Abstract:Rational or irrational drug use is actually relative.The aims of rational drug use are safety,effectiveness and economy,all of which can't be always satisfied together.Otherwise,any treatment protocol can not be fit to all of the characteristics of disease,patients and pharmacy.The judgment of rational drug use is also related owing to the relativity of the credibility and reliability of the results of clinical trials and clinical information,etc.At last,we must emphasize that the result of rational drug use is not always satisfactory,as it might be influenced by many factors.In clinical work,we must do our best to avoid irrational drug use.But if can't,we should estimate the benefits and harms,and then make scientific decision.Furthermore,pharmaceutical care and doctor-patient communication should be enhanced.

Abstract:OBJECTIVE:To explore the application of path analysis in pharmacoeconomics.METHODS:The data in clinical trial were fitted by path analysis model.The influencing factors and their relationships were revealed by parameter estimation in the model,and then the pharmacoeconomics was evaluated.RESULTS:The relationship of therapeutic regimen,therapeutic effectiveness and hospitalization cost were reflected by the path analysis model quantitatively and objectively.The goodness of fit index(GFI)of the model was 0.943 2,and the adjustive goodness of fit index(AGFI)was 0.858 1,which proclaiming that the fitting of the model was good.CONCLUSION:Pharmacoeconomics evaluation might be conducted effectively through path analysis.

Abstract:The article analyzes the situation on irrational use of antibiotics in China and why there is irrational drug use in medicalinstitutions, it points out that the promotion of rational drug use is a long-term, tough and complicated work. Abstract:Explanatory clinical trials do not have the most suitable design to evaluate the impact of a new pharmaceutical product in clinical practice, in terms of effectiveness and costs, that is to say, efficiency. Pharmacoeconomic simulation models can be used to this end, using combined efficacy data obtained through a meta-analysis or efficiency clinical trials. It is permissible that these trials are not blinded for the physician and the patient, but whenever possible they should be blinded for the clinicians who interpret data within the study. Assignment of patients in treatment groups should be done at random to safeguard the studies internal validity. Pharmacoeconomic simulation models can be useful in the early phases of clinical research (phases I and II), but should be further developed particularly in Phase IIIA, in order to provide backing for price fixing and the Health System's reimbursement of a new drug. At least one pragmatic pharmacoeconomical clinical study should be carried out in Phases IIDB and IV, to obtain cost and efficiency data under “real conditions”.

Abstract:Objective Evaluate the drug-use state which was infected by the adjustments of medicine price's evaluation methods in Campus hospital of Zhejiang University,objective to provide evidence based strategies on guiding reasonable clinical medication and the drug manufacturing and trade system.maintenance.Methods The consumption of medicine in terms of quantity and amount of money in our hospital during 2007～2008 was investigated.The date was analyzed using DDDs sequence analytical methods.Results In 2008,the money of collecting medicine increased by 26.78 percent but the retail sales only increased by 19.75 percent,over 2007.No significant changes were found in medicine using.Conclusion The bubble price of medicine and the burden on patients could be reduced by the adjustments of medicine price's evaluation methods.

Abstract:In order to help health administrators and hospital staff, this essay provides an introduction to the origin and development of drug utilization evaluation in developed countries and the contents of retrospective, current and prospective evaluation of drug utilization. Among them, the operation of drug utilization evaluation includes establishing committee, making standards of drug utilization evaluation, collecting and evaluating information, and intervention measures and their effects. It also addresses the significant role of drug utilization evaluation as an important reference for Chinese hospital and health administrators and health insurance firms executives, making the drug utilization evaluation in hospitals, outcome of drug therapy, care quality and income improved.

Abstract:As a technical tool to guide the optimal allocation of healthcare resources, pharmacoeconomic evaluation has been gradually applied to national healthcare decisions, including the adjustment of the National Reimbursement Drug List in China. With the acceleration of drug market approval and access to insurance coverage, the demand for pharmacoeconomic evaluation in healthcare decision-making is also increasing. Designing a pharmacoeconomic evaluation alongside clinical trials has become an important research direction and the source of evidence. In order to facilitate the standardization of this research field, the current consensus focuses on the topic of how to conduct a pharmacoeconomic evaluation alongside clinical trials from the Chinese healthcare system and decision-making context. The consensus group reconciled expert opinions and developed recommendations covering the entire process of study design, data collection and management, data analysis, and methods and results reporting. A total of 24 recommendations regarding to methodology and practice were developed from eighteen aspects including target population, selection of comparators, calculation of sample size, data collection of healthcare resource utilization, data collection of health-related quality of life, cost analysis, health outcome analysis, addressing the missing and censored data, etc., with the aim to provide reference for scholars and decision makers in this research filed.

