: The use of self-stabilizing zero-profile 3D-printed artificial vertebral bodies in the treatment of cervical spondylotic myelopathy not only achieves similar effectiveness to 3D-printed artificial vertebral bodies, but also reduces operation time and the incidence of postoperative dysphagia.