Abstract:Objective To value the promoting effect on the rational use of drugs and quality of medical practice by clinical pharmacists(CPs)' work-going around the wards and joining in the clinical conference.Methods The two groups were chosen: 55 patients without CPs' concern randomised as group A,and 55 patients under CPs' concern randomised as group B.The days in hospital,total cost,the cost of all drugs,the cost of antibacterials,the days of antibacterial administration,the sorts of antibacterials used of the two groups were counted by the records as charged,which were statistically analysed by t test with SPSS 13.0.So CPs' value was proved.Results Between group A and group B,the days in hospital had no statistical difference(P>0.05);the total fees,the cost of all drugs,the cost of antibacterials,the days of antibacterial administration significantly lowered(P<0.01).Conclusion The work of CPs in medical practice can promote the rational and safe use of drugs,and can reduce medical cost,and plays an important role on solving the health matters,such as,high medical cost.Laws are needed to support CPs' work.

Abstract:With the premise of drug safety and effectiveness, pharmacoeconomic evaluation can provide optimal solutions for diversified decision-making application scenarios from different research perspectives while maximizing the rational utilization of existing healthcare resources. Chinese patent medicine is an essential component of pharmaceutical utilization in China and a significant part of healthcare expenditure in China. However, the economic evaluation of post-marketing Chinese patent medicine is lacking. These evaluations often lack standardization, exhibit varying quality, and are unable to effectively support healthcare decisions, indicating a need for improvement in overall quality. Given this situation, this project has gathered leading experts from China and has strictly adhered to the requirements of the group standards set by the China Association of Traditional Chinese Medicine in developing Guidelines for economic evaluation of post-marketing Chinese patent medicine, aiming to provide methodological guidance for the post-market pharmacoeconomic evaluation of Chinese patent medicine, enhancing the standardization of pharmacoeconomic evaluations of Chinese patent medicine and the scientific validity of research results, and thereby elevating the overall quality of pharmacoeconomic evaluations for post-marketing Chinese patent medicine. The guidelines adhere to the framework provided by relevant laws and regulations in China and technical guidance documents. It is based on guidance from traditional Chinese medicine(TCM) theories, focusing on the unique characteristics of TCM. It covers various aspects of pharmacoeconomic evaluation, including fundamental principles, research topic selection, research question definition, study design type selection, cost identification and measurement, health outcomes, and evaluation methods. The guidelines offer methodological recommendations and decision guidance to address common issues and challenges in the pharmacoeconomic evaluation of post-marketing Chinese patent medicine.

"Too much medicine": Insights and explanations from economic theory and research

Abstract:Increasing attention has been paid in recent years to the problem of "too much medicine", whereby patients receive unnecessary investigations and treatments providing them with little or no benefit, but which expose them to risks of harm. Despite this phenomenon potentially constituting an inefficient use of health care resources, it has received limited direct attention from health economists. This paper considers "too much medicine" as a form of overconsumption, drawing on research from health economics, behavioural economics and ecological economics to identify possible explanations for and drivers of overconsumption. We define overconsumption of health care as a situation in which individuals consume in a way that undermines their own well-being. Extensive health economics research since the 1960s has provided clear evidence that physicians do not act as perfect agents for patients, and there are perverse incentives for them to provide unnecessary services under various circumstances. There is strong evidence of the existence of supplier-induced demand, and of the impact of various forms of financial incentives on clinical practice. The behavioural economics evidence provides rich insights on why clinical practice may depart from an "evidence-based" approach. Moreover, behavioural findings on health professionals' strategies for dealing with uncertainty, and for avoiding potential regret, provide powerful explanations of why overuse and overtreatment may frequently appear to be the "rational" choice in clinical decision-making, even when they cause harm. The ecological economics literature suggests that status or positional competition can, via the principal-agent relationship in health care, provide a further force driving overconsumption. This novel synthesis of economic perspectives suggests important scope for interdisciplinary collaboration; signals potentially important issues for health technology assessment and health technology management policies; and suggests that cultural change might be required to achieve significant shifts in clinical behaviour.

Abstract:This article reports the recommendations of the Panel on Cost Effectiveness in Health and Medicine, sponsored by the US Public Health Service, on standardised methods for conducting cost-effectiveness analyses. Although not expressly directed at analyses of pharmaceutical agents, the Panel’s recommendations are relevant to pharmacoeconomic studies.The Panel outlines a ‘Reference Case’ set of methodological practices to improve quality and comparability of analyses. Designed for studies that inform resource-allocation decisions, the Reference Case includes recommendations for study framing and scope, components of the numerator and denominator of cost-effectiveness ratios, discounting, handling uncertainty and reporting.The Reference Case analysis is conducted from the societal perspective, and includes all effects of interventions on resource use and health. Resource use includes ‘time’ resources, such as for caregiving or undergoing an intervention. The quality-adjusted life-year (QALY) is the common measure of health effect across Reference Case studies. Although the Panel does not endorse a measure for obtaining quality-of-life weights, several recommendations address the QALY. The Panel recommends a 3% discount rate for costs and health effects.Pharmacoeconomic studies have burgeoned in recent years. The Reference Case analysis will improve study quality and usability, and permit comparison of pharmaceuticals with other health interventions.
